ABSTRACT Introduction: Survival of patients living with HIV has increased and low bone mineral density and fractures are reported in this population. Chronic kidney disease causes bone disorder, which also contributes to low bone mineral density, fractures, and vascular calcification. This study aimed to assess the osteometabolic profile in HIV patients undergoing hemodialysis. Methods: Data of 21 patients were assessed. Laboratory: calcium, phosphorus, albumin, intact parathyroid hormone, total alkaline phosphatase, 25OH-vitamin D, CD4 count and HIV viral load. Bone mineral density was assessed by bone densitometry, and x-rays were taken to determine vascular calcification scores. Results: The median age of patients was 48 years (81% male) and the median times of HIV infection and hemodialysis were 132 and 120 months, respectively. Median serum calcium and phosphorus levels were normal and the median intact parathyroid hormone was 360 pg/mL. Osteopenia was diagnosed in 33% of patients and osteoporosis in 33%; 24% of patients had previously suffered fractures. The Kauppila score was higher in patients with fractures (p=0.040). Vascular calcification was identified in 12 (57%) patients. Inverse correlations were demonstrated between the Kauppila score and the T-score of the femoral neck (p<0.001) and the T-score of the lumbar spine (p<0.001); and between the Adragão score and T-score of the femoral neck (p=0.001) and the T-score of the lumbar spine (p=0.001). Conclusion: A high prevalence was observed of low bone mineral density, fractures and vascular calcification in HIV patients on hemodialysis. It is necessary to confirm these results with larger studies and with a control group of non-HIV patients.

ABSTRACT Current instrumentation systems cannot fully prepare oval root canal systems. This may cause accumulation of hard tissue debris and fail to eliminate bacteria from areas inaccessible to instrumentation, which could perpetuate periapical inflammation and jeopardize the success of endodontic treatment Aim To evaluate the performance of two endodontic systems in oval canals by investigating the changes in volume, unprepared areas, and centering ability of XP-endo Shaper (XPS) and WaveOne Gold (WOG) in oval canals using microcomputed tomography (micro-CT) Materials and Method Thirty mandibular canines were scanned before and after preparation with WOG (25/.07 and 35/.06) or XPS (30/.01) to evaluate the volume, surface area, and canal centralization at 4 mm and 10 mm from the apical foramen Results Volume and surface area increased significantly after preparation with both systems (p<0.05). However, no significant difference was observed in the unprepared areas, regarding either the entire canal (26.21% for WOG and 30.10% for XPS), or the apical segment (18.82% for WOG and 14.63% for XPS) (p >0.05) Conclusions Both systems maintained canal centralization, with no difference between them. XPS and WOG had similar shaping abilities in the mandibular canine, but left almost one third of the unprepared areas.

RESUMO Os sistemas de instrumentação atuais são incapazes de preparar completamente os sistemas de canais radiculares do canal oval, o que pode levar ao acúmulo de detritos de tecido duro e manter micro-organismos em áreas inacessíveis à instrumentação. Essas bactérias poderiam perpetuar a inflamação periapical e comprometer o sucesso do tratamento endodôntico Objetivo Para avaliar o comportamento de dois sistemas endodônticos em canais ovais, esse estudo investigou as alterações no volume, áreas não preparadas e capacidade de centralização do XP-endo Shaper (XPS) e do WaveOne Gold (WOG) em canais ovais usando microtomografia computadorizada (micro-CT) Material e métodos Trinta caninos inferiores foram escaneados antes e depois do preparo com WOG (25/.07 e 35/.06) ou XPS (30/.01) para avaliar o volume, a área de superfície e a centralização do canal a 4 mm e 10 mm do forame apical Resultados O volume e a área de superfície aumentaram significativamente após o preparo com ambos os sistemas (p<0,05). No entanto, não foram observadas diferenças significativas nas áreas não preparadas, não apenas em todo o canal (26,21% para WOG e 30,10% para XPS), mas também no segmento apical (18,82% para WOG e 14,63% para XPS) (p >0,05) Conclusão Ambos os sistemas mantiveram a centralização do canal, sem diferenças entre eles. O XPS e o WOG tiveram habilidades de modelagem semelhantes no canino mandibular, mas deixaram quase um terço das áreas do canal sem preparo.

