Abstract Numerical and experimental analysis of visible light communication (VLC) systems based on orthogonal chirp division multiplexing (OCDM) and orthogonal frequency division multiplexing (OFDM) are presented in this paper. Unlike most of the related publications, in this work we numerically compared OCDM and OFDM based VLC systems with the same equalization process, i.e., employing an one tap equalizer (OTE) in both systems. Simulation results in line-of-sight VLC channels show that both systems have the same performance when baseband multicarrier signals are used. The impact of the optical modulation index and the bias current of a previously characterized light-emitting diode were also evaluated. Nevertheless, the experimental demonstrations, in which bandpass signals were used due to the bandpass characteristic of the exploited setup, show that OFDM-based system outperforms the VLC links with OCDM signals, when the OTE equalizer is used in both systems. Abstract Background Most of the evidence about the impact of the post-acute COVID-19 Syndrome (PACS) reports individual symptoms without correlations with related imaging. Objectives To evaluate cardiopulmonary symptoms, their predictors and related images in COVID-19 patients discharged from hospital. Methods Consecutive patients who survived COVID-19 were contacted 90 days after discharge. The Clinic Outcome Team structured a questionnaire evaluating symptoms and clinical status (blinded for hospitalization data). A multivariate analysis was performed to address the course of COVID-19, comorbidities, anxiety, depression, and post-traumatic stress during hospitalization, and cardiac rehabilitation after discharge. The significance level was set at 5%. Results A total of 480 discharged patients with COVID-19 (age: 59±14 years, 67.5% males) were included; 22.3% required mechanical ventilation. The prevalence of patients with PACS-related cardiopulmonary symptoms (dyspnea, tiredness/fatigue, cough, and chest discomfort) was 16.3%. Several parameters of chest computed tomography and echocardiogram were similar in patients with and without cardiopulmonary symptoms. The multivariate analysis showed that PACS-related cardiopulmonary-symptoms were independently related to female sex (OR 3.023; 95% CI 1.319-6.929), in-hospital deep venous thrombosis (OR 13.689; 95% CI 1.069-175.304), elevated troponin I (OR 1.355; 95% CI 1.048-1.751) and C-reactive protein during hospitalization (OR 1.060; 95% CI 1.023-1.097) and depression (OR 6.110; 95% CI 2.254-16.558). Conclusion PACS-related cardiopulmonary symptoms 90 days post-discharge are common and multifactorial. Beyond thrombotic and markers of inflammation/myocardial injury during hospitalization, female sex and depression were independently associated with cardiopulmonary-related PACS. These results highlighted the need for a multifaceted approach targeting susceptible patients.
Resumo Fundamento A maioria da evidência sobre o impacto da síndrome COVID pós-aguda (PACS, do inglês, post-acute COVID-19 syndrome) descreve sintomas individuais sem correlacioná-los com exames de imagens. Objetivos Avaliar sintomas cardiopulmonares, seus preditores e imagens relacionadas em pacientes com COVID-19 após alta hospitalar. Métodos Pacientes consecutivos, que sobreviveram à COVID-19, foram contatados 90 dias após a alta hospitalar. A equipe de desfechos clínicos (cega quanto aos dados durante a internação) elaborou um questionário estruturado avaliando sintomas e estado clínico. Uma análise multivariada foi realizada abordando a evolução da COVID-19, comorbidades, ansiedade, depressão, e estresse pós-traumático durante a internação, e reabilitação cardíaca após a alta. O nível de significância usado nas análises foi de 5%. Resultados Foram incluídos 480 pacientes (idade 59±14 anos, 67,5% do sexo masculino) que receberam alta hospitalar por COVID-19; 22,3% necessitaram de ventilação mecânica. A prevalência de pacientes com sintomas cardiopulmonares relacionados à PACS (dispneia, cansaço/fadiga, tosse e desconforto no peito) foi de 16,3%. Vários parâmetros de tomografia computadorizada do tórax e de ecocardiograma foram similares entre os pacientes com e sem sintomas cardiopulmonares. A análise multivariada mostrou que sintomas cardiopulmonares foram relacionados de maneira independente com sexo feminino (OR 3,023; IC95% 1,319-6,929), trombose venosa profunda durante a internação (OR 13,689; IC95% 1,069-175,304), nível elevado de troponina (OR 1,355; IC95% 1,048-1,751) e de proteína C reativa durante a internação (OR 1,060; IC95% 1,023-1,097) e depressão (OR 6,110; IC95% 2,254-16,558). Conclusão Os sintomas cardiopulmonares relacionados à PACS 90 dias após a alta hospitalar são comuns e multifatoriais. Além dos marcadores trombóticos, inflamatórios e de lesão miocárdica durante a internação, sexo feminino e depressão foram associados independentemente com sintomas cardiopulmonares relacionados à PACS. Esses resultados destacaram a necessidade de uma abordagem multifacetada direcionada a pacientes susceptíveis.

