Abstract Objective Pregnancy in women with lupus poses a higher risk of complications compared with the general population. The present study aimed to determine and describe the obstetric and neonatal outcomes of pregnant women with lupus. Materials and Methods We conducted an observational retrospective study of pregnant women with the diagnosis of lupus, who were selected and followed at the Maternal-Fetal Medicine Clinic of our institution between January 2013 and July 2018. We analyzed 59 pregnancies and 52 newborns, and collected data regarding sociodemographic features, the preconception period, pregnancy, childbirth, postpartum and the newborn. A descriptive analysis of the variables was performed. Results In 58% of the cases, the pregnancy was uneventful. We registered flares in 25% of the cases, preeclampsia in 3%, fetal growth restriction in 12%, gestational loss in 10%, preterm labor in 10%, postpartum complications in 20%, and small for gestational age newborns in 17% of the cases. Conclusions Most pregnancies in women with lupus have favorable obstetric and neonatal outcomes. Prenatal counseling, adequate multidisciplinary surveillance, and optimized treatment of the disease are fundamental pillars for these good results.

Resumo Objetivo A gravidez em mulheres com lúpus representa um risco maior de complicações em comparação com a população em geral. O presente estudo teve como objetivo determinar e descrever os resultados obstétricos e neonatais de gestantes com lúpus. Materiais e Métodos Realizamos um estudo retrospectivo observacional de gestantes com diagnóstico de lúpus, selecionadas e acompanhadas no Ambulatório de Medicina Materno-Fetal de nossa instituição entre janeiro de 2013 e julho de 2018. Analisamos 59 gestações e 52 recém-nascidos e coletamos dados referentes às características sociodemográficas, período pré-concepcional, gravidez, parto, pós-parto e nascimento. Foi realizada uma análise descritiva das variáveis. Resultados Em 58% dos casos, a gravidez transcorreu sem intercorrências. Registramos surtos em 25% dos casos, pré-eclâmpsia em 3%, restrição do crescimento fetal em 12%, perda gestacional em 10%, trabalho de parto prematuro em 10%, complicações pós-parto em 20% e recém-nascidos pequenos para a idade gestacional em 17% dos casos. Conclusões A maioria das gestações em mulheres com lúpus tem resultados obstétricos e neonatais favoráveis. Aconselhamento pré-natal, vigilância multidisciplinar adequada e tratamento otimizado da doença são pilares fundamentais para esses bons resultados.

Abstract The challenge of understanding the social and academic needs of deaf children in the educational context has been the subject of deep analysis in the literature, but the same does not happen in relation to deaf mothers and the performance of their role as responsible for the education of their children. Often, mothers who are deaf face communicational and attitudinal barriers, requiring adjustments in the family-school relationship, in order to guarantee them the fundamental right to participate in school decisions and in the school life of their students. This study aimed to explore the perspective of mothers who are deaf on their involvement in the school life of their hearing children, in order to identify areas of political action and practices for mobilizing facilitators of family-school relationships. For this, a focus group was held with 10 mothers who are deaf, whose children attended basic education – 1st and 2nd cycles – in Portuguese public schools. The results suggest that, despite the mothers’ willingness and commitment to get involved in their children’s school life, this participation is restricted by the way in which communication and the family-school relationship is traditionally structured. The perceived barriers are associated with the avoidance of situations of interaction with the school, in addition to the concern for the lack of knowledge of their children’s school life. The widespread existence of interpreters in schools or the training of the school community for the use of sign language are some of the changes discussed by mothers who are deaf in achieving their right to participate in the school life of their children.

