Abstract Introduction Oral pre-exposure prophylaxis (PrEP) with emtricitabine/tenofovir (FTC/TDF) is highly effective in preventing HIV infection. This study aimed to identify factors associated with PrEP early loss to follow-up (ELFU) among gay, bisexual and other men who have sex with men (MSM), travestis and transgender women (TGW). Methodology This was a prospective cohort study evaluating TGW and MSM who initiated PrEP at the Evandro Chagas National Institute of Infectious Diseases (INI-Fiocruz) from 2014 to 2020. ELFU was defined as not returning for a PrEP visit within 180 days after first dispensation. Exposure variables included age, gender, race, education, transactional sex, condomless anal intercourse [CAI] (both in the past six months), binge drinking and substance use (both in past three months) and syphilis diagnosis at baseline. Multilevel logistic regression models with random intercepts and fixed slopes were used to identify factors associated with ELFU accounting for clustering of participants according to their PrEP initiation study/context (PrEP Brasil, PrEParadas, ImPrEP and PrEP SUS). Results Among 1,463 participants, the median age was 29 years (interquartile range 24-36), 83% self-identified as MSM, 17% as TGW, 24% were black, 37% mixed-black/pardo and 30% had < 12 years of education. Fifteen percent reported transactional sex, 59% reported CAI, 67% binge drinking, 33% substance use, and 15% had a syphilis diagnosis. Overall, 137 participants (9.7%) had ELFU. Younger age (18-24 years) (adjusted odds ratio [aOR] 1.9, 95%CI:1.2-3.2), TGW (aOR 2.8, 95%CI:1.6-4.8) and education < 12 years (aOR 1.9, 95%CI:1.2-2.9) were associated with greater odds of ELFU. Conclusion TGW, young individuals and those with lower education were at higher risk of PrEP ELFU. Our results suggest that the development of specific strategies targeting these populations should be a priority, through policies that aim to reduce the incidence of HIV infection.

ABSTRACT: Dental and periodontal affections consist of health concerns of great importance in sheep flocks, as they affect the health and welfare of animals and represent one of the main causes of premature slaughter. Studies on the occurrence of these conditions in sheep are scarce in Brazil. The present study aimed to evaluate the prevalence of periodontal diseases and tooth wear in sheep flocks in the State of Goiás. For this, a clinical oral examination was conducted on 325 sheep from seven farms in six municipalities of the State. Periodontal lesions, characterized by gingival recession in at least one incisor tooth, were observed in 48.3% (157/325) of the sheep, while in cheek teeth it occurred in 13.2% (43/325) of the evaluated sheep. Dental wear on cheek teeth was identified in 69.2% (255/325) of the sheep, and 10.7% (35/325) of the ovines showed dental wear on incisor teeth. Sheep older than 36 months had an increase in the frequency of tooth wear. A total of 171 (52.62%) out of the 325 examined sheep had signs of bleeding on probing, which is compatible with gingivitis, and 69 (40.36%) were older than 36 months. Thirty-eight (11.69%) out of the 325 evaluated ovines had gingivitis in all teeth, among which 21 (55.26%) were older than 36 months. Logistic regression analysis allowed the observation that age was associated with the occurrence of gingivitis. Less frequent changes such as periodontal pockets and loss of at least one tooth were also observed in 4.9% (16/325) and 17.8% (58/325) of the sheep, respectively. It indicates that the presence of periodontal diseases and tooth wear is a common occurrence in the evaluated flocks. Therefore, there is a need to include the oral health of sheep in the routine of the farm as an essential preventive measure to improve the quality of life, animal welfare, and zootechnical indices.
RESUMO: As afecções dentárias e periodontais constituem uma preocupação sanitária de grande importância em rebanhos ovinos, uma vez que afetam a saúde e o bem-estar dos animais, e representam uma das principais causas para o abate prematuro. No Brasil, estudos sobre a ocorrência dessas afecções em ovinos são escassos. Assim, o presente estudo teve como objetivo avaliar a prevalência de doenças periodontais e desgaste dentário em rebanhos ovinos no estado de Goiás. Para isso, realizou-se o exame clínico bucal em 325 ovinos em sete propriedades rurais de seis municípios do estado. As lesões periodontais, caracterizadas por recessão gengival em, pelo menos, um dente incisivo, foram observadas em 48,3% (157/325) dos ovinos, enquanto nos mastigatórios ocorreu em 13,2% (43/325) dos ovinos avaliados. O desgaste dentário em dentes mastigatórios foi identificado em 69,2% (255/325) dos ovinos e 10,7% (35/325) dos ovinos apresentaram desgaste dentário em dentes incisivos. Ovinos com idade superior a 36 meses tiveram um aumento na frequência do desgaste dentário. Dos 325 ovinos examinados, 171 (52,62%) apresentaram sinais de sangramento à sondagem, compatíveis com o quadro de gengivite, sendo que 69 (40,36%) apresentaram idade superior a 36 meses. Dos 325 ovinos avaliados, 38 (11,69%) apresentaram gengivite em todos os dentes, dos quais 21 (55,26%) apresentaram idade superior a 36 meses. Pela análise de regressão logística, foi possível observar que a idade mostrou associação com a ocorrência de gengivite. Alterações menos frequentes como a bolsa periodontal e a perda de, pelo menos, um elemento dentário também foram observadas em 4,9% (16/325) e 17,8% (58/325) dos ovinos, respectivamente. Os resultados do presente estudo indicam que a presença doenças periodontais e desgaste dentário é uma ocorrência comum nos rebanhos avaliados. Portanto, enfatiza-se a necessidade de incluir a saúde bucal dos ovinos na rotina da propriedade como uma medida preventiva essencial para melhorar a qualidade de vida e o bem-estar dos animais, bem como os índices zootécnicos.

