RESUMO Objetivo Verificar e comparar os efeitos da terapia manual laríngea (TML) e da estimulação elétrica nervosa transcutânea (TENS) na diadococinesia laríngea de mulheres disfônicas. Método Participaram 20 mulheres com nódulos vocais, divididas igualmente por sorteio em: Grupo TML–aplicação de TML; Grupo TENS–aplicação de TENS; ambos receberam 12 sessões de tratamento, duas vezes por semana, 20 minutos cada, pelo mesmo terapeuta. As mulheres foram avaliadas quanto à diadococinesia (DDC) laríngea em três momentos, diagnóstico, pré-tratamento e pós-tratamento, o que produziu três grupos de medidas. A gravação da DDC foi realizada por meio da repetição entrecortada das vogais: /a/ e /i/. A análise da DDC foi realizada pelo programa Motor Speech Profile Advanced (MSP)-KayPentax. Os parâmetros da DDC das três avaliações foram comparados entre si pelo teste t pareado (p≤0,05). Resultados Parâmetros DDC se apresentaram semelhantes na fase sem tratamento, indicando que não houve variabilidade individual ao longo do tempo. Não houve modificação em relação à velocidade da DDC após intervenções, mas após TML, a DDC da vogal /i/ se apresentou mais estável em relação à duração do período e à intensidade das emissões. Estes resultados indicam que TML melhorou a coordenação de movimentos das pregas vocais à fonação. Não houve modificações dos parâmetros da DDC em relação à estabilidade das emissões após TENS. Conclusão TML promove maior regularidade de movimentos diadococinéticos das pregas vocais em mulheres disfônicas, o que amplia o conhecimento sobre o efeito do reequilíbrio da musculatura laríngea na função fonatória, já TENS não proporciona efeitos na diadococinesia laríngea.

ABSTRACT Purpose To verify and compare the effect of transcutaneous electrical nerve stimulation (TENS) and laryngeal manual therapy (LMT) on laryngeal diadochokinesis (DDK) of dysphonic women. Methods Twenty women with bilateral vocal nodules participated and were equally divided into: LMT Group – LMT application; TENS Group – TENS application; both groups received 12 sessions of treatment, twice a week, with a duration of 20 minutes each, applied by the same therapist. The women were evaluated as to laryngeal DDK at three moments: diagnostic, pre-treatment, and post-treatment, which produced three groups of measurements. The DDK recording was performed with intersected repetition of vowels /a/ and / i/. The analysis of vowels was performed by the program Motor Speech Profile Advanced (MSP)-KayPentax. The DDK parameters of the three evaluations were compared by means of the paired t-test (p≤0.05). Results The measurements of laryngeal DDK parameters were similar in the phase without treatment, indicating no individual variability over time. There was no change with respect to the speed of DDK after intervention, but after LMT, DDK of the vowel /i/ was more stable in terms of the duration of the emissions and intensity of emissions repeated. These results show improved coordination of vocal folds movement during phonation. There were no changes in the DDK parameters following TENS. Conclusion LMT provides greater regularity of movement during laryngeal diadochokinesis in dysphonic women, which extends knowledge on the effect of rebalancing the larynx muscles during phonation, although TENS does not impact laryngeal diadochokinesis.

Uma das alterações mais complexas que acometem as pregas vocais é a incompetência ou insuficiência glótica. Pode ser causada por alterações de mobilidade, fibroses, atrofias ou arqueamento das pregas vocais, e pode levar, entre outras situações como aspiração e tosse pouco efetiva, a graus variados de disfonia. A partir do início do século 20, surgiram vários procedimentos cirúrgicos para a reabilitação da competência aerodinâmica e valvular da glote, por meio da injeção de substâncias heterólogas no espaço paraglótico. Os enxertos autólogos, como a gordura e a fáscia muscular, inseridos ou injetados nas pregas vocais inicialmente mostraram resultados promissores além de segurança e riscos mínimos de reações indesejadas. Neste artigo de revisão, os autores discutem o uso da gordura e da fáscia muscular na incompetência glótica, abordando aspectos históricos, o processo inflamatório gerado após a enxertia, as técnicas cirúrgicas mais utilizadas e o rendimento dos enxertos.

Glottic insufficiency has been managed since the beginning of the twentieth century. The autologous grafts, as fat and muscular fascia, have shown safety and good results. The authors discuss the advantages and disadvantages of using fat and fascia in the management of glottic insufficiency, regarding historical aspects, inflammatory process and surgical techniques.

