ABSTRACT Introduction Clitoroplasty constitutes an important step in feminizing surgery for congenital adrenal hyperplasia (CAH) ( 1 ). In this video we present a technique that aims to preserve clitoral sensitivity and engorgement while minimizing the risk of neurovascular lesion. Materials and methods We present a video of a three-year-old girl with history of CAH classical form, PRADER-III, who underwent clitoroplasty. After an initial endoscopic evaluation of the urogenital sinus, the clitoris was degloved and a rectangular incision was made on the ventral corpora cavernosa 15mm above the corpora bifurcation and 0.5 mm below the coronal sulcus. The cavernous tissue was partially resected. The upper and lower borders of the rectangular gap were closed by a 5-0 PDS running suture similar to the Mikulicz technique. Next, the edge of the glans was deepithelialized to reduce its size. For improved clitoral positioning, the clitoris was sutured to the pubic fat. From that point onward the procedure followed that of a standard vaginoplasty using the en-bloc technique ( 2 - 4 ). Thus far we have performed this technique in 33 patients, with 31 of them being girls with CAH and 2 being women with clitoral hypertrophy. Conclusion Corporoplasty is a simplified technique for clitoroplasty, with the advantage being that is faster and safer than the technique that involves the dissection of the neurovascular bundle. In addition, corporoplasty has the possible benefit of preserving the cavernosal blood flow that permits the engorgement of the clitoris during sexual arousal.

RESUMO A Covid-19 é uma ameaça global que atrai pesquisadores de todas as áreas a fim de conhecer a história natural da doença e seus parâmetros epidemiológicos. O desenvolvimento tecnológico de vacinas, testes para diagnóstico e alvos terapêuticos tem mobilizado grande quantidade de recursos em curto tempo. A Avaliação de Tecnologias em Saúde (ATS), baseada em uma análise sistemática das propriedades, efeitos e/ou impactos das tecnologias em Saúde através de métodos e instrumentos validados internacionalmente para avaliar a qualidade e rigor metodológico dos estudos que demonstrem eficácia, efetividade e segurança, está sofrendo pressões, sem precedentes, devido à pandemia. Este artigo traz uma reflexão sobre a autorização emergencial do uso de tratamentos e testes diagnósticos que conflita com os preceitos da ATS. Apresentaremos também uma avaliação das tecnologias não farmacológicas de contenção da doença e seus impactos na economia doméstica e internacional, bem como as intervenções judiciais que impactam as decisões dos gestores no contexto excepcional da pandemia de Covid-19.

ABSTRACT Covid-19 is a global threat that attracts researchers from all areas to understand the natural history of the disease and its epidemiological parameters. The technological development of vaccines, diagnostic tests, and therapeutic targets has mobilized many resources in a short time. Health technology assessment (HTA), based on a systematic analysis of the properties, effects and/or impacts of health technologies through internationally-validated methods and instruments to assess the quality and methodological rigor of studies that demonstrate effectiveness, efficacy, and safety, is under unprecedented pressure due to the pandemic. This article also reflects on the emergency authorization of the use of treatments and diagnostic tests that conflict with HTA precepts. We will also evaluate non-pharmacological technologies of containment of the disease and their impacts on the domestic and international economy, as well as the judicial interventions that impact the decisions of managers in the exceptional context of the covid-19 pandemic.

Resumo Escoliose idiopática do adolescente (EIA) atinge de 2% a 4% de jovens no Brasil. Repetidas exposições aos exames radiológicos, no acompanhamento desta deformidade, podem ser danosos à saúde. O objetivo deste estudo é apresentar um protocolo de fotogrametria, como método não ionizante para quantificação da escoliose, e relacioná-lo ao método radiológico de Cobb. Dezesseis indivíduos portadores de escoliose idiopática (21,4 ± 6,1 anos de idade e 19,8±0,2 de índice de massa corporal) foram submetidos à radiografia posteroanterior do tronco, de pé e, posteriormente, fotografia do tronco posterior, após receberem marcadores anatômicos nos processos espinhosos das vértebras C7 até L5. As imagens foram encaminhadas para análise independente de dois examinadores treinados na quantificação da escoliose para o tipo de imagem recebida. A média angular torácica de Cobb e de fotogrametria foram 36,14° e 36,43°, respectivamente. A diferença média entre os métodos foi de 4,1°. Não houve diferença estatisticamente significante (p-valor < 5%) entre eles. A fotogrametria, por ser não ionizante, ter baixo custo e ser portátil, poderá representar uma alternativa ao método radiológico. Novos estudos são necessários no aprimoramento das técnicas não ionizantes no rastreamento da EIA.

