ABSTRACT: One of the main issues related to the management of forage legumes is the fact that these plants can be found in a broad diversity of morphology, structures and growth habits. This study evaluated the structural and productive characteristics of forage legumes with different growth habits [Clitoria ternatea L.; Desmanthus pernambucanus (L.) Thellung; and Stylosanthes spp. cv. Campo Grande], in response to two harvesting frequencies (60 and 90 days). The experimental design was completely randomized in blocks, with a split-plot scheme and four replications (n=4). The type of forage legume influenced the number of secondary branches, the number of leaves per branch, leaf area index (LAI), light interception (LI), percentage of leaves and stems (%), leaf/stem ratio, and pod production (P<0.05). D. pernambucanus had the lowest LI and LAI (P < 0.05). Stylosanthes spp. cv. Campo Grande and C. ternatea showed a greater leaf percentage and leaf/stem ratio (P < 0.05). There was an effect of the harvesting frequency on the percentage of stems, leaves, pod production, and total production (P < 0.05). Longer harvesting frequencies affected differently these legumes, especially the accumulation of stems in D. pernambucanus, which displayed an intense increase after 60 days of growth. The choice of the best harvesting frequency for these legumes evaluated should consider the proposal of the usage. In our study, it was shown that harvesting at 60 days did not affect the total forage mass accumulated, which possibly is the most advantageous choice.

RESUMO: Uma das principais questões relacionadas ao manejo de leguminosas forrageiras é o fato de que essas plantas podem ser encontradas em uma ampla diversidade de morfológica, estruturais e de hábitos de crescimento. O objetivo deste estudo foi avaliar características morfológicas, estruturais e produtivas de leguminosas forrageiras com diferentes hábitos de crescimento [Clitoria ternatea L.; Desmanthus pernambucanus (L.) Thellung; e Stylosanthes spp. cv. Campo Grande], em resposta a duas frequências de colheita (60 e 90 dias). O delineamento experimental foi inteiramente casualizado em blocos, com esquema de parcelas subdivididas e quatro repetições (n=4). O tipo de leguminosa forrageira influenciou no número de ramos secundários, número de folhas por ramo, índice de área foliar (IAF), interceptação luminosa (IL), porcentagem de folhas e colmos (%), razão folha/colmo e produção de vagens (P<0,05). D. pernambucanus apresentou os menores IL e IAF (P<0,05). Stylosanthes spp. cv. Campo Grande e C. ternatea apresentaram maior porcentagem de folhas e relação folha/colmo (P<0,05). Houve efeito da frequência de colheita na porcentagem de produção de hastes, folhas e vagens e na produção total (P<0,05). Frequências de colheita mais longas afetaram de forma diferente as leguminosas forrageiras avaliadas, principalmente o acúmulo de caules em D. pernambucanus, que apresentou aumento intenso após 60 dias de crescimento. A escolha da melhor frequência de colheita para essas leguminosas avaliadas deve levar em consideração a sua forma de uso. No presente estudo, foi demonstrado que a colheita aos 60 dias não afetou a massa forragem total acumulada, o que possivelmente é a escolha mais vantajosa.

Resumo: Neste artigo, realizamos a primeira análise sistemática e abrangente da incorporação dos Estudos sobre as Mulheres, de Género e Feministas (EMGF) nos currículos do ensino superior em Portugal. Mediante uma análise de conteúdo dos planos de estudo dos cursos e dos programas das unidades curriculares disponíveis nos websites das instituições, mapeamos um quadro de incorporação incipiente, acessória e precária dos EMGF, típica de fases precoces da institucionalização do campo. O estudo evidencia que há ainda um longo caminho a percorrer para uma efetiva e consistente integração dos EMGF no ensino superior no país.

Abstract: In this article, we conduct the first systematic and comprehensive analysis of the incorporation of Women’s, Gender and Feminist Studies (WGFS) in higher education curricula in Portugal. Through a content analysis of course syllabi and study program plans available on institutional websites, we draw a picture of incipient, accessory and precarious incorporation of WGFS, typical of the early stages of the field’s institutionalization. The study shows that there is still a long way to go for an effective and consistent integration of WGFS in higher education in the country.

