Abstract Introduction Computed tomography (CT) details of the external auditory canal (EAC) are not fully covered in the literature, so building up base for the CT evaluation and description is important. Preoperative details of the EAC are mandatory before any approach or procedure involving the canal. Objective To determine the different dimensions, measurements, and grading of the EAC by CT scan that were not previously published. Methods The CTscans of 100 temporal bones (200 sides) were included. Axial images were acquired with multiplanar reformates to obtain delicate details in coronal and sagittal planes for all subjects. Results At the EAC entry, the mean vertical length (height) was 7.75 ± 1 mm, and its mean horizontal length (width) was 6.1 ±0.8. At the bony cartilaginous junction of the EAC, the mean vertical length was 7.88 ±1 mm, and its mean horizontal length was 6.22 ± 0.9. At the EAC isthmus, the mean vertical length was 6.8 ± 0.97 mm, and its mean horizontal length was 5.2 ± 0.76. At the medial end of the EAC, the mean vertical length was 7.1 ±0.9 mm, and its mean horizontal length was 5.4 ± 0.85. There were no reported significant differences between right and left sides in all dimensions. Males showed significantly longer vertical and horizontal dimensions of the EAC entry, vertical dimension of the isthmus, and vertical dimension of the medial end of the EAC than females. Conclusion This study improves otologists and radiologists' awareness of EAC variations in the ear field and can be of help to residents in training.

ABSTRACT Purpose: Posterior urethral valves (PUVs) are the most common cause of congenital bladder obstruction in boys. Our aim was to assess the impact of early diagnosis and fulguration of PUVs on bladder function and compare their functional and urodynamic outcome with children who underwent delayed intervention. Materials and Methods: We retrospectively evaluated 153 patients who underwent primary valve ablation from two tertiary hospitals between 2001 and 2018. Patients have been divided into 2 groups, group 1 included 69 patients who were detected antenatally and underwent early fulguration of PUVs while group 2 included 84 children presented postnatally and underwent delayed valve ablation. The recorded data throughout follow-up in renal function tests, urodynamics and changes in the upper urinary tracts were evaluated and compared. Results: Median age at time of valve ablation was 10 days in group 1 and 7 months in group 2. The median follow-up period was 6.5 and 7 years in group 1 and 2, respectively. Chronic kidney disease (CKD) developed in 15 (22%) boys in group 1 while in group 2 it was observed in 31 (37%), p=0.04. While Q-max, mean bladder capacity and post-void residual (PVR) volumes were comparable in both groups, percent PVR was significantly higher in group 2 (3.27 vs. 1.44, p=0.002). Detrusor overactivity was slightly different in both groups (p = 0.07). Conclusions: Compared to delayed intervention, primary ablation of PUVs during the early neonatal life possibly provides the optimum chance to have optimum renal function without impact on bladder function.

ABSTRACT The emergence and spread of extensively drug-resistant tuberculosis (XDR-TB) is a serious threat to global health. Therefore, its rapid diagnosis is crucial. The present study aimed to characterize mutations conferring resistance to second line drugs (SLDs) within multidrug Mycobacterium tuberculosis (MDR-MTB) isolates and to estimate the occurrence of XDR-TB in Casablanca, Morocco. A panel of 200 MDR-TB isolates was collected at the Pasteur Institute between 2015-2018. Samples were subjected to drug susceptibility testing to Ofloxacin (OFX), Kanamycin (KAN) and Amikacin (AMK). The mutational status of gyrA, gyrB, rrs, tlyA and eis was assessed by sequencing these target genes. Drug susceptibility testing for SLDs showed that among the 200 MDR strains, 20% were resistant to OFX, 2.5% to KAN and 1.5% to AMK. Overall, 14.5% of MDR strains harbored mutations in gyrA, gyrB, rrs and tlyA genes. From the 40 OFXR isolates, 67.5% had mutations in QRDR of gyrA and gyrB genes, the most frequent one being Ala90Val in gyrA gene. Of note, none of the isolates harbored simultaneously mutations in gyrA and gyrB genes. In eight out of the 200 MDR-TB isolates resistant either to KAN or AMK, only 25% had A1401G or Lys89Glu change in rrs and tlyA genes respectively. This study is very informative and provides data on the alarming rate of fluoroquinolone resistance which warrants the need to implement appropriate drug regimens to prevent the emergence and spread of more severe forms of Mycobacterium tuberculosis drug resistance.

