Pediatric inflammatory multisystemic syndrome in Brazil: sociodemographic characteristics and risk factors to death

Abstract Objective: To determine the incidence of MIS-C in Brazil, describe the clinical and sociodemographic characteristics of the pediatric population affected by MIS-C and compare mortality and lethality outcomes with isolated Covid-19 and MIS-C cases. Methods: Observational and retrospective cohort study of cases of MIS-C associated with Covid-19 in the Brazilian population between 04/01/2020 and 04/17/2021. Data from the Ministry of Health's epidemiological bulletin up to the 15th epidemiological week of 2021, were used. The analyzes were descriptive through absolute and relative frequencies. The significance level is 5% in Stata 16.0 package. Results: Between 04/01/2020 and 04/07/2021, 903 cases of MIS-C associated with Covid-19 were notified in Brazil, of which, the largest part (55.26%) were male, between 0 and 4 years old (45.29%), from the Southeast region (38.76%). The deaths (61; 6.7%) were higher in the female gender, between 0 and 4 years old (47.54%) and in the Southeast region (34.43%). It was identified that the risk of death by MIS-C related to Covid-19 is 5.29 (CI = 2.83; 9.87 and P-value = <0.001) times higher in adolescents from 15-19 years old than in other age groups when compared to 0-4 years old children. Also, the residency in North region was as risk factor to death (RR = 3.72, IC = 1.29; 10.74 e P-value = 0.008). Conclusion: In this study, despite the numbers showing more deaths from zero to 4 years old, the risk for teenagers is notably higher. In addition, Brazil's Northern region is a risk factor that reaffirms social inequality and poor access to health.

Abstract The pathogenesis of systemic lupus erythematosus (SLE) is complex. Few studies in Brazilian population have addressed cell phenotypes associated with immunological responses and their associations with SLE activity. The aim of this study is to investigate cell phenotypes associated to SLE diagnosis, treatment and activity. Twenty-eight SLE female patients (17 inactive, 11 active) and 10 healthy women were included in this study. Markers of natural killer (Nk), T and B cells in peripheral blood were evaluated by flow cytometry. Nkt cells were decreased only in SLE active patients. Activated CD4+, regulatory T FoxP3+ and B cells were decreased in both active and inactive SLE patients, compared to control group. The data corroborate the disruption of immune regulatory response in SLE patients and suggest phenotipic changes as possible biomarkers of SLE activity.

A systematic review and meta-analysis of the clinical performance of implant-supported overdentures retained by CAD-CAM bars

Abstract Currently, there is no consensus on the indications and clinical performance of implant-supported overdentures (IODs) involving computer-aided design and manufacturing (CAD-CAM) bars. Objective: To evaluate the performance of IODs involving CAD-CAM bars. Methodology: A comprehensive search of studies published until May 2023 was conducted in many databases, including PubMed/MEDLINE, Web of Science, Cochrane Library, and SciELO, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). The population, intervention, comparison, outcome (PICO) question was: “How do IODs retained by bars fabricated by CAD-CAM technology perform in daily clinical practice?” The meta-analysis included clinical studies based on effect size and a two-tailed null test with a 95% confidence interval (CI). Results: Ten studies were included in the meta-analysis. Among them, nine reported a 100% implant survival rate for all CAD-CAM milled bars. Complications were reported in two studies with CAD/CAM-milled titanium bars, and one study reported more fractures in soldered gold bars used in maxillary rehabilitation. However, no fractures were observed in IODs retained by PEEK and zirconia bars. According to six studies, biological complications, including peri-implantitis, were minimal in the BioHPP and PEEK bar groups, while no cases were reported in the titanium or zirconia bar groups. CAD-CAM-milled zirconia bars had higher plaque and bleeding indices compared with titanium bars, as evidenced by findings from five studies. All four studies that evaluated Oral Health Impact Profile (OHIP) scores showed a positive effect of IODs retained by CAD-CAM milled titanium bars on quality of life. Patient satisfaction and acceptance by prosthodontists were significantly high, according to the results of five studies. Conclusion: Overdentures retained with CAD-CAM milled titanium bars show great potential for use in daily clinical practice. Moreover, patient and practitioner satisfaction was very high when this method was used.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Abstract: Background: Amaranthus hybridus L. (Amaranthaceae) is an annual, dicotyledonous species that is considered one of the main weed infesting agricultural production systems. Some weeds species are considered host plants to insect pests, serving as “green bridges” for subsequent attacks on the main crop during the season, which can cause significant losses. Recording the occurrence of insects that can reach pest status is important for the deployment of mitigation measures against possible damage and economic losses to crops. The presence of Spodoptera eridania (Cramer) (Lepidoptera: Noctuidae) has not been previously registered in association with A. hybridus plants. Objective: Thus, the present study is the first report to describe the occurrence of S. eridania on A. hybridus in the municipality of Piracicaba, São Paulo, Brazil. Methods: Images of the larvae while feeding on A. hybrids were obtained using an Olympus® E-410 digital camera. The pictures were then sent to the Laboratory of Ecotoxicology and IPM (LEMIP) of the Department of Entomology of Lavras Federal University (UFLA) for species identification. Results: We observed S. eridania larvae larger than 7 cm feeding on the inflorescences and leaves of A. hybridus . Conclusions: This note reports the first occurrence of S. eridania larvae feeding on A. hybridus in the city of Piracicaba, SP, Brazil.