OBJECTIVE: Coronavirus disease 2019 (COVID-19) is associated with high mortality among hospitalized patients and incurs high costs. Severe acute respiratory syndrome coronavirus 2 infection can trigger both inflammatory and thrombotic processes, and these complications can lead to a poorer prognosis. This study aimed to evaluate the association and temporal trends of D-dimer and C-reactive protein (CRP) levels with the incidence of venous thromboembolism (VTE), hospital mortality, and costs among inpatients with COVID-19. METHODS: Data were extracted from electronic patient records and laboratory databases. Crude and adjusted associations for age, sex, number of comorbidities, Sequential Organ Failure Assessment score at admission, and D-dimer or CRP logistic regression models were used to evaluate associations. RESULTS: Between March and June 2020, COVID-19 was documented in 3,254 inpatients. The D-dimer level ≥4,000 ng/mL fibrinogen equivalent unit (FEU) mortality odds ratio (OR) was 4.48 (adjusted OR: 1.97). The CRP level ≥220 mg/dL OR for death was 7.73 (adjusted OR: 3.93). The D-dimer level ≥4,000 ng/mL FEU VTE OR was 3.96 (adjusted OR: 3.26). The CRP level ≥220 mg/dL OR for VTE was 2.71 (adjusted OR: 1.92). All these analyses were statistically significant (p<0.001). Stratified hospital costs demonstrated a dose-response pattern. Adjusted D-dimer and CRP levels were associated with higher mortality and doubled hospital costs. In the first week, elevated D-dimer levels predicted VTE occurrence and systemic inflammatory harm, while CRP was a hospital mortality predictor. CONCLUSION: D-dimer and CRP levels were associated with higher hospital mortality and a higher incidence of VTE. D-dimer was more strongly associated with VTE, although its discriminative ability was poor, while CRP was a stronger predictor of hospital mortality. Their use outside the usual indications should not be modified and should be discouraged.

This study provides a risk assessment and risk maps related to the consumption of water contaminated by Al, Ba, Fe and Pb in an industrial area in the Brazilian Amazon. A total of 120 samples of drinking water were collected from 26 locations in the municipality of Barcarena, Pará State. Multiple elements were analyzed by inductively coupled plasma optical emission spectrometry. The quantifiable elements in the samples were Al, Ba, Fe and Pb. Risk assessment was performed according to U.S. Environmental Protection Agency (USEPA) procedures. Results indicate that the highest potential risk of non-carcinogenic adverse health effects for Al was in São João Island; for Ba, Fe and Pb (hazard quotient (HQ) > 1) were in Porto da Balsa community, in the city of Barcarena and Distrito Industrial community, respectively. Maps showed that areas located near Barcarena’s industrial complex are the most affected by water contamination. Therefore, these populations are at higher risk of non-carcinogenic problems, especially children and the elderly, since the majority of the population resides in these areas. Geospatial analysis contributed to delimiting and analyzing risk-change trends in the region, expanding the scope of results to a decision-making process.

