Abstract The shock index (IS) is the quotient between the heart rate (HR) and the systolic blood pressure (SBP) (IS: HR / SBT), and the age-adjusted shock index (ISA) multiplying the IS by age. We evaluated its predictive value for the combined in-hospital event (EC), death and / or cardiogenic shock (CS) and for individual events in the patients included in the Argentine registry of ST-segment elevation infarction (ARGEN-ST-AMI); 248 with CS on admission were excluded. ROC curves were made for both indices using the best cut-off point to dichotomize the population. The analysis included 2928 subjects. Age (median) 60 years (IQR 25-75% 53-68), men 80%, EC: 6.4%; 30.5% had IS ≥ 0.67, and they had a higher incidence of EC: 11% vs. 4% (p < 0.001), cardiogenic shock (8% vs. 2.6%, p <0.0001) and death (7.3% vs. 3%), p <0.0001) than patients with IS < 0.67. A 28% had ISA ≥ 41.5. These presented plus EC: 14% vs. 3%, p < 0.001, SC: 10% vs. 2%, (p < 0.001) and death: 9.5% vs. 2.3%, (p < 0.001) compared with patients with values < 41.5. The area under the ROC curve of the ISA for EC was significantly better than that of the IS (0.72 vs. 0.62, p < 0.001). In the multivariate analysis models performed, the IS had an OR: 2.56 (95% CI 1.56-4.02; p < 0.001) and the ISA: 3.43 (95% CI 2.08-5.65; p < 0.001) for EC. The IS and ISA predict death and / or the development of in-hospital cardiogenic shock in an unselected population of ST elevation infarcts.

Resumen El índice de shock (IS) se obtiene mediante un cálculo simple del cociente entre la frecuencia cardíaca (FC) y la tensión arterial sistólica (PAS) (IS: FC/TAS) y el índice de shock ajustado por edad (ISA) multiplicando el IS x edad. Evaluamos su valor predictivo para el evento combinado intrahospitalario (EC) muerte y/o shock cardiogénico (SC) y de los eventos individuales en los pacientes incluidos en el registro argentino de infarto con elevación del segmento ST (ARGEN-IAM-ST). Se excluyeron 248 con SC de ingreso. Se realizaron curvas ROC para ambos índices utilizando el mejor punto de corte para dicotomizar la población. Se incluyeron 2928 pacientes. Edad (mediana) 60 años (RIC 25-75% 53-68), varones 80%, EC: 6.4%. Un 30.5% tuvo IS ≥ 0.67 y éstos presentaron mayor incidencia de EC: 11% vs. 4% (p < 0.001), shock cardiogénico (8% vs. 2.6%, p <0.0001) y muerte (7.3% vs. 3%, p < 0.0001) que los pacientes con IS < 0.67. Un 28% tuvo ISA ≥ 41.5. Estos presentaron más EC: 14% vs. 3%, p < 0.001, SC: 10% vs. 2%, (p < 0.001) y muerte: 9.5% vs. 2.3%, (p < 0.001) comparados con los pacientes con valores ISA < 41.5. El área bajo la curva ROC del ISA para EC fue significativamente mejor que la del IS (0.72 vs. 0.62, p < 0.001).En los modelos de análisis multivariados reali zados, el IS tuvo un OR de 2.56 (IC95% 1.56-4.02; p < 0.001) y el ISA de 3.43 (IC95% 2.08-5.65; p<0.001) para EC. El IS y el ISA predicen muerte y/o el desarrollo de shock cardiogénico intrahospitalario en una población no seleccionada de infartos con elevación del ST.

Hypothesis: the endovenous administration of glutamine, independently of the type of nurtrition received, can reduce the ICU length of stay, the incidence of infections and the mortality in the traumatic patients admitted to the ICU. Objectives: The main objective is to assess the efficacy of glutamine suplementation, given intravenously, to reduce the incidence of infectious complications, mortality and ICU length of stay in the traumatic patients admitted to the ICU. Other objectives are: 1) to assess the efficacy of glutamine in different groups of patients according to the severity and the plasma levels of glutamine. 2) Record all the adverse events due to the intravenous administration of glutamine. Methods: prospective, randomized, doble-blind and multicenter study with two parallel groups: placebo and treatment group. The patients who fulfill the inclusion criteria will receive either glutamine or placebo, independently of the type on nutrition. Glutamine will be administered as a pharmaconutrient at 0.5 g/kg/day during 5 days as a continous perfusion.

