Resumo Introdução: As técnicas endoscópicas são atualmente consideradas abordagens de primeira linha no manejo das fístulas associadas a cirurgia bariátrica. O uso off-label de dispositivos de oclusão do septo cardíaco (CSDO) é uma técnica nova que tem demonstrado resultados favoráveis no encerramento de defeitos extra-vasculares, incluindo fístulas gastrointestinais. Relatos de caso prévios reportaram resultados semelhantes com o CSDO Amplatzer™ para o tratamento de fístulas gastrointestinais pós cirurgia bariátrica. No entanto, o uso de dispositivos alternativos semelhantes para esse fim ainda não foi descrito. Relato de Caso: Este relato de caso apresenta o primeiro uso reportado do CSDO Occlutech® para tratamento de fístula gastrocutânea crônica após cirurgia bariátrica revisional. Apesar do aparente sucesso inicial - nenhum extravasamento de contraste através do dispositivo na fluoroscopia após a colocação do CSDO, houve recorrência da drenagem fistulosa devido ao deslocamento parcial do dispositivo. A colocação de um segundo dispositivo entre os discos do primeiro acabou por encerrar o orifício fistuloso. Discussão: Nas fístulas gastrointestinais crônicas, o trajeto epitelizado muitas vezes não é passível de aplicação dos métodos endoscópicos tradicionais, levando a múltiplas tentativas fracassadas de encerramento. A nova aplicação de Occlutech® CSDO pode evitar o risco de uma laparotomia de alto risco nesses casos. Equipamentos endoscópicos adequados, bem como o envolvimento de equipe multidisciplinar são condições primordiais para garantir o sucesso do tratamento.

Abstract Introduction: Endoscopic techniques are now considered the first-line approach for the management of bariatric surgery-related fistulas. The off-label use of cardiac septal defect occluders (CSDO) is an emerging technique that has demonstrated favorable outcomes for the closure of extra-vascular defects, including gastrointestinal (GI) disruptions. Previous case reports have reported similar results with the CSDO Amplatzer™ for the management of GI disruptions following bariatric surgery. However, the use of similar alternative devices for this purpose has not yet been described. Case Presentation: This case report presents the first reported use of the Occlutech® CSDO for the treatment of a chronic gastrocutaneous fistula after bariatric revisional surgery. Despite apparent initial success - no extravasation of contrast material through the device in the contrast study after the CSDO placement - fistula closure failed due to partial dislodgement of the device. The placement of a second device between the discs of the former one ultimately sealed the fistulous orifice. Discussion: In chronic GI fistulas, the mature tract is often not liable to the application of standard endoscopic methods, leading to failed closure attempts. A new application of Occlutech® CSDO can obviate the clinical burden of a high-risk laparotomy in these cases. Appropriate endoscopic equipment as well as the involvement of a multidisciplinary team are prime conditions to ensure successful patient outcomes.

ABSTRACT BACKGROUND: Achalasia is an esophageal motility disorder, and myotomy is one of the most used treatment techniques. However, symptom persistence or recurrence occurs in 9 to 20% of cases. AIMS: This study aims to provide a practical approach for managing the recurrence or persistence of achalasia symptoms after myotomy. METHODS: A critical review was performed to gather evidence for a rational approach for managing the recurrence or persistence of achalasia symptoms after myotomy. RESULTS: To properly manage an achalasia patient with significant symptoms after myotomy, such as dysphagia, regurgitation, thoracic pain, and weight loss, it is necessary to classify symptoms, stratify severity, perform appropriate tests, and define a treatment strategy. A systematic differential diagnosis workup is essential to cover the main etiologies of symptoms recurrence or persistence after myotomy. Upper digestive endoscopy and dynamic digital radiography are the main tests that can be applied for investigation. The treatment options include endoscopic dilation, peroral endoscopic myotomy, redo surgery, and esophagectomy, and the decision should be based on the patient’s individual characteristics. CONCLUSIONS: A good clinical evaluation and the use of proper tests jointly with a rational assessment, are essential for the management of symptoms recurrence or persistence after achalasia myotomy.

