RESUMEN Apendicitis aguda es la secuencia de inflamación, perforación, formación de absceso y peritonitis, causado con mayor frecuencia por obstrucción del lumen apendicular. La apendicitis es la patología quirúrgica no obstétrica más frecuente en el embarazo. Su incidencia puede ser variable y se presenta con mayor proporción durante el primer y segundo trimestre de gestación. Se presenta el caso de una paciente de 34 años de edad, con 36 semanas de gestación. Se diagnostica abdomen agudo quirúrgico y posible apendicitis aguda complicada. En sala de operaciones se confirma diagnóstico de apendicitis aguda complicada, revelando un plastrón apendicular y evolución favorable. Durante la gestación el diagnóstico de la apendicitis es difícil debido a los cambios anatómicos y fisiológicos que se producen, por lo que recalcamos la importancia de describir esta condición en el embarazo.
ABSTRACT Acute appendicitis is the sequence of inflammation, perforation, abscess formation and peritonitis, most frequently caused by obstruction of the appendicular lumen. Appendicitis is the most frequent non-obstetric surgical pathology in pregnancy. Its incidence can be variable and occurs with a greater proportion during the first and second trimester of pregnancy. The case of a 34-year-old patient with 36 weeks of gestation is presented. Surgical acute abdomen and possible complicated acute appendicitis are diagnosed. In the operating room, the diagnosis of acute appendicitis was confirmed, revealing an appendiceal plastron and favorable evolution. During pregnancy, the diagnosis of appendicitis is difficult due to the anatomical and physiological changes that occur, so we emphasize the importance of describing this condition during pregnancy.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Effect of light intensity and nutrient concentration on growth and pigments of the green microalga Tetraselmis suecica

ABSTRACT Tetraselmis suecica is a green microalga that thrives under a wide range of conditions, used in the commercial culture of fish, mollusk, and crustacean larvae for supplementing the demand for fertilizers. Its pigments have applications in human health care as drug products, vitamins, and cosmetics. Growth and pigment concentration of T. suecica were evaluated in experimental cultures with different nutrient concentrations and light intensities to determine the most appropriate culture conditions to optimize the production of biomass and pigments. Chlorophyll-a, chlorophyll-b, lutein, violaxanthin, α, β-carotene, and neoxanthin concentrations were evaluated under three different nutrient conditions (441.5/18.1, 883/36.3, and 1766/76.2 μM of NaNO3/NaH2PO4) and four light intensities (50, 150, 300, and 750 μmol quanta m−2 s−1). Increases in either or both of these factors lead to increases in the concentration of all pigments. Chlorophyll-a reached up to 5×103 mg m−3, chlorophyll-b up to 2500 mg m−3, lutein 600 mg m−3, violaxanthin 300 mg m−3, α, β-carotene 500 mg m−3, and neoxanthin 400 mg m−3. Growth rate (μ) attained values of 1.6 d−1. An index to evaluate the efficiency of pigment production by light intensity (called LER) was computed. The highest LER was recorded at 50 μmol quanta m−2 s−1 and a nutrient concentration of 1766/76.2 μM (NaNO3/NaH2PO4); this treatment optimizes pigment production with the lowest light intensity. Our results show that the optimum light intensity should be selected according to the objective of the culture, either maximizing pigment concentration for harvesting at higher concentrations or reducing production costs regarding light consumption.

