Abstract Objective: To determine the effect of a training program using simulation-based mastery learning on the performance of residents in pediatric intubations with videolaryngoscopy. Method: Retrospective cohort study carried out in a tertiary pediatric hospital between July 2016 and June 2018 evaluating a database that included the performance of residents before and after training, as well as the outcome of tracheal intubations. A total of 59 pediatric residents were evaluated in the pre-training with a skills’ checklist in the scenario with an intubation simulator; subsequently, they were trained individually using a simulator and deliberate practice in the department itself. After training, the residents were expected to have a minimum passing grade (90/100) in a simulated scenario. The success of the first attempted intubation, use of videolaryngoscopy, and complications in patients older than 1 year of age during the study period were also recorded in clinical practice. Results: Before training, the mean grade was 77.5/100 (SD 15.2), with only 23.7% (14/59) of residents reaching the minimum passing grade of 90/100. After training, 100% of the residents reached the grade, with an average of 94.9/100 (SD 3.2), p < 0.01, with only 5.1% (3/59) needing more practice time than that initially allocated. The success rate in the first attempt at intubation in the emergency department with videolaryngoscopy was 77.8% (21/27). The rate of adverse events associated with intubations was 26% (7/27), representing a serious event. Conclusions: Simulation-based mastery learning increased residents’ skills related to intubation and allowed safe tracheal intubations with video laryngoscopy.

ABSTRACT: In Brazil, snakes from the Bothrops genus are responsible for thousands of accidents, and their venoms are mainly composed of proteolytic enzymes. Although the antibothropic serum produced by the Brazilian Institutes is remarkably efficient, more studies are necessary, especially in veterinary medicine. The venom contain enzymes and non-enzymatic proteins that interfere with hemostasis leading to hemorrhage or even thrombosis. Possible treatment associations with known bothropic antivenom were the reason for the development of the present study. The aim of this study was to evaluate hemostasis alterations caused by Bothrops alternatus venom in rabbits followed by treatments with anti-bothropic serum, tranexamic acid and desmopressin. Twenty New Zealand rabbits were distributed into five groups (n=4) that were experimentally envenomed with 150mcg/kg of B. alternatus venom via intramuscular injection and treated as follow: Group 1 (G1) was the positive control and received venom and PBS/BSA; Group 2 (G2) was treated with tranexamic acid; Group 3 (G3) with desmopressin; Group 4 (G4) with tranexamic acid and anti-bothropic serum; and Group 5 (G5) with anti-bothropic serum and desmopressin. Blood samples were collected before venom administration, and one, four, eight and 12 hours after, for Partial activated partial thromboplastin time, Prothrombin Time, Thrombin Time and fibrinogen evaluation. Thrombin generation (TG) test was carried out with a pool of samples from final times (8 and 12h). At the end of 12h, all animals were euthanized and necropsy was conducted. Samples from muscle tissue, heart, lungs and kidney were analyzed. Classic coagulation tests showed no significant differences amongst groups and times. However, TG indicated that the venom causes a hypocoagulability state, which was not reversed by proposed treatments. Histology showed muscle inflammation, hemorrhage and necrosis, as well as hemorrhage in other tissues with no differences amongst groups. B. alternatus envenomation causes hypocoagulability detected by TG assay, but not through classical coagulation tests. The use of tranexamic acid and desmopressin for hemostasis stabilization after inoculation of the venom did not show advantage in coagulation restoration.

Abstract This study was carried out to investigate the microbial profile and endotoxin levels of endodontic-periodontal lesions of periodontal origin. Periodontal and endodontic samples were taken from periodontal pockets and necrotic root canals of 10 teeth with endodontic-periodontal lesions. Evidencing of 40 different bacterial species were determined in each endodontic and periodontal sample using the checkerboard DNA-DNA hybridization method and Kinetic chromogenic LAL assay was used for quantification of endotoxins. Fisher’s exact test correlated the bacterial species with the endodontic or periodontal microbiota. The endotoxin levels (EU/mL) found in samples of the root canal and periodontal pocket were compared by the Wilcoxon test (p<0.05). Bacteria and LPS units were found in 100% of the endodontic and periodontal samples. The species E. faecium, P. acnes, G. morbillorum, C. sputigena and L. buccalis were strongly correlated with the endodontic microbiota and P. nigrescens with the periodontal microbiota. P. intermedia, P. endodontalis and V. parvula were more prevalent in both endodontic and periodontal microbiots. The endotoxin levels in the periodontal pocket (89600 EU/mL) were significantly higher than in the root canal (2310 EU/mL). It was concluded that the microbiota present in the periodontal and endodontic tissues is similar, with a higher prevalence of species of the orange complex and a higher level of endotoxin in the periodontal pockets.