ABSTRACT Introduction: Improving survival of Acute Lymphoblastic Leukemia (ALL) in adult patients has been a challenge. Despite intensive chemotherapy treatment, overall survival is poor. However, several studies demonstrate that young adult patients have better survival when treated with pediatric-based intensive regimens. Considering these results, We decided to treat newly diagnosed ALL patients according to age and risk factors. The goal of this study was to describe the results of this intensive chemotherapy treatment approach for ALL adult patients diagnosed at our institution. Methods: Fifty-eight ALL patients, diagnosed from 2004 to 2013, were included in the analysis. Patients were assigned to either the St. Jude Total Therapy XIIIB high-risk arm (St Jude) or the CALGB 8811 (CALGB). The Kaplan-Meier survival curve was used for the survival analyses and the Cox proportional hazard regression, for multivariable analysis. Results: The overall survival was 22.9% at 10 years. The St. Jude improved survival, compared to the CALGB (p = 0.007), with 32.6% vs. 7.4% survival rate at 10 years. However, no survival benefit was found for patients younger than 20 years old (p = 0.32). The multivariable analysis demonstrated that undetectable minimal residual disease (MRD) and hematopoietic stem cell transplantation (HSCT) had beneficial impact on survival (p = 0.0007 and p = 0.004, respectively). Conclusion: ALL is a disease of poor prognosis for adults. The joint effort to standardize treatment and seek solutions is the way to start improving this scenario.

ABSTRACT Background: We evaluated the association between polymorphisms in the tumor necrosis factor alpha (TNF-α) (-G308A) gene and upper gastrointestinal bleeding (UGIB) in schistosomiasis. Methods: This was a transverse study involving 294 Brazilian patients infected with Schistosoma mansoni. Results: The homozygous A/A genotype in TNF-α (-G308A) showed a risk association (prevalence ratio = 1.90, p = 0.008) with UGIB. There was no statistically significant difference in serum TNF-α levels between the clinical groups. Conclusions: The polymorphic TNF-α (-G308A) can be a risk factor for UGIB, in addition to being a potentially predictive factor for the severity of UGIB in schistosomiasis.

Abstract Introduction Post-COVID-19 syndrome is characterized by chronic fatigue and myalgia, among other symptoms, which can limit activities of daily living. Physical therapy protocols with multicomponent exercises combine strength, resistance, balance and gait, producing sig-nificant improvements in functional mobility. Objective Evaluate whether multicomponent rehabilitation is effective in improving functional mobility and quality of life in individuals with post-COVID-19 syndrome. Methods Randomized controlled trial with 59 volunteers, divided into an intervention (IG) and control group (CG), all of whom underwent initial assessment (T0). The IG performed 24 sessions over 12 weeks and the CG did not undergo training. Functional mobility was analyzed using the 6-minute walk test (6MWT) and the Berg balance scale, while the SF-36 questionnaire assessed quality of life. Results In the 6MWT, the IG covered 464.40 + 81.26 meters (T0) and 518.60 + 82.68 meters (T4). The IG Berg scale scores were 48.00 + 4.00 (T0) and 51.90 + 4.26 (T4). In terms of quality of life, the IG obtained mean scores of 96.26 + 10.14 (T0) and 102.60 + 5.53 (T4). None of these measures was statistically significant. Conclusion Individuals who underwent the multicomponent phys-iotherapy protocol showed statistically nonsignificant increases in the variables studied.