OBJECTIVES: To prospectively evaluate demographic, anthropometric and health-related quality of life (HRQoL) in pediatric patients with laboratory-confirmed coronavirus disease 2019 (COVID-19) METHODS: This was a longitudinal observational study of surviving pediatric post-COVID-19 patients (n=53) and pediatric subjects without laboratory-confirmed COVID-19 included as controls (n=52) was performed. RESULTS: The median duration between COVID-19 diagnosis (n=53) and follow-up was 4.4 months (0.8-10.7). Twenty-three of 53 (43%) patients reported at least one persistent symptom at the longitudinal follow-up visit and 12/53 (23%) had long COVID-19, with at least one symptom lasting for >12 weeks. The most frequently reported symptoms at the longitudinal follow-up visit were headache (19%), severe recurrent headache (9%), tiredness (9%), dyspnea (8%), and concentration difficulty (4%). At the longitudinal follow-up visit, the frequencies of anemia (11% versus 0%, p=0.030), lymphopenia (42% versus 18%, p=0.020), C-reactive protein level of >30 mg/L (35% versus 0%, p=0.0001), and D-dimer level of >1000 ng/mL (43% versus 6%, p=0.0004) significantly reduced compared with baseline values. Chest X-ray abnormalities (11% versus 2%, p=0.178) and cardiac alterations on echocardiogram (33% versus 22%, p=0.462) were similar at both visits. Comparison of characteristic data between patients with COVID-19 at the longitudinal follow-up visit and controls showed similar age (p=0.962), proportion of male sex (p=0.907), ethnicity (p=0.566), family minimum monthly wage (p=0.664), body mass index (p=0.601), and pediatric pre-existing chronic conditions (p=1.000). The Pediatric Quality of Live Inventory 4.0 scores, median physical score (69 [0-100] versus 81 [34-100], p=0.012), and school score (60 [15-100] versus 70 [15-95], p=0.028) were significantly lower in pediatric patients with COVID-19 at the longitudinal follow-up visit than in controls. CONCLUSIONS: Pediatric patients with COVID-19 showed a longitudinal impact on HRQoL parameters, particularly in physical/school domains, reinforcing the need for a prospective multidisciplinary approach for these patients. These data highlight the importance of closer monitoring of children and adolescents by the clinical team after COVID-19.

Resumo A triquinelose é uma doença causada por nematoides do gênero Trichinella. Dez espécies foram descritas. A identificação das espécies a partir das larvas só foi possível com ferramentas moleculares. O abate doméstico de suínos e o consumo de animais de caça, sem inspeção veterinária, são as principais fontes de infecção humana. A caça de carnívoros nativos é proibida no Chile por questões de conservação e pelo fato de que esses animais favorecem o controle de pragas. A caça ilegal de um puma (Puma concolor) aconteceu em setembro de 2020. No Chile, a identificação molecular de larvas de Trichinella, obtida pelo exame de sequências nucleares (segmento de expansão V) e mitocondriais (citocromo C oxidase subunidade I) são descritas. Tanto a amplificação da região do segmento de expansão V quanto a análise filogenética de um segmento de um fragmento da sequência da subunidade I da Citocromo c-oxidase, confirmaram que as larvas pertenciam a T. spiralis. O caso aqui descrito representa a primeira evidência de caça ilegal de um mamífero protegido infectado com Trichinella no Chile, destacando a perspectiva de “Saúde Única” para enfrentar esta doença na interfase rural-silvestre.
Abstract Trichinellosis is a zoonosis caused by nematodes of the Trichinella genus, of which 10 species have been described. Species identification when larvae is found is only possible using molecular tools. Meat from pigs and game animals not subjected to veterinary inspection are the main sources of human infections. The hunting of native carnivores is prohibited in Chile due to conservation issues and the fact that those animals favor pest control. The illegal hunting of a cougar (Puma concolor) occurred in September 2020. Herein, the molecular identification of Trichinella larvae, by analyzing nuclear (expansion segment V) and mitochondrial (cytochrome C oxidase subunit I) sequences are described. Both the amplification of the expansion segment V region and the phylogenetic analysis of a segment of a fragment of the cytochrome c-oxidase subunit I sequence confirmed that the larvae belonged to T. spiralis. The case described herein represents the first evidence of illegal hunting of a protected mammal infected with Trichinella in Chile, highlighting the ‘One Health’ perspective to face this disease in the rural-sylvatic interphase.

