ABSTRACT Introduction: The identification of complications of primary palatoplasty may vary among professionals from different areas of health due to the lack of standardization of the fistula classification. This study aimed to verify the consensus among professionals of plastic surgery (PC) and Speech-Language Pathology (SLP), regarding the occurrence of fistula, according to what was reported in the same craniofacial service. Methods: Analysis of the chart’s records of the areas of the PC and SLP of 466 patients with cleft lip and palate was performed about the presence and location of fistula about the presence and location of fistulas, as reported in 466 medical records of patients with a history of unilateral cleft lip and palate. To compare the findings between both areas, a gold standard classification for the occurrence of fistula (GSF) was established by an experienced plastic surgeon. Results: The PC area reported that 25% of the 466 patients had a fistula compared to the 37% reported by the SLP, while the GSF indicated fistula in 35% of the cases. The Kappa statistic reveals regular agreement between GSF and PC (r = 0.32) and substantial agreement between GSF and SLP (r = 0.63). Conclusion: There was a discrepancy between the areas of Speech-Language Pathology and plastic surgery regarding the occurrence and location of the fistula after primary palatoplasty in the same craniofacial center. The data indicates the need to create and implement a standardized fistula classification system. In this way, craniofacial teams can use it effectively, taking advantage of the scientific evidence that emerges from the results of cleft lip and palate treatment.

RESUMO Introdução: A identificação de complicações da palatoplastia primária pode variar entre profissionais de diferentes áreas da saúde, devido à falta de padronização da classificação de fístula. Este estudo teve o objetivo de verificar o consenso entre profissionais da cirurgia plástica (CP) e da fonoaudiologia (FGA), quanto à ocorrência de fístula, conforme reportado em um mesmo serviço craniofacial. Métodos: Foi realizada uma análise dos registros das áreas da CP e FGA quanto à presença e localização de fístulas, conforme reportado em 466 prontuários de pacientes com história de fissura transforame incisivo unilateral. Para comparar os achados entre ambas as áreas uma verificação padrão ouro da ocorrência de fístula (POF) foi estabelecida por um cirurgião plástico experiente. Resultados: A área da CP reportou que 25% dos 466 pacientes apresentaram fístula comparado à 37% reportado pela FGA, enquanto o POF indicou fístula em 35% dos casos. Estatística Kappa revela concordância regular entre POF e CP (r=0,32) e concordância substancial entre POF e FGA (r=0,63). Conclusão: Observou-se discordância entre as áreas da fonoaudiologia e da cirurgia plástica quanto à ocorrência e localização da fístula após a palatoplastia primária, em um mesmo centro craniofacial. Os dados apontam para a necessidade da criação e da implementação de um sistema de classificação de fístula padronizado, que possa ser utilizado de forma efetiva por equipes craniofaciais favorecendo evidências científicas dos resultados do tratamento da fissura labiopalatina.

RESUMO O uso de álcool e drogas na gestação é difundido dentre os fatores de risco para as alterações fonoaudiológicas. Contudo, é observada a necessidade de estudos sobre uso dos mesmos e sua influência no processo interventivo. Foram apresentados dados da história clínica, avaliação pré e pós-intervenção de uma criança com 6 anos, com queixa de disfluências gagas e pais usuários de álcool e drogas desde o período de concepção até o momento do processo de intervenção relatado. Na avaliação foram aplicados os protocolos Stuttering Severity Instrument, Teste de Linguagem Infantil: prova de Fonologia, Vocabulário e Fluência e o Checklist de Habilidades Comunicativas Verbais. A criança foi diagnosticada com gagueira de grau leve a moderado, simplificações fonológicas e vocabulário defasado, dificuldade em manter turnos comunicativos, narrativa simplificada e queixa de dificuldade escolar. A criança foi submetida à intervenção nas áreas de Linguagem com enfoque na fluência e na fonologia, evoluindo na avaliação pós-intervenção. A alta vulnerabilidade familiar, a falta de um ambiente estimulador e o consumo de álcool e drogas durante a gestação, podem contribuir para as alterações de linguagem, fluência e, consequentemente, aprendizagem da criança exposta, devendo ser acompanhada pelo fonoaudiólogo para que se forneça apoio social à família.

ABSTRACT Drug use during pregnancy is a well-known risk factor for the manifestation of speech-language disabilities. However, the necessity of studies directed towards drug use and its influence on interventional speech process in cases of alterations of child language and fluency is observed. In this study, clinical history data are presented as well as pre and post-assessment tests (Speech and Language) and the description of an interventional process of a six-year child. The main communication complaint about the child was stuttering. In addition to the child’s clinical history, there was also knowledge of alcohol and legal and illegal drug use by the mother from before the moment of conception until this intervention process was reported. The protocols Stuttering Severity Instrument (SSI), Child Language Test (ABFW): phonology, vocabulary, and fluency tests, along with the Checklist of Verbal Communication Skills were applied. Based on these assessments, the child was diagnosed with mild to moderate stuttering, phonological simplifications, vocabulary inferior to that expected for his age, difficulty maintaining communicative turns, simplified narrative skills, alterations in psycholinguistic processes, and complaints from the school about his learning. The child underwent speech therapy in the areas of fluency and Child Language (phonological approach), showing evolution in the post-intervention assessment. Thus, in the present case, it became clear that the concomitant use of alcohol with exposure to cigarettes and marijuana may trigger changes in the acquisition and development of language, fluency and, consequently, learning.