Resumo Introdução A síndrome pós-COVID-19 caracteriza-se por fadiga crônica, mialgia, entre outros sintomas que podem causar limitações na realização das atividades de vida diária. Protocolos de fisioterapia com exercício de multicomponentes combinam força, resistência, equi-líbrio e marcha, e produzem melhorias significativas na mobilidade funcional. Objetivo Avaliar se a reabilitação de multicomponentes é eficaz em promover melhora na mobilidade funcional e qualidade de vida em indivíduos com síndrome pós-COVID-19. Métodos Ensaio clínico randomizado, controlado, realizado com 59 voluntários, divididos em grupo intervenção (GI) e grupo controle (GC). Todos fizeram avaliação inicial (T0); GI realizou 24 sessões durante 12 semanas e GC não realizou treinamento. A análise da mobilidade funcional foi realizada através do teste de caminhada de 6 minutos (TC6m) e da escala de equilíbrio de Berg. O questionário SF-36 avaliou a qualidade de vida. Resultados A distância percorrida no TC6m pelo GI foi de 464,40 + 81,26 metros (T0) e de 518,60 + 82,68 metros (T4). A pontuação na escala de Berg do GI foi de 48,00 + 4,00 (T0) e de 51,90 + 4,26 (T4). Na qualidade de vida, GI apresentou escore com média de 96,26 + 10,14 (T0) e 102,60 + 5,53 (T4). Nenhuma destas medidas foram estatisticamente significantes. Conclusão Os indivíduos que foram submetidos ao protocolo fisioterapêutico de multicomponentes apresentaram incrementos nas variáveis estudadas sem significância estatística.

Abstract This study aimed to verify the impact of high-fat diet consumption for a prolonged period on oxidative stress, fetal growth, umbilical vascular system, and placental structures in pregnant goats. Twenty-two pregnant goats were grouped into the control diet (n= 11) and fat diet (n = 11). Flaxseed meal was added to the fat diet, replacing the corn grain of concentrate, from gestational day 100 to delivery date. Diets were isonitrogenous and isoenergetic, differing in fat content (2.8% vs. 6.3% dry matter). The fat group showed higher feed intake and total plasma lipid levels than the control group (P < 0.001). No difference was found in placentome, and umbilical vascular development. Fat diet-fed goats exhibited a lower systolic peak in the umbilical artery. At delivery, placental traits were similar with the exception of the cotyledon width (P = 0.0075), which was smaller in the fat group and cotyledon surface (P = 0.0047) for multiple pregnancy of fat diet. Cotyledonary epithelium showed more intense staining of lipid droplets and a greater area for lipofuscin staining in the fat group compared to control group (P < 0.001). The mean live weight of the kids was lower in the fat group in the first week after delivery than in control group. Thus, in goats, the continuous administration of a high-fat diet during pregnancy does not appear to modify the fetal-maternal vascular structures but has an impact on a part of the placental structure; therefore, its use must be carefully evaluated.

Abstract Objective: Cancer illness is a stressful event that affects children’s quality of life. This study verified the association between coping and quality of life in 13 children with cancer (6 to 12 years old) from a public hospital in the Southeastern region of the country. Methods: The participants answered the Hospitalization Coping Scale and Pediatric Quality of Life 3.0 (Cancer Module). Descriptive and inferential statistical analyses were applied. A high average of adaptive coping and coping behaviors, such as talking, playing, and taking medicine, were observed. Results: The results suggest a potential positive correlation between quality of life related to the domain nausea, which also showed high scores, and adaptive coping, demonstrating children’s difficulty in dealing with nausea, even after using adaptive coping strategies, and this domain was significant in reducing the quality of life perceived. Conclusion: Understanding coping and qualitywww of life may steer intervention in Pediatric Oncology.

Resumo Objetivo: O adoecimento por câncer é um evento estressor que afeta a qualidade de vida das crianças. Este estudo verificou associações entre coping e qualidade de vida em 13 crianças com câncer (6 a 12 anos), atendidas em um hospital público da região sudeste do país. Método: Os participantes responderam a Escala de Coping da Hospitalização e Pediatric Quality of Life 3.0 (Cancer Module). Análises estatísticas descritivas e inferenciais foram aplicadas. Médias elevadas de coping adaptativo e de comportamentos de coping, como conversar, brincar e tomar remédio, foram verificadas. Resultados: Os resultados sugerem uma possível correlação positiva entre qualidade de vida relacionada ao domínio “enjoo”, que também apresentou escores altos, e coping adaptativo, demonstrando a dificuldade das crianças para lidarem com náuseas, mesmo usando estratégias de coping adaptativo, sendo esse domínio significativo na diminuição da qualidade de vida percebida. Conclusão: Foi concluído que a compreensão do coping e da qualidade de vida pode direcionar a assistência psicológica em Oncologia Pediátrica.