Abstract Zoonotic spillover is a phenomenon characterized by the transfer of pathogens between different animal species. Most human emerging infectious diseases originate from non-human animals, and human-related environmental disturbances are the driving forces of the emergence of new human pathogens. Synthesizing the sequence of basic events involved in the emergence of new human pathogens is important for guiding the understanding, identification, and description of key aspects of human activities that can be changed to prevent new outbreaks, epidemics, and pandemics. This review synthesizes the connections between environmental disturbances and increased risk of spillover events based on the One Health perspective. Anthropogenic disturbances in the environment (e.g., deforestation, habitat fragmentation, biodiversity loss, wildlife exploitation) lead to changes in ecological niches, reduction of the dilution effect, increased contact between humans and other animals, changes in the incidence and load of pathogens in animal populations, and alterations in the abiotic factors of landscapes. These phenomena can increase the risk of spillover events and, potentially, facilitate new infectious disease outbreaks. Using Brazil as a study model, this review brings a discussion concerning anthropogenic activities in the Amazon region and their potential impacts on spillover risk and spread of emerging diseases in this region.

Rapid and Inexpensive Colorimetric Detection of Total Serum Protein Using Microzone Plates Wax-Printed on Polyester Films

This report describes a rapid and inexpensive colorimetric method based on biuret assay for total serum protein analysis using wax printed microzone plates and a desktop scanner. The colorimetric assay was performed in 96-microzone plates fabricated on polyester films through a wax printing protocol. The image scanning mode and the concentration of the biuret reagents were optimized to achieve the highest detectability levels. The best analytical response was obtained using the slide scanning mode as well as 4% CuSO4 and 150 mmol L-1 potassium sodium tartrate solutions. The proposed method offered linear relationship for the bovine serum albumin concentration range between 25 and 200 µmol L-1 with a limit of detection of 9.7 µmol L-1. The colorimetric approach revealed satisfactory precision (relative standard deviation values between 0.6 and 4.4%). The bioanalytical feasibility was demonstrated through the total protein analysis in serum samples collected from dogs previously diagnosed with kidney disease. The total serum protein concentrations varied from 960 ± 19 to 1661 ± 38 µmol L-1 and the achieved values revealed good accordance with those obtained through a reference method. Based on the reported results, wax printed microzone plates have demonstrated simplicity and potential to be adopted as alternative platforms for bioassays.

Abstract The development of self-disinfectant devices is highly needed to prevent and control infections, mainly caused by virus. In the past years, coronaviruses have been a threat to humanity, causing severe epidemics of respiratory infections such as severe acute respiratory syndrome (SARS), in 2003, and Middle East respiratory syndrome (MERS) in 2012, and presently the SARS-CoV2 is causing the COVID-19 pandemic. Previous studies have demonstrated that surface contamination play a significant role in the spreading of viruses. These studies demonstrated that the production of highly reactive species by copper alloys contributes to rapid elimination of viruses. Nanostructured materials such as semiconductors TiO2, Co3O4 CuO, NiO, and TiO2, and silver nanoparticles can decrease the virus viability on the surfaces when associated with polymers and textiles, especially in conditions of light exposure. In addition, graphene oxide is rising as a promising material for inactivation of viruses due to its capacity of destroying the viral envelope and capsid. The virucidal property of these materials can be enhanced by increasing their functionalization with photosensitizers. The present mini-review brings subsidies for the development of new advanced self-disinfectant materials that can be used in the manufacture of gloves, masks, and a variety of other devices.

