Abstract Introduction: We investigated the clinical course and outcomes of patients submitted to cardiovascular surgery in Brazil and who had developed symptoms/signs of coronavirus disease 2019 (COVID-19) in the perioperative period. Methods: A retrospective multicenter study including 104 patients who were allocated in three groups according to time of positive real time reverse transcriptase-polymerase chain reaction (RT-PCR) for the SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2): group 1, patients who underwent cardiac surgery > 10 days after positive RT-PCR; group 2, patients with a positive RT-PCR within 10 days before or after surgery; group 3, patients who presented positive RT-PCR > 10 days after surgery. The primary outcome was mortality and secondary outcomes were postoperative complications, intensive care unit (ICU) length of stay, and postoperative days of hospitalization. Results: The three groups were similar with respect to age, the European System of Cardiac Operative Risk Evaluation score, and comorbidities, except hypertension. Postoperative complications and death were significantly higher in groups 2 and 3 than in group 1, and no significant difference between groups 2 and 3 was seen. Group 2 showed a high prevalence of surgery performed as an urgent procedure. Although no significant differences were observed in ICU length of stay, total postoperative hospitalization time was significantly higher in group 3 than in groups 1 and 2. Conclusion: COVID-19 affecting the postoperative period of patients who underwent cardiovascular surgery is associated with a higher rate of morbidity and mortality. Delaying procedures in RT-PCR-positive patients may help reduce risks of perioperative complications and death.

Resumo Este estudo de caso objetivou analisar o cuidado à saúde de famílias assentadas no interior do Estado de Pernambuco, mediante observação participante, entrevistas individuais com profissionais de saúde e representante do Movimento dos Trabalhadores Rurais Sem Terra - MST, grupos focais com as famílias de dois assentamentos e diário de campo. Observou-se que as práticas de saúde estavam relacionadas ao cuidado popular por meio do uso de plantas medicinais, ações de prevenção, como vacinação, puericultura, acompanhamento a hipertensos e diabéticos, tratamento da água e destino do lixo. Identificou-se que fatores socioeconômicos, culturais e educacionais impactavam, de maneira negativa, na condição de saúde, enquanto a formação dos profissionais e a capacidade resolutiva da Atenção Primária limitavam a oferta de ações. Apesar de as práticas de saúde estarem de acordo com a Política Nacional da Atenção Básica, havia lacunas entre o fazer saúde técnico e o fazer saúde popular. Destacaram-se as ações realizadas pelo Movimento, como o diagnóstico da condição de saúde e planejamento, a partir da educação popular. As necessidades de saúde apresentadas pelas famílias demandavam das equipes competências e habilidades específicas para o cuidado à saúde integral implicado com a realidade socioeconômica, cultural e sanitária dos assentamentos da Reforma Agrária.
Abstract This case study aimed to analyze the health care of families settled in the countryside of Pernambuco state, Brazil, through participant observation, individual interviews with health professionals and representative of the Landless Workers Movement (MST), focus groups with families from two settlements and field diary. Health practices were related to the popular care through the use of medicinal plants, preventive actions, such as vaccination, childcare, monitoring of hypertensive and diabetic patients, water treatment and waste destination. We identified that socioeconomic, cultural and educational factors had a negative impact on health conditions, while the training of professionals and the resolving capacity of Primary Care limited the supply of actions. Although health practices were in accordance with the National Primary Care Policy, there were gaps between making technical health and making health popular. The actions carried out by the Movement were highlighted, such as the diagnosis of health status and planning, based on popular education. The health needs presented by the families demanded of the teams specific skills and abilities for the integral health care implied with the socioeconomic, cultural and sanitary reality of the Agrarian Reform settlements.

