As lentes de contato terapêuticas são úteis para o tratamento de uma série de doenças da superfície ocular. Suas principais finalidades são: reduzir a dor; proteger a córnea; facilitar e manter a cicatrização epitelial; restaurar a câmara anterior e liberar medicamentos na superfície ocular. Há uma variedade de tipos e materiais, sendo que a escolha depende da doença a ser tratada, tempo de uso e necessidades fisiológicas do olho doente. Doenças como a ceratopatia bolhosa, erosão recorrente do epitélio corneano, olho seco e defeitos epiteliais pós-operatórios, além de várias outras doenças que afetam a superfície ocular podem ser tratadas com a utilização das lentes de contato terapêuticas. Elas não devem ser utilizadas em processos infecciosos ou em pacientes que estão impossibilitados de retornar para acompanhamento regular. A adaptação, controle e manutenção adequados das lentes de contato terapêuticas são fundamentais para prevenir complicações, como: neovascularização corneana, conjuntivite papilar gigante ou ceratite infecciosa. O controle e manutenção foram facilitados com o advento das lentes de contato terapêuticas de silicone-hidrogel. Essas lentes permitem trocas menos freqüentes devido a sua maior oxigenação e, em casos selecionados, aceleram a reepitelização corneana.

Therapeutic contact lenses are useful in a variety of ocular surface diseases. Their main indications are: to relieve the pain; protect ocular surface; promote corneal healing and epithelial regeneration; seal a leaking corneal wound and deliver ophthalmic drugs on the ocular surface. There are several kinds of lens designs and materials, and their choice is dependent on the specific disease to be treated, the duration of treatment and the physiologic needs of the diseased cornea. Bullous keratopathy, recurrent epithelial erosion syndrome, dry eye and postoperative epithelial defects are amongst their indications. Therapeutic contact lenses should not be indicated in the presence of active infectious keratitis or when the patient is not compliant. Corneal neovascularization, giant papillary conjunctivitis and infectious keratitis are serious complications, which can be prevented by correctly fitting and maintaining the therapeutic contact lenses. Silicon-hydrogel therapeutic contact lenses, due to their higher oxygen permeability, allow extended wear schedules, decreasing the need for frequent lens replacement.

OBJETIVO: Verificar a existência de associações entre grau evolutivo e padrão topográfico do ceratocone com as lentes de contato (LC) adaptadas. MÉTODOS: Estudo retrospectivo do tratamento de 454 pacientes (881 olhos) com ceratocone, 746 adaptados com lentes de contato, de julho de 1996 a junho de 2000. Classificou-se o ceratocone segundo grau evolutivo e padrão topográfico. A primeira lente de contato testada foi rígida gás-permeável (RGP) monocurva. Quando não se alcançou a adaptação desejada, testou-se as rígidas gás-permeáveis bicurvas e outros desenhos, procurando-se relação lente-córnea com livramento apical ou discreto toque apical. Em caso de insucesso com as rígidas foram testadas as lentes de contato gelatinosas, esféricas, tóricas e com desenhos especiais. RESULTADOS: Para 15 (3,3%) pacientes foram receitados óculos; 111 olhos (12,6%) permaneceram sem correção; 746 (84,68%) olhos foram adaptados com LC; 39 olhos (4,44%) de 39 pacientes (8,6%) foram encaminhados para transplante de córnea. O grau evolutivo mais encontrado foi o avançado (575 olhos - 66,86%), seguido do incipiente (160 olhos - 18,6%). O valor ceratométrico médio do meridiano mais plano (K) foi de 47,80 (±3,73) para os olhos adaptados com lentes de contato gelatinosas; de 49,03 (±3,7) para as lentes de contato rígidas gás-permeáveis monocurvas; de 51,62 (±3,6) para as lentes de contato bicurvas e de 51,14 (±3,5) para o sistema "piggyback". Quanto à classificação topográfica, o mais freqüente foi o periférico inferior (436 olhos - 50,7%), seguido pelo central assimétrico (348 olhos - 40,46%). No olho direito, a análise das variáveis padrão topográfico e tipo de lente adaptada demonstrou associação estatisticamente significante (p=0,045) de lentes de contato gelatinosas tóricas e sistema "piggyback" com percentual maior de adaptações nos ceratocones centrais. Com as lentes de contato rígidas gás-permeáveis monocurvas e bicurvas houve percentual semelhante de adaptações em relação aos padrões topográficos encontrados. No olho esquerdo não se observou associação estatisticamente significante, apenas tendências. CONCLUSÕES: A maioria dos ceratocones pode ser adaptada com lentes de contato rígidas monocurvas, mas é importante contar com outros desenhos para adaptar os cones de graus avançados e severos. Isso permite, para a maior parte dos pacientes, postergar a necessidade do transplante de córnea. As lentes de contato gelatinosas tóricas e o sistema "piggyback" foram mais adaptados nos cones centrais; enquanto nos cones periféricos inferiores, foram as lentes de contato monocurvas e as bicurvas.