Abstract Background: meeting the needs of families with a member who has a Neuromuscular Disorder (ND), enables an improvement in their quality of life, minimizing the possibility of illness and allowing greater family contribution to their functions. Objectives: characterize the needs of families with a member who has a ND in the functional dimension. Methodology: a quantitative, descriptive, cross-sectional study was carried out with an intentional sample of 30 families. The operative matrix of the Dynamic Model of Family Assessment and Intervention (MDAIF) was used as a data collection instrument. Data processing and analysis were performed by descriptive statistics. Results: most members who have a ND are dependent on self-care, clothing, hygiene and using the bathroom; families have saturation of home care role and caregiver role. Conclusion: family nurses will be able to help families by creating programs that allow: -the readjustment of family roles where there is saturation; -the possibility of working and supporting caregivers; -specific training regarding the people who have a ND.

Resumo Enquadramento: dar resposta às necessidades das famílias com membro Portador de Patologia Neuromuscular (PPN), possibilita uma melhoria na qualidade de vida dos mesmos, minimizando a possibilidade de doença e permite maior contribuição familiar nas suas funções. Objetivos: este estudo tem como objetivo caracterizar as necessidades das famílias com membro PPN, na sua dimensão funcional. Metodologia: foi realizado um estudo quantitativo, descritivo, transversal, com amostra intencional de 30 famílias. Utilizámos um formulário construído com base na matriz operativa do Modelo Dinâmico de Avaliação e Intervenção Familiar (MDAIF) como instrumento de colheita de dados. Tratamento e análise de dados efetuados pela estatística descritiva. Resultados: os membros portadores de patologia neuromuscular, na sua maioria são dependentes no autocuidado vestuário, higiene e uso do sanitário; a família apresenta a saturação do papel do cuidado doméstico e a saturação do papel de prestador de cuidados. Conclusão: o enfermeiro de família poderá dar resposta às famílias com a criação de programas que permitam: -reajuste dos papeis familiares onde existe saturação; -possibilidade de trabalhar e dar apoio ao prestador de cuidados; -formação específica no que respeita aos cuidados ao membro PPN.

Resumen Marco contextual: responder a las necesidades de las familias con un miembro con Patología Neuromuscular (PPN), posibilita una mejora en su calidad de vida, minimizando la posibilidad de enfermedad y permitiendo una mayor contribución familiar a sus funciones. Objetivos: este estudio tiene como objetivo caracterizar las necesidades de las familias con un miembro con Patología Neuromuscular (PPN), en su dimensión funcional. Metodología: se realizó un estudio cuantitativo, descriptivo, transversal, con una muestra intencional de 30 familias. Se utilizó un formulario construido sobre la matriz operativa del Modelo Dinámico de Evaluación e Intervención Familiar (MDAIF) como instrumento de recolección de datos. Procesamiento y análisis de datos realizado por estadística descriptiva. Resultados: la mayoría de los afiliados con patología neuromuscular son dependientes del autocuidado, vestimenta, higiene y uso del baño; la familia presenta la saturación del rol de cuidado doméstico y la saturación del rol de cuidador. Conclusión: la enfermera de la familia podrá responder a las familias con la creación de programas que permitan: - reajuste de los roles familiares donde hay saturación; -posibilidad de trabajar y apoyar al cuidador; -formación específica respecto al cuidado de la persona con PPN.

