OBJETIVO: Definir o perfil nutricional de idosos institucionalizados. M ÉTODOS: Estudo de campo comparativo correlacional e quantitativo realizado em uma Instituição de Longa Permanência, em São Paulo (SP), entre dezembro de 2010 a janeiro de 2012. Para definição do diagnóstico nutricional, coletaram-se dados de prontuários acerca de índice de massa corporal, circunferências, dobra cutânea tricipital, área muscular do braço, espessura do músculo adutor do polegar, força de preensão palmar e resultados das provas bioquímicas. As variáveis antropométricas foram apresentadas como média, desvio padrão e percentis, sendo agrupadas segundo o gênero e estratificadas por idade. O nível de significância estatística foi de p<0,05. RESULTADOS: Selecionaram-se 102 idosos, sendo 84 do gênero feminino. Excesso de peso foi o diagnóstico antropométrico que mais ocorreu nos homens (n=11; 61%) detectando-se depleção proteica a partir dos 70 anos e possíveis casos de obesidade sarcopênica. Todas as mulheres apresentaram-se eutróficas (n=84; 100%). Entretanto, em 27% (n=23) delas, evidenciou-se a depleção proteica. CONCLUSÃO: São necessários outros estudos antropométricos que permitam definir padrões de referências locais, estratificados por gênero e faixa etária. A diferença entre populações e fatores, como critérios de inclusão e exclusão e características metodológicas, limitam a utilização de padrões internacionais, interferindo na confiabilidade do diagnóstico nutricional.

OBJECTIVE: To define the nutritional profile of institutionalized elderly individuals. METHODS: Comparative correlation and quantitative field study conducted in a Long-Stay Institution in Sao Paulo (SP), Brazil, between December 2010 and January 2012. To define nutritional diagnosis, data were collected from patient files, such as body mass index, circumferences, triceps skinfold, muscle area of the arm, thickness of the adductor pollicis, handgrip strength, and biochemical test results. The anthropometric variables were presented as mean, standard deviation, and percentages, and were grouped by gender and stratified by age. The level of statistical significance was p<0.05. RESULTS: One hundred and two elderly individuals were selected, and 84 were females. Excess weight was the most common anthropometric diagnosis in men (n=11; 61%), with the detection of protein depletion in those aged 70 years, and possible cases of sarcopenic obesity. All women were in good health conditions (n=84; 100%). However, in 27% (n=23) of them, protein depletion was evident. CONCLUSION: More anthropometric studies are necessary which would allow a definition of local reference standards, stratified by gender and age group. The difference between populations and factors, such as inclusion and exclusion criteria, and methodological characteristics, limit the use of international standards, interfering in the reliability of the nutritional diagnosis.

OBJETIVO: Identificar o estado nutricional e o consumo alimentar de indivíduos com psoríase sistêmica e artropática associada. MÉTODOS: Pesquisa exploratória e transversal, na qual avaliaram-se 34 homens, de 19 a 60 anos, atendidos em um Centro de Psoríase, separando-os em Grupo PS (com psoríase sistêmica) e Grupo PAS (com sistêmica mais artropática). A avaliação nutricional deu-se pelo emprego da antropometria; bioimpedância e plestimografia de corpo inteiro. Aspectos clínicos e nutricionais foram investigados pela anamnese clínica, nutricional e recordatório de 24 horas. Empregou-se o teste General Linear Model (p < 0,05) para avaliação estatística. RESULTADOS: Segundo Índice de Massa Corporal, 29,4% (n = 10) apresentaram-se eutróficos; 41,2% (n = 14) com sobrepeso e 29% (n = 10) com obesidade. A maioria dos avaliados (60%; n = 21) apresentou valor da porcentagem de gordura (avaliada pela antropometria, bioimpedância e plestimografia de corpo inteiro) acima da normalidade (> 25%) e com risco alto para complicações metabólicas segundo CC e índice de obesidade, sem diferença estatística significativa entre os grupos. O consumo alimentar médio de lipídio, calorias e proteína apresentou-se acima do recomendado, sendo 58,8% para os lípides (319,17 ± 241,02 mg de colesterol e 17,42 ± 11,4 g de ácidos graxos saturados); 29,4% para as calorias e 67,6% para as proteínas. Assim, independentemente do tipo de psoríase, encontrou-se consumo excessivo de calorias, lípides, colesterol e ácidos graxos, além de maior ocorrência de excesso de peso. CONCLUSÃO: A amostra apresentou estado nutricional comprometido, aumento do risco para doenças crônicas relacionadas à obesidade, agravamento das lesões e má qualidade de vida.