RESUMO RACIONAL: A acalasia é um distúrbio da motilidade esofágica e a miotomia é uma das técnicas de tratamento mais utilizadas. No entanto, a persistência ou recorrência dos sintomas ocorre em 9 a 20%. OBJETIVOS: Este estudo visa fornecer uma abordagem prática para o manejo da recorrência ou persistência dos sintomas de acalasia após miotomia. MÉTODOS: Foi realizada uma revisão crítica para reunir evidências para uma abordagem racional no manejo da recorrência ou persistência dos sintomas de acalasia após miotomia. RESULTADOS: Para o manejo adequado de um paciente com acalásia com sintomas significativos após miotomia, como disfagia, regurgitação, dor torácica e perda de peso, é necessário classificar os sintomas, estratificar a gravidade, realizar exames adequados e definir uma estratégia de tratamento. Uma investigação diagnóstica diferencial sistemática é essencial para cobrir as principais etiologias de recorrência ou persistência dos sintomas após a miotomia. A endoscopia digestiva alta e a radiografia digital dinâmica são os principais exames que podem ser aplicados para investigação. As opções de tratamento incluem dilatação endoscópica, POEM (miotomia endoscópica oral), remiotomia e esofagectomia, e a decisão deve ser baseada nas características individuais do paciente. CONCLUSÕES: Uma boa avaliação clínica e a utilização de exames adequados, juntamente com uma avaliação racional, são essenciais para o manejo da recorrência ou persistência dos sintomas após miotomia por acalasia.

ABSTRACT Objective: The purpose of this study was to develop and apply an online questionnaire to assess knowledge in Refractive Surgery in the analyzed population. Methods: An online questionnaire was applied to a population composed of 840 volunteer participants over 18 years of age. Results: The mean age of population was 37.85 years, of whom 60.1% were female. 20.95% of the participants reported having undergone previous eye surgery, 73.86% of which had a refractive objective. Among these, 73.08% were satisfied / very satisfied with the result. The most important criterion when choosing a refractive surgeon was the indication by a friend who underwent the procedure (43.81%). It is noteworthy that the participants who underwent refractive surgery disagreed more than the other people that “the surgery is a simple and risk-free procedure” (p = 0.0045) and that “the main objective of the surgery is to “zero” the diopter” (p = 0.0252). Conclusion: It is essential to improve population's preoperative knowledge about Refractive Surgery, as well as continuing education for ophthalmologists. Therefore, this study encourages the development of new education tools with clear and easily accessible information, which should be informative, and not convincing.

RESUMO Objetivo: Desenvolver e aplicar um questionário eletrônico para avaliação do conhecimento em cirurgia refrativa na população analisada. Métodos: Foi aplicado um questionário eletrônico a uma população composta de 840 participantes voluntários maiores de 18 anos. Resultados: A média da idade dos participantes foi 37,85 anos, sendo 60,1% do feminino. Dentre os participantes, 20,95% reportaram ter sido submetidos a alguma cirurgia ocular prévia, sendo 73,86% com objetivo refrativo. Entre estes, 73,08% mostraram-se satisfeitos/muito satisfeitos com o resultado. O critério mais importante na escolha de um cirurgião refrativo foi a indicação por um conhecido que realizara a cirurgia (43,81%). Destaca-se que os participantes submetidos à cirurgia refrativa discordaram que “a cirurgia é um procedimento simples e isento de riscos” mais do que as demais pessoas (p=0,0045) e também que “o objetivo principal da cirurgia é ‘zerar’ o grau” (p=0,0252). Conclusão: Nota-se a necessidade de melhorar o conhecimento pré-operatório da população sobre cirurgia refrativa e também de educação continuada para os oftalmologistas. Este estudo fomenta o desenvolvimento de novas ferramentas de educação com informações claras e de fácil acesso, que tenham caráter informativo, e não de convencimento.

