Resumo Introdução: O Programa Nacional de Alimentação Escolar (PNAE) destaca-se por ser um dos maiores programas do mundo de alimentação escolar, contemplando o Direito Humano à Alimentação Adequada a estudantes brasileiros. Entretanto, alguns estudos têm observado baixa aceitabilidade e adesão à alimentação escolar. Objetivo: Analisar a percepção de merendeiras e escolares acerca da aceitabilidade e adesão à alimentação escolar. Metodologia: Pesquisa qualitativa realizada através de grupos focais com merendeiras de três escolas e escolares do sexto ao nono ano de escola pública de Sumidouro, Rio de Janeiro, Brasil. Os grupos focais foram realizados utilizando-se roteiros semiestruturados e gravados em áudio e vídeo, posteriormente transcritos e analisados por Análise de Conteúdo. Resultados: Foram identificados seis temas: autoconhecimento, adesão, aceitabilidade, preparo e distribuição da alimentação escolar, cardápio e ambiente. Conclusão: O emprego de esforços na melhoria da infraestrutura e aspecto do ambiente alimentar, com a aquisição de utensílios adequados, alteração no modelo de serviço de distribuição e no modo de preparo das refeições, resultando em cardápios variados, são estratégias que podem contribuir para melhorar a adesão e aceitabilidade à alimentação escolar. Essas mudanças favorecem a promoção de saúde no espaço escolar, com foco em políticas públicas saudáveis.

Abstract Introduction: National School Feeding Program (PNAE) stands out for being one of the largest school feeding programs in the world, contemplating the Human Right to Adequate Food for Brazilian students. However, some research works have observed low acceptability and adherence to school meals. Objective: To analyze the perception of school lunch ladies and students about acceptability and adherence to school meals. Methodology: Qualitative research carried out through focus groups with lunch ladies from three schools and students from the sixth to the ninth grade of a public school in Sumidouro, Rio de Janeiro state, Brazil. The focus groups were carried out using semi-structured scripts and recorded in audio and video, later transcribed and analyzed by Content Analysis. Results: Six themes were identified: self-knowledge, adherence, acceptability, preparation and distribution of school meals, menu and environment. Conclusion: The use of efforts to improve the infrastructure and aspect of the food environment, with the acquisition of adequate utensils, change in the distribution service model and in the way of preparing meals, resulting in varied menus, are strategies that can contribute to improve adherence and acceptability to school meals. These changes favor the promotion of health in the school environment, focusing on healthy public policies.

Abstract The COVID-19 pandemic has had an impact around the world, impacting various sectors of society, including education and consequently school physical education. In this way, we sought to investigate, through a descriptive-exploratory field research, with a qualitative approach to the data, the perceptions of Physical Education teachers from a metropolitan region of southern Brazil about the repercussions of the pandemic on their pedagogical practices. To this end, sixteen teachers participated in this study, nine from the public school system and seven from the private network, through semi-structured interviews, online, during the months of June and July 2021. The data, which were organized through the NVivo-12 software and analyzed by the content analysis technique, point to difficulties, facilities and learning resulting from experiences during the pandemic. Aspects of emergency remote teaching, the application of security protocols in physical education classes and the potential of physical education for students in the current and future context were addressed. From these findings it was possible to verify that these teachers needed, more than ever, to develop their adaptive skills. This is because this study demonstrated that Physical Education classes were the only time when children are out of the classroom, being perceived as a moment of liberation, after experiencing the restrictions imposed by the pandemic, considering that these teachers provide feelings of lightness, joy, and manifestation of expressions. Thus, the findings of this study can contribute to reflections and overcoming of future challenges related to the teaching of School Physical Education. For this, future research is needed to verify how these consequences are reverberating over time.