OBJECTIVE: To identify the nutritional status and food intake of individuals with systemic psoriasis and psoriatic arthritis associated. METHODS: This is an exploratory and cross-sectional study with 34 men aged between 19 and 60 years seen at a Psoriasis Center. Participants were divided into systemic psoriasis group and arthritic-systemic psoriasis associated group. For nutritional assessment we used anthropometry, bioelectrical impedance analysis and whole-body plethysmography. Clinical and nutritional information were assessed using the clinical and nutritional history-taking, and the 24-hour dietary recall. For statistics the general linear model test (p < 0.05) was used. RESULTS: According to the body mass index 29.4% patients (n = 10) were eutrophic, 41.2% (n = 14) overweight and 29% (n = 10) obese. Almost all individuals (60%; n = 21) had body fat percentage above normal levels (> 25%) and a high risk for metabolic complications according to the waist circumference and the obesity index, however, there were no statistically significant differences between groups. The mean food intake, total fat, calories and protein were above recommended levels, being 58.8% for lipids (319.17 ± 241.02 mg of cholesterol and 17.42 ± 11.4 g saturated fatty acids); 29.4% for calories and 67.6% for proteins. Thus, regardless of the psoriasis type, an excessive consumption of calories, lipids, fatty acids, cholesterol and a higher incidence of overweight were found. CONCLUSION: The sample showed an abnormal nutritional condition, an increased risk for chronic diseases related to obesity, worsening of the psoriatic lesions, and poor quality of life.

RESUMO Objetivo: Avaliar e comparar a efetividade de diferentes métodos utilizados para avaliação nutricional de pacientes ambulatoriais hemiplégicos que sofreram acidente vascular encefálico. Métodos: Estudo transversal realizado com pacientes adultos e idosos, de ambos os gêneros, atendidos em um centro de reabilitação. As variáveis analisadas foram medidas antropométricas, bioimpedância, força de preensão palmar e espessura do músculo adutor do polegar. Utilizou-se o teste do χ2 de Pearson para verificar a associação entre as variáveis analisadas com nível de significância α = 5%. Resultados: Ao avaliar a associação entre os indicadores de massa muscular, observou-se que a força de preensão palmar em ambos os gêneros foi positivamente correlacionada com a circunferência muscular do braço (p = 0,0196) e massa magra (p = 0,0002). Já a massa gordurosa mensurada pelo método de bioimpedância apresentou relação inversa significante com a preensão palmar (r = −0,3879). A espessura do músculo adutor do polegar apresentou associação significante com massa magra (p = 0,0052) e preensão palmar (p = 0,0024). Conclusão: Neste estudo, a força de preensão palmar e espessura do músculo adutor do polegar foram bem correlacionadas com as mensurações determinadas pela antropometria e pela bioimpedância. Os resultados apontam a aplicabilidade da dinamometria e da espessura do músculo adutor do polegar na prática clínica como métodos de avaliação nutricional para essa população, especialmente idosos, pois detectam alterações funcionais não captadas por outros parâmetros em curto prazo, sendo importantes para identificação precoce do risco nutricional.