Hipótesis: La suplementación endovenosa con el dipéptido N(2)-L-alanil-L-glutamina, en pacientes traumáticos ingresado en la UCI puede ejercer un efecto beneficioso en cuanto a reducción de la incidencia de infecciones, de la estancia en UCI y de la mortalidad, independientemente del tipo de nutrición (enteral o parenteral) administrada. Objetivos: El objetivo principal es medir la eficacia de la suplementación con el dipéptido, administrado de forma endovenosa, en reducir la incidencia de las complicaciones infecciosas, la mortalidad y la estancia en UCI de los pacientes traumáticos ingresados en la UCI. Otros objetivos secundarios son: 1) Evaluar la eficacia de L-alanil-L-glutamina en diferentes grupos de gravedad y en relación con los niveles plasmáticos de glutamina. 2) Recoger los posibles efectos adversos del dipéptido administrado de forma endovenosa. Metodología: Ensayo clínico prospectivo, multicéntrico, aleatorizado y doble ciego, con dos grupos paralelos de tratamiento, para evaluar eficacia frente a placebo. Los pacientes traumáticos ingresados en la UCI que cumplan los criterios de inclusión/no exclusión, serán aleatorizados para recibir L-alanil-L-glutamina o placebo, independientemente del tipo de nutrición que reciba el paciente (enteral o parenteral) El dipéptido será administrado como un farmaconutriente en forma de perfusión continua durante 24 horas y durante 5 días a 0,5 g/kg/día.

Ovarian teratomas are the most frequent ovarian blastomas in childhood and adolescence. They arise from totipotential germ cells and can contain ectodermal, mesodermal and endodermal tissue. The struma ovarii is a teratoma in which thyroid tissue predominates. This is a very rare condition during childhood and adolescence. There are benign and malignant strumas ovarii, and they represent 2% and 0.3%, respectively, of all teratomas. They are mainly asymptomatic and they can manifest as an abdominal-pelvic tumor and cause ascites and hydrothorax. A 15-year-old girl with a benign struma ovarii is described. The adolescent presented with a slowgrowing abdominal tumor. She was rejected at school, due to a possible pregnancy.

Los teratomas de ovario son los blastomas ováricos más frecuentes en la etapa infanto-juvenil en la mujer: derivados de las células germinales totipotenciales pueden contener tejidos del ectodermo, mesodermo y endodermo. El estruma ovárico es un teratoma con predominio de tejido tiroideo. Son excepcionales en la etapa infanto-juvenil. Existen estrumas benignos y malignos y se presentan con una frecuencia de 2% y 0,3% respectivamente dentro de la frecuencia total de los teratomas. Asintomáticos en su mayoría, pueden manifestarse como tumor abdominopélvico y producir ascitis e hidrotórax. Se presenta un caso de estruma ovárico benigno en una adolescente de 15 años, que consultó por tumor abdominal de crecimiento lento, lo que motivó rechazo social en su medio escolar por presunción de embarazo.

Nitric oxide synthase activity was measured in Langerhans islets isolated from control and streptozotocin diabetic rats. The activity of the enzyme was linear up to 150 µg of protein from control rats and was optimal at 0.1 µM calcium, when it was measured after 45 min of incubation at 37oC in the presence of 200 µM arginine. Specific activity of the enzyme was 25 x 10-4 nmol [3H]citrulline 45 min-1 mg protein-1. Streptozotocin diabetic rats exhibited less enzyme activity both in total pancreas homogenate and in isolated Langerhans islets when compared to control animals. Nitric oxide synthase activity measured in control and diabetic rats 15 days after the last streptozotocin injection in the second group of animals corresponded only to a constitutive enzyme since it was not inhibited by aminoguanidine in any of the mentioned groups. Hyperglycemia in diabetic rats may be the consequence of impaired insulin release caused at least in part by reduced positive modulation mediated by constitutive nitric oxide synthase activity, which was dramatically reduced in islets severely damaged after streptozotocin treatment.
