Aim To evaluate the resistance of the union between a glass fiber post and radicular dentine after cleaning the root with 17% EDTA and filling with different endodontic cements. Methods Forty uniradicular bovine incisors were removed to obtain root lengths of 18 mm. Endodontic treatment was performed on all roots using different filling cements (zinc oxide and eugenol-based, OZE; cement based on epoxy resin, AH) and cleaning solutions (saline, SA or EDTA), which made it possible to obtain four groups: OZESA, OZEEDTA, AHSA and AHEDTA. Subsequently, 12 mm of filling material was removed from the roots, and they were prepared to receive fiber posts luted with resin cement. To execute the mechanical cycles (2x106 cycles, 90 N, 4 Hz), coronal reconstruction was performed with a silicon matrix. The roots were then sliced (2-mm thick) to perform the push-out test. The results were analyzed using analysis of variance (one factor and two factors) and Tukey’s test (α=0,05). Results Bond strength (Mpa) was significantly higher for OZEEDTA (9,18) and AHEDTA (8,70) than for OZESA (6,06) AHSA (8,7). OZEEDTA also presented the highest values in the cervical region (15,18) but was significantly lower in the apical region (2,99). However, AHEDTA had a homogeneous bond strength in all thirds. Conclusion Regardless of the endodontic cement used, EDTA was used as an irrigating solution, culminating in a higher bond strength between the glass fiber post and dentin.

ABSTRACT Objective: This study aims to perform a morphometric analysis and explore the characteristics of the surgical corridor of the anterior to psoas approach in the Brazilian population through magnetic resonance imaging (MRI). Methods: Two hundred spinal MRI scans of patients aged between 18 and 80 years were evaluated using axial cuts at L2-L5 levels and a sagittal cut, T2 weighted. The relationship between the left psoas muscle and the abdominal aorta or the left common iliac artery was analyzed. The anterior to psoas corridor was defined as the shortest distance between the posterolateral aspect of the aorta or inferior vena cava or the nearest iliac vessel and the anteromedial aspect of the ipsilateral psoas muscle. Results: 104 females and 96 males with a mean age of 49,68±2.04 (range 18–80) years. The mean anterior to psoas distance at the L2-L3 level was 14,17±0.75mm; at the L3-L4 level was 12,08±0.77m,m and at the L4-L5 level was 9,12±0.77mm. The surgical corridors at all levels were larger in the older population. Conclusion: In most Brazilian patients, the anterior to psoas approach can be a good alternative for lumbar intervertebral fusions. As a routine in preoperative examination and surgical planning, lumbar MRI is fundamental in preoperative evaluation for anterior to psoas approach surgery. Level of Evidence IV; Descriptive study.

Resumen: Objetivo: El objetivo de estudio es realizar un análisis morfométrico y explorar las características del corredor quirúrgico de la vía anterior al psoas en la población brasileña, a través de resonancias magnéticas (RM). Métodos: Se evaluaron 200 RM de la columna lumbar de pacientes entre 18 y 80 años, utilizando cortes axiales a niveles L2-L5 y un corte sagital, con ponderación T2. Se analizó la relación entre el músculo psoas izquierdo y la aorta abdominal o la arteria ilíaca común izquierda. El corredor anterior al psoas se definió como la distancia más corta entre la cara posterolateral de la aorta o la vena cava inferior o el vaso ilíaco más cercano y la cara anteromedial del músculo psoas ipsilateral. Resultados: La población estudiada estuvo constituida por 104 mujeres y 96 hombres con una edad media de 49,68±2,04 (18-80) años. La distancia media anterior al psoas en nivel L2-L3 fue de 14,17 ± 0,75 mm; en el nivel L3-L4 fue de 12,08 ± 0,77 mm y en el nivel L4-L5 fue de 9,12 ± 0,77 mm. Los corredores quirúrgicos en todos niveles se consideraron más grandes en la población anciana. Conclusión: El abordaje del psoas anterior puede ser una alternativa para la fusión intervertebral lumbar en la mayoría de los pacientes brasileños. Como rutina en examen preoperatorio, la resonancia magnética lumbar es importante en la evaluación preoperatoria de cirugías con abordaje anterior del psoas. Resumo: Objetivo: O objetivo deste estudo é realizar uma análise morfométrica e explorar as características do corredor cirúrgico da via anterior ao psoas na população brasileira, através de exames de ressonância magnética. Métodos: Duzentos exames de ressonância magnética da coluna lombar de pacientes entre 18 e 80 anos foram avaliados, utilizando cortes axiais nos níveis L2-L5 e um corte sagital na ponderação T2. A relação entre o músculo psoas esquerdo e a aorta abdominal ou a artéria ilíaca comum esquerda foi analisada. O corredor anterior ao psoas foi definido como a menor distância entre a face posterolateral da aorta ou veia cava inferior ou o vaso ilíaco mais próximo e a face anteromedial do músculo psoas ipsilateral. Resultados: A população estudada foi de 104 mulheres e 96 homens com idade média de 49,68±2,04 (18-80) anos. A média da distância anterior ao psoas no nível L2-L3 foi de 14,17 ± 0,75mm; no nível L3-L4 foi de 12,08 ± 0,77mm e no nível L4-L5 foi de 9,12 ± 0,77mm. Os corredores cirúrgicos em todos os níveis foram considerados maiores na população idosa. Conclusão: A abordagem anterior ao psoas pode ser uma boa alternativa para a fusão intervertebral lombar na maioria dos pacientes brasileiros. Como rotina no exame pré-operatório e no planejamento cirúrgico, a ressonância magnética lombar tem fundamental importância na avaliação pré-operatória de cirurgias de abordagem anterior ao psoas. Abstract The early development of Markiana nigripinnis is described by morphological characters, pigmentation, and morphometry. Larvae were obtained through semi-natural breeding, collected, fixed, and identified according to their development. Eighty individuals with standard lengths ranging from 3.1 to 24.3 mm were analyzed. Larvae are poorly developed at hatching, with a relatively large yolk sac and finfold. At the preflexion stage, the eyes are pigmented, the mouth and anus are functional, the yolk is completely absorbed, and the pectoral fin bud emerges. At flexion, the first rays of the caudal, anal, and dorsal fins become evident. The pelvic fin bud emerges only at the postflexion stage, in addition to the complete absorption of the finfold. Pigmentation is distributed throughout the body, with a greater concentration on the top of the head, around the mouth, and at the base of the caudal fin. The myomere total number ranged from 34 to 49 (16–23 preanal, and 18–27 postanal). Juveniles show morphological characteristics like adults. The fins ray number are pectoral: 11–13, pelvic: 5–7, dorsal: 8–11, caudal: 16–27, and anal 30–47. The morphometric relationships reveal variations in growth along the early ontogeny of the species.

