Cochlear radiation dose and hearing loss in patients with vestibular schwannoma undergoing radiosurgery: systematic review

Abstract Objectives: To determine the cut-off point of the cochlear radiation dose as a risk factor for hearing loss in patients with vestibular schwannoma treated with radiosurgery. Methods: A systematic review of the literature was performed without language or publication year restrictions in the MEDLINE/PubMed, EMBASE, Web of Science, LILACS/VHL and Cochrane Library databases. Studies that met the following criteria were included: 1) population: adults of both sexes who underwent radiosurgery for vestibular schwannoma treatment; 2) exposure: cochlear radiation; 3) outcome: hearing loss; 4) type of study: cohort. Two independent reviewers conducted the entire review process. The registration number in PROSPERO was CRD42020206128. Results: From the 333 articles identified in the searches, seven were included after applying the eligibility criteria. There was no standardization as to how to measure exposure or outcome in the included studies, and most studies did not present sufficient data to enable meta-analysis. Conclusion: It was not possible to determine a cut-off point for high cochlear dose that could be considered a risk factor for hearing loss.

ABSTRACT: Spirochetes in the genus Borrelia (Spirochaetes) are the etiological agents of two zoonotic diseases that have rodents as reservoirs: the Lyme disease, transmitted by ixodid ticks (Ixodidae), and tick-borne relapsing fever, which is transmitted mostly by argasid ticks (Argasidae). The information on the epidemiology of pathogens that affect animals is scarce, especially in places that have been barely studied, such as the Caatinga biome. Thus, in the present study, we aimed to detect DNA of Borrelia in the blood of wild and domestic mammals, as well as in ticks collected in the Catimbau National Park, State of Pernambuco, Northeast Brazil. Blood was collected from 77 captured wild animals belonging to Didelphidae (45), Echimyidae (21), Cricetidae (7), Caviidae (2), and Dasypodidae (2) families. Additionally, blood samples from 120 sheep, 119 goats, 70 dogs, and 47 cattle were obtained. A total of 286 ticks were collected in the environment and identified as Ornithodoros cf. tabajara (188) and Ornithodoros rietcorreai (98); in addition, larvae of Ornithodoros spp. were collected on Thrichomys laurentius (4 specimens) and Monodelphis domestica (1); larvae of O. rietcorreai on Didelphis albiventris (2) and T. laurentius (1); Amblyomma parvum on T. laurentius (4); Rhipicephalus sanguineus (sensu lato) on dogs (121) and Rhipicephalus microplus on cattle (47). All blood samples, 120 free-living ticks, 46 O. rietcorreai and 74 O. cf. tabajara, and 12 R. sanguineus s.l. were submitted to DNA extraction to perform a polymerase chain reaction (nested PCR), aiming at amplifying a fragment of the flaB gene present in all species of Borrelia. Only one free-living specimen of O. cf. tabajara (1.3%) was positive. Negative detections in mammals may be related to the absence of infection or the low concentration of the spirochetes in the hosts. The low prevalence of Borrelia found in ticks can be explained by the low infection rates that these spirochetes depict in their vectors. New studies focusing on the detection of antibodies, as well as on the sequencing and isolating Borrelia spp. should be encouraged for a better understanding of the epidemiology of these bacterial agents in the region.
RESUMO: No gênero Borrelia (Spirochaetes) encontram-se dois grupos de agentes responsáveis por duas doenças zoonóticas, que têm roedores como reservatórios e acometem humanos: o grupo de Lyme, transmitido por carrapatos ixodídeos, e a febre recorrente, que pode ser transmitida por carrapatos argasídeos ou piolhos. Poucas são as informações sobre epidemiologia e agentes patogênicos que acometem animais, podendo representar uma riquíssima fonte de informações, principalmente em locais nunca estudados, pois podem ser bioindicadores de qualidade do ambiente e saúde. Desta forma, no presente estudo foi realizada a pesquisa de Borrelia spp. em amostras de sangue de mamíferos silvestres e domésticos, assim como de carrapatos, do Parque Nacional do Catimbau, Pernambuco. Foi realizada a captura e colheita sanguínea de 77 animais pertencentes às famílias Didelphidae (45), Echimyidae (21), Cricetidae (7), Caviidae (2) e Dasypodidae (2). Adicionalmente, amostras de sangue foram obtidas de 120 ovinos, 119 caprinos, 70 cães e 47 bovinos. Foram coletados 286 carrapatos no ambiente, identificados como Ornithodoros cf. tabajara (188) e Ornithodoros rietcorreai (98); além de carrapatos coletados nos animais: Ornithodoros spp. em Thrichomys laurentius (4 espécimes) e Monodelphis domestica (1); O. rietcorreai em Didelphis albiventris (2) e T. laurentius (1); Amblyomma parvum em T. laurentius (4); Rhipicephalus sanguineus (sensu lato) em cães (121) e R. microplus em bovinos (47). Todas as amostras sanguíneas, 120 carrapatos de vida livre, sendo 46 O. rietcorreai e 74 O. cf. tabajara, e 12 R. sanguineus s. l. foram submetidos à extração de DNA para realização da reação em cadeia da polimerase (nested-PCR), visando amplificação de um fragmento do gene flaB presente em todas as espécies de Borrelia. Não houve amplificação nas amostras sanguíneas dos animais, no entanto em um espécime de O. cf. tabajara de vida livre (1,3%) foi detectado o DNA de Borrelia. A não detecção nos mamíferos pode estar relacionada com a ausência de infecção ou à baixa concentração do agente nos hospedeiros. A baixa prevalência encontrada nos carrapatos pode ser justificada devido à baixa infecção nos vetores. Novos estudos focando na determinação de anticorpos, assim como no sequenciamento e isolamento de Borrelia devem ser estimulados para uma melhor compreensão da epidemiologia do agente na região.

