ABSTRACT The efficacy of direct-acting antivirals (DAAs) in the treatment of chronic hepatitis C (CHC) in liver transplant recipients is poorly understood, and several factors, including immunosuppression, drug interactions, elevated viraemia, and intolerance to ribavirin (RBV), can reduce cure rates. We conducted a real-life study on liver transplant recipients with CHC treated with a combination of sofosbuvir (SOF) and daclatasvir (DCV) or simeprevir (SIM), with or without RBV, followed-up for 12 to 24 weeks. The treatment effectiveness was assessed by determining the sustained virological response (SVR) rates at 12 or 24 weeks after the treatment cessation. Eighty-four patients were evaluated, with a mean age of 63.4 ± 7.4 years, HCV genotype 1 being the most prevalent (63.1%). Nineteen patients (22.7%) had mild fibrosis (METAVIR < F2) and 41 (48.8%) significant fibrosis (METAVIR ≥ F2). The average time between liver transplantation and the start of treatment was 4 years (2.1-6.6 years). The SOF + DCV regimen was used in 58 patients (69%). RBV in combination with DAAs was used in seven patients (8.3%). SVR was achieved in 82 patients (97.6%), and few relevant adverse events could be attributed to DAA therapy, including a patient who stopped treatment due to a headache. There was a significant reduction in ALT, AST, GGT and FA levels, or the APRI index after 4 weeks of treatment, which remained until 12/24 weeks post-treatment. DAA treatment of CHC in liver-transplanted patients achieved a high SVR rate and resulted in the normalization of serum levels of liver enzymes.

RESUMO Nas regiões semi-áridas, as altas taxas de evaporação interferem na disponibilidade de água no solo. Normalmente, é difícil medir e modelar a evaporação devido à complexidade dos métodos disponíveis, ao baixo teor de umidade do solo e à baixa concentração de vapor de água no ar. Isso também pode dificultar o monitoramento e a simulação da evapotranspiração nessas regiões. Assim, o método da Câmara Portátil (CP) é utilizado para medir diretamente a evaporação e a evapotranspiração em tempo real e em curtos intervalos de tempo, dando uma estimativa mais detalhada desses processos. O objetivo deste estudo foi avaliar a evaporação através da transferência de massa no solo numa barragem subterrânea sob diferentes profundidades e condições do lençol freático do ambiente semiárido do Estado de Pernambuco, nordeste do Brasil, através dos valores preditos pelo modelo SiSPAT e medidos pelo método de câmara portátil. Para o propósito da modelagem e também para conhecer melhor o comportamento do solo, as propriedades hidráulicas do solo foram determinadas pelo método de Beerkan. O método da Câmara Portátil apresentou resultados consistentes com o potencial de evaporação do solo nu no semiárido pernambucano/ Nordeste do Brasil, atingindo cerca de 1.800 mm por ano. O modelo SiSPAT foi bastante satisfatório para a simulação da evaporação do solo em diferentes condições da profundidade do lençol freático. Os valores encontrados para a evaporação do solo com a simulação do método SiSPAT e da Câmara Portátil (CP) diferiram em 1,43% e 4,44% para os casos em que o lençol freático estava em 0,20 e 1,20m de profundidade, respectivamente.

ABSTRACT In semi-arid regions, there is a high evaporation, which leads to soil dryness, interfering in the availability of water in the soil. Usually it is difficult to measure and model the evaporation due to the complexity of the available methods, the low soil water content and the low concentration of water vapor in the air. This can also make it difficult to monitor and simulate the evapotranspiration in these regions. Thus, the Portable Chamber method is used to directly measure evaporation and evapotranspiration, because this technique allows real time estimation and in short time intervals, giving a more detailed estimation of those processes. The objective of this study was to evaluate the evaporation through the mass transfer in the soil in an underground dam under different water table depths and conditions of the semi-arid environment of Pernambuco State in Brazil, through the values predicted by the SiSPAT model and measured by the portable chamber method. For the purposing of modeling and also to better know the soil behavior, soil hydraulic properties were determined though the Beerkan method. The portable chamber method was applied for one of the first times in a semi-arid region of Northeastern Brazil, and it was consistent with the potential evaporation of bare soil, reaching about 1,800 mm per year. The SiSPAT model was quite satisfactory for simulation of soil evaporation in different conditions of the water table depth. The values found for soil evaporation with the simulation of the SiSPAT and the Portable Chamber (PC) method differed in 1.43% and 4.44% for cases where the water table was at 0.20 and 1.20m of depth, respectively.

ABSTRACT Introduction: Feet wounds are very common and require multidisciplinary approach for prevention, treatment and rehabilitation. When involving the calcaneus, they offer even greater difficulty and may complicate with osteomyelitis. Debridement of devitalized tissue and antibiotics are important steps for treatment. For the reconstruction, local or free flaps are needed. However, not all patients, due to systemic conditions or local blood supply, are not candidates for this type of reconstruction and some times are submitted to amputations. Cases Report: The authors report two cases in which subtotals calcanectomies were used for the treatment of wounds in the calcaneus. In both cases, amputations were avoided.

