Resumo Justificativa e objetivos: Há poucos dados na literatura que caracterizam o padrão de uso de analgésicos na América Latina e no Brasil. Também se sabe pouco sobre o subtratamento da dor e sua influência no hábito de automedicação analgésica. O objetivo desta pesquisa é definir o padrão de uso de analgésicos entre os portadores de dor crônica (DC) e a sua potencial associação à automedicação analgésica. Método: Estudo observacional transversal com amostra de população urbana. A dor crônica foi definida como aquela presente por pelo menos 90 dias. A pesquisa foi aprovada pelo Comitê de Ética em Pesquisa institucional. Resultados: Foram incluídos 416 indivíduos; 45,7% (n = 190) portadores de dor crônica, sendo os do sexo feminino (72,3%; p = 0,04) os mais acometidos. A automedicação analgésica é praticada por 78,4% dos portadores de dor crônica. O tratamento analgésico vigente mais frequente é composto pelos anti-inflamatórios não esteroides (AINES), dipirona e paracetamol. Os opioides fracos são pouco usados e apenas 2,6% dos indivíduos com dor crônica fazem uso desses analgésicos. Nenhum dos indivíduos estava em uso de opioides potentes. Conclusões: A prática de automedicação analgésica é frequente entre os portadores de dor crônica, o que pode ser consequência da pouca prescrição de analgésicos mais potentes, como os opioides. Pode-se também dizer que, pelos dados apresentados, não ocorre uma crise de uso recreativo de opioides na população estudada.

Abstract Background and objectives: There are few data in the literature characterizing the pattern of analgesic use in Latin American countries, including Brazil. Little is known about the undertreatment of pain and its influence on the habit of self-medication with analgesics. The aim of this study is to define the pattern of analgesic use among chronic pain patients and its potential association with self-medication with analgesics. Method: Cross-sectional observational study with an urban population sample. Chronic pain was defined as a pain lasting for at least 90 days. The study was approved by the Research Ethics Committee of the institution. Results: 416 subjects were included; 45.7 % (n = 190) had chronic pain, with females (72.3 %; p = 0.04) being the most affected. Self-medication with analgesics is practiced by 78.4% of patients with chronic pain. The most common current analgesic treatment consists of non-steroidal anti-inflammatory drugs (dipyrone and acetaminophen). Weak opioids are rarely used and only 2.6% of subjects with chronic pain were taking these analgesics. None of the subjects were taking potent opioids. Conclusions: The practice of self-medication with analgesics is frequent among patients with chronic pain, which may be due to the underprescription of more potent analgesics, such as opioids. It can also be said that, given the data presented, there is no crisis of recreational opioid use in the studied population.

A incorporação de materiais vegetais específicos associados à solarização do solo tem sido um avanço promissor no controle de fungos fitopatogênicos habitantes do solo. O objetivo do trabalho foi avaliar determinados efeitos da incorporação e decomposição de brócolis, mamona, mandioca brava e mansa, no solo, em condições de microcosmo mantido em BOD (37±2ºC), sobre o micélio de Fusarium oxysporum f. sp. lycopersici Raça 2, Macrophomina phaseolina, Rhizoctonia solani AG-4 HGI e de Sclerotium rolfsii. Assim, quatro ensaios idênticos foram instalados em conjunto de microcosmos, com cinco tratamentos e quatro períodos de tempo diferentes e independentes (7, 14, 21 e 28 dias). O parâmetro avaliado foi os efeitos inócuo, fungistático e fungicida dos tratamentos sobre o micélio dos fungos. Verificou-se efeito fungistático e fungicida no crescimento micelial de F. oxysporum f. sp. lycopersici Raça 2, R. solani AG-4 HGI e de S. rolfsii. Os fungos que apresentaram efeito fungistático apresentaram uma velocidade média de crescimento micelial inferior ao controle geral, que consistiu na incubação dos fungos em temperatura de 25±2ºC. O efeito fungicida ocorreu aos 21 dias de incubação para F. oxysporum e R. solani e aos 28 dias para S. rolfsii. Para M. phaseolina, observou-se apenas efeito inócuo. Associação da temperatura de 37±2ºC mais o período de tempo dos tratamentos foi o fator responsável pelos efeitos fungistático e fungicida no micélio dos fitopatógenos estudados. Essa associação também interferiu na velocidade do crescimento micelial dos fungos que apresentaram efeito fungistático.