Electrochemical methods are powerful in the characterization and design of redox-modulating agents. We, herein, report the electrochemical investigation, in aprotic medium, of eleven synthetic 3-thio-substituted-nor-beta-lapachones, along with the determination of cytotoxic activity and correspondent selectivity index, against several cancer cell lines and one normal cell. Four of the quinones are novel compounds. The redox behavior is representative of two independent systems: the easy reduction of the quinone moiety and, at far more negative potential, the reductive cleavage of the C-S-C bonding; and the anodic part controlled by the oxidation of the sulfur moiety. The compounds have shown relevant cytotoxic activity, with emphasis on 3-phenyl-thio-2,2-dimethyl-2,3-dihydronaphtho[1,2-b]furan-4,5-dione (compound 2), which mechanism of molecular action was shown to be related to reactive oxygen species (ROS) release. Despite the absence of a linear correlation, there is a trend: the majority of the thionaphthoquinones, with values of first wave reduction potential, less negative than -0.65 V, were active. The less electrophilic compound (3-(cyclohexylthio)-2,2-dimethyl-2,3-dihydronaphtho[1,2-b]furan-4,5-dione, cyclohexyl derivative) is also the less cytotoxic toward cancer cells. Agents containing chalcogens and quinones can be used to attack entities with a disturbed redox balance.

In this report, a rapid and selective analytical method for the determination of monoterpenes in Alpinia zerumbet essential oil (AZEO) by gas chromatography (GC) flame ionization detection (FID) was developed, optimized and validated. The suitability of six different capillary columns was investigated by polarity phase constants calculated based on the methodology proposed by Rohrschneider and McReynolds. The most suitable column was then used in a central composite design applying the following GC factors to investigate responses based on peak resolutions and analysis time: initial oven temperature, heating rate and carrier gas flow rate. The optimized method has initial oven temperature of 60 ºC, heating rate of 5.1 ºC min-1 and flow rate of 1.0 mL min-1 using the DB-35 capillary column. The validation acceptance criteria were met in all cases. The method was successfully applied for the quantification of major monoterpenes found in AZEO in different samples of Alpinia zerumbet.

ABSTRACT Dialypetalanthus is a monospecific genus that occurs in the Amazon Basin of Brazil, Bolivia and Peru and occupies a controversial position among the Rubiaceae. We continue this taxonomic discussion with the overall aim of clarifying the systematic position of D. fuscescens within the Rubiaceae. To accomplish this, we analyzed the ontogeny of its gynoecium, in particular the floral meristem, as well as the development of the carpels and cauline placentation. Gynosporogenesis and the differentiation of the carpellary septa and ovules were also described. Dialypetalanthus fuscescens was classified according to evolutionary diagrams found in the literature. The following characteristics were observed in D. fuscescens: 1) permanence of the floral meristem in the central basal part of the early flower bud; 2) dual origin of carpellary septum; 3) trizonate ovular primordia with only one fertile gynospore per ovule; and 4) cellular proliferation in the chalazal region. Embryological results confirm the classification of D. fuscescens in the subfamily Ixoroideae. These results are distinct from any others previously proposed in the evolutionary diagram. Therefore, we conclude that this study has presented evidence strongly suggesting that Dialypetalanthus possesses new morphological-type of ovule we refer to as the Dialypetalanthus-type.

