ABSTRACT Introduction: The mandibular buccal bifurcation cyst is a rare inflammatory odontogenic cyst that typically develops at the buccal region of the first or second permanent mandibular molars of children aged 6 to 15 years. The tooth involved is vital and shows an increased probing depth on the buccal surface. Radiographically, it is characterized by a unilocular radiolucent area, well circumscribed on the buccal osseous cortical, that it is difficult to visualize on periapical radiography. The diagnosis is primarily clinical and radiographic, and the treatment is usually the cystic enucleation without extraction of the tooth. Objective: To report a case of spontaneous involution of a unilateral mandibular buccal bifurcation cyst. Case presentation: This report describes the clinical and radiographic characteristics of an unusual case of unilateral buccal bifurcation cyst in a 6-year-old patient, presenting as a slight expansion in the buccal region of the first permanent mandibular molar. The treatment of choice was the clinical and radiographic follow-up due to the spontaneous involution of the cyst. There were no recurrences during follow-up and the tooth remained vital and erupted normally. Conclusions: The possibility of spontaneous involution is a characteristic that must always be observed, in order to avoid unnecessary surgical procedures.

RESUMEN Introducción: El quiste de bifurcación bucal mandibular es un quiste odontogénico inflamatorio poco frecuente que, en general, se desarrolla en la región bucal de primeros y segundos molares mandibulares permanentes en niños de 6-15 años. El diente involucrado es vital y muestra una mayor profundidad de sondeo en la superficie bucal. Radiográficamente, se caracteriza por un área radiolúcida unilocular, bien circunscrita en la cortical ósea bucal, que es difícil de visualizar en la radiografía periapical. El diagnóstico es principalmente clínico y radiográfico y el tratamiento suele ser la enucleación quística sin extracción del diente. Objetivo: Informar un caso de involución espontánea de un quiste de bifurcación vestibular mandibular unilateral. Presentación del caso: Caso inusual de quiste de bifurcación bucal unilateral en un paciente de 6 años, que se presenta como una ligera expansión de la región vestibular del primer molar mandibular permanente. El tratamiento de elección fue el seguimiento clínico y radiográfico debido a la involución espontánea del quiste. No hubo recurrencias durante el seguimiento y el diente permaneció vital y brotó normalmente. Conclusiones: La posibilidad de involución espontánea es una característica que siempre se debe observar para evitar procedimientos quirúrgicos innecesarios.

Resumo Objetivo O Brasil é um país grande, heterogêneo e diverso, marcado por desigualdades sociais, econômicas e regionais. A natimortalidade é uma preocupação global, principalmente em países de renda baixa e média. Este estudo investigou a prevalência e os possíveis determinantes da natimortalidade em diferentes regiões do Brasil. Métodos Estudo transversal que incluiu todas as mulheres em idade reprodutiva que estiveram grávidas nos últimos cinco anos registradas na Pesquisa Nacional sobre Demografia e Saúde (PNDS-2006/07). A regressão logística foi usada para avaliar a relação entre região e outras características maternas e risco de natimortalidade. Resultados A prevalência de natimortos no Brasil foi de 14,82 a cada 1.000 nascimentos, com grande variação de acordo com a região do país e uma prevalência mais alta entre as mais precárias. As regiões Norte e Nordeste tiveram as taxas de natimortalidade mais altas em comparação com a região Centro-Oeste, que perdurou após o ajuste das diversas variáveis de confusão - inclusive nível de pobreza e etnia. A baixa idade e a obesidade maternas também estavam relacionadas a taxas de natimortalidade mais elevadas. Conclusão No Brasil, a região influencia o risco de natimortalidade, com riscos muito mais altos no Norte e no Nordeste. A variação no nível de pobreza não explica esse achado. Futuras pesquisas sobre o assunto devem explorar outras possíveis explicações, como cuidado pré-natal e tipo de parto, bem como o papel dos sistemas de saúde público e privado com relação à natimortalidade. As estratégias de prevenção devem ser direcionadas a essas regiões historicamente desfavorecidas, como garantir acesso e qualidade da assistência durante a gravidez e perto do momento do nascimento.