RESUMO Objetivo: comparar os valores de nasalância em amostras de fala com e sem o uso de fricativa faríngea e, também, com e sem hipernasalidade. Métodos: um total de 840 amostras de fala foi analisado neste estudo. As amostras foram julgadas por três juízas experientes por consenso quanto aos aspectos hipernasalidade e fricativa faríngea. Os julgamentos foram distribuídos em quatro grupos: G1: 255 amostras de fala julgadas como representativas de hipernasalidade; G2: 130 amostras julgadas como representativas do uso de fricativa faríngea e hipernasalidade; G3: 280 amostras julgadas como representativas de fala normal em falantes com história de fissura labiopalatina; G4: 175 amostras julgadas como representativas de fala normal em falantes sem história de fissura labiopalatina. Para análise dos dados foi utilizando o teste Kruskal-Wallis e quando houve diferença estatisticamente significante foi aplicado o teste Dunn's para comparar os grupos aos pares. Resultados: os julgamentos aferidos por consenso pelas três juízas permitiram a identificação de amostras representativas do uso de fricativa faríngea e da presença e ausência de hipernasalidade. Foram estabelecidos valores de nasalância (média e desvio padrão) para cada grupo e observou-se que houve diferença estatisticamente significante entre os grupos com alteração de fala (G1 e G2) e aqueles sem alteração (G3 e G4). A diferença entre o grupo com hipernasalidade (G1) e o grupo com FF (G2) não foi significante. Conclusão: o uso de FF não influenciou significantemente os valores de nasalância para a amostra estudada.

ABSTRACT Purpose: to compare nasalance scores between speech samples with and without pharyngeal fricative and with and without hypernasality. Methods: a total of 840 speech samples was analyzed in this study. The samples were rated by three experienced judges with consensus regarding the aspects of hypernasality and pharyngeal fricative. The ratings were distributed into 4 groups: G1: 255 samples rated as representative of presence of hypernasality; G2: 130 samples rated as representative of use of pharyngeal fricative and hypernasality; G3: 280 samples rated as representative of normal speech for speakers with history of cleft palate; G4: 175 samples rated as representative of normal speech for speakers without history of cleft palate. Statistical analysis involved the Kruskal-Wallis test and when significant the Dunn's test was used to compared pairs of data. Results: the ratings established with agreement between the 3 experienced judges allowed for the identification of the samples representative of use of pharyngeal fricative and hypernasal speech. Nasalance scores were establish for each group revealing a significant difference between groups G1+G2 (representative of speech errors) and groups G3+G4 (representative of normal speech). The difference between the group with hypernasality (G1) and the group with pharyngeal fricative (G2) was not significant. Conclusion: the use of pharyngeal fricative did not influence significantly the nasalance values obtained for the studied sample.

Objetivo Correlacionar o desempenho motor oral nas tarefas de diadococinesia, de acordo com o tipo de fissura e com a presença de ceceio na fala, em crianças com fissura labiopalatina, operadas. Métodos Foram estudadas 30 crianças com fissura completa unilateral de lábio e palato e 30 com fissura isolada de palato, operadas, com idade entre 9 e 12 anos. Dois fonoaudiólogos analisaram a repetição de frases com os fones [s] e [z] para identificar a presença de ceceio e a análise da diadococinesia ocorreu a partir do registro da repetição das sílabas “ta” e “ca” e da sequência “pataca”. Na análise, o teste Kappa verificou a concordância entre os avaliadores, o teste Qui-quadrado a frequência de ceceio entre os grupos e o teste t os valores da diadococinesia, segundo o tipo de fissura e a presença de ceceio. Resultados O número de emissões por segundo e os tempos médios entre as emissões não demonstraram diferença significativa entre os grupos. Houve boa concordância entre os avaliadores quanto à presença de ceceio, com maior frequência nos casos com fissura completa unilateral de lábio e palato. A comparação entre os valores da diadococinesia e a presença de ceceio não evidenciou diferença significativa. Conclusão O desempenho nas tarefas da diadococinesia não sofreu alteração segundo o tipo de fissura labiopalatina. A presença de ceceio na fala foi maior no grupo com fissura completa unilateral de lábio e palato, sem modificação, porém, no desempenho das tarefas da diadococinesia.

Purpose To correlate the performance of the oral motor tasks of diadochokinesis in children with repaired cleft lip and palate to the type of cleft and to the presence of lisp. Methods We studied 30 children, aged 9 to 12 years, with complete unilateral cleft lip and palate and 30 with isolated cleft palate. Two speech language pathologists analyzed repetition phrases with the phones [s] and [z] to identify the presence of lisp. The analysis of diadochokinesis was performed by registering the repetition of the syllables “ta” and “ka” and the sequence “pataka”. The Kappa test was used to verify agreement between the evaluators, the Chi-square test was used to compare the lisp frequency between the groups, and the t-test was used to compare the diadochokinesis values between the same groups as well as between individuals with and without lisp. Results The mean number of emissions per second and the mean time between emissions were not significantly different between groups. Good agreement was observed between the examiners for the presence of lisp in [s] and [z], with the most evident agreement in the complete unilateral cleft lip and palate group. The comparison between the diadochokinesis values and the presence of lisp revealed no significant difference. Conclusion The performance of diadochokinesis tasks did not change according to the type of cleft. Further, the presence of lisp speech was higher in patients with complete unilateral cleft lip and palate, but this did not affect performance of diadochokinesis tasks.