Resumo Introdução: Paralisia facial periférica pode ocorrer devido a uma grande variedade de causas e está associada ao comprometimento de atividades diárias básicas, como comer, beber, falar e comunicação social, afeta a qualidade de vida dos pacientes. O facial disability index é um questionário autoaplicado desenvolvido especialmente para avaliar o impacto da disfunção facial sobre aspectos físicos e psicossociais dos pacientes. Embora tenha sido validado e se mostrado superior a outros questionários, ainda não fora submetido à adaptação transcultural e validação para a língua portuguesa. Objetivo: Realizar a tradução, adaptação cultural e validação do facial disability index para o português falado no Brasil. Método: A tradução e adaptação cultural do facial disability index foram realizadas em diferentes estágios conforme recomendações internacionais para adaptação de medidas de resultados. Para a verificação de confiabilidade e validação da versão em português, o facial disability index foi aplicado em 100 pacientes na forma de teste/reteste. Resultados: A confiabilidade da versão em português do facial disability index foi considerada adequada, com coeficiente alfa de Cronbach de 0,73. A avaliação de correlação intraclasse foi de 0,79 (95% IC 0,71-0,85 ) e de 0,85 (95% IC 0,78-0,89) para as subescalas física e de bem-estar social, respectivamente. Houve uma correlação significativa entre a escala de bem-estar social da versão em português do facial disability index e as dimensões de função social e saúde mental do questionário geral de qualidade de vida Short Form - 36. Também foi demonstrada uma correlação entre a versão em português do facial disability index e o grau de disfunção facial de acordo com a escala global de House-Brackmann. Conclusão: A versão adaptada do facial disability index para o português falado no Brasil se apresenta como um instrumento válido e confiável para avaliação do impacto da disfunção facial sobre aspectos físicos e psicossociais dos pacientes.
Abstract Introduction: Facial paralysis may occur due to a variety of causes. It is associated to the impairment of some basic daily activities such as eating, drinking, speaking and social communication, which affects the quality of life of these patients. The facial disability index is a short form auto reported outcome questionnaire used to assess patient with facial paralysis. It has been validated and proved to be superior to other general health related quality of life questionnaires. Objective: We aim to do the cultural adaptation and validate the facial disability index into Brazilian Portuguese. Methods: Translation and cultural-adaptation following the stages recommended by the International Society of Pharmacoeconomics Outcomes Research task force. The questionnaire was administered to 100 patients for evaluation of reliability and validation. Results: The reliability of the Portuguese version of the facial disability index was found to be adequate, with a Cronbach's alfa coefficient of 0.73 for the complete scale. Intra-class correlation was 0.79 (95% CI: 0.71-0.85) and 0.85 (95% CI: 0.78-0.89) for the physical and social well-being subscales. There was a significant correlation between the social well-being subscale of the Portuguese version of the facial disability index and the social function and mental health components of the SF-36. There was also a correlation between the facial disability index and the degree of facial dysfunction according to the House-Brackmann global scale. Conclusion: This adapted version of the facial disability index provides a valid and reliable instrument to assess the physical and psychosocial impact of facial nerve dysfunction in Brazilian-speaking patients.

