Abstract: Introduction: Screening for Autism Spectrum Disorders (ASD) using the Modified Checklist for Autism in Toddlers, Revised with Follow-up (M-CHAT-R/F) increases early detection, allowing early interventions and improving prognosis. This tool is part of the management in case of suspected ASD in several clinical guidelines. The objective of this article was to conduct the concurrent and discrimi nant validation and the reliability analysis of M-CHAT-R/F in the Chilean population. Patients and Method: This is the second stage of the cross-cultural adaptation of cross-sectional design. M-CHAT- R/F was applied to a sample of 20 children with suspected ASD and 100 randomly selected healthy control children, aged between 16-30 months. Autism Diagnostic Observation Schedule (ADOS-2), considered as reference, was applied to the 20 patients of the clinical sample, to 20 children of the healthy control sample and to those cases of the healthy control sample with M-CHAT-R/F positive. Cronbach alpha was calculated, as well as M-CHAT-R/F and ADOS-2 correlation, sensitivity, and specificity analyses. Results: In the healthy sample, M-CHAT-R/F was positive in two patients, with one of them positive and the other one negative for ASD with ADOS-2 test. In the clinical sample, M- CHAT-R/F was positive in all cases, three of them were negative in the ADOS-2 test. The Alfa relia bility of M-CHART-R/T was 0,889, the discriminant sensitivity and specificity were 100% and 98%, and the concurrent ones were 100% and 87.5% respectively. Conclusions: The Chilean M-CHAT- R/F version was reliable, sensitive and specific, similar to the original test, which opens the possibility for its use in clinical samples and for research. Validating M-CHAT-R/F is an ongoing process which must be further developed.

Resumen: Introducción: El tamizaje de los Trastornos del Espectro Autista (TEA) mediante el Modified Chec klist for Autism in Toddlers - Revised with Follow Up (M-CHAT-R/F) aumenta la detección precoz, posibilitando intervenciones tempranas y mejorando el pronóstico. Este instrumento es parte del algoritmo de manejo ante la sospecha de TEA en diversas guías clínicas. El objetivo fue realizar la validación concurrente, discriminante y el análisis de confiabilidad del M-CHAT-R/F en una pobla ción chilena. Pacientes y Método: Esta es la segunda etapa de la adaptación transcultural, de diseño transversal. Se aplicó M-CHAT-R/F a una muestra de 20 niños con sospecha de TEA y 100 niños de control sano seleccionados al azar, de 16-30 meses de edad. Se aplicó Autism Diagnostic Observation Schedule (ADOS-2), considerado como referencia, a los 20 pacientes de la muestra clínica, a 20 niños de la muestra de control sano y a aquellos casos de la muestra de control sano con M-CHAT-R/F po sitivo. Se calculó alfa de Cronbach, análisis de correlación de M-CHAT-R/F y ADOS-2 y sensibilidad y especificidad. Resultados: En el grupo de control sano, M-CHAT-R/F resultó alterado en 2 pacien tes, siendo uno positivo y otro negativo para TEA con ADOS-2. En muestra clínica el M-CHAT-R/F fue positivo en todos, con test de ADOS-2 negativo en 3 casos. La confiabilidad Alfa del M-CHAT- R/F fue =0,889, la sensibilidad y especificidad discriminante de 100 y 98% y la concurrente 100% y 87,5% respectivamente. Conclusión: M-CHAT-R/F en su versión chilena resultó fiable, sensible y específico de manera similar al original, lo cual abre la posibilidad de su utilización en población clínica y para investigación. La validación es un proceso continuo que se debe profundizar.

BACKGROUND Leprosy remains a health problem in many countries, with difficulties in diagnosis resulting in delayed treatment and more severe disabilities. Antibodies against several Mycobacterium leprae antigens have, however, shown value as diagnostic and/or prognostic markers. OBJECTIVES The objective of this study was to evaluate serum immunoglobulin (Ig) IgM and IgG subclass reactivity against three M. leprae specific antigens: NDO-HSA, a conjugate formed by natural octyl disaccharide bound to human serum albumin; LID-1, the fusion protein product of the ml0405 and ml2331 genes; and NDO-LID, a combination of LID-1 and NDO. METHODS Sera from healthy controls, paucibacillary (PB) and multibacillary (MB) leprosy patients, and their respective household contacts, were evaluated for the presence of antigen-specific IgM, IgG, and IgG subclass antibodies by enzyme-linked immunosorbent assay (ELISA). The sensitivity and specificity of each ELISA were evaluated by receiver operating characteristic (ROC) curve analysis. FINDINGS Our data confirm that serum IgM antibodies against NDO-HSA and IgG antibodies against LID-1, as well as IgG/M antibodies against NDO-LID, are markedly increased in MB patients. For the first time, our data reveal a selective increase in IgG1 and IgG3 antibodies against LID-1 and NDO-LID in MB patients, demonstrating that these antibody isotypes are suitable for differentiation between MB and PB patients. ROC curve analysis indicates an improved capacity for diagnosing MB leprosy patients using the detection of IgG antibodies, particularly the IgG1 isotype, specific to LID-1 and NDO-LID over the performance levels attained with NDO-HSA. CONCLUSIONS Our findings indicate that serological tests based on the detection of antigen-specific IgG1 antibodies are a useful tool to differentiate MB from PB patients, and indicate the enhanced performance of the LID-1 and NDO-LID antigens in the serodiagnosis of leprosy.