Resumen La pandemia del COVID-19 ha tenido un impacto en todo el mundo y con una afectación de diversos sectores de la sociedad, entre ellos, la educación y, consecuentemente, la educación física escolar. De esta forma, buscamos investigar, a través de un estudio de campo descriptivo-exploratoria con abordaje cualitativo de los datos, las percepciones del cuerpo docente de Educación Física de una región metropolitana del sur de Brasil sobre las repercusiones de la Pandemia en sus prácticas pedagógicas. Participaron de este estudio dieciséis docentes, nueve del sistema escolar público y siete de la red privada, mediante entrevistas semiestructuradas en línea, durante los meses de junio y julio de 2021. Los datos, fueron organizados a través del software NVivo -12, y examinados por la técnica de análisis de contenido, señalan dificultades, facilidades y aprendizajes derivados de las experiencias durante la Pandemia. Se abordaron aspectos de la enseñanza a distancia de emergencia, la aplicación de protocolos de seguridad en las clases de educación física y el potencial de la educación física para el estudiantado en el contexto actual y futuro. A partir de estos hallazgos fue posible verificar que el cuerpo docente necesita, más que nunca, desarrollar sus habilidades adaptativas. Esto porque este estudio demostró que las clases de Educación Física fueron el único momento en que los niños están fuera del aula, siendo percibido como un momento de liberación, después de experimentar las restricciones impuestas por la pandemia, considerando que estes docentes brindan sentimientos de ligereza, alegría y manifestación. de expresiones Así, los hallazgos de este estudio pueden contribuir para la reflexión y superación de futuros desafíos relacionados con la enseñanza de la Educación Física Escolar. Para ello, se necesitan futuras investigaciones que verifiquen cómo estas consecuencias están repercutiendo en el tiempo.

Resumo A pandemia da COVID-19 teve um impacto mundial e afetou vários setores da sociedade, incluindo a educação e, consequentemente, a Educação Física escolar. Desta forma, procuramos investigar, através de um estudo de campo, descritivo-exploratório, que possui uma abordagem qualitativa dos dados, as percepções dos professores de Educação Física em uma região metropolitana do sul do Brasil sobre as repercussões da Pandemia em suas práticas de ensino. Dezesseis professores participaram deste estudo, nove do sistema escolar público e sete da rede privada, através de entrevistas on-line semiestruturadas, durante os meses de junho e julho de 2021. Os dados, os quais foram organizados com o auxílio do software NVivo-12 e analisados com base na técnica de análise de conteúdo, indicaram as dificuldades, facilidades e aprendizados advindos das experiências adquiridas durante a pandemia. Aspectos do ensino à distância de emergência, a aplicação de protocolos de segurança nas aulas de Educação Física e o potencial da Educação Física para os alunos no contexto atual e futuro foram abordados. A partir destas descobertas foi possível verificar que o corpo docente precisa, mais do que nunca, desenvolver suas habilidades adaptativas. Isto porque este estudo demonstrou que as aulas de Educação Física se manifestaram como o único momento em que as crianças estão fora da sala de aula, sendo percebida como um momento de libertação, após vivenciarem as restrições impostas pela pandemia, tendo em vista que estes professores proporcionam sentimentos de leveza, alegria e manifestação de expressões. Assim, os achados deste estudo podem contribuir para a reflexão e superação de futuros desafios relacionados ao ensino da Educação Física Escolar. Para isto, torna-se necessário pesquisas futuras para verificar como estas consequências estão sendo reverberadas ao longo do tempo.