ABSTRACT Introduction: Low back pain is one of the most common complaints among cyclists. The disharmony of the cyclist-bike combination may be a predisposing factor. Bike Fit is a technique that aims to adjust the bike to the individual characteristics of the cyclist. Objectives: To investigate the relationship between the cyclist’s position on the bicycle and the occurrence of complaints of low back pain. Methods: Data obtained during Bike Fit from 62 amateur cyclists were used in the study. Cyclists were filmed during the act of pedaling on a stationary roller and image analysis was performed using Kinovea® software. Data related to complaints of low back pain and positioning on the bicycle were used in the Chi-Square test and binary logistic regression. Results: The mean age was 38.06 ± 8.82 years, 87.7% of the sample was composed of men and low back pain was found in 40.3% (25/62) of the participants. Univariate analysis showed a positive correlation between low back pain and the following variables: ankle dorsiflexion angle (X2=6.947, p=0.014) and upper limb reach (X2=5.247; p=0.032). Binary logistic regression showed a positive association between reaching with the upper limbs and low back pain (r=2.728; p=0.002) and a negative association between knee advancement and low back pain (r= -2.281; p=0.007). Conclusion: Cyclists with low back pain present changes in their position on the bicycle, which reinforces the importance of evaluating the cyclist/bike combination. However, it is not possible to state whether the positional changes observed in the study sample are causes or consequences of low back pain. RESUMEN Introducción: El dolor lumbar es una de las quejas más frecuentes en los practicantes de ciclismo. Puede tener como factor predisponente la falta de armonía del conjunto ciclista-bicicleta. Bike Fit es una técnica que tiene como objetivo ajustar la bicicleta a las características individuales del ciclista. Objetivos: Investigar la relación entre la posición del ciclista en la bicicleta y la aparición de quejas de lumbalgia. Métodos: En el estudio se utilizaron datos obtenidos durante el Bike Fit de 62 ciclistas aficionados. Los ciclistas fueron filmados durante el acto de pedalear sobre un rodillo estacionario y el análisis de las imágenes se realizó utilizando el software Kinovea®. Los datos relacionados con las quejas de dolor lumbar y la posición en la bicicleta se utilizaron en la prueba de Chi-Cuadrado y regresión logística binaria. Resultados: La edad media fue de 38,06 ± 8.82 años, el 87,7% de la muestra estuvo compuesta por hombres y se constató dolor lumbar en el 40,3% (25/62) de los participantes. El análisis univariado mostró una correlación positiva entre el dolor lumbar y las siguientes variables: ángulo de dorsiflexión del tobillo (X2 = 6,947, p = 0,014) y alcance de las extremidades superiores (X2 = 5,247; p = 0,032). La regresión logística binaria mostró una asociación positiva entre el alcance de las extremidades superiores y el dolor lumbar (r = 2,728; p = 0,002) y una asociación negativa para el avance de la rodilla y el dolor lumbar (r = -2,281; p = 0,007). Conclusiones: Los ciclistas con dolor lumbar presentan cambios en su posición sobre la bicicleta, lo que refuerza la importancia de la evaluación del conjunto ciclista-bicicleta. Sin embargo, no es posible afirmar si los cambios posicionales observados en la muestra estudiada son causas o consecuencias del dolor lumbar. Os dados relacionados com a queixa de dor lombar e ao posicionamento na bicicleta foram utilizados no teste do Qui-quadrado e de regressão logística binária. Resultados: A média de idade foi de 38,06 ± 8,82 anos, 87,7% da amostra foi composta por homens e a dor lombar foi constatada em 40,3% (25/62) dos participantes. A análise univariada mostrou correlação positiva entre a dor lombar e as seguintes variáveis: ângulo de dorsiflexão do tornozelo (X2 = 6,947, p = 0,014) e alcance dos membros superiores (X2 = 5,247; p = 0,032). Abstract This paper analyzes how two babies, ages ten to twelve months, explored the space in an infants’ classroom during their entry process in an Early Childhood Education Center in Belo Horizonte, Brazil. While in the field, during the 2017 school year, the following research instruments were used: participant observation, video recordings and fieldnotes. Ethnography in education, in dialogue with cultural-historical psychology and Henri Lefebvre’s production of space theory, enabled us to perceive the baby in a space that is both product and production of such a person. The babies experienced and explored the classroom space during their entry process in the daycare center in different ways: (i) by watching, (ii) walking, (iii) crawling, (iv) touching, (v) fighting for toys, (vi) crying, (vii) learning how to care, (viii) sleeping, (ix) interacting with their peers and other people around them. The babies’ exploration trajectories in this classroom were constituted by the dialectic unity [perception/action], as babies [perceive/act] on the space, the objects, and the people seeking to familiarize themselves with this new space. There was, then, an active search, by the babies, for a recognition of the space and people, so that, perhaps, they could feel belonging to this social environment.

Resumo Este artigo teve por objetivo analisar como dois bebês, com idades entre dez e doze meses, exploraram o espaço de uma sala de berçário durante seu processo de inserção em uma Escola Municipal de Educação Infantil (EMEI) de Belo Horizonte. No tempo de permanência em campo, durante o ano letivo de 2017, utilizaram-se os seguintes instrumentos de pesquisa: observação participante, videogravações e anotações em diários de campo. A etnografia em educação, em diálogo com a psicologia histórico-cultural e com a teoria da produção do espaço de Henri Lefebvre, possibilitou perceber o/a bebê em um espaço que é produto e produção de tal pessoa. Os/as bebês vivenciaram e exploraram o espaço do berçário, durante seu processo de inserção na creche, de maneiras distintas: (i) por meio do olhar, (ii) do caminhar, (iii) do engatinhar, (iv) do toque, (v) das disputas por brinquedos, (vi) do choro, (vii) do aprendizado do cuidado, (viii) do sono, (ix) dos encontros desses bebês com seus pares e com outras pessoas ali presentes. As trajetórias de exploração dos/as bebês nesse berçário foram marcadas pela unidade dialética [percepção/ação], pois os/as bebês [percebem/agem] sobre o espaço, os artefatos e sobre as pessoas em busca de se familiarizarem com esse novo espaço em que foram inseridos. Houve, então, uma ativa busca, por parte dos/as bebês, de um conhecimento/reconhecimento do espaço e das pessoas para que, talvez, pudessem se sentir pertencentes a esse meio social.

