Resumo Introdução: Os profissionais da educação são um dos grupos ocupacionais mais importantes e representam uma das principais partes da economia da sociedade moderna. As demandas vocais mais citadas pelos professores são falar em pé, falar muito e em ambiente fechado, correspondendo às situações mais frequentes encontradas no ensino. Além disso, a permanência prolongada na posição ortostática pode repercutir em outros sistemas e gerar desvios posturais, dor e redução da funcionalidade. Objetivo: Verificar alterações no limiar de dor à pressão no índice de incapacidade e na postura craniocervical de professoras com queixas vocais e osteomusculares e com laringe normal, após liberação miofascial - pompage. Métodos: Ensaio clínico controlado e randomizado. Foram realizados anamnese, videolaringoscopia, triagem auditiva, avaliação postural clínica e fotogramétrica por meio do protocolo SAPo®, preenchimento do protocolo de autoavaliação de dor cervical Neck Disability Index e limiar de dor nos músculos cervicais por meio do Pain Pressure Threshold. A terapia miofascial com pompage teve um total de 24 sessões de 40 minutos cada, três vezes por semana, em 28 professoras do grupo estudo (GE) e 28 do grupo controle (GC). Em seguida, os grupos foram reavaliados. Resultados: O GE apresentou melhora significativa no limiar de dor de todos os músculos avaliados, na postura da maioria dos segmentos corporais avaliados e na incapacidade cervical. No GC houve melhora significativa no ângulo A2. Conclusão: Após terapia de liberação miofascial com pompage, os sujeitos apresentaram redução da dor cervical e da incapacidade funcional, aumento no limiar de dor e melhora da postura.

Abstract Introduction: Education professionals are one of the most important occupational groups and represents one of the main parts of the economy of modern society. The vocal demands most mentioned by the teachers are talking while standing up, talking a lot and in a closed environment, which corresponds to the most frequent situations encountered in teaching. In addition, remaining in the standing position for a prolonged period may have repercussions on other systems and generate postural deviations, pain and reduced functionality. Objective: To verify changes in pressure pain threshold, in disability index and in craniocervical posture of female teachers with vocal and musculoskeletal complaints, and with normal larynx, after myofascial release - pompage. Methods: This study was a controlled and randomized clinical trial. The following procedures were performed: anamnesis, videolaryngoscopy, hearing screening, clinical and photogrammetric postural assessment using the SAPo® protocol, completion of the neck pain self-assessment protocol Neck Disability Index, and pain threshold in the cervical muscles using the Pain Pressure Threshold. Myofascial therapy with pompage had a total of 24 sessions of 40 min each, three times a week, in 28 teachers assigned to the study group (SG), and 28 to the control group (CG). Afterwards, the groups were reassessed. Results: The SG presented a significant improvement in the pain threshold of all the muscles evaluated, in the posture of most of the body segments evaluated, and in the cervical disability. In the CG there was a significant improvement in angle A2 after therapy. Conclusion: After myofascial release therapy with pompage, the subjects presented a reduction in cervical pain and in functional disability, an increase in pain threshold, and posture improvement.

Resumo Introdução: A voz é muito influenciada pela respiração e pela musculatura abdominal. Objetivo: Verificar os efeitos imediatos da estimulação cervical e liberação diafragmática na função respiratória e fonatória de mulheres adultas sem queixas vocais. Método: Realizaram-se manobras de relaxamento e trabalho excêntrico do diafragma junto a uma manobra articulatória da terceira vertebra cervical. Fizerem parte da intervenção 24 mulheres sem queixas vocais, com idades entre 18 e 35 anos. Todas as voluntárias foram submetidas a uma avaliação de força da musculatura respiratória, do tempo máximo de fonação da vogal /a/, do nível de pressão sonora e análise vocal acústica, pré e pós-intervenção fisioterapêutica. Foram realizados os testes t-student para amostras independentes e correlação de Spearman com nível de significância de 5%. Resultados: Verificou-se aumento significativo no tempo máximo de fonação da vogal de /a/ e do nível de pressão sonora modal. Na análise acústica, houve redução dos valores do desvio-padrão da frequência fundamental; do quociente de perturbação do pitch suavizado; da variação da frequência fundamental e da variação da amplitude. Conclusão: As manobras fisioterapêuticas de estimulação cervical e liberação diafragmática melhoraram a qualidade vocal em relação ao tempo de sustentação da emissão, à pressão sonora, à estabilidade e ao ruído do sinal glótico.