RESUMO A neurodegeneração com acúmulo cerebral de ferro (sigla em inglês NBIA) representa um grupo heterogêneo e complexo de doenças neurodegenerativas hereditárias, caracterizada pelo acúmulo cerebral de ferro, especialmente nos núcleos da base. O quadro clínico das NBIAs em geral inclui distúrbios do movimento, particularmente parkinsonismo e distonia, disfunção cognitiva, sinais piramidais e anormalidades da retina. As formas de NBIA descritas até o momento incluem neurodegeneração associada a pantothenase kinase (PKAN), neurodegeneração associada a phospholipase A2 (PLAN), neuroferritinopatia, aceruloplasminemia, neurodegeneração associada a beta-propeller protein (BPAN), síndrome de Kufor-Rakeb, neurodegeneração associada a mitochondrial membrane protein (MPAN), neurodegeneração associada a “fatty acid hydroxylase” (FAHN), neurodegeneração associada a coenzyme A synthase protein (CoPAN) e síndrome de Woodhouse-Sakati. Esta revisão é uma orientação para o diagnóstico das NBIAs, partindo das características clínicas e achados de neuroimagem, até a etiologia genética.
ABSTRACT Neurodegeneration with brain iron accumulation (NBIA) represents a heterogeneous and complex group of inherited neurodegenerative diseases, characterized by excessive iron accumulation, particularly in the basal ganglia. Common clinical features of NBIA include movement disorders, particularly parkinsonism and dystonia, cognitive dysfunction, pyramidal signs, and retinal abnormalities. The forms of NBIA described to date include pantothenase kinase-associated neurodegeneration (PKAN), phospholipase A2 associated neurodegeneration (PLAN), neuroferritinopathy, aceruloplasminemia, beta-propeller protein-associated neurodegeneration (BPAN), Kufor-Rakeb syndrome, mitochondrial membrane protein-associated neurodegeneration (MPAN), fatty acid hydroxylase-associated neurodegeneration (FAHN), coenzyme A synthase protein-associated neurodegeneration (CoPAN) and Woodhouse-Sakati syndrome. This review is a diagnostic approach for NBIA cases, from clinical features and brain imaging findings to the genetic etiology.

Aminonaphthoquinone Mannich Bases Derived from Lawsone and Their Copper(II) Complex Derivatives: Synthesis and Potential Cholinesterase Inhibitors as Identified by On-flow Assay

A new series of Mannich bases derived from 2-hydroxy-1,4-naphthoquinone (lawsone), substituted benzaldehydes and two primary amines, and their Cu2+ complexes were synthesized and evaluated for their potential as selective cholinesterase inhibitors (ChEIs). Immobilized capillary enzyme reactors (ICERs) bearing butyrylcholinesterase (BChE) and acetylcholinesterase (AChE) were used not only for the on-flow screening assay but also for determining the inhibitory potency and equilibrium binding constants of the lead inhibitors. Eight copper complexes were identified and characterized as potent reversible and selective ChEIs with inhibitory potencies (IC50) and constants of inhibition (Ki) ranging from 1.24 to 11.5 µmol L-1. One of the compounds was particularly promising, showing IC50 and Ki values of 1.24 ± 0.01 and 1.06 ± 0.01 µmol L-1, respectively, for huAChE. These values were lower than those for the standard inhibitor galanthamine (IC50 = 206 ± 30.0 and Ki = 126 ± 18.0 µmol L-1). Even though, it is showing noncompetitive inhibition of huAChE and linear mixed-type inhibition of eeAChE. These complexes showed a promising cholinesterase inhibitory activity and can be used as model inhibitors.

Resumo Apresenta-se uma síntese atualizada de informações sobre algas no Brasil objetivando refinar os dados reunidos até o presente, bem como avaliar os avanços sobre o conhecimento da diversidade de algas no país desde a publicação do Catálogo de Plantas e Fungos do Brasil. Os resultados de 2015 mostraram um acréscimo de 1.250 espécies (35.7%) a um total de 4.747 em relação a 2010. As classes mais diversas em número de espécies foram Bacillariophyceae, Conjugatophyceae, Florideophyceae, Cyanophyceae, Dinophyceae e Euglenophyceae. Bacillariophyceae e Cyanophyceae tiveram o maior acréscimo de espécies no intervalo de cinco anos. A região Sudeste e Sul foram as mais diversas, porém, as regiões Nordeste com os estados do Piauí e Sergipe e Centro-Oeste com os estados de Mato Grosso, Goiás e Distrito Federal destacaram-se no cenário da biodiversidade nacional. Apesar da escassez de taxonomistas e da infraestrutura limitada, os resultados obtidos evidenciaram um avanço significativo no conhecimento da diversidade de algas no país nesse período de cinco anos, iniciando uma mudança quanto as diferenças geográficas regionais.
Abstract An updated synthesis of cyanobacteria and algae information is presented for Brazil aiming to refine the data gathered to date and evaluate the progress of the biodiversity knowledge about these organisms in the country since the publication of the Catálogo de Plantas e Fungos do Brasil. The results of 2015 showed an increase of 1,250 species (35.7%) when compared to 2010, reaching a total of 4,747 species. The most diverse classes in species number were the Bacillariophyceae, Conjugatophyceae, Florideophyceae, Cyanophyceae, Dinophyceae and Euglenophyceae. Bacillariophyceae and Cyanophyceae had the highest increase in species number in the five-year interval. The Southeast and South regions were the most diverse, however, the Northeast, with the states of Piauí and Sergipe, and the Central-west region, with Mato Grosso, Goiás and Distrito Federal, also stood out in the national algal biodiversity scenario. Despite the shortage of taxonomists and limited infrastructure, the results showed a significant improvement in the knowledge regarding the diversity of cyanobacteria and algae in the country during the study period, starting to even out regional geographical differences caused by subsampling.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Viral suppression and adherence among HIV-infected children and adolescents on antiretroviral therapy: results of a multicenter study