RESUMO Introdução: Feridas em pés são muito frequentes e requerem abordagem multidisciplinar para a sua prevenção, tratamento e reabilitação. Quando acometem o calcâneo, oferecem dificuldade ainda maior e podem apresentar complicação com a ocorrência de osteomielite. Debridamento de tecido desvitalizado e antibioticoterapia são etapas obrigatórias para o tratamento. Na reconstrução, retalhos locais ou livres são necessários. Porém, nem todos os pacientes, devido a condições sistêmicas ou de vascularização local, são candidatos a esse tipo de reconstrução e acabam sendo submetidos a amputações. Relato de caso: Os autores relatam dois casos nos quais foram utilizadas calcanectomias subtotais para o tratamento de feridas em calcâneo. Em ambos os casos, foram evitadas as amputações.

O objetivo deste estudo foi identificar os tempos da indução e recuperação anestésica em cavalos marinhos (Hippocampus reidi) expostos ao óleo essencial de Lippia alba (OE), assim como a eficácia do OE como um agente redutor de estresse no transporte desta espécie. Os cavalos marinhos foram colocados em aquários contendo 1 litro de água e diferentes concentrações de OE (0, 10, 20, 50, 150, 300 e 450 µL L-1), após a indução, os peixes foram transferidos à aquários livre de anestésico para avaliar o tempo de recuperação. Em um outro experimento os cavalos marinhos foram transportados em sacos plásticos contendo 15 µL L-1 do OE por 4 ou 24h. A concentração crescente do OE diminuiu proporcionalmente o tempo exigido para a indução da anestesia. O óleo essencial (15 µL L-1) inibiu o aumento nos níveis de glicose sanguínea provocada pelo transporte por 4 ou 24 h. O transporte por 24 h igualmente diminuiu o número de linfócitos e aumentou o número de neutrófilos, estas alterações foram evitadas com o uso do OE na água. Estes resultados demonstram que no cavalo marinho o OE é eficaz para a sedação e transporte na faixa de 10-20 µL L-1 e para a anestesia profunda recomenda-se 150 µL L-1.

The aim of this study was to identify the times of anesthetic induction and recovery in slender seahorses (Hippocampus reidi) that were exposed to the essential oil of Lippia alba (EO), as well as the efficacy of EO as a stress-reducing agent in the transport of this species. Slender seahorses were placed in 1-L aquaria containing different concentrations of EO (0, 10, 20, 50, 150, 300 and 450 µL L-1), and after induction, fish were transferred to aquaria that were free of anesthetic to evaluate their recovery time. In an additional experiment, slender seahorses were transported in plastic bags with 15 µL L-1 of EO for 4 or 24 h. The increased concentration of EO proportionally decreased the time required for the induction of anesthesia. EO treatment (15 µL L-1) inhibited the increase in blood glucose levels that was provoked by transportation for 4 or 24 h. Transportation for 24 h also decreased the number of lymphocytes and increased the neutrophil count, and these effects were avoided with the addition of EO to the water. These results demonstrate that EO was effective as an anesthetic at concentrations of 10-20 µL L-1 for slight sedation and transport and at 150 µL L-1 for deep anesthesia in the slender seahorse.

Estudo transversal de base populacional investigou determinantes de sobrepeso em crianças de 12 a 59 meses em Porto Alegre (n = 2.660). Razões de chance (RC) de sobrepeso, definido por peso/altura > 2 escores-z do padrão NCHS, foram estimadas para condições sócio-econômicas e demográficas, ambiente social imediato e eventos de saúde da criança. A prevalência de sobrepeso foi 6,5%. No modelo multivariado, a chance de sobrepeso na criança associou-se positivamente à escolaridade materna (escolaridade > 12 anos, RC = 2,36; IC95%: 1,21-4,60; 9-11 anos, RC = 2,07; IC95%: 1,16-3,70) e à renda familiar per capita > 2 salários mínimos (RC = 1,86; IC95%: 1,13-3,08) e negativamente ao trabalho materno remunerado (RC = 0,72; IC95%: 0,52-0,99). A chance foi maior para os nascidos grandes para idade gestacional (RC = 2,29; IC95%: 1,36-3,85) e menor para os nascidos pequenos (RC = 0,57; IC95%: 0,33-0,99), comparados aos nascidos com peso adequado. Escolaridade paterna, qualificação profissional dos pais e idade materna ao nascimento da criança evidenciaram efeito somente no modelo bruto. São necessários programas para prevenir sobrepeso em crianças, com especial atenção às famílias e crianças em maior risco.

Risk factors for overweight were investigated in a cross-sectional survey of children aged 12-59 months in the Southern Brazilian city of Porto Alegre (n = 2,660). Odds ratios (OR) for overweight, defined by weight/height > 2 z-scores of the NCHS standards, were estimated for socioeconomic and demographic conditions, social environment, and childhood health events. Prevalence of overweight was 6.5%. In the multivariate model, the odds of overweight were positively associated with maternal education (schooling > 12 years, OR = 2.36; 95%CI: 1.21-4.60; 9-11 years, OR = 2.07; 95%CI: 1.16-3.70) and family income per capita > 2 times the minimum wage (OR = 1.86; 95%CI: 1.13-3.08) and negatively associated with maternal work (OR = 0.72; 95%CI: 0.52-0.99). Odds were higher for children born large-for-gestational-age (OR = 2.29; 95%CI: 1.36-3.85) and lower for children born small (OR = 0.57; 95%CI: 0.33-0.99), as compared to those born with adequate birth weight for gestational age. Paternal schooling, parental occupation, and maternal age at the child's birth were associated with overweight in the unadjusted model only. Programs are needed to prevent overweight during childhood, with special attention to families and children at increased risk.