Resumo O desenvolvimento inicial de Markiana nigripinnis foi descrito considerando os caracteres morfológicos, a pigmentação e a morfometria. Indivíduos foram obtidos por meio de reprodução seminatural, coletados, acondicionados, fixados e identificados conforme seu período e estágio de desenvolvimento. Foram analisados 80 indivíduos com comprimento padrão variando de 3,1 a 24,3 mm. As larvas são pouco desenvolvidas à eclosão, com saco vitelino relativamente grande e presença de membrana embrionária. Em pré-flexão, os olhos estão pigmentados, a boca e o ânus são funcionais, o vitelo é completamente absorvido e surge o botão da nadadeira peitoral. Em flexão, os primeiros raios das nadadeiras caudal, anal e dorsal tornam-se evidentes. O botão da nadadeira pélvica aparece somente em pós-flexão, além da completa absorção da nadadeira embrionária. A pigmentação se distribui pelo corpo todo, com maior concentração no topo da cabeça, ao redor da boca e na base da nadadeira caudal. O número total de miômeros variou de 34 a 49 (16–23 pré e 18–27 pós-anal). Os juvenis apresentaram características morfológicas semelhantes ao adulto. O número de raios das nadadeiras é peitoral: 11–13, pélvica: 5–7, dorsal: 8–11, caudal: 16–27 e anal 30–47. As relações morfométricas revelam variações no crescimento ao longo da ontogenia inicial da espécie.

Abstract Objective To describe a reference curve for cervical length (CL) in mid-trimester twin gestations using transvaginal ultrasound (TVU) and to investigate whether short CL increases spontaneous preterm birth (sPTB) in asymptomatic twin pregnancies. Methods This was a prospective cohort study performed at 17 outpatient antenatal facilities of Brazil with women at 18 0/7 to 22 6/7 weeks of gestation who participated in a randomized clinical trial screening phase (P5 trial) between July 2015 and March 2019. TVU was performed to provide CL measurement in all screened women. Almost all women with CL ≤ 30 mm received vaginal progesterone 200mg/day and they were also randomized to receive cervical pessary or not. We considered data from the CL distribution among asymptomatic twin pregnancies and analyzed CL and its association with PTB generating receiver operating characteristics (ROC) curves and Kaplan-Meier curves. Results A total of 253 pregnant women with twins were included in the distribution curve. The mean CL was 33.7 mm and median was 35.5mm. The 10th percentile was 17.8mm. We identified a PTB rate of 73.9% (187/253) with 33.6% of sPTB < 37 (85/253) and 15% (38/253) of sPTB < 34 weeks. The best cutoff point to predict sPTB < 37 was 24.15 mm. However, the ROC curve showed a poor performance (0.64). The Kaplan-Meier survival curves identified that only CL values ≤ 20mm were associated to sPTB < 34 weeks. Conclusion A cutoff point of CL ≤ 20 mm can be interesting point to identify short cervix in Brazilian twin pregnancies. However, in Brazilian asymptomatic twin pregnancies, CL does not show a good performance to predict PTB.

Resumo Objetivo Descrever uma curva de referência da medida do colo uterino no Segundo trimestre de gestações gemelares através de ultrassonografia transvaginal (TVU) e investigar a correlação entre a medida do colo uterino (CL) e o parto prematuro espontâneo (sPTB) em pacientes assintomáticas. Métodos Foi realizado uma coorte prospectiva multicêntrica em 17 centros de referência do Brasil com mulheres com gestação gemelar entre 18 0/7 a 22 6/7 semanas de gestação que participaram da primeira fase de um ensaio clínico randomizado (P5 trial) entre Julho/2015 a Março/2019. TVU foi realizada para obter a medida do colo uterino em todas as mulheres. A maioria das mulheres com CL ≤30 mm receberam progesterona por via vaginal 200mg/dia e estas foram randomizadas para receber ou não um pessário cervical. Este estudo considerou dados da medida do colo uterino entre mulheres assintomáticas, desenvolvendo uma curva de referência para gestantes gemelares e sua capacidade de predição do parto prematuro através de curva ROC (receiver operating characteristics) e curvas de sobrevida de Kaplan-Meyer. Resultados O total de 253 gestantes foram incluídos no estudo, A média do CL foi 33.7mm e a mediana 35.5mm. O Percentil 10 do CL foi 17.8mm. A taxa de parto prematuro foi de 73.9% (187/253) com 33.6% de sPTB < 37 (85/253) e 15% (38/253) de sPTB < 34 semanas. O melhor ponto de corte para predizer sPTB < 37 foi 24.15 mm, entretanto a curva ROC demonstrou baixa performance (0.64). A curva de Kaplan-Meier para sPTB identificou que apenas CL ≤ 20 mm estavam associados a sPTB < 34 semanas. Conclusão Colo uterino ≤20 mm pode ser um interessante ponto de corte para identificar colo curto entre gestações gemelares assintomáticas brasileiras. Entretanto, a medida do colo uterino não apresentou boa performance para predizer parto prematuro.