RESUMO: Os gafanhotos das famílias Acrididae e Romaleidae (Orthoptera) estão entre os insetos desfolhadores de helicônias, e vem ganhando status de pragas em cultivos comerciais dessas flores no Brasil. O presente estudo objetivou identificar o gafanhoto de ocorrência no município de Santo Antônio de Pádua, RJ (Brasil) causador de desfolhamento em helicônias; avaliar o efeito de diferentes níveis de sombreamento sobre a população desse gafanhoto e parâmetros produtivos de helicônias; e se existe preferência do gafanhoto para oviposição entre as helicônias avaliadas. O experimento foi em delineamento em blocos ao acaso, em esquema de parcelas subdivididas (quatro níveis de sombreamento: 0%, 30%, 50% e 80% na parcela e quatro espécies de Heliconia: H. psittacorum, H. stricta, H. wagneriana e H. psittacorum x H. spathocircinata ‘Golden Torch’ na subparcela), com quatro repetições. O gafanhoto desfolhador foi identificado como Cornops frenatum frenatum (Acrididae). À medida que o sombreamento se tornou mais denso, diminuíram o número de orifício de oviposição do gafanhoto e o número de brotações laterais. O sombreamento não influenciou o número de ninfas e adultos de C. f. frenatum e o número de hastes florais. H. wagneriana foi a espécie mais preferida para oviposição por C. f. frenatum. Os resultados sugerem que o uso de tela para sombreamento das plantas de helicônia pode auxiliar no controle da população de C. f. frenatum, porém, deve-se observar o padrão de exigência de luminosidade das helicônias para garantir a produtividade.
ABSTRACT: Grasshoppers of the families Acrididae and Romaleidae (Orthoptera) are among the insects that defoliate heliconias and have been gaining status as pests of commercial crops of these plants in Brazil. The objectives of the present study were to identify the grasshopper defoliating heliconias in the municipality of Santo Antônio de Pádua, RJ (Brazil), to evaluate the effect of different levels of shade on the population of this grasshopper and the production parameters of heliconias, and to determine if this grasshopper has an oviposition preference among the heliconias evaluated. The experiment was in a completely randomized block design, in subdivided plots (four levels of shade in the plot, 0%, 30%, 50% and 80%, and four species of Heliconia: H. psittacorum, H. stricta, H. wagneriana and H. psittacorum x H. spathocircinata ‘Golden Torch’ in the subplot), with four replications. The grasshopper was identified as Cornops frenatum frenatum (Acrididae). An increase in shade resulted in a decrease in the number of oviposition holes from the grasshopper and the number of lateral buds. Shade did not influence the number of C. f. frenatum nymphs and adults and the number of flower stems. H. wagneriana was the most preferred species for oviposition by C. f. frenatum. Results suggested using screens to shade heliconia plants can help control C. f. frenatum populations, however, the light requirements of the heliconias should be considered to guarantee productivity.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Neste trabalho são descritos os sinais clínicos, patologia clínica e patologia de 24 bovinos com leucose bovina enzoótica atendidos na Clínica de Bovinos de Garanhuns da Universidade Federal Rural de Pernambuco. Esses casos representaram 0,5% de 4.758 bovinos atendidos entre os anos de 2000 e 2010. A doença afetou 22 (91,7%) fêmeas e dois machos. Vinte e um animais (87,5%) eram de raças leiteras (seis Holandês, 13 Girolando, um Jersey e um Pardo Suíça) e três (12,5%) eram da raça Nelore. Vinte e três animais (95,8%) tinham idade entre 3 e 8 anos e um era mais jovem. Todos eram criados em regime de confinamento ou semi-confinamento. Clinicamente todos os animais apresentaram aumento dos linfonodos superficias. Outros sinais frequentes foram hiporexia, diminuição da produção de leite, emagrecimento progressivo, escore corporal baixo, desidratação, hipomotilidade dos pré-estômagos e fezes alteradas e em pouca quantidade. Com menor frequência foram observados exoftalmia, dispneia e aumento de volume do útero. No leucograma foi constatada leucocitose média de 34.082/µL, com linfocitose de 21.814/µL e neutrofilia de 10.906/µL. Treze animais foram necropsiados e os demais foram enviados pelos proprietários para o abate. Dos treze animais abatidos todos apresentaram lesões nos linfonodos superficiais, seis nos linfonodos mesentéricos, seis no intestino, três no abomaso, um no coração, um no fígado, um no rúmen, um no útero e um no rim. Diante da importância desta enfermidade e dos prejuízos causados pela mesma é necessário alertar produtores sobre os cuidados a serem tomados durante a aquisição de animais, assim como da necessidade de implantar medidas que evitem a difusão da doença entre as fazendas.