RESUMO: Através de um estudo retrospectivo, 493 gatos testados para FeLV e FIV foram selecionados para análise da associação entre as alterações hematológicas e a positividade no teste imunoenzimático. Variáveis individuais e hematológicas foram consideradas para verificar a influência dos resultados utilizando análise de regressão logística univariada e multivariada. Um total de 153 de 493 gatos avaliados foram positivos para o FeLV (31%), 50 foram positivos para o FIV (10,1%) e 22 foram positivos para FIV e FeLV (4,4%). Análise multivariada detectou uma associação significativa entre a infecção pelo FeLV e a idade abaixo de 1 ano (P=0,01), idade entre 1 a 10 anos (P=0,03) e raça mista (P=0,04). Gatos machos foram mais predispostos a serem positivos para FIV (P=0,002). Com base nas alterações hematológicas, gatos positivos para o FeLV tem maior odds para apresentar anemia, leucopenia e linfopenia que os negativos. Gatos positivos para FIV possuem maiores chances de apresentarem anemia que os gatos negativos. A identificação dos fatores associados à infecção relacionados ao perfil do animal e a correlação com os distúrbios hematológicos com a infecção, pode ser útil para detecção das doenças retrovirais em gatos.

ABSTRACT: Using a retrospective study, 493 cats tested for FeLV and FIV were selected for analysis of the association between hematologic findings and positivity at immunoassay test. Individual and hematologic variables were assessed considering the influence of results using univariate and multivariate logistic regression analysis. Out 153 of the 493 cats were positive for FeLV (31%), 50 were positive for FIV (10.1%) and 22 were positive for both FIV and FeLV (4.4%). Multivariate analysis detected significant associations between FeLV infection and age below 1 year (p=0.01), age from 1 to 10 years (p=0.03), and crossbreed (p=0.04). Male cats were more likely to be FIV-positive (p=0.002). Regarding hematological changes, FeLV-positive cats have higher odds to anemia, leukopenia and lymphopenia than FeLV-negative cats. FIV-positive cats are more likely to have anemia than negative. Identification of associated factors related to animal status and correlation of hematological disorders with infection by retroviruses in cats could be useful for detecting these retroviral diseases in cats.

CONTEXTUALIZAÇÃO: A fadiga é um sintoma comum e inespecífico associado aos problemas crônicos de saúde nos idosos. As alterações e adaptações do processo de envelhecimento associadas à natureza complexa e multidimensional da fadiga favorecem a interação de múltiplos fatores na gênese desse fenômeno. OBJETIVOS: Investigar a associação dos fatores clínicos, funcionais e inflamatórios com a fadiga muscular e a autopercebida em idosas. MÉTODOS: Participaram 135 idosas sedentárias da comunidade, com média de idade de 71,2±4,57. Questionário estruturado e teste funcional foram utilizados para avaliar as características clínicas e funcionais. As concentrações plasmáticas dos mediadores inflamatórios (IL-6 e sTNFR1) foram dosadas pelo método ELISA. A fadiga muscular foi mensurada pelo dinamômetro isocinético, e a fadiga autopercebida, pela Escala Visual Analógica (EVA). A análise estatística foi realizada pela regressão linear múltipla e pelo Coeficiente de Correlação de Spearman, com nível de significância de 5%. RESULTADOS: Os modelos de regressão demonstraram que os fatores idade, índice de massa corporal (IMC), nível de atividade física, capacidade funcional e pico de torque foram associados à fadiga muscular (R²=0,216, p<0,01). A fadiga autopercebida foi associada ao número de comorbidades, estado depressivo, nível de atividade física, capacidade funcional, pico de torque e saúde percebida (R²=0,227, p<0,01). CONCLUSÃO: O presente estudo mostrou uma interação psicofísica da fadiga, identificando os principais fatores associados à fadiga muscular e à autopercebida em idosas. Esses achados demonstram a importância da avaliação e tratamento dos fatores modificáveis tanto na fadiga muscular quanto na autopercebida, buscando um melhor desempenho físico-funcional dos idosos.