Carbamazepine is an antiepileptic drug which is considered one of the persistent compounds detected in Malaysian aquatic environment. In this present study, a full factorial experimental design was applied for the analysis of effect NaCl amount, initial concentration of carbamazepine, applied voltage and treatment time on the electrochemical removal percentage of carbamazepine. However, applied voltage was found as the most significant factor with p = 0.00 lower than 95% confidence level. The investigation of effect NaCl and Na2SO4 on the pathway degradation for carbamazepine was provided using liquid chromatography-time of flight/mass spectrometry (LC-TOF/MS). The results showed that new by-products were identified such chloro-epoxy carbamazepine, and other by-products, were detected in the presence of NaCl and Na2SO4 electrolytes. High inhibition percentage of Escherichia coli (E. coli) bacteria was observed at 24.0 h incubation time for both NaCl and Na2SO4 electrolytes after 80 and 20 min of treatment, respectively.

Abstract Jet stirred tank reactors are usually preferred to mechanically stirred tank reactors in the case of high pressure stirred tank reactors in order to avoid the mechanical problems inherent with mechanically agitated reactors. Owing to the impact force exercised by the fluid jet on the tank bottom, the tank bottom is subjected to severe corrosion. The aim of the present work is to address this problem using an accelerated system which simulates natural diffusion-controlled corrosion of metals. Diffusion-controlled corrosion of a fixed copper disc, at the bottom of a cylindrical tank, under a single phase forced jet flow of acidified dichromate solution was investigated. Variables studied were: physical properties of the solution, solution velocity through different diameters of jet nozzle, distance between the jet outlet nozzle and the copper disc surface; and the effect of using a drag reducing polymer. An overall correlation was obtained: Sh = 0.579 Sc 0.33 Re 0.975 (h/d) 0.049 valid for 850 < Sc < 1322, 1852 < Re < 5000 and 0.3 < h/d < 1.2. Addition of polyethylene oxide as a drag reducing polymer was found to diminish the rate of corrosion under single phase forced jet flow by an amount depending on the operation conditions.

The 2(-4(chloro phenyl-1H- benzo[d]imidazol)-1-yl)phenyl)methanone (CBIPM) was synthesized and tested as a corrosion inhibitor for mild steel in 1.0 M HCl solution using electrochemical and spectroscopy measurements. Results obtained showed that this compound has a good inhibition properties for mild steel corrosion in acidic medium which its inhibition efficiency value reaches 98.9 % at 10-3 M. So, the potentiodynamic polarization curves showed that the CBIPM has a mixed type inhibitor. In addition, it is shown that the obtained results were confirmed by the UV-vis spectroscopic measurements. It is found that the inhibitor forms a complex with Fe2+ ion. On the other hand, the experimental results were completed by quantum chemical calculations using DFT method at the B3LYP/6-31G* level of theory. It is found that the theoretical and experimental results are in good agreement.

Resumo Introdução: A detecção precoce da nefropatia diabética (ND) é importante. O ácido úrico sérico (AUS) tem um papel ainda desconhecido no desenvolvimento de ND. Objetivo: Estudar a relação entre AUS e hipertensão, nefropatia precoce e progressão da doença renal crônica (DRC) no diabetes mellitus tipo 2 (DM2). Métodos: O estudo contou com 986 participantes, de acordo com a presença e a duração do diabetes, os pacientes foram classificados em três grupos. O Grupo I incluiu 250 participantes saudáveis. O Grupo II incluiu 352 pacientes com início de diabetes < 5 anos. O Grupo III incluiu 384 pacientes com o aparecimento de diabetes > 5 anos. Todos os participantes foram submetidos a exame clínico completo, medidas antropométricas, exames laboratoriais - incluindo hemoglobina glicosilada (HbA1C), bem como a razão entre triglicérides e lipoproteína de alta densidade (TG/HDL-C), AUS, razão creatinina/albumina (RCA) urinária, e taxa estimada de filtração glomerular (eTFG). Resultados: A razão AUS, PA, HbA1c, TG/HDL-C e RCA foi significativamente maior no grupo III do que no grupo I, II e em II do que I. A eTFG foi significativamente menor no grupo III do que nos grupos I, II e no II do que no I (p < 0,001). Idade, IMC, PA, HbA1c, razão TG/HDL-C, RCA, foram positivamente correlacionados com AUS, enquanto que a TFG esteve negativamente correlacionada. O AUS a níveis > 6,1 mg/dl, > 6,2 mg/dl e > 6,5 mg/dl apresentou maior sensibilidade e especificidade para identificar hipertensão, nefropatia precoce e declínio da eTFG, respectivamente. Conclusão: Mesmo elevados níveis de AUS, foi associado ao risco de hipertensão, nefropatia precoce e declínio da eTFG. Além disso, o nível de AUS pode identificar o início da hipertensão, nefropatia precoce e progressão da DRC em DM2.

