Abstract INTRODUCTION: Dengue is an important mosquito-borne disease in tropical and subtropical regions. Adhesion molecules have not been systematically characterized in the renal tissue of patients with severe dengue (SD). The objective of this study was to detect viral antigens in samples from patients that evolved with SD, correlating with the expression of ICAM-1, VCAM-1, VE-cadherin, and E-selectin to contribute to a better understanding of the pathophysiology of SD. METHODS: Kidney specimens from patients with SD were selected according to clinical and laboratorial data and submitted to histological and immunohistochemistry analysis. A semiquantitative evaluation was performed considering positive immunostaining in 20 glomeruli. RESULTS: Viral antigens were mainly detected in distal tubules. The intense immunostaining of VCAM-1 and ICAM-1 was observed. The expression of E-selectin was discrete, and VE-cadherin expression varied from mild to moderate. VCAM-1 was slightly intense in the glomerular capsule; the expression of ICAM-1 was diffuse. E-selectin was diffuse, and VE-cadherin varied from mild to moderate. The most frequent histological findings were glomerular congestion, mild glomerulitis, acute renal injury, and glomerular atrophy. CONCLUSIONS: The results appear to demonstrate an imbalance between vascular endothelial permeability regulating events in renal lesions in SD. The increase in the expression of ICAM-1 and VCAM-1 is an in-situ indicator of higher permeability with a consequent influx of cells favoring the inflammation of the endothelium. These molecules are important in the pathophysiology of the disease and provide the possibility of developing new markers for the evaluation, clinical follow-up, and therapeutic response of patients with SD.

Lipoblastoma e lipoblastomatose são lesões tumorais benignas dos tecidos moles similares ao tecido adiposo fetal. Na literatura, apenas 16 casos de lipoblastoma na região do pescoço foram relatados e, destes, somente 3 foram descritos na região posterior do pescoço. Hibernomas são tumores benignos raros do tecido adiposo composto por células gordurosas marrons. Foram descritos cerca de dez casos desses tumores na região cervical. Este artigo relatou dois casos raros de tumores do tecido adiposo. No primeiro caso, tratou-se de criança do gênero masculino, com 12 meses de idade, com aparecimento de massa cervical na região posterior do pescoço. A ressecção completa da lesão foi realizada, e o estudo patológico revelou lipoblastomatose. O segundo caso ocorreu em um homem de 36 anos com massa cervical anterior, que se movia com a deglutição. Após ressecção à análise histopatológica mostrou hibernoma.

Lipoblastoma and lipoblastomatosis are rare benign soft-tissue tumoral lesions resembling fetal adipose tissue. A total of 16 cases of lipoblastoma of the neck were reported in the literature, and only 3 were described in the posterior side of the neck. Hibernoma is a rare benign adipose tumor composed of brown fat cells and only about ten cases occurring in the cervical area have been reported. We reported two rare cases of adipose tissue tumors. The first case was a male infant aged 12 months who had a cervical mass on the posterior side of the neck. He underwent a complete resection of the lesion and the pathologic study revealed lipoblastomatosis. The second case was a 36-year-old man with an anterior cervical mass, which moved with swallowing. A resection was made and the histological analysis showed hibernoma.

OBJETIVO: Avaliar a distribuição dos subtipos moleculares dos tumores de mama diagnosticados em mulheres brasileiras jovens e determinar a frequência de perda de heterozigose (LOH) no gene BRCA1 entre os diferentes subtipos moleculares de tumores. MÉTODOS: Setenta e dois casos de carcinoma invasivo de mama diagnosticados em mulheres entre 19 e 40 anos de idade foram classificados de acordo com o subtipo molecular utilizando um painel imunoistoquímico e para a análise de LOH foram utilizados três marcadores intragênicos para o gene BRCA1 (D17S1322, D17S855, D17S1323). RESULTADOS: Treze casos (18%) apresentaram perfil imunoistoquímico compatível com carcinoma do tipo basal (basal-like tumor). Quarenta casos (55%) foram classificados como tumores do tipo luminal A; 11% (oito casos) do tipo luminal B, 13% (nove casos) corresponderam a tumores com superexpressão de HER2 (HER2-overexpressing tumors) e dois casos corresponderam a carcinomas ER/HER2 negativos sem expressão de marcadores basais. LOH foi detectada em quatro dos 16 casos informativos para o marcador D17S1322 e em um dos quatro casos informativos para D17S855. Instabilidade de microssatélites (MSI) foi observada em dois casos, um do tipo basalóide e um do tipo luminal A. CONCLUSÃO: Carcinomas do tipo "basal-like" corresponderam ao segundo subtipo molecular mais frequente entre os tumores de mama diagnosticados neste grupo de mulheres e foram restritos às mulheres com idade inferior a 35 anos. Não houve diferença significativa na frequência de LOH no gene BRCA1 entre os subtipos moleculares (p= 0,62). A frequência de LOH e de instabilidade de microssatélite em BRCA1 foi baixa neste grupo de pacientes, porém podem indicar um pequeno grupo de cânceres de mama com características moleculares específicas distintas.