Temperature gradients in domestic cats over seven-years-old: descriptive analysis

RESUMO: Os gradientes de temperatura são técnicas de monitoração dinâmicas e não-invasivas que fornecem informações sobre o fluxo sanguíneo periférico, e têm sido relacionados ao prognóstico de paciente com choque circulatório. O presente estudo avaliou os gradientes de temperatura em 47 felinos domésticos idosos aferindo as temperaturas central (retal) e periférica (palmar, plantar e região medial do rádio). Os gradientes encontrados foram 7,5°C para o centro-periférico; 5,6°C para o periférico-ambiental; 2,7°C para o skin-diff; e 0°C para o member-diff. As variáveis idade e sexo não pareceram influenciar as mensurações. Não há, em conhecimento dos autores, descrição prévia dos gradientes de temperatura em felinos domésticos idosos, e por isso esse estudo pretende contribuir com o entendimento sobre a capacidade de resposta de vasoconstrição nesse grupo de animais.
ABSTRACT: The temperature gradients are dynamic and noninvasive monitoring techniques that provide information on peripheral blood flow and have been related to the prognosis of patients with circulatory shock. This study evaluated 47 elderly domestic cats’ temperature gradients, and we measured central (rectal) and peripheral (palmar, plantar and medial region of the radio) temperatures. Values found in this study are compatible with studies in young felines and differ from dogs and humans. The mean gradients found were 7.5°C for the central-peripheral; 5.6°C for the peripheral-environmental; 2.7°C for the skin-diff; and 0°C for the member-diff and the variables age and gender do not seem to influence these measurements. To the authors’ knowledge, there is no description of temperature gradients in elderly domestic cats, so this study pretends to clarify the vasoconstriction response in this group of animals.

Abstract In plant breeding, the dialelic models univariate have aided the selection of parents for hybridization. Multivariate analyses allow combining and associating the multiple pieces of information of the genetic relationships between traits. Therefore, multivariate analyses might refine the discrimination and selection of the parents with greater potential to meet the goals of a plant breeding program. Here, we propose a method of multivariate analysis used for stablishing mega-traits (MTs) in diallel trials. The proposed model is applied in the evaluation of a multi-environment complete diallel trial with 90 F1’s of simple maize hybrids. From a set of 14 traits, we demonstrated how establishing and interpreting MTs with agronomic implication. The diallel analyzes based on mega-traits present an important evolution in statistical procedures since the selection is based on several traits. We believe that the proposed method fills an important gap of plant breeding. In our example, three MTs were established. The first, formed by plant stature-related traits, the second by tassel size-related traits, and the third by grain yield-related traits. Individual and joint diallel analysis using the established MTs allowed identifying the best hybrid combinations for achieving F1’s with lower plant stature, tassel size, and higher grain yield.

