ABSTRACT Background: Hypersensitivity reactions during chemotherapy have been increasingly documented in children. Drug desensitization is an unknown procedure for most oncologists, despite allowing patients to be treated with first-line agents, affecting the final prognosis. Currently, there are no guidelines for drug desensitization in pediatric age, and the general principles adopted for adults have been applied to children. Objective: To describe the experience of an Immunoallergology department of a central hospital in the desensitization to carboplatin and methotrexate in pediatric age. Results: Desensitizations were performed on carboplatin in 17 patients, in a total of 112 procedures, and on methotrexate in 4 children, with a success rate of 88% for carboplatin and 100% for methotrexate. Conclusion: As far as the authors are aware, this is the largest European series of desensitizations to chemotherapy in pediatric age, including procedures in infants and in non-immediate reactions.

RESUMO Fundamentos: As reações de hipersensibilidade durante a quimioterapia em crianças têm sido documentadas de forma crescente. A dessensibilização a fármacos é um procedimento desconhecido por grande parte dos oncologistas, apesar de permitir que os doentes sejam tratados com agentes de primeira linha, afetando o prognóstico final. Atualmente, ainda não existem normas de orientação de dessensibilização a fármacos em idade pediátrica e os princípios gerais adotados para os adultos têm sido aplicados em crianças. Objetivo: Descrever a experiência de um serviço de imunoalergologia de um hospital central na dessensibilização à carboplatina e ao metotrexato em idade pediátrica. Resultados: Foram realizadas dessensibilizações à carboplatina em 17 crianças, num total de 112 procedimentos, e ao metotrexato em 4 crianças, com uma taxa de sucesso de 88% para a carboplatina e de 100% para o metotrexato. Conclusão: Tanto quanto é do conhecimento das autoras, esta é a maior série europeia de dessensibilizações a quimioterápicos em idade pediátrica, tendo sido realizadas, inclusivamente, procedimentos num lactente e numa reação não imediata.

Abstract Chagas disease is a neglected parasitic disease caused by Trypanosoma cruzi, whose treatment has remained unsatisfactory for over 50 years, given that it is limited to two drugs. Benznidazole (BZN) is an efficient antichagasic drug used as the first choice, although its poor water-solubility, irregular oral absorption, low efficacy in the chronic phase, and various associated adverse effects are limiting factors for treatment. Incorporating drugs with such characteristics into nanostructured lipid carriers (NLC) is a promising alternative to overcome these limiting obstacles, enhancing drug efficacy and bioavailability while reducing toxicity. Therefore, this study proposed NLC-BZN formulations in different compositions prepared by hot-melt homogenization followed by ultrasound, and the optimized formulation was characterized by FTIR, DRX, DSC, and thermogravimetry. Biological activities included in vitro membrane toxicity (red blood cells), fibroblast cell cytotoxicity, and trypanocidal activity against epimastigotes of the Colombian strain of T. cruzi. The optimized NLC-BZN had a small size (110 nm), negative zeta potential (-18.0 mV), and high encapsulation (1.64% of drug loading), as shown by infrared spectroscopy, X-ray diffraction, and thermal analysis. The NLC-BZN also promoted lower in vitro membrane toxicity (<3% hemolysis), and 50% cytotoxic concentration (CC50) for NLC-BZN in L929 fibroblast cells (110.7 µg/mL) was twice the value as the free BZN (51.3 µg/mL). Our findings showed that the NLC-BZN had higher trypanocidal activity than free BZN against the epimastigotes of the resistant Colombian strain, and this novel NLC-BZN formulation proved to be a promising tool in treating Chagas disease and considered suitable for oral and parenteral administration.