ABSTRACT Objective To evaluate the accuracy of corneal tomography with Scheimpflug images (Pentacam® AXL, OCULUS, Wetzlar, Germany) in scanning patterns with 25 and 50 images, by analyzing their repeatability and reproducibility in normal eyes, eyes with keratoconus and with keratoconus after crosslinking. Methods Prospective, cross-sectional, mixed study including patients who had normal corneas, or who had keratoconus, who had not undergone corneal surgery, with the exception of crosslinking. Patients who had undergone surgery for intracorneal ring implantation or corneal transplantation were excluded. Three measurements were performed with 25 images and three measurements were performed with 50 images on each eye included in the study. Results The study evaluated a total of 54 eyes of 41 patients. Of these, 10 (18.52%) eyes had normal corneas, without previous eye surgery, 22 (40.74%) were eyes with keratoconus, without previous eye surgery, and 22 (40.74%) were eyes with keratoconus, with crosslinking for keratoconus. In the study of repeatability, we noticed a greater variation of values in all indices in patients with KC compared to those with normal eyes, in most of the parameters analyzed. Conclusion There is no significant difference between the scanning methods with 25 and 50 images. However, 25 scans has advantages that make it the main method suggested. The presence of keratoconus has a negative impact on the repeatability of both types of examination, and there is great agreement between the methods in the three groups studied. The high repeatability coefficient in some parameters suggests that these data should be used with care in the assessment of keratoconus progression.

RESUMO Objetivo Avaliar a precisão da tomografia corneana com imagens de Scheimpflug (Pentacam® AXL, OCULUS, Wetzlar, Alemanha) nos padrões de escaneamento com 25 e 50 imagens, verificando sua repetibilidade e reprodutibilidade em olhos normais, com ceratocone e com ceratocone após crosslinking. Métodos Estudo prospectivo, transversal, misto, no qual foram incluídos os pacientes que tinham córneas normais, ou com ceratocone; e não tinham realizado cirurgias corneanas, com exceção do crosslinking. Foram excluídos os pacientes que tivessem realizado outras cirurgias, como implante de anel intracorneano ou transplante de córnea. Foram realizadas três medidas com 25 imagens e três medidas com 50 imagens em cada olho incluído no estudo. Resultados O estudo avaliou 54 olhos de 41 pacientes. Destes, dez (18,52%) olhos eram com córneas normais e sem cirurgia ocular prévia aleatoriamente selecionados de 5 pacientes; 22 (40,74%) olhos aleatoriamente selecionados de 18 pacientes com ceratocone, sem cirurgia ocular prévia; e 22 (40,74%) olhos com ceratocone de 18 pacientes operados por crosslinking. No estudo da repetibilidade, percebemos uma maior variação de valores em todos os índices nos pacientes com ceratocone comparados aos de pacientes com olhos normais, em grande parte dos parâmetros analisados. Conclusão Não houve diferença significativa entre os métodos de escaneamento por 25 e 50 imagens, sendo o escaneamento por 25 imagens mais conveniente. A presença de ceratocone teve impacto negativo na repetibilidade de ambos os tipos de exame, e foi grande a concordância entre os métodos nos três grupos estudados. O coeficiente de repetibilidade permite considerar a relevância clínica dos dados para avaliar a progressão de ceratocone.

Resumen: La transición epitelio-mesénquima (TEM) es la capacidad de las células epiteliales y mesenquimales de intercambiar de fenotipo de forma transitoria; su identificación en células tumorales carcinoma- tosas se ha asociado con fenotipos clínicos agresivos. En sarcomas, este evento biológico se encuentra en estudio. Objetivo: evaluar la expresión de dos factores de transcripción involucrados en la TEM mediante inmunohistoquímica en osteosarcoma en pacientes pediátricos y su asociación con desenlaces clínicos. Pacientes y Método: Estudio retrospectivo en menores de 18 años con diagnóstico de osteosarcoma. Se realizó inmunohistoquímica para Snail y Twist-1 en muestras obtenidas al momento del diagnóstico. Posteriormente fue explorada la correlación entre la positividad de los marcadores con los desenlaces clínicas y con la supervivencia global. Resultados: 53 pacientes fueron incluidos. Para Snail, se encontró positivo en localización citoplasmático en 26 casos (49,1%) y esto se correlacionó con la presencia de metástasis múltiples (p = 0,02) y metástasis de localización ósea distantes (p = 0,01). Twist fue positivo en localización nuclear en 45 casos (84,9%) y no se encontraron aso ciaciones con las variables clínicas analizadas. Conclusiones: Snail y Twist-1, se detectaron en una frecuencia importante en muestras de pacientes pediátricos con osteosarcoma. Snail citoplasmático se correlacionó con presencia de enfermedad metastásica múltiple y metástasis óseas distantes. La positividad de ambos marcadores sugiere la activación de estas proteínas como reguladores de eventos de TEM en este tumor y se plantea un rol en los fenómenos que se relacionan con la presentación clínica de la enfermedad.

