Resumen Las infecciones intrabdominales son frecuentes en todos los niveles de atención en salud, por ende, es necesario mantener un alto nivel de sospecha clínica, realizando las medidas más rápidas y costoefectivas para confirmar el diagnóstico y así ofrecer de una forma precisa y dirigida la terapéutica multidisciplinaria, siendo esta la única manera de tener impacto en la morbilidad de esta infección, disminuyendo la mortalidad y minimizando las complicaciones y los costos de la atención en salud. Las infecciones intrabdominales se encuentran ligadas a la aparición y selección de las mutantes resistentes tanto en las bacterias como en los hongos, convirtiéndose en la actualidad en una gran problemática en la salud pública. La creciente resistencia bacteriana al asociarse a mayor posibilidad de dificultades en el tratamiento antimicrobiano incrementa la mortalidad. Este consenso basado en la evidencia, reúne las recomendaciones en el diagnóstico y en el tratamiento de las infecciones intrabdominales en la población pediátrica y de adultos. Con un estricto seguimiento de la resistencia bacteriana y estimulando el control de los factores de riesgo que tienen mas impacto en la aparición de este fenómeno, este consenso pretende ser una practica guía de fácil implementación, y con periódicas actualizaciones favorecerá y facilitará el manejo multidisciplinario y la adecuación del manejo terapéutico de las infecciones intrabdominales.
Abstract Intra-abdominal infections are frequent at all levels of health care, therefore, it is necessary to maintain a high level of clinical suspicion, performing the fastest and most cost-effective measures to confirm the diagnosis and offer a precise and targeted multidisciplinary therapy, this being the only way to have an impact on the morbidity of this infection, reducing mortality and minimizing the complications and costs of health care. Intra-abdominal infections are linked to the appearance and selection of resistant mutants in both bacteria and fungi, becoming currently a major public health problem. Increasing bacterial resistance when associated with a greater possibility of difficulties in antimicrobial treatment increases mortality. This evidence-based consensus brings together the recommendations for the diagnosis and treatment of intra-abdominal infections in the pediatric and adult population. With strict monitoring of bacterial resistance and stimulating the control of the risk factors that have the greatest impact on the appearance of this phenomenon, this consensus is intended to be a practical guide that is easy to implement, and with periodic updates it will favor and facilitate multidisciplinary and the adequacy of the therapeutic management of intra-abdominal infections.

Abstract Background: Currently, a double-J catheter is placed in 70% of patients that undergo ureteroscopy because it is believed to decrease obstruction secondary to ureteral edema or stone fragments. However, catheter placement is not innocuous, given that it can result in a variety of side effects, such as urinary frequency, urgency, hematuria, dysuria, and tenesmus, as well as increase costs. The aim of the present study was to evaluate current double-J catheter management in patients after ureteroscopy and the associated postoperative symptoms. Materials and methods: A retrospective study analyzing ureteroscopies performed at the Hospital Dr Manuel Gea Gonzalez throughout 2017 on patients above 18 years of age was conducted. Urology service re-admission frequency during the first postoperative week, the symptoms associated with double-J catheter use, and postoperative complications were evaluated. Results: A total of 105 ureteroscopies were registered. Male sex was predominant (55%) and semirigid ureteroscopies were the most prevalent procedures (71%). A double-J catheter was preoperatively placed in 69 patients and postoperatively placed in 41 patients. During follow-up, the patients with double-J catheter placement presented with associated symptoms warranting an emergency room visit. Pollakiuria was the only significant association found (p=0.001). Ninety-five patients were stone-free, 61 (64.2%) of whom had double-J catheter placement. Two patients with no catheter presented with residual stones. Conclusions: Of the 61 patients with double-J catheter, 57 (82%) presented with at least one symptom related to catheter placement. A total of 64.2% patients with a double-J catheter had uncomplicated ureteroscopy. Therefore, we believe it is necessary to conduct a randomized, prospective study with a larger number of patients to confirm the previously reported results and more clearly and objectively establish the indications for double-J catheter use at our hospital.