OBJECTIVE: To evaluate the distribution of molecular subtypes of breast tumors diagnosed in young Brazilian women and to analyze the frequency of loss of heterozygocity (LOH) in BRCA1 among different molecular subtypes of early-onset breast cancer. METHODS: Samples from 72 cases of invasive breast carcinoma diagnosed in women aged between 19 and 40 years were evaluated using an immunohistochemical panel of biomarkers. Three intragenic BRCA1 locus microsatellites, D17S1322, D17S1323, and D17S855, were PCR amplified from matched normal (lymphocyte) and tumor DNAs for (LOH) analysis. RESULTS: We found 13 cases (18%) that had an immunohistochemical profile consistent with being basal-like. Forty cases (55%) were luminal A type; 11% (8 cases) were luminal B type, 13% (9 cases) were HER2-overexpressing tumors and two cases were ER-/HER2- carcinomas lacking basal marker expression. Four of the 16 informative cases at D17S1322, one of the four informative cases at D17S855, and none of the five informative cases at D17S1323 displayed LOH (four basal-like and one Luminal A). Microsatellite instability (MSI) at D17S855 and D17S1322 was found in two cases (one a basal-like and one Luminal A). CONCLUSION: In our study, basal-like tumor was the second most frequent molecular type among young Brazilian women and was only observed in women diagnosed under the age of 35 years. There was no significant difference of LOH at BRCA1 locus rates between basal-like breast tumors and not-basal-like breast tumors (p=0.62). LOH in BRCA1 and MSI in these breast cancers were not frequent but may indicate a small group of breast cancers with a specific molecular makeup.

CONTEXTO: p53, c-erbB-2 e receptor epidermal do fator de crescimento (EGFR) são proteínas associadas ao câncer, geralmente expressas nos carcinomas espinocelulares (CEC) de cabeça e pescoço, porém seu valor prognóstico permanece controverso. OBJETIVO: Avaliar o impacto prognóstico da expressão de p53, c-erbB-2 and EGFR nos CECCP. TIPO DE ESTUDO: Prospectivo. LOCAL: Departamento de Cirurgia de Cabeça e Pescoço e Otorrinolaringologia do Hospital AC Camargo, São Paulo. MÉTODOS: A expressão de p53, c-erbB-2 and EGFR em CECCP e mucosas de 54 pacientes foram estudadas por imunoistoquímica e estes dados foram correlacionados com dados histoclínicos e sobrevida. RESULTADOS: Foram observadas: associação direta da expressão de p53 em CEC e mucosas (p = 0,001); perda da expressão de c-erbB-2 (-) da mucosa normal para o CEC (p = 0,04); menor freqüência da associação da expressão de c-erbB-2 positivo com EGFR negativo nos CEC (p = 0,02); e entre o EGFR positivo e o índice mitótico (p = 0,03). A sobrevida atuarial para 60 meses foi maior para os pacientes que apresentavam disseminação para gânglios no pescoço sem ruptura capsular (48.3% p= 0,02), até um gânglio positivo (52.3%, p = 0,004), e margens cirúrgicas livres (47.0%, p = 0,01), quando comparadas aos pacientes que apresentavam ruptura capsular nos gânglios do pescoço (20,2%), dois ou mais gânglios positivos (18,7%), e margens cirúrgicas comprometidas (0,0%), respectivamente. Pacientes apresentando CEC p53 positivo e EGFR negativo apresentaram uma sobrevida maior (75.0%, p = 0,03) quando comparados ao grupo de pacientes remanescentes (33,1%). A análise multivariada confirmou o impacto positivo de p53 positivo e EGFR negativo na sobrevida (p = 0,02). DISCUSSÃO: Este estudo encontrou associações da expressão de p53, c-erbB-2 e EGFR com dados histoclínicos e com o prognóstico. De especial interesse, os resultados sugerem que a perda da expressão mucosa de c-erbB-2 pode estar envolvida com a carcinogênese; que a expressão de EGFR está relacionada com o índice mitótico dos CEC e que a presença da expressão de p53 com perda da expressão de EGFR nos CEC pode ser um fator prognóstico. CONCLUSÕES: Estudos propectivos adicionais devem ser realizados para confirmar a influência de p53, c-erbB-2 e EGFR nos dados histoclínicos e na sobrevida.