Abstract: The epithelial-mesenchymal transition (EMT) is the ability of epithelial and mesenchymal cells to exchange phenotypes transiently. Its identification in carcinomatous cells has been associated with aggressive clinical phenotypes. In sarcomas, this ability is under study. Objective: to evaluate the expression of two transcription factors involved in EMT by immunohistochemistry in pediatric osteosarcoma and its association with clinical outcomes. Patients and Method: A retrospective cohort study in children under 18 years of age with osteosarcoma diagnosis. Immunohistochemistry was performed for Snail and Twist-1 expressions from samples collected at the time of diagnosis. Correlations between immunohistochemistry and the clinical outcomes and overall survival were performed. Results: 53 patients were included. There were 26 positive cytoplasmic cases (49.1%) in Snail expression and were correlated with the presence of multiple metastases (p = 0.02) and distant bone metastases (p = 0.01). On the other hand, 45 cases (84.9%) were positive in Twist-1 expression in the nuclear location, showing no association with the analyzed clinical variables. Conclusions: Snail and Twist-1 were frequently expressed in pediatric cases of osteosarcoma. Cytoplasmic Snail was correlated with the presence of multiple metastatic disease and distant bone metastases. The positivity of both markers suggests the activation of these proteins as regulators of EMT events in this tumor, suggesting a role in the phenomena related to the clinical presentation of the disease.

ABSTRACT Objective: To analyze and compare COVID-19 patient characteristics, clinical management and outcomes between the peak and plateau periods of the first pandemic wave in Portugal. Methods: This was a multicentric ambispective cohort study including consecutive severe COVID-19 patients between March and August 2020 from 16 Portuguese intensive care units. The peak and plateau periods, respectively, weeks 10 - 16 and 17 - 34, were defined. Results: Five hundred forty-one adult patients with a median age of 65 [57 - 74] years, mostly male (71.2%), were included. There were no significant differences in median age (p = 0.3), Simplified Acute Physiology Score II (40 versus 39; p = 0.8), partial arterial oxygen pressure/fraction of inspired oxygen ratio (139 versus 136; p = 0.6), antibiotic therapy (57% versus 64%; p = 0.2) at admission, or 28-day mortality (24.4% versus 22.8%; p = 0.7) between the peak and plateau periods. During the peak period, patients had fewer comorbidities (1 [0 - 3] versus 2 [0 - 5]; p = 0.002) and presented a higher use of vasopressors (47% versus 36%; p < 0.001) and invasive mechanical ventilation (58.1 versus 49.2%; p < 0.001) at admission, prone positioning (45% versus 36%; p = 0.04), and hydroxychloroquine (59% versus 10%; p < 0.001) and lopinavir/ritonavir (41% versus 10%; p < 0.001) prescriptions. However, a greater use of high-flow nasal cannulas (5% versus 16%, p < 0.001) on admission, remdesivir (0.3% versus 15%; p < 0.001) and corticosteroid (29% versus 52%, p < 0.001) therapy, and a shorter ICU length of stay (12 days versus 8, p < 0.001) were observed during the plateau. Conclusion: There were significant changes in patient comorbidities, intensive care unit therapies and length of stay between the peak and plateau periods of the first COVID-19 wave.