ABSTRACT Objective: Characterizing the role of an Allergy and Clinical Immunology (ACI) department in the evaluation of patients admitted to care in other specialties. Methods: Review of all inpatient collaboration requests to the ACI department between January 2018 and June 2019. Analysis of patients’ clinical and demographic data, causes of admission, aim of the collaboration request, evaluation and case management was performed. Results: We identified 177 collaboration requests, corresponding to 157 admissions of patients with an age range from 7 months to 89 years, 52% of which were women. The majority of the requests (83%) were due to suspected drug hypersensitivity reaction, followed by guidance on the treatment of allergic comorbidity and asthma exacerbations. These 177 requests resulted in 446 medical visits (average of 2.6 visits per admission). Drug hypersensitivity reaction was confirmed in 64%, with SCAR (severe cutaneous adverse reactions) in 19% and anaphylaxis in 6%. Mortality rate was 5.1%; 46.2% had a re-evaluation at the outpatient clinic. Conclusions: Allergy and Clinical Immunology specialists primarily work at the outpatient setting. However, they can have a very significant role in the management of hospitalized patients particularly in the diagnosis and treatment of uncommon and allergy related diseases.

RESUMO Objetivo: Avaliar e caracterizar o papel da especialidade de Imunoalergologja (IA) no apoio a doentes internados ao cuidado de outras especialidades. Métodos: Revisão dos pedidos de colaboração realizados ao serviço de IA entre janeiro de 2018 a junho de 2019, através da consulta dos processos clínicos dos doentes, com colheita de dados demográficos, motivo de internamento e do pedido de colaboração, diagnóstico imunoalergológico e orientação. Resultados: Foram identificados 177 pedidos de colaboração correspondentes a 157 internamentos de doentes com idade entre os 7 meses e os 89 anos, 52% dos quais do sexo feminino. Os pedidos mais frequentes resultaram da suspeita de reação de hipersensibilidade a fármacos (83%), seguido de pedidos para apoio na decisão terapêutica e exacerbação de asma. Os 177 pedidos originaram 446 visitas médicas de IA a internamentos (média de 2,6 visitas/doente). A suspeita de hipersensibilidade a fármacos foi confirmada em 64% dos casos, com diagnóstico de reações cutâneas graves em 19% e anafilaxia em 6%. A taxa de mortalidade foi de 5,1%, e 46,2% foram posteriormente avaliados em consulta externa de IA. Conclusões: Apesar do papel predominantemente orientado para a atividade em ambulatório, o apoio do Imunoalergologista ao internamento é essencial, sobretudo em patologias específicas, e por vezes incomuns, da especialidade.

Abstract Objectives: This work aimed to evaluate and compare the surface roughness, contact angle, and C. albicans adhesion in three materials for removable denture bases: polymethylmethacrylate and the thermoplastic resins polypropylene and polyolefin. Methods: Nine samples were manufactured, three of each material (n=3): polypropylene, polyolefin, and polymethylmethacrylate (control group). The thermoplastic resins were fabricated by injection technique and the polymethylmethacrylate by compression technique. After laboratory polishing, the surface roughness (Ra and Rz) was assessed with the 3D optical roughness meter NPFLEX, and the contact angle was measured with the optical tensiometer Contact angle system OCA15. C. albicans adhesion was assessed through colony- forming unit assays (n=9). ANOVA and Kruskal-Wallis tests were done to compare groups, considering a significance level of 0.05. Results: All values obtained for the control group were lower than those obtained for the thermoplastic resins. Statistically significant differences were found: for Ra between polypropylene (0.15±0.072 μm) and polymethylmethacrylate (0.03±0.006 μm) (p=0.045); for the contact angle between the three groups (p<0.001) - polymethylmethacrylate (26.57±6.719°), polypropylene (57.57±11.098°), and polyolefin (69.06±8.955°); and for C. albicans adhesion between polymethylmethacrylate and polypropylene (p=0.007) and polymethylmethacrylate and polyolefin (p=0.013). Conclusions: In the tested conditions and for the materials assessed, polymethylmethacrylate showed the lowest roughness, contact angle, and C. albicans adhesion, thus exhibiting better properties for removable denture bases than the thermoplastic materials.