PURPOSE: To determine associations between degrees of severity and topographical patterns of keratoconus with fitted contact lenses (CL). METHODS: Retrospective study of 454 patients (881 eyes) with keratoconus, 746 fitted with contact lenses, from July 1996 to June 2000. Keratoconus was classified according to degrees of severity and topographical pattern. The first tested contact lens was the spherical rigid gas permeable. When it did not achieve the desired fitting, the bicurve rigid gas permeable and other designs were tried, in search of an apical clearance or slight apical touch fitting relationship. RESULTS: For 15 (3.3%) patients glasses were prescribed; 111 eyes (12.6%) remained uncorrected; 746 (84.68%) eyes were fitted with contact lenses; for 39 eyes (4.44%) of 39 patients (8.6%) penetrating keratoplasty was recommended. The most common degree of severity found was advanced (575 eyes - 66.86%), followed by the mild variety (160 eyes -18.6%). The average keratometric value of the flattest meridian (K) was 47.80 (±3.73) for the eyes fitted with soft CL; 49.03 (±3.7) for the spherical rigid gas permeable contact lenses; of 51.62 (±3.6) for the bicurve contact lenses and 51.14 (±3.5) for the piggyback system. Concerning the topographical pattern, the most frequent was inferior peripheral cone (436 eyes - 50.7%), followed by central asymmetrical cone (348 eyes - 40.46%). In the right eye, the analysis of the variables, topographical pattern and kind of fitted lens showed a statistically significant association (p=0.045) of toric soft contact lenses and piggyback system with a higher percentage of fittings in central cone. Concerning the rigid spherical and bicurve lenses, the fitting percentage was similar in relation to the found topographical pattern. In the left eye, statistically significant associations were not observed, only tendencies. CONCLUSION: Spherical contact lenses can be fitted to the majority of patients with keratoconus, but it is important to rely on other designs to fit the advanced and severe cases. This allows most patients to postpone the need for corneal transplant. Toric soft contact lenses and piggyback system were more frequently fitted to central cones; whereas in inferior peripheral cones, spherical and bicurve contact lenses were those more used.

Objetivo: Estudar o comportamento e características de usuários de lentes de contato ligados à área de saúde. Método: Realizou-se um levantamento entre universitários da área de saúde da Pontifícia Universidade Católica do Paraná, em Curitiba, no período de 23 a 27 de novembro de 1998. Para a coleta de dados foi utilizado um questionário auto-aplicável, previamente testado. Resultados e Discussão: Dentre 1.173 estudantes, 207 (17,2%) usavam lentes de contato, sendo 78,3% do sexo feminino e 21,7% do sexo masculino. Eram usuários de lentes rígidas 12,1% e 87,9% utilizavam hidrofílicas, demonstrando um aumento progressivo das últimas quando se compara a estudos nacionais anteriores. Dos usuários de lentes de contato hidrofílicas, 57,6% utilizavam descartáveis e desses 88,5% não as utilizavam de acordo com os padrões de uso e descartabilidade estabelecidos. Do número total de usuários, 37,7% relataram algum problema ocular durante o uso. Embora 97,1% tenham feito a adaptação com o oftalmologista, 14,2% dos usuários de descartáveis não obtiveram novas lentes em clínicas oftalmológicas. Por outro lado, 81,1% procuraram o oftalmologista, semestral ou anualmente, para a revisão da adaptação. Recomendação: Para reduzir o número de complicações e a desistência do uso de lentes de contato, o usuário deve ser educado sobre a forma correta de utilizá-las, ser orientado sobre os sinais e sintomas de alerta para problemas oculares e receber as informações também por escrito.

Purpose: To study the behavior and characteristics of contact lens wearers involved in health areas. Methods: To conduct a survey among students of health related courses at the Pontifícia Universidade Católica do Paraná, in Curitiba, during the period from 23rd to 27th November 1998. A previously tested, self-applicable questionnaire was used to collect data. Results and Discussion: Among 1,173 students, 207 (17.2%) used contact lenses, of which 78.3% were female and 21.7% were male. Twelve point one percent (12.1%) used rigid and 87.9% soft lenses, showing a progressive increase in the use of soft lenses when compared to previous national studies. Of the soft lens wearers, 57.6% used disposable lenses and of these, 88.5% did not follow the established standards for use and disposability. Of the total number of wearers, 37.7% reported ocular problems during contact lens use. Although 97.1% of the individuals of this study were fitted for contact lenses by an ophthalmologist, 14.2% obtained new lenses outside ophthal-mologic clinics. On the other hand, 81% consulted an oph-thalmologist one or twice yearly to check lens fitting. Recommendation: To reduce the number of complications and the discontinuance of wearing lenses, the user must be properly educated in the use of lenses and how to identify signs and symptoms indicating ocular problems as well as having this information in writing.
