CONTEXT: A prevalência de obesidade tem aumentado significativamente nos últimos anos. Bypass gástrico em Y-de-Roux, uma das técnicas cirúrgicas realizadas no tratamento da obesidade, envolve exclusão de parte do estômago. Atrofia, metaplasia intestinal e câncer gástrico têm sido associados com infecção pelo Helicobacter pylori. OBJETIVOS: Avaliar a prevalência de achados endoscópicos e alterações histopatológicas em pacientes obesos mórbidos em relação à presença de células inflamatórias, atividade inflamatória, hiperplasia linfóide, infecção pelo H. pylori, atrofia e metaplasia intestinal na mucosa gástrica. MÉTODOS: Achados na endoscopia digestiva alta e histopatologia gástrica foram analisados em 126 pacientes obesos na avaliação pré-operatória de cirurgia bariátrica. RESULTADOS: Anormalidades endoscópicas do trato digestivo superior foram diagnosticadas em 73/126 pacientes, com três pacientes (2,4%) apresentando úlcera gástrica e um paciente (0,8%) com úlcera duodenal. A histopatologia das biopsias gástricas dos obesos estudados revelou 65,1% de processo inflamatório na mucosa, atividade inflamatória em 50,0%; infecção pelo H. pylori em 53,2%, hiperplasia linfóide em 50,0%, atrofia e/ou metaplasia intestinal em 16,7%. CONCLUSÕES: No presente estudo, através de endoscopia digestiva alta e histopatologia de biopsias gástricas, foram detectados 57,9% de pacientes com anormalidades endoscópicas, prevalência de infecção pelo H. pylori em 53% e atrofia e/ou metaplasia intestinal em 16,7%.

CONTEXT: The prevalence of obesity has been increasing in modern society. Roux-en-y gastric bypass is a bariatric surgery that involves the exclusion of significant part of the stomach. Atrophy, intestinal metaplasia and gastric cancer have been associated with infection by Helicobacter pylori. OBJECTIVES: To evaluate the presence of endoscopy findings and histological changes in morbid obese patients for the presence of inflammatory cells, inflammatory activity, lymphoid hyperplasia, H. pylori infection, atrophy and intestinal metaplasia in the gastric mucosa. METHODS: Upper digestive endoscopy and gastric histopathological were studied in 126 obese patients in the preoperative evaluation for bariatric surgery. RESULTS: Upper digestive endoscopy abnormalities were diagnosed in 73/126 (57.9%) patients. In three patients (2.4%) the upper gastrointestinal endoscopy diagnosed gastric ulcer and one patient (0.8%) had duodenal ulcer. The histopathological from gastric biopsies of these obese patients showed 65.1% of mucosa inflammation, inflammatory activity in 50.0%, infection by H. pylori in 53.2%, lymphoid hyperplasia in 50.0% and atrophy and/or intestinal metaplasia in 16.7%. CONCLUSIONS: In present study, with routine preoperative upper gastrointestinal endoscopy and histopathological examination, were detected 57.9% patients with endoscopy abnormalities, high prevalence of infection by H. pylori (53%) and 16.7% of gastric atrophy and/or intestinal metaplasia.

CONTEXTO: O câncer gástrico é a segunda causa mais comum de mortes relacionadas à neoplasia em todo o mundo. Embora o Helicobacter pylori seja classificado como um carcinógeno classe I, a presença da infecção não é um fator que isoladamente possa conduzir ao câncer gástrico e, uma das possíveis justificativas, é a existência de diferentes linhagens de H. pylori com diferentes graus de virulência. OBJETIVO: Investigar a associação entre H. pylori cagA-positivo e câncer gástrico, utilizando a reação em cadeia de polimerase (PCR) para a detecção desta linhagem bacteriana. MÉTODOS: Vinte e nove pacientes com câncer gástrico foram pareados por sexo e por idade (± 5 anos) com 58 pacientes sem câncer gástrico, submetidos a endoscopia digestiva alta. Todos os pacientes foram avaliados quanto à presença de infecção pelo H. pylori (com teste da urease, análise histológica e PCR para os genes ureA e 16SrRNA) e pela linhagem cagA desta bactéria (com PCR para o gene cagA). RESULTADOS: Avaliando a presença de infecção por H. pylori cagA-positivo, verificou-se que a taxa da infecção era significativamente mais alta no grupo de pacientes com câncer gástrico, quando comparado com o grupo controle, ocorrendo em 62,1% e em 29,3%, respectivamente (OR = 3,95; CI 95% 1,543-10,096). CONCLUSÕES: Há associação entre H. pylori cagA-positivo e risco de câncer gástrico.