The aim of this study was to investigate impacts of Amazon Forest (AF) fire and conversion to pasture on carbon accumulation in particle size fractions and organic matter (OM) composition of an Acrisol. Soil samples were collected (0.00-2.00 m depth) in three sites: native AF (NAF); AF under natural regeneration for two years after burning (BAF); 23-years old Brachiaria pasture after AF burning (BRA). Assuming NAF area as reference, BAF and BRA areas showed negative carbon balance when carbon emitted to the atmosphere at AF burning is taken into account. Soil OM aromaticity and hydrophobicity, assessed via 13C nuclear magnetic resonance, in BRA and BAF were similar to that in NAF. Fire and post-fire land use altered the carbon distribution in sand, silt and clay along the soil profile and seem to have affected organo-mineral and OM self-assemblage interactions, since the relation between total soil carbon and carbon in clay was asymptotic in BAF and linear in NAF and BRA.

Abstract: The aims of this study were to investigate whether treatments with zinc edetate (Zn) and diphenyl diselenide ((PhSe)2) enhance performance, immune responses, protein metabolism, and oxidant/antioxidant status in calf serum and muscle. Animals were divided into four groups (n=6 each): control (without supplementation), and groups supplemented on days 50 and 70 of life with (PhSe)2, Zn, and a combination of (PhSe)2 and Zn. Animals treated with (PhSe)2 gained more weight by experimental day 220 than did the control group, but there was no difference by the end of the experiment (day 300). The absolute number of leukocytes and lymphocytes increased in groups Zn and (PhSe)2+Zn on day 20 of experiment, but decreased on day 40 in groups (PhSe)2, and (PhSe)2+Zn. The number of monocytes decreased in all groups compared with control. One of the principal findings was that (PhSe)2+Zn together had beneficial effects on protein metabolism, represented by increases total protein and globulin levels, compared with the control group. The combination of (PhSe)2 and Zn led to low levels of TBARS and ROS in serum and muscle, and stimulated antioxidant enzyme activities. Thus, supplementation with (PhSe)2+Zn may be a compelling approach to augmenting the calf antioxidant system during weaning.

Abstract Rheumatic heart disease (RHD) remains a major cause of preventable death and disability in children and young adults. Despite significant advances in medical technology and increased understanding of disease mechanisms, RHD continues to be a serious public health problem throughout the world, especially in low- and middle-income countries. Echocardiographic screening has played a key role in improving the accuracy of diagnosing RHD and has highlighted the disease burden. Most affected patients present with severe valve disease and limited access to life-saving cardiac surgery or percutaneous valve intervention, contributing to increased mortality and other complications. Although understanding of disease pathogenesis has advanced in recent years, key questions remain to be addressed. Preventing or providing early treatment for streptococcal infections is the most important step in reducing the burden of this disease.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