Resumo Objetivos: Este trabalho pretende avaliar e comparar os parâmetros de rugosidade de superfície, ângulo de contacto e adesão de C. albicans em três materiais de base para próteses removíveis: o polimetilmetacrilato e as resinas termoplásticas polipropileno e poliolefina. Métodos: Foram confecionados 9 espécimes, 3 de cada um dos seguintes materiais (n=3): polipropileno, poliolefina e polimetilmetacrilato (grupo controlo). As resinas termoplásticas foram produzidas por técnica de injeção, e o polimetilmetacrilato por técnica de compressão. Após o polimento laboratorial, avaliou-se a rugosidade de superfície (Ra e Rz) com o rugosímetro ótico 3D NPFLEX e o ângulo de contacto com o tensiómetro ótico Contact angle system OCA15. A adesão de C. albicans foi avaliada por quantificação das unidades formadoras de colónias (n=9). Os testes ANOVA e Kruskal-Wallis foram utilizados para comparação dos resultados entre os grupos, com um nível de significância de 0,05. Resultados: Todos os valores obtidos para o grupo de controlo foram inferiores aos das resinas termoplásticas. Verificaram-se diferenças estatisticamente significativas: na Ra, entre o polipropileno (0,15±0,072 μm) e o polimetilmetacrilato (0,03±0,006 μm) (p=0,045); no ângulo de contacto, entre os 3 grupos (p<0,001) - PMMA (26,57±6,719°), polipropileno (57,57±11,098°) e poliolefina (69,06±8,955°); e na adesão de C. albicans, entre o polimetilmetacrilato e a polipropileno (p=0,007), e entre o polimetilmetacrilato e a poliolefina (p=0,013). Conclusões: Nas condições testadas e para os materiais estudados, o polimetilmetacrilato apresentou menor rugosidade, menor ângulo de contacto e menor adesão de C. albicans, evidenciando melhores características para base de prótese removível do que os materiais termoplásticos.

Abstract Background Migraine affects 1 billion people worldwide and > 30 million Brazilians; besides, it is an underdiagnosed and undertreated disorder. Objective The need to disseminate knowledge about the prophylactic treatment of migraine is known, so the Brazilian Headache Society (SBCe, in the Portuguese acronym) appointed a committee of authors with the objective of establishing a consensus with recommendations on the prophylactic treatment of episodic migraine based on articles from the world literature as well as from personal experience. Methods Meetings were held entirely online, with the participation of 12 groups that reviewed and wrote about the pharmacological categories of drugs and, at the end, met to read and finish the document. The drug classes studied in part II of this Consensus were: antihypertensives, selective serotonin reuptake inhibitors, serotonin and norepinephrine reuptake inhibitors, calcium channel blockers, other drugs, and rational polytherapy. Results From this list of drugs, only candesartan has been established as effective in controlling episodic migraine. Flunarizine, venlafaxine, duloxetine, and pizotifen were defined as likely to be effective, while lisinopril, enalapril, escitalopram, fluvoxamine, quetiapine, atorvastatin, simvastatin, cyproheptadine, and melatonin were possibly effective in prophylaxis of the disease. Conclusions Despite an effort by the scientific community to find really effective drugs in the treatment of migraine, given the large number of drugs tested for this purpose, we still have few therapeutic options.

Resumo Antecedentes Migrânea afeta um bilhão de pessoas em todo o mundo e mais de 30 milhões de brasileiros; além disso, é um distúrbio subdiagnosticado e subtratado. Objetivo Sabe-se sobre a necessidade de difundir o conhecimento sobre o tratamento profilático da migrânea; por isso, a Sociedade Brasileira de Cefaleias (SBCe) nomeou um comitê de autores com o objetivo de estabelecer um consenso com recomendações sobre o tratamento profilático da migrânea episódica com base em artigos da literatura mundial, assim como da experiência pessoal. Métodos As reuniões foram realizadas inteiramente online, com a participação de 12 grupos que revisaram e escreveram sobre as categorias farmacológicas dos medicamentos e, ao final, reuniram-se para a leitura e conclusão do documento. As classes de medicamentos estudadas na parte II deste Consenso foram: anti-hipertensivos, inibidores seletivos de recaptação de serotonina, inibidores de recaptação de serotonina e noradrenalina, bloqueadores dos canais de cálcio, outros medicamentos e politerapia racional. Resultados Desta lista de medicamentos, apenas o candesartan foi estabelecido como eficaz no controle da migrânea episódica. Flunarizina, venlafaxina, duloxetina e pizotifeno foram definidos como provavelmente eficazes, enquanto lisinopril, enalapril, escitalopram, fluvoxamina, quetiapina, atorvastatina, sinvastatina, ciproheptadina e melatonina foram possivelmente eficazes na profilaxia da doença. Conclusões Apesar do esforço da comunidade científica em encontrarmedicamentos realmente eficazes no tratamento da migrânea, dado o grande número de medicamentos testados para este fim, ainda dispomos de poucas opções terapêuticas.