The article reports epidemiological data, clinical signs, and laboratory and pathological findings in 24 cattle with enzootic bovine leukosis observed in the Clinic of Garanhuns, at the Federal Rural University of Pernambuco. The 24 cases represented 0.5% of 4,758 cattle examined from 2000 to 2010. The disease affected 22 (91.7%) females and two males. Twenty one of the animals were dairy (six Holstein, 13 girolando, one Brown Swiss and one Jersey), and three were for meat production (Nelore). Twenty three animals were 3-8 years of age and one was younger. All were raised in confinement or semi-confinement. All animals showed enlarged superficial lymph nodes. Other frequent clinical signs were hyporexia, decreased milk production, progressive weight loss, dehydration, hypomotility of the fore stomachs, and altered scant feces. Exophthalmia, dyspnea, and enlarged uterus were observed with less frequency. Leukocytosis (mean of 34,082 leukocytes/µL) with lymphocytosis (21,814 lymphocytes/µL) and neutrophilia (10,906lymphocytes/µL) was observed in the white blood count. Thirteen bovines were necropsied and 11 were slaughtered. Gross lesions were observed on the superficial lymph nodes of all animals necropsied. Six had lesions in the mesenteric lymph nodes, six in the gut, three in the abomasum, two in the uterus, one in the heart, one in the rumen, one in heart, and one in the liver. Due to the importance of enzootic bovine leukosis it is necessary for the farmers to introduce animals free of the disease and to establish a strict health policy for its control.

OBJETIVO: Compilar e debater conhecimentos sobre os rótulos, as embalagens e a divulgação e promoção de alimentos processados que progressivamente passam a compor a dieta da população infanto-juvenil. FONTES DE DADOS: Artigos nos bancos de dados eletrônicos, Medline e SciELO nos últimos dez anos, nas línguas portuguesa e inglesa, utilizando os descritores "criança", "marketing", "hábitos alimentares", "televisão", "educação em Saúde". Também foram consultados livros, textos recentes e artigos considerados relevantes para realização dessa revisão. SÍNTESE DOS DADOS: O artigo discute o marketing de alimentos, segundo os tópicos: propagandas, embalagens e rótulos. Quanto à publicidade televisiva, foi abordada sua influência na formação dos hábitos alimentares infanto-juvenis, a qualidade nutricional dos alimentos veiculados e as alternativas para que pais e educadores possam lidar com os novos padrões de consumo. Além disso, debateram-se as embalagens enquanto meios de comunicação entre fabricante e consumidor. Sobre os rótulos, foram abordadas as novas regras da Agência Nacional de Vigilância Sanitária a respeito da Rotulagem Nutricional Obrigatória, bem como a importância da leitura adequada para a adoção de escolhas alimentares saudáveis. CONCLUSÕES: Espera-se que este artigo seja instrumento de atualização dos profissionais da Saúde, proporcionando atuação mais incisiva no processo de educação em saúde e nutrição. Torna-se urgente a adoção de práticas preventivas com esclarecimento aos pais e familiares cujos filhos participam ativamente da sociedade de consumo, visando minimizar os efeitos deletérios da ingestão rotineira de alimentos obesogênicos.