The air quality during the Summer 2016 Olympic Games in two Olympic zones, Maracanã (Tijuca) and Deodoro, was studied. Volatile organic compounds (VOCs) were collected and analyzed following method TO-15 (United States Environmental Protection Agency), during and in the days after the Games. The total VOC (C4-C12) concentrations were 39.2-160.9 and 36.6-191.4 µg m-3 for Tijuca and Deodoro, respectively. These concentrations were lower than values previously obtained in Tijuca and other areas of the city. VOC speciation and benzene/toluene ratios were also different as a consequence of the restrictions imposed on vehicular transit during the Olympic Games. Concentrations were discussed considering the reactivity and ozone forming potential of individual compounds. Criteria pollutants, determined by the Tijuca Automatic Monitoring Station, were also analyzed. The preventive policies were successful in decreasing the concentrations of CO and PM10, leading to a decrease in mean ozone levels. However, air quality indexes were not dramatically reduced, because of the increase in NOx emissions and isolated events of higher ozone levels.

ABSTRACT Purpose: To evaluate the impact of the patient position on the outcomes of PCNL among patients with complex renal stones. Material and Methods: From July 2011 to July 2014, we collected prospective data of consecutive patients who underwent PCNL. We included all patients with complex stones (Guy's Stone Score 3 or 4 (GSS) based on a CT scan) and divided them based on the position used during PCNL (prone or supine). The variables analyzed were gender, age, body mass index, ASA score, stone diameter, GSS, number of punctures, calyx puncture site, intercostal access and patient positioning. Complications were graded according to the modified-Clavien Classification. Success was considered if fragments ≤ 4mm were observed on the first postoperative day CT scan. Results: We analyzed 240 (46.4%) of 517 PCNL performed during the study period that were classified as GGS 3-4. Regarding patient positions, 21.2% were prone and 79.8% were supine. Both groups were comparable, although intercostal access was more common in prone cases (25.5% vs 10.5%; p=0.01). The success rates, complications, blood transfusions and surgical times were similar for both groups; however, there were significantly more visceral injuries (10.3% vs 2.6%; p=0.046) and sepsis (7.8% vs 2.1%; p=0.042) in prone cases. Conclusion: Supine or prone position were equally suitable for PCNL with complex stones and did not impact the success rates. However, supine position was associated with fewer sepsis cases and visceral injuries.

Abstract: In several countries, bovine tuberculosis, caused by Mycobacterium bovis, is an economic and public health problem. Worldwide disease eradication programs are implemented with policies based on tuberculin testing and slaughter of reactive animals. However, little is known about the costs and the effectiveness of the eradication policies. We evaluated the cost-effectiveness of bovine tuberculosis diagnostic protocols on a multidisciplinary approach, applied in a naturally infected herd. Regarding the cost-effectiveness analysis (C/Ef) of ante-mortem diagnostic protocols, the Cervical Comparative Test (C/Ep=4.68), when used alone, is the diagnostic protocol most cost-effective for a naturally infected herd. For post-mortem confirmatory diagnostic, the histopathology (C/Ep=17.47) associated with the Cervical Simple Test and Cervical Comparative Test was the most cost-effective choice for the animals studied in this herd. However, the only diagnostic protocol that was able to identify 100% of the infected animals was the ELISA associated with the IFN test.

The aim of this study was to investigate the clinical functioning of one of the dimensions of the Dimensional Clinical Personality Inventory (IDCP), Attention Seeking, and to establish a cutoff considering the diagnosis of histrionic personality disorder, whose pathological characteristics are directly related to this dimension. The IDCP was applied to 1954 participants (62.20% women), between 18 and 90 years (M = 29.30, SD = 11.38), divided into three groups: patients with histrionic personality disorders (n = 14), patients with other personality disorders (n = 168), and those without diagnoses (n = 1772). The parameters of items and persons were estimated by the rating scale model, and reliability measures were generated, as well as the item-person map and map of items, ANOVA for comparison between groups, and ROC curve. The results suggest suitability of the Attention Seek dimension in clinical functioning; however, problems that seem inherent to the histrionic personality disorder were reflected in the setting of the dimension.