ABSTRACT Objective: To identify the lingual foramina in a Brazilian population of 210 individuals through Cone-Beam Computed Tomography images, in order to guide the installation of dental implants in the region of the mental symphysis. Methods: After identifying the lingual foramina on a parasagittal section, four measurements were taken: distance to the alveolar ridge, distance to the mandibular lower border, distance to the limit of the vestibular cortical, and alveolar ridge inclination angle. The measurements were compared in relation to gender by the Mann-Whitney test. The association between gender and the number of foramina was done by the chi-square test. Pearson’s correlation analyzed the linear relationship between age and number of foramina. Results: Lingual foramina are present in 99.6% of the research participants. In the upper-inferior direction, a mean distance to the alveolar ridge of 21.4 mm was noted, while the mean distance to the lower mandibular border was 13.1 mm. In the bucco-lingual direction, a mean distance from the lingual foramina to the vestibular cortical of 15.2 mm is noted. The inclination angle of the alveolar ridge had a mean of 25.4º. Regarding gender, the test was not significant only for the distance to the vestibular cortical. Conclusion: The lingual foramina had a descending way in 100% of cases, being positioned mainly in the middle thirds (66.5%) and lower thirds (32.7%) of the mandibular ridge height. The use of dental implants up to 13 mm in length and up to 4 mm in diameter in the region of the symphysis mentualis is recommended.

RESUMO Objetivos: Identificar as foraminas linguais em uma população brasileira de 210 indivíduos, por meio de imagens de tomografia computadorizada de feixe cônico, a fim de orientar a instalação de implantes dentários em região de sínfise mentual. Métodos: Após identificação das foraminas linguais em corte parasagital, foram realizadas quatro medidas: distância até à crista alveolar, distância até à borda inferior da mandíbula, distância até o limite da cortical vestibular e ângulo de inclinação do rebordo alveolar. As medidas foram comparadas em relação ao sexo pelo teste Mann-Whitney. A comparação entre os sexos e o número de foraminas foi feito pelo teste qui-quadrado. A correlação de Pearson analisou a relação linear entre idade e número de foraminas. Resultados: As foraminas linguais estão presentes em 99,6% dos indivíduos. No sentido súpero-inferior, nota-se uma distância média até à crista alveolar de 21,4 mm, enquanto a distância média até à borda inferior da mandíbula foi de 13,1 mm. No sentido vestíbulo-lingual, nota-se uma distância média da foramina lingual até a cortical vestibular de 15,2 mm. O ângulo de inclinação do rebordo alveolar apresentou uma média de 25,4º. Em relação ao sexo, o teste só não foi significativo para a distância até a cortical vestibular. Conclusão: As foraminas linguais apresentam um trajeto descendente em 100% dos casos, posicionando-se, principalmente, nos terços médios (66,5%) e terços inferiores (32,7%) da altura do rebordo mandibular. Recomenda-se o uso de implantes dentários de até 13 mm de comprimento e de até 4mm de diâmetro em região de sínfise mentual.