OBJECTIVE: To debate knowledge on labeling, packaging, releases and promotion of processed foods that have been included progressively in the diets of children and adolescents. DATA SOURCE: On-line articles from Medline and SciELO database published over the last ten years in Portuguese and English using the keywords "child", "marketing", "eating habits", "television", "health education". Also, books, recent texts and articles considered relevant for the present review were included. DATA SYNTHESIS: The present article discusses the food marketing according to propaganda, packaging and labeling. This review also approaches the influence of TV advertising on building of children and adolescents' eating habits; the nutrition quality of advertised foods and alternatives that parents and teachers could use to deal with these new patterns of consumption. Additionally, it is discussed packaging as a communication tool between manufacturer and consumer. Regarding labeling, the "Agência Nacional de Vigilância Sanitária" (Anvisa) new rules on Regulatory Nutritional Labeling as well as the importance of proper label reading for healthy choices are discussed. CONCLUSIONS: The present article is intended to be a tool for updating health professionals and encouraging them to act more effectively in health education and nutrition. It is urgent to adopt prevention practices enlightening parents and relatives whose children actively participate in the consumption society in order to minimize the harmful effects of the obesogenic foods routine ingestion.

A syndrome de Pendred (SP) é uma doença autossômica recessiva caracterizada por surdez neurossensorial, bócio e defeito de organificação do iodo. A perda auditiva está associada a anormalidades do ouvido interno, desde a dilatação isolada do aqueduto vestibular (DAV) até uma típica displasia coclear. Mutações no gene que codifica a pendrina (SLC26A4), um transportador de cloreto/iodeto, têm sido associadas à SP. Descrevemos as características clínicas e moleculares de uma grande família consangüínea portadora de uma mutação no gene SLC26A4. O caso-índice era uma paciente do sexo feminino, brasileira, 26 anos, portadora de surdez congênita, que apresentava um volumoso bócio multinodular e hipotireoidismo desde a puberdade. Outros cinco irmãos eram surdos: um irmão tinha fenotipo semelhante, três também tinham bócio, porém com função tiroideana normal e um irmão tinha apenas um discreto aumento da tiróide. Outros quatro irmãos não apresentavam alteração tiroideana ou auditiva. Os pais eram primos de primeiro grau e tinham audição normal. A mãe era saudável, exceto por hipotireoidismo subclínico; o pai era falecido. O teste do perclorato no caso-índice revelou a liberação de 21% do iodo incorporado duas horas após a administração de 1 g de KClO4. Os exames audiológicos mostraram perda auditiva profunda em todos os indivíduos afetados; TC e RMN dos ossos temporais mostraram DAV em todos eles. O DNA genômico foi isolado do sangue total dos seis irmãos afetados e dos quatro não-afetados, da mãe e do controle. A região codificante do gene PDS (éxons 2-21), incluindo as junções éxon/íntron, foram amplificadas por PCR e seqüenciadas. Foi detectada a deleção de uma base (T) na posição 1197 do éxon 10, em homozigoze, nos seis irmãos afetados. A mãe e dois irmãos não-afetados eram heterozigotos para a mutação, que foi descrita inicialmente por Everett e cols. A mutação 1197delT provavelmente resulta em um erro de fase de leitura (frameshift) e em uma proteína truncada. A existência de fenocópias da SP e a variabilidade fenotípica intrafamiliar são bem conhecidas. O diagnóstico definitivo requer análise molecular. O presente estudo ilustra o valor e os desafios da análise mutacional em pacientes selecionados com SP.
Pendred Syndrome (PS) is an autossomal recessive disorder characterized by sensorineural deafness, goiter and iodide organification defect. The hearing loss is associated with inner ear abnormalities, ranging from an isolated enlarged vestibular aqueduct (EVA) to a typical coclear dysplasia. Mutations in the gene that encodes pendrin (SLC26A4), a chloride/iodide transporter, have been shown to be associated with PS. We describe the clinical and molecular characteristics of a large consanguineous family harboring a mutation in the SLC26A4 gene. The proband was a 26-year-old deaf Brazilian woman who presented a bulky multinodular goiter and hypothyroidism since puberty. Five other siblings were deaf: one brother had a similar phenotype, three siblings also had goiters but normal thyroid function tests, and one brother had only a subtle thyroid enlargement. Other 4 siblings had no thyroid or hearing disorder. Parents were first degree cousins and had normal hearing. The mother was healthy, except for subclinical hypothyroidism; the father was deceased. A perchlorate test in the proband showed a discharge of 21% of the incorporated iodide 2h after the administration of 1g of KClO4. Audiological examinations showed profound hearing loss in all deaf subjects; CT and MRI of the temporal bones showed EVA in all of them. Genomic DNA was isolated from whole blood, from the 6 affected and 4 unaffected siblings, the mother and control. The coding region of the PDS gene (exons 2-21), including exon/intron boundaries, were amplified by PCR and sequenced. A single base-pair (T) deletion at position 1197 of exon 10 was detected in homozygous state in the 6 deaf siblings. The mother and 2 unaffected siblings were heterozygous for this mutation, which has been described by Everett et al. The 1197delT mutation is predicted to result in a frameshift and a truncated protein. The existence of PS phenocopies and intrafamilial phenotypic variability are well documented. The definite diagnosis requires molecular analysis. Our study illustrates the value and challenges of mutational analysis in selected patients with PS.