OBJETIVO: Avaliar a adesão ao tratamento antirretroviral entre portadores de HIV acompanhados em centros pediátricos. MÉTODOS: Trata-se de estudo transversal multicêntrico. Os prontuários ambulatoriais foram revistos e aplicadas escala de adesão, avaliação de qualidade de vida (WHOQOL-BREF), ansiedade, depressão e uso indevido de álcool/substâncias entre cuidadores. Os desfechos incluíram autorrelato 100% de adesão nos últimos três dias e carga viral do HIV (CV) < 50 cópias/mL. RESULTADOS: 260 indivíduos foram incluídos, 79% crianças e 21% adolescentes; 93% das crianças e 77% dos adolescentes relataram 100% de adesão; 57% das crianças e 49% dos adolescentes tinham CV < 50 cópias /mL. Na análise univariada, diagnóstico do HIV por triagem devido à infecção materna, cuidador com pontuação menor para ansiedade e maior nos domínios físico e psicológico do WHOQOL-BREF se mostraram independentemente associados a 100% de adesão. Intervalos mais curtos entre visitas de farmácia foram associados com CV < 50 cópias /mL (p ≤ 0,01). Regressão multivariada mostrou que os cuidadores sem abuso de álcool/outras drogas (OR = 0,49; IC95% 0,27-0,89) e o intervalo médio entre visitas de farmácia < 33 dias (OR = 0,97; IC95% 0,95-0,98) foram associados com CV < 50 cópias/mL; cuidador com menores escores para ansiedade (OR = 2,57; IC95% 1,27-5,19) e diagnóstico de crianças por triagem devido à infecção materna (OR = 2,25; IC95% 1,12-4,50) foram associados com 100% de adesão. CONCLUSÕES: Programas de HIV pediátrico devem avaliar qualidade de vida e sintomas de ansiedade e depressão dos cuidadores. Registros de farmácia são essenciais na identificação de adesão insatisfatória.
OBJECTIVE: To evaluate treatment adherence among perinatally-infected pediatric human immunodeficiency virus (HIV) patients followed in pediatric centers in Brazil. METHODS: This was a cross-sectional multicenter study. Medical records were reviewed and adherence scale, assessment of caregivers' quality of life (WHOQOL-BREF), anxiety, depression, and alcohol/substances use/abuse were assessed. Outcomes included self-reported 100% adherence in the last three days and HIV viral load (VL) < 50 copies/mL. Statistical analyses included contingency tables and respective statistics, and multivariable logistic regression. RESULTS: 260 subjects were enrolled: 78% children and 22% adolescents; 93% of caregivers for the children and 77% of adolescents reported 100% adherence; 57% of children and 49% of adolescents had VL < 50 copies/mL. In the univariate analyses, HIV diagnosis for screening due to maternal infection, lower caregiver scores for anxiety, and higher scores in physical and psychological domains of WHOQOL-BREF were associated with 100% adherence. Shorter intervals between pharmacy visits were associated with VL < 50 copies/mL (p ≤ 0.01). Multivariable regression demonstrated that caregivers who did not abuse alcohol/other drugs (OR = 0.49; 95% CI: 0.27-0.89) and median interval between pharmacy visits < 33 days (OR = 0.97; 95% CI: 0.95-0.98) were independently associated with VL < 50 copies/mL; whereas lower caregiver scores for anxiety (OR = 2.57; 95% CI: 1.27-5.19) and children's HIV diagnosis for screening due to maternal infection (OR = 2.25; 95% CI: 1.12-4.50) were found to be independently associated with 100% adherence. CONCLUSIONS: Pediatric HIV programs should perform routine assessment of caregivers' quality of life, and anxiety and depression symptoms. In this setting, pharmacy records are essential to help identify less-than-optimal adherence.