RESUMO Objetivo: Analisar e comparar as características de pacientes críticos com a COVID-19, a abordagem clínica e os resultados entre os períodos de pico e de platô na primeira onda pandêmica em Portugal. Métodos: Este foi um estudo de coorte multicêntrico ambispectivo, que incluiu pacientes consecutivos com a forma grave da COVID-19 entre março e agosto de 2020 de 16 unidades de terapia intensiva portuguesas. Definiram-se as semanas 10 - 16 e 17 - 34 como os períodos de pico e platô. Resultados: Incluíram-se 541 pacientes adultos com mediana de idade de 65 [57 - 74] anos, a maioria do sexo masculino (71,2%). Não houve diferenças significativas na mediana de idade (p = 0,3), no Simplified Acute Physiology Score II (40 versus 39; p = 0,8), na pressão parcial de oxigênio/fração inspirada de oxigênio (139 versus 136; p = 0,6), na terapia com antibióticos na admissão (57% versus 64%; p = 0,2) ou na mortalidade aos 28 dias (24,4% versus 22,8%; p = 0,7) entre o período de pico e platô. Durante o período de pico, os pacientes tiveram menos comorbidades (1 [0 - 3] versus 2 [0 - 5]; p = 0,002); fizeram mais uso de vasopressores (47% versus 36%; p < 0,001) e ventilação mecânica invasiva na admissão (58,1% versus 49,2%; p < 0,001), e tiveram mais prescrição de hidroxicloroquina (59% versus 10%; p < 0,001), lopinavir/ritonavir (41% versus 10%; p < 0,001) e posição prona (45% versus 36%; p = 0,04). Entretanto, durante o platô, observou-se maior uso de cânulas nasais de alto fluxo (5% versus 16%; p < 0,001) na admissão, remdesivir (0,3% versus 15%; p < 0,001) e corticosteroides (29% versus 52%; p < 0,001), além de menor tempo de internação na unidade de terapia intensiva (12 versus 8 dias; p < 0,001). Conclusão: Houve mudanças significativas nas comorbidades dos pacientes, nos tratamentos da unidade de terapia intensiva e no tempo de internação entre os períodos de pico e platô na primeira onda da COVID-19.

ABSTRACT This work addresses the multi-objective reconfiguration of distribution networks with the presence of distributed generation. We work with two objective functions in the problem statement: active power losses and grid voltage index. While in the solution of the model, an elitist evolutionary algorithm of non-dominated ordering (NSGA-II) is used, which is implemented with the help of MATLAB, determining a set of feasible solutions. The proposed methodology is implemented in reconfiguring the test circuit, 33 nodes, 37 branches, and 4 distributed generators of the IEEE. Two case studies are analyzed, one without the presence of distributed generation and the other in the presence of four distributed generators; it is possible to reduce the power losses of the analyzed network and improve the voltage profile. The results are compared with other investigations and methods that have used the same test circuit, evidencing its relevance.

RESUMEN Este trabajo aborda la reconfiguración multiobjetivo de las redes de distribución con presencia de generación distribuida. En el planteamiento del problema se trabaja con dos funciones objetivos; las pérdidas de potencia activa y el índice de tensión de la red. Mientras que en la solución del modelo se emplea un algoritmo evolutivo elitista de ordenamiento no dominado (NSGA-II), el que se implementa con ayuda del MATLAB, determinando un conjunto de soluciones factibles. La metodología propuesta es implementada en la reconfiguración del circuito de prueba, 33 nodos, 37 ramas y 4 generadores distribuidos, de la IEEE. Se analizan dos casos de estudios, uno sin presencia de generación distribuida y el otro en presencia de cuatro generadores distribuidos; se logra reducir las pérdidas de potencia de la red analizada y mejorar el perfil de tensión. Los resultados son comparados con otras investigaciones y métodos que han empleado el mismo circuito de prueba evidenciando su pertinencia.

Abstract The remote sensing techniques must be used to obtain long-term information in remote areas, like the Antarctic continent, to monitor the environmental productivity and its changes. The aim of this work was to analyze the surface reflectance profile patterns for the Antarctic biological soil crusts (algae, lichens, and mosses) in an area of Nelson Island (South Shetland Islands, maritime Antarctic), calculated from Landsat and Sentinel-2 images to identify its similarities and differences due to targets, sensors and acquired date. The surface reflectance values for Antarctic biological soil crusts are similar for those observed for biological soil crusts in other Earth extreme environments, like deserts. In Landsat images, the differences among biological soil crusts surface reflectance were identified at visible and near-infrared wavelengths and for Sentinel-2 images, the differences occur at visible, red-edge and shortwave infrared wavelengths, showing the feasibility of using surface reflectance products to identify these different crusts, despite its inherent pixel spectral mixture. Long-term biophysical parameters from such crusts as retrieved from orbital data is not possible due to very low cloud-free images over the Antarctic, which prevents building a consistent surface reflectance time-series which covers all biological soil crusts growth season.

