RESUMO A contemporaneidade e a docência nas áreas de saúde e formação profissional, tem se transformado e evoluído nos últimos tempos, com a introdução das metodologias ativas no processo ensino-aprendizagem, nas instituições de ensino superior no Brasil. Essas metodologias ativas de aprendizagem, como a ABP, Aprendizagem Baseada em Problemas, são utilizadas com a finalidade de que estudantes da área da saúde adquiram o conhecimento de forma significativa, em relação ao ensino tradicional. A educação médica no Brasil tem sido assunto de muitas discussões sobre a formação profissional em saúde e vem passando por significativas mudanças nos últimos anos. O presente artigo tem por objetivo relatar uma experiência docente no ensino de graduação em Medicina, nos componentes curriculares Mecanismo de Agressão e Defesa I e II (MADs), na Universidade do Estado da Bahia – Campus I. MADs é um componente curricular atualmente presente em diferentes cursos da área de saúde do Brasil. Este componente é composto pelas disciplinas microbiologia, parasitologia, imunologia e patologia, lecionado de forma integrada. Foram aplicadas metodologias ativas de ensino, do tipo aprendizagem baseada em problemas (ABP), que abrangeram sessões tutoriais, atividades em laboratórios, apresentações científicas e o uso de filmes. Além disto, foram utilizados recursos lúdicos e a ferramenta MOODLE no campus virtual. Esta proposta teve início no ano de 2012 e vem sendo aplicada a partir desta data até os dias atuais. A experiência vivenciada nas MADs, com a utilização de variadas formas de metodologias de ensinagem, tem permitido “romper” com o tradicional, isto é, tem levado o discente a desenvolver habilidades, competências e construção dos saberes. Estas novas alternativas de ensino nos permitiu uma interação mais ampla entre a relação discente-docente e nos fez refletir e avançar no processo avaliativo, que tem sido realizado de forma ampla e processual. Além disso, um impacto positivo para sua formação pode ser observado. No que se refere ao processo de ensino, aprendizagem e avaliações, novas propostas pedagógicas devem ser inseridas priorizando o caráter formativo e de desenvolvimento dos discentes, para que haja a construção de competências necessárias ao perfil estabelecido pelas DCNs, Diretrizes Curriculares Nacionais, do curso de Graduação em Medicina e demais cursos de saúde.

ABSTRACT Changes are occurring in health teaching and professional training, due to the introduction of active methodologies to the teaching-learning process in Brazilian higher education institutions. These active learning methodologies, such as PBL (Problem-Based Learning), are used to help medical students acquire knowledge in a more relevant way, compared to traditional teaching. Medical education in Brazil has been the subject of much discussion, and has undergone significant changes in recent years. The objective of the present work is to report the teaching experience in the curricular components Mechanisms of Aggression and Defense I (MAD I) and Mechanisms of Aggression and Defense II (MAD II) of Universidade do Estado da Bahia [State University of Bahia] - Campus I in undergraduate teaching in Medicine. These subjects are composed of the disciplines microbiology, parasitology, immunology (MAD I and II) and pathology (MAD II), taught in an integrated way. Active teaching methodologies were applied, such as problem-based learning, laboratory activities, scientific presentations and the use of film. In addition, the MOODLE tool was used in the virtual campus. This proposal began in 2012 and has been applied since then. The experience of the MADs, with the use of different forms of teaching methodologies, has led to a transformation of traditional teaching, leading students to develop their skills and competencies and build knowledge. These new teaching alternatives allow for a broader interaction between the students and the teacher and led us to reflect and advance in the evaluation process, which has been carried out in a broad and procedural way. Moreover, a positive impact on the students’ training was observed. As for the processes of teaching, learning and evaluation, new pedagogical proposals should be introduced, prioritizing the formative and developmental character of students so that the necessary competences can be built into the profile established by the National Curricular Guidelines (DCNs) of the Undergraduate Medicine course and other courses in the area of health.

Resumo O parâmetro T 90 de Escherichia coli é muito empregado na modelagem hidrodinâmica de plumas de emissários submarinos. Apesar da extensa literatura disponível que aplica T 90 em modelos hidrodinâmicos, não existe uma padronização metodológica para sua avaliação que seja validada até hoje. Além das taxas de decaimento, muitos outros processos biológicos, químicos e físicos deveriam ser considerados nas modelagens hidrodinâmicas. Nas últimas três décadas, embora um extenso conhecimento sobre biologia e ecologia microbiana tenha sido desenvolvido, pouco deste conhecimento foi incorporado às aplicações. Consequentemente, os modelos de plumas de esgotos não lograram reproduzir de maneira fiel os processos ambientais. No presente artigo, os trabalhos relevantes que poderiam melhorar a determinação do T 90 , sob abordagens mais robustas e realistas foram revisados. A literatura mais relevante desde os anos 1990 foi considerada, juntamente com alguns trabalhos relevantes mais antigos. A pesquisa focou em técnicas e metodologias de avaliação de aspectos ecológicos e de biologia celular. Uma breve discussão dos métodos aplicados à avaliação do T 90 examinou aspectos como a representatividade das incubações, escala dos experimentos de incubação e procedimentos de medição, com ênfase em modelos de decaimento bacteriano. Os fatores que afetam o decaimento bacteriano como radiação solar, salinidade e interação com sólidos foram considerados à luz do extenso registro da literatura. Finalmente, aspectos biológicos que afetam o decaimento como predação e resistência genética foram analisados. O entendimento atualizado resultante da pesquisa com T 90 pode contribuir para o desenvolvimento de procedimentos de gestão que podem melhorar a qualidade da água em sistemas costeiros e continentais, dando soluções para problemas de saúde em populações humanas.