Resumo Objetivo: Este estudo com acompanhamento de longo prazo visou a avaliar o quadro clínico, os achados laboratoriais, o perfil histológico, os tratamentos e os resultados de crianças e adolescentes com hepatite autoimune. Métodos: Foram analisados os prontuários médicos de 828 crianças e adolescentes com HAI. Foi usado um questionário para coletar os dados anônimos sobre o quadro clínico, os achados bioquímicos e histológicos e os tratamentos. Resultados: De todos os pacientes, 89,6% tinham hepatite autoimune-1 e 10,4% hepatite autoimune-2. O sexo feminino foi predominante nos dois grupos. A idade média no início dos sintomas foi 111,5 (6; 210) e 53,5 (8; 165) meses nos pacientes com hepatite autoimune-1 e hepatite autoimune-2, respectivamente. Foi observado início clínico agudo em 56,1% e 58,8% e sintomas insidiosos em 43,9% e 41,2% dos pacientes com hepatite autoimune-1 e hepatite autoimune-2, respectivamente. A probabilidade de insuficiência hepática foi 1,6 vezes maior para hepatite autoimune-2; 3,6% e 10,6% dos pacientes com hepatite autoimune-1 e hepatite autoimune-2, respectivamente, apresentaram insuficiência hepática fulminante; o risco foi 3,1 vezes maior para hepatite autoimune-2. Os níveis de gamaglobulina e imunoglobulina G foram significativamente maiores nos pacientes com hepatite autoimune-1, ao passo que os níveis de imunoglobulina A e C3 foram menores em pacientes com hepatite autoimune-2; 22,4% dos pacientes apresentaram cirrose e a remissão bioquímica foi atingida em 76,2%. A taxa de sobrevida atuarial foi de 93,0%. Um total de 4,6% pacientes foram submetidos a transplante de fígado e 6,9% morreram (hepatite autoimune-1: 7,5%; hepatite autoimune-2: 2,4%). Conclusões: Nesta grande série clínica de crianças e adolescentes brasileiros, a hepatite autoimune-1 foi mais frequente e os pacientes com hepatite autoimune-2 mostraram maiores taxas de remissão da doença com respostas mais rápidas aos tratamentos. Os pacientes com hepatite autoimune-1 apresentaram maior risco de óbito.
Abstract Objective: This large study with a long-term follow-up aimed to evaluate the clinical presentation, laboratory findings, histological profile, treatments, and outcomes of children and adolescents with autoimmune hepatitis. Methods: The medical records of 828 children and adolescents with autoimmune hepatitis were reviewed. A questionnaire was used to collect anonymous data on clinical presentation, biochemical and histological findings, and treatments. Results: Of all patients, 89.6% had autoimmune hepatitis-1 and 10.4% had autoimmune hepatitis-2. The female sex was predominant in both groups. The median age at symptom onset was 111.5 (6; 210) and 53.5 (8; 165) months in the patients with autoimmune hepatitis 1 and autoimmune hepatitis-2, respectively. Acute clinical onset was observed in 56.1% and 58.8% and insidious symptoms in 43.9% and 41.2% of the patients with autoimmune hepatitis-1 and autoimmune hepatitis-2, respectively. The risk of hepatic failure was 1.6-fold higher for autoimmune hepatitis-2. Fulminant hepatic failure occurred in 3.6% and 10.6% of the patients with autoimmune hepatitis-1 and autoimmune hepatitis-2, respectively; the risk was 3.1-fold higher for autoimmune hepatitis-2. The gamma globulin and immunoglobulin G levels were significantly higher in autoimmune hepatitis-1, while the immunoglobulin A and C3 levels were lower in autoimmune hepatitis-2. Cirrhosis was observed in 22.4% of the patients; biochemical remission was achieved in 76.2%. The actuarial survival rate was 93.0%. A total of 4.6% underwent liver transplantation, and 6.9% died (autoimmune hepatitis-1: 7.5%; autoimmune hepatitis-2: 2.4%). Conclusions: In this large clinical series of Brazilian children and adolescents, autoimmune hepatitis-1 was more frequent, and patients with autoimmune hepatitis-2 exhibited higher disease remission rates with earlier response to treatment. Patients with autoimmune hepatitis-1 had a higher risk of death.