RESUMO: A ocorrência e a frequência de alterações hepáticas em aves, incluindo frangos de corte (FC), galinhas poedeiras (GP) e aves de subsistência (AS) foram investigadas. O estudo retrospectivo e prospectivo (2006-2021) foi realizado com um total de 300 casos de alterações hepáticas. Aves industriais (FC e GP) foram frequentemente acometidas (88%) e as alterações não infecciosas foram as mais comumente diagnosticadas (69%). Quanto à etiologia ou condição, as alterações hepáticas foram classificadas da seguinte forma: alterações degenerativas (42%), bacterianas (28%), metabólicas (15%), tóxicas (8%), virais (3%), neoplásicas (2%), bem como doenças por protozoários (1,5%) e distúrbios circulatórios (0,5%). Em relação ao tipo de ave, alterações degenerativas, tóxicas e hepatites virais foram mais frequentes nos FC. Distúrbios circulatórios e metabólicos, assim como hepatites bacterianas, foram diagnosticados com maior frequência nas GP. Neoplasias e hepatite por protozoário ocorreram com maior frequência em AS. O exame macroscópico associado à histopatologia possibilitou o diagnóstico da alteração hepática em 59% dos casos. Considerando a hepatite bacteriana em aves comerciais, o diagnóstico etiológico é de grande importância, em vista do risco para a saúde pública, apesar da óbvia importância devido às perdas de produtividade e condenação no processamento.

ABSTRACT: The occurrence and the frequency of hepatic changes in chickens, including broiler (BC), layer (LC) and backyard chickens (BYC) were investigated. The retrospective and prospective study (2006-2021) was conducted with a total of 300 cases of liver disorders. Industrial poultry (BC and LC) were frequently affected (88%) and noninfectious changes were the most diagnosed (69%). Considering etiology or conditions, the hepatic changes were classified as follows: degenerative changes (42%), bacterial (28%), metabolic (15%), toxic (8%), viral (3%), neoplastic (2%), protozoal diseases (1.5%) and circulatory disorders (0.5%). Regarding the type of bird, degenerative, toxic changes and viral hepatitis were more frequent in BC. Circulatory and metabolic disorders, as well as bacterial hepatitis, were more frequently diagnosed in LC. Neoplastic and protozoal hepatitis occurred more frequently in BYC. The macroscopic examination in association with histopathology enabled the diagnosis of the hepatic changes in 59% of the cases. Considering bacterial hepatitis in commercial poultry, the etiological diagnosis is highly important, in view of the risk for public health, despite the obvious importance due to the productivity losses and condemnation at processing.

Lack of diversity regarding genetic and environmental backgrounds weakens the generalization and clinical applicability of research findings on psychotic disorders. Notably, Latin Americans have been generally neglected in genetic studies, comprising less than 2% of genome-wide association study samples. But Latin American populations represent a unique opportunity for research, given the exceptionally high ethnic admixture of this group. Increasing genetic diversity is essential to improve the fine mapping of known regions associated with psychotic disorders, discover novel genetic associations, and replicate studies. Additionally, Latin America is characterized by massive social, political, and economic inequalities, all known risk factors for mental health issues, including psychotic disorders. This article aims to 1) discuss the challenges and advantages of studying Latin America’s particular genetic makeup and environmental context; 2) review previous studies conducted in the region; and 3) describe three Latin American research initiatives in progress: the Neuropsychiatric Genetics of Psychosis in Mexican Populations (NeuroMEX), the Paisa, and the Latin American Network for the Study of Early Psychosis (ANDES) studies.