ABSTRACT We describe a case of a 33-years-old woman who presents with severe acute bilateral visual loss secondary to massive exudative hypertensive maculopathy as the first sign of immunoglobulin A nephropathy. The patient’s ophthalmic examination showed bilateral cotton-wool spots, flame-shaped retinal hemorrhages, diffuse narrow arterioles, optic disk edema, and exudative maculopathy. Systemic workup demonstrated a systolic and diastolic blood pressure of 240 mmHg and 160 mmHg, respectively, proteinuria, and hematuria, suggesting kidney disease as the causative condition. A kidney biopsy confirmed immunoglobulin A nephropathy. She was treated with systemic corticosteroids, antihypertensive drugs, and a single bilateral intravitreal injection of aflibercept. There was a prompt resolution of macular edema and vision improvement. Our case draws attention to the fact that severe bilateral visual loss can be the first sign of severe hypertension. Secondary causes, such as immunoglobulin A nephropathy, should be ruled out.

RESUMO Nosso objetivo é descrever uma paciente de 33 anos de idade, com perda visual bilateral grave por maculopatia hipertensiva exsudativa como o primeiro sinal da nefropatia por imunoglobulina A. A fundoscopia revelou a presença de manchas algodonosas, hemorragias em chama-de-vela, estreitamento arteriolar difuso, edema de disco óptico e maculopatia exsudativa bilateral. A pressão arterial sistólica foi de 240mmHg e a diastólica de 160 mmHg associado a proteinúria e hematúria, sugerindo a presença de doença renal. A biópsia renal confirmou a nefropatia por imunoglobulina A. A paciente foi tratada como corticoide sistêmico, drogas anti-hipertensivas e uma única dose intravítrea de Aflibercept em ambos os olhos. Houve rápida melhora do edema macular e da acuidade visual. Nosso caso chama a atenção para o fato de que a perda visual bilateral grave pode ser a primeira apresentação de uma doença hipertensiva sistêmica. Causas secundárias como a nefropatia por imunoglobulina A devem ser afastadas.

Abstract Introduction The deficiency of ADA2 (DADA2) is a rare autoinflammatory disease provoked by mutations in the ADA2 gene inherited in a recessive fashion. Up to this moment there is no consensus for the treatment of DADA2 and anti-TNF is the therapy of choice for chronic management whereas bone marrow transplantation is considered for refractory or severe phenotypes. Data from Brazil is scarce and this multicentric study reports 18 patients with DADA2 from Brazil. Patients and methods This is a multicentric study proposed by the Center for Rare and Immunological Disorders of the Hospital 9 de Julho - DASA, São Paulo - Brazil. Patients of any age with a confirmed diagnosis of DADA2 were eligible for this project and data on clinical, laboratory, genetics and treatment were collected. Results Eighteen patients from 10 different centers are reported here. All patients had disease onset at the pediatric age (median of 5 years) and most of them from the state of São Paulo. Vasculopathy with recurrent stroke was the most common phenotype but atypical phenotypes compatible with ALPS-like and Common Variable Immunodeficiency (CVID) was also found. All patients carried pathogenic mutations in the ADA2 gene. Acute management of vasculitis was not satisfactory with steroids in many patients and all those who used anti-TNF had favorable responses. Conclusion The low number of patients diagnosed with DADA2 in Brazil reinforces the need for disease awareness for this condition. Moreover, the absence of guidelines for diagnosis and management is also necessary (t).