Resumo O desafio de entender as necessidades sociais e acadêmicas de crianças surdas no contexto educativo tem sido alvo de profunda análise na literatura, mas o mesmo não acontece em relação às mães surdas e ao desempenho do papel de responsáveis pela educação dos seus filhos. Com frequência, as mães surdas enfrentam barreiras de natureza comunicacional e atitudinal, tornando necessárias adequações na relação família-escola, de modo a lhes garantir o direito fundamental de participar nas decisões da escola e da vida escolar dos seus educandos. Assim, com este estudo pretendeu-se explorar a perspectiva de mães surdas sobre o seu envolvimento na vida escolar dos seus filhos ouvintes procurando identificar linhas de ação política e do domínio das práticas que visem a mobilização de facilitadores da relação família-escola. Para isso, realizou-se um grupo focal com 10 mães surdas, cujos filhos frequentavam o ensino básico – 1.º e 2.º ciclos –, de escolas públicas portuguesas. Os resultados sugerem que, apesar da vontade e do empenho das mães surdas para se envolverem na vida escolar dos filhos, esta participação é restringida pela forma como a comunicação e a relação família-escola está tradicionalmente estruturada. As barreiras sentidas estão associadas ao evitamento de situações de interação com a escola, acrescido da preocupação pelo desconhecimento da vida escolar dos seus filhos. A existência generalizada de intérpretes nas escolas ou a formação/capacitação da comunidade escolar para o uso de língua gestual são algumas das mudanças discutidas pelas mães surdas na consecução do seu direito à participação na vida escolar dos seus filhos.

Resumo O objetivo deste estudo foi: (1) descrever as variações semanais de carga aguda (CA), razão da carga de trabalho aguda:crônica, dor musuclar tardia (DOT) e fadiga; (ii) analisar as variações da carga semanal de trabalho e bem-estar entre três períodos da temporada (P1, P2 e P3); e (iii) analisar as relações entre carga de trabalho e medidas de bem-estar. Quinze jogadores profissionais de basquetebol de um clube de primeira liga Européia foram monitorados ao longo de uma temporada utilizando a escala CR-10 Borg e o questionário de Hooper. A CA semanal e a razão da carga de trabalho aguda:crônica (RCTAC) foram calculadas semanalmente para monitorar a carga interna. Além disso, os valores de DOT e fadiga foram calculados semanalmente. Maiores valores da CA, DOT e fadiga foram ensontrados durante o início da temporada e o maior valor de RCTAC foi encontrado durante o segundo período. No geral, a CA apresentou grandes correlações com a DOT (r = 0,60) e fadiga (r = 0,62). Os resultados deste estudo indicam que a carga é maior no primeiro período e depois diminui ao longo da temporada. Os resultados também motraram que a CA está mais relacionada com os parâmetros de bem-estar do que a RCTAC.

Abstract The aims of this study were: (i) to describe weekly variations of acute load (AL), acute:chronic workload ratio, delayed onset muscle soreness (DOMS), and fatigue; (ii) to analyze variations of weekly workload and well-being in three periods of the season (P1, P2, and P3); and (iii) to analyze the relationships between workload and well-being measures. Fifteen professional basketball players from a first-league European club were monitored throughout the season using the CR-10 Borg scale and the Hooper questionnaire. Weekly AL and acute:chronic workload ratio (ACWR) were weekly calculated for monitoring of the internal load. In addition, DOMS and fatigue values were weekly calculated. Greater AL, DOMS, and fatigue values were found during the early season, and the highest ACWR value was found during the second period. Overall, AL presented large correlations with DOMS (r=0.60) and fatigue (r=0.62). The results of this study indicate that load is higher in the first period and then decreases throughout the season. The results also showed that AL is more closely related to well-being parameters than ACWR.

Introduction: The use of iodinated contrast angiography in the diagnostic and therapeutic approach to arteriovenous access (AVA) problems is limited by nephrotoxicity and allergic reactions. The use of alternatives such as CO2 or ultrasound guidance has increased over the last few decades. Purpose: To evaluate the results and complications of the diagnostic and therapeutic procedures of AVA performed at our centre without the use of iodinated contrast. Methods: We evaluated retrospectively all patients submitted to endovascular diagnostic and/or therapeutic procedures using CO2 or ultrasound at our hospital between January 2013 and December 2016. Results: 30 procedures were performed in 22 patients: 15 ultrasound-guided angioplasties and 15 CO2 procedures (14 angioplasties and 1 diagnostic angiography). The mean age of patients was 74±10 years (41--94). Most procedures (90%) were performed on proximal arteriovenous fistulas (AVFs) and 3 were performed on distal AVFs. Iodinated contrast was not used because of its potential nephrotoxicity. Ultrasound-guided angioplasty was performed to treat outflow stenoses in the majority (73%) of the cases. The result was favourable in all procedures. One was complicated by venous rupture. Regarding angioplasties with CO2, outflow stenoses were the most treated (38%). The result was favourable in all procedures. Diagnostic angiography with CO2 was performed in a patient with signs of limb venous hypertension and was inconclusive. There were two complications in CO2 procedures: formation of pseudoaneurysm at the place of insertion of the introducer (n=1) and transient headache and nausea (n=1). Three of the patients who underwent angioplasty with CO2 and one patient who underwent ultrasound-guided procedure were submitted to balloon-assisted maturation. Conclusion: In our experience, the use of alternative methods with no iodinated contrast (CO2/ultrasound) in the evaluation and treatment of vascular access problems in patients with renal dysfunction is effective and safe