Abstract The Brazilian Headache Society (Sociedade Brasileira de Cefaleia, SBCe, in Portuguese) nominated a Committee of Authors with the aim of establishing a consensus with recommendations regarding prophylactic treatment for episodic migraine based on articles published in the worldwide literature, as well as personal experience. Migraine affects 1 billion people around the world and more than 30 million Brazilians. In addition, it is an underdiagnosed and undertreated disorder. It is well known within the medical community of neurologists, and especially among headache specialists, that there is a need to disseminate knowledge about prophylactic treatment for migraine. For this purpose, together with the need for drug updates and to expand knowledge of the disease itself (frequency, intensity, duration, impact and perhaps the progression of migraine), this Consensus was developed, following a full online methodology, by 12 groups who reviewed and wrote about the pharmacological categories of the drugs used and, at the end of the process, met to read and establish conclusions for this document. The drug classes studied were: anticonvulsants, tricyclic antidepressants, monoclonal anti-calcitonin gene-related peptide (anti-CGRP) antibodies, beta-blockers, antihypertensives, calcium channel inhibitors, other antidepressants (selective serotonin reuptake inhibitors, SSRIs, and dual-action antidepressants), other drugs, and polytherapy. Hormonal treatment and anti-inflammatories and triptans in minimum prophylaxis schemes (miniprophylaxis) will be covered in a specific chapter. The drug classes studied for part I of the Consensus were: anticonvulsants, tricyclic antidepressants, monoclonal anti-CGRP antibodies, and beta-blockers.

Resumo A Sociedade Brasileira de Cefaleia (SBCe) nomeou um Comitê de Autores com o objetivo de estabelecer um consenso com recomendações sobre o tratamento profilático da enxaqueca episódica com base em artigos da literatura mundial e da experiência pessoal. A enxaqueca é um distúrbio subdiagnosticado e subtratado que acomete um bilhão de pessoas no mundo e mais de 30 milhões de brasileiros. É conhecido na comunidade médica de neurologistas e, sobretudo, dos especialistas em cefaleia, a necessidade de se divulgar o conhecimento sobre o tratamento profilático da enxaqueca. Com esta finalidade, aliada às necessidades de atualizações de drogas e de se aumentar o conhecimento sobre a doença em si (frequência, intensidade, duração, impacto e talvez a progressão da enxaqueca), foi elaborado este Consenso, com metodologia totalmente on-line, por 12 grupos que revisaram e escreveram sobre as categorias farmacológicas das drogas e, ao final, reuniram-se para a leitura e conclusão do documento. As classes de drogas estudadas para este Consenso foram: anticonvulsivantes, antidepressivos tricíclicos, anticorpos monoclonais do antipeptídeo relacionado ao gene da calcitonina (peptídeo relacionado ao gene da calcitonina — anti-CGRP), betabloqueadores, anti-hipertensivos, inibidores dos canais de cálcio, outros antidepressivos (inibidores seletivos de recaptação de serotonina, ISRSs, e antidepressivos de ação dual), outras drogas, e politerapia. O tratamento hormonal, bem como anti-inflamatórios e triptanas em esquema de profilaxia mínima (miniprofilaxia), será abordado em um capítulo próprio. RESUMO A maior parte das infraestruturas hídricas foram projetadas para condições climáticas e demandas que estão mudando rapidamente. Neste estudo, foi investigada a magnitude das inundações e a segurança de barragens em cenários de mudanças climáticas, utilizando um conjunto de modelos climáticos do CMIP6, acoplados a um modelo hidrológico. Foram comparadas as condições climáticas históricas e futuras de uma bacia hidrográfica do Ceará, Nordeste do Brasil. Os modelos revelaram uma ampla gama de níveis de risco de inundação e falha hidrológica. Metade dos cenários climáticos indicaram uma redução no período de retorno das cheias. Uma inundação associada a uma chuva de 1000 anos teve sua probabilidade de ocorrência cerca de 12 vezes maior, quando comparado o cenário SSP5 8.5 com as condições históricas. No cenário mais críticos, a lâmina d’água ultrapassou a altura da crista da barragem. Ao considerar a cheia associada a uma chuva decamilenar, o risco de colapso da barragem foi aumentado. As mudanças climáticas podem aumentar o risco de falha da infraestrutura hidráulica, que precisa ser adaptada para garantir a segurança das partes interessadas do sistema hídrico.