OBJETIVO: comparar a rigidez de três diferentes montagens de placa em ponte com a da haste intramedular bloqueada, em tíbias de cadáveres com fratura tipo C. MATERIAIS E MÉTODOS: vinte tíbias humanas captadas de cadáveres, submetidas à fratura do tipo C; quinze fixadas com placas em ponte, divididas em 03 grupos, de acordo com o tamanho das placas (10, 14 e 18 furos) e 05 fixadas com hastes intramedulares bloqueadas. Todas as tíbias foram expostas a cargas progressivas e semelhantes. Foram medidos os deslocamentos de ambos fragmentos (proximal e distal), nos planos sagital, coronal e axial do espaço, conforme incremento gradual de carga. RESULTADOS: tíbias fixadas com placas em ponte de 18 furos apresentam um comportamento biomecânico semelhante às fixadas com haste intramedular bloqueada. CONCLUSÕES: Em fraturas do tipo C em tíbias há maior mobilidade do segmento ósseo distal no plano coronal, quando a fratura é fixada com placas em ponte de 14 e 18 furos que quando fixada com haste intramedular bloqueada sem fresagem. Apesar dessa maior mobilidade, os movimentos relativos entre os fragmentos fraturários nos GHB e GP18 tendem a ser semelhantes entre si.
OBJECTIVE: To compare the stiffness of three different assemblies of bridging plates with intramedullary locking nails in cadaveric models of tibial fractures type C. MATERIALS AND METHODS: Twenty cadaveric tibias subjected to type C fractures; fifteen were fixed with the bridging plate technique and divided into three groups, according to the plate size (10, 14 and 18 holes), and five were fixed with intramedullary nail. All the tibias were exposed to similar and progressive loads. Dislocation of both fragments (proximal and distal) was measured on three planes (sagittal, coronal and axial), as the load was increased. RESULTS: tibias fixed with the 18 hole bridging plate have the same biomechanical behavior of tibias fixed with intramedullary locking nails. CONCLUSIONS: In type C tibial fractures, there is more mobility at the distal segment on the coronal plane when the fracture is fixed with 14 and 18 holes bridging plates as compared to fractures fixed with non-reamed intramedullary locking nails. Even with this mobility, the relative movements between the fragments at GHB and GP18 seem to be similar.

Gastric cancer is the forth most frequent malignancy and the second most common cause of cancer death worldwide. DNA methylation is the most studied epigenetic alteration, occurring through a methyl radical addition to the cytosine base adjacent to guanine. Many tumor genes are inactivated by DNA methylation in gastric cancer. We evaluated the DNA methylation status of ANAPC1, CDKN2A and TP53 by methylation-specific PCR in 20 diffuse- and 26 intestinal-type gastric cancer samples and 20 normal gastric mucosa in individuals from Northern Brazil. All gastric cancer samples were advanced stage adenocarcinomas. Gastric samples were surgically obtained at the João de Barros Barreto University Hospital, State of Pará, and were stored at -80°C before DNA extraction. Patients had never been submitted to chemotherapy or radiotherapy, nor did they have any other diagnosed cancer. None of the gastric cancer samples presented methylated DNA sequences for ANAPC1 and TP53. CDKN2A methylation was not detected in any normal gastric mucosa; however, the CDKN2A promoter was methylated in 30.4% of gastric cancer samples, with 35% methylation in diffuse-type and 26.9% in intestinal-type cancers. CDKN2A methylation was associated with the carcinogenesis process for ~30% diffuse-type and intestinal-type compared to non-neoplastic samples. Thus, ANAPC1 and TP53 methylation was probably not implicated in gastric carcinogenesis in our samples. CDKN2A can be implicated in the carcinogenesis process of only a subset of gastric neoplasias.