Resumo O campo da Odontologia do Trabalho tem considerado lastro teórico-científico. Esta revisão de escopo objetiva desvelar o estado da arte acerca do tema em sete bases de pesquisas indexadas, considerando quase 100 anos de publicações (1921-2020). Das 337 produções inicialmente identificadas, realizou-se recorte para os últimos seis anos (2015-2021/janeiro), aplicando-se metodologia proposta pela extensão do Prisma para revisões de escopo (Prisma-ScR). Contudo, plataformas de revisões rápidas usadas para tomada de decisões em formulações de políticas públicas ou em aperfeiçoamentos de sistemas de saúde responderam por apenas 20 publicações, enquanto a amostra de 34 produções revelou peculiaridades: pesquisas privilegiam regimes de trabalho; associam o campo de estudo e prática ao modelo stricto sensu da saúde ocupacional; desconsideram concepções amplas e humanizantes do campo da Saúde do Trabalhador; tendem a priorizar a própria saúde ocupacional odontológica e parecem reduzir o conceito do acidente de trabalho a doenças. Tais questões precisam ser superadas se a perspectiva for manter-se em vanguarda, promovendo saúd e e segurança em contextos contemporâneos de trabalho, pois, embora seja evidente a transversalidade desse campo de saberes e práticas, é necessário mais, é preciso ousar para retirá-la da invisibilidade, seja em organizações privadas ou públicas de trabalho.

Abstract The field of Occupational Dentistry has considered theoretical-scientific ballast. This scoping review aims to reveal the state of the art on the subject in seven indexed research bases, considering almost 100 years of publications (1921-2020). Of the 337 productions initially identified, a cut was made for the last six years (2015-2021/January), applying the methodology proposed by the PRISMA extension for scope reviews (PRISMA-ScR). However, rapid review platforms used for decision making in formulating public policies or in improving health systems accounted for only 20 publications, whereas the sample of 34 productions revealed peculiarities: research favors work regimes; associates the field of study and practice with the stricto sensu model of occupational health; disregards broad and humanizing concepts in the field of Worker’s Health; tends to prioritize its own occupational dental health, and seems to reduce the concept of work accidents to illnesses. Such issues need to be overcome if the perspective is to remain at the forefront, promoting health and safety in contemporary work contexts, since, although the transversality of this field of knowledge and practices is evident, more is needed, daring to remove it from invisibility whether in private or public work organizations is necessary.

Abstract The use of biopolymers and platelet-rich fibrin (PRF) is currently investigated as an excellent application biomaterial in the tissue engineering field. Natural biomaterials with application potential in tissue engineering and regenerative medicine are due to their characteristics in biocompatibility, biodegradability, and mechanical characteristic. Though, the basic knowledge of three‐dimensional fibrin scaffold and chitosan/alginate processing technology combined with an understanding of the physical-chemical properties of this natural biomaterial is necessary for proper application in regenerative medicine and tissue engineering. This mini review summarizes the information on the composition of chitosan/alginate and PRF in medical area and discusses recent developments with a special focus on its use for the application of drug delivery. The most important advances and stimulating applications are studies in regenerative medicine and tissue engineering, for wound healing to treat burns, tumor treatment, nanoparticle carriers, and drug delivery systems.

Developing biomaterials for tissue regeneration is a promising alternative for the recovery of various tissues, including bone. Atorvastatin (ATV) has a series of beneficial effects which includes bone anabolism, vasodilating, and anti-inflammatory actions. The main objective of this work was to produce and characterize polycaprolactone (PCL) matrices incorporated with ATV. Samples were prepared by the solvent casting technique. Scanning electron microscopy (SEM), X-Ray diffraction (XRD), Fourier transform infrared spectrometry (FTIR), thermogravimetric analysis (TGA), differential scanning calorimetry (DSC) and in vitro release studies were performed. FTIR analysis showed that no chemical bonds were formed between PCL and ATV. SEM analysis showed that the amount of ATV affects sample morphology. According to XRD and thermal analysis, the main ATV characteristics were maintained. The studies showed that PCL/ATV samples release the drug in a prolonged way since its release reaches around 50% after 15000 minutes of analysis and the model that showed the best fit for the studied matrices was the Higuchi model, with a correlation coefficient above 0.95.