BACKGROUND: Fatigue is a common and nonspecific symptom associated with chronic health problems in the elderly. The modifications and adaptations of the aging process associated with complex and multidimensional nature of fatigue favors the interaction of multiple factors in the genesis of this phenomenon. OBJECTIVES: To investigate the association of clinical, functional and inflammatory factors with muscle fatigue and self perceived fatigue in elderly women. METHODS: Participated in the study one hundred and thirty five community elderly women, all sedentary, with a mean age of 71.2±4.57. A structured questionnaire and functional testing were used to evaluate clinical and functional characteristics. Plasma concentrations of inflammatory mediators (IL-6 and sTNFR1) were measured by ELISA method. Muscle fatigue was measured by isokinetic dynamometer and self-perceived fatigue was measured by a visual analog scale. Statistical analysis was performed by multiple linear regression and Spearman correlation coefficient with statistical significance of 5%. RESULTS: The regression models showed that the variables age, body mass index, physical activity level, functional capacity and peak torque were associated with muscle fatigue (R²=0.216, p<0.01). Self-perceived fatigue was associated with number of comorbidities, depression, physical activity level, functional capacity, peak torque and perceived health (R²=0.227, p<0.01). CONCLUSION: This study showed a psychophysical interaction of the fatigue, by identifying the main factors associated with muscle fatigue and self-perceived fatigue in elderly women. These findings demonstrate the importance of evaluation and treatment of modifiable factors in both muscle fatigue and self-perceived fatigue, seeking a better physical and functional performance of elders.

No presente trabalho, um teste imunoenzimático por competição (cELISA) foi padronizado com a proteína recombinante de superfície rMSP5, clonada a partir do gene msp5 do isolado PR1 de A. marginale. O sequenciamento mostrou que o gene que codifica a rMSP5/PR1 tem 98% de identidade com os isolados Flórida e Santa Maria, 97% com isolados de Pernambuco, Brasil e Havana, Cuba e 91% com A. centrale. O teste de cELISA-PR1 foi comparado com os testes de IFI e cELISA-USA. Para a padronização e validação do cELISA-PR1, foram utilizados 380 soros bovinos comprovadamente positivos ou negativos pelo cELISA-USA. Desse total, 245 soros positivos eram de animais de área endêmica e 135 soros eram negativos, de área livre de anaplasmose. Na padronização, foram utilizados 283 soros de bovinos, dos quais 135 eram negativos e 148 positivos. Os testes de cELISA-PR1 e IFI apresentaram especificidade de 100 e 99,3%, sensibilidade de 100 e 98%, com coeficiente kappa de 0,993 e 0,978, respectivamente. Na validação do teste, foram utilizados 245 soros de bovinos de áreas endêmicas para anaplasmose, testados pelo cELISA-PR1 e IFI e apresentaram especificidade de 96,7 e 69,1%, sensibilidade de 98,9 e 96,3% e coeficiente kappa de 0,956 e 0,699, respectivamente. Esses resultados permitem afirmar que o teste de cELISA-PR1 apresentou performance equivalente ao cELISA-USA, podendo também ser utilizado em estudos epidemiológicos, programas de controle e movimentação internacional de animais, enquanto a IFI, com os resultados menos precisos apresentados, não deve ser utilizada em situações que requerem maior rigor no diagnóstico.

A competitive enzyme-linked immunosorbent test using the PR1 recombinant major surface protein 5 (rMSP5-PR1-ELISA) of Anaplasma marginale was standardized and validated using sera from anaplasmosis free and endemic regions. The sequencing of the msp5 gene of PR1 isolate showed 98% of identity with the Florida and Saint Maries isolates, 97% with Brazil (Pernambuco) and Havana isolates; and 91% with A. centrale. The cELISA-PR1 test was compared to IFI and cELISA-USA. For the standardization and validation of the cELISA-PR1, 380 bovine sera were used, whereas 245 truly positives and 135 truly negatives sera tested by the cELISA-USA. In the standardization of the cELISA-PR1 135 negative and 148 positive bovine sera were used. The cELISA-PR1 and IFI tests showed 100 and 99.3% specificity, 100 and 98%, sensibility, and a kappa coefficient of 0.993 and 0.978, respectively. For test validation, 245 bovine sera from an anaplasmosis endemic area were analyzed by the cELISA-PR1 and IFI, which showed 96.7 and 69.1% specificity, 98.9 e 96.3% sensibility and kappa coefficient of 0.956 and 0.699, respectively. These results indicate that the cELISA-PR1, likewise the cELISA-USA, could sensitively and specifically detect cattle naturally infected with A. marginale and would be recommended for epidemiological studies, eradications program, and regulation of international cattle movement, while IFI, which presented lower specificity should not be used in situations that demand more specific diagnosis.