OBJETIVO: Avaliar o preenchimento dos dados sobre gravidez, parto e recém-nascido na Caderneta de Saúde da Criança (CSC) e conhecer a percepção das mães sobre a função deste instrumento em Belo Horizonte, Minas Gerais. MÉTODOS: Estudo transversal descritivo de 797 CSC de crianças nascidas entre abril e dezembro de 2005 e entregues às mães na maternidade. Foi utilizado protocolo para avaliar as primeiras páginas da CSC por observação direta e feitas duas perguntas às mães. O preenchimento de cada item foi categorizado como presente, ausente ou incorreto. RESULTADOS: O nome da criança não estava registrado em 44% das CSC. O início do pré-natal estava sem preenchimento em 40% e o número de consultas, em 31%. O tipo de parto estava em branco em 15% das CSC; a idade gestacional, em 24%; e o Apgar de cinco minutos, em 23% dos casos. Peso ao nascer, comprimento e perímetro cefálico não foram informados em 9, 10 e 15%, respectivamente. O campo destinado à orientação para levar o neonato à Unidade Básica de Saúde (UBS) no quinto dia de vida não foi preenchido em 75%. Apenas 33% das mães receberam explicações sobre a CSC na maternidade. Para 313 mães, a CSC estava relacionada ao acompanhamento do crescimento e/ou desenvolvimento do seu filho. CONCLUSÕES: O estudo mostrou falhas no preenchimento da CSC, sugerindo necessidade de esforços para que ela cumpra seu papel na promoção da saúde infantil.

A written protocol was used to assess the first pages of the CHR and mothers were asked two questions. Each CHR item was classified as correctly, incorrectly or not filled in. RESULTS: The name of the child was not in the CHR in 44%. The first prenatal visit and the number of prenatal visits were absent in respectively 40 and 31% of the records. The type of birth was not recorded in 15% of the CHR, as well as the gestational age in 24% and the Apgar score at five minutes in 23%. The newborns weight, length and head circumference were not informed in 9, 10 and 15%, respectively. The slot with the orientation to take the newborn to the first pediatric visit on the 5th day of life was blank in 75%. Only 32% of the mothers received information about the CHR at the maternity ward. To 313 mothers, the CHR was important to record follow-up of growth and/or development of their child. CONCLUSIONS: An important lack of information was noted in the CHR. Efforts should be directed to improve the quality of information in order to enhance children's health promotion.

Cerebral aspergillosis is a rare cause of brain expansive lesion in AIDS patients. We report the first culture-proven case of brain abscess due to Aspergillus fumigatus in a Brazilian AIDS patient. The patient, a 26 year-old male with human immunodeficiency virus (HIV) infection and history of pulmonary tuberculosis and cerebral toxoplasmosis, had fever, cough, dyspnea, and two episodes of seizures. The brain computerized tomography (CT) showed a bi-parietal and parasagittal hypodense lesion with peripheral enhancement, and significant mass effect. There was started anti-Toxoplasma treatment. Three weeks later, the patient presented mental confusion, and a new brain CT evidenced increase in the lesion. He underwent brain biopsy, draining 10 mL of purulent material. The direct mycological examination revealed septated and hyaline hyphae. There was started amphotericin B deoxycholate. The culture of the material demonstrated presence of the Aspergillus fumigatus. The following two months, the patient was submitted to three surgeries, with insertion of drainage catheter and administration of amphotericin B intralesional. Three months after hospital admission, his neurological condition suffered discrete changes. However, he died due to intrahospital pneumonia. Brain abscess caused by Aspergillus fumigatus must be considered in the differential diagnosis of the brain expansive lesions in AIDS patients in Brazil.