Resumo O modelo em cascata é uma estrutura comumente utilizada para avaliar a provisão de serviços ecossistêmicos, destacando seus benefícios para a sociedade e atribuindo valores não monetários e monetários a esses serviços. Adaptando este modelo, apresentamos uma metodologia para estabelecer os indicadores mais adequados para monitorar programas de Pagamento por Serviços Ecossistêmicos (PSA) hídricos no Brasil. Por meio de um processo participativo, um conjunto de indicadores para as funções ecossistêmicas foi atribuído a cada serviço ecossistêmico (SE) considerado no estudo. Em seguida, os indicadores foram classificados segundo os seguintes critérios: clareza, viabilidade, sensibilidade e relevância. Os resultados demonstraram que os critérios clareza e relevância foram os que os especialistas consideraram como os mais importantes para escolher um indicador. No geral, podemos observar que para todos os SE avaliados, os indicadores de maior pontuação foram aqueles analíticos e bem estabelecidos pela literatura. A lista de indicadores pode apoiar o monitoramento de PSA na Floresta Atlântica. Além disso, a metodologia desenvolvida pode ser facilmente aplicada em outras áreas e fornecer suporte para a definição dos indicadores mais adequados para monitorar o PSA baseado em água em diferentes contextos nos trópicos.

Abstract The cascade model is a commonly applied framework to evaluate ecosystem services provision, highlighting their benefits to society and assigning non-monetary and monetary values to these services. Adapting this model, we present a methodology to establish the most suitable indicators for monitoring Payment for Ecosystem Services (PES) programs related to water resources in Brazil. Through a participatory process, a set of ecosystem functions indicators were assigned for each ecosystem service (ES) considered in the study. The indicators were then classified following these criteria: clarity, viability, sensitivity, and relevance. The indicators were organized by their final score according to each evaluated criterion. The results demonstrated that “clarity” and “relevance” criteria were those most important for the experts to choose an indicator. In general, we could also observe a preference for analytical and well-established indicators in the literature for all ES evaluated. The indicators list presented can support the PES program monitoring in Brazil. Additionally, the methodology developed can be easily applied in other areas and provide the definition of the most suitable indicators to monitor water-based PES in different Brazilian contexts.

ABSTRACT Parasitoids of the genus Trichogramma are promising for the biological control of insect pests in several crops, including cassava, which is severely attacked by Erinnyis ello L., 1758 (Lepidoptera: Sphingidae). Evaluating the biological aspects of these parasitoids to understand their dynamics is an important step towards the implementation of this control strategy in the field. Thus, our objective was to evaluate the biology of Trichogramma manicobai Brun, Moraes & Soares, 1984, and T. marandobai Brun, Moraes & Soares, 1986 in E. ello eggs. The parasitoids were obtained by collecting E. ello eggs from a commercial production of cassava, and the host’s eggs were obtained from laboratory and greenhouse rearings. The average duration of a generation (T), net reproduction rate (R0), intrinsic rate of increase (rm), and the finite rate of increase (λ) were estimated, and from these, the fertility life table was calculated. The results indicated that T. marandobai has both higher net reproduction rate and a higher intrinsic rate of increase as well as requires less time to double its population than T. manicobai. Thus, T. marandobai has potential for natural and conservative biological control of E. ello. In addition, its potential in applied biological control should be evaluated through studies on the viability of its mass rearing in alternative hosts and its dispersion behavior in the field.

