Abstract The aim of this study was to characterize the salivary metabolomic profile in adolescents with juvenile systemic lupus erythematosus (jSLE). A total of 24 adolescents with jSLE (15.92 ± 2.06 years) and 12 systemically healthy controls (15.25 ± 2.7 years) were included in the study. Participants underwent rheumatologic testing and periodontal examination, with the recording of plaque index (PI), probing depth (PD), clinical attachment level (CAL), and bleeding on probing index (BPI). Unstimulated whole saliva was collected from both groups and stored at -80 ºC. The salivary proton nuclear magnetic resonance (1H-NMR) spectra were acquired in a spectrometer operating at 500 MHz. Partial least squared discriminant analysis (PLS-DA) and orthogonal PLS-DA (O-PLS-DA) were used for statistical analysis. Mean CAL and PI were significantly increased in the group with jSLE (p < 0.01). Patients with jSLE presented a significantly different salivary metabolic profile (accuracy = 0.54; R2 = 0.86; Q2 = -0.293), significantly higher salivary levels of N-acetyl sugars, and significantly reduced levels of phenylalanine, glycine, taurine, hydroxybutyrate, and valerate compared with healthy controls (p < 0.05). It is suggested that the salivary metabolomic profile analyzed by 1H NMR in patients with jSLE presents a different fingerprint that the systemically healthy subjects. Integrating the variation of metabolites with the identification of the metabolic pathways involved seems to provide a better understanding of the influence of systemic disease on salivary metabolites.

RESUMO A doença causada pelo novo coronavírus, ou COVID-19, foi descrita recentemente e tornou-se uma questão de saúde mundial. Seu diagnóstico de certeza é dado pela reação em cadeia da polimerase. A tomografia computadorizada de alta resolução, entretanto, mostra-se útil no contexto atual de pandemia, especialmente nos casos mais graves, na avaliação da extensão da doença, em possíveis diagnósticos diferenciais e na pesquisa de complicações. Em pacientes com quadro clínico suspeito e achados de imagem típicos, nos quais ainda não há resultado laboratorial ou a reação em cadeia da polimerase não se encontra disponível, ainda se discute o papel desse exame. Importante ressaltar que parte dos pacientes apresenta exames laboratoriais falsos-negativos, notadamente em casos iniciais, o que pode retardar medidas de isolamento, favorecendo a propagação da doença. Dessa forma, o conhecimento da COVID-19 e de suas manifestações nos exames de imagem é de extrema importância para os médicos envolvidos no atendimento, sejam clínicos ou radiologistas.

ABSTRACT The disease caused by the new coronavirus, or COVID-19, has been recently described and became a health issue worldwide. Its diagnosis of certainty is given by polymerase chain reaction. High-resolution computed tomography, however, is useful in the current context of pandemic, especially for the most severe cases, in assessing disease extent, possible differential diagnoses and searching complications. In patients with suspected clinical symptoms and typical imaging findings, in which there is still no laboratory test result, or polymerase chain reaction is not available, the role of this test is still discussed. In addition, it is important to note that part of the patients present false-negative laboratory tests, especially in initial cases, which can delay isolation, favoring the spread of the disease. Thus, knowledge about the COVID-19 and its imaging manifestations is extremely relevant for all physicians involved in the patient care, clinicians or radiologists.