ABSTRACT Objective: To perform an analysis of the anatomy of the great vessels relevant to the access for anterior lumbar interbody fusion (ALIF), determining the level of their bifurcation, the distance between the iliac vessels at L5-S1, the morphological configuration of the left iliac vein and the presence of fatty tissue between the vessel and the disc. Methods: Two hundred magnetic resonance imaging (MRI) scans of the lumbar spine of patients (18-80 years old) were evaluated using axial, coronal, and sagittal cuts at levels L1-S1 in T2 weighting. The interiliac distance was defined as the measurement between the left iliac vein and the right iliac artery. The presence of fatty tissue was defined as the identification of space between the vessel and the disc. Vessel morphology was divided into oval and flat. Results: The population’s average age was 49.6 years, with 52% being female. The average interiliac distance at L5-S1 was 27.48mm. The bifurcation of the aorta artery was identified at the level of L4 in 56.3%, as well as the confluence of the iliac veins (37.2%). The left iliac vein was identified as oval in 69% of patients and flat in 31% of patients. Fat tissue was evidenced in 60.5% of the exams. Conclusion: As a routine preoperative examination and surgical planning, lumbar MRI is fundamental in investigating the anatomy regarding anterior approach surgeries, allowing an effective assessment of the relationships between the great vessels and the lumbar spine. RESUMEN Objetivo: Realizar un análisis de la anatomía de los grandes vasos relevantes para el acceso en artrodesis intersomática lumbar anterior (ALIF), determinando el nivel de su bifurcación, la distancia entre los vasos ilíacos en L5-S1, la configuración morfológica de la vena ilíaca izquierda y la presencia de tejido graso entre el vaso y el disco. Métodos: Se evaluaron 200 imágenes de resonancia magnética (RM) de la columna lumbar de pacientes (18-80 años) mediante cortes axiales, coronales y sagitales en los niveles L1-S1, en ponderación T2. La distancia interilíaca se definió como la medida entre la vena ilíaca izquierda y la arteria ilíaca derecha. La presencia de tejido graso se definió como la identificación de espacio entre el vaso y el disco. La morfología de los vasos se dividió en ovalados y planos. Resultados: La edad media de la población fue de 49,6 años, de los cuales 52% eran mujeres. La distancia interilíaca media en L5-S1 fue de 27,48 mm. La bifurcación de la arteria aorta se identificó a nivel de L4 en 56,3%, así como la confluencia de las venas ilíacas (37,2%). La vena ilíaca izquierda se identificó como ovalada en 69% y plana en 31%. Se evidenció tejido graso en 60,5% de los exámenes. Conclusión: Como rutina en examen preoperatorio, la RM lumbar es fundamental en la investigación de anatomía de cirugías de abordaje anterior, permitiendo una evaluación eficaz de las relaciones entre los grandes vasos y la columna lumbar. Métodos: duzentos exames de ressonância magnética (RM) da coluna lombar de pacientes (18-80 anos) foram avaliados, utilizando cortes axiais, coronais e sagitais nos níveis L1-S1, na ponderação T2. A distância interilíaca foi definida como a medida entre a veia ilíaca esquerda e artéria ilíaca direita. A presença de tecido gorduroso foi definida como identificação de espaço entre o vaso e o disco. A morfologia do vaso foi dividida em oval e plana. Resultados: A idade média da população foi de 49,6 anos, sendo 52% mulheres. A distância média interilíacas em L5-S1 foi 27,48 mm. A bifurcação da artéria aorta foi identificada ao nível de L4 em 56,3%. A confluência das veias ilíacas também foi mais frequente ao nível de L4, representando 37,2%. A veia ilíaca esquerda foi identificada com o formato oval em 69% e plana em 31% dos pacientes. Tecido gorduroso foi evidenciado em 60,5% dos exames. Conclusão: Como rotina no exame pré-operatório e no planejamento cirúrgico, a RM lombar tem fundamental importância na investigação da anatomia visando cirurgias de abordagem anterior, pois permite uma avaliação eficaz das relações entre os grandes vasos e a coluna lombar. Resumo Lesões Cervicais Não Cariosas (LCNC) são defeitos do tecido dentário, não relacionados à cárie, frequentemente observados na prática odontológica. O objetivo do estudo foi avaliar os efeitos da profundidade e espessura da dentina de LCNC na resposta à dor dentária por meio de testes de diagnóstico clínico. 86 dentes de 14 pacientes com LCNC foram avaliados por: profundidade da LCNC, testes clínicos (estímulo evaporativo, para detectar níveis de dor de hipersensibilidade dentinária, teste térmico frio para classificar a saúde pulpar, estímulos percussivos para avaliação do tecido perirradicular e tomografia computadorizada de feixe cônico (TCFC), para avaliação da espessura de dentina remanescente (EDR). Em termos de profundidade, a amostra foi dividida em dois grupos: G1- dentes com LCNCs ≤1,0mm e G2- dentes com LCNCs entre 1,1-2,0 mm. Os dados de dor dentária foram comparados pelo teste de Mann-Whitney e a EDR pelo teste t-Student e correlações pelo teste de Pearson (p <0,05). A profundidade da LCNC não influencia a resposta da dor dentária ao estímulo evaporativo (p = 0,129, teste térmico frio (p = 0,125), teste diagnóstico clínico de percussão vertical (p = 0,317) e horizontal (p = 0,119), porém o G1 apresentou maior EDR (p <0,001) e o teste de correlação mostrou que LCNC mais profunda apresenta menor EDR (p = 0,011 / r = -0,273). Conclusão: Dentes com LCNC de até 2mm de profundidade apresenta níveis semelhantes de dor para hipersensibilidade dentinária, pulpar e saúde perirradicular, independente da profundidade da LCNC. Entretanto, lesões com profundidade ≤1,0mmin apresentaram mais EDR nos achados tomográficos.

Abstract Non-carious Cervical Lesions (NCCL) are dental tissue defects, non-related to caries, frequently observed in the dental practice. The aim of this study was to evaluate the effects of NCCL on dentin depth and thickness and the response to dental pain by means of clinical diagnostic tests. 86 teeth from 14 patients with NCCL were assessed by: depth of NCCL, clinical tests (evaporative stimulus, to detect pain levels of dentin hypersensitivity, cold thermal test to classify pulp health, percussive stimuli to evaluate the periradicular tissues and cone beam computed tomography (tomography to evaluate remaining dentin thickness (RDT). In terms of depth, the sample was divided into two groups: G1- teeth with NCCLs ≤1.0mm and G2- teeth with NCCLs between 1.1-2.0 mm. Dental pain data were compared by Mann-Whitney test and RDT by Student’s t-test and correlations by the Pearson test (p<0.05). The depth of NCCL does not influence dental pain response to evaporative stimulus (p=0.129), cold thermal test (p = 0.125), vertical (p = 0.317) and horizontal (p = 0.119) percussion clinical diagnostic tests. However, G1 showed more RDT (p<0.001), and the correlation test showed that deeper NCCL presents smaller remaining dentin thickness (p=0.011/r=-0.273). In conclusion, tooth with NCCL up to 2mm-depth presents similar levels of pain for dentin hypersensitivity, pulp and periradicular tissue independent to NCCL depth, however, lesions with ≤1.0mm-depth showed greater RDT in tomographic findings.