Resumen Antecedentes: Actualmente el 70% de los pacientes operados de ureteroscopia reciben colocación de catéter doble J debido a que se cree que disminuye la obstrucción secundaria a edema ureteral o a fragmentos de lito, pero la colocación no es inocua, ya que conlleva una gran variedad de efectos secundarios como frecuencia, urgencia, hematuria, disuria y tenesmo, así como un impacto económico. Nuestro objetivo es evaluar el manejo actual del catéter doble J en pacientes posoperados de ureteroscopia y los síntomas posquirúrgicos asociados. Materiales y métodos: Estudio retrospectivo de ureteroscopias realizadas en el Hospital Dr. Manuel Gea González durante el 2017 en pacientes mayores a 18 años. Se analizaron las frecuencias en readmisiones al servicio de urgencias durante la primera semana posoperatoria y los síntomas asociados al uso de catéter doble J, así como complicaciones posoperatorias. Resultados: Se registraron 105 ureteroscopias. El sexo masculino fue el predominante en hasta 55%; las ureteroscopias semirrígidas fueron las más prevalentes con un 71%. Se colocó preoperatoriamente catéter doble J en 69 pacientes y posoperatoriamente se colocaron en 41 pacientes. Durante el seguimiento los pacientes con colocación de catéter JJ refirieron síntomas asociados a la colocación del catéter, lo cual ameritó visita a urgencias, encontrando únicamente asociación significativa para polaquiuria (p=0.001). Se obtuvo un total de 95 pacientes libre de litiasis, de los cuales a 61 (64.2%) se les colocó catéter doble J y 2 quedaron con litiasis residual y sin catéter. Conclusiones: De los 61 pacientes que se les colocó catéter doble J, 57 (82%) presentaron al menos un síntoma relacionado con el catéter; el 64.2% fueron ureteroscopias no complicadas a las cuales se les colocaron catéter doble J, por lo que creemos necesario realizar un estudio prospectivo y aleatorizado con un mayor número de pacientes para confirmar los resultados previamente reportados y establecer de manera más clara y objetiva las indicaciones para el uso del catéter doble J en nuestro hospital.

Abstract Attention-deficit hyperactivity disorder (ADHD) is one of the most prevalent disorder during childhood. The present study aimed to evaluate if the Conners Continous Performance Test was able to discriminate among ADHD and normal children completed the Conners computarized test. Significant differences between normal and clinical sample were found. All children selected as normal did not fit the clinical profile. Only 50% of the children considered as having ADHD fit the clinical profile. Implications for the issue of overdiagnosis of the disorder are discussed.
Resumen El trastorno de déficit atencional con hiperactividad (TDAH) constituye uno de los cuadros de mayor prevalencia durante la niñez. El presente trabajo se focalizó en evaluar si el Test de Rendimiento Continuo de Conners era capaz de discriminar entre niños con y sin TDAH diagnosticados por el docente. Se conformó una muestra no probabilística de 30 niños escolares (15 clínicos y 15 normales) a través de la aplicación del cuestionario de atención. Todos los niños completaron la aplicación del test computarizado de Conners. Se observaron diferencias significativas entre la muestra normal y clínica. Todos los niños seleccionados como normales no encajaron el perfil clínico. Solo el 50% de los casos considerados con TDAH encajaron el perfil clínico. Se analizan las implicancias del sobre/diagnóstico del trastorno.