Abstract Adolescent idiopathic scoliosis (AIS) affects 2% to 4% of young people in Brazil. Repeated exposures to radiation used in the monitoring of the deformity can be harmful to the health. This study aimed to present a photogrammetry protocol as a non-ionizing method to quantify scoliosis and relate it to the Cobb radiological method. Sixteen individuals with idiopathic scoliosis (age: 21.4 ± 6.1 years, body mass index: 19.8 ± 0.2 kg/m2) underwent standing posteroanterior X-ray examination of the trunk. Additionally, markers were placed on the spinal processes of the C7 to L5 vertebrae, and posterior trunk photographs were taken. All images were sent for independent analysis by two examiners who were trained in the quantification of scoliosis. The average of the thoracic curvature evaluated through the photogrammetry and Cobb methods were 36.43° and 36.14°, respectively. With an average difference of 4.1°, the methods were not statistically different (p < 0.05). As a non-ionizing method that is low cost and portable, photogrammetry may represent a suitable alternative to the radiological method. Further studies are needed for the improvement of non-ionizing techniques in AIS screening.

Resumo Introdução: A postura do indivíduo sentado, utilizada rotineiramente na execução de grande variedade de tarefas, constitui-se num potencial aumentado para o desenvolvimento da postura de projeção da cabeça, a qual pode ocasionar sérios comprometimentos à saúde de vários sistemas no corpo humano. Objetivo: Um estudo do equilíbrio estático foi realizado, relacionando a posição da cabeça com a curvatura da coluna lombar em três diferentes posturas do indivíduo sentado. Métodos: Foi realizado o cálculo do valor aproximado de força e momento fletor dos músculos extensores da cabeça na manutenção do equilíbrio estático em cada uma das seguintes posturas: (A) sem uso de encosto para as costas; (B) com uso de encosto de 100° de inclinação; (C) com uso de encosto de 100° associado a um suporte lombar cilíndrico em nível da vértebra L3. Resultados: Os ângulos tragus-C7 foram 43°, 50° e 52°; os ângulos de Frankfort foram 5°, 9° e 9°; as forças musculares extensoras da cabeça foram 53,0N, 59,7N e 43,5N e os momentos fletores foram 2,60N.m, 2,05N.m e 1,78N.m, nas posturas A, B e C, respectivamente. Conclusão: Os resultados revelaram que a postura sentada com encosto inclinado a 100° e calço lombar, onde houve a menor distância horizontal tragus-L3, apresentou menor esforço para a musculatura cervical extensora na manutenção do equilíbrio da cabeça. Este estudo demonstrou a necessidade da preservação do eixo lombar fisiológico, caracterizado pelo posicionamento da vértebra L3, para garantir o bom posicionamento da cabeça.

Abstract Introduction: The sitting position routinely used for a wide variety of tasks increases the potential of developing forward head posture, which can seriously compromise the health of different systems in the human body. Objective: A static equilibrium analysis was conducted, comparing the position of the head with the lumbar curve in three different sitting positions. Methods: The approximate force and flexion moment of the head extensor muscles in static equilibrium was calculated in each of the following positions: (A) without a backrest; (B) using a backrest with a 100° tilt angle; (C) using a 100° tilted backrest associated with a cylindrical lumbar support cushion at the level of the L3 vertebra. Results: The C7-tragus angles were 43°, 50° and 52°; Frankfort horizontal plane (FH) angles were 5°, 9° and 9°; force of the head extensor muscles was 53.0N, 59.7N and 43.5N and flexion moments were 2.60Nm, 2.05Nm and 1.78Nm, in positions A, B and C, respectively. Conclusion: The results revealed that the sitting position using a 100° tilted backrest and lumbar support with the smallest L3-tragus horizontal distance required less effort by the head and neck extensor muscles to retain the head in equilibrium. This study demonstrated the need to preserve the physiology of the lumbar spine, characterized by the position of the L3 vertebra, in order to ensure good head position.

Pulmonary hypertension (PH) in pregnancy is a rare disorder that carries a high risk to mother and child, and as such, it is considered a contraindication to becoming pregnant. However, there are few published reports related to the diagnosis of this condition after delivery. We describe three PH cases diagnosed after their normal pregnancies and deliveries. Although the causes are unknown, several mechanisms such as hypercoagulation, placental hypoxia or amniotic fluid embolism have been considered as possible causes. It is difficult to define whether a PH diagnosed in the postpartum period, relates to an earlier asymptomatic PH period that was triggered by the physiological stress of labor or if it is a recently acquired condition. Despite the lack of data to support the absence of PH previous to pregnancy in our three patients, lack of events during this period, asymptomatic and normal deliveries, lead us to believe that they did not suffer this disease prior to pregnancy; considering that high hemodynamic demands impair a ventricle with little reserve, and its subsequent appearance at time of delivery.

