Resumo Para contribuir com a compreensão dos mecanismos relacionados à sintomatologia e aos sinais associados à erupção dentária, investigou-se a presença de mastócitos nos tecidos pericoronários na fase intraóssea (Grupo 1) e submucosa (Grupo 2), comparando-os entre si e com a mucosa bucal (Grupo 3). Em cada grupo, 14 espécimes foram analisados microscopicamente em cortes corados com hematoxilina e eosina, e imunocitoquimicamente marcados com Ckit e Triptase. Pelos resultados obtidos, concluiu-se que a quantidade/densidade dos mastócitos é diferente nos tecidos foliculares de acordo com a fase de erupção, o que permite inferir que: 1) O traumatismo decorrente da mastigação sobre o conjunto “mucosa bucal com o folículo pericoronário na submucosa” pode explicar porque o epitélio reduzido exporia o esmalte às células do tecido conjuntivo; 2) A exposição das proteínas do esmalte com propriedades antigênicas corresponderia à liberação de antígenos sequestrados que levariam à interação de IgE e mastócitos em número aumentado na região; e 3) A consequente degranulação e liberação de mediadores no local, como histamina, leucotrienes, prostaglandinas, proteases, citocinas e fatores de crescimento, contribuem para a compreensão dos sinais e sintomatologia atribuídos à erupção dentária.

Abstract This study contributes to the understanding of the mechanisms associated with signs and symptoms of tooth eruption, by investigating the presence of mast cells in pericoronal tissues during the intraosseous (Group 1) and submucosal (Group 2) phases of eruption. We compared findings for these two groups with each other and with those for the oral mucosa (Group 3). In each group, 14 specimens were analyzed microscopically after hematoxylin and eosin staining and immunohistochemical analysis of c-Kit and tryptase expression. Results revealed that the number and density of mast cells is different in follicular tissues according to the eruption phase, which may mean that: 1) masticatory trauma of the oral mucosa and dental follicles in the submucosa may explain why reduced enamel epithelium exposes enamel to the cells of the connective tissue; 2) exposure of antigenic enamel proteins might correspond to the release of sequestered antigens, which may lead to the interaction of IgE and a greater number of mast cells in the region; and 3) the consequent degranulation and the local release of mediators, such as histamine, leukotrienes, prostaglandins, proteases, cytokines and growth factors, contribute to the understanding of signs and symptoms associated with tooth eruption.

Quanto ao melhor momento para se aplicar ou recomendar a clareação dentária aos pacientes ortodônticos, alguns esclarecimentos e orientações são explanados para responder questionamentos como: 1) Por que depois do tratamento ortodôntico se constitui a melhor oportunidade para tal procedimento?; 2) Por que não realizar a clareação dentária imediatamente antes do tratamento ortodôntico?; 3) Se poderia realizá-la em alguma condição especial, e qual seria?; 4) Por que não se deveria clarear os dentes durante o tratamento ortodôntico?; 5) Se possível em algumas situações especiais, quando seriam essas situações especiais? No presente artigo, se destacará porque é fundamental sempre proteger a mucosa e a região cervical, independentemente do momento em que se fizer uma clareação dentária relacionada ou não ao tratamento ortodôntico. O mecanismo de como, por que e se é ou não conveniente clarear os dentes antes da finalização dos tratamentos ortodônticos ainda representa uma sugestão clínica, um procedimento em análise e um conhecimento empírico à espera de sua comprovação ou desmitificação científica. Apesar do esmalte dentário ter uma circulação do líquido adamantino, que propicia um metabolismo próprio e específico, que pode vir a ser, cientificamente, a base para explicar e fundamentar a clareação dentária antes e durante o tratamento ortodôntico, ainda assim devemos ser muito cautelosos.

With regard to the best moment for carrying out or recommending dental bleaching to orthodontic patients, some explanations and orientations are given in order to answers the following questions: 1) Why orthodontic treatment completion is considered the best opportunity for carrying out the procedure? 2) Why dental bleaching should not be performed immediately before orthodontic treatment? 3) If that would be possible at any special case, what would that be? 4) Why dental bleaching should not be performed during orthodontic treatment? 5) If that would be possible at any special case, what would that be? This article highlights why it is essential to protect both the mucosa and the cervical region, regardless of the moment when dental bleaching is performed, whether associated with orthodontic treatment or not. The "how", "why" and "if" of whether or not it is convenient to perform dental bleaching before orthodontic treatment are still a matter of clinical suggestion, as it is a procedure that is under analysis, empirical knowledge waiting for scientific proof or disproof! Although tooth enamel has adamantine fluid flowing within it, providing a specific metabolism that is peculiar to its own and which could scientifically explain and base the option of carrying out teeth whitening before and during orthodontic treatment, we must still be very careful.

