OBJETIVO: comparar as mudanças relacionadas à autoestima e satisfação com aparência, entre o pré-operatório e o pós-operatório, de pacientes submetidos à cirurgia ortognática, e avaliar a qualidade de vida e mudanças psicossociais desses pacientes seis meses após a cirurgia. MÉTODOS: estudo qualitativo, de caráter observacional do tipo longitudinal, com amostra de 15 pacientes que apresentavam deformidades dentofaciais e que foram submetidos à cirurgia ortognática. Foram aplicados um questionário e dois formulários, em duas fases: uma pré-cirúrgica e outra pós-cirúrgica. RESULTADOS: os resultados mostraram que 13,3% dos pacientes obtiveram melhora em relação à autoestima e, principalmente, em relação à satisfação com a aparência. Foi também constatada melhoria nas relações sociais, profissionais e familiares. Em relação à avaliação da qualidade de vida segundo o questionário da Organização Mundial da Saúde, observou-se que a média mais baixa correspondeu ao domínio meio ambiente. CONCLUSÕES: a cirurgia ortognática traz consigo uma carga emocional elevada, que deve ser considerada antes e após o ato cirúrgico, pois o estado psicológico do paciente repercute em fatores favoráveis ou desfavoráveis durante a sua recuperação, influenciando de diferentes formas na qualidade de vida, autoestima e satisfação com a aparência.

OBJECTIVE: To compare changes related to self-esteem and appearance satisfaction between pre and postsurgical phases in patients undergoing orthognathic surgery and to assess the quality of life and psychosocial changes of these patients six months after surgery. METHODS: A longitudinal observational qualitative study was performed. The sample comprised 15 patients with dentofacial deformities who underwent orthognathic surgery. One questionnaire and two forms were answered during pre and postoperative phases. RESULTS: The results showed that 13.3% of patients demonstrated self-esteem improvement, especially in relation to appearance satisfaction. Improvements were also noted in social, occupational and family relationships. With regard to the assessment of quality of life, according to the World Health Organization questionnaire, the lowest improvement averages corresponded to environmental control. CONCLUSION: Orthognathic surgery brings along many emotional changes that should be considered before and after surgery, since the patients' psychological state may be favorable and/or unfavorable during recovery, influencing their quality of life, self-esteem and appearance satisfaction.

The multiform erythema appears as a systemic disease where skin and the mucous membranes have participation in relation to some factors such as bacterial or viral infections and in particular the drugs administration, analgesics and antibiotics in general. The aim of present paper was the presentation of case of big multiform erythema triggering by antimicrobials. Authors present the case of a male patient aged 29 with appearance of ulcerous bullous-vesicular lesions in lips, gums, tongue and genital mucosa under treatment with 400 mg norfloxacin due to urinary tract infection for a week. We made support treatment using mouthwashes for oral hygiene and corticoids ointment for ulcer protection, antihistaminics and nutritional guiding of a hypercaloric and hyperprotein liquid diet. This syndrome is characterized as an acute bullous eruptive process involving the patient's quality of life and there are not specific laboratory tests thus its diagnosis must to be based on a highly detailed review of anamnesis and the clinical findings.

El eritema multiforme, aparece como una enfermedad sistémica con la participación de la piel y las membranas mucosas en relación con varios factores como las infecciones bacterianas o virales, y en particular la administración de drogas, analgésicos y antibióticos en general. Se presenta un paciente masculino de 29 años de edad con eritema multiforme mayor desencadenado por antimicrobianos con la aparición de lesiones vesiculares-bulloso-ulcerosas en las regiones de los labios, encías, la lengua y la mucosa genital en tratamiento de una infección del tracto urinario con norfloxacino 400 mg por una semana. Fue realizado un tratamiento de soporte con el uso de colutorios para la higienización bucal y pomada a base de corticoide para protección de las úlceras, antihistamínicos y orientación nutricional de dieta líquida hipercalórica e hiperproteica. Este síndrome está caracterizado como un proceso eruptivo buloso agudo que compromete la calidad de vida del paciente y no hay pruebas de laboratorio específicas por lo que su diagnóstico debe estar basado en la revisión minuciosa de la anamnesis y en los hallazgos clínicos.

OBJETIVO: Avaliar o uso do OCT3 (Carl-Zeiss) para medir a espessura da camada de fibras nervosas em pacientes com neuroretinite unilateral subaguda difusa (DUSN) e correlacionar com a acuidade visual. MÉTODOS: Foi medido a espessura da camada de fibras nervosas, utilizando programa "RNFL thickness 3.4" e a melhor acuidade visual de pacientes com DUSN entre janeiro de 2005 e dezembro 2006. RESULTADOS: Trinta e oito pacientes, com idade entre 9-42 anos foram selecionados para este estudo, sendo que 20 casos apresentavam larva viva localizada. A média da RNFL foi 71,55 ± 27,26 nos olhos com DUSN e 103,07 ± 20,66 nos olhos contralaterais (p<0,001). Correlação de Pearson entre a acuidade visual e a espessura da camada de fibras nervosas foi r= -0,522 (p<0,001) nos olhos com DUSN e r= -0,097 (p=0,509) nos olhos contralaterais. CONCLUSÃO: A espessura da camada de fibras nervosas de pacientes com DUSN apresenta uma correlação diretamente proporcional com a acuidade visual. Novos estudos são necessários para reforçar a correlação entre a acuidade visual e a espessura da camada de fibras nervosas nos pacientes com DUSN com a finalidade de acompanhar os pacientes após o tratamento.

PURPOSE: To measure retinal nervous fiber layer (RNFL) thickness using OCT3 (Carl-Zeiss) in patients with diffuse unilateral subacute neuroretinitis (DUSN) with or without live worm and correlate it with visual acuity. METHODS: RNFL thickness, using RNFL thickness 3.4 program and best corrected visual acuity were measured in patients with DUSN between January 2005 and December 2006. RESULTS: Thirty-eight patients, aged 9 - 42 years were selected, of whom 20 had live worm. Mean RNFL was 71.55 ± 27.26 in the DUSN eye and 103.07 ± 20.66 in the contralateral eye (p<0.001). Pearson's correlation between visual acuity and RNFL was r= -0.522 (p<0.001) in the DUSN eye and r= -0.097 (p=0.509) in the contralateral eye. CONCLUSION: RNFL thickness in DUSN patients is directly proportional to visual acuity. Further research is needed to reinforce the correlation between visual acuity and thickness of the nerve fibers in patients with DUSN to follow them after the treatment.

Objective: The aim of this study was to compare 2% lidocaine and 2% mepivacaine with 1:100,000 epinephrine for postoperative pain control. Study design: A group of 35 patients, both genders were recruited, whose had ages ranged from 13 to 27 years-old and had two inferior third molars in similar positions to be extracted. The cartridges were distributed to the patients according to a randomised pattern, where lidocaine was in the control group and mepivacaine in the experimental group. Results: Results showed no significant association between the anesthetics and postoperative pain, pulp sensibility after one hour, gender, tooth position and duration of the surgical procedure. Conclusions: It was shown that lidocaine and mepivacaine have similar time of anesthesia, they are adequate for surgical procedures that last one hour, and there was no difference between the two anesthetics in relation to the severety of post-operative pain.
