Abstract Background: Cardiovascular diseases are the leading cause of mortality worldwide and Mexico is no exception. The epidemiological data obtained in 1990 showed that cardiovascular diseases represented 19.8% of all causes of death in our country. This figure increased significantly to 25.5% for 2015. Some national surveys suggest that more than 60% of the adult population has at least one risk factor for cardiovascular disease (obesity or overweight, hypertension, smoking, diabetes, dyslipidemias). On the other hand, data from the Pan American Health Organization have linked the process of atherosclerosis as the first cause of premature death, significantly reducing life expectancy, which has enormous social repercussions. Objective: This document constitutes the Clinical Practice Guide (CPG) prepared at the initiative of the Mexican Society of Cardiology in collaboration with the Mexican Society of Nutrition and Endocrinology, AC, National Association of Cardiologists of Mexico, AC, Mexican Association for the Prevention of Atherosclerosis and its Complications, AC, National Normative Committee of General Medicine, AC, National College of Geriatric Medicine, AC, College of Internal Medicine of Mexico, AC, Mexican Society of Angiology and Vascular and Endovenous Surgery, AC, Mexican Institute of Research Nephrological, AC and the Mexican Academy of Neurology, A.C.; with the methodological support of the Ibero-American Agency for the Development and Evaluation of Health Technologies, in order to establish recommendations based on the best available evidence and agreed upon by an interdisciplinary group of experts. The objective of this document is to provide evidence-based recommendations to help decision makers in the diagnosis and treatment of dyslipidemias in our country. Material and methods: This document complies with international quality standards, such as those described by the Institute of Medicine of the USA, the Institute of Clinical Excellence of Great Britain, the Scottish Intercollegiate Guideline Network and the Guidelines International Network. A multidisciplinary group of clinical experts and methodologists with experience in systematic reviews of the literature and the development of clinical practice guidelines was formed. A scope document was agreed upon, relevant clinical questions were established, the best available evidence critically evaluated in systematic literature reviews was exhaustively identified, and clinical recommendations were developed. The modified Delphi Panel methodology was used to achieve an adequate level of consensus in each of the recommendations contained in this CPG. Results: 23 clinical questions were agreed upon which gave rise to their respective clinical recommendations. Conclusions: We consider that this document contributes to better clinical decision-making and becomes a point of reference for clinicians and patients in the management of dyslipidemias and this contributes to reducing the morbidity and mortality derived from atherosclerotic cardiovascular events in our country.

Abstract The Ocoaxo complex of pines is integrated by 3 species (O. assimilis, O. cardonai, O. varians) that feed on the sap of the needles of Pinus spp. causing chlorosis and its eventual fall. The population growth of these insects has caused the defoliation of large areas of forest in several localities of the country, therefore they have been considered pests, although basic aspects of their biology and distribution are still unknown. In the present work, we analyzed the records of specimens deposited in entomological collections and online databases that allowed us to define the geographic space where these species are present in interaction with the ecosystems of Mexico, in addition to providing the food spectrum of their adult stage, identifying the environmental factors that are potentially associated with their presence, as well as knowing the period in which they are found throughout the year. The analysis of these data also allowed us to generate spatial hypotheses of their distribution, estimate the limits of the areas where they have been reported and recognize potential areas for monitoring.
Resumen El complejo Ocoaxo de los pinos está integrado por 3 especies (O. assimilis, O. cardonai, O. varians) que se alimentan de la savia de las acículas de Pinus spp. provocando clorosis y su eventual caída. Los crecimientos poblacionales de estos insectos han provocado la defoliación de grandes superficies de bosque en varias localidades del país, por lo cual se les ha dado el estatus de plaga, a pesar de que aún se desconocen aspectos básicos de su biología y distribución. En el presente trabajo se analizaron los registros de especímenes depositados en colecciones entomológicas y bases de datos en línea, que permitieron definir el espacio geográfico donde estas especies están presentes en interacción con los ecosistemas de México, además de proporcionar el espectro alimenticio de su estado adulto, identificar los factores ambientales que potencialmente están asociados a su presencia, así como conocer el periodo en el que se encuentran a lo largo del año. Los análisis de estos datos también permitieron generar hipótesis espaciales de su distribución, estimar los límites de las áreas donde se han reportado y reconocer áreas potenciales para su monitoreo.