ABSTRACT Most water infrastructure was designed for climate conditions and demands that have been rapidly changing. In this study, we investigate flood magnitude and dam safety under climate change, using an ensemble of CMIP6 climate models, coupled to a hydrological model. We compare historical and future climate conditions of a watershed in Ceará, Northeast Brazil. Climate models revealed a wide range of risk levels of flood and hydrological failure. Half of the climate scenarios indicated a reduction in the flood return period. A flood associated with a 1000-year storm had an occurrence probability about 12 times higher when comparing the SSP5 8.5 scenario with historical conditions. In one more critical scenario, the water depth exceeded the height of the dam's crest. When considering a flood associated with a decamillennial storm, dam collapse risk was increased. Climate change might increase the risk of water infrastructure failure, which needs to be adapted to ensure the safety of the water system stakeholders.

ABSTRACT The objective was to evaluate the effect of different irrigation systems and harvest frequencies on the cultivation of cactus pear. The study was conducted in the semi-arid region, in the municipality of Russas, CE, Brazil. The experiment was a split plot randomized block design, with four replications. Plots were represented by irrigation systems (surface drip, microsprinkler, Micro Spray Jet, and conventional sprinkler) and subplots by harvest frequencies (6, 9, 12, and 18 months). Cactus pear biomass production characteristics were evaluated. Most of the productive characteristics evaluated were influenced by harvest frequency and irrigation system. The harvest frequency of 12 months combined with conventional sprinkler system resulted in a total dry biomass production of 16,400 kg ha−1. The conventional sprinkler system showed the highest number of second-order cladodes (4.9 cladodes plant−1). The conventional sprinkler and Micro Spray Jet systems showed the highest values for various structural characteristics. For water use efficiency (WUE), the highest value was found at the frequency of six months (8.46 kg ha−1 mm−1). Regarding the effects of irrigation system on WUE, the highest values were observed with the conventional sprinkler system (8.20 kg ha−1 mm−1). The harvest frequency of 12 months combined with the conventional sprinkler system presents better results in the evaluated productive characteristics, mainly the cactus pear production by area.

Abstract Background: Joint pain in the absence or with little synovitis is observed in a large percentage of HTLV-1 infected subjects. As the virus infect CD4 +and CD8 +positive, macrophages and B cells an exaggerated production of pro-inflammatory cytokines is detected in these patients. However, the possible association of HTLV-1 infection with autoimmune diseases has not been documented definitively and the clinical characteristics of HTLV-1 associated arthropathy has not been defined. The objective this study is to describe clinic and radiographic features in HTLV-1-infected individuals with complaints of joint pain. Methods: Cross-sectional study enrolling HTLV-1-infected individuals with chronic joint pain, aged up to 75 years, both genders and seronegative controls with osteoarthritis. All participants underwent conventional radiography of the hips, knees and ankles. Results: Eighty-one HTLV-1 infected patients and 30 subjects with osteoarthritis participated in the study. Polyarticular and symmetrical arthritis prevailed in the HTLV-1 positive group (54%), while oligoarticular and asymmetrical (44%) were more common in controls ( p < 0.05). The frequency of enthesophytes (90%) in HTLV-1-infected patients was greater than in the control group (73%) ( p < 0.05). Radiographic features were similar in HTLV-1 carriers and in patients with probable or definite HTLV-1 associated myelopathy. The presence of enthesophytes in the absence of joint space reduction or osteophytes was only observed in HTLV-1-infected individuals ( p < 0.001). Magnetic resonance imaging of the ankles of five HTLV-1-infected patients and five controls demonstrated a higher frequency of enthesitis, bursitis and osteitis in the HTLV-1 infected group. Conclusion: HTLV-1-associated arthropathy is clinically characterized by symmetrical polyarthralgia and the main radiological finding is the presence of enthesophytes in the absence of osteophytes and joint space narrowing.