ABSTRACT Throughout life, the lens produces new fibers arranged concentrically, which increase its anteroposterior diameter and weight, making its nucleus more compact and hardened. Hypermature cataract is an advanced stage of this fiber proliferation, which can trigger a variety of complications. Spontaneous rupture of the anterior lens capsule evolving with anterior displacement of the nucleus is a rare complication, with few cases published in the literature. We describe the case of a 68-year-old female patient, who presented spontaneous rupture of the anterior lens capsule with anterior displacement of the nucleus in the left eye, without a history of ocular trauma. The patient underwent phacoemulsification and clinical control of intraocular pressure, improving her condition.

RESUMO Ao longo da vida, o cristalino produz novas fibras dispostas de forma concêntrica, que aumentam seu diâmetro anteroposterior e peso, tornando seu núcleo mais compacto e endurecido. A catarata hipermadura é uma forma de progressão avançada dessa proliferação de fibras, que pode desencadear uma variedade de complicações. A ruptura espontânea da cápsula anterior do cristalino, evoluindo com deslocamento anterior do núcleo, é uma complicação rara e com poucos casos publicados na literatura. Descrevemos o caso de uma paciente do sexo feminino, 68 anos, que apresentou ruptura espontânea da cápsula anterior do cristalino com deslocamento anterior do núcleo em olho esquerdo sem histórico de trauma ocular. A paciente foi submetida à facoemulsificação do cristalino e ao controle da pressão intraocular, evoluindo com melhora do quadro clínico.

Abstract Clinical practice guidelines (CPG) are developed to align standards of health care around the world, aiming to reduce the incidence of misconducts and enabling more effective use of health resources. Considering the complexity, cost, and time involved in formulating CPG, strategies should be used to facilitate and guide authors through each step of this process. The main objective of this document is to present a methodological guide prepared by the Epidemiology Committee of the Brazilian Society of Rheumatology for the elaboration of CPG in rheumatology. Through an extensive review of the literature, this study compiles the main practical recommendations regarding the following steps of CPG drafting: distribution of working groups, development of the research question, search, identification and selection of relevant studies, evidence synthesis and quality assessment of the body of evidence, the Delphi methodology for consensus achievement, presentation and dissemination ofthe recommendations, CPG quality assessment and updating. Resumo Enquadramento: a intervenção dos profissionais de saúde no processo de luto é uma necessidade de saúde no processo de fim-de-vida do doente paliativo e sua família. Objetivo: mapear a evidência científica sobre quais as intervenções desenvolvidas pelos profissionais de saúde na gestão do luto no contexto de cuidados paliativos. Metodologia: scoping review, seguindo a mnemónica P (participantes), C (conceito) e C (contexto) recomendada pelo Joanna Briggs Institute. Resultados: foram selecionados três artigos, dois dos quais se referem a programas de gestão do luto pela perda de uma familiar criança e um pela perda de familiar adulto. As intervenções desenvolvidas pelas equipas de saúde na gestão do luto iniciam-se no processo de fim-de-vida da pessoa. Relacionam-se com o suporte, criação de memórias e manutenção do contacto com a família após a morte da pessoa. Conclusão: o processo de luto associado à perda de uma criança é muito complexo e requer uma visão muito particular. Mais estudos devem ser desenvolvidos sobre as intervenções direcionadas na gestão de luto em cuidados paliativos.

Abstract Background: the intervention of health professionals in the grieving process is a health need in the end-of-life process of the palliative patient and his family. Objective: to map the scientific evidence on which interventions health professionals develop in the grief management in the palliative care setting. Methodology: scoping review, following the mnemonic P (participants), C (concept) and C (context) recommended by the Joanna Briggs Institute. Results: three articles were selected, two of which refer to grief management programs for the loss of a child family member and one for the loss of an adult family member. The interventions developed by the health teams in the management of grief begin in the person's end-of-life process. They are related to support, creation of memories and maintenance of contact with the family after the person´s death. Conclusion: the grieving process associated with the loss of a child is very complex and requires a very particular view. Further studies should be developed on targeted interventions in grief management in palliative care.