RESUMO CONTEXTO E OBJETIVO: Rinoplastia é uma das operações mais procuradas por indivíduos com o transtorno dismórfico corporal. O objetivo deste estudo foi adaptar culturalmente e validar a Body Dysmorphic Symptoms Scale. DESENHO E LOCAL: Estudo de validação cultural desenvolvido no ambulatório de cirurgia plástica de um hospital universitário público. MÉTODOS: Oitenta pacientes consecutivos de ambos os gêneros que desejavam submeter-se à rinoplastia foram selecionados entre fevereiro de 2014 e março de 2015. Trinta pacientes participaram da fase de adaptação cultural do instrumento. A reprodutibilidade foi testada em 20 pacientes e a validade de construto em 50 pacientes, correlacionando-se a escala com a Yale-Brown Obsessive Compulsive Scale para transtorno dismórfico corporal. RESULTADOS: A versão brasileira do instrumento mostrou alfa de Cronbach de 0,805 e excelente reprodutibilidade interobservador (coeficiente de correlação intraclasse, CCI = 0,873; P < 0,001) e intraobservador (CCI = 0,939; P < 0,001). Houve diferença significante entre os escores totais de pacientes com e sem sintomas (P < 0,001). Observou-se forte correlação (r = 0,841; P < 0,001) entre a Yale-Brown Obsessive Compulsive Scale para transtorno dismórfico corporal e a Body Dysmorphic Symptoms Scale. A área sob a curva característica de operação do receptor (ROC) foi de 0,981, revelando boa acurácia para discriminar a presença de sintomas para transtorno dismórfico corporal; 46% dos pacientes apresentaram sintomas do transtorno dismórfico corporal e 54% dos pacientes apresentaram sintomas obsessivo-compulsivos moderados a graves relacionados com a aparência. CONCLUSÃO: A versão brasileira da Body Dysmorphic Symptoms Scale é um instrumento reprodutível que apresenta validade de face, conteúdo e construto.

ABSTRACT: CONTEXT AND OBJECTIVE: Rhinoplasty is one of the most sought-after esthetic operations among individuals with body dysmorphic disorder. The aim of this study was to cross-culturally adapt and validate the Body Dysmorphic Symptoms Scale. DESIGN AND SETTING: Cross-cultural validation study conducted in a plastic surgery outpatient clinic of a public university hospital. METHODS: Between February 2014 and March 2015, 80 consecutive patients of both sexes seeking rhinoplasty were selected. Thirty of them participated in the phase of cultural adaptation of the instrument. Reproducibility was tested on 20 patients and construct validity was assessed on 50 patients, with correlation against the Yale-Brown Obsessive Compulsive Scale for Body Dysmorphic Disorder. RESULTS: The Brazilian version of the instrument showed Cronbach's alpha of 0.805 and excellent inter-rater reproducibility (intraclass correlation coefficient, ICC = 0.873; P < 0.001) and intra-rater reproducibility (ICC = 0.939; P < 0.001). Significant differences in total scores were found between patients with and without symptoms (P < 0.001). A strong correlation (r = 0.841; P < 0.001) was observed between the Yale-Brown Obsessive Compulsive Scale for Body Dysmorphic Disorder and the Body Dysmorphic Symptoms Scale. The area under the receiver operating characteristic curve was 0.981, thus showing good accuracy for discriminating between presence and absence of symptoms of body dysmorphic disorder. Forty-six percent of the patients had body dysmorphic symptoms and 54% had moderate to severe appearance-related obsessive-compulsive symptoms. CONCLUSIONS: The Brazilian version of the Body Dysmorphic Symptoms Scale is a reproducible instrument that presents face, content and construct validity.