La aspergilosis cerebral es una causa rara de lesión expansiva cerebral en pacientes con SIDA. Presentamos el primer reporte de un absceso cerebral causado por Aspergillus fumigatus en un paciente brasileño con SIDA. El paciente, de 26 años de edad, presentaba antecedentes de infección por el virus de la inmunodeficiencia humana (VIH), tuberculosis pulmonar y toxoplasmosis cerebral. Manifestó fiebre, tos, disnea y dos episódios de convulsiones. La tomografía computadorizada (TC) demostró una lesión hipodensa parasagital y bi-parietal con realce periférico e importante efecto de masa. Se inició tratamiento anti-Toxoplasma. Tres semanas después, el paciente evidenció confusión mental y una nueva TC de cráneo mostró aumento de la lesión. Se realizó biopsia cerebral con drenaje de 10 mL de material purulento. El examen micológico directo reveló hifas hialinas septadas. Se inició anfotericina B deoxicolato. La cultura del material demostró presencia de Aspergillus fumigatus. En los siguientes dos meses el paciente fue sometido a otras tres cirugías, insertándose un catéter de drenaje y administrándose anfotericina B intralesional. Tres meses después de la admisión hospitalaria, la condición neurológica del paciente sufrió discretos cambios. Sin embargo, falleció debido a neumonia intrahospitalaria. Aunque muy raros, los abscesos cerebrales causados por Aspergillus fumigatus deben ser considerados en el diagnóstico diferencial de las lesiones expansivas cerebrales en pacientes con SIDA.

Análise dos critérios diagnósticos dos distúrbios do metabolismo de glicose e variáveis associadas à resistência insulínica

Os critérios diagnósticos dos distúrbios do metabolismo de glicose foram estabelecidos pela Organização Mundial de Saúde (OMS)/National Diabetes Data Group (NDDG) no início da década de 1980. Em 1997, a American Diabetes Association (ADA) sugeriu novos critérios diagnósticos, baseados na interpretação da glicemia de jejum. Neste estudo, 56 indivíduos foram submetidos ao teste de tolerância à glicose oral (oGTT) (75g) e aplicamos ambos os critérios diagnósticos para estimar a prevalência de distúrbios de metabolismo de glicose. Dos 56 indivíduos, 11 (19,6% - grupo 1) foram considerados intolerantes pelos critérios da OMS/NDDG e seis (10,7% - grupo 2), como glicemia de jejum alterada (GJA) pelos novos critérios da ADA. Comparamos variáveis clínicas e bioquímicas (idade, índice de massa corporal, pico de insulina durante o oGTT, área sob a curva de insulina durante o oGTT e concentrações séricas basais da proteína carreadora dos fatores de crescimento insulina-símiles 1 (IGFBP-1) entre ambos os grupos, não observando diferença significativa em nenhuma das variáveis (43 ± 13 x 46 ± 4,4 anos, 29,5 ± 3,2 x 27,2 ± 2,6kg/m², 153,7 ± 100,7 x 171,3 ± 145,6µUI/ml, 12.040 ± 8.488 x 13.970 ± 12.170 e 14,4 ± 9,3 x 19,4 ± 11,8ng/ml, respectivamente, p = NS). Entretanto, quando comparamos estas mesmas variáveis entre o grupo de indivíduos considerados normais pelos critérios da ADA e intolerantes pelos critérios da OMS/NDDG (n = 8, grupo 3) e o grupo de indivíduos considerados normais por ambos os critérios (n = 42, grupo 4), observamos que não houve também diferença em relação à idade (42 ± 15 x 38 ± 10 anos, respectivamente, NS); entretanto o grupo 3 apresenta índice de massa corporal (IMC) (29,5 ± 3,9 x 24,5 ± 3,5kg/m², respectivamente, p < 0,02), pico de insulina durante o oGTT (115,2 ± 29,1 x 84,4 ± 56,5µUI/ml, respectivamente, p < 0,02) e área sob a curva de insulina durante o oGTT (9.112 ± 2.323 x 6.649 ± 4.438, respectivamente, p < 0,007) com valores médios superiores ao grupo 4. O grupo 3 apresentou ainda concentrações séricas basais de IGFBP-1 com valores em média inferiores ao grupo 4 (14,9 ± 10,1 x 28,9 ± 17,6ng/ml, respectivamente, p < 0,03).