OBJETIVO: O transplante de pâncreas (TP) é atualmente o único tratamento disponível capaz de estabelecer estado euglicêmico permanente e de independência da insulina nos portadores de Diabetes Mellitus Tipo 1. Neste estudo são apresentados os resultados do TP realizados em um centro paranaense. MÉTODO: De janeiro de 2001 até abril de 2003, foram realizados 24 transplantes de pâncreas-rim simultâneos (TPRS), e um Transplante de Pâncreas Isolado (TPI) no Hospital e Maternidade Angelina Caron. RESULTADOS: No seguimento de 8,2 meses (1-27), a taxa de sucesso para pâncreas e rim foi de 74 %. A sobrevida dos pacientes foi de 76 %. A principal causa de insucesso foi trombose pancreática em três casos (12%) e renal em dois (8%). Não ocorreu nenhum episódio de rejeição. Todos os doentes com enxertos funcionantes apresentam-se normoglicêmicos sem necessidade de insulina. CONCLUSÕES: O transplante simultâneo de rim e pâncreas éterapêutica com alto índice de sucesso para pacientes diabéticos com insuficiência renal terminal.
BACKGROUND: Pancreatic transplantation (PT) is the only treatment available to keep long term normoglicemic state and insulin independence in type 1 diabetic patients. We present the results of PT performed in a center at the State of Paraná. METHODS: From January 2001 until april 2003, 24 patients received simultaneous pancreas-kidney transplant (SPK) and one isolated pancreatic transplant at Hospital Angelina Caron. RESULTS: With a mean follow-up of 8,2 months (range 1-27), the pancreas and kidney success rate was 74%. Patient survival was 76%. Leading cause of pancreas and kidney losses was thrombosis in three pancreas (12%) and two kidneys (8%). No episode of rejection occurred in any of the transplanted patients. All patients with successful grafts are insulin-free since transplantation. CONCLUSIONS : This series confirms that SPK is a highly successful procedure for diabetic patients with renal failure.

OBJETIVO: Demonstrar a possibilidade de aplicar alternativa técnica e tática operatória na lesão do tronco da artéria coronária esquerda com avaliação dos resultados tardios, aos cinco anos de observação. MÉTODO: Entre julho de 1998 a março de 2002, 210 pacientes foram submetidos a revascularização miocárdica sem circulação extracorpórea (CEC), sendo 119 (56,7%) do gênero masculino. As idades variaram entre 32 e 81 anos, com média de 59,7 anos e desvio padrão de 10,5 anos. A lesão no tronco da artéria coronária esquerda esteve presente em 48 (22,8%) pacientes, levando em conta graus de lesões acima de 70% e a lesão em artéria coronária direita esteve associada em 10 (20,8%) casos. Todos pacientes apresentavam fração de ejeção acima de 40% no ecocardiograma pré-operatório. RESULTADOS: A estratificação do número de enxertos por paciente foi: 2 enxertos (52,1%); 3 enxertos (43,5%) e 4 enxertos (4,4%). Não ocorreu infarto agudo do miocárdo peri-operatório, fibrilação ventricular ou óbito no transoperatório. O seguimento foi de 100% até 60 meses com avaliação clínica cardiológica, ocorreu apenas 1 óbito de causa cardíaca após 1 mês de seguimento. A taxa sobrevivência aos 5 anos de seguimento foi de 96,0±3,9%. CONCLUSÕES: Nas lesões do tronco da artéria coronária esquerda, temidas pelos cirurgiões cardíacos, acreditamos que a técnica e a tática sem uso da CEC podem ser úteis, uma vez que os pacientes estejam com estabilidade hemodinâmica.
OBJECTIVE: Our aim is to demonstrate the possibility of applying the alternative tactic of off-pump coronary artery bypass surgery to left main coronary artery disease and to assess the long-term results over a 5-year follow-up period. METHODS: Among 210 subjects submitted to off-pump coronary artery bypass, 119 (56.7%) were male. Their ages ranged from 32 to 81 years with a mean age of 59.7. A left coronary artery branch lesion of at least 70% was demonstrated in 48 (22.8%) of the individuals and a right coronary lesion was associated in 10 (20.8%) of the cases. RESULTS: A breakdown of the number of grafts per patient was: 2 for 52.1%, 3 for 43.5% and 4 for 4.4% of the surgeries. All of the patients were extubated in the intensive care unit after a 6-hour stay. No left ventricular dysfunction was evidenced after a seven-day total hospitalization period. One death, due to cardiac problems, occurred after one month. Follow-up with clinical cardiac evaluation was total until 60 months with a survival rate at 5 years of 96.0% ± 3.9%. CONCLUSION: In left main coronary artery disease, which is feared by cardiac surgeons, we believe that the technique of off-pump coronary artery bypass could be very useful. If the patients are hemodynamically stable and undergo an elective operation, the operative period can be passed in a safe and tranquil manner.