A mastite bovina é uma doença importante na bovinocultura de leite, devido à sua alta incidência e perdas econômicas associadas principalmente com a produção de leite reduzida e aos custos do tratamento. O uso de antimicrobianos para o tratamento de casos clínicos e no período seco tem levantado preocupações quanto à seleção de cepas bacterianas resistentes. Isso também pode refletir na saúde pública, uma vez que bactérias resistentes, como o Staphylococcus aureus meticilina-resistente (MRSA), podem ser transmitidas aos seres humanos por contato direto com animais infectados ou produtos lácteos. A resistência das bactérias aos agentes antimicrobianos aumentou, em geral, devido a tratamentos ineficazes. Estudos realizados no Brasil com amostras não planejadas mostram aumento no padrão de resistência, principalmente em S. aureus. A exposição ao tratamento antimicrobiano repetido ao longo das lactações consecutivas de vacas pode ser um fator predisponente para o desenvolvimento da resistência antimicrobiana em bactérias que infectam o úbere. Assim, o objetivo deste estudo foi determinar a possível associação causal entre resistência antimicrobiana em bactérias isoladas a partir do leite bovino e dados como idade e período de lactação. As amostras de leite foram coletadas de 21 rebanhos leiteiros do Rio Grande do Sul, Brasil, selecionados aleatoriamente a partir da população-alvo de 1.656 explorações leiteiras semi-intensivas, estratificada por tamanho do rebanho. A bactéria foi considerada a unidade amostral, e para a estimativa de prevalência foram utilizados os seguintes parâmetros: uma frequência de 35% de Staphylococcus sp. resistentes à penicilina; um nível de confiança de 90%; e uma precisão absoluta de 12%. As bactérias foram isoladas de amostras de leite compostas de todos os quartos mamários de cada vaca após descartar os primeiros três ou quatro jatos de leite. Para acessar os potenciais fatores de risco, características dos animais foram obtidas através de uma entrevista com os produtores. Os exames laboratoriais foram realizados de acordo com as recomendações do National Mastitis Council. Um total de 242 isolados foi obtido de 195 vacas a partir da amostra do rebanho total (251 vacas). A prevalência de infecções foi descrita em grupos de acordo com o perfil epidemiológico: bactérias ambientais, contagiosas e outras. Estas perfizeram 57,3%, 26,3% e 11,2%, respectivamente, dos animais amostrados. Testes de suscetibilidade antimicrobiana contra 12 diferentes antimicrobianos foram realizados em 159 isolados. No total, 30% dos isolados testados mostraram resistência a pelo menos três grupos diferentes de antimicrobianos e foram classificados como multirresistentes. Foram observadas as freqüências mais elevadas de resistência contra a ampicilina para os estafilococos coagulase-negativo, seguida de eritromicina para estafilococos coagulase-positivo e tetraciclina para estreptococos. A análise de regressão logística mostrou uma relação significativa entre a idade das vacas e a presença de estafilococos coagulase-positivo multirresistentes e distribuição de classes diferentes de bactérias nos diferentes estratos etários, o que sugere uma concorrência dinâmica ao longo do tempo (p < 0,05). Animais com três a quatro anos tiveram 13,7 vezes mais chances (IC95% 1,4 - 130,2, p = 0,02) de ter estafilococos coagulase-positivo multirresistentes em comparação com aqueles com dois ou três anos. O tempo de exposição a agentes infecciosos e consequentes terapias sugere uma maior chance de colonização do úbere por patógenos resistentes devido à pressão de seleção repetida durante a vida.
Bovine mastitis is an important disease in dairy cattle due to its high incidence and economic losses associated mainly with reduced milk production and treatment costs. The use of antimicrobials to treat clinical cases and at dry off raises the concern of selection of resistant bacterial strains. This may also reflect on public health, since resistant bacteria, such as methicillin-resistant Staphylococcus aureus (MRSA), may be transmitted to humans by direct contact with infected animals or by dairy products. The resistance of bacteria to antimicrobials has risen, in general, due to ineffective therapy. Studies in Brazil with non-random samples show increase in resistance pattern, mainly in S. aureus. The exposition to repeated antimicrobial treatment throughout the consecutive lactations of cows may be a predisposing factor to development of antimicrobial resistance in bacteria that infect the udder. Thus, the aim of this study was to determine the possible causal association between antimicrobial resistance in bacteria isolated from bovine udder milk and animal data such as age and lactation period. Milk samples were collected from 21 randomly selected dairy herds from Rio Grande do Sul, southernmost Brazilian state, from the target population of 1656 semi-intensive dairy farms, stratified by her size. The sample unit was considered the bacteria, and for the prevalence estimation a frequency of 35% Staphylococcus sp. penicillin resistant; an absolute precision of 12%; and 90% confidence level were used. Bacteria were isolated from composite milk samples obtained from all quarters of each cow after discarding the initial three or four streams of milk. To access potential risk factors, animal characteristics were obtained through an interview with the producers. Laboratory tests were done according to National Mastitis Council recommendations. A total of 242 isolates was obtained from 195 cows out of 251 cows sampled. The prevalence of animal infections was described in groups according to the epidemiological profile: environmental, contagious and other bacteria. These were 57.3%, 26.3% and 11.2%, respectively of the sampled animals. Antimicrobial susceptibility tests against 12 different antimicrobials were performed in 159 isolates. Altogether, 30% of the isolates tested showed resistance to at least three different antimicrobial groups and were classified as multidrug-resistant. Higher frequencies of resistance were observed against ampicillin to coagulase-negative staphylococci, followed by erythromycin to coagulase-positive staphylococci and tetracycline to streptococci. The logistic regression analysis showed a significant relationship between age of the cows and presence of multidrug-resistant coagulase-positive staphylococci and distribution of different class of bacteria, suggesting a competition dynamic throughout the ages (p < 0.05). Animals with three to four years old had 13.7 times more chances (IC95% 1.4 - 130.2; p = 0,02) to have multidrug-resistant coagulase-positive staphylococci compared to those with two to three years. Time of exposure to infectious agents and consequent therapies suggests a greater chance of udder's colonization by resistant pathogens due to repeatedly selection pressure during lifetime.