Abstract Objectives: to evaluate the effectiveness of non-pharmacological measures used by obstetric nurses to relieve pain during labor. Methods: this is a systematic review of the databases carried out in the United States National Library of Medicine, Cumulative Index to Nursing and Allied Heal Literature, Scopus, Web of Science and Latin American and Caribbean Literature in Health Sciences, the descriptors were: labor pain, labor, obstetric and obstetric nursing. The search and selection followed the PRISMA recommendations, were carried out from August to September 2020, and randomized clinical trials were eligible and analyzed using descriptive statistics. Results: 17 clinical trials made up the final sample, which highlight the use of non-pharmacological measures with diversified benefits for labor, namely: thermal therapy (20%); massage/sacral massage (15%); Swiss ball exercises (15%); acupressure (15%); auriculotherapy (10%); music therapy (10%); aromatherapy (5%); acupuncture (5%); and dance (5%). Conclusion: the non-pharmacological measures found in this review are efficient to promote pain reduction during labor, associated with a decrease in the use of drug interventions.

Resumo Objetivos: avaliar a efetividade das medidas não farmacológicas utilizadas por enfermeiros obstetras para o alívio da dor durante o trabalho de parto. Métodos: trata-se de uma revisão sistemática realizada nas bases de dados United States National Library of Medicine, Cumulative Index to Nursingand Allied Heal Literature, Scopus, Web of Science e Literatura Latino-americana e do Caribe em Ciências da Saúde, os descritores foram: labor pain, labor, obstetric e obstetric nursing. A busca e seleção seguiu as recomendações do PRISMA, aconteceu de agosto a setembro de 2020, foram elegíveis ensaios clínicos randomizados e foram analisados por meio de estatística descritiva. Resultados: 17 ensaios clínicos compuseram a amostra final, os quais destacam a utilização de medidas não farmacológicas com benefícios diversificados para o trabalho de parto, a saber: terapia térmica (20%); massagem/massagem sacral (15%); exercícios em bola suíça (15%); acupressão (15%); auriculoterapia (10%); musicoterapia (10%); aromaterapia (5%); acupuntura (5%); e dança (5%). Conclusão: as medidas não farmacológicas encontradas nesta revisão são eficientes para promover a redução da dor durante o trabalho de parto, associando-se com a diminuição do uso de intervenções medicamentosas.

Abstract Background Obtaining an adequate image of the needle by ultrasound reduces complications resulting from punctures, increasing patient safety and reducing hospitalization costs. Objectives To verify human perception in relation to number of pixels, while also identifying the best puncture angle and which needle should be used, and to evaluate whether there is a difference if needle visualization software is used. Methods 20 images were analyzed by 103 students who classified them as being sufficient or insufficient and were compared with the quality observed using photoshop. We evaluated whether there were differences between puncture angles of less than 45º and more than 45º, between IV catheter and introducer needles, and between images obtained with and without visualization software. Results There was a higher percentage of sufficient ratings for images those that had more than 60 pixels and when the puncture angle was less than 45º, with significant associations between students’ evaluations and each of these groups (p < 0.001). The percentages of images classified as sufficient were higher for images in which a IV catheter was used and also higher for those using the needle visualization software, with significant associations between the results for students’ classifications and each of these groups (p < 0.001). Conclusions The human eye classifies an image as sufficient according to higher numbers of pixels. Images of punctures at angles smaller than 45º in relation to the surface, of punctures performed with a IV catheter, and when using specific visualization software are also better detected by the human eye.