Informality, Gender Employment Gap, and COVID-19 in Mexico: Identifying Persistence and Dynamic Structural Effects

Abstract The objective is to analyze the impact of the COVID-19 pandemic on the dynamics of the Mexican labor market (formal-informal employment) by gender. It is built consistent micro-founded time-series from 1987:Q1 to 2019:Q4 using the Mexican urban employment surveys and estimate a VAR model linking aggregate production and each market segment. Our results suggest significant adverse effects on formal employment resulting from the COVID-19 pandemic, with lengthy job recovery for females and males. The informal sector in both genders presents a lower forecasted response to the initial production shock but substantial observed employment losses, potentially linked to structural changes in the market. In the COVID-19 crisis, the informal sector is not a substitute for formal employment losses. The complexity of this crisis suggests crafting policies to improve the easiness of the market to enhance formal job recovery while promoting gender equality. Our main contribution is to estimate the diverse employment losses by segments and a critical structural change in the labor market dynamics resulting from the COVID-19 pandemic focusing on urban employment.
Resumen El objetivo es analizar el impacto de la pandemia del COVID-19 en la dinámica del mercado laboral mexicano (empleo formal-informal) por género. Se construyen series de tiempo microfundamentales consistentes desde 1987:Q1 hasta 2019:Q4 utilizando las encuestas de empleo urbanas mexicanas, y se estima un modelo VAR que vincula la producción agregada y cada segmento del mercado. Nuestros resultados sugieren que hay efectos adversos significativos en el empleo formal como resultado de la pandemia del COVID-19, con una larga recuperación del empleo para las mujeres y los hombres. El sector informal, en ambos sexos, presenta una respuesta pronosticada menor al choque de producción inicial, pero pérdidas sustanciales de empleo observadas, potencialmente vinculadas a cambios estructurales en el mercado. En la crisis de COVID-19, el sector informal no es un sustituto de las pérdidas de empleo formal. La complejidad de esta crisis sugiere la elaboración de políticas que mejoren la facilidad del mercado para potenciar la recuperación del empleo formal al tiempo que se promueve la igualdad de género. Nuestra principal contribución es estimar las diversas pérdidas de empleo por segmentos y un cambio estructural crítico en la dinámica del mercado laboral resultante de la pandemia de COVID-19, centrando el análisis en el empleo urbano.

Clinical effects of a hypercaloric and hyperproteic oral suplemment enhanced with w3 fatty acids and dietary fiber in postsurgical ambulatory head and neck cancer patients

Background: Patients with head and neck cancer undergoing surgery have a high risk of nutritional complications. Objective: The aim of our study was to investigate the influence of a hypercaloric and hyperproteic oral supplement enriched with w3 FATTY ACIDS and fiber in clinical parameters in head and neck tumor postsurgical ambulatory patients with or without radiotherapy. Design: A population of 37 ambulatory postsurgical patients with oral and laryngeal cancer was enrolled. At Hospital discharge postsurgical head and neck cancer patients were asked to consume two units per day of a hypercaloric and hyperproteic oral supplement for a twelve week period. Results: The mean age was 63.8+/-7.1 years (16 female/21 males). Duration of supplementation was 98.1±19.1 days. A significant increase of albumin and transferrin levels was observed, in total group and in patients undergoing radiotherapy and without it. No differences were detected in weight and other anthropometric parameters in total group and in patients with radiotherapy during the protocol. Nevertheless, patients without radiotherapy showed a significant improvement of BMI; weight, fat free mass and fat mass. Conclusions: An omega 3 and fiber enriched formula improved seric protein levels in ambulatory postoperative head and neck cancer patients. Improvement of weight, fat mass and fat free mass was observed in patients whom not received radiotherapy during the follow up.
Antecedentes: Los pacientes con tumores de cabeza y cuello sometidos a cirugía presentan un alto riesgo de complicaciones nutricionales. Objetivo: El principal objetivo de nuestro trabajo fue evaluar la influencia de un suplemento hiperenergético e hiperproteico enriquecido en ácidos grasos w3 y fibra en pacientes postquirúrgicos ambulatorios con tumores de cabeza y cuello, con y sin radioterapia. Diseño: Una muestra de 37 pacientes postquirúrgicos ambulatorios con tumores de cabeza y cuello fue evaluada. Tras el alta hospitalaria, los pacientes recibieron dos envases al día de un suplemento hiperproteico e hipercalórico durante 12 semanas. Resultados: La edad media fue de 63.8+/-7.1 años (16 mujeres/21 varones). La duración media de la suple-mentación fue de 98.1±19.1 días. Se detectó un aumento significativo en los niveles de albúmina y transferrina en los pacientes del grupo global, en los que recibieron radioterapia y en los que no al recibieron. El peso y los parámetros antropométricos no mejoraron en el grupo global ni en los que recibieron radioterapia. Sin embargo los pacientes que no recibieron radioterapia tuvieron un aumento de la masa magra, peso y masa grasa Conclusiones: La formula enriquecida en omega 3 y fibra mejoró lo niveles de proteínas séricas. Por otra parte los pacientes que no recibieron radioterapia durante el seguimiento presentaron un aumento del peso, masa grasa y masa magra.