La presencia de hipertensión pulmonar (HP) en el embarazo es poco frecuente y conlleva un alto riesgo para madres e hijos. Existe escasa bibliografía relacionada al diagnóstico de la misma luego del parto. Se describen tres pacientes a quienes se diagnostica HP luego de cursar sus embarazos y partos libres de eventos. A pesar de desconocerse las causas, son varios los mecanismos propuestos, como la hipercoagulabilidad, la hipoxia placentaria o la embolia de líquido amniótico. Resulta difícil definir si la HP diagnosticada en el puerperio, corresponde a una HP en período asintomático que fue desenmascarada por el estrés fisiológico del parto o es una condición de reciente comienzo. A pesar de la falta de datos que avalen la ausencia de HP previa al embarazo en nuestras tres casos, el curso libre de eventos en sus embarazos, sin síntomas y con partos normales, indican que no padecían esta enfermedad hasta el momento del parto, y que la desarrollaron posteriormente. De haberla padecido antes se hubieran presentado síntomas previos al parto o en el puerperio inmediato, ya que las demandas hemodinámicas deterioran gravemente a un ventrículo con poca reserva.

O objetivo desse artigo é apresentar as características gerais de uma nova metodologia para planejamento energético urbano, detalhando os procedimentos para integração de condicionantes de morfologia urbana, que constitui a primeira etapa de aplicação metodológica. Esta abordagem é inovadora e focada na integração de estratégias e soluções em morfologia e mobilidade urbanas, edifícios, energia e meio ambiente para planejamento de áreas em desenvolvimento urbano, novas ou a serem requalificadas, na escala de distritos, bairros e unidades de vizinhança. A análise dos resultados da aplicação metodológica no caso da operação urbana Água Branca indicou que a ocupação urbana por edificações e as densidades construída e populacional necessitavam ser compatibilizadas com a área livre requerida ao adequado dimensionamento da seção viária, no que tange a aspectos de climatologia e de provimento de infraestrutura e demanda para mobilidade urbana. Esta era uma premissa para o aproveitamento da luz e da energia solar em edificações e para que o sistema de transporte urbano fosse mais funcional e energeticamente mais eficiente.

The aim of this paper is to present the general characteristics of a new methodology for urban energy planning, detailing the procedures for integrating the constraints of urban morphology, which is the first stage of the methodological application. This approach is innovative and focused on the integration of strategies and solutions in urban morphology and mobility, buildings, energy and environment for the planning of urban development areas, either new or to be reclassified, at the district and neighborhood scales. The results of the application of this methodology in the case of Agua Branca urban operation indicated that the occupied area and the built environment and population densities needed to be harmonized with the free area required for the appropriate sizing of the road section, with respect to both the urban climatology parameters, and the provision of infrastructure and demand for urban mobility. That was a premise for the use of sunlight and solar energy in buildings as well as for having more functional and more energy efficient urban transport system.

OBJETIVO: Analisar os resultados, em 30 dias, em pacientes submetidos de forma eletiva à revascularização arterial total do miocárdio sem circulação extracorpórea (CEC), e identificar preditores de morbimortalidade com esta estratégia cirúrgica. MÉTODO: Entre maio de 1999 e fevereiro de 2004, efetuaram-se 203 cirurgias de revascularização miocárdica (CRM) com revascularização arterial total sem CEC, em pacientes com doença de múltiplos vasos (três vasos 81,7%, doença de um vaso excluída). Reportaram-se variáveis pré-operatórias e comorbidade: média de idade 63,9 ± 9,13 anos, homens 182 (89,5%), hipertensão 132 (65%), tabagismo 125 (61%), hipercolesterolemia 152 (74,8%), infarto agudo do miocárdio prévio (mais de 30 dias) 73 (35%), disfunção ventricular moderada a grave 31 (15%), reoperação cinco (2,5%). A revascularização arterial total incluiu anastomoses em T e seqüenciais com artéria torácica interna esquerda (100%), torácica interna direita (56,6%) e artéria radial (63%). O número total de anastomoses foi 576 (média de três pontes/paciente), todas efetuadas com estabilizadores mecânicos externos. Não form realizadas anastomoses proximais na aorta. Cirurgia com CEC foi realizada em três (1,5%) pacientes. Noventa por cento dos pacientes foram extubados na sala de operações. Para a análise estatística, utilizou-se teste de regressão logística múltipla. RESULTADOS: A incidência de fibrilação atrial pós-operatória foi de 12,8% (26), insuficiência renal oligoanúrica 3% (seis), diálise 0,49% (um), infarto de miocárdio pós-operatório 1,47% (três), baixo débito cardíaco 4% (oito), reoperação por sangramento 1,47% (três), mediastinite 1,47% (três), acidente vascular cerebral 1,47% (três). A mortalidade intra-hospitalar foi de 2,45% (cinco). O único preditor independente de morbidade em 30 dias foi a idade (p=0,033; OR 1,04; IC 95%: 1-1,08). CONCLUSÃO: A cirurgia de revascularização miocárdica sem circulação extracorpórea utilizando condutos arteriais para a doença de múltiplos vasos é factível com baixa morbimortalidade em 30 dias.