CONTEXT: Gastric cancer is the second most common cause of cancer related death worldwide. Although Helicobacter pylori has been classified as a class I carcinogen, the presence of infection is not a factor that alone is able to lead to gastric cancer, and one of the possible explanations for this is the existence of different strains of H. pylori with different degrees of virulence. OBJECTIVES: To investigate the association between cagA-positive H. pylori and gastric cancer, using polymerase chain reaction (PCR) for the detection of this bacterial strain. METHODS: Twenty-nine patients with gastric cancer were matched by sex and age (± 5 years) with 58 patients without gastric cancer, submitted to upper gastrointestinal endoscopy. All patients were evaluated for the status of infection by H. pylori (through urease test, histological analysis and PCR for the genes ureA and 16SrRNA) and by cagA-positive strain (through PCR for cagA gene). RESULTS: Evaluating the presence of infection by cagA-positive H. pylori, it was verified that the rate of infection was significantly higher in the group with gastric cancer when compared with the matched controls, occurring in 62.1% and 29.3%, respectively (OR = 3.95; CI 95% 1.543-10.096). CONCLUSIONS: There is an association between cagA-positive H. pylori strain and risk of gastric cancer.

RACIONAL: Esôfago de Barrett de segmento curto é definido pela presença de mucosa semelhante à gástrica no esôfago distal, extensão <3 cm, com metaplasia intestinal na histopatologia. Esôfago de Barrett é fator de risco ao adenocarcinoma do esôfago. Enquanto esôfago de Barrett se desenvolve como resultado do refluxo gastroesofágico crônico, metaplasia intestinal da cárdia é conseqüência da infecção crônica pelo Helicobacter pylori e está associada à metaplasia gástrica distal. Epitélio colunar com metaplasia intestinal localizada no esôfago distal, extensão <3 cm (condição denominada de esôfago de Barrett de segmento curto) ou a mesma entidade localizada no estômago proximal (condição denominada de metaplasia intestinal da cárdia) por ser muitas vezes confundida. OBJETIVO: Estudar a associação de esôfago de Barrett segmento curto (extensão <3 cm) com metaplasia intestinal gástrica (corpo e antro) e infecção pelo Helicobacter pylori. PACIENTES E MÉTODOS: Participaram do estudo 89 pacientes com mucosa do esôfago distal semelhante à gástrica, extensão <3 cm. Os pacientes foram questionados sobre sintomas de doença do refluxo gastroesofágico. Biopsias foram obtidas imediatamente abaixo da transição escamocolunar e do corpo e antro gástrico para investigação de metaplasia intestinal e pesquisa de Helicobacter pylori. RESULTADOS: Quarenta e dois dos 89 (47,2%) pacientes foram diagnosticados com metaplasia intestinal esofágica na histopatologia. A idade média foi significativamente mais alta no grupo com metaplasia intestinal esofágica. Nos dois grupos estudados (com e sem metaplasia intestinal de esôfago distal), a relação masculino/feminino, sintomas de doença do refluxo gastroesofágico e infecção pelo Helicobacter pylori foi semelhante. Metaplasia intestinal gástrica (corpo e antro) foi diagnosticada em 21 de 42 (50,0%) dos pacientes com metaplasia intestinal esofágica e 7 de 47 (14,9%) do grupo com mucosa colunar esofágica, mas sem evidência de metaplasia intestinal. CONCLUSÃO: Metaplasia intestinal é achado freqüente em pacientes com mucosa semelhante à gástrica no esôfago distal, extensão <3 cm. No presente estudo, metaplasia intestinal de segmento curto no esôfago está associada à metaplasia gástrica distal. A freqüência de sintomas de refluxo gastroesofágico e infecção pelo Helicobacter pylori não mostrou diferenças significativas nos dois grupos estudados.