<p>In the middle of 1980’s, the ceramic superconductors were novel materials that exhibit superconductivity at temperatures larger than that of liquid nitrogen (T = 77 K). This cryogenic liquid is easier to be obtained and cheaper than liquid helium (T = 4.2 K). Thus, the possibility of applications of such materials increased and they are suited to the efforts to search clean energy sources and the growth of a sustainability society. This is possible due to the properties of the superconductors as zero resistivity and perfect diamagnetism. In this work we will present the arrangement of a superconducting levitation experiment that can be used as a motivation for high school students and also to encourage them to choose a scientific career.</p>

Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Besides degree-days, which considers air temperature, there are several bioclimatic unit systems. However, their use and limitation is little known for coffee crop. Therefore, this study evaluated the performance of four bioclimatic indices to estimate the length of the sub-periods flowering-light green fruit, light green fruit-cherry fruit and flowering-cherry fruit, in coffee crop. The thermal index, accumulation of photosynthetic active radiation, Geslin's heliothermic index and the accumulation of the reference evapotranspiration were evaluated during the agricultural year 2008/2009 for ten coffee cultivars, in Uberlândia-MG, Brazil. The data for each bioclimatic index were analyzed with the classical statistics, determining the mean, the standard error, and the coefficient of variation. This last parameter was the basis for the choice of the bioclimatic index used for the quantification of the length of the sub-periods for each coffee cultivar and for the classification of the cycle as early, medium and late. Geslin's heliothermic index and accumulation of the reference evapotranspiration presented the smallest mean coefficients of variation among the cultivars.

In the present study the effects of phosphorus nutrition and arbuscular mycorrhiza (AM) were evaluated on Cd toxicity to Trema micrantha (L.) Blum.. In a first experiment, trema seedlings were raised in a P-enriched substrate (0, 100, 200 and 400 mg dm-3) and with an AM treatment (inoculation with Glomus etunicatum). After sixty days of growth, plants were transferred to Clark nutrient solution either without Cd added or amended with this metal at concentration of 5, 15 and 45 µmol L-1. After the transplant to solution with treatments, plants were allowed to grow for additional forty days, when treatment effects were evaluated. Phosphorus effects on amelioration of Cd toxicity were also evaluated in a second experiment in which P at concentration of 0.07; 0.5; 1 and 2 mmol L-1 and Cd at 0, 5, 10 and 15 µmol L-1 were combined and simultaneously applied in the growing nutrient solution. It was found that Cd exhibited high inhibition on growth and on AM colonization even at the lowest tested concentration (5 µmol L-1). Enhanced P nutrition favored plant growth by reducing Cd concentration in plant shoots and consequently Cd phytotoxicity, whereas AM inoculation had no significant effect. An inverse relationship between P supply and Cd plant uptake and accumulation was found in the second experiment. The chemical speciation analysis of the nutrient solution showed that P application had no significant interference on Cd available forms, therefore indicating that the P-ameliorating effect on Cd toxicity results from "in plant " interactions that cause reduction in translocation of Cd absorbed by the roots.

As alterações histopatológicas incluíam necrose e fibrose do miocárdio, congestão centrolobular passiva crônica do fígado, edema pulmonar e degeneração esponjosa da substância branca do encéfalo.
Two outbreaks of poisoning by Tetrapterys multiglandulosa in cattle and the experimental reproduction of the toxicosis in sheep are described. Both outbreaks occurred on the same farm in the municipality of Bataiporã, state of Mato Grosso do Sul, Brazil. The first outbreak occurred in July-October 2004 and involved a cattle population at risk of 290 pregnant cows, which were introduced into a 60 hectare pasture with a legal reservation area heavily infested by T. multiglandulosa. Of these, 230 cows (79.3%) aborted, had stillbirths or delivered weak calves that died few days after birth. Seven cows died, and one cow and a 10-day-old calf were necropsied. The second outbreak occurred in September-October 2005, 40 days after 285 2-year-old heifers were introduced into the same pasture infested by T. multiglandulosa and where the first outbreak had occurred in the previous year. Nine heifers got sick and died, and three of then were necropsied. Clinical signs of affected cattle, including a 10-day-old calf, were marked lethargy, loss of weight with distension of the abdomen (ascites), subcutaneous dependant edema, distended and pulsating jugular veins, dyspnea and cardiac arrhythmia. Necropsy findings included a round and dilated heart with whitish and firm areas in the myocardium, and changes related to cardiac failure such as cavitary edema, nutmeg liver, pulmonary edema, a large blood clot in the left ventricle. Histopathological changes included necrosis and fibrosis in the myocardium, chronic passive hepatic centrolobular congestion, pulmonary edema, and spongy degeneration in the white matter of the brain. Experimental sheep died 29 (Sheep 1) and 35 (Sheep 2) days after being fed average daily doses of T. multiglandulosa corresponding to 14g/kg (Sheep 1) and 7,5 g/kg (Sheep 2) per day. Clinical signs were observed from the 7th day (Sheep1) and the 4th day (Sheep 2) of the experiment and included tachycardia, arrhythmia, lethargy and head pressing. Necropsy and histopathologic findings in both experimental sheep were very similar to those observed in affected cattle of the two spontaneous outbreaks.