The diagnostic criteria of the glucose metabolism disorders had been established by the World Health Organization (WHO)/National Data Group (NDDG) in the beginning of the 1980's. In 1997, the American Diabetes Association (ADA) suggested new criteria, based upon the interpretation of the fasting glucose. In this study, 56 subjects underwent an oral glucose tolerance test (oGTT), and we have applied both criteria to estimate the prevalence of the glucose metabolism disorders. Out of 56 subjects, 11 (19,6% - Group 1) had been considered glucose intolerants by the criteria of the WHO/NDDG and 6 (10,7% - Group 2) as impaired fasting glycemia (IFG) by the ADA new criteria. We have compared clinical and biochemical variables (age, BMI, insulin peak during oGTT, area under the insulin curve during oGTT and serum concentrations of IGFBP-1) between both groups, not observing differences in any of the variables (43 ± 13 x 46 ± 4.4 years, 29.5 ± 3.2 x 27.2 ± 2.6kg/m², 153.7 ± 100.7 x 171.3 ± 145.6µUI/ml, 12,040 ± 8,488 x 13,970 ± 12,170 e 14.4 ± 9.3 x 19.4 ± 11.8ng/ml, respectively, p = NS). However, when we compare these same variables between the subjects considered normal by the ADA criteria and intolerants by the WHO/NDDG (n = 8, Group 3) and the subjects considered normal by both criteria (n = 42, Group 4), we observed that it also did not have difference in relation to age (42 ± 15 x 38 ± 10 years, respectively, NS), however, Group 3 presents BMI (29.5 ± 3.9 x 24.5 ± 3.5kg/m², respectively, p < 0.02), insulin peak (115.2 ± 29.1 x 84.4 ± 56.5µUI/ml, respectively, p < 0.02) and area under the insulin curve (9,112 ± 2,323 x 6,649 ± 4,438, respectively, p < 0.007)with higher values than Group 4. Efeito do estresse hídrico sobre a eficiência do halosulfuron no controle de tiririca

Foi conduzido um trabalho em casa de vegetação com o objetivo de estudar o efeito de 10 dias de estresse hídrico (Yw de ³2,6; -2,2; -0,35 e -0,2 MPa) sobre a eficiência do halosulfuron (150g p.c./ha+Aterbane-0,25% v/v) no controle de tiririca em pós-emergência . A aplicação do halosulfuron em plantas de tiririca sob estresse de até -2,6 MPa controlou satisfatoriamente a tiririca desde que irrigadas até 24 horas após a aplicação, mas quando a irrigação ocorreu 48 horas após, houve comprometimento na translocação do herbicida nas plantas. Quando o estresse foi menor que -2,0 MPa, o herbicida manteve-se eficaz no controle de tiririca, principalmente para aquelas com Yw de -0,35 MPa.
A grennhouse experiment was carried out to evaluate the effect of 10 days water stress (Yw values of ³2.6; -2.2 -0.35, and -0.2 MPa) on the efficacy of pos-emergent application of halosulfuron (150g c.p./ha+Aterbane 0.25% v/v) in the control of purple nutsedge. When the suitable water supplying began in a period of 24 hours after the herbicide spraying, halosulfuron gave an excellent control of purple nutsedge under every water stress conditions. When the suitable irrigation conditions began 48 hours after herbicide spraying, there was a lower halosulfuron efficacy. When the stress was lower than -2.0 MPa, the herbicide gave a good control, mainly in the Yw of -0.35 MPa.