OBJETIVOS: Analisar o padrão topográfico da gastrite por H. pylori em relação ao status cagA e associação com úlcera duodenal (UD) e carcinoma gástrico precoce (CaGp). MATERIAL E MÉTODO: Estudamos prospectivamente 160 biopsias endoscópicas e 40 peças de gastrectomia por CaGp. Amostras do antro e do corpo na pequena e grande curvaturas, da incisura e do tumor, foram processadas rotineiramente para histologia com histoquímica para caracterização da metaplasia intestinal (MI). O Helicobacter pylori foi avaliado por histologia, imuno-histoquímica, cultura e reação em cadeia de polimerase (PCR) e o status cagA por PCR. A gastrite crônica (GC) foi classificada segundo o sistema Sydney. Realizou-se estudo topográfico em 130 pacientes, analisando os parâmetros de gastrite comparativamente entre as áreas e em relação ao status cagA. RESULTADOS: Cento e vinte e um pacientes apresentaram GC, 24 UD e 14 eram normais. Detectaram-se amostras cagA-positivas em 59 pacientes com GC, 17 com UD e 24 com CaGp. Todos os parâmetros de GC foram significativamente mais intensos nos pacientes infectados por amostras cagA-positivas, que se associavam ainda à presença de atrofia e MI. A MI foi significativamente mais intensa na porção média da pequena curvatura antral do que nas demais regiões. Nos pacientes com GC e CaGp, infectados por amostras cagA-positivas, inflamação e atividade acometiam igualmente o antro e corpo distal, enquanto a atrofia e a MI predominavam no antro, particularmente na pequena curvatura do antro médio. Nos pacientes cagA-negativos a GC era predominantemente antral, e na UD a GC apresentava predominância antral, independentemente do status cagA. CONCLUSÕES: Nossos dados sugerem que o padrão de gastrite por H. pylori se relaciona com fatores de virulência da bactéria. As lesões pré-neoplásicas são significativamente mais intensas na pequena curvatura do antro médio, onde surge a maioria dos carcinomas gástricos (CaG).

AIMS: We aimed to investigate the topographical pattern of H. pylori gastritis in relation to cagA status and association with duodenal ulcer (DU) and early gastric carcinoma (EGCa). MATERIALS AND METHOD: We studied chronic gastritis (CG) in 160 patients submitted to upper endoscopy and in 40 patients with EGCa submitted to gastrectomy. Samples were taken from the antrum and corpus in the lesser and greater curvatures, incisura and from the tumour and were processed routinely for histology and histochemistry for intestinal metaplasia (IM) characterisation. H. pylori was evaluated by histology, immunohistochemistry, culture, and PCR; cagA status was evaluated by polymerase chain reaction (PCR). CG was analyzed according to the updated Sydney System. In 130 patients a topographic study was conducted by the comparative analysis of the gastritis parameters among the areas and according to cagA status. RESULTS: CG was observed in 121 patients, 24 had UD and 14 were normal. cagA-positive strains were detected in 59 patients with GC, 17 DU and 24 EGCa. All gastritis parameters were significantly more intense in the cagA-positive group than in cagA-negative one. Infection by cagA-positive strains was associated with atrophy and IM. IM was significantly more intense in the middle portion of the lesser curvature in the antral mucosa. In CG or EGCa patients infected with cagA-positive strains, inflammation and activity compromised equally the antrum and distal corpus, whereas atrophy and IM were more frequently found in the antral mucosa. In DU, CG was antral-predominant irrespectively of cagA status. CONCLUSIONS: According to our data the topographical pattern of H. pylori-associated gastritis seems to be associated with cagA status. Precancerous lesions were significantly more intense in the antral lesser curvature, where most gastric carcinomas arise.