Vários materiais têm sido injetados ou inseridos em pregas vocais na tentativa de solucionar a incompetência glótica. Contudo, poucos são os estudos que avaliam o processo cicatricial decorrente da enxertia destes materiais. OBJETIVO: O objetivo desta pesquisa foi avaliar a concentração de colágeno após enxerto microcirúrgico de fáscia muscular e gordura na prega vocal de coelho FORMA DE ESTUDO: experimental MATERIAL E MÉTODO: Dezenove coelhos foram submetidos à inserção de enxerto na prega vocal direita, sendo nove de fáscia e dez de gordura. A prega vocal esquerda foi submetida ao mesmo procedimento, sem enxertia. Os coelhos foram sacrificados após 90 e 180 dias. O colágeno foi analisado morfometricamente através do método da Picrossírius-polarização com a utilização do software Image Pro Plus RESULTADOS: Houve aumento do colágeno em todos grupos enxertados quando comparados com o grupo controle. A concentração do colágeno encontrada nos coelhos submetidos a enxerto de gordura foi significativamente maior quando comparados à concentração do colágeno nos coelhos submetidos a enxerto de fáscia muscular, tanto com 90 quanto com 180 dias CONCLUSÃO: A enxertia de gordura e fáscia lata na prega vocal de coelho promoveu maior deposição de colágeno do que no grupo controle, sendo mais exuberante na inserção de gordura.

Several materials have been injected or introduced in the vocal folds in attempt of solving the glottic insufficiency. However, few studies have evaluated the cicatricial process due to the implantation of these materials. AIM: The objective of this research was to evaluate the concentration of collagen after microsurgery graft of muscular fascia and fat in the vocal folds of rabbit STUDY DESIDN: experimental MATERIAL AND METHOD: Nineteen rabbits were submitted to the graft insert in the right vocal fold, being nine of fascia and ten of fat. The left vocal fold was submitted to the same process, except for the insertion of fat or fascial graft. The rabbits were sacrificed after 90 and 180 days. The collagen was analyzed through the method of the Picrosirius-polarization using the Image Pro Plus software RESULTS: There was prevalence of the collagen in all grafted groups when compared with the group control. The concentration of the collagen found in the rabbits submitted to fat graft was significantly larger when compared to the concentration of the rabbits submitted to graft of muscular fascia, either with 90 as with 180 days CONCLUSION: The fat and muscular fascia implantation in the vocal folds of rabbit promoted production of collagen, being more intense with fat.

Em algumas situações clínicas ou durante procedimentos cirúrgicos sobre o arcabouço laríngeo, faz-se importante o conhecimento preciso das dimensões das pregas vocais. OBJETIVO: Avaliar a existência de correlação entre as características dos indivíduos estudados como altura, peso, idade e sexo com as dimensões macroscópicas da porção membranosa das pregas vocais. FORMA DO ESTUDO: Experimental. MÉTODOS: Foram estudados 24 cadáveres do Serviço de Verificação de Óbito entre Abril e Outubro de 2001, sendo 13 homens e 11 mulheres. De cada indivíduo, foram obtidos os dados de altura, peso e idade com posterior remoção da laringe. Foi utilizado paquímetro digital para as medidas das dimensões das pregas vocais. RESULTADOS: Não foram encontradas diferenças entre as pregas vocais direita e esquerda quanto ao comprimento (p=0,91), largura (p=0,93) ou espessura (p=0,72). As pregas vocais masculinas foram maiores (p<0,001) e mais largas (p=0,04) que as femininas, não havendo diferença quanto à espessura (p=0,11). Foi observada uma forte correlação (r=0,76) entre a altura dos indivíduos e o comprimento das pregas vocais. Não houve correlação entre altura e largura (r=0,36) ou espessura (r=0,39) das pregas vocais. CONCLUSÕES: As pregas vocais masculinas foram maiores e mais largas do que as femininas. O comprimento da porção membranosa das pregas vocais variou em função da altura dos indivíduos.

In some clinical situations or during laryngoplastic procedures, it is important the knowledge of the dimensions of the vocal folds. AIM: The aim of this study was to evaluate the correlation existence among the individuals' characteristics studied as height, weight, age and sex with the macroscopic dimensions of the membranous portion of the vocal folds. STUDY DESIGN: Experimental. METHODS: We studied 24 cadavers (13 men and 11 women) submitted to autopsy from April and October of 2001. Of each individual, we obtained height, weight and age with subsequent removal of the larynx. We used digital pachimetry for the measures of the dimensions of the vocal folds. RESULTS: We did not find difference between the vocal folds right and left in length (p = 0,91), width (p = 0,93) or thickness (p = 0,72). The masculine vocal folds were larger (p <0,001) and wider (p = 0,04) that the feminine ones, and it did not have difference between the thickness (p = 0,11). We observed a strong correlation (r=0,76) between the individuals' height and the length of the vocal folds. We did not find correlation between height and width (r=0,36) or thickness (r=0,39) of the vocal folds. CONCLUSIONS: Males vocal folds were larger and wider than females'. The length of the membranous portion of the vocal folds varied in function of the individuals' height.