Abstract Introduction: The voice is heavily influenced by breathing and abdominal muscles. Objective: To verify the immediate effects of cervical stimulation and diaphragmatic release on the respiratory and phonatory function of adult women with no vocal complaints. Method: Relaxation maneuvers and eccentric work of the diaphragm were performed together with articulatory maneuver of the third cervical vertebra. Twenty-four women without vocal complaints, aged between 18 and 35 years were part of the intervention. All volunteers were submitted to an evaluation of respiratory muscle strength, maximum phonation time of the vowel /a/, sound pressure level and acoustic vocal analysis, before and after physiotherapeutic intervention. Statistical analysis consisted of the Student's t-test for independent samples and Spearman's correlation. Significance level was set at 5%. Results: There was a significant increase in the maximum phonation time of the vowel /a/ and in the modal sound pressure level. Regarding the acoustic analysis, there was a reduction in the standard deviation values of the fundamental frequency; in the smoothed pitch disturbance quotient; and in the fundamental frequency and amplitude variations. Conclusion: Cervical stimulation and diaphragmatic release improved vocal quality regarding duration of emission, sound pressure, and stability and noise of the glottic signal.

Abstract Introduction For the singer, the voice is a medium used to express feelings that capture the listener. Every singing style has specific demands, and a vocal alteration may prevent the singer from meeting them. Objective To compare the singing style, the vocal habits, and the general health data of professional singers. Methods Cross-sectional, quantitative and retrospective study of a survey database. Data on the singing style, the vocal habits, and the health conditions and history of 57 professional singers, 31 female singers and 26 male singers, aged from 19 to 57 years old (average of 32 years old), from a mid-sized town were analyzed Results Therewas a prevalence of female (54 ± 2%) popular singers (91 ± 2%), in the adult age (51 ± 2%), nonsmokers (89 ± 2%), nonusers of alcohol (77 ± 2%), with respiratory problems (53 ± 2%), mainly rhinitis (23 ± 2%), and without other health problems. There was a significant use of alcohol in males (p = 0.010); among the alcohol users, there was a significant presence of respiratory problems (p = 0.046), of pharyngitis/tonsillitis (p = 0.003), and of gastroesophageal reflux (GER) (p = 0.043); there was a significant presence of GER in subjects reporting endocrine problems (p = 0.023), of gastritis (p = 0.023), and of pharyngitis/tonsillitis (p = 0.030). Conclusion There was a predominance of adult professional popular female singers, with complaints of respiratory issues (with a higher prevalence of rhinitis), without other general health issues, of nonsmokers, and of nonusers of alcohol.

RESUMO Introdução O recrutamento muscular é essencial para a manutenção do sistema respiratório e, em condições fisiopatológicas, reduz sua força muscular, levando à diminuição das pressões respiratórias e pressão aérea subglótica. Objetivo: Verificar e correlacionar a ativação da musculatura que compõe o centro de força corporal, os tempos máximos de fonação e a pressão sonora de mulheres adultas. Métodos Coleta do tempo máximo de fonação de /a/ e pressão sonora modal; avaliação da ativação do músculo transverso do abdome, multífido, assoalho pélvico e da pressão expiratória máxima de dez mulheres de 19 a 28 anos, sem queixas vocais e sem afecções laríngeas diagnosticadas por otorrinolaringologista. Foi aplicado o teste de Spearman. Resultados Cem por cento de ativação satisfatória do transverso do abdome; correlação positiva não significativa entre a pressão sonora, o tempo máximo de fonação e os valores de pressão expiratória máxima; maioria das mulheres com tempo máximo de fonação de /a/ discretamente abaixo do esperado e pressão sonora com valores dentro do esperado; ativação satisfatória do multífido, musculatura do assoalho pélvico e escala de Oxford, com correlação positiva, mas não significativa. Conclusão Neste grupo de mulheres adultas sem queixas vocais e sem afecções laríngeas, não foi verificada correlação entre a ativação da musculatura que compõe o centro de força corporal, os tempos máximos de fonação e a pressão sonora. No entanto, a maioria das mulheres apresentou ativação muscular satisfatória e, ao mesmo tempo, valores discretamente abaixo da normalidade para tempo máximo de fonação e normais, para pressão sonora.