INTRODUÇÃO: Vários estudos foram realizados para definir preditores de eventos adversos pós-intervenção coronária percutânea (ICP). Pacientes cujo fluxo fracionado de reserva do miocárdio (FFR) pós-procedimento é < 0,90 apresentam índice de eventos cardíacos adversos maiores em 6 meses pelo menos 3 vezes maior do que aqueles cujo FFR é ≥ 0,90. Este estudo teve por objetivo identificar fatores clínicos, angiográficos e do procedimento associados a FFR pós-ICP < 0,90. MÉTODOS: Foram incluídos 193 pacientes (256 lesões) tratados entre 2004 e 2005, e o FFR foi medido antes e depois da ICP em todos os vasos tratados. Os pacientes foram divididos nos grupos FFR < 0,90 e FFR ≥ 0,90. Análise multivariada por regressão logística foi utilizada para determinar as razões de chances (odds ratio - OR) ajustadas. RESULTADOS: Foi possível obter o FFR em todas as lesões. Não se observou diferença nos parâmetros clínicos entre os dois grupos de pacientes. Houve diferença em alguns parâmetros angiográficos e do procedimento, porém, ao aplicarmos o modelo de regressão logística, a única variável que se associou com FFR pós-ICP < 0,90 foi o tratamento da artéria descendente anterior (OR = 12,1; IC 95% 6,4-22,9; P < 0,01). CONCLUSÕES: A única variável preditora de FFR pós-ICP < 0,90 foi o tratamento da artéria descendente anterior.

BACKGROUND: Several studies were performed to define predictors of adverse events after percutaneous coronary intervention (PCI). Patients whose post-procedure myocardial fractional flow of reserve (FFR) is < 0.90 have an incidence of major cardiac events at 6 months at least 3 times higher than those whose FFR is ≥ 0.90. The aim of this study was to identify clinical, angiographic and procedure-related factors associated with a post-PCI FFR < 0.90. METHODS: One hundred and ninety-three patients (256 lesions) treated between 2004 and 2005 were included, and FFR was measured before and after PCI in all of the treated vessels. Patients were divided into groups with FFR < 0.90 and FFR ≥ 0.90. Logistic regression multivariate analysis was used to determine the adjusted odds ratio (OR). RESULTS: FFR was measured in all lesions. No difference was observed in clinical parameters between groups. There were differences in angiographic and procedure-related parameters, however, when the logistic regression model was used, the only variable associated to post-PCI FFR < 0.90 was the treatment of the left anterior descending artery (OR, 12.1; 95% CI, 6.4-22.9; P < 0.01). CONCLUSIONS: The only predictor of a FFR < 0.90 after PCI was the treatment of the left anterior descending artery.

INTRODUÇÃO: A criação das Unidades de Pronto Atendimento (UPAs) veio atender a uma necessidade urgente do governo brasileiro em melhorar o atendimento à população carente. Rapidamente as UPAs se tornaram centros de referência para tratamento da dor torácica nessa população, em decorrência da agilidade do atendimento, da disponibilidade de medicamentos de última geração e da existência de pessoal treinado. Este estudo visa a descrever o perfil e a evolução hospitalar de pacientes com síndrome coronária aguda atendidos nessas unidades e posteriormente encaminhados a um hospital terciário, onde foram tratados por angioplastia com implante de stent. MÉTODO: No período de julho a dezembro de 2009, 300 pacientes foram encaminhados das UPAs para o Hospital Santa Helena (Cabo Frio, RJ). Desses pacientes, 164 (202 lesões) foram submetidos a implante de stent e divididos em dois grupos: grupo I, formado por 86 pacientes com síndrome coronária aguda com supradesnível do segmento ST; e grupo II, formado por 78 pacientes com síndrome coronária aguda sem supradesnível do segmento ST. Os dois grupos foram comparados em relação a suas características clínicas e angiográficas e a eventos cardíacos adversos maiores hospitalares. RESULTADOS: As características clínicas e angiográficas foram similares nos dois grupos, com exceção do tabagismo. Foi obtido sucesso angiográfico em 99% dos procedimentos. O tempo médio de internação (UPA + hospital) foi de seis dias. Houve 6 eventos cardíacos adversos maiores hospitalares, 5 dos quais no grupo I e 1 no grupo II (5,8% vs. 1,3%; P = 0,60). CONCLUSÃO: O tratamento percutâneo tardio dos pacientes com síndrome coronária aguda atendidos nas UPAs é um procedimento seguro e eficaz, cursando com altas taxas de sucesso e baixos índices de complicações hospitalares.