Abstract Overview and Aims: In early vulvar cancer, lymph node status is the most important prognostic factor. Sentinel lymph node biopsy (SLNB) is the minimally invasive procedure that leads to the most significant reduction in the classical high morbidity associated with the systematic inguinofemoral lymphadenectomy. Besides quality control, the aim of this study is to document and monitor the overall experience around SLNB at a referral Centre. Study design: Retrospective study. Population: All cases of vulvar carcinoma where SLNB was performed as part of the treatment for vulvar cancer at the authors’ Department, from January, 2016 to December, 2019 meeting the criteria: unifocal tumour confined to the vulva, tumour size < 4 cm, stromal invasion > 1 mm and no evidence of inguinal metastasis. Methods: Detailed description of the technique used in lymphoscintigraphy, SLNB, primary tumour treatment and histopathology with analysis of patient files collecting data of demographics, pathology, accuracy, surgery, perioperative results, feasibility and survival. Results: The average age was 68 years and Charlson score ≥ 5 was 55.6%. Primary site was labia majora in 44.4% and mid-line was affected in 27.8%. Mean tumour diameter was 20.2 mm. SLN detection rate was 100%, bilateral in 27.8%. 1 out of 38 nodes removed was positive and average number of nodes per patient was 2.1. There were no intraoperative complications. A single case of groin recurrence in 15 months mean follow-up time and 2 cases of non-related deaths occurred. 27.8% and 5.6% patients had short and long-term postoperative complications, respectively, mostly infectious. The average global survival/disease-free survival was 45.1/44.7 months. Conclusions: SLNB is a reliable and safe minimally invasive technique that should be performed by experienced gynaecological oncologists in well-equipped and multidisciplinary Centres. A rigorous preoperative planning combined with multidisciplinary surgical coordination are cornerstones for the success of this procedure applied to early-stage vulvar cancer.

Resumo Visão geral e objetivos: No cancro vulvar inicial, o estadiamento ganglionar é o fator prognóstico mais importante. A biópsia de gânglio sentinela (BGS) é o procedimento minimamente invasivo que ocasiona a redução mais significativa da elevada morbilidade associada à clássica linfadenectomia inguinofemoral sistemática. Além do controlo de qualidade, o objetivo deste estudo é documentar e monitorizar a experiência geral acerca da BGS num Centro de referência. Desenho do estudo: Estudo retrospetivo. População: Todos os casos de carcinoma vulvar em que a BGS foi realizada como parte integrante do tratamento para cancro vulvar no Departamento dos autores, de Janeiro de 2016 a Dezembro de 2019, cumprindo os seguintes critérios: tumor unifocal confinado à vulva, tamanho do tumor < 4 cm, invasão estromal > 1 mm e sem evidência de metástases inguinais. Métodos: Descrição detalhada da técnica utilizada na linfocintigrafia, BGS, tratamento do tumor primário e histopatologia com análise de processos clínicos dos pacientes, recolhendo dados demográficos, patológicos, de acurácia, acerca da cirurgia, resultados perioperatórios, de exequibilidade e de sobrevida. Resultados: A média de idades foi de 68 anos e o Índice de Charlson ≥ 5 foi de 55.6%. O local primário foi o grande lábio em 44.4% e a linha mediana foi afetada em 27.8%. O diâmetro médio do tumor foi de 20.2 mm. A taxa de deteção do GS foi de 100%, bilateral em 27.8%. 1 dos 38 gânglios linfáticos removidos foi positivo e o número médio de gânglios removidos por paciente foi de 2.1. Não se registaram complicações intraoperatórias. Um caso único de recorrência inguinal em 15 meses de tempo médio de seguimento e dois casos de óbitos não relacionados com a doença ocorreram. 27.8% e 5.6% dos pacientes tiveram complicações pós-operatórias de curto e longo prazo, respetivamente, principalmente de causa infeciosa. A média de sobrevida global/sobrevida livre de doença foi de 45.1/44.7 meses. Conclusões: A BGS é uma técnica minimamente invasiva confiável e segura que deve ser realizada por ginecologistas oncológicos experientes, em Centros multidisciplinares bem equipados. Um planeamento pré-operatório rigoroso aliado a uma coordenação cirúrgica multidisciplinar são os pilares para o sucesso deste procedimento aplicado ao cancro da vulva em estadio inicial.

Resumo Introdução: O linfoma de tecido linfoide associado à mucosa (MALT) é relativamente incomum, representando 5% de todos os linfomas não-Hodgkin. O local de envolvimento extra-nodal mais comum é o trato gastrointestinal, com a maioria dos casos a afetar o estômago (até 75% de todos os linfomas de MALT gastrointestinais). O envolvimento primário do cólon ocorre apenas em 2.5% dos doentes, sendo mais comummente observado como lesão polipoide única em avaliação endoscópica. Apresentação de caso: Apresentamos o caso de uma mulher de 61 anos, cuja colonoscopia após pesquisa de sangue oculto nas fezes positiva evidenciou lesões pseudo-polipoides superficialmente erosionadas no cólon ascendente proximal e no ângulo hepático. A análise histológica (microscópica e imunohistoquímica) de ambas as lesões foi concordante e compatível com linfoma de MALT. A tomografia computorizada de estadiamento revelou lesões nodulares pulmonares concomitantes, a maior localizada no lobo superior esquerdo com cerca de 34 mm. Devido ao estado disseminado da doença, a doente foi proposta para terapêutica sistémica com bendamustina e rituximab. À data de submissão deste caso, a doente continuava assintomática e sob tratamento com quimioterapia. Conclusão: Com este caso clínico pretendemos salientar a diversidade de apresentações possíveis no linfoma de MALT, bem como as suas localizações mais atípicas para as quais todos os gastroenterologistas devem estar atentos e com baixo limiar de suspeição.