ABSTRACT This study aims to assess COVID-19 and other respiratory viruses in pediatric patients. Between April 17 and September 30, 2020, we collected 1,566 respiratory samples from 1,044 symptomatic patients who were younger than 18 years old to assess SARS-CoV-2 infection. Of these, 919 were analyzed for other respiratory pathogens (ORP). Patients with laboratory-confirmed COVID-19 or ORP were included. We evaluated 76 pediatric COVID-19 infections and 157 other respiratory virus infections. Rhinovirus occurred in 132/157 (84%). COVID-19 patients who were significantly older, had more fevers, headaches and pneumonia than those with ORP. The median white blood cell count was lower in patients with SARS-CoV-2 than in those with ORP (6,470 versus 8,170; p=0.02). COVID-19 patients had significantly worse symptoms than those with ORP.

ABSTRACT. To be considered drought-tolerant, a tomato cultivar is required to present some level of tolerance at all developmental stages of plant growth. Since drought tolerance is a stage-specific phenomenon, genotype assessment must be performed separately at all developmental stages. In this study, we used a multi-trait index based on factor analysis and genotype-ideotype distance (FAI-BLUP index) to properly rank 49 tomato genotypes according to their tolerance to drought stress conditions at germination and seedling stages. Seeds of 47 introgression lines, which cultivar M82 is considered drought-sensitive, and the Solanum pennellii wild accession LA 716, which is considered drought-tolerant, were subjected to a control condition, where seeds were soaked in distilled water, and a drought condition, where seeds were soaked in a polyethylene glycol (PEG) solution (-0.3 MPa). Drought stress, induced by PEG, had a significant impact on all nine germination and growth performance-related traits; there was a reduction in shoot length (SL), total length (TL), initial germination percentage (IGP), final germination percentage (FGP), germination velocity index (GVI), and germination rate (GR). In contrast, the root-to-shoot ratio (R/S) and time to reach 50% germination (T50) increased under drought stress. Root length (RL) was less affected by drought, and in some genotypes, it was even increased. As expected, LA 716 ranked closest to the drought tolerance ideotype. IL 1-4-18, IL 2-3, IL 1-2, IL 9-2, and IL 10-1 were the most drought-tolerant at the germination stage. These results will serve as guidance for breeders who are aiming at developing drought-resistant tomato cultivars.

Abstract This study aimed to evaluate the effect of extrinsic factors (meat type, price, and nutritional claims) on purchase intention and the perception of intrinsic factors (tenderness, juiciness and flavor) in buffalo loin in a region of Brazil where the marketing of buffalo meat is not common. Photos of the steaks were treated and labels were designed to simulate the retail purchase process, with the effects of extrinsic factors being evaluated through conjoint analysis. Sensory tests of difference from control, purchase intention, and overall acceptance were performed for intrinsic factors. Regarding the purchase process, a determining factor was the price, with buffalo meat having a better purchase intention when associated with a lower price. The lowest price also proved to be more important than the presence of nutritional information for most respondents, however, nutritional information was necessary for consumers characterized over 50 years old. Good acceptance was perceived after tasting buffalo meat, even with less tenderness and juiciness compared to Nellore, and only a minority respondents were willing to pay more for it. The buffalo meat presents a potential option for the consumer, when offered in association with marketing strategies that emphasize nutritional and/or sensory information, such as tenderness and juiciness.

Resumo O objetivo deste estudo foi avaliar o efeito de fatores extrínsecos (tipo de carne, preço e alegações nutricionais) sobre a intenção de compra e a percepção de fatores intrínsecos (maciez, suculência e sabor) em lombo de búfalo, em uma região do Brasil onde a comercialização de carne de búfalo não é comum. As fotos dos bifes foram tratadas e os rótulos foram projetados para simular o processo de compra a varejo, sendo os efeitos dos fatores extrínsecos avaliados através da conjoint analysis. Testes sensoriais de diferença do controle, intenção de compra e aceitação global foram realizados para os fatores intrínsecos. Em relação ao processo de compra, um fator determinante foi o preço, sendo que a carne de búfalo teve melhor intenção de compra quando associada a um menor preço. O menor preço também mostrou ser mais importante do que a presença de informações nutricionais para a maioria dos entrevistados, porém as informações nutricionais foram valorizadas pelos consumidores com mais de 50 anos. Foi percebida boa aceitação após degustação para a carne de búfalo, mesmo com menor maciez e suculência em relação à nelore, sendo que apenas uma minoria dos respondentes estava disposta a pagar mais por ela. A carne bubalina apresenta potencial opção para o consumidor quando sua ofertada é associada a estratégias de marketing, contendo informações nutricionais e/ou sensoriais, como maciez e suculência.