BACKGROUND: Emergency Care Units (ECUs) were created to meet an urgent need of the Brazilian government to improve health care to low-income population. ECUs quickly became reference centers to treat chest pain in this population, due to their ability to provide prompt medical care, availability of last generation drugs and well trained staff. This study is aimed at describing the profile and inhospital follow-up of patients with acute coronary syndromes seen in these units and later referred to a tertiary hospital, where they were treated by percutaneous coronary intervention with stent implantation. METHOD: Three hundred patients were referred from ECUs to Santa Helena Hospital (Cabo Frio, RJ) from July to December, 2009. One hundred and sixty four patients (202 lesions) were treated by percutaneous intervention with stent and were divided in two groups: group I included 86 patients with acute coronary syndromes with ST segment elevation and group II included 78 patients with acute coronary syndromes without ST segment elevation. Clinical and angiographic characteristics and in-hospital major adverse cardiac events were compared between the two groups. RESULTS: Clinical and angiographic characteristics were similar in both groups except for smoking. Angiographic success was obtained in 99% of the procedures. Mean hospitalization time (ECU + tertiary hospital) was 6 days. There were 6 in-hospital major adverse cardiac events, 5 in group I and 1 in group II (5.8% vs. 1.3%; P = 0.60). CONCLUSION: Late percutaneous revascularization of patients with acute coronary syndromes seen at ECUs is a safe and effective procedure, with high success and low complication rates.

INTRODUÇÃO: O stent recoberto por titânio-óxido nítrico (Titan) mostrou-se eficaz para redução da hiperplasia neointimal comparado ao stent convencional em animais e seres humanos. Foi elaborado um estudo clínico prospectivo e randomizado do stent Titan versus stent de aço inoxidável, cujo objetivo foi comparar o índice de eventos cardíacos adversos maiores (ECAM) após dois anos. MÉTODO: No período de outubro de 2005 a maio de 2006, 200 pacientes (228 lesões) foram submetidos a implante de stent eletivo e randomizados para dois grupos: I) stent metálico (100) e II) stent Titan (100). O acompanhamento dos doentes foi realizado por consulta clínica. ECAM foram definidos como: morte, infarto agudo do miocárdio ou nova revascularização da lesão-alvo. As sobrevidas livres de ECAM e angina após dois anos foram comparadas entre os dois grupos. RESULTADOS: Foi obtido sucesso em todos os procedimentos. As características clínicas e angiográficas foram similares nos dois grupos. O período médio de seguimento foi de 482 ± 264 dias. Aos 30 dias, o índice de ECAM foi de 2% no grupo I e de 1% no grupo II (P = 0,32). Após dois anos, a sobrevida livre de ECAM foi melhor no grupo Titan que no grupo stent metálico (93,6% vs. 82,4%; P = 0,036), assim como a sobrevida livre de angina (84% vs. 71%; P = 0,02). CONCLUSÕES: O implante do stent Titan em pacientes com insuficiência coronariana crônica é seguro e efetivo, e cursa com índices de ECAM e angina após dois anos menores que os do stent de aço inoxidável.

BACKGROUND: Stent coating with titanium-nitride oxide (Titan) has been shown to reduce neointimal hyperplasia in both animals and humans. A prospective, randomized, clinical study was designed to compare the incidence of major adverse cardiac events (MACE) after two years in patients with the Titan stent versus the stainless steel stent. METHODS: Two hundred patients (228 stenosis) submitted to stent implantation between October 2005 and May 2006 were randomized into two groups: I) Bare metal stent (100); II) Titan (100) and followed clinically. MACE was defined as a composite of death, acute myocardial infarction or clinically driven revascularization of target lesion. MACE and angina free survival after 2 years were compared between the two groups. RESULTS: All procedures were successful in both groups. Baseline clinical and angiographic characteristics were similar in both groups. Mean follow-up period was 482 ± 264 days. After 30 days, MACE was 2% in group I and 1% in group II (P = 0.32). MACE free survival at 2 years was significantly better in the Titan group than in the bare metal stent group (93.6% vs. 82.4%; P = 0.036) as well as angina free survival (84% vs. 71%; P = 0.02). CONCLUSIONS: Revascularization with Titan stent in chronic coronary artery disease is safe, effective, and reduces the incidence of MACE and angina after two years when compared with the bare metal stent.