O objetivo desse estudo foi analisar uma série de casos de pacientes brasileiros com doença de Niemann-Pick tipo C (NP-C). Método Correlação entre manifestações clínicas, alterações laboratoriais, estudo molecular e resposta ao tratamento foram realizadas. Resultado A amostra consiste de 5 pacientes com idade entre 8 e 26 anos. Paralisia do olhar vertical supranuclear, ataxia cerebelar, demência, distonia e disartria estavam presentes em todos os casos. Coloração de filipina na cultura de fibroblastos mostrou padrão “clássico” em dois pacientes e padrão “variante” em três casos. O estudo molecular encontrou mutações no gene NPC1 em todos os alelos. O tratamento com miglustate foi realizado em 4 pacientes. Conclusão Embora coloração de filipina seja utilizada para confirmar o diagnóstico, o histiócito azul-marinho no aspirado de medula óssea frequentemente auxilia a confirmar o diagnóstico de NP-C. A mutação p.P1007A está correlacionada com o padrão “ variante” na coloração de filipina. A resposta ao tratamento com miglustate parece estar correlacionada com a idade e escore de desabilidade no momento do diagnóstico.
The aim of the study was to analyze a series of Brazilian patients with Niemann-Pick disease type C (NP-C). Method Correlations between clinical findings, laboratory data, molecular findings and treatment response are presented. Result The sample consisted of 5 patients aged 8 to 26 years. Vertical supranuclear gaze palsy, cerebellar ataxia, dementia, dystonia and dysarthria were present in all cases. Filipin staining showed the “classical” pattern in two patients and a “variant” pattern in three patients. Molecular analysis found mutations in the NPC1 gene in all alleles. Miglustat treatment was administered to 4 patients. Conclusion Although filipin staining should be used to confirm the diagnosis, bone marrow sea-blue histiocytes often help to diagnosis of NP-C. The p.P1007A mutation seems to be correlated with the “variant” pattern in filipin staining. Miglustat treatment response seems to be correlated with the age at disease onset and disability scale score at diagnosis.
