Cistos ósseos uniloculares são as entidades mais comuns que afetam a região maxilofacial. O mecanismo de proliferação e expansão permanece obscuro. As metaloproteinases (MMPs) estão associadas a diversas condições patológicas. O objetivo do presente estudo foi correlacionar o aspecto radiográfico (área) e a presença de MMP-2 e MMP-9 em cistos dentígeros, cistos radiculares e tumores odontogênicos queratocísticos. A área radiográfica de cada lesão foi calculada usando a fórmula matemática da área de elipse. Todas as amostras foram submetidas à análise imunoistoquímica para estas enzimas. A área radiográfica média foi de 284,17 mm2, 235,81 mm2 e 381,81 mm2, respectivamente. As análises estatísticas não mostraram associação entre a imunorreatividade de MMPs e área radiográfica das lesões em todas as patologias estudadas, exceto para MMP-2 e cistos radiculares, nas quais as lesões menores tinham maior imunomarcação para esta enzima. Os resultados demonstraram que a quantidade de imunomarcação da MMP-2 e MMP-9 estão envolvidos com cistos dentígeros e radiculares na expansão óssea, ao passo que estas enzimas parecem estar relacionados com o comportamento biológico dos tumores odontogénicos queratocísticos, indicando invasão e proliferação celular. Além disso, há uma relação inversa entre a MMP-2 e MMP-9 em tumores odontogénicos queratocísticos (p=0,03; rs= -0,660), indicando atividade em diferentes regiões.

Unilocular bone cysts are the most common entities affecting the maxillofacial region. The mechanism of proliferation and expansion remains unclear. Metalloproteinases (MMPs) are associated to diverse pathological conditions. The aim of the present study was to correlate the radiographic aspect (area) and the presence of MMP-2 and MMP-9 in dentigerous cysts, radicular cysts and keratocystic odontogenic tumors. The radiographic area of each lesion was calculated using the mathematical formula of the ellipse area. All specimens were subjected to immunohistochemical analysis for these enzymes. The average radiographic area was 284.17 mm2, 235.81 mm2 and 381.81 mm2, respectively. Statistical analyses revealed no association between the immunoreactivity of MMPs and radiographic area of the lesions in all pathologies studied, except for MMP-2 and radicular cysts, for which smaller lesions had increased immunostaining for this enzyme. The results demonstrate that quantities of MMP-2 and MMP-9 are especially involved with dentigerous and radicular cysts in expansion, whereas these enzymes seem to be related to the biological behavior of keratocystic odontogenic tumors, indicating invasion and cell proliferation. Moreover, there is an inverse association between MMP-2 and MMP-9 in keratocystic odontogenic tumors (p=0.03; rs=-0.660), indicating activity in different regions.

OBJETIVO: Relatar o caso de um paciente com tetralogia de Fallot e sua condição bucal. DESCRIÇÃO DO CASO: Paciente do gênero masculino atendido no Curso de Especialização em Odontopediatria da Escola de Aperfeiçoamento Profissional da Associação Brasileira de Odontologia da seção Paraná dos cinco aos sete anos de idade. O exame clínico intrabucal inicial revelou quadro de cárie severa da infância. Durante o acompanhamento, verificou-se a erupção dos primeiros molares permanentes com a presença de defeitos de desenvolvimento de esmalte e perda de estrutura dental. À anamnese, a mãe relatou que o paciente foi portador de tetralogia de Fallot e que a cirurgia corretora foi realizada com um ano e 11 meses de idade. COMENTÁRIOS: A formação do esmalte dental dos primeiros molares permanentes ocorre a partir do primeiro mês de vida e é finalizada entre dois e quatro anos de idade. Neste caso, tal etapa coincidiu com o período anterior à cirurgia para correção da tetralogia de Fallot, fato que pode ter interferido negativamente no processo de mineralização dos primeiros molares permanentes. Os defeitos de desenvolvimento do esmalte podem levar a perda de estrutura dental, favorecendo o aparecimento da cárie dentária. Dessa forma, o odontopediatra deve realizar um diagnóstico precoce dessas alterações e intervir preventivamente para evitar a perda de tais dentes. Uma anamnese bem detalhada e a maior interação entre pediatras e odontopediatras podem proporcionar melhor acompanhamento de crianças com fatores de risco para apresentar defeitos de desenvolvimento do esmalte dentário.