ABSTRACT Objective: To evaluate and compare the effectiveness of different methods used for nutritional assessment of outpatients who had hemiplegic stroke. Methods: A cross-section study with adult and elderly patients of both genders enrolled in a rehabilitation center. The analyzed variables were anthropometric measurements, bioelectrical impedance, hand grip strength and thickness of the adductor pollicis muscle. The Pearson χ2 test was used to check the association between variables with a significance level of α = 5%. Results: When evaluating the association between indicators of muscle mass, it was observed that the hand grip strength in both genders was positively correlated with arm muscle circumference (p = 0.0196) and lean mass (p = 0.0002). Fat mass measured by the bioelectrical impedance method already showed a significant inverse relationship with the grip (r = −0.3879). The thickness of the adductor pollicis muscle showed significant association with lean mass (p = 0.0052) and hand grip (p = 0.0024). Conclusion: In this study, the hand grip strength and thickness of the adductor pollicis muscle were well correlated with measurements determined by anthropometry and bioimpedance. The results show the applicability of grip strength and thickness of the adductor pollicis muscle in clinical practice as nutritional assessment methods for this population, especially elderly patients, since they detect functional changes not captured by other parameters in the short term and are important for early identification of risk nutrition.

Resumo Objetivo: Descrever o perfil antropométrico e a composição corporal de idosos por meio da antropometria, analisando a aplicabilidade das equações de predição de Petroski (1995) e de Durnin & Womersley (1974).Métodos: Foram avaliados 37 idosos de ambos os gêneros. As medições antropométricas foram realizadas segundo as diretrizes daInternational Society for the Advancement of Kineanthropometry, e determinaram as variáveis massa corporal, estatura, oito dobras cutâneas e sete perímetros musculares. Calcularam-se índice de massa corporal (IMC), relação cintura-quadril (RCQ), somatório de cinco dobras cutâneas e densidade corporal pelas equações de predição para idosos de Petroski (1995) e de Durnin & Womersley (1974) com conversão ao percentual de gordura (%G) pela equação de Siri (1961). Para posterior discussão, aplicou-se o teste t Student (p<0,05).Resultados: As variáveis idade, massa corporal, estatura e IMC não diferiram entre os gêneros. O somatório de cinco dobras cutâneas resultou em eutrofia para ambos os gêneros. A RCQ indicou "risco moderado" para homens e "risco alto" para mulheres, de complicações metabólicas. A equação de Durnin & Womersley (1974) subestimou nos indivíduos do gênero masculino e superestimou no gênero feminino o %G obtido pela equação de Petroski (1995), respectivamente. Por ambas as equações, os indivíduos do gênero feminino apresentaram %G superior aos do gênero masculino. Conclusões:Os indivíduos de ambos os gêneros apresentam elevada adiposidade corporal, especialmente na região omental. A equação de Petroski (1995) atendeu de modo satisfatório aos critérios de aplicação utilizados e mostrou ser adequada para a população em questão.

Abstract Objective: To describe anthropometric profile and body composition of elderly by anthropometry and evaluate the applicability of Petroski (1995) and Durnin & Womerseley (1974) protocols.Methods: We evaluated 37 elderly individuals of both genders. The anthropometric measurements were performed according to the guidelines of the International Society for the Advancement of Kineanthropometry and established the variables weight, height, eight skinfolds and seven muscle girths. It was calculated body mass index (BMI), waist-to-hip ratio (WHR), sum of five skinfolds and body density by Petroski (1995) and Durnin & Womersley (1974) elderly protocols with the conversion to percentage of body fat (%BF) by Siri (1961) protocol. For further discussion, it was applied the Studentt test (p<0.05). Results: Age, weight, height and BMI did not differ between genders. The sum of five skinfolds resulted in normal range for both genders. The WHR indicated "moderate risk" for men, and "high risk" for women to develop metabolic complications. The Durnin & Womersley (1974) protocol underestimated in male subjects and overestimated in female subjects the %BF obtained by the equation of Petroski (1995), respectively. For both protocols, female subjects showed higher %BF than male subjects. Conclusions: We conclude that individuals of both genders have high body fat, especially in the abdominal region. The Petroski (1995) protocol answered satisfactorily to the applying criteria used and showed to be adequate for this population.