Resumen La agricultura orgánica es la medida agroambiental y alternativa agroecológica que resulta en una agricultura de valor agregado. Este estudio determinó a través de la interpolación del mapeo de análisis geoestadístico si las medidas orgánicas utilizadas fueron biointensivas, y se modificó el suelo para cumplir con los requisitos para el cultivar de arándano. El área de estudio fue un huerto de arándanos de 0.88 hectáreas, en el municipio de Cusihuriachi, Chihuahua, México, que había sido sometido a 3 tratamientos distintos de enmienda orgánica del suelo para camas elevadas: (t1) yeso y azufre, (t2) yeso, azufre y compost y (t3) yeso, azufre, compost y polvo de aserrín de pino. Veinticuatro muestras de suelo se tomaron de 0 a 30 centímetros en todo el huerto de las camas elevadas, 75 días después de la aplicación de los 3 tratamientos del suelo. Las técnicas de análisis de suelo se usaron para determinar el porcentaje de arcilla en el suelo, el pH del suelo, la cantidad de nitratos mg.kg-1 de suelo y el porcentaje de materia orgánica en el suelo. El análisis geoestadístico se determinó a través de grupos de similitud y diferencia con el agrupamiento Ward. Con los resultados del análisis del clúster Ward se generó un mapa interpolado para cada parámetro al utilizar ArcMap y la interpolación de ponderación de distancia inversa. La conclusión de este estudio fue que el mapeo de interpolación era una herramienta de visualización útil para determinar que no había diferencia ponderada entre los 3 tratamientos de enmienda y que ninguno de los tratamientos fue lo suficientemente efectivo como para emendar el suelo a las condiciones deseadas por el cultivar de arándano en los parámetros observados. Los mapas también fueron útiles para determinar las condiciones específicas del suelo, fuera del área de tratamiento, que podrían ser utilizadas por el productor para determinar un plan de gestión de agricultura de precisión, que podría considerarse como una medida agroambiental adicional.
Abstract Organic agriculture are the agri-environmental measures and agri-ecological alternatives which result in value-added agriculture. This study determined through interpolation of geostatistical analysis mapping if the organic measures were bio-intensive, modifying the soil to meet the requirements for the blueberry cultivar. The area of study was a blueberry orchard of 0.88 hectares, in the municipality of Cusihuriachi, Chihuahua, México, which had undergone three distinct organic soil emendation treatments to raised-beds: (t1) gypsum and sulfur, (t2) gypsum, sulfur and compost, and (t3) gypsum, sulfur, compost and pine saw dust. Twenty-four soil samples were taken at 0 to 30 centimeters throughout the orchard from the raised beds, 75 days after the application of the three soil treatments. Soil analysis techniques were used to determine percent of clay in soil, pH of soil, amount of nitrates mg.kg-1 of soil, and percent of organic material in soil. The geostatistical analysis was determined through groups of similarity and difference with cluster Ward grouping. With the results from the analysis of cluster Ward an interpolated map was generated for each parameter using ArcMap and inverse distance weighting interpolation. The conclusion from this study was that interpolation mapping was a useful visualization tool, to determine that there was no weighted difference between the three emendation treatments, and that none of the treatments were effective enough to emendate the soil to the conditions desired by blueberry cultivar for the parameters observed. The maps were also helpful in determining specific soil conditions, outside of the treatment area, which could be utilized by the producer to determine a precision agriculture management plan, which could be considered as an additional agri-environmental measurement.

Resumo: O objetivo deste estudo foi estabelecer dados de referência sobre a produção lacrimal (STT-1) e pressão intraocular (PIO) em lobos-guará em cativeiro. Foram utilizados 10 lobos-guará, saudáveis e adultos. Os animais foram contidos com cambão e mordaça e mantidos em decúbito lateral esquerdo. O TLS foi realizado no terço médio da pálpebra inferior, durante um minuto. A córnea foi dessensibilizada e a PIO mensurada com tonopen. A média do TLS-1 dos olhos direitos e esquerdo foi 11±5mm/min, não havendo diferença significativa entre os olhos (p=0,960). A média da PIO dos olhos direitos e esquerdo foi 20±6mmHg, não observando diferença entre os olhos direitos e esquerdos (p=0,836). Média STT-1 foi menor do que, e PIO foi o mesmo que os níveis normais em cães. Não houve diferença estatística na idade dos animais e valores STT-1 e da PIO. No presente trabalho, os níveis médios de guará STT-1 foram mais baixos em comparação com as observadas nos cães, enquanto que a pressão intraocular foi a mesma nos lobos guará como em cães. Devido ao aumento da incidência na prestação de cuidados de emergência para vítimas de lobos-guará atropelamentos e incêndios, determinar os valores de referência dos parâmetros oculares podem melhorar o diagnóstico correto e tratamento de doenças.