Introducción: el trasplante combinado de hígado y riñón (CLK) ha mostrado ser una buena alternativa para pacientes con diagnóstico concomitante de enfermedad renal crónica (ERC) y enfermedad hepática terminal. Algunos estudios han mostrado además un beneficio inmunológico del trasplante combinado, con disminución de la tasa de rechazo del injerto renal. Objetivo: describir las indicaciones y los resultados clínicos en receptores de trasplante CLK en un hospital de alta complejidad. Materiales y métodos: se seleccionaron los pacientes con trasplante CLK del registro institucional de trasplante (TRENAL) entre 2000 y 2011. Se describieron las características demográficas y clínicas y se estimó la sobrevida de los pacientes y de los injertos con el método Kaplan Meier. Resultados: en un período de 11 años, se realizaron 16 trasplantes CLK. Esto corresponde al 1.51% de los trasplantes de riñón y al 3.48% de los trasplantes de hígado hechos en la institución durante el mismo periodo de tiempo. La mayoría de los receptores fueron de sexo masculino (10/16). La mediana de la edad fue 56.5 años. La mediana del MELD fue de 17 (RIQ: 12.5-20.5, Rango 8-24). El diagnóstico hepático más frecuente fue cirrosis por NASH (4/16). Todos los pacientes tenían diagnóstico de ERC, en 4 casos secundaria a diabetes mellitus. Las indicaciones más comunes del trasplante fueron ascitis de difícil manejo, encefalopatía recurrente y desnutrición. El tiempo promedio de isquemia en frío del hígado fue de 7,3 horas, y del riñón 9,6 horas. La sobrevida de los pacientes y de los injertos hepático y renal fue de 87,5% al año y 67,3% a los 5 años. Cuatro pacientes murieron, 2 casos como consecuencia de sepsis y otros 2 por malignidad. Conclusión: el trasplante CLK en la Fundación Valle del Lili tiene resultados clínicos satisfactorios y comparables a los reportados en otros centros.
Introduction: Combined liver and kidney transplantation (CLK) has been shown to be a good alternative for patients with concomitant diagnosis of chronic kidney disease (CKD) and end-stage liver disease. Some studies have also shown immunological benefits from combined transplantation with decreased rates of kidney graft rejection. Objective: The objective of this study was to describe the indications and clinical outcomes of CLK transplant recipients in a highly complex hospital. Materials and Methods: CLK transplant patients were selected from the institutional transplant registry (Trenal) from 2000 to 2011. Demographic and clinical characteristics were described and survival of patients and grafts were estimated with the Kaplan-Meier method. Results: Over a period of 11 years, 16 CLK transplants were performed. This was 1.51% of kidney transplants and 3.48% of liver transplants done in the institution during this period. Most recipients were male (10/16). The median age was 56.5 years. The median MELD was 17 (IQR: 12.5 to 20.5, range: 8 to 24). The most frequent diagnosis was liver cirrhosis due to NASH (4/16). All patients had been diagnosed with CKD: four cases were secondary to diabetes mellitus. The most common indications for transplants were difficult to manage ascites, recurrent encephalopathy and malnutrition. The average liver cold ischemia time was 7.3 hours, and the average kidney cold ischemia time 9.6 hours. The five-year liver graft survival rate was 87.5%, and the five-year kidney graft survival rate was 67.3%. Four patients died: two as the result of sepsis and two as the result of malignancy. Conclusion: CLK transplantation results at the Fundación Valle del Lili have been satisfactory and comparable to those reported by other transplant centers.

In previous immuno-epidemiological studies of the naturally acquired antibody responses to merozoite surface protein-1 (MSP-1) of Plasmodium vivax, we had evidence that the responses to distinct erythrocytic stage antigens could be differentially regulated. The present study was designed to compare the antibody response to three asexual erythrocytic stage antigens vaccine candidates of P. vivax. Recombinant proteins representing the 19 kDa C-terminal region of MSP-1(PvMSP19), apical membrane antigen n-1 ectodomain (PvAMA-1), and the region II of duffy binding protein (PvDBP-RII) were compared in their ability to bind to IgG antibodies of serum samples collected from 220 individuals from the state of Pará, in the North of Brazil. During patent infection with P. vivax, the frequency of individuals with IgG antibodies to PvMSP1(19), PvAMA-1, and PvDBP-RII were 95, 72.7, and 44.5% respectively. Although the frequency of responders to PvDBP-RII was lower, this frequency increased in individuals following multiple malarial infections. Individually, the specific antibody levels did not decline significantly nine months after treatment, except to PvMSP1(19). Our results further confirm a complex regulation of the immune response to distinct blood stage antigens. The reason for that is presently unknown but it may contribute to the high risk of re-infection in individuals living in the endemic areas.