Os pacientes com lúpus eritematoso sistêmico (LES) apresentam fatores de risco para a aquisição de hepatite C, como hospitalizações e hemotransfusões, e compartilham com os pacientes infectados pelo vírus da hepatite C (HCV) várias manifestações clínico-laboratoriais relacionadas a auto-imunidade. OBJETIVOS: O objetivo do presente estudo é determinar a prevalência de hepatite C em uma população com LES acompanhada no ambulatório de programas especiais - Colagenoses - do Hospital Universitário Clementino Fraga Filho (HUCFF-UFRJ). MÉTODOS: Foi realizado um estudo de corte-transversal, incluindo os primeiros 91 pacientes com o diagnóstico de LES provável ou definitivo procedentes do referido ambulatório, no período de abril a setembro de 1997. Todos os doentes foram avaliados, sob o ponto de vista clínico e laboratorial, para determinação de atividade de doença lúpica. Foram realizados o teste de triagem UBI HCV EIA 4.0 e o teste suplementar INNO-LIA HCV AbIII nos 91 pacientes selecionados e , em 32 pacientes escolhidos ao acaso e nos pacientes reativos aos testes imunossorológicos foi realizada a pesquisa do RNA-HCV pelo método da reação de polimerização em cadeia (PCR). RESULTADOS: Observou-se um aumento significativo da prevalência de anti-HCV na amostra, quando comparada à população de doadores de sangue do HUCFF (6,6% versus 1,39% com o teste EIA, valor p=0,02 e intervalo de confiança 95%=5,5 a 13,8), contudo , considerando-se resultados obtidos com os testes confirmatórios imunoblot e PCR aplicados em série ( prevalência de HCV de 2,2%, com intervalo de confiança 95%=0,2 a 7,7) , a prevalência de HCV em pacientes com LES não parece mais alta que em doadores de sangue. CONCLUSÕES: A prevalência de hepatite C na amostra estudada utilizando-se um teste de triagem (EIA) foi significativamente maior do que a de uma população de doadores de sangue (6,6% versus 1,39%). Porém, aplicando-se em série os testes confirmatórios INNO-LIA<FONT FACE=Symbol>â</FONT> e PCR ( prevalência de hepatite C de 2,2%) parece não haver evidências de que pacientes com LES tenham maior risco de adquirir o HCV do que a população em geral, embora a prevalência tende a ser maior que a população de doadores de sangue. Em pacientes com diagnóstico provável de LES deve-se avaliar o custo/benefício de se investigar HCV e crioglobulinemia mista essencial (CME).

BACKGROUND: Patients with systemic lupus erythematosus (SLE) are at risk for the acquisition of hepatitis C, owing to frequent hospitalization and hemotransfusion. Many clinical and laboratory manifestations related to auto-imunity are shared between Lupus and HCV infected patients. OBJECTIVE: To determine the prevalence of hepatitis C in the population with SLE monitored in the out-patient service of the special programs in - Collagenoses - of the University Hospital Clementino Fraga Filho (HUCFF-UFRJ). METHODS: A Cross sectional study was performed, including 91 patients with the diagnosis of probable or definitive SLE (with 3 or more of the 11 diagnostic criteria for SLE used by the American College of Rheumatology - ACR) proceeding from the mentioned center between April / 97 to September / 97. Patients were evaluated on clinical and laboratory basis to determine activity of lupus disease (SLEDAI). All of patients were tested with UBI HCV EIA 4.0 assay and INNO-LIA HCV AbIII confirmatory assay. PCR was performed on all EIA positive INNO-LIA HCV AbIII assay positive patients besides 32 patients negatives in both serological assays. RESULTS: A significative increase in HCV prevalence among SLE patients was observed when compared with a control group of blood donors at HUCFF-UFRJ (6.6 and 1.39%, respectively; p=0,02 and confidence interval of 95%=5.5-13.8 ). However, if confirmative assays (INNO-LIA HCV ABIII and PCR ) are used, this difference can not be observed any more HCV prevalence of 2.2% with confidence interval of 95% = 0.2-7.7 ). CONCLUSIONS: HCV prevalence using ELISA assay for screening showed a significant difference between SLE patients and a blood donors control group ( 6.6 and 1.39% respectively). However, if confirmatory assays are subsequently applied, no difference in HCV prevalence among both groups can be observed. Patients with a possible diagnosis of SLE should have a cost/benefit analisys of testing for HCV.