Abstract Introduction: Mucosa-associated lymphoid tissue (MALT) lymphoma is relatively uncommon and accounts for only 5% of all non-Hodgkin lymphomas. The most common site of extranodal involvement is the gastrointestinal (GI) tract, with most cases affecting the stomach (up to 75% of all GI MALT lymphomas). Colonic disease occurs in only 2.5% of cases, most commonly manifesting as a single polypoid lesion on endoscopic evaluation. Case Presentation: We present the case of a 61-year-old woman whose colonoscopy (after a positive fecal occult blood test as part of colorectal cancer screening) revealed superficially ulcerated pseudo-polypoid lesions in the ascending proximal colon and hepatic flexure; microscopical and immunohistochemical analysis of the tissue sample was compatible with MALT lymphoma. Staging computed tomography showed concomitant nodular pulmonary lesions, the largest being located in the superior left lobe and 34 mm in size. Due to the disseminated state of the disease, systemic treatment with bendamustine and rituximab was initiated. Up to the time of submitting this paper, the patient was still asymptomatic and under chemotherapy treatment. Conclusion: With this case report, we aim to demonstrate the diversity of presentation of MALT lymphoma as well as its less typical locations; gastroenterologists should have an awareness of these and a low suspicion threshold.

RESUMO: A propagação seminal de pequizeiro acarreta grande variabilidade na taxa de germinação e no genótipo das mudas produzidas, mostrando a necessidade do desenvolvimento de técnicas para a propagação vegetativa dessa espécie, como a alporquia. Os objetivos deste estudo foram avaliar a eficácia da alporquia, bem como o efeito do diâmetro do caule e da planta-mãe na sobrevivência, no calejamento e no enraizamento de alporques de pequizeiro. O processo de alporquia foi realizado em meados de dezembro de 2019, nos seguintes diâmetros de caule: inferior a 15 mm; 15-19 mm; 20-24 mm; 25-29 mm e superior a 29 mm. Os tratamentos foram testados em quatro repetições de 10 alporques. Após três meses, avaliaram-se as porcentagens de sobrevivência, de calejamento e de enraizamento, além do vigor de enraizamento. As porcentagens de sobrevivência e de calejamento nos alporques, independentemente da planta matriz, foram superiores a 85%, mostrando o grande potencial dessa técnica na reprodução de pequizeiro. A taxa de diferenciação de calos em raízes variou entre as matrizes. Considerando os parâmetros avaliados, diâmetros de caule de 20 a 24 mm são os mais adequados para a alporquia em pequizeiro. Esses resultados indicam o potencial da técnica de alporquia para a produção comercial de mudas de pequizeiro.

ABSTRACT: Seed propagation of pequizeiro (Caryocar brasiliense) has many disadvantages, including low germination rates and high genetic variability of the resulting plants. Therefore, vegetative propagation techniques, such as the air layering process, are potentially important for this species. This study evaluated the effects of stem diameter and parental plant on the air layering efficacy of pequizeiro. The air layering process was carried out in mid-December 2019, in the following stem diameters: less than 15 mm; 15-19 mm; 20-24 mm; 25-29 mm and above 29 mm. The treatments comprised four replicates of 10 air layers. After 3 months, the percentages of survival, callus formation and rooting were evaluated, in addition to the rooting vigor. The percentages of survival and callus formation observed in the air layering, regardless the parental plant, were higher than 85%, showing the high potential of this technique in the propagation of pequizeiro. The callus differentiation rate in roots varied between the parental plants. Considering the evaluated parameters, stem diameters ranging from 20 to 24 mm are the most suitable ones for the air layering process in pequizeiro. These results indicated the high potential of the air layering technique for the commercial propagation of pequizeiro.

ABSTRACT Tomatoes are the most important and grown vegetable crop in the world. The salicylic acid (SA) application could improve crop yields due the positive physiological effects of this plant growth regulator. Thus, this study aimed to evaluate the possible effects of SA application on leaf regarding the intensification of antioxidant enzymes activities, chlorophyll a fluorescence, gas exchange, and tomato production against environmental stress. This experiment was conducted by the use of Colossal tomato hybrid in a protected environment between July and December 2019. Therefore, a randomized block design with five SA doses was used, as follows: 0; 0.5; 1; 1.5, and 2 mM. Then, applications were performed weekly from 15 to 60 days after transplantation (DAT). At the 45th and the 60th DAT, the enzymes activities were analysed, such as superoxide dismutase (SOD), catalase activity (CAT) and peroxidase (POD), lipid peroxidation, proline content, chlorophyll a fluorescence, gas exchange, and plant height. At the end of the experiment, fruit weight, total and commercial production were also evaluated. Results indicated that foliar application of SA reduced the environmental stress in plants through the intensification of antioxidant system that reduced lipid peroxidation and qNP and increased the efficiency of photosystem II and ETR. Furthermore, gas exchange was also influenced by the action of SA in gs, favouring A and A/Ci. The SA dose between 0.5 and 0.8 mM positively enabled the total and commercial production of tomatoes. Therefore, foliar application of SA reduced oxidative damage, and increased photosynthetic efficiency and fruit production.