ABSTRACT Introduction Muscle recruitment is essential for the maintenance of the respiratory system, which under pathophysiological conditions reduces its muscular strength, leading to a decrease in respiratory pressures and subglot air pressure. Purpose To investigate the correlation among muscle activation that makes up the center of physical force, the maximum phonation time and the sound pressure of adult women. Methods Collection of maximum phonation time of /a/ and modal sound pressure; assessment of muscle activation transversus, multifidus, pelvic floor and maximal expiratory pressure of ten women (19-28 years old) without vocal complaints and without laryngeal disorders diagnosed by otolaryngologist. The Spearman test was applied. Results A hundred per cent satisfactory activation of the transversus belly, no significant positive correlation among sound pressure, maximum phonation time and the maximal expiratory pressure values; majority of women with maximum phonation time of /a/ slightly below expectations and sound pressure values within the expected activation and satisfactory the multifidus, perineal muscles and Oxford scale, with positive correlation, but not significant. Conclusion Among adult women without vocal complaints and no laryngeal disorders studied, no correlation was found among the activation of the muscles that make up the center of physical force, maximum phonation time and the sound pressure, although most women has made satisfactory muscle activation and while slightly below normal values for maximum phonation time and normal sound pressure.

RESUMO Esta revisão da literatura levantou publicações científicas nacionais e internacionais sobre aquecimento e desaquecimento vocais da voz falada, por meio das bases de dados Lilacs, MedLine e Scielo, com o objetivo de identificar e descrever os parâmetros metodológicos e os efeitos das propostas de aquecimento e desaquecimento vocal descritas na literatura nacional e internacional. Foi realizada a localização e seleção dos estudos por meio de levantamento de textos publicados sobre o assunto no período de 1999 a 2013. Foram incluídos artigos originais de pesquisa; publicados na língua portuguesa ou inglesa; que pesquisaram aquecimento e desaquecimento vocal; associados ou não. O tempo de execução recomendado para o aquecimento variou de 15 a 30min ou três séries de 15 repetições, e para o desaquecimento foi de 5 a 15min. Os exercícios mais utilizados para o aquecimento foram sons fricativos sonoros e vibrantes de língua ou de lábios, em escalas ascendentes, e para o desaquecimento vocal foram sons nasais, fricativos sonoros e vibrantes de língua e ou de lábios, em escalas descendentes. Os exercícios mostraram mudanças vocais positivas, observadas por meio de medidas vocais acústicas e perceptivoauditivas, de autoavaliação e aerodinâmicas relacionadas à fonação. Houve unanimidade quanto à importância da realização do aquecimento e desaquecimento vocais para profissionais da voz e aos resultados positivos após a realização dos procedimentos. Não foi encontrada concordância quanto ao tempo de execução e os exercícios utilizados.

ABSTRACT For this literature review, we researched national and international scientific publications on vocal warm-up and cool-down of the speaking voice, through databases such as Lilacs, MedLine and Scielo, in order to identify and describe the methodological parameters and the effects of the vocal warm-up and cool-down proposals that are described in national and international literature. We performed the location and selection of studies through a survey of texts published on the theme from 1999 to 2013. We included original research articles; published in Portuguese or English; with emphasis on vocal warm-up and cool-down; associated or not. The recommended time of execution for warm-up ranged from 15 to 30 minutes or three sets of 15 repetitions, and the cool-down from 5 to 15min. The most used exercises for warm-up were voiced fricative sounds and vibrant of tongue or lips, in ascending scales, and for the vocal cool-down the nasal sounds, fricative sounds and vibrant of tongue or lips, in descending scales, were the most used exercises. The exercises showed positive vocal changes, observed by vocal acoustic and auditory-perceptual vocal and, self-evaluation and aerodynamic related to phonation. There was unanimity on the importance of completing the vocal warm-up and cool-down for voice professionals and on the positive results after carrying out the procedures. There was no agreement on the time of execution and on the exercises that were used.

O tema deste estudo é a Síndrome de tensão musculoesquelética, musculatura laríngea extrínseca e postura corporal. Tem como objetivo descrever, a partir de revisão de literatura,as características da musculatura extrínseca da laringe; postura corporal; Síndrome de tensão musculoesquelética e suas implicações vocais; avaliação e tratamento fisioterapêutico e fonoaudiológico. Foi realizado levantamento bibliográfico dos últimos 13 anos em que foram incluídos artigos cujo objetivo e/ou corpo do texto estivessem em consonância com os objetivos do estudo. A pesquisa foi realizada nos bancos de dados das bases Literatura Latino-Americana e do Caribe em Ciências da Saúde, Biblioteca Regional de Medicina, PublicMedline, Medical Literature Analysis and Retrieval System on Line, Scientific Electronic Library Online e Google Schoolar. Estabeleceram-se relações importantes do âmbito fisioterapêutico e fonoaudiológico, dentre as quais se destacam as relações musculares, posturais e funcionais na sinergia do aparato vocal. Na fisioterapia para redução da tensão muscular, a literatura aponta uso de eletroterapia transcutânea, laser de baixa intensidade, laser acupuntura, crioterapia, bem como terapia manual com tração, massagens, manipulações e mobilizações cervicais, associadas ou não a exercícios, alongamentos, relaxamento isométrico, mobilização assistida dos tecidos moles, exercícios terapêuticos visando à correção e equilíbrio muscular, reeducação respiratória diafragmática e terapia com flexão craniocervical. Na fonoaudiologia, foram encontrados apenas estudos com a manipulação digital da laringe e exercícios de trato vocal semiocluído.