FUNDAMENTO: A angiografia vem sendo utilizada como padrão de referência para definição de doença arterial coronariana (DAC), embora suas limitações sejam conhecidas. O valor da medida do fluxo fracionado de reserva do miocárdio (FFR) na avaliação da DAC está bem estabelecido. OBJETIVO: O objetivo deste estudo é avaliar a acurácia da angiografia em definir as lesões isquêmicas e sua correlação com o FFR. MÉTODOS: Duzentos e cinqüenta pacientes foram incluídos no estudo (471 vasos). Todas as estenoses > 50% pela estimativa visual da angiografia (EVA) foram avaliadas medindo-se o FFR. Se o FFR <0,75 a lesão foi tratada, se o FFR >0,75 a lesão não foi tratada. As lesões foram divididas em moderadas (<70% - 327) e graves (125) pela QCA. Foram determinados o coeficiente de correlação entre o grau de estenose (%DE), o FFR e a acurácia da EVA em definir se uma lesão era ou não isquêmica. RESULTADOS: Foi possível obter o FFR em 96% das lesões. %DE e FFR médios de 56±8% e 0,74 e 76±6% e 0,48 para as lesões moderadas e graves respectivamente. Notou-se pobre correlação entre o %DE e o FFR, especialmente nas lesões moderadas (Spearman rho = - 0.33, p<0,0001). A acurácia da EVA comparada com FFR foi de 57% e 96% nas lesões moderadas versus graves. CONCLUSÃO: A angiografia coronária não é adequada para avaliar a importância funcional das lesões coronarianas, sendo necessário associá-la a um método funcional capaz de fazê-lo, especialmente nas lesões moderadas.

BACKGROUND: The angiography has been used as a reference standard to define coronary artery disease (CAD), although its limitations are well-known. The significance of the myocardial fractional flow reserve (FFR) in the assessment of CAD is well established. OBJECTIVE: The aim of this study was to evaluate the accuracy of angiography when defining ischemic lesions and its correlation with FFR. METHODS: Two hundred and fifty consecutive patients (471 arteries) were included in this study. All stenoses > 50% at the angiography visual estimate (AVE) were assessed by FFR measurements. When FFR was < 0.75, stenting was performed; when FFR was > 0.75, no interventional treatment was carried out. Offline quantitative coronary angiography (QCA) was performed in all stenoses, which were divided in intermediate (< 70% - 327) and severe (125). The correlation coefficients between the diameter of the stenosis (%DS) and FFR and the accuracy of VA of the angiography when assessing ischemia were determined. RESULTS: FFR could be obtained in 452 lesions (96%). Mean %DS and FFR were 56 ± 8% and 0.74 and 76 ± 6% and 0.48 for moderate and severe stenoses, respectively. Concordance between QCA and FFR was poor, especially in intermediate stenoses (Spearman's rho = - 0.33, p<0.0001). Visual assessment resulted in an accuracy of 57% and 96% in intermediate and severe lesions, respectively. CONCLUSION: Neither the visual assessment of an angiogram nor QCA can accurately predict the significance of most intermediate coronary stenoses, which emphasizes the importance of associating it to a functional evaluation of the coronary circulation, resulting in an adequate treatment of these stenoses.

Das cardiopatias congênitas, a origem anômala da artéria interventricular anterior apresenta incidência de 1:300.000, com alta mortalidade até o primeiro ano de vida, mas que na presença de boa circulação colateral para a artéria relacionada à anomalia, pode manter o paciente assintomático até a vida adulta. Relatamos o caso, raro, de um paciente de 43 anos, oligossintomático e com função ventricular normal que apresentava essa doença e foi submetido a tratamento cirúrgico sem circulação extracorpórea.