The incorporation of plant materials associated with soil solarization has been a promising progress to control soilborne phytopathogenic fungi. The aim of this study was to evaluate certain effects of incorporation and decomposition of broccolis, castor bean, wild and sweet cassava, in the soil, under microcosm conditions maintained in BOD (37±2ºC), on the mycelium of Fusarium oxysporum f. sp. lycopersici race 2, Macrophomina phaseolina, Rhizoctonia solani AG-4 HGI and Sclerotium rolfsii. Thus, four identical experiments were established with five treatments of four different and independent periods (7, 14, 21 and 28 days). The assessed parameter was the innocuous, fungistatic and fungicide effects of the treatments on the mycelium of fungi. Fungistatic and fungicide effect was verified for the mycelium of F. oxysporum f. sp. lycopersici race 2, R. solani AG-4 HGI and S. rolfsii. Fungi with fungistatic effect presented lower velocity of mycelial growth compared to the general control, which consisted of incubation of the fungi at a temperature of 25±2ºC. Fungicide effect occurred at 21 days of incubation for F. oxysporum and R. solani and at 28 days for S. rolfsii. Only innocuous effect was observed for M. phaseolina. The factor that caused fungistatic and fungicide effect on the mycelium of fungi was the association of the temperature of 37±2ºC with the period of the treatments. That association also interfered in the mycelial growth velocity of the fungi that showed fungistatic effect.

O objetivo deste trabalho é apresentar os resultados pós-operatórios preliminares de 16 pacientes submetidos a implante de bioprótese valvular de pericárdio bovino com preservação de suas valvas naturais, operados seqüencialmente no período de março a setembro de 1989. As cúspides insuficientes foram pregueadas no anel mitral com pontos em U passados nas bordas livres e no anel mitral, sem quaisquer ressecções. Foram feitos estudos comparativos no pré e no pós-operatório das seguintes grandezas: diâmetro do átrio esquerdo, diâmetro do ventrículo esquerdo (VE), fração de ejeção e gradiente transvalvares mitrais (pico e médio). A avaliação estatística foi feita através do teste T de Student. O valor médio do diâmetro do VE no pré-operatório era de 67,31 mm, caindo para 60,50 mm no pós-operatório. O valor crítico de T foi 2,131, sendo o valor observado para a variação do diâmetro de VE igual a 3,18. O diâmetro de átrio esquerdo (AE) variou de 60,25 mm para 49,31 mm com T igual a 6,72. O valor médio da fração de ejeção no pré-operatório foi de 0,72, variando para 0,65 com T igual a 2,68. Os gradientes (valores médios) transvalvares variaram de 23,7 mm de Hg de pico e 12,5 de média, respectivamente, para 9,10 e 5,125. Estes valores residuais são perfeitamente aceitáveis de acordo com a literatura e a própria experiência do Serviço. Não houve complicações ou óbitos na série. Em nenhum paciente ocorreu gradiente na via de saída de VE que pudesse sugerir obstrução pela cúspide anterior. Os autores concluem pela eficácia da técnica a qual é de fácil e rápida execução, afasta desinserções atrioventrículares, não gera gradientes, favorece ótimo desempenho ventricular com rápida e acentuada regressão dos diâmetros de AE e VE.

The preliminary postoperative results in 16 patients submitted to valvular replacement utilising bovine pericardial valvular bioprostheses, without resection of their natural cusps, are presented. These patients were operated on sequentially from March to September 1989. The incompetent cusps were sutured to the mitral annulus with U stitches placed at their free edges. Pre and postoperative evaluation of the following parameters were obtained: left atrial diameters, left ventricular diameters, ejection fraction and mitral transvalvular gradients (peak and mean). Statistical evaluation utilising the Student's T test was performed. Preoperative mean value of the left ventricular diameter was 67.31 mm, decreasing to 60.50 mm in the postoperative evaluation. Critical value of T was 2.131, the value observed for the left ventricle variation being 3.18. The left atrial diameter varied from 60.25 mm to 49.31 mm (T equal to 6.72). Preoperative ejection fraction mean value was 0.72 and in the postoperative was 0.65 (T = 2.68). Mean transvalvular gradients varied from 23.7 mm (peak) and 12.5 mm (mean) to 9.10 and 5.125 respectively. These residual values are acceptable according to the current literature and the experience of our Institution. There were no complications or deaths in the present series. In no patient gradients in the left ventricular out flow tract suggesting obstruction by the anterior cusp were observed. The authors consider it an efficient technique, easily and rapidly performed, without danger of atrioventricular desinsertions and without significant gradients. Ventricular performance is enhanced and the left atrial and left ventricular diameters decrease considerably.