Esta pesquisa teve por objetivo identificar dentre os trabalhadores de enfermagem, de quatro hospitais da região de Ribeirão Preto-SP, aqueles que foram acometidos por acidente de trabalho com material pérfuro-cortante e encaminhados para avaliação a um serviço especializado no tratamento de doenças infecciosas, os que foram contaminados e as condutas adotadas diante do acidente. Trata-se de pesquisa de campo de caráter descritivo. A amostra foi constituída por 30 sujeitos e os dados foram coletados através de consulta ao prontuário médico dos trabalhadores. Os resultados revelaram que nenhum dos trabalhadores foi contaminado pelos vírus HBV, HCV, ou HIV, no entanto, foi observado que apenas 23,33% dos trabalhadores compareceram a todos os retornos agendados para verificação de possível soroconversão. Em relação às condutas tomadas diante do acidente, foi indicado o uso de quimioprofilaxia em 76,67% dos casos, exames sorológicos em 100% dos casos e imunização contra hepatite em 9,99% dos casos. Devido à grande ocorrência de acidentes percutâneos, segundo estimativas oficiais de outros países, concluímos que maior atenção deva ser direcionada para a prevenção desses acidentes, bem como ao rigor do seguimento pós-exposição ocupacional.

This study aimed to identify, among nursing workers from four hospitals in the region of Ribeirão Preto-SP, Brazil, victims of occupational accidents with cutting and piercing material, who were sent for evaluation at a service specialized in treating infectious diseases, individuals who were contaminated and the conduct adopted as a result of the accidents. This is a descriptive field research. The sample consisted of 30 subjects and data were collected by consulting the workers' medical files. The results showed that none of the workers had been contaminated by HBV, HCV or HIV. However, it was observed that only 23.33% of them had kept all the scheduled appointments in order to verify a possible serum conversion. Concerning the conducts adopted as a result of the accident, the use of chemoprophylaxis was recommended in 76.67% of the cases, serological tests in 100% and immunization against Hepatitis in 9.99%. Due to the high occurrence of percutaneous accidents, according to official estimates from other countries, it was concluded that more attention must be given to the prevention of these accidents, as well as to the strict follow-up of workers after occupational exposure.

Esta investigación tuvo como objetivo identificar entre los trabajadores de enfermería de cuatro hospitales de la región de Ribeirão Preto, SP, Brasil, afectados por accidente de trabajo con material corto-punzante y remitidos para evaluación a un servicio especializado en el tratamiento de enfermedades infecciosas, los que fueron contaminados y las conductas adoptadas con relación al accidente. Se trata de una investigación de campo de carácter descriptivo. La muestra estuvo constituida por 30 sujetos y los datos fueron recolectados a través de consulta al informe médico de los trabajadores. Los resultados revelaron que ninguno de los trabajadores fue contaminado por el virus HBV, HCV o HIV, además se observó que solamente 23,33% de los trabajadores comparecieron a todos los controles programados para la verificación de una posible suero-conversión. En relación con las conductas tomadas con relación al accidente se indicó el uso de la quimioprofilaxia en 76.67% de los casos, exámenes serológicos en 100% de los casos e inmunización para Hepatitis en solamente 9.99% de los casos. Debido a la gran ocurrencia de accidentes percutáneos según estimativas oficiales de otros países, concluimos que una mayor atención debe ser dada a la prevención de accidentes así como también al rigor del seguimiento después de la exposición ocupacional.

Foram estudados 24 ovários pertencentes a 13 cutias adultas (em dois exemplares pesquisou-se apenas o ovário esquerdo), sendo 7 gestantes e 6 não gestantes, provenientes do Núcleo de Estudos e Preservação de Animais Silvestres da Universidade Federal do Piauí. Os órgãos foram observados in loco para descrição de sua topografia, mensurados, submetidos a cortes histológicos e observados em Microscópio Óptico. Os ovários apresentam-se de forma elipsóide, achatados dorsoventralmente, situando-se na região sub-lombar, caudalmente aos rins, com pequenas áreas transparentes em sua superfície externa. A margem mesovárica e a face lateral estão cobertas pelo mesossalpinge (bolsa ovárica). O ovário direito, apresenta em média: peso - 0,082g; comprimento - 0,83cm; largura - 0,49cm e espessura - 0,24cm; o ovário esquerdo: peso - 0,058g; comprimento - 0,74cm; largura - 0,45cm e espessura - 0,23cm. Histologicamente, constam de epitélio de revestimento cúbico simples, córtex periférico e medula central, constituída basicamente por tecido conjuntivo frouxo entremeado por vasos sangüíneos. Nas fêmeas gestantes foram observados de dois a três grandes corpos lúteos centrais e muitos outros menores periféricos; nas não gestantes os corpos lúteos são menores e mais numerosos. O córtex é rico em tipos celulares de natureza conjuntiva e em folículos em diferentes estágios de amadurecimento, os quais migram da margem mesovárica para a extremidade tubárica à medida que aumentam de tamanho. Conclui-se que os ovários da cutia, macro e microscopicamente, seguem o padrão observado nos demais roedores sexualmente ativos.