Abstract: The purpose of this study was to establish baseline data on lacrimal quantity (STT-1) and intraocular pressure (IOP) in captive maned wolves. Ten healthy adult maned wolves were contained with a snare pole and muzzle and kept in decubitus of the left side. STT-1 measurement was performed on the lateral third of the lower conjunctival sac for one minute. The cornea was desensitized and intraocular pressure was measured with an tonopen. Average STT-1 in both eyes was 11±5mm.min-1, with no statistical difference between the left and right eye (p=0.960). Average IOP in both eyes was 20±6mmHg, with no statistical difference between the left and right eye (p=0.836). Average STT-1 was lower than, and IOP was the same as normal levels found in dogs. There was no statistical difference in the age of the animals, and STT-1 and IOP values. In the present paper, average maned wolf STT-1 levels were lower compared with those found in dogs, while the IOP was the same in maned wolves as in dogs. Due to the increased incidence in providing emergency care for maned wolf victims of road kill and fires, determination reference values of ocular parameters may improve the correct diagnosis and treatment of the disease.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Perfil clínico y funcional de los pacientes con insuficiencia cardíaca sistólica y disfunción renal

Background Renal dysfunction is strongly associated with chronic heart failure, and is a frequent and progressive complication of this condition, with clinical outcomes which depend on the functional class and pharmacological treatment. Objectives To define the prevalence and the clinical and functional profile of patients with renal dysfunction in the setting of heart failure with depressed ejection fraction. Material and Methods Creatinine clearance was measured in 132 patients with heart failure and an ejection fraction <40%. Renal dysfunction was defined as a creatinine clearance <60 ml/min. Patients underwent routine lab tests, echocardiogram, bioelectrical impedance analysis and 6-minute walk test. Results The prevalence of renal dysfunction was 43.2% (57 patients). Among these patients, there was a greater prevalence of women and elder subjects, yet the prevalence of coronary artery disease was low. Lab tests showed lower levels of hemoglobin, albumin, ferritin and triiodothyronine, and greater prevalence of measurable levels of troponin T. Body mass index (BMI), basal metabolic rate, muscle mass and phase angle were lower, while the ratio of extracellular water to total body water was greater in patients with renal dysfunction. The distance walked in the 6-minute test was lower. The use of beta blockers and amiodarone was the only difference in therapy. At multivariate analysis, age, NTproBNP level, the presence of positive troponin T and muscle mass as a continuous variable were independent predictors of renal dysfunction in patients with systolic heart failure. Conclusions Renal dysfunction is frequent in patients with heart failure with depressed ejection fraction and is associated with distinctive features that may contribute to explain the clinical picture.
Introducción La disfunción renal está fuertemente asociada con la insuficiencia cardíaca crónica. Es una complicación habitual y progresiva de esta condición, con una evolución clínica que fluctúa con la clase funcional y el tratamiento farmacológico. Objetivos Definir la prevalencia y el perfil clínico y funcional de pacientes con disfunción renal en el contexto de insuficiencia cardíaca con baja fracción de eyección. Material y métodos En 132 pacientes con insuficiencia cardíaca y fracción de eyección < 40% se midió la depuración de creatinina. Se definió disfunción renal a una depuración de creatinina < 60 ml/ min. Se realizaron analítica sanguínea, ecocardiograma, bioimpedanciometría y caminata de 6 minutos. Resultados La prevalencia de disfunción renal fue del 43,2% (57 pacientes). Comparados con el resto, en estos pacientes hubo mayor prevalencia de sexo femenino, mayor edad y menos frecuentemente etiología coronaria. En el laboratorio tuvieron valores menores de hemoglobina, albúmina, ferritina y triyodotironina y mayor prevalencia de troponina T dosable. Fueron menores el índice de masa corporal (IMC), la tasa metabólica basal, la masa muscular y el ángulo de fase, mientras que la relación agua extracelular/agua corporal total fue mayor. La distancia recorrida en la prueba de 6 minutos fue menor. Respecto del tratamiento, sólo difirió el uso de betabloqueantes y amiodarona. En el análisis multivariado, la edad, el nivel de NT-proBNP, la presencia de troponina T positiva y la masa muscular como variable continua fueron predictores independientes de disfunción renal en pacientes con insuficiencia cardíaca sistólica. Conclusiones La disfunción renal es frecuente en pacientes con insuficiencia cardíaca con baja fracción de eyección y se vincula con características distintivas que pueden contribuir a explicar el cuadro clínico.