Design: A population of 33 ambulatory postsurgical patients with oral and laryngeal cancer was enrolled. At Hospital discharge postsurgical head and neck cancer patients were asked to consume two units per day of a w3 enriched immunoenhanced powdered formula for a twelve week period. Results: The mean age was 61.3 ± 9.1 years (6 female/27 males). Duration of supplementation was 95.9 ± 21.1 days. A significant increase of albumin and transferrin levels was observed, in total group and in patients undergoing radiotherapy and without it. No differences were detected in weight and other anthropometric parameters in total group and in patients with radiotherapy during the protocol. Nevertheless, patients without radiotherapy showed a significant improvement of BMI; weight, fat free mass and fat mass. Conclusions: At dose used, an omega 3 enriched powdered formula improved seric protein levels in ambulatory postoperative head and neck cancer patients. Tras el alta hospitalaria, los pacientes recibieron dos envases al día de un suplemento inmunoenriquecido con omega 3 en polvo durante 12 semanas. Resultados: La edad media fue de 61,3 ± 9,1 años (6 mujeres/27 varones). La duración media de la suplementación fue de 95,9 ± 21,1 días. Se detectó un aumento significativo en los niveles de albúmina y transferrina en los pacientes del grupo global, en los que recibieron radioterapia y en los que no al recibieron. El peso y los parámetros antropométricos no mejoraron en el grupo global ni en los que recibieron radioterapia. Influence of an immuno-enhanced formula in postsurgical ambulatory patients with head and neck cancer

Background: Patients with head and neck cancer undergoing surgery have a high incidence of ambulatory postoperative complications. Objective: The aim of our study was to investigate the influence of an oral immunoenhanced supplement (arginine and glutamine) on nutritional and biochemical parameters in postsurgical ambulatory patients with head and neck tumor. Design: A population of 39 ambulatory postsurgical patients with oral and laryngeal cancer was enrolled. At Hospital discharge postsurgical head and neck cancer patients were asked to consume two units per day of a specially designed enhanced supplement for a twelve week period. Results: The mean age was 60.2+/-13.1 years (9 female/30 males). Duration of supplementation was 90.8+20 days. A significant increase of albumin (3.1±0.6 g/dl vs 4.12+0.7g /dl;p<0.05), prealbumin (21.4±6.3 mg/dl vs 22.4+5.9 mg/dl;p<0.05) and transferrin (198.8±45.2 mg/dl vs 253.8+60.7 mg/dl; p<0.05) levels were observed. No differences were detected in weight and other anthropometric parameters. Ten patients (41.3%) received radiotherapy along the enhanced supplementation period and only 5 (20% of patients with radiotherapy) developed a clinical oral mucositis. Conclusions: At dose used, arginine and glutamine enhanced formula improved seric protein levels in ambulatory postoperative head and neck cancer patients with a low rate of oral mucositis in the subgroup with radiotherapy.

Objetivo: El principal objetivo de nuestro trabajo fue evaluar la influencia de un suplemento oral inmunoenriquecido con arginina y glutamina en pacientes postquirúrgicos ambulatorios con tumores de cabeza y cuello. Diseño: Una muestra de 39 pacientes postquirúrgicos ambulatorios con tumores de cabeza y cuello fue evaluada. Tras el alta hospitalaria, los pacientes recibieron dos bricks al día de un suplemento inmunoenriquecido durante 12 semanas. Resultados: La edad media fue de 60,2+/-13,1 años (9 mujeres/30 varones). La duración media de la suplementación fue de 90,8+20 días. Se detectó un aumento significativo en los niveles de albúmina (3,1±0,6 g/dl vs 4,12+0,7 mg/dl;p<0,05), prealbúmina (21,4±6,3 mg/dl vs 22,4+5,9 mg/dl;p<0,05) y transferrina (198,8±45,2 mg/dl vs 253,8+60,7 mg/dl; p<0,05). No se detectaron diferencias en el peso ni en otras variables antropométricas. Un total de 10 pacientes (41,3%) recibieron radioterapia durante la suplementación, de los cuales solo 2 (20% de los pacientes con radioterapia) desarrollaron mucositis oral con significación clínica. Conclusiones: A la dosis usada, la formula enriquecida con arginina y glutamina mejoró lo niveles de proteínas séricas. Por otra parte los pacientes suplementados presentaron una baja tasa de mucositis asociada a la radioterapia.