RESUMO RACIONAL: A doença de von Hippel Lindau é uma síndrome autossômica dominante que se caracteriza por maior incidência de tumores em diferentes órgãos. OBJETIVO: Descrever um caso de pancreatoduodenectomia em paciente do sexo masculino de 30 anos com von Hippel Lindau. MÉTODO: Apresentamos o caso e a revisão da literatura realizada para otimizar o manejo do paciente, que apresentava feocromocitoma, hemangioblastoma capilar na retina periférica e dois tumores neuroendócrinos no pâncreas. RESULTADOS: O maior tumor pancreático localizava-se no processo uncinado medindo 31 mm. A lesão menor estava localizada no corpo proximal do pâncreas e foi detectada apenas na tomografia computadorizada por emissão de pósitrons com DOTATOC-68Ga. A investigação genética revelou uma mutação no locus NM_000551.3 c.482G>A (p.Arg161Gln) do gene supressor humano de Von Hippel-Lindau. O tumor no processo era maior que 30mm e o paciente apresentava mutação no exon 3. Indicamos pancreatoduodenectomia envolvendo o corpo proximal do pâncreas para ressecar em bloco ambos os tumores. No pós-operatório o paciente apresentou coleção líquida peripancreática que foi tratada como fístula pancreática grau B, com resolução clínica da complicação. Ele recebeu alta hospitalar no vigésimo primeiro dia pós-operatório. CONCLUSÕES: o manejo de pacientes com doença de von Hippel Lindau e tumores neuroendócrinos pancreáticos é complexo e deve ser centrado em instituições terciárias com grande volume de cirurgia pancreática. Embora a literatura atual auxilie na tomada de decisão na maioria das situações, cada etapa do tratamento requer análise e discussão entre diferentes especialidades médicas, incluindo cirurgiões, clínicos, radiologistas e anestesiologistas.

ABSTRACT BACKGROUND: The von Hippel-Lindau disease is a highly penetrant autosomal dominant syndrome characterized by tumor predisposition in different organs. AIM: This study aimed to describe a case of a pancreatoduodenectomy for a 30-year-old male patient with von Hippel-Lindau disease. METHODS: We present a case study and the literature review aiming at the state-of-the-art management of a patient with pheochromocytoma, capillary hemangioblastoma in the peripheral retina, and two neuroendocrine tumors in the pancreas. RESULTS: A larger pancreatic lesion was located in the uncinate process, measuring 31 mm. The smaller lesion was located in the proximal pancreas and was detected only on the positron emission tomography-computed tomography scan with DOTATOC-68Ga. Genetic investigation revealed a mutation in the locus NM_000551.3 c.482G>A (p.Arg161Gln) of the Von Hippel-Lindau Human Suppressor gene. The uncinate process tumor was larger than 30 mm and the patient had a mutation on exon 3; therefore, we indicated a pancreatoduodenectomy involving the proximal pancreas to resect both tumors en bloc. During the postoperative period, the patient presented a peripancreatic fluid collection, which was treated as a grade B pancreatic fistula with clinical resolution of the complication. On postoperative day 21, he was discharged home. CONCLUSION: The management of patients with von Hippel-Lindau disease and pancreatic neuroendocrine tumors is complex and must be centered on tertiary institutions with a large volume of pancreatic surgery. Although the current literature assists in decision-making in most situations, each step of the treatment requires analysis and discussion between different medical specialties, including surgeons, clinicians, radiologists, and anesthesiologists.

RESUMO A cana-de açúcar é uma cultura de elevada demanda de água, sendo cultivo importante para o Nordeste do Brasil, região que apresenta limitação hídrica ao longo do ano para as plantas de cana. Assim, torna-se importante desenvolver estratégias para reduzir o volume de água de irrigação para a cultura. Objetivou-se no presente estudo determinar a data de plantio da cana-de-açúcar que coincida com a máxima disponibilidade de água proveniente da precipitação, durante seu ciclo de desenvolvimento. A precipitação efetiva foi estimada a partir do balanço de água do solo efetuado durante três ciclos de cultivo da cana-de açúcar, variedade RB 92579. A cultura foi conduzida sob irrigação por gotejamento subsuperficial, tendo sido plantada em cinco meses: Outubro, Novembro, Dezembro, Janeiro e Fevereiro, correspondentes à estação seca na região. O experimento foi conduzido na Usina Açúcar e Álcool Coruripe S.A., município de Coruripe, Estado de Alagoas, Brasil, durante os anos de 2012 a 2016. Para todas as datas de plantio e ciclos de crescimento avaliados, a precipitação média efetiva foi igual a 30% da precipitação total, na condição de cultivo sob irrigação, e de 54,5% quando não se considerou a irrigação como componente do balanço de água. Novembro foi o mês de plantio com a menor lâmina de irrigação aplicada no ciclo de cultivo da cana-de-açúcar, com economia potencial de água variando de 5 a 129 mm.

ABSTRACT Sugarcane is both an important crop for the Brazilian Northeast economy, which faces severe water scarcity, and a water-intensive crop. Thus, it is important to develop irrigation strategies to reduce irrigation water demand in the region. This study aims to determine the sugarcane planting date that results in the maximum rainwater availability to the crop in the growing cycle. The crop effective precipitation was estimated from a soil water balance performed during three planting cycles of sugarcane, cultivar ‘RB 92579’. The crop was planted under subsurface drip irrigation in five months: October, November, December, January, and February, corresponding to the dry season period of the region. The experiment was conducted at the Açúcar e Álcool Coruripe Mill, located in the Coruripe municipality, State of Alagoas, Brazil, during the years 2012 to 2016. For all planting dates and growing cycles studied, the average effective rainy precipitation was equal to 30% of the total rainfall under irrigated conditions and 54.5% without considering the irrigation component in the soil water balance. November was the planting date that resulted in the minimum irrigation depth for the sugarcane growing cycle, with the potential irrigation water saving ranging from 5 to 129 mm.