O sangue de cordão umbilical e placentário (SCUP) é uma rica fonte de células-tronco (CT) hematopoéticas e é amplamente utilizado como substituto da medula óssea em casos de transplante. As células do SCUP possuem vantagens sobre as células da medula óssea (MO), principalmente por serem mais jovens e apresentarem maior taxa proliferativa. Além dos progenitores hematopoéticos, o sangue de cordão umbilical contém progenitores endoteliais e mesenquimais, sugerindo sua possível aplicação nos novos protocolos de terapia celular para diferentes tecidos. Na presente revisão, discutimos a importância do armazenamento do sangue de cordão umbilical autólogo e as pesquisas desenvolvidas para a sua aplicação em doenças degenerativas.
Umbilical Cord Blood is a rich source of hematopoietic stem cells widely used as a substitute of bone marrow (BM) in transplants. Cells from umbilical cord blood present advantages over BM cells, mainly as they are younger and a have higher proliferative rate. Besides hematopoietic stem cells, umbilical cord blood contains endothelial and mesenchymal progenitor cells, suggesting their possible application in cell therapy protocols for different tissues. In this paper, we discuss the importance of autologous umbilical cord blood storage and the research on stem cell transplantation for degenerative diseases.

Um dos fatores que podem determinar a eficácia de parasitóides é como esses agentes de controle biológico exploram o habitat de seus hospedeiros. Este estudo avaliou a resposta de fêmeas de Trichogramma pretiosum Riley (Hymenoptera: Trichogrammatidae) à oviposição da praga Tuta absoluta (Meyrick) (Lepidoptera: Gelechiidae) em tomate industrial. Mariposas de T. absoluta foram liberadas em gaiolas contendo plantas de tomate e, após 12h, foi realizada a liberação de fêmeas de T. pretiosum. Após 24h da liberação do parasitóide, a oviposição da praga foi mapeada de acordo com as estruturas no dossel da planta de tomate e, posteriormente, foram determinados os níveis de parasitismo. O parasitismo de ovos da praga variou de 1,5 a 28%. Não houve influência significativa das estruturas da planta na taxa de parasitismo dos ovos da praga, exceto pela ausência de parasitismo na parte apical da planta. Ambos os níveis de oviposição de T. absoluta e parasitismo por T. pretiosum foram maiores no terço superior da planta de tomate.
One important factor determining the efficacy of parasitoids is the way they exploit different host patch. This study evaluated the response of females of Trichogramma pretiosum Riley (Hymenoptera: Trichogrammatidae) to the oviposition sites of Tuta absoluta (Meyrick) (Lepidoptera: Gelechiidae) on processing tomato plants. In fully developed caged tomato plants T. absoluta moths were released, followed by the release of T. pretiosum females 12h later. After 24h of parasitoid release, the moth oviposition sites were mapped according to the plant canopy, and levels of parasitism assessed. The parasitism rate varied from 1.5 to 28%. There was not influence of plant structures on parasitism, except for the absence of parasitism on the plant apex. Levels of both T. absoluta oviposition and parasitism by T. pretiosum were higher on the upper third of the plant, decreasing downward along the plant canopy.

The eggs and five nymphal instars of Phloea subquadrata Spinola, 1837 are described and illustrated. Three new host plants are registered, and data on the development and biology of the insect are provided and discussed.