Resumo Contexto A obtenção de uma imagem adequada da agulha pelo ultrassom diminui complicações decorrentes de punções, trazendo segurança para os pacientes e diminuindo custos com hospitalização. Objetivos Verificar a percepção do olho humano em relação aos pixels, identificar qual o melhor ângulo da punção e qual agulha deve ser utilizada e avaliar se há diferença com o uso de software de visualização de agulha. Métodos Vinte imagens foram analisadas por 103 alunos, que as classificaram como sendo suficientes ou insuficientes, e comparadas com a qualidade observada pelo photoshop. Avaliou-se se havia diferença entre punções com menos de 45º e mais de 45º, entre cateter EV e agulha introdutora e entre imagens obtidas com e sem software de visualização. Resultados Houve um percentual mais elevado de imagens suficientes entre aquelas que tinham mais de 60 pixels e quando o ângulo era menor que 45º, com associação significativa entre a classificação das avaliações pelos alunos e cada um desses grupos (p < 0,001). O percentual de imagens suficientes foi maior nas imagens realizadas com cateter EV e naquelas que utilizaram o software de visualização da agulha, ocorrendo associação significativa entre os resultados da classificação pelos alunos e cada um desses grupos (p < 0,001). Conclusões O olho humano classifica a imagem como sendo suficiente de acordo com a maior quantidade de pixels. Imagens puncionadas com ângulos menores que 45º em relação à superfície, realizadas com cateter EV e utilizando software específico de visualização também são mais bem detectadas pelo olho humano.

Abstract Objective To validate an instrument on competence for managing Family Health Strategy (FHS) teams. Methods This is a descriptive and methodological research with a quantitative approach carried out between 2019 and 2020 with a focus on the nationwide validity of FHS management competence assessment instrument. The validity of competences was performed using the Delphi technique and internal consistency was examined using Cronbach’s alpha. Results Using the Delphi technique, performed with experts from areas related to the construct, 47 competences were validated, subdivided into three dimensions: knowledge, skills and attitudes. From then on, the 47 competences were broken down into 25 items of an assessment instrument. This instrument was applied with managers of Basic Health Units, in order to proceed with internal consistency assessment between items. The correlation matrix between the instrument items showed a strong correlation, which means that the instrument has high levels of reliability. Conclusion The validity process demonstrated internal consistency between items so that the instrument for assessing management competences proved to be suitable for application in research within the scope of FHS.

Resumen Objetivo Validar un instrumento de competencias para la gestión de equipos de Estrategia Salud de la Familia (ESF). Métodos Se trata de una investigación descriptiva y metodológica de enfoque cuantitativo realizada entre los años 2019 y 2020 centrada en la validación de ámbito nacional del Instrumento: Evaluación de las competencias de gestión en la ESF. La validación de las competencias se realizó por medio del método Delphi y la consistencia interna se examinó por medio del alfa de Cronbach. Resultados Mediante el método Delphi, realizado con especialistas de áreas relacionadas con el constructo, se validaron 47 competencias, subdivididas en tres dimensiones: conocimientos, habilidades y actitudes. A partir de ahí, se desglosaron las 47 competencias en 25 ítems del instrumento de evaluación. Este instrumento se aplicó con gerentes de unidades básicas de salud, con la finalidad de proceder a la evaluación de la consistencia interna entre los ítems. La matriz de correlación entre los ítems del instrumento evidenció una fuerte correlación, lo que representa que el instrumento tiene altos niveles de fiabilidad. Conclusión El proceso de validación demostró consistencia interna entre los ítems, de modo que el instrumento de evaluación de las competencias de gestión demostró ser apto para uso en investigaciones en el ámbito de la ESF.

A validação das competências ocorreu por meio da técnica Delphi e a consistência interna foi examinada por meio do Alpha de Cronbach. Conclusão O processo de validação demonstrou consistência interna entre os itens, de modo que o instrumento de avaliação das competências gerenciais mostrou-se apto para aplicação em pesquisas no âmbito da ESF.

BACKGROUND Chikungunya is a mosquito-borne virus that has been causing large outbreaks in the Americas since 2014. In Brazil, Asian-Caribbean (AC) and East-Central-South-African (ECSA) genotypes have been detected and lead to large outbreaks in several Brazilian states. In Rio Grande do Sul (RS), the southernmost state of Brazil, the first cases were reported in 2016. OBJECTIVES AND METHODS We employed genome sequencing and epidemiological investigation to characterise the Chikungunya fever (CHIKF) burden in RS between 2017-2021. FINDINGS We detected an increasing CHIKF burden linked to travel associated introductions and communitary transmission of distinct lineages of the ECSA genotype during this period. MAIN CONCLUSIONS Until 2020, CHIKV introductions were most travel associated and transmission was limited. Then, in 2021, the largest outbreak occurred in the state associated with the introduction of a new ECSA lineage. CHIKV outbreaks are likely to occur in the near future due to abundant competent vectors and a susceptible population, exposing more than 11 million inhabitants to an increasing infection risk.