RESUMO O objetivo deste trabalho foi avaliar o efeito de diferentes níveis de sombreamento arbóreo da espécie Clitoria fairchildiana sobre a composição bromatológica do capim-tanganica (Megatyrsus maximus Jacq. cv. Tanganica), nas estações do verão I (2011/2012), primavera (2012) e verão II (2012/2013). Para tanto, foi instalado um experimento sob delineamento inteiramente casualizado, com quatro tratamentos (níveis de sombreamento) e cinco repetições em Seropédica, Rio de Janeiro, Brasil. Foram avaliados os teores de: matéria seca (MS), proteína bruta (PB), fibra em detergente neutro (FDN) e em detergente ácido (FDA), proteína insolúvel em detergente neutro (PIDN) e em detergente ácido (PIDA), lignina (LIG) e matéria mineral (MM). Os dados foram analisados pelo PROC MIXED do SAS®, com medidas repetidas no tempo, e as médias dos tratamentos comparadas pelo teste de Tukey (P<0,05); ou pelo PROC REG do SAS® para as análises de regressão (P<0,05). Houve efeito de interação entre nível de sombreamento e estação do ano para teores de MS, PB, FDN, FDA, PIDN, PIDA e MM. Os teores de lignina foram influenciados apenas pelo nível de sombreamento. Os teores de MS, FDN, FDA e lignina foram reduzidos, enquanto aqueles de PB, PIDN e MM aumentaram linearmente em função dos níveis de sombreamento. Maiores teores de PIDA foram verificados somente na primavera. O aumento do sombreamento arbóreo de Clitoria fairchildiana melhora a composição bromatológica da forragem do capim-tanganica, durante o verão.

ABSTRACT This study aimed to evaluate the effect of different Clitoria fairchildiana tree shading levels on the Tanganyika grass (Megatyrsus maximus Jacq. cv. Tanganyika) chemical composition, in summer I (2011/2012), spring (2012) and summer II (2012/2013) seasons. For this purpose, an experiment was conducted in a completely randomized design with four treatments (shading levels) and five repetitions at Seropédica, state of Rio de Janeiro, Brazil. The following variables were evaluated: dry matter (DM), crude protein (CP), neutral detergent (NDF) and acid detergent (ADF) fiber, neutral (NDIP) and acid (ADIP) detergent insoluble protein, lignin (LIG) and mineral matter (MM) contents. Data were analyzed by PROC MIXED SAS®, with repeated measures in time, and treatment means compared by Tukey’s test (P<0.05) or by PROC REG (P<0.05 for regression analysis. There was a significant interaction between season and shading level for DM, CP, NDF, ADF, NDIP, ADIP and MM contents (P<0.05). Lignin content was only influenced by shading level (P<0.05). The DM, NDF, ADF and lignin contents were reduced by shading increase while CP, NIDP and MM contents were increased. Higher ADIP contents were found only in the spring. The increase in Clitoria fairchildiana tree shading improves the chemical composition of Tanganyika grass forage during summer season.

Resumo Objetivo: Demonstrar as principais complicações hemorrágicas após nefrolitotripsia percutânea, bem como os resultados após o tratamento por embolização arterial transcateter (EAT) em um centro de radiologia intervencionista. Materiais e Métodos: Coleta e análise de dados retrospectivos de pacientes submetidos a EAT por complicações hemorrágicas após nefrolitotripsia percutânea. Resultados: O sintoma mais comum foi hematúria macroscópica, presente em cinco pacientes no momento da readmissão ao pronto-socorro, e nestes pacientes identificamos três pseudoaneurismas isolados, dois casos de combinação de pseudoaneurisma e sangramento ativo (hematoma perirrenal) e um caso de associação de pseudoaneurisma e fístula arteriocalicinal. Fístula arteriovenosa foi diagnosticada em três pacientes, não sendo observada em associação com outras lesões vasculares. Não identificamos fístula arteriocalicinal isolada, somente associada a pseudoaneurisma. Cinco pacientes foram submetidos a embolização por micromolas 6 × 15 mm e 6 × 20 mm. Quatro pacientes foram submetidos a embolização por Histoacryl e Lipiodol. Não observamos complicações pela angiotomografia computadorizada de controle. Conclusão: A angiotomografia computadorizada apresenta alta acurácia diagnóstica e guarnece o radiologista intervencionista de dados para um planejamento terapêutico individualizado. EAT é um procedimento seguro e eficaz e pode ser utilizado como primeira linha para o tratamento de complicações hemorrágicas resultantes de procedimentos percutâneos renais.

Abstract Objective: To identify the main hemorrhagic complications after percutaneous nephrolithotomy, as well as the results obtained with transcatheter arterial embolization (TAE) at an interventional radiology center. Materials and Methods: This was a retrospective analysis of patients undergoing TAE for the treatment of hemorrhagic complications after percutaneous nephrolithotomy. All patients underwent computed tomography angiography (CTA). Results: We evaluated a total of nine patients. At emergency department readmission, the most common symptom was macroscopic hematuria, which was seen in five patients. Three patients had an isolated pseudoaneurysm, two had a pseudoaneurysm together with active bleeding (perirenal hematoma), and one had a pseudoaneurysm together with arteriocalyceal fistula. Arteriovenous fistula was diagnosed in three patients and was not seen in combination with other vascular lesions. We did not identify arteriocalyceal fistula in isolation. Five patients underwent TAE with 6 × 15 mm and 6 × 20 mm microcoils. Four patients underwent TAE with n-butyl-2-cyanoacrylate and ethiodized oil. Follow-up CTAs revealed no complications. Conclusion: Because of its high diagnostic accuracy, CTA provides the interventional radiologist with valuable data for individualized therapeutic planning. The TAE procedure is safe and effective. It can therefore be used as a first-line treatment for hemorrhagic complications resulting from percutaneous renal procedures.
