ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Post-hatch fasting and infectious bronchitis vaccination affect growth, gastrointestinal development, and morphometric parameters of the small intestine of broiler breeders up to three weeks of age

ABSTRACT The influence of fasting and vaccination against infectious bronchitis (IB) on growth, gastrointestinal development, and morphometric parameters of the small intestine of newborn broiler breeders was investigated. Weight gain (WG), feed conversion ratio (FCR), gastrointestinal visceral growth, residual yolk sac, and morphometric parameters of the small intestine were measured in chicks aged up to 21 d. We showed that WG was lowest in chicks exposed to longer fasting periods (72 h), although compensatory gains with increasing age were also observed. Unvaccinated chicks performed better at seven days; however, at 21 d of age, a compensatory response led to better FCR in vaccinated birds. Allometric growth of the gastrointestinal tract was affected following longer fasting periods, but this effect occurred predominantly in younger birds (three days). Compared with non-fasted, birds fasted for 72 h had lower villus height and crypt depth along the small intestine at three days and lower villus height in the duodenum and jejunum at seven days. However, at 14 and 21 days of age, the previous changes in intestinal morphology imposed by fasting had been offset. The IB vaccine, applied in the first day of life, does not affect the allometric growth of the gastrointestinal organs, but causes momentary loss in the intestinal morphometry, which contributes to the reduction of WG at seven days. Our results suggest that long fasting periods after hatching (48 and 72 h) should be avoided in broiler breeders due to the resulting delay in gastrointestinal organ development and growth impairment up to 21 d of life.

ABSTRACT This study aimed to assess the effects of glutamine as feed additive on performance and intestinal mucosa morphometry of broiler chickens vaccinated against coccidiosis. A total of 400 day-old male chicks were randomly assigned to four treatments (NVNG – no vaccination, no glutamine supplementation; NVG – no vaccination, glutamine supplementation (10 g kg−1); VNG – vaccination, no glutamine supplementation; VG – vaccination, glutamine supplementation) replicated four times with 25 birds per replicate. A commercial sprayed-on vaccine against coccidiosis containing Eimeria acervulina, E. maxima, E. mivati, and E. tenella was administered at the hatchery. Broiler performance was evaluated from 1-28 days, and morphometric parameters were analyzed at 14, 21, and 28 days of age. Body weight gain and feed intake were negatively affected by vaccination, but not by glutamine. Vaccination increased crypt depth in the duodenum and jejunum at 21 and 28 days. In conclusion, this study showed that glutamine was not able to increase weight gain of broiler chickens, irrespective of whether the animals were vaccinated or not against coccidiosis. Glutamine supplementation was able to improve feed conversion in vaccinated birds suggesting trophic effect on intestinal epithelium improving.

Urinary symptoms in Parkinson's disease: prevalence and associated factors

Trata-se de estudo clínico transversal, envolvendo 61 pacientes com doença de Parkinson idiopática (DP) examinados consecutivamente e comparados a 74 indivíduos do grupo controle. Foram incluídos apenas pacientes com DP que preencheram os critérios de diagnóstico estabelecidos. A ressonância magnética do encéfalo e a ultra-sonografia da próstata foram utilizadas como critérios de exclusão. O objetivo do estudo foi avaliar a prevalência de sintomas do trato urinário inferior na DP e verificar associações entre sexo, idade e fatores clínicos às disfunções urinárias. Sintomatologia urinária foi encontrada em 39,3% dos parkinsonianos e 10,8% do grupo controle. Todos os pacientes sintomáticos apresentavam sintomas irritativos e 25% destes tinham sintomas obstrutivos. A sintomatologia urinária não estava associada ao sexo, duração e gravidade da doença, hipotensão postural e sintomas ortostáticos, assim como com os sinais cardeais e tratamento com a levodopa.
The authors present a cross-sectional study involving 61 patients with idiopathic Parkinson's disease (PD) who were consecutively examined and compared to a control group with 74 subjects. Only patients who fulfilled the standard diagnostic criteria for PD and whose brain magnetic resonance imaging was normal were included. The objective of the study was to evaluate the prevalence of inferior urinary tract symptoms in PD and to study the possible association between clinical factors to urinary dysfunction. ln the patient group, 39.3% presented urinary symptoms when compared to 10.8% in the control group. All symptomatic patients presented irritative symptoms. The most common irritative symptom PD was nocturia, followed by frequency and urinary incontinence. Around 25% of the patients presented functional obstructive symptoms determined by the disease. The most frequent obstructive symptom was incomplete emptying of the bladder. Only the age of the patients and control group were correlated with urinary dysfunction.