OBJECTIVE: To report a case of a child with tetralogy of Fallot and his oral conditions. CASE DESCRIPTION: A male patient was assisted at the Pediatric Dentristry Specialization Course of the Brazilian Dentistry Association in the State of Paraná, Brazil, from five to seven years old. At the first intrabucal examination, the child presented a severe caries disease. During their treatment, enamel developmental defects were detected in the first permanent molar tooth, with loss of enamel structure. His mother reported that the patient had a surgical correction of tetralogy of Fallot cardiac malformation when he was 23 months old. COMMENTS: The enamel development of the first molar starts in the first month of life and ends around two to four years old. In this patient, the period of mineralization of the first molar was coincident with the period prior to the surgical correction of the cardiac problem. Probably, the development of dental enamel was impaired by the systemic repercussion of this congenital anomaly. Enamel developmental defects can present loss of dental structure, increasing the risk of caries. Therefore, the pediatric dentists should diagnose dental enamel developmental defects early, in order to prevent serious damages to the teeth. A detailed anamnesis and a good interaction between pediatricians and pediatric dentists will provide a better follow-up of children at risk of having dental enamel developmental defects.

OBJETIVOS: O objetivo do presente estudo foi comparar o início da erupção do primeiro dente decíduo em crianças nascidas prematuras (< 38 semanas) e a termo (38-42 semanas), com peso normal (³ 2.500 g), baixo peso (< 2.500 g) e muito baixo peso (< 1.500 g), para avaliar se a erupção dentária é afetada por prematuridade e baixo peso ao nascimento. MÉTODOS: Foram anotados os registros neonatais e o início da erupção do primeiro dente decíduo de 146 crianças, 77 prematuras e 69 à termo, na faixa etária de 5 a 36 meses, sem distinção de sexo, que se encontravam em acompanhamento médico no Ambulatório de Pediatria do Hospital Universitário Evangélico de Curitiba - Paraná. Para análise dos dados foi considerada a idade biológica, pós-concepção ou corrigida - idade gestacional mais a idade cronológica da criança no mês que erupcionou o primeiro dente. RESULTADOS: Os resultados mostraram que, de acordo com a idade cronológica, as crianças prematuras e com muito baixo peso ao nascimento tiveram um significante atraso na erupção dental. Entretanto, quando considerada a idade corrigida, não foi encontrada diferença estatisticamente significante entre os grupos. CONCLUSÃO: O atraso na erupção pode ser simplesmente devido ao nascimento precoce e não a um desenvolvimento dental atrasado.

OBJECTIVE: The aim of the present study was to compare the beginning of eruption of the first deciduous tooth in preterm infants (<38 weeks) with full-term infants (38 and 42 weeks) of normal birth weight (³2.500g), low birth weight (< 2.500g) and very low birth weight (<1.500g), in order to evaluate if premature birth and low birth weight would affect tooth eruption. METHODS: The neonatal records and the moment of eruption of the first deciduous tooth of 146 infants - 77 preterm infants and 69 full-term infants, ranging from 5 to 36 months old, of both genders - were recorded. All of them were under care at the Pediatric Ambulatory of Hospital Universitário Evangélico at Curitiba - Parana. Data were analyzed considering biological age and post-conception, or corrected, age - which is the gestational age plus the infant's chronological age at the month of eruption of the first deciduous tooth. RESULTS: Results showed that when chronological age is considered, tooth eruption in preterm and very low birth weight infants is importantly delayed. However, when corrected age is considered, no statistically significant differences were found among groups. CONCLUSION: The delayed eruption may be related to the premature birth and not to a delay in dental development.