Brazilian multicenter study of 71 patients with juvenile-onset Takayasu's arteritis: clinical and angiographic features

RESUMO Objetivo: Descrever as características clínicas e angiográficas da arterite de Takayasu em crianças e adolescentes brasileiros. Métodos: Foi feita coleta retrospectiva de dados de 71 crianças e adolescentes acompanhados em 10 centros brasileiros de referência em reumatologia pediátrica. A avaliação foi feita em três tempos: início dos sintomas até o diagnóstico, do 6º ao 12º mês de diagnóstico e última consulta. Resultados: Dos 71 pacientes selecionados, 51 (71,8%) eram meninas. As médias de idade de início dos sintomas e de tempo até diagnóstico foram 9,2 anos (± 4,2) e 1,2 anos (± 1,4), respectivamente. No fim do estudo, 20 pacientes estavam em atividade de doença, 39 em remissão e cinco haviam evoluído a óbito. Os sintomas mais frequentes nas avaliação inicial, segunda avaliação e avaliação final foram, respectivamente, os constitucionais, os musculoesqueléticos e os neurológicos. A redução de pulsos periféricos foi o sinal cardiovascular mais frequente e a elevação da velocidade de hemossedimentação foi o achado laboratorial mais frequente nos três períodos de avaliação. O teste tuberculínico foi reagente em 41%. A estenose foi a lesão angiográfica mais encontrada, a artéria abdominal foi o segmento mais afetado e tipo angiográfico IV o mais frequente. A maioria (90%) fez terapia com glicocorticoides, 85,9% necessitaram de outro imunossupressor e 29,6% foram submetidos à angioplastia. Conclusão: Este é o maior estudo de arterite de Takayasu juvenil e nós observar elevado número de pacientes com idade inferior a 10 anos e a predominância de sintomas constitucionais no início da doença.
ABSTRACT Objective: To describe the clinical and angiographic characteristics of Takayasu's arteritis in Brazilian children and adolescents. Methods: A retrospective data collection was performed in 71 children and adolescents followed in 10 Brazilian reference centers in Pediatric Rheumatology. The evaluation was carried out in three different time points: from onset of symptoms to diagnosis, from the 6 th to 12th month of diagnosis, and in the last visit. Results: Of 71 selected patients, 51 (71.8%) were girls. The mean age of onset of symptoms and of time to diagnosis was 9.2 (± 4.2) years and 1.2 (± 1.4) years, respectively. At the end of the study, 20 patients were in a state of disease activity, 39 in remission and 5 had evolved to death. The most common symptoms in baseline assessment, second evaluation, and final evaluation were, respectively: constitutional, musculoskeletal, and neurological symptoms. A decrease in peripheral pulses was the most frequent cardiovascular signal, and an increase in erythrocyte sedimentation rate was the most frequent laboratory finding in all three evaluation periods. The tuberculin test was positive in 41% of those tested. Stenosis was the most frequent angiographic lesion, abdominal artery was the most affected segment, and angiographic type IV the most frequent. Most (90%) participants were treated with glucocorticoids, 85.9% required another immunosuppressive drug, and 29.6% underwent angioplasty. Conclusion: This is the largest study on juvenile-onset Takayasu arteritis, and a high number of patients under the age of 10 years, with predominance of constitutional symptoms early in the disease, was observed.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Avaliação clínica e hematológica de equinos submetidos ao modelo de laminite por oligofructose, tratados ou não com um agente antagonista de receptores CXCR1/2