ABSTRACT Objectives: to analyze the influence of sociodemographic and clinical variables, as well as the surgical checklist adherence score, on the occurrence of surgical site infection among patients undergoing myocardial revascularization. Methods: an observational, longitudinal, retrospective study was conducted at a university hospital, involving 266 medical records of patients who underwent myocardial revascularization surgery. Instruments containing sociodemographic, clinical, and infection-related variables were used, along with the Perioperative Surgical Safety Checklist. Descriptive, bivariate, and logistic regression analyses were employed. Results: surgical site infection occurred in 89 (33.5%) patients. There was a statistically significant association between body temperature outside the range of 36 degrees Celsius to 36.5 degrees Celsius (p=0.01), the presence of invasive devices (p=0.05), surgical procedures with the anticipation of critical events (p<0.001), and the occurrence of infection. Conclusions: body temperature, the presence of invasive devices, and surgical procedures with the anticipation of critical events were significant factors contributing to an increased risk of infection.

RESUMEN Objetivos: analizar la influencia de variables sociodemográficas y clínicas, así como del puntaje de adherencia a la lista de verificación quirúrgica en aparición de infección del sitio quirúrgico en pacientes sometidos a revascularización miocárdica. Métodos: estudio observacional, longitudinal y retrospectivo realizado en un hospital universitario con 266 expedientes de pacientes sometidos a cirugía de revascularización miocárdica. Se utilizaron instrumentos que contenían variables sociodemográficas, clínicas y relacionadas con la incidencia de infecciones del sitio quirúrgico, así como la Lista de Verificación de Seguridad Quirúrgica Perioperatoria. Se realizaron análisis descriptivos, bivariados y de regresión logística. Resultados: la infección del sitio quirúrgico se produjo en 89 (33,5%) pacientes. Hubo una asociación estadísticamente significativa entre la temperatura fuera del rango de 36°C a 36,5°C(p=0,01), la presencia de dispositivos invasivos(p=0,05) y los procedimientos quirúrgicos con previsión de eventos críticos(p<0,001) y la aparición de infección. Conclusiones: la temperatura corporal, presencia de dispositivos invasivos y los procedimientos quirúrgicos con previsión de eventos críticos fueron factores significativos para aumentar el riesgo de infección.

RESUMO Objetivos: analisar a influência de variáveis sociodemográficas e clínicas e do escore de adesão ao checklist cirúrgico sobre a ocorrência de infecção de sítio cirúrgico entre pacientes submetidos à revascularização miocárdica. Métodos: estudo observacional, longitudinal, retrospectivo, realizado em hospital universitário, com 266 prontuários de pacientes submetidos à cirurgia de revascularização miocárdica. Utilizaram-se instrumentos contendo variáveis sociodemográficas, clínicas e relacionadas à incidência de infecções de sítio cirúrgico; e a Lista de Verificação de Segurança Cirúrgica Perioperatória. Empregaram-se análises descritivas, bivariadas e regressão logística. Resultados: a infecção de sítio cirúrgico ocorreu em 89 (33,5%) pacientes. Houve associação estatisticamente significativa entre temperatura fora da faixa entre 36°C e 36,5°C (p=0,01), presença de dispositivos invasivos (p=0,05) e procedimentos cirúrgicos com previsão de eventos críticos (p<0,001) e ocorrência de infecção. Conclusões: temperatura corporal, presença de dispositivos invasivos e procedimentos cirúrgicos com previsão de eventos críticos foram fatores significativos para o aumento do risco de infecção.