Introduction: Postsurgical patients with head and neck cancer could have a high rate of ambulatory complications. The aim of our study was to investigate whether oral ambulatory nutrition of post surgical head and neck cancer patients with recent weight loss, using an omega 3 fatty acids and arginine diet could improve nutritional status and complications. Design: A population of 30 ambulatory post surgical patients with oral and laryngeal cancer and recent weight loss was enrolled. At Hospital discharge post surgical head and neck cancer patients were asked to consume two cans per day of a specially designed omega 3 fatty acid and arginine enhanced supplement during 12 weeks. Results: The average age was 62.7 + 11.1 years (3 females/27 males). The average days of dietary suplementation was 90.8 + 22 days. Blood albumin, prealbumin, transferring and lymphocites concentrations improved. C reactive protein decreased. Neither weight improvement nor other anthropometric parameters improved. A positive correlation between ratio w3/w6and kg of fat mass (r = 0.69; p < 0.05) and with midarm circumference (r = 0.62; p < 0.05) were detected. Patients were divided in two groups; group I (ratio dietary intake w3/w6 < 0,51) and group II (ratio dietary intake w3/w6 > 0,51). Group II had more final fat mass than group I (17.52 + 6.3 kg vs. 13.9 + 5.1 kg; p < 0.05). Conclusion: At dose taken, inmuno-enhanced formula increased blood proteins (albumin, prealbumin and transferrin) concentrations and decresed CRP. The increase of fat mass was higher in patients with better w3/w6 ratios.
Fundamento y objetivo: Los pacientes con cáncer de cabeza y cuello sometidos a cirugía pueden presentar una alta tasa de complicaciones ambulatorias. El objetivo de nuestro estudio fue investigar la influcenia de una fórmula enteral enriquecida en ácidos grasos omega 3 y arginina en la situación nutricional asi como en la tasa de complicaciones clínicas (infecciones postoperatorias, complicaciones de la herida quirúrgica) de estos pacientes. Pacientes y métodos: Se realizó un estudio prospectivo de los pacientes con cáncer de cabeza y cuello intervenidos en nuestro centro (n = 30). Tras el alta hospitalaria se les administró 2 sobres día de un preparado enriquecido en ácidos grasos omega 3 y arginina durante 12 semanas. Realizandose una valoración nutricional en el tiempo basal (tras alta hospitalaria) y a las 12 semanas de tratamiento, así como una valoración de las complicaciones postoperatorias ambulatorias. Resultados: La edad media fue de 62,7 ± 11,1 años (3 mujer/27 varones). La duración media de la suplementación fue de 90,8 ± 22 días. Se detectó una elevación significativa de las concentraciones séricas de albúmina, prealbúmina, transferrína y linfocitos. Los niveles séricos de PCR disminuyeron de una manera significativa (71,83 ± 19,7 md/dl vs 25,4 ± 10,1 mg/dl; p < 0,05). No se detectó ganancia de peso ni variación en otras variables antropométricas. Se detectó una correlación positiva entre la ratio w3/w6 de la ingesta de ácidos poliinsaturados y los kgs de masa grasa (r = 0,69; p < 0,05), así como la circunferencia muscular del brazo (r = 0,62; p < 0,05). Al dividir a los pacientes en grupo I (ingesta ratio w3/w6 < 0,51) y grupo II (ingesta ratio w3/w6 > 0.51), el grupo II mostró mayor masa grasa final que el grupo I (17,52 ± 6,3 kg vs. 13,9 ± 5,1 kg; p < 0,05). Conclusiones: A la dosis utilizada, esta fórmula inmunoenriquecida mejora los niveles séricos de albúmina, prealbúmina, transferrina y linfocitos, disminuyendo los niveles de proteína C reactiva en pacientes con cáncer de cabeza y cuello intervenidos. El incremento de masa grasa es superior en los pacientes con una ratio favorable en la ingesta de w3/w6.