OBJECTIVE: To analyze intra-hospital results in patients undergoing elective off-pump total arterial revascularization and identify morbidity and mortality predictors using this surgical strategy. METHOD: From May 1999 to February 2004, 203 myocardial revascularization procedures with total arterial revascularization without cardiopulmonary bypass (off-pump) were carried out in patients with multivessel disease (three vessels 81.7 %, one vessel disease was excluded). We report pre-surgical variables and co-morbidities: average age 63.9 ± 9.13 years, men 182 (89.5%), hypertension 132 (65%), smokers 125 (61%), hypercholesterolemia 152 (74.8%), previous myocardial infarction (> 30 days) 73 (35%), moderate to severe ventricular dysfunction 31 (15%), redo five (2.5%). Total arterial revascularization included T-grafts and sequential grafts with left internal mammary (100%), right internal mammary (56.6%) and radial (63%) arteries. The total number of distal anastomosis was 576 (mean of three grafts/patient), all carried out with external mechanical stabilizers. There were no proximal aortic anastomoses. Conversion to on-pump surgery occurred in three (1.5%) patients; 90% of the patients was extubated in the operating room. The multiple logistic regression test was used for statistical analysis. RESULTS: The postoperative incidence of atrial fibrillation was 12.8% (26), oligoanuric renal failure 3% (six), dialysis 0.49% (one), postoperative myocardial infarction 1.47% (three), low cardiac output 4% (eight), Redo for bleeding 1.47% (three), mediastinitis 1.47% (three), stroke 1.47% (three). Intra-hospital mortality was 2.45% (five). The only independent 30 day morbidity predictor was age (p=0.033; OR 1.04; IC 95%: 1-1.08). CONCLUSION: Off-pump myocardial revascularization with arterial conduits for multiple vessel disease is feasible with a low 30-day morbidity and mortality.

During this study, fifteen shark heads of sand tiger shark Carcharias taurus (Rafinesque, 1810) were analyzed. The studied material was obtained in the Santos Fishing Terminal, São Paulo, Brazil. The heads dissection was focused on the characterization of the mandibular muscles and on the description of the mandibular branch of the trigeminal nerve. The C. taurus mandibular muscles are represented by: m. preorbitalis, m. levator palatoquadrati, m. quadratomandibularis and m. intermandibularis. The origin of the trigeminal nerve of C. taurus is located in a lateral portion of the medulla oblongata. In the orbita, the trigeminal nerve branches off to originate the mandibular branch that innervates the muscles which are derived from the mandibular arch. The proximal branches of the trigeminal nerve mandibular branch innervate the m. levator palatoquadrati. The muscles preorbitalis and quadratomandibularis receive fibers from the intermediate branches of the trigeminal nerve mandibular branch and the distal ramification of the mandibular branch can be visualized in the m. intermandibularis

Fueron analizadas 15 cabezas de tiburón toro Carcharias taurus (Rafinesque, 1810), obtenidas en el Terminal Pesquero de Santos, São Paulo, Brasil. La disección permitió caracterizar los músculos mandibulares y describir la rama mandibular del nervio trigémino. Los músculos mandibulares de C. taurus son: M. preorbitalis, M. levator palatoquadrati, M. quadratomandibularis y M. intermandibularis. El origen aparente del nervio trigémino de C. taurus está localizado en la porción lateral de la médula oblonga. En la órbita, las ramas del nervio trigémino salen para dar origen a la ramificación mandibular, la cual inerva a los músculos derivados del arco mandibular. Las ramificaciones proximales de la rama mandibular del nervio trigémino inervan al M. levator palatoquadrati. Los Mm. preorbitalis y quadratomandibularis reciben fibras de las ramas intermedias de la ramificación mandibular del nervio trigémino, y el M. intermandibularis recibe la ramificación distal de la rama mandibular del nervio trigémino