Abstract Objective Brazil is a large, heterogeneous, and diverse country, marked by social, economic, and regional inequalities. Stillbirth is a global concern, especially in low- and middle-income countries. This study investigated the prevalence and possible determinants of stillbirth in different regions of Brazil. Methods This is a cross-sectional study including all women of reproductive age who had had a pregnancy in the last five years, enrolled in the most recent Brazilian Demographic and Health Survey (DHS/PNDS-2006/07). Logistic regression was used to assess the association between region and other maternal characteristics and stillbirth risk. Results The prevalence of stillbirth in Brazil was 14.82 per 1000 births, with great variation by region of the country, and a higher prevalence among the most deprived. The North and Northeast regions had the highest odds of stillbirth compared to the Center-West, which persisted after adjustment for multiple confounders - including deprivation level and ethnicity. Low maternal age and maternal obesity were also related to higher odds of stillbirth. Conclusion In Brazil, the region influences stillbirth risk, with much higher risk in the North and Northeast. Variation in socioeconomic level does not explain this finding. Further research on the subject should explore other possible explanations, such as antenatal care and type of delivery, as well as the role of the private and public health systems in determining stillbirth. Preventive strategies should be directed to these historically disadvantaged regions, such as guaranteeing access and quality of care during pregnancy and around the time of birth.

OBJETIVO: Avaliar aspectos funcionais e psicológicos dos pacientes imediatamente após alta da unidade de terapia intensiva. MÉTODOS: Coorte prospectiva. Na primeira semana após alta da unidade de terapia intensiva, por meio de uma entrevista estruturada, foram aplicados questionários e escalas referentes à avaliação do grau de dependência e da capacidade funcional (escalas de Barthel modificada e Karnofsky), e aos problemas psíquicos (questionário hospitalar de ansiedade e depressão), além da escala de sonolência de Epworth, em todos os sobreviventes com mais de 72 horas de internação na unidade de terapia intensiva, admitidos de agosto a novembro de 2012. RESULTADOS: Nos 79 pacientes incluídos no estudo, houve aumento do grau de dependência após a alta da unidade de terapia intensiva, quando comparados aos dados pré-hospitalização, por meio da escala de Barthel modificada (57±30 versus 47±36; p<0,001). Nos 64 pacientes independentes ou parcialmente dependentes previamente à internação (Karnofsky >40), o prejuízo foi uniforme em todas as categorias da escala de Barthel modificada (p<0,001). Já nos 15 pacientes previamente muito dependentes (Karnofsky <40), o prejuízo ocorreu somente nas categorias de higiene pessoal (p=0,01) e na capacidade de subir escadas (p=0,04). Na avaliação dos distúrbios psicológicos, os transtornos do humor (ansiedade e/ou depressão) ocorreram em 31% dos pacientes e os distúrbios do sono em 43,3%. CONCLUSÃO: Em pacientes internados na unidade de terapia intensiva por 72 horas ou mais, observaram-se redução da capacidade funcional e aumento do grau de dependência na primeira semana após alta da unidade de terapia intensiva, bem como elevada incidência de sintomas depressivos, de ansiedade e distúrbios do sono.

OBJECTIVE: To assess the functional and psychological features of patients immediately after discharge from the intensive care unit. METHODS: Prospective cohort study. Questionnaires and scales assessing the degree of dependence and functional capacity (modified Barthel and Karnofsky scales) and psychological problems (Hospital Anxiety and Depression Scale), in addition to the Epworth Sleepiness Scale, were administered during interviews conducted over the first week after intensive care unit discharge, to all survivors who had been admitted to this service from August to November 2012 and had remained longer than 72 hours. RESULTS: The degree of dependence as measured by the modified Barthel scale increased after intensive care unit discharge compared with the data before admission (57±30 versus 47±36; p<0.001) in all 79 participants. This impairment was homogeneous among all the categories in the modified Barthel scale (p<0.001) in the 64 participants who were independent or partially dependent (Karnofsky score ≥40) before admission. The impairment affected the categories of personal hygiene (p=0.01) and stair climbing (p=0.04) only in the 15 participants who were highly dependent (Karnofsky score <40) before admission. Assessment of the psychological changes identified mood disorders (anxiety and/or depression) in 31% of the sample, whereas sleep disorders occurred in 43.3%. CONCLUSIONS: Patients who remained in an intensive care unit for 72 hours or longer exhibited a reduced functional capacity and an increased degree of dependence during the first week after intensive care unit discharge. In addition, the incidence of depressive symptoms, anxiety, and sleep disorders was high among that population.