Resumo O artigo discute a relação entre a existência e a utilização de espaços escolares para práticas de leitura e o desempenho em exames de proficiência em leitura de estudantes de escolas públicas e privadas no Brasil. Neste trabalho, espaços de leitura são compreendidos como bibliotecas escolares e salas de leitura. Foram utilizados os resultados do Sistema de Avaliação da Educação Básica de 2017, de alunos do 9º ano do ensino fundamental. Com base nesses dados e por meio de uma análise fatorial, elaborou-se um indicador de espaços de leitura. A partir desse indicador, foram realizadas estatísticas descritivas e inferenciais que demonstraram a pertinência do constructo. O pressuposto estabelecido é de que apenas a existência dos recursos escolares não é condição suficiente para a garantia do aprendizado, dependendo também do trabalho pedagógico nesses espaços e suas condições de uso. Os achados indicam associação entre a existência e utilização dos espaços de leitura nas escolas com melhores resultados obtidos pelos estudantes no tocante à proficiência em leitura. Os resultados apontam também que os espaços de leitura repercutem mais em escolas de nível socioeconômico mais baixo, nas quais a existência de recursos escolares, associados à utilização adequada, se convertem em vantagens, em termos de aprendizado para os alunos.

Abstract The article discusses the relationship between the existence and use of school spaces for reading practices and the performance in reading proficiency exams of students from public and private schools in Brazil. In this work, reading spaces are understood as school libraries and reading rooms. The results of the 2017 Basic Education Assessment System of students in the 9th year of elementary school were used. Based on these data and through a factor analysis, an indicator of reading spaces was elaborated. From this indicator, descriptive and inferential statistics were realized, demonstrating the relevance of the construct. The established assumption is that the existence of school resources alone is not a sufficient condition to guarantee learning, also depending on the pedagogical work in these spaces and their conditions of use. The findings indicate an association between the existence and use of reading spaces in schools with better results obtained by students in terms of reading proficiency. The results also point out that reading spaces have more repercussions in schools of lower socioeconomic level, in which the existence of school resources, associated with proper use, become advantages in terms of learning for students.

Abstract Background Cardiovascular diseases are the main cause of death in women and the accuracy of currently available risk scores is questionable. Objective To reclassify the risk estimated by the Framingham Risk Score (FRS) in asymptomatic middle-aged women by incorporating family history, exercise testing variables, and subclinical atherosclerosis markers. Methods This cross-sectional study included 509 women (age range, 46-65 years) without cardiovascular symptoms. Those at low or intermediate risk by the FRS were reclassified to a higher level considering premature family history of acute myocardial infarction and/or sudden death; four variables from exercise testing; and two variables related to subclinical atherosclerosis markers. The homogeneity of these variables according to the FRS was verified by Pearson chi-square test (p<0.05). Results According to the FRS, 80.2%, 6.2%, and 13.6% of the women were classified as low (<5%), intermediate (5-10%), and high (>10%) risks, respectively. The intermediate-risk stratum showed the highest increase (from 6.2% to 33.3%) with addition of family history; followed by addition of chronotropic index <80% (to 24.2%); functional capacity <85% (22.2%), coronary calcium score >0 (20.6%); decreased one-minute heart rate recovery ≤12 bpm (15.2%); carotid intima-media thickness >1 mm and/or carotid plaque (13.8%) and ST-segment depression (9.0%). The high-risk stratum increased to 14.4% with the addition of reduced heart rate recovery and to 17.1% with the coronary calcium score. Conclusion Incorporation of premature family history of cardiovascular events, exercise testing abnormal parameters, and subclinical atherosclerosis markers into the FRS led to risk reclassification in 3.0-29.7% of asymptomatic middle-aged women, mainly by an increase from low to intermediate risk.

Resumo A colpopexia sacral é um dos procedimentos padrão para tratar o prolapso de órgãos pélvicos apical. Na maioria dos casos, uma tela sintética é usada para facilitar a colposuspensão. A espondilodiscite é uma complicação rara, mas potencialmente grave, que deve ser prontamente diagnosticada e tratada, apesar da falta de consenso no manejo dessa complicação. Relatamos um caso de espondilodiscite após histerectomia supracervical laparoscópica e colpopexia sacral tratada conservadoramente. Também apresentamos uma revisão da literatura sobre essa complicação rara. Uma abordagem conservadora sem remoção da tela pode ser possível em pacientes selecionadas (estáveis, sem lesões vaginais, exposição da tela ou comprometimento neurológico grave). Hemoculturas e cultura de biópsias guiadas por imagem devemser realizadas para direcionar a antibioticoterapia. O tratamento conservador versus o cirúrgico deve ser avaliado regularmente, dependendo da progressão clínica e analítica. Uma equipe multidisciplinar é de suma importância no acompanhamento desses pacientes.