From all congenital cardiopathies, anomalous origin of anterior interventricular artery occurs once per 300,0000 live births, reporting high mortality in the first year after birth. However, if good collateral circulation is available for the artery related to the abnormality, the patient may be kept asymptomatic until mature age. This is the report on a rare case of a 43-year-old patient who was oligosymptomatic, with normal ventricular function in this pathology presentation. The patient was submitted to surgical treatment with no extracorporeal circulation.

INTRODUÇÃO: A decisão de realizar uma intervenção coronária percutânea (ICP) em pacientes com angina deve se basear em sinais objetivos de isquemia. A angiografia vem sendo utilizada como padrão-ouro para definição de doença arterial coronariana (DAC), embora suas limitações sejam conhecidas. Por outro lado, o valor da medida do fluxo fracionado de reserva do miocárdio (FFR) na avaliação da DAC está bem estabelecido. OBJETIVO: Avaliar a acurácia da angiografia em definir as lesões culpadas e sua correlação com o FFR, em todos os pacientes admitidos para ICP eletiva em nossa Instituição, durante determinado período. MÉTODO: Foram incluídos no estudo 250 pacientes (471 vasos). Foram avaliadas todas as estenoses >50% pela estimativa visual, selecionadas por três cardiologistas independentes, medindo-se o FFR. Quando o FFR era < 0,75, a lesão foi tratada; se o FFR era >0,75, a lesão não foi tratada. Foi realizada QCA off-line de todas as lesões, divididas em moderadas (< 70% - 327) e graves (125). Foram determinados o coeficiente de correlação entre o grau de estenose (%DE) e o FFR para os dois grupos e a acurácia da estimativa visual da angiografia em definir se uma lesão era ou não isquêmica, tomando-se o FFR como padrão-ouro. RESULTADOS: Foi possível obter o FFR em 96% das lesões (452). %DE e FFR médios de 56 ± 8% e 0,74 e 76 ± 6% e 0,48 para as lesões moderadas e graves. Notouse pobre correlação negativa entre o %DE e o FFR, principalmente nas lesões moderadas (Spearman rho = -0,33, p<0,0001). A acurácia da estimativa visual da angiografia comparada com o FFR foi de 57% e 96%, respectivamente, nas lesões moderadas e graves. CONCLUSÕES: A angiografia coronária não é um método capaz de prever adequadamente a importância funcional das lesões coronarianas, tornando clara a necessidade de associá-la a um método funcional capaz de orientar corretamente o tratamento dessas estenoses.

Despite this, the selection of lesions to be stented is often based on the subjectively interpreted angiogram alone. Fractional flow reserve (FFR) is an established objective methodology to indicate which lesions are ischemia-producing. OBJECTIVE: The aim of this study, in patients admitted for elective percutaneous coronary intervention (PCI), was to evaluate the accuracy of angiography in defining culprit lesions, taking FFR as the gold standard, and also its correlation with FFR METHODS: Two hundred and fifty consecutive patients (471 arteries) scheduled for PCI were included in this study. All stenoses > 50% by visual estimation, initially selected to be stented by 3 independent reviewers were assessed by measurement of the FFR. When the FFR was < 0.75, stenting was performed; when the FFR was > 0.75, no interventional treatment was given. Offline quantitative coronary angiography (QCA) was performed for all stenoses, divided into intermediate (< 70% - 327) and severe (125). The correlation coefficient between the degree of stenosis (%DS) and FFR was determined as was the accuracy of the visual assessment of angiography in evaluating ischemia taking FFR as the gold standard. RESULTS: Optimal FFR readings were obtained in 452 lesions (96%). %DS and FFR were 56 ± 8% and 0.74 for moderate and 76 ± 6% and 0.49 for severe stenosis. Concordance between QCA and FFR was poor especially in intermediate stenosis (Spearman's rho = -0.33, p<.0001). Visual assessment resulted in accuracies of 57% and 96% for intermediate and severe lesions, respectively. CONCLUSIONS: Thus, neither visual assessment of an angiogram nor QCA can accurately predict the significance of most intermediate coronary stenoses, stressing the importance of associating these to a functional evaluation of the coronary circulation which can correctly guide treatment of stenosis.