CONTEXT: p53, c-erbB-2 and epidermal growth factor receptor (EGFR) are cancer-related proteins that are usually expressed in head and neck squamous cell carcinoma (SCC). Their prognostic value remains controversial. OBJECTIVE: To evaluate the prognostic impact of p53, c-erbB-2 and EGFR expression in head and neck SCC. TYPE OF STUDY: Prospective. SETTING: Head and Neck Surgery Department, Hospital AC Camargo, São Paulo. METHODS: Fifty-four patients were studied for p53, c-erbB-2 and EGFR expression in head and neck SCC and adjacent mucosa, via immunohistochemistry. These data were correlated with histoclinical data and survival. RESULTS: There was a direct association of p53 expression in SCC and mucosa (p = 0.001); loss of c-erbB-2 expression (-) from normal mucosa to SCC (p = 0.04); lower frequency of association of c-erbB-2 (+) with EGFR (-) in SCC (p = 0.02); and a direct association of EGFR (+) expression in SCC and mitotic index (p = 0.03). The 60-month actuarial survival rates for patients presenting lymph node metastasis were higher when there was no capsule rupture by SCC (48.3%; p = 0.02), no more than one positive lymph node (52.3%; p = 0.004) or clear surgical margins (47.0%; p = 0.01), in comparison with patients presenting capsule rupture (20.2%), two or more positive lymph nodes (18.7%) or compromised surgical margins (0.0%), respectively. Patients presenting SCC p53 (+) and EGFR (-) demonstrated greater survival (75.0%; p = 0.03) than for the remaining group (33.1%). Multivariate analysis confirmed the positive impact of p53 (+) and EGFR (-) on survival (p = 0.02). DISCUSSION: Associations were found for p53, c-erbB-2 and EGFR expression with histoclinical data and prognosis. Interestingly, these results suggest that loss of mucosal c-erbB-2 expression could be involved in SCC carcinogenesis; EGFR expression in SCC is related to tumor mitotic index; and presence of p53 with absence of EGFR expression in head and neck SCC may be a prognostic factor for survival. CONCLUSIONS: Further prospective studies should be conducted to confirm the influence of p53, c-erbB-2 and EGFR on histoclinical data and prognosis.

O controle de tratamento da leishmaniose mucosa (LM) pode ser realizado pelo exame clínico e o acompanhamento dos títulos sorológicos da reação de imunofluorescência indireta (RIFI). Estudamos a correlação entre a presença de antígeno no tecido através da reação de imuno-histoquímica, os títulos da reação de imunofluorescência indireta e os achados anatomopatológicos, em quinze pacientes com LM, antes e após as lesões estarem cicatrizadas pela avaliação otorrinolaringológica, e avaliamos qual destes parâmetros pode ter utilidade no seguimento. Após a terapêutica houve negativação do antígeno tecidual em quatro doentes (grupo A), sendo a redução ou negativação dos títulos da RIFI estatisticamente significante (p<0.05), o que não ocorreu nos doentes, em que houve permanência do antígeno posteriormente ao tratamento (grupo B), sugerindo que o acompanhamento sorológico deva ser realizado conjuntamente com a pesquisa do antígeno tecidual, associação que não conhecemos ter sido utilizada anteriormente em outro estudo. A negativação do antígeno tecidual e o comportamento dos títulos da RIFI nos doentes do grupo A, provavelmente indicam menor possibilidade de recidiva. Ao exame anatomo-patológico, o processo inflamatório persistiu após a terapêutica, mesmo no grupo A, sugerindo que a permanência da atividade inflamatória em lesões clinicamente cicatrizadas, possivelmente correlaciona-se com a presença do antígeno ou a algum fator desconhecido.

Treatment of mucosal leishmaniasis (ML) can be controlled by clinical examination and by serologic titers by the indirect immunofluorescence serologic reaction (IISR). We studied the correlation between the presence of antigen in tissue determined by immunohistochemistry, the IISR titers and the anatomopathologic findings in fifteen patients with ML before and after healing of the lesions as determined by otorhinolaryngologic evaluation, and evaluated these parameters to determine which of them could be useful during follow-up. Tissue antigens became negative in four patients (group A) after treatment, with a statistically significant reduction or negativity of IISR titers (p<0.05). This did not occur in patients in whom the antigen persisted after treatment (group B), suggesting that serologic follow-up should be performed together with the search for tissue antigen, a combination which, to our knowledge, has not been used in previous studies. The negativity of tissue antigens and the behavior of IIRS titers in group A patients probably indicate a lower possibility of recurrence. Upon anatomopathologic examination the inflammatory process was found to persist after treatment even in group A, suggesting that the permanence of inflammatory activity even in clinically healed lesions is possibly correlated with the presence of the antigen or of some unknown factor.