O recrutamento de leucócitos aos tecidos é uma parte essencial da resposta imune inata e esse processo de forma desregulada pode resultar em lesões aos tecidos. Assim, a infiltração de leucócitos tem sido implicada na patogênese de laminite aguda em equinos. Os objetivos dessa pesquisa foram verificar a ação da ICXCR1/2 sobre os sinais clínicos e parâmetros hematológicos de cavalos com laminite induzida por oligofrutose. Doze equinos receberam oligofrutose (10g/kg de peso vivo PO) no tempo 0 e foram divididos em 2 grupos: tratados (30mg/kg p.v. ICXCR1/2 IV, nos tempos 6, 12, 18 e 24 h) e não tratados. As frequências cardíaca e respiratória, temperatura retal, coloração de membranas mucosas, presença e intensidade de pulso digital, sensibilidade ao exame com pinça de casco e grau de claudicação segundo Obel, bem como parâmetros hematológicos e bioquímicos (hemograma e as concentrações sanguíneas de glicose, uréia, creatinina, ALT, AST, FA, GGT, bilirrubina total e proteína total) foram aferidos nos tempos 0, 6, 12, 18, 24, 36, 48, 60 e 72 horas . O modelo usando oligofructose foi adequado para induzir sinais de laminite e de sinais de endotoxemia, como diarreia, febre e leucocitose em cavalos sem raça definida de origem nacional. Também, não foram observadas quaisquer reações adversas clínicas ou hematológicas relacionadas ao uso intravenoso do antagonista de CXCR1/2, contudo essa substância, quando administrada na dose de 30mg/kg de peso vivo, 4 vezes ao dia, por 4 aplicações, não foi capaz de prevenir os sinais clínicos e as alterações hematológicas causadas pela administração de oligofructose nos equinos deste estudo.
Leucocytes recruitment to tissues is an essential part of the innate immune response and an unregulated process can result in tissue damage. Thus, leucocytes infiltration has been implicated in the pathogenesis of acute laminitis. The objectives of this stud were to determine the effect of an antagonist for CXCR1/2, a chemokine receptor for neutrophils attraction on clinical signs and hematological parameters in horses given oligofructose to induce laminitis. Twelve horses were given oligofructose (10g/kg bw PO) in time 0 and divided into two groups: one treated (30mg/kg bw. ICXCR1/2 IV, times 6, 12, 18 e 24 h) and the other not treated. Cardiac and respiratory frequency, rectal temperature, mucous membrane colour, digital pulse, hoof sensitivity and Obel's grade of lameness were recorded. Values for RBC, WBC and blood glucose, BUN, creatinin, ALT, AST, alkaline phosphatase, GGT, total bilirubin and serum protein concentrations were measured on times 0, 6, 12, 18, 24, 36, 48, 60 e 72 h. All the horses given oligofructose developed signs of endotoxemia like diarrhea, fever and leukocytosis and laminitis. Also, CXCR1/2 antagonist treatment did not cause any adverse effects. However, this substance when injected intravenously (30mg/kg) 6/6 hours in 4 applications, did not ameliorate clinical and hematological signs of endotoxemia.

In the present investigation we evaluate methods for the isolation and growth of marine-derived fungal strains in artificial media for the production of secondary metabolites. Inoculation of marine macroorganisms fragments in Petri dishes proved to be the most convenient procedure for the isolation of the largest number of strains. Among the growth media used, 3% malt extract showed the best result for strains isolation and growth, and yielded the largest number of strains from marine macroorganisms. The percentage of strains isolated using each of the growth media which yielded cytotoxic and/or antibiotic extracts was in the range of 23-35%, regardless of the growth media used. Further investigation of extracts obtained from different marine-derived fungal strains yielded several bioactive secondary metabolites, among which (E)-4-methoxy-5-(3-methoxybut-1-enyl)-6-methyl-2H-pyran-2-one is a new metabolite isolated from the Penicillium paxilli strain Ma(G)K.