Afin de déterminer l'efficacité de la 99mTc-cyprofloxazine cubaine dans la détection des infections bactériennes dans les prothèses de hanche, une étude expérimentale, de procédé, et analytique transversale auprès de 15 patients suspectés d'infection, envoyés au département de Médecine nucléaire du Service scientifique international d'orthopédie " Frank País " entre le 1e janvier 2003 et le 31 décembre 2004, est réalisée. Des scintigraphies osseuses à 99mTc-méthyldiphosphate et à 99mTc-cyprofloxazine à 1 h, à 4h, et à 24 h, après injection, ont été effectuées ; la culture/biopsie a été utilisée pour la confirmation. En examinant visuellement tous les patients, une sensibilité de 100%, une spécificité de 75%, une fiabilité diagnostique de 93.3%, une valeur prédictive positive de 91.6% et une valeur prédictive négative de 100%, ont été obtenues. La relation infection/santé a montré des différences significatives entre les patients avec et sans infection à différentes heures du prélèvement, avec un niveau de signification d'a = 0.05. L'efficacité de la formule cubaine de 99mTc-cyprofloxazine dans la détection des infections bactériennes des prothèses de hanche a été démontrée.
An experimental, analytical cross-sectional study was conducted in 15 patients with suspicion of infectious process that were referred to "Frank País" International Scientific Orthopedic Complex from January 1st, 2003 to December 31st, 2004, aimed at determining the efficacy of the Cuban 99mTc-ciprofloxacin in the evaluation of bacterial infections of the hip prostheses. Bone scintigraphies with 99mTc-methylendiphosphonate and 99mTc-ciprofloxacin were performed at 1, 4 and 24 hours after the injection. The culture/biopsy was used to confirm the criteria. On the visual analysis of the patients, sensitivity was obtained in 100 %, specificity in 75 %, diagnostic accuracy in 93.3 %, positive predictive value in 91.6 %, and negative predictive value in 100 %. The injured/healthy ratio showed significant differences between the patients with and without infection at the diverse hours of acquisition, with a level of significance of a = 0.05. The effectiveness of the Cuban formulation of 99mTc-ciprofloxacin in the evaluation of bacterial infections of the hip prostheses was proved.
Con el propósito de determinar la eficacia de la 99mTc-ciprofloxacina cubana en la evaluación de las infecciones bacterianas en las prótesis de cadera, se realizó un estudio de concordancia en 15 pacientes con sospecha de un proceso infeccioso; que se remitieron al departamento de Medicina Nuclear del Complejo Científico Ortopédico Internacional "Frank País", en un período comprendido entre el 1ro. de enero de 2003 hasta el 31 de diciembre de 2004. Se emplearon gammagrafías óseas con 99mTc-metiléndifosfonato y 99mTc-ciprofloxacina a la hora, 4 y 24 h después de la inyección, el cultivo/biopsia se utilizó como criterio de confirmación. Al analizar visualmente todos los pacientes se obtuvo una sensibilidad en el 100 %, una especificidad en el 75 %, una exactitud diagnóstica en el 93,3 %, un valor predictivo positivo en el 91,6 % y un valor predictivo negativo en el 100 %. La relación lesión/sano mostró diferencias significativas entre los pacientes con y sin infección en las diferentes horas de adquisición, con un nivel de significación de a= 0,05. Se demostró la efectividad de la formulación cubana de 99mTc-ciprofloxacina en la evaluación de las infecciones bacterianas de las prótesis de cadera.

In this paper further information concerning the ecological and biogeographical aspects of scorpionism is presented. Some of the information already outlined in three previous publications(10,14,16) is assessed, and new data given concerning the pattern of distribution presented by Tityus serrulatus in Brazil. Some new ideas are also proposed regarding the possible evolutionary advantages and disadvantages of parthenogenesis versus sexuality in both the short-term and the long-term.

Recently public health problems resulting from scorpion stings have shown an alarming increase in various tropical and subtropical countries such as Brazil, Mexico, Tunisia and Morocco. In some regions of Brazil, particularly in the States of Minas Gerais, São Paulo, Bahia, Goiás and the Federal District, more than 6000 scorpion stings with over 100 deaths were reported during a three-year period. In this paper, we attempt to demostrate the effects of human activities on the environment and their consequences on the distribution of dangerous species of scorpions. This topic is discussed in relation to the evolutionary ecology, life history strategies and asexual reproduction of scorpions.