Information about orthodontic movement of teeth with hypercementosis is scarce. As cementum deposition continues to occur, cementum is expected to change the shape of the root and apex over time, but this has not yet been demonstrated. Nor has it ever been established whether it increases or decreases the prevalence of root resorption during orthodontic treatment. The unique biological function of the interconnected network of cementocytes may play a role in orthodontic movement and its associated root resorptions, but no research has ever been conducted on the topic. Unlike cementum thickness and hypercementosis, root and apex shape has not yet been related to patient age. A study of the precise difference between increased cementum thickness and hypercementosis is warranted. Hypercementosis refers to excessive cementum formation above and beyond the extent necessary to fulfill its normal functions, resulting in abnormal thickening with macroscopic changes in the tooth root, which may require the delivery of forces that are different from conventional mechanics in their intensity, direction and distribution. What are the unique features and specificities involved in moving teeth that present with hypercementosis? Bodily movements would be expected to occur, since inclination might prove difficult to achieve, but would the root resorption index be higher or lower?

O objetivo do presente artigo é investigar a relação fluorose/cáries em escolas, de acordo com o nível de fluoreto no abastecimento público de água. A amostra consistiu em 360 alunos de doze anos de idade, de ambos os sexos, que frequentam as escolas próximas da região em que nasceram. As escolas foram classificadas em três grupos de acordo com a presença de fluoreto no abastecimento de água: 1) fluoretação na Estação de Tratamento de Água (ETA); 2) fluoretação direta em poços; e 3) áreas não fluoretadas (ANF). Os testes deχe de Goodman (significância = 5%) foram utilizados para avaliar a associação entre a origem da água e o grau de fluorose. Os resultados mais predominantes foram presença de cáries em toda a amostra (P<0,05); ausência de fluorose em ambos gêneros, para indivíduos brancos e aqueles vivendo em áreas abastecidas com água fluoretada diretamente de poços e não fluoretadas (P<0,05). Não houve diferença no grau de severidade da fluorose entre as fontes de abastecimento de água (P>0,05). As cáries ainda são uma importante questão nessa população, apesar do estabelecimento de medida preventiva. A observação da fluorose em alunos vivendo em áreas não fluoretadas confirma a presença de outras fontes de fluorose.

The sample comprised of twelve-year-old daytime students (360) of both genders, attending schools close to the region where they were born. The schools were classified into three groups according to the presence of fluoride in the water supply: 1) fluoridation in the Water Treatment Station-WTS; 2) Direct Fluoridation in Wells-DFW; and 3) Non Fluoridated Areas-NFA. The χ2 and Goodman tests (significance=5%) were used to evaluate the association between the origin of water and degree of fluorosis; percentage of distribution and descriptive measures were used for investigation of the degree of fluorosis. The following outcomes were predominant: presence of dental caries in the entire sample (P<0.05); absence of fluorosis in both genders, for white individuals and subjects living in areas supplied by both DFW and NFA (P<0.05). There was no difference in the severity of fluorosis between the sources of water supply (P>0.05). Dental caries is still an important problem in this population, despite the establishment of preventive measured. The observation of fluorosis in students living in non-fluoridated areas confirms the presence of other sources of fluoride.

OBJETIVO: Apresentar as características clínicas, tratamento cirúrgico e achado histológico de um caso de lipoidoproteinose. DESCRIÇÃO DO CASO: Criança do sexo masculino, cinco anos de idade, branco, que procurou atendimento odontológico na Universidade. A mãe da criança relatou presença de intensa halitose e dificuldade na alimentação e higienização bucal, decorrentes de crescimento gengival generalizado nos arcos dentários superior e inferior. No exame clínico, verificaram-se comprometimento funcional e estético generalizado (rouquidão, artralgia bilateral no joelho e tornozelo, lesões tumorais nas orelhas, entre outros), além de extensa hiperplasia gengival em ambos os arcos dentários. Optou-se pelo tratamento cirúrgico, com remoção do tecido hiperplásico e exodontia de todos os dentes decíduos e de dois permanentes. O exame histopatológico da peça cirúrgica confirmou o diagnóstico de lipoidoproteinose. COMENTÁRIOS: A lipoidoproteinose é uma doença rara caracterizada pela deposição da substância hialina na pele, membranas mucosas e nos órgãos internos. Os sinais que podem surgir após o nascimento, são: rouquidão; lesões pápulo-nodulares na cabeça, pescoço e membros; lesões papulares amareladas nas margens das pálpebras. O curso desta doença é benigno e crônico.

OBJECTIVE: Report clinical characteristics, histological findings and treatment of a patient with lipoid proteinosis. CASE DESCRIPTION: Five year-old male patient was assisted in the university clinic for dental treatment. The mother reported the presence of intense halitosis and difficulties for feeding and oral hygiene due to extensive gingival hyperplasia on the maxillary and mandibular arches. Clinical examination revealed generalized functional and esthetic involvement, including hoarseness; bilateral knee and ankle arthralgia; tumoral lesions on the ears and gingival hyperplasia throughout the extent of both dental arches. The surgical treatment removed the hyperplastic tissue and all deciduous teeth, as well as two permanents teeth. Histopathological examination of the specimen confirmed the diagnosis of lipoid proteinosis. COMMENTS: Lipoid proteinosis is a rare disorder characterized by progressive deposition of hyaline material in the skin, mucous membranes and organs. Clinical signs appear after birth and are characterized by hoarseness, papulonodular lesions on the head, neck and limbs and yellowish papules on the eyelid margins, among others. The disease has a benign and chronic evolution.