BACKGROUND: Short segment Barrett's esophagus is defined by the presence of <3 cm of columnar-appearing mucosa in the distal esophagus with intestinal metaplasia on histophatological examination. Barrett's esophagus is a risk factor to develop adenocarcinoma of the esophagus. While Barrett's esophagus develops as a result of chronic gastroesophageal reflux disease, intestinal metaplasia in the gastric cardia is a consequence of chronic Helicobacter pylori infection and is associated with distal gastric intestinal metaplasia. It can be difficult to determine whether short-segment columnar epithelium with intestinal metaplasia are lining the esophagus (a condition called short segment Barrett's esophagus) or the proximal stomach (a condition called intestinal metaplasia of the gastric cardia). AIMS: To study the association of short segment Barrett's esophagus (length <3 cm) with gastric intestinal metaplasia (antrum or body) and infection by H. pylori. PATIENTS AND METHODS: Eight-nine patients with short segment columnar-appearing mucosa in the esophagus, length <3 cm, were studied. Symptoms of gastroesophageal reflux disease were recorded. Biopsies were obtained immediately below the squamous-columnar lining, from gastric antrum and gastric corpus for investigation of intestinal metaplasia and H. pylori. RESULTS: Forty-two from 89 (47.2%) patients were diagnosed with esophageal intestinal metaplasia by histopathology. The mean-age was significantly higher in the group with esophageal intestinal metaplasia. The two groups were similar in terms of gender (male: female), gastroesophageal reflux disease symptoms and H. pylori infection. Gastric intestinal metaplasia (antrum or body) was diagnosed in 21 from 42 (50.0%) patients in the group with esophageal intestinal metaplasia and 7 from 47 (14.9%) patients in the group with esophageal columnar appearing mucosa but without intestinal metaplasia. CONCLUSION: Intestinal metaplasia is a frequent finding in patients with <3 cm of columnar-appearing mucosa in the distal esophagus. In the present study, short segment intestinal metaplasia in the esophagus is associated with distal gastric intestinal metaplasia. Gastroesophageal reflux disease symptoms and H. pylori infection did not differ among the two groups studied.

OBJETIVO: O Rio Grande do Sul apresenta significativos coeficientes de mortalidade ao câncer de esôfago, com diferenças importantes nas microregiões, sugerindo a ação de fatores de riscos externos. Este estudo tem como objetivo mostrar a relação do câncer de esôfago com o hábito de fumar, ingerir álcool e mate. CASUÍSTICA E METODOLOGIA: Foram entrevistados 55 pacientes com câncer de esôfago e 110 pacientes controles, sem evidências de tumor à endoscopia. RESULTADOS: Não foram observadas diferenças em relação ao sexo e idade entre casos e controles (p>0,05). Entre os casos foram verificados mais freqüentemente a presença de indivíduos cuja profissão estava ligada à atividade agrícola (RC 3,3; 95 % IC 0,9-11,2), pai com antecedente de câncer (RC 6,9; 95 % IC 1,9-25,6), fumantes e ex-fumantes (RC 2,5; 95 % IC 1,1-5,9), tomadores e ex-tomadores de bebidas alcoólicas (RC 5,3; 95 % IC 2,6-11,0), tomadores e ex-tomadores de mate (RC 3,6; 95 % IC 1,3-9,8). Outras variáveis mais freqüentes entre os casos de câncer de esôfago: fumantes > 20 anos, entre os tipos de álcool o uso de cachaça, ingestão de álcool diariamente, ingestão de mate diariamente e o hábito de adicionar sal nos alimentos. CONCLUSÕES: Fumo, álcool, mate, agricultores e antecedentes de pai com câncer foram significativamente mais freqüentes entre os casos de câncer de esôfago em relação aos controles. Mate foi significativo, independente da quantidade ingerida/dia.