Divergência genética entre genótipos de milho (Zea mays L.) em ambientes distintos

O objetivo deste trabalho foi caracterizar morfologicamente a dissimilaridade genética entre 25 genótipos de milho (Zea mays L.), visando a seleção dos mais divergentes e de maior potencial produtivo, por meio da análise multivariada, em cinco ambientes de cultivo do Sul do Brasil. As linhagens endogâmicas dos dois grupos heteróticos foram provenientes do banco de germoplasma da empresa KSP Sementes Ltda. Utilizaram-se 15 linhagens como genitores femininos e oito linhagens como genitores masculinos. Os ensaios foram conduzidos na safra agrícola 2011/2012 em cinco ambientes, com delineamento de blocos completos ao acaso com três repetições por local. Foram avaliados 15 caracteres de interesse agronômico. A divergência genética foi avaliada por meio do método de agrupamento de Tocher e UPGMA. As estimativas da D² indicam os pares de genótipos 6 e 22 como mais distantes geneticamente, e 1 e 14 os pares mais similares. Foram identificados nove grupos divergentes em ambos os métodos de agrupamento. Os caracteres profundidade de grãos, diâmetro do sabugo, massa de sabugo, diâmetro de espiga e peso de 100 grãos foram os principais determinantes na quantificação da divergência genética. Para o melhoramento de milho, os genótipos com maiores potenciais são 5, 15, 12, 20, 9, 25 e 10.
The aim of this study was to morphologically characterize the genetic dissimilarity among 25 maize (Zea mays L.) genotypes in order to select the most divergent and higher yield potential, by multivariate analysis, in five cultivation environments in southern Brazil. The inbred lines of the two heterotic groups were from the genebank company KSP Seeds Ltda. Fifteen inbred lines were used as female parents and eight inbred lines as male parents. The tests were conducted during the harvest 2011/2012 in five environments, in randomized complete block design with three replications per location. We evaluated 15 characters of agronomic interest. Genetic divergence was assessed by the clustering method of Tocher and UPGMA. Estimates of D² indicated genotype pairs 6 and 22 as genetically more distant, and 1 and 14 the most similar pairs. Nine different groups were identified in both clustering methods. The depth of grain, cob diameter, cob weight, ear diameter and weight of 100 grains characters were the main determinants in the quantification of genetic divergence. For breeding purposes, genotypes 5, 15, 12, 20, 9, 25 and 10 are those of higher potential.

Abstract Objective: The purpose of this study was to evaluate the effect of a cycle ergometer exercise program on exercise capacity and inspiratory muscle function in hospitalized patients with heart failure awaiting heart transplantation with intravenous inotropic support. Methods: Patients awaiting heart transplantation were randomized and allocated prospectively into two groups: 1) Control Group (n=11) - conventional protocol; and 2) Intervention Group (n=7) - stationary cycle ergometer exercise training. Functional capacity was measured by the six-minute walk test and inspiratory muscle strength assessed by manovacuometry before and after the exercise protocols. Results: Both groups demonstrated an increase in six-minute walk test distance after the experimental procedure compared to baseline; however, only the intervention group had a significant increase (P =0.08 and P =0.001 for the control and intervention groups, respectively). Intergroup comparison revealed a greater increase in the intervention group compared to the control (P <0.001). Regarding the inspiratory muscle strength evaluation, the intragroup analysis demonstrated increased strength after the protocols compared to baseline for both groups; statistical significance was only demonstrated for the intervention group, though (P =0.22 and P <0.01, respectively). Intergroup comparison showed a significant increase in the intervention group compared to the control (P <0.01). Conclusion: Stationary cycle ergometer exercise training shows positive results on exercise capacity and inspiratory muscle strength in patients with heart failure awaiting cardiac transplantation while on intravenous inotropic support.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