RESUMO O objetivo desse trabalho foi avaliar os atributos físicos, químicos e sensoriais de variedades de manga cultivadas no sistema orgânico de produção. Foram avaliados frutos de dez variedades de manga cultivadas no sistema orgânico, na região da Chapada Diamantina, BA. As seguintes características físicas e químicas foram avaliadas: comprimento, largura, massa, rendimento em polpa, cor da polpa (L*, C* e h*), sólidos solúveis (SS), acidez titulável (AT), carotenoides totais (CT) e ácido ascórbico. O teste de aceitação sensorial foi realizado por 50 consumidores utilizando a escala hedônica estruturada de nove pontos e os atributos acidez, doçura e firmeza da polpa foram avaliados utilizando a escala do ideal. Diferenças entre as variedades foram observadas para algumas características físicas, químicas e atributos sensoriais. As variedades de manga apresentaram rendimento de polpa igual ou superior a 60%, sendo adequadas para o processamento. De acordo com a análise de componentes principais (ACP) os teores de sólidos solúveis, carotenoides totais, massa do fruto, acidez titulável, h* e L* foram as características mais importantes para diferenciar as variedades avaliadas. As variedades Ubá e Beta apresentaram os maiores teores de SS, enquanto a Favo de Mel caracterizou-se pelos maiores valores de AT e L*. As variedades Favo de Mel, Palmer, Surpresa, Juazeiro 2, Imperial e Joa apresentaram frutos com maiores massa e valores de h* e as variedades Amrapali e Roxa Embrapa 141 apresentaram os maiores teores de carotenoides totais. As variedades Imperial, Palmer, Beta e Joa foram as mais aceitas para todos os atributos sensoriais.

ABSTRACT This study aimed to assess the physical, chemical, and sensory attributes of mango varieties grown under the organic farming. Fruits from ten varieties of mango grown under the organic system in the Chapada Diamantina region, BA, Brazil, were assessed. The following physical and chemical characteristics were assessed: fruit mass, pulp yield, pulp color (L*, C*, and h*), soluble solids (SS), titratable acidity (TA), total carotenoids, and ascorbic acid. The sensory acceptance test was performed with 50 consumers using the nine-point structured hedonic scale and the attributes acidity, sweetness, and firmness of the pulp were assessed using the just-about-right scale. Differences among the varieties of mango grown under the organic production system were observed for physical, chemical and sensory attributes. The varieties showed a percentage of pulp equal or above 60%, which makes them more suitable for processing. According to the PCA analysis, the characteristics soluble solids, total carotenoids, fruit mass, titratable acidity, h* and L* were considered the most important to differentiate the mango varieties evaluated. Ubá and Beta varieties had the highest levels of SS, while Favo de Mel was characterized by the highest values of TA and L*. The varieties Favo de Mel, Palmer, Surpresa, Juazeiro 2, Imperial and Joa had fruits with higher mass and h* values and the varieties Amrapali and Roxa Embrapa 141 had the highest levels of total carotenoids. Imperial, Palmer, Beta and Joa varieties were the most accepted for all sensory attributes.

Resumo Embora o diagnóstico correto possa ser muitas vezes realizado pelo exame clínico, há várias situações em que os métodos de imagem representam a melhor opção para avaliar hérnias inguinais, seu conteúdo e eventuais complicações. Além disso, os abaulamentos da região inguinal nem sempre são hérnias, pois outras lesões como tumores, cistos e hematomas também acometem este sítio. O objetivo deste ensaio é demonstrar alguns diferentes conteúdos que podem ser diagnosticados no interior de hérnias inguinais. A diferenciação dos tipos de estruturas herniadas é de suma importância no planejamento do tratamento.

Abstract Although the correct diagnosis of inguinal hernias can often be made by clinical examination, there are several situations in which imaging methods represent the best option for evaluating such hernias, their content, and the possible complications. In addition, bulging of the inguinal region is not always indicative of a hernia, because other lesions, including tumors, cysts, and hematomas, also affect the region. The objective of this pictorial essay is to demonstrate what can be identified within inguinal hernias. Differentiating the types of herniated structures is of absolute importance for planning the appropriate treatment.