BACKGROUND: Rio Grande do Sul, a State in southern Brazil, shows high mortality rates for esophageal cancer. Important geographical variations suggest external factors in the etiology. OBJECTIVE: The relationship between smoking, alcohol and "maté" to esophageal cancer. METHOD: 55 patients with esophageal cancer were interviewed, soon after the endoscopy, about risk factors; 110 patients who did not showed esophageal tumor in the endoscopy were the controls. RESULTS: among the patients with esophageal cancer there were more farmer-workers (OD 3.3; 95% CI 0.9-11.2), father with antecedent cancer (OD 6.9; 95% CI 1.9-25.6), smoking and ex-smoking (OD 2.5; 95 % CI 1.1-5.9), drinkers and ex- of alcohol (OD 5.3; 95% CI 2.6-11.0), drinkers and ex- of "maté" (OD 3.6; 95 % CI 1.3-9.8). Others variables more frequent in the cases were: smoking for > 20 years; kind of alcohol (spirits), drinking alcohol every day, drinking "maté" every day and adding salt to food. CONCLUSIONS: Smoking, alcohol, "maté", farmer-workers and antecedent of father with cancer were significantly more frequent in the cases of esophageal cancer than in the controls.

O estudo de 214 mielocintilografias feitas em 200 pacientes permitiu aos autores chegar às seguintes conclusões: 1) a mielocintilografia com RIHSA - 131I oferece novas possibilidades no campo do diagnóstico neurológico, principalmente no que tange aos processos expansivos, inflamatorios e malformações, sendo limitadas suas possibilidades no diagnóstico de hérnias dos núcleos pulposos; 2) a mielocintilografia não substitui métodos mielográficos convencionais, os dois métodos devendo ser usados de maneira a se completarem; 3) há casos em que a mielocintilografia com RIHSA apresenta vantagem sobre a mielografia com os meios de contraste positivos, principalmente no diagnóstico de aracnoidites, avulsões de raízes e malformações do canal vertebral; 4) a mielocintilografia com RIHSA tornou-se um método de uso corrente no diagnóstico neurológico, devido à facilidade de execução do exame, à boa tolerância desta substância pelo organismo, à sua rápida absorção e completa eliminação.

The study of 214 myeloscintilographies made on 200 patients have led the authors to the following conclusions: 1) myeloscintilography with RIHSA opens up a series of new possibilities in the field of neurological diagnosis, especially concerning with malformations, inflammatory and expansive processes, its possibilities in cases of disc herniations being of a narrower scope; 2) the method does not intend to substitute the conventional myelography with contrast media, both methods having to be used so as to complement each other; 3) there are cases in which myeioscintilography with RIHSA presents advantages over myelography with the positive means of contrast, especially in the diagnosis of arachnitis, nerve avulsions and malformations of the vertebral canal; 4) myeloscintilography with RIHSA has become a method of current use in neurological diagnosis because of the facility with which the examination is made, by the good tolerance of the drug, by its rapid absorption and complete elimination.

Das Studium von 214 Myeloszintigraphien, die in 200 Patienten durchgeführt wurden, làsst feststellen: 1) die Myeloszintigraphie bietet eine Reihe neuer Mõglichkeiten in Neurologie, besonders was die Diagnostik raumbeschrãnkender und arachnoiditischer Prozessen sowie der Missbildungen betrifft; 2) sie kann und will kein Ersatz der Kontrastmittelmyelo-graphie sein, die beiden Methoden Zusammen erweitern das diagnostiche Rüstzeug der Neurologie; 3) in mancher Hinsicht dürfte sie gegenüber der Anwendung von Kontrastmitteln, besonders in der Diagnose von Arachoiditiden, Wurzelausrisse und Missbildungen des Spinalkanals Vorteile haben; 4) sie ist durch technisch leichte Handhabung, gute Vertraglichkeit, rasche Resorption und vollstãndige Elimination der radioaktiven Substanz, und gute diagnostische Leistungsfáhigkeit ein beachtenswertes Verfahren.