O objetivo deste trabalho foi avaliar, em doses crescentes de fósforo, o efeito de lâminas de irrigação no teor foliar de fósforo de cafeeiros em estágio de primeiro ano de produção. O plantio (Topázio MG-1190) foi realizado em janeiro de 2010 e os tratamentos diferenciados a partir de novembro de 2011. Utilizou-se o delineamento experimental em blocos ao acaso com três repetições, com tratamentos dispostos no esquema fatorial 5 x 4 os quais se compunham por cinco lâminas de irrigação correspondentes às frações (0,4; 0,7; 1,0; 1,3 e 1,6) da lâmina calculada pelo coeficiente de cultura e quatro doses de fósforo (0, 80, 240 e 720 kg ha-1). Foi utilizado o sistema de irrigação por gotejamento enquanto a adubação fosfatada (monoamônio fosfato purificado) foi feita via fertirrigação, em doze parcelamentos iguais durante o ano. Foram avaliados os teores foliares de fósforo aos dois, quatro, seis, oito, dez e doze meses após o início dos tratamentos. Tanto em aplicação única de fósforo, no plantio, quanto na aplicação de doses crescentes parceladas via fertirrigação, observou-se que no primeiro ano de produção o teor foliar de fósforo de cafeeiros aumenta com aumento da lâmina de irrigação.
The objective of this study was to evaluate the effect of increasing phosphorus doses and different irrigation depths on a phosphorus content of leaf of coffee plants at the stage of first harvest. Coffee (Topázio MG-1190) orchard was planted in January 2010. The treatments were imposed after November 2011. An experimental design with randomized blocks arranged in a 5 x 4 factorial scheme with three replications was used. Treatments consisted of five irrigation depths, that corresponded to fractions (0.4; 0.7; 1.0; 1.3 and 1.6) of the depth based on the crop coefficient, and four phosphorus doses (0, 80, 240 and 720 kg ha-1). A drip irrigation system was used. The phosphate fertilizer used was monoammonium phosphate that was applied via fertigation in twelve equal split applications. Leaf phosphorus content was determined at the interval of two, four, six, eight, ten, and twelve months after the application of treatments. For both single application of phosphorus at planting, as in the application of increasing doses, applied by fertigation, it was observed that, in the first year of coffee harvest, leaf content of phosphorus in coffee plants increases with increasing water depth.

The aim of this study was to confirm clinical diagnosis of paratuberculosis in two cows showing suggestive clinical signs of the disease. Based on clinical signs, in culture and in IS900 PCR results from the individual milk samples it was possible to diagnose paratuberculosis in the cows studied.

Blendas compatíveis de amido termoplástico e polietileno de baixa densidade compatibilizadas com ácido cítrico

Blendas de polietileno de baixa densidade (PEBD) e amido termoplástico (TPS) modificado foram preparadas via extrusão reativa (REX) e caracterizadas por microscopia eletrônica de varredura (MEV), espectroscopia na região do infravermelho com transformada de Fourier (FTIR), absorção de umidade em ambiente com 53% de umidade relativa e difração de raios-X. Em uma primeira etapa, foi preparado o TPS- modificado, via REX a partir de uma mistura de amido de milho, ácido cítrico e glicerol. As blendas TPS-PEBD foram preparadas pelo processamento de uma mistura do TPS modificado e PEBD, ambos em peletes, em uma extrusora de rosca simples. Foi observado um significativo efeito de compatibilização, atribuído especialmente à redução da viscosidade da fase TPS e consequente redução da tensão interfacial entre as fases de TPS e PEBD. Os espectros de FTIR apresentaram deslocamentos de bandas de absorção do amido confirmando o efeito de compatibilização pelo ácido cítrico. Observou-se também significativa alteração da morfologia das blendas, especialmente para as blendas preparadas com adição de 1,0 - 1,5 % de ácido cítrico, que apresentaram estrutura da fase dispersa mais fina e homogênea.
Blends of citric acid-modified thermoplastic starch (TPS) and low density polyethylene (LDPE) were prepared by reactive extrusion (REX) and characterized by scanning electron microscopy (SEM) of the fragile fracture surfaces, Fourier-transform infrared spectroscopy (FTIR), water absorption in 53% relative humidity environment and by X-ray diffraction. The modified TPS was prepared by the reactive extrusion of a mixture of corn starch, citric acid and glycerol (30%). TPS-LDPE blends were then prepared by the processing of TPS and PEBD both in pellets in a single screw extruder. It was observed an important compatibilization effect, attributed to the reduction of TPS melt viscosity and consequently the reduction of the interfacial tension between TPS and PEBD phases. The FTIR spectra showed shifting of starch bands confirming the compatibilization effect of citric acid. Important change on blend morphology as function of the TPS modification were observed, in particular for the blends prepared with the addition of 1,0 - 1,5% AC which showed a more homogeneous and finer dispersed phase.