Abstract The Escherichia coliT 90 parameter is widely employed in outfall plume hydrodynamic models. Although there is a great deal of academic literature estimating T 90 , no standard methodology has been validated so far. Estimation of die-off rates should consider many other biological, chemical and physical processes. Over the last three decades, while a great deal of new knowledge about microbial biology and ecology has become available, it has not been fully incorporated into the overall research. Consequently, hydrodynamic models of sewage plumes lack consistency with environmental dynamics and frequently do not fully reproduce processes. This article scrutinized the relevant works that could improve T 90 assessment using the most robust and realistic approaches. The most important related literature since the 1990s was considered, together with earlier relevant works. The research focused on techniques and methodologies, especially their ecologic and cellular biology aspects. A brief discussion of the methods applied to evaluate the T 90 of bacteria examined aspects like representativeness of incubation, incubation scale, measurement procedures and models’ representations of bacterial decay. The factors affecting population decay, such as solar radiation, salinity and interaction with solids, were considered in the light of the very large literature registry. Finally, biological aspects affecting decay, like predation and genetic resistance, were also analyzed. The resulting updated understanding of T 90 may contribute to better managerial procedures that could improve water quality in freshwater and coastal systems, and consequently improve human health outcomes.

Objetivo: Relatar a experiência de um hospital terciário no tratamento de pacientes pediátricos com imunodeficiências primárias com transplante de células-tronco hematopoéticas. Métodos: De 2007 a 2010, foram realizados oito transplantes em sete pacientes pediátricos com imunodeficiências primárias: imunodeficiência combinada grave (n = 2); imunodeficiência combinada (n = 1); doença granulomatosa crônica (n = 1); síndrome hiper-IgM (n = 2); síndrome IPEX (n=1). Resultados: Dois pacientes foram transplantados com medula óssea de irmãos HLA-idênticos; seis transplantes foram feitos com doadores não aparentados (medula óssea: n = 1; sangue de cordão umbilical: n = 5). Todos os pacientes haviam tido episódios de infecção grave previamente ao tratamento. Um paciente recebeu apenas globulina antitimocítica antes do transplante de células-tronco hematopoéticas, três transplantes foram feitos com quimioterapia de intensidade reduzida e quatro após quimioterapia mieloablativa. Dois pacientes morreram precocemente e não foram avaliados em relação à enxertia. Três pacientes tiveram enxertia completa, dois evoluíram com falha primária de pega, um deles recebeu um segundo transplante com pega do enxerto. Dois pacientes morreram de toxicidade do transplante (síndrome da obstrução sinusoidal hepática), um paciente morreu de insuficiência respiratória por infecção por parainfluenza já existente antes do transplante. Quatro pacientes estão vivos e bem entre 60 dias e 14 meses após o transplante. Conclusão: A condição do paciente ao transplante é o fator mais importante no sucesso do tratamento. O diagnóstico precoce dos pacientes e a possibilidade de encaminhá-los mais rapidamente para tratamento em centros de referência podem melhorar substancialmente a sobrevida e a qualidade de vida deles.

Objective: To report the experience of a tertiary care hospital with allogeneic hematopoietic stem cell transplantation in children with primary immunodeficiencies. Methods: Seven pediatric patients with primary immunodeficiencies (severe combined immunodeficiency: n = 2; combined immunodeficiency: n = 1; chronic granulomatous disease: n = 1; hyper-IgM syndrome: n = 2; and IPEX syndrome: n = 1) who underwent eight hematopoietic stem cell transplants in a single center, from 2007 to 2010, were studied. Results: Two patients received transplants from HLA-identical siblings; the other six transplants were done with unrelated donors (bone marrow: n = 1; cord blood: n = 5). All patients had pre-existing infections before hematopoietic stem cell transplants. One patient received only anti-thymocyte globulin prior to transplant, three transplants were done with reduced intensity conditioning regimens and four transplants were done after myeloablative therapy. Two patients were not evaluated for engraftment due to early death. Three patients engrafted, two had primary graft failure and one received a second transplant with posterior engraftment. Two patients died of regimen related toxicity (hepatic sinusoidal obstruction syndrome); one patient died of progressive respiratory failure due to Parainfluenza infection present prior to transplant. Four patients are alive and well from 60 days to 14 months after transplant. Conclusion: Patients’ status prior to transplant is the most important risk factor on the outcome of hematopoietic stem cell transplants in the treatment of these diseases. Early diagnosis and the possibility of a faster referral of these patients for treatment in reference centers may substantially improve their survival and quality of life.

BACKGROUND: Several studies have shown that stress and emotional reactions can affect immune responses in animals and humans. OBJECTIVE: The aim of this study was to evaluate hematological and immunological effects of stress on air traffic controllers. METHODS: Thirty air traffic controllers and 15 aeronautical information service operators were evaluated. The groups were divided as information service operators with 10 years or more of experience (AIS>10) and with less than 10 years in the profession (AIS<10) and air traffic controllers with 10 years or more of experience (ATCo>10) and with less than 10 years in the profession (ATCo<10). Blood samples were drawn at 8:00 a.m. and 2:00 p.m. The paired t-test was used to compare monocyte and nitric oxide concentrations and ANOVA was used for the other parameters. RESULTS: The ATCo>10 group presented a significantly lower phagocytosis rate of monocytes at 2:00 p.m. compared to 8:00 a.m. Moreover, the ATCo>10 group presented lower hemoglobin, mean corpuscular hemoglobin concentration, platelet and leukocyte levels, and increased cortisol concentrations at 8:00 a.m. compared to the other groups. Additionally, this group had lower phagocytosis rate of monocytes, and hemoglobin, platelet, leukocyte, basophils and nitric oxide levels at 2:00 p.m. compared to the other groups. CONCLUSION: Stress seems to greatly affect immune responses of air traffic controllers with more than ten years of experience.