Resumen Marco contextual: la intervención de los profesionales sanitarios en el proceso de duelo es una necesidad sanitaria en el proceso de final de vida del paciente paliativo y su familia. Objetivo: mapear la evidencia científica sobre cuáles son las intervenciones desarrolladas por los profesionales de la saluden la gestión del duelo en el contexto de los cuidados paliativos. Metodología: scoping review, siguiendo los nemotécnicos P (participantes), C (concepto) y C (contexto) recomendados por el Instituto Joanna Briggs. Resultados: se seleccionaron tres artículos, dos de los cuales se refieren a programas de manejo del duelo por la pérdida de un familiar infantil y uno por la pérdida de un familiar adulto. Las intervenciones desarrolladas por los equipos de salud en el manejo del duelo comienzan en el proceso de final de vida de la persona. Están relacionados con el apoyo, la creación de recuerdos y el mantenimiento del contacto con la familya tras la muerte de la persona. Conclusión: el proceso de duelo asociado a la pérdida de un hijo es muy complejo y requiere una vision muy particular. Deben desarrollarse más studios sobre las intervenciones específicas en la gestión del duelo en los cuidados paliativos.

RESUMO Este texto pretende repensar a disputa de epistemologias feministas no que tange às construções discursivas de gênero, tendo como contexto os pensamentos de Freud e Lacan. Trata-se da crítica de Butler às teorias feministas binárias relativamente à substancialização do corpo sexual: se o corpo sexual é construído pelo discurso, ele também se porta como um elemento subversor do discurso, já que não pode ser completamente determinado pelo binarismo cultural de gênero. Utilizamos parte do debate que Butler trava com a psicanálise freudiana e lacaniana como um “caso modelo” no qual tais questionamentos epistemológicos seriam analisados: de início, a psicanálise coloca-se como um dos principais alvos críticos da “segunda onda” feminista; em segundo lugar, a psicanálise também teria sido mobilizada a favor da causa feminista binária; contudo, para Butler, tais feministas não teriam criticado suficientemente a psicanálise, por não discutirem a raiz corporal essencializada que sustentaria a divisão binária entre sexos; finalmente, Butler ressalta a potencialidade crítica que a psicanálise comporta, dada a resistência subversiva e plástica do inconsciente, das pulsões, do desejo e do corpo, noções centrais para a epistemologia feminista não binária e remodelação do construtivismo discursivo.

ABSTRACT This text intends to rethink the dispute of feminist epistemologies regarding the conception of gender discursive constructions from the background of Freud and Lacan’s thesis. Our basis is Butler’s critique of binary feminist theories regarding the substantialization of the sexual body: if the sexual body is constructed by discourse, it is also a subversive element of discourse, since it cannot be completely determined by cultural gender binarism. We use excerpts from the debate between Butler-Freudian and Lacanian psychoanalysis as a “model case” where such epistemological questions could be analyzed: at first, psychoanalysis is one of the main critical targets of the feminist “second wave”; secondly, psychoanalysis would also have been mobilized in favor of the binary feminist cause; however, for Butler, such feminists would not have sufficiently criticized psychoanalysis, for not attacking the essentialized sexual bodily root that would sustain the binary division between sexes; finally, Butler emphasizes the critical potential that psychoanalysis holds, given the subversive and plastic resistance of the unconscious, drive, desire and body, central notions for non-binary feminist epistemology and remodeling of discursive constructivism.

SUMMARY OBJECTIVE: This study aimed to evaluate the agreement in the indication of implantable cardioverter-defibrillators in patients with Hypertrophic cardiomyopathy, as per the 2014 European Society of Cardiology and 2020 American Heart Association recommendations, and evaluate fragmented QRS as a predictor of cardiovascular outcome. METHODS: Retrospective cohort with 81 patients was evaluated between 2019 and 2021. Patients with hypertrophic cardiomyopathy ≥16 years old were included. Exclusion criteria include secondary myocardiopathy and follow-up <1 year. Kappa coefficient was used to determine the agreement. Survival and incidence curves were determined by Kaplan-Meier method. A p<0.05 was considered significant. RESULTS: The fragmented QRS was identified in 44.4% of patients. There were no differences between patients with and without fragmented QRS regarding clinical parameters, echocardiography, fibrosis, and sudden cardiac death risk. During follow-up of 4.8±3.4 years, there was no sudden cardiac death, but 20.6% patients with implantable cardioverter-defibrillator had at least one appropriate shock. Three of the seven appropriate shocks occurred in European Society of Cardiology low- to moderate-risk patients. Three shocks occurred in moderate-risk patients and four in American Heart Association high-risk patients. Overall recommendations agreement was 64% with a kappa of 0.270 (p=0.007). C-statistic showed no differences regarding the incidence of appropriate shock (p=0.644). CONCLUSION: sudden cardiac death risk stratification algorithms present discrepancies in implantable cardioverter-defibrillator indication, both with low accuracy.