A síndrome da apnéia e hipopnéia obstrutiva do sono (SAHOS) é, atualmente, considerada um problema de saúde pública por causar aumento da morbi-mortalidade cardiovascular e acidentes de trânsito. A polissonografia assistida é o padrão-ouro para o diagnóstico e acompanhamento destes pacientes. No entanto, por ser onerosa, demorada e de acesso restrito, outros métodos tem sido desenvolvidos. A escala de sonolência de Epworth (ESE) é uma avaliação subjetiva, porém, rápida, sem custos e simples de ser aplicada. OBJETIVO: Avaliar a correlação entre a pontuação da ESE e o índice de apnéia e hipopnéia (IAH) da polissonografia de pacientes com SAHOS. FORMA DE ESTUDO: Clínico retrospectivo. MATERIAL E MÉTODO: Revisão de prontuário de 66 pacientes com queixa de roncopatia que foram submetidos a procedimento cirúrgico (uvulopalatofaringoplastia com ou sem abordagem nasal). Avaliaram-se a pontuação da ESE e o IAH da polissonografia pré e pós-operatórios. RESULTADOS: 78,7% pacientes com grau normal de IAH tiveram pontuação de ESE menor do que 10 e 65% pacientes com grau severo de IAH tiveram pontuação maior do que 10. Não houve resultados estatisticamente significantes para os grupos moderado e leve. CONCLUSÃO: A escala de Epworth pode distinguir os graus normais e severos sem, no entanto, determinar os graus moderado e leve. Assim, pode ser utilizada para acompanhamento de pacientes com SAHOS sem, no entanto, substituir a polissonografia uma vez que não consegue avaliar todos os graus de severidade.
Today obstructive sleep apnea–hypopnea syndrome (OSAHS) is a public health issue, since it increases cardiovascular morbidity-mortality rate and the risk of car crashes. Overnight polysomnography is the gold standard for diagnosis and follow-up of affected patients. However, because the test is expensive, time-consuming and of difficult access, others methods have been proposed. Although the Epworth Sleepiness Scale (ESS) is subjective, the questionnaire is simple, easy to be applied and free of charge. AIM: to compare Epworth Sleepiness Scale scores and apnea-hypopnea index (AHI), measured by polysomnography, in patients diagnosed with OSAHS. STUDY DESIGN: clinical retrospective study. METHOD: chart analysis of 66 patients complaining from snoring, who underwent surgery (uvulopalatopharyngoplasty with/without nasal surgery). ESS score and AHI were evaluated before and after surgery. RESULTS: 78% of patients with normal AHI, scored < 10 in the ESS and 65% of patients with severe AHI scored >10. There were no statistically significant results for groups presenting mild and moderate apnea. CONCLUSION: ESS can detect normal and severe levels of apnea, but is not able to detect mild and moderate levels. Therefore, ESS can be used in the follow-up of patients with OSAHS, however, it cannot replace polysomnography because it does not detect all levels of apnea.

A laringectomia parcial é uma alternativa no tratamento do câncer de laringe. Este tipo de cirurgia tem vantagens na qualidade de vida do paciente, pois permite a não realização de traqueostomia permanente. OBJETIVO: Comparar os resultados das laringectomias parciais com e sem realização de traqueostomia. FORMA DE ESTUDO: Caso-controle. MATERIAL E MÉTODO: Foram estudados 22 pacientes submetidos a laringectomias parciais, sendo 11 sem traqueostomia e 11 com traqueostomia no intra-operatório. RESULTADOS: O grupo de pacientes sem traqueostomia apresentou vantagens quanto ao tempo de cirurgia, de internação e de permanência da sonda nasoenteral. CONCLUSÃO: A laringectomia parcial sem traqueostomia é um procedimento vantajoso para o paciente, diminuindo a morbidade e não apresentando diferença quanto a positividade das margens das lesões.
Partial laryngectomy is an alternative in the treatment of laryngeal cancer. This kind of surgery has advantages in quality of life, because it allows procedure without permanent tracheostomy. AIM: Compare results of partial laryngectomies with and without tracheostomy. STUDY DESIGN: Case-control. MATERIAL AND METHOD: Twenty-two patients submitted to partial laryngectomy were analyzed, 11 without tracheostomy and 11 with tracheostomy in the same surgical act. RESULTS: The group of patients without tracheostomy shows advantages in the matter of time of surgery, hospitalization and use of nasoenteral tube. CONCLUSION: The laryngectomy without tracheostomy is an advantageous procedure lowing morbidity and showing no difference in the positivity of lesion bounds.