It were studied 24 ovaries belonging to 13 adult agoutis (in two copies it was just researched the left ovary), being 7 pregnant and 6 not pregnant, coming of the "Núcleo de Estudos e Preservação de Animais Silvestres", of the Federal University of Piauí. The organs were described in loco, measured, submitted to section and observed in Optical Microscope. The ovaries, ellipsoids, were located in the sub-lumbar area, flow to the kidneys, flat, with external surface evidencing small transparent areas and the margin mesovarian and lateral face coming covers for the mesossalpinge (ovaric bag). For the right ovary, was observed on the average: weight - 0,082g; length - 0,83cm; width - 0,49cm and thickness - 0,24cm; the left ovary: weight - 0,058g; length - 0,74cm; width - 0,45cm and thickness - 0,23cm. Histologically, this gonads were correspondents with the majority of the rodents sexually actives studies, showing epithelium of simple cubic coating, the outlying cortex and the central medulla (constituted basically by conjunctive slack tissue mixed with vases of blood). In the pregnant females it was counted two to three great central luteous bodies and many other smaller. In the not pregnant the luteous bodies was smaller and more numerous. The cortex is rich in cellular types of conjunctive nature and follicles in different phases of the maturation, which migration of the mesovarian margin for the tubaric extremity, as they increase of size. It was conclued that the ovaries of the agoutis, macro and microscopically, coming the pattern observed in the others rodents actives sexually.

Estudamos a placenta quanto aos seus aspectos morfológicos em 30 caprinos sem raça definida, adultos oriundos do Estado do Piauí, mediante a análise de esquemas de modelos obtidos pela injeção de látex Neoprene "650". Obtivemos 17 casos, ou seja, 56,66% de gestações únicas e 13 casos, ou seja, 43,33% de gestações gemelares. A área do hilo placentário varia de 1 a 6 cm de diâmetro e está situada no centro da placenta, região caracterizada pela apresentação do pedículo umbilical. O formato dos cotilédones varia de ovóide, circular, elíptica, piriforme ou reniforme, sendo que a maioria mostra-se ovóide. O número total de cotilédones em 30 gestações é igual a 3.117, sendo a média 104 cotilédones por gestação. Nas gestações únicas em 16 casos (94,11%), o corno uterino gestante apresenta maior número de cotilédones do que o corno uterino não-gestante. Nas gestações gemelares, em 7 observações (53,84%), o corno uterino esquerdo mostra maior número de cotilédones do que o corno uterino direito.

The morphologic aspects of the goat placenta were the subjects of our research. Thirty adult, mixed-breed goats, from the State of Piauí (Brazil), were studied, and the models obtained by Neoprene "650" latex injection were analyzed. We found that in 17 cases (56.66%) single gestation occurred, while in 13 animals (43.33%) gemellary gestations were present. The diameter of the hilus placental area ranged from 1 to 6 centimeters. It was located in the center of the placenta, an area characterized by the presence of the umbilical pedicle. Concerning the cotyledon shape, the great majority was ovoid, although circled, elliptical, pyriform and reniform shapes were also found. The total number of cotyledons in all thirty pregnancies was 3.117, which shows an average of 104 cotyledons per gestation. In 16 cases of single gestation (94.11%), the pregnant uterine horn had a greater number of cotyledons than the contralateral one. In 7 animals with gemellary gestation (53.84%), the left uterine horn had a greater number of cotyledons than the right uterine one.