Abstract Background Management delays imply worse outcomes in rheumatoid arthritis (RA) and, therefore, should be minimized. We evaluated changes in diagnostic and treatment delays regarding RA in the last decades in Brazil. Methods Adults fulfilling the ACR/EULAR (2010) criteria for RA were assessed. Delays in diagnosis and treatment, and the frequencies of early management initiation within thresholds (windows of opportunity) of 3, 6, and 12 months from symptoms onset were evaluated. The Mann–Kendall trend test, chi-squared tests with Cramer’s V effect sizes and analysis of variance were conducted. Results We included 1116 patients: 89.4% female, 56.8% white, mean (SD) age 57.1 (11.5) years. A downward trend was found in diagnostic (tau = - 0.677, p < 0.001) and treatment (tau = - 0.695, p < 0.001) delays from 1990 to 2015. The frequency of early management increased throughout the period, with ascending effect sizes across the 3-, 6-, and 12-month windows (V = 0.120, 0.200 and 0.261, respectively). Despite all improvements, even in recent years (2011–2015) the diagnostic and treatment delays still remained unacceptably high [median (IQR): 8 (4–12) and 11 (5–17) months, respectively], with only 17.2% of the patients treated within the shortest, 3-month window. Conclusion The delays in diagnosis and treatment of RA decreased during the last decades in Brazil. Improvements (effect sizes) were greater at eliminating extreme delays (≥ 12 months) than in attaining really short management windows (≤ 3 months). Very early treatment was still an unrealistic goal for most patients with RA.

Abstract Background Early rheumatoid arthritis (RA) offers an opportunity for better treatment outcomes. In real-life settings, grasping this opportunity might depend on access to specialized care. We evaluated the effects of early versus late assessment by the rheumatologist on the diagnosis, treatment initiation and long-term outcomes of RA under real-life conditions. Methods Adults meeting the ACR/EULAR (2010) or ARA (1987) criteria for RA were included. Structured interviews were conducted. The specialized assessment was deemed “early” when the rheumatologist was the first or second physician consulted after symptoms onset, and “late” when performed afterwards. Delays in RA diagnosis and treatment were inquired. Disease activity (DAS28-CRP) and physical function (HAQ-DI) were evaluated. Student's t, Mann-Whitney U, chi-squared and correlation tests, and multiple linear regression were performed. For sensitivity analysis, a propensity score-matched subsample of early- vs. late-assessed participants was derived based on logistic regression. The study received ethical approval; all participants signed informed consent. Results We included 1057 participants (89.4% female, 56.5% white); mean (SD) age: 56.9 (11.5) years; disease duration: 173.1 (114.5) months. Median (IQR) delays from symptoms onset to both RA diagnosis and initial treatment coincided: 12 (6–36) months, with no significant delay between diagnosis and treatment. Most participants (64.6%) first sought a general practitioner. Notwithstanding, 80.7% had the diagnosis established only by the rheumatologist. Only a minority (28.7%) attained early RA treatment (≤ 6 months of symptoms). Diagnostic and treatment delays were strongly correlated (rho 0.816; p < 0.001). The chances of missing early treatment more than doubled when the assessment by the rheumatologist was belated (OR 2.77; 95% CI: 1.93, 3.97). After long disease duration, late-assessed participants still presented lower chances of remission/low disease activity (OR 0.74; 95% CI: 0.55, 0.99), while the early-assessed ones showed better DAS28-CRP and HAQ-DI scores (difference in means [95% CI]: −0.25 [−0.46, −0.04] and − 0.196 [−0.306, −0.087] respectively). The results in the propensity-score matched subsample confirmed those observed in the original (whole) sample. Conclusions Early diagnosis and treatment initiation in patients with RA was critically dependent on early access to the rheumatologist; late specialized assessment was associated with worse long-term clinical outcomes.