Abstract Sacral colpopexy is one of the standard procedures to treat apical pelvic organ prolapse. In most cases, a synthetic mesh is used to facilitate the colposuspension. Spondylodiscitis is a rare but potentially serious complication that must be promptly diagnosed and treated, despite the lack of consensus in the management of this complication.We report one case of spondylodiscitis after a laparoscopic supracervical hysterectomy and sacral colpopexy treated conservatively. We also present a literature review regarding this rare complication. A conservative approach without mesh removal may be possible in selected patients (stable, with no vaginal lesions, mesh exposure or severe neurologic compromise). Hemocultures and culture of imageguided biopsies should be performed to direct antibiotic therapy. Conservative versus surgical treatment should be regularly weighted depending on clinical and analytical progression. A multidisciplinary team is of paramount importance in the follow-up of these patients.

Abstract INTRODUCTION: We evaluated the association between genetic polymorphisms in exon 1 (A/O alleles) and promoter regions at positions -550 (H/L variant, rs11003125) and -221 (X/Y variant, rs7096206) MBL2 and periportal fibrosis regression. METHODS: This was a retrospective cohort study involving 114 Brazilians infected with Schistosoma mansoni, who were subjected to follow-up for three years after specific treatment for schistosomiasis to estimate the probability of periportal fibrosis regression. RESULTS: A risk association was observed between polymorphism at the exon 1 MBL2 and periportal fibrosis regression. CONCLUSIONS: This study suggests that the polymorphism of exon 1 MBL2 may potentially be used to predict periportal fibrosis regression in this population.

Resumo Embora o consumo de açaí (Euterpe oleracea) seja há tempos um importante componente da dieta dos povos amazônicos, o açaizeiro tem recentemente atraído tanto o interesse econômico quanto científico devido à presença de urna vasta gama de compostos bioativos encontrados no pericarpo de seus frutos. As sementes de açaí representam o principal subproduto após a extração da polpa e podem ser potencialmente utilizadas para a produção de etanol, mas esse processo é dificultado pelo conhecimento limitado sobre a biología das sementes, sua composição química e a deposição de reserva de padrões durante o desenvolvimento das sementes. O objetivo deste trabalho é descrever o desenvolvimento morfoanatômico das sementes, bem como identificar os principais compostos orgânicos armazenados nas sementes. Para atingir esse objetivo, foram realizadas análises histológicas e histoquímicas das sementes em desenvolvimento. Os resultados revelaram que a semente é albuminosa, bitegmica e que tegumento interno da semente dá origem as ruminações do endosperma. Além disso, os resultados indicam que as reservas nutritivas das sementes de açaí são encontradas nas paredes celulares espessadas pelo endosperma na forma de polissacarídeos de reserva. Nossas descobertas fornecem informações úteis para futuros estudos sobre biología reprodutiva, propagação e melhoria de uma cultura tao lucrativa.

Abstract Although the consumption of açaí (Euterpe oleracea) pulp has long been an important component of the diet of the peoples from the Amazon, the açaí palm tree has recently attracted economic and scientific interest because of its vast array of bioactive compounds found in the fruit pericarp. The açaí seeds are the largest byproduct after pulp extraction and have potential for use in ethanol production, but this process is hindered by limited knowledge of seed biology, chemical composition and pattern reserve deposition during seed development. The aim of this work was to describe the morphoanatomical development of the seeds, as well as to identify the main organic compounds stored in the seeds. To achieve this goal, histological and histochemical analyses were performed on developing seeds. Results showed the seed is albuminous, bitegmic and that ingrowths of the seed coat give rise to a ruminate endosperm. Moreover, the nutritive reserves of açaí seeds are found in the endosperm thickened cell walls as reserve polysaccharides. Our findings provide information for future studies dealing with reproductive biology, propagation and the improvement of this profitable crop.