Abstract Introduction: Early detection diabetic nephropathy (DN) is important. Whether serum uric acid (SUA) has a role in the development of DN is not known. Objective: To study the relationship between SUA and hypertension, early nephropathy and progression of chronic kidney disease (CKD) in type 2 diabetes mellitus (T2DM). Methods: The total number of the study was 986 participants, according to presence and duration of diabetes were classified into three groups. Group I; including 250 healthy participants. Group II; including 352 with onset of diabetes < 5 years. Group III; including 384, with the onset of diabetes > 5 years. All participants were submitted to complete clinical examination, anthropometric measurements, laboratory investigations, including glycosylated hemoglobin (HbA1C), as well triglycerides to high-density lipoprotein ratios (TG/HDL-C), SUA, urinary albumin/creatinine ratio (ACR) and estimated glomerular filtration rate (eGFR). Results: SUA, BP, HbA1c, TG/HDL-C ratio, and ACR levels were significantly higher in group III than group I, II and in II than I. eGFR significantly lower in group III than group I, II and in II than I (p < 0.001). Age, BMI, BP, HbA1c, TG/HDL-C, ACR, were positively correlated with SUA, while GFR negatively correlated. SUA at level of > 6.1 mg/dl, > 6.2 mg/dl and > 6.5 mg/dl had a greater sensitivity and specificity for identifying hypertension, early nephropathy and decline eGFR respectively. Conclusion: Even high normal SUA level, was associated with the risk of hypertension, early nephropathy and decline of eGFR. Moreover SUA level may identify the onset of hypertension, early nephropathy and progression of CKD in T2DM.

The present work reports the occurrence and monitoring of 11 pharmaceuticals (i.e., caffeine, prazosin, enalapril, carbamazepine, nifedipine, gliclazide, levonorgestrel, simvastatin, hydrochlorothiazide, diclofenac-Na and mefenamic acid) in surface water and in the influent and effluent of sewage treatment plants (STPs) and hospitals (HSPs). A total of 105 water samples were analyzed using solid-phase extraction combined with liquid chromatography-time-of-flight/mass spectrometry (SPE-LC-TOF/MS). The mean concentrations of the detected pharmaceuticals in STP influent and effluent ranged from < limit of quantification (LOQ) to 3909 ng L−1 and 12 to 577 ng L−1, respectively. The mean concentrations of the detected pharmaceuticals in the hospital influent and effluent ranged from 28 to 1644 ng L−1 and 20 to 1540 ng L−1, respectively. The highest concentration detected in the sampling points was 9099 ng L−1 for caffeine in influent STP. The presence of prazosin has never been reported before in literature. In this study, prazosin was detected in all studied samples, and the highest concentration was 525 ng L−1 in influent STP. Chemometric analysis was used to assess the presence of pharmaceuticals in samples.

The cooling slope (CS) casting is the simplest and cheapest technique for producing feedstock materials with non-dendritic microstructure. Such materials are required for semi-solid metal (SSM) processing methods such as thixoforming. In the present investigation, the effect of the pouring temperature and the water cooling on the thixotropic microstructure of commercial A319 Al-Si cast alloy was studied. The results showed that increasing the pouring temperature slightly reduces the bulk porosity of the CS ingots. The ingots poured with water-cooling exhibited slightly lower porosity content than those poured with without water-cooling. Generally, the primary α-Al grains exhibited higher shape factor near the edge of ingot than the middle and center. Ingots poured with water-cooling exhibited lower grain size and shape factor than those poured without water-cooling. Increasing the pouring temperature increases the size α-Al grains.

OBJECTIVE: The present study aimed to investigate the mechanisms underlying the anti-inflammatory and anti-angiogenic effects of ethyl-p-methoxycinnamate isolated from Kaempferia galanga. METHODS: The anti-inflammatory effects of ethyl-p-methoxycinnamate were assessed using the cotton pellet granuloma assay in rats, whereby the levels of interleukin-1 and tumor necrosis factor-α were measured in the animals' blood. In addition, the levels of interleukin, tumor necrosis factor, and nitric oxide were measured in vitro using the human macrophage cell line (U937). The analgesic effects of ethyl-p-methoxycinnamate were assessed by the tail flick assay in rats. The anti-angiogenic effects were evaluated first by the rat aortic ring assay and, subsequently, by assessing the inhibitory effects of ethyl-p-methoxycinnamate on vascular endothelial growth factor, proliferation, migration, and tube formation in human umbilical vein endothelial cells. RESULTS: Ethyl-p-methoxycinnamate strongly inhibited granuloma tissue formation in rats. It prolonged the tail flick time in rats by more than two-fold compared with the control animals. The inhibition of interleukin and tumor necrosis factor by ethyl-p-methoxycinnamate was significant in both in vivo and in vitro models; however, only a moderate inhibition of nitric oxide was observed in macrophages. Furthermore, ethyl-p-methoxycinnamate considerably inhibited microvessel sprouting from the rat aorta. These mechanistic studies showed that ethyl-p-methoxycinnamate strongly inhibited the differentiation and migration of endothelial cells, which was further confirmed by the reduced level of vascular endothelial growth factor. CONCLUSION: Ethyl-p-methoxycinnamate exhibits significant anti-inflammatory potential by inhibiting pro-inflammatory cytokines and angiogenesis, thus inhibiting the main functions of endothelial cells. Thus, ethyl-p-methoxycinnamate could be a promising therapeutic agent for the treatment of inflammatory and angiogenesis-related diseases.