ABSTRACT Motivated by the lack of studies that standardize and optimize the parameters of texture tests, this study aimed to determine the operating conditions for TPA to maximize the discrimination among samples of fruit preserves. The texture of the commercial guava preserves was evaluated using a texturometer. The Design Central Composite Rotational (DCCR) method was applied with four independent variables: speed test, sample volume, time between compression cycles and compression percentage. Only the compression percentage and test speed were significantly influenced by the texture parameters evaluated. The optimum operating region of TPA to better discriminate differences in texture parameters depended on the variable to be optimized, and for adhesiveness a compression of 75% and a compression speed of 0.23 mm·s are recommended. To detect differences among the samples for the parameters of cohesiveness, gumminess and resilience, the use of 15% compression and 2.59 mm·s speed is suggested. In both cases, one must employ the shortest time between two cycles and use a smaller sample size to save both the time of analysis and of the sample, respectively. For the parameters of hardness, elasticity and chewiness, optimal regions were not identified.

ABSTRACT Background: Non-metastatic castration resistant prostate cancer (M0 CRPC) has seen important developments in drugs and diagnostic tools in the last two years. New hormonal agents have demonstrated improvement in metastasis free survival in M0 CRPC patients and have been approved by regulatory agencies in Brazil. Additionally, newer and more sensitive imaging tools are able to detect metastasis earlier than before, which will impact the percentage of patients staged as M0 CRPC. Based on the available international guidelines, a group of Brazilian urology and medical oncology experts developed and completed a survey on the diagnosis and treatment of M0 CRPC in Brazil. These results are reviewed and summarized and associated recommendations are provided. Objective: To present survey results on management of M0 CRPC in Brazil. Design, setting, and participants: A panel of six Brazilian prostate cancer experts determined 64 questions concerning the main areas of interest: 1) staging tools, 2) treatments, 3) side effects of systemic treatment/s, and 4) osteoclast-targeted therapy. A larger panel of 28 Brazilian prostate cancer experts answered these questions in order to create country-specific recommendations discussed in this manuscript. Outcome measurements and statistical analysis: The panel voted publicly but anonymously on the predefined questions. These answers are the panelists' opinions, not a literature review or meta-analysis. Therapies not yet approved in Brazil were excluded from answer options. Each question had five to seven relevant answers including two non-answers. Results were tabulated in real time. Conclusions: The results and recommendations presented can be used by Brazilian physicians to support the management of M0 CRPC patients. Individual clinical decision making should be supported by available data, however, for Brazil, guidelines for diagnosis and management of M0 CRPC patients have not been developed. This document will serve as a point of reference when confronting this disease stage.

Abstract Brazilian Purpuric Fever (BPF) is a hemorrhagic pediatric illness caused by Haemophilus influenzae biogroup aegyptius (Hae), a bacterium that was formerly associated with self-limited purulent conjunctivitis. BPF is assumed to be eradicated. However, the virulence mechanisms inherent to Hae strains associated with BPF is still a mystery and deficient in studies. Here, we aim to analyze the role of the autotransporter genes related to adherence and colonization las, tabA1, and hadA genes through RT-qPCR expression profiling and knockout mutants. Relative quantification by real-time PCR after infection in human cells and infant rat model suggests that las was initially downregulated probably duo to immune evasion, tabA1, and hadA were overexpressed in general, suggesting an active role of TabA1 and HadA1 adhesins in Hae in vitro and in vivo. Transformation attempts were unsuccessful despite the use of multiple technical approaches and in silico analysis revealed that Hae lacks genes related to competence in Haemophilus, which could be part of the elucidation of the difficulty of genetically manipulating Hae strains.