OBJECTIVE: To confirm the occurrence of an outbreak of viral meningitis in 1996 in the province of Tucuman, Argentina, and to study the outbreak's epidemiological characteristics. METHODS: We analyzed information from the National Epidemiological Surveillance System of the Ministry of Health (MOH) of Argentina for 1994-1998 that had been provided by the MOH's Bureau of Epidemiology. We calculated incidence rates using population estimates for the years 1994-1998 developed by the National Statistics and Census Institute, based on the 1991 census. We studied frequencies with contingency tables, using the chi-square method with Yates' correction. Results were considered significant when P < 0.05. RESULTS: We confirmed the occurrence of an outbreak of 189 cases of viral meningitis between 11 February and 18 May 1996. The incidence of cases in Tucuman province increased between 1995 and 1996, from 0.5 to 19.3 cases per 100 000 person-years. That 1996 rate in Tucuman was significantly higher than what was seen in the rest of the country (2.8 cases per 100 000 person-years). Of the 189 cases, 142 of them (75.1%) occurred in children less than 9 years old. Out of 111 samples studied, 65 of them (58.6%) were enterovirus-positive. Through reverse transcription-nested polymerase chain reaction, enteroviruses were found in 66.3% (53/80) of the cases studied by this method, versus in only 29.6% (24/81) of the cases studied through viral isolation. In the 22 samples that were serotyped, echovirus type 4 was identified in 15 of them (68%): 5 by isolation, 3 by sequencing, and 7 by both methods. During the Tucuman outbreak, at least 56% of the cases were hospitalized. This viral meningitis outbreak shows the capacity of enteroviruses to spread and cause disease. CONCLUSIONS: The use of molecular methods makes it possible to rapidly diagnose the etiological virus and to better control an outbreak. Recognizing this outbreak in Tucuman sooner could have averted the majority of the hospitalizations and the indiscriminate use of antibiotics.

OBJETIVO: Confirmar la existencia de un brote de meningitis viral en 1996 en la provincia de Tucumán, Argentina, y estudiar sus características epidemiológicas. MÉTODOS: Se analizó información obtenida del Sistema Nacional de Vigilancia Epidemiológica (SINAVE) del Ministerio de Salud de Argentina para el período de 1994-1998, la cual fue provista por la Dirección de Epidemiología de dicho ministerio. Para el cálculo de incidencias se usaron estimaciones poblacionales para los años 1994-1998 realizadas por el Instituto Nacional de Estadística y Censos (INDEC) sobre la base del censo de 1991. El estudio de frecuencias se realizó mediante el análisis de tablas de contingencia de doble entrada, según el método de ji cuadrado con la corrección de Yates. Se consideró significativo el resultado cuando P < 0,05. RESULTADOS: Se confirmó la presencia de un brote de 189 casos entre el 11 de febrero y el 18 de mayo de 1996. La incidencia de casos en la provincia mostró un aumento entre 1995 y 1996 (de 0,5 a 19,3 casos por 100000 años-persona) y dicha incidencia fue significativamenrte mayor que la observada en el resto del país (19,3 frente a 2,8 casos por 100000 años-persona). El 75,1% de los casos ocurrió en niños menores de 9 años (142/189). Se detectó la presencia de Enterovirus (EV) en 65 de las 111 muestras estudiadas (58,6%). Mediante la reacción en cadena de la polimerasa (RCP) anidada con transcripción inversa se logró detectar EV en 66,3% (53/80) de los casos estudiados por este método, en comparación con solo 29,6% (24/81) de los estudiados mediante aislamiento viral. Se identificó echovirus tipo 4 en 15 (68%) en las 22 muestras tipificadas (5 por aislamiento, 3 por secuenciación y 7 por ambos métodos). Este brote demuestra la capacidad de los EV para diseminarse y producir enfermedad en la población. Durante el brote, por lo menos 56% de los casos fueron hospitalizados. CONCLUSIONES: El uso de métodos moleculares permitió el diagnóstico rápido del virus etiológico y posibilitó un mejor control del brote. El reconocimiento temprano de este podría haber evitado la mayoría de las hospitalizaciones y el uso indiscriminado de antibióticos.