Fármacos são substâncias químicas sintéticas ou naturais que podem ser encontrados em medicamentos de prescrição médica, drogas terapêuticas isentas de prescrição e medicamentos veterinários. A ocorrência de fármacos no meio ambiente e no ciclo da água em quantidades vestigiais (na faixa de nanogramas a poucos microgramas por litro) tem sido amplamente discutida na literatura na década passada. O aumento na frequência de detecção é em grande parte atribuída aos avanços nas técnicas analíticas e instrumentação. Este artigo descreve o desenvolvimento, otimização e validação de um método de análise simultânea de sete produtos farmacêuticos de diferentes classes cafeína (CAF), prazosina (PRZ), maleato de enalapril (ENL), carbamazepina (CBZ), nifedipina (NFD), levonorgestre l (GNL), sinvastatina (SMV) utilizando extração em fase sólida (SPE cartuchos Oasis HLB ) seguido por cromatografia líquida acoplada a espectrometria de massas com tempo de voo e ionização por electrospray (LC-ESI-TOF/MS). A faixa linear de calibração, 0,5-250 µg L-1, proporcionou coeficientes de correlação linear (R2) acima de 0,99 para todos os compostos. Os limites de quantificação instrumental (IQL) para todos os produtos farmacêuticos variou de 0,5-5 µg L-1 no solvente por injeção direta de uma mistura padrão. A eficiência de extração (EE%), para a maioria dos compostos, foi superior a 40 e 60%, em água de rio e em água pura, respectivamente. O limite de quantificação (LOQ) para todos os produtos farmacêuticos variou de 13-800 ng L-1 para água de rio. A precisão inter e intra dia do método foi calculado, como o desvio-padrão relativo (RSD%) de 2,33-22,3% e 0,6-9,9% , respectivamente, exce to para a cafeína, que apresentou um RSD% de 20,1% a 50 µg L-1. O efeito da matriz variou entre 10-41%. Dos sete produtos farmacêuticos, seis compostos farmacêuticos foram detectados na amostra de água de rio.

Pharmaceuticals are synthetic or natural chemicals that can be found in prescription medicines, over-the-counter therapeutic drugs and veterinary drugs. The occurrence of pharmaceuticals in the environment and the water cycle at trace levels (in the range of nanograms to low micrograms per litre) has been widely discussed and published in literature in the past decade. The increase in detection is largely attributable to the advances in analytical techniques and instrumentation. This paper describes development, optimisation and validation of a method for the simultaneous analysis of 7 multi-class pharmaceuticals (caffeine (CAF), Prazosin (PRZ), enalapril maleate (ENL), carbamazepine (CBZ), nifedipine (NFD), levonorgestrel (LNG), simvastatin (SMV)) using solid phase extraction (SPE cartridges Oasis HLB) followed by liquid chromatography-time-of-flight/mass spectrometry (LC-ESI-TOF/MS). Its linearity, 0.5-250 µg L-1, provided determination coefficients (R²) above 0.99 for all compounds. The Instrumental Quantification Limits (IQLs) for all pharmaceuticals ranged from 0.5-5 µg L-1 in the solvent as a standard mixture (i.e., direct injection). The extraction efficiency (EE%) was more than 40% and 60% in river and deionised water for most of compounds, respectively. Limit of quantification (LOQ) for all pharmaceutic als ranged from 13-800 ng L-1 in spiked river water. The inter and intra day precision of the method, calculated as the relative standard deviation (RSD%), 2.33-22.3% and 0.6-9.9%, respectively, except for caffeine, which has a RSD% of 20.1% at 50 µg L-1. Matrix effect was investigated and ranged from 10-41%. Out of seven pharmaceuticals, six pharmaceutical compounds were detected in the river water sample.