The theme of this study is the musculoskeletal stress syndrome, extrinsic laryngeal muscles and body posture. The purpose is to describe, from the literature review, the characteristics of the extrinsic muscles of the larynx; posture; vocal implications of musculoskeletal stress syndrome; evaluation and physical therapy and voice therapy. We performed a literature review of the last 13 years which included articles aimed and/or body textwere in line with the objectives of this study. The research was conducted in the databases of Literatura Latino-Americana e do Caribe em Ciências da Saúde, Biblioteca Regional de Medicina, Public Medline, Medical Literature Analysis and Retrieval System on Line, Scientific Electronic Library Online and Google Schoolar. Were established important relationships within the physical therapy and voice therapy, among which stand out the relationships muscular, postural and functional synergy in the vocal apparatus. In physical therapy to reduce muscle tension, the literature indicates use of transcutaneous electrical stimulation, low level laser, laser acupuncture, cryotherapy, manual therapy and traction, massage, cervical manipulations and mobilizations, associated or not to exercise, stretching, isometric relaxation , assisted soft tissue mobilization, therapeutic exercises aimed at correcting balance and muscle, diaphragm and respiratory rehabilitation therapy craniocervical flexion. In speech therapy, studies found only through digital manipulation of the larynx and semi-ocluded vocal tract exercises.

OBJETIVO: verificar o nível de pressão sonora (NPS) modal e o tempo máximo de fonação (TMF) de /a/ de mulheres adultas, sem queixas vocais ou afecções laríngeas, antes, imediatamente após e cinco minutos após a execução da técnica de finger kazoo (FK). MÉTODO: coleta do TMF/a/, medida da frequência fundamental (f0) e seu desvio-padrão e do NPS modal, de 32 mulheres entre 18 e 40 anos de idade sem queixas vocais ou afecções laríngeas antes (M1), imediatamente após três séries de 15 repetições do FK (M2), e cinco minutos após silêncio (M3). Testes Spearman, Tukey e Friedman. RESULTADOS: aumento do NPS modal em M2 (com significância) e em M3; aumento de TMF/a/ em M2 e M3. CONCLUSÃO: no grupo pesquisado de mulheres adultas sem queixas vocais ou afecções laríngeas, houve aumento do NPS modal imediatamente após três séries de 15 repetições da técnica de FK, mostrando que a técnica pode influenciar a pressão sonora, a loudness e a projeção vocal.

PURPOSE: to determine the modal sound pressure level (SPL) and maximum phonation time (MPT) /a/ adult woman without vocal complaints or laryngeals disorders, before, immediately after and five minutes after the execution of the technique of Finger kazoo (FK). METHOD: collection of the MPT /a/, measure of fundamental frequency (f0) and its standard deviation, and measuring the modal SPL, 32 woman between 18 and 40 years of age without vocal complaints or laryngeal disorder before (M1), after three sets of 15 repetitions of FK (M2), and five minutes after silence (M3). Spearman, Tukey and Friedman tests. RESULTS: increase of modal NPS M2 (significance), and M3, increased of MPT /a/ in M2 and M3. CONCLUSION: in the studied group of adult woman without vocal complains or laryngeal disorders, there was a higher in the modal NPS immediately after three sets of 15 repetitions of the FK technique, showing that the technique can influence the sound pressure the loudness and vocal projection.

A doença de Erdheim-Chester é uma rara histiocitose de células não-Langerhans, de etiologia desconhecida, que apresenta manifestações sistêmicas, atingindo ossos, sistema nervoso central, olhos, pulmões, mediastino, rins e retroperitônio. Relatamos dois casos que cursaram com a apresentação típica da doença. Os achados radiológicos foram concordantes com a literatura e orientaram a suspeita diagnóstica, confirmada pelo exame imuno-histoquímico.

Erdheim-Chester disease is a rare non-Langerhans cell histiocytosis of unknown etiology, affecting multiple organ system, involving bones, central nervous system, eyes, lungs, mediastinum, kidneys and retroperitoneum. The authors report two cases that progressed with the typical presentation of the disease. Radiological findings were in agreement with literature and guided the diagnosis, confirmed by immunohistochemistry.
