Natural infestation by ticks as cause of death in beef cattle

ABSTRACT: Although infestation by Rhipicephalus microplus is mentioned as the cause of death among cattle, there are only studies with artificial infestations in which intense parasitism led to the death of cattle due to severe anemia. This study seems to be the first to describe the epidemiology, clinical-pathological condition, and diagnostic conduct of infestation by R. microplus as the cause of death of European breed calves introduced in shady, enclosed pastures in regions with rainfall prior to the onset of the outbreak. The affected calves presented apathy, anorexia, and anemia and were severely infested by R. microplus. The lethality rate of the outbreak was 36.5%. After the rainy period, the dry season facilitated a decrease in the nutritional status of the affected cattle. The delay in the definitive diagnosis contributed to the economic loss of the farm due to the cost of erroneous treatments and the number of dead cattle. The main factors that contributed to the deaths were climatic, environmental, and individual. This study demonstrates that severe infestations by R. microplus in low-quality pasture-raised calves are fatal and emphasizes the importance of diagnostic tools to reduce the economic loss caused by death due to infestation by ticks.
RESUMO: Embora a infestação por Rhipicephalus microplus seja mencionada como causa de morte em bovinos, existem apenas estudos com infestações artificiais em que o intenso parasitismo levou à morte de bovinos por anemia severa. O presente trabalho parece ser o primeiro a descrever a epidemiologia, o quadro clínico e patológico e a conduta diagnóstica da infestação por R. microplus como causa de mortes em bezerros de sangue europeu introduzidos em pastagens sombreadas e vedadas oriundas de regiões em que houve chuvas 30 a 60 dias antes do início dos casos clínicos. Os bezerros afetados apresentaram apatia, palidez das mucosas oral e oculares, anorexia, anemia e estavam severamente infestados por exemplares de R. microplus. A taxa de letalidade do surto foi de 36,5%. Macroscopicamente, carcaça pálida e baixo escore corporal foram as alterações predominantes no bovino necropsiado. Após os períodos chuvosos, a temporada de seca facilitou a queda no estado nutricional dos bovinos afetados. A demora na realização do diagnóstico definitivo contribuiu para o prejuízo econômico da propriedade decorrente de custos com tratamentos equivocados e pelo aumento no número de bovinos mortos. Os principais fatores que contribuíram para as mortes foram climáticos, ambientais (pastagens sombreadas, vedadas e em estágio de seca) e individuais (bezerros de raças europeias foram a categoria afetada). Este trabalho demonstra que infestações severas por R. microplus em bezerros criados em pastagens que possuem baixa qualidade são fatais devido a quadros graves de anemia, e enfatiza a importância das ferramentas diagnósticas na redução dos prejuízos econômicos causados pelas perdas decorrentes da infestação por carrapatos.

RESUMO Mesoteliomas malignos correspondem a um tipo raro de câncer que afeta as superfícies mesoteliais, normalmente a pleura e o peritônio. São associados à exposição ao amianto, porém, devido ao período de latência de mais de 30 anos e seu difícil diagnóstico, a maioria dos casos não é detectada até que estágios avançados sejam alcançados. As opções de tratamento para esse tipo de tumor são bastante limitadas e a taxa de sobrevida varia de 12 a 36 meses. Esta revisão discute a fisiopatologia molecular, o diagnóstico atual e as opções terapêuticas mais recentes para a doença.
ABSTRACT Malignant mesotheliomas are rare types of cancers that affect the mesothelial surfaces, usually the pleura and peritoneum. They are associated with asbestos exposure, but due to a latency period of more than 30 years and difficult diagnosis, most cases are not detected until they reach advanced stages. Treatment options for this tumor type are very limited and survival ranges from 12 to 36 months. This review discusses the molecular physiopathology, current diagnosis, and latest therapeutic options for this disease.

RESUMO Os fungos micorrízicos arbusculares (FMA) e os endofíticos “dark septate” (DSE) podem promover o incremento da biomassa vegetal, a depender das condições edafoclimáticas e da interação com a planta hospedeira. O objetivo deste trabalho foi avaliar a interação de fungos micorrízicos arbusculares exóticos e fungos endofíticos DSE nativos no crescimento inicial de Paspalum millegrana Schrad. ex Schult. O delineamento experimental utilizado foi inteiramente casualizado composto pelo cultivo de Paspallum millegrana em quatro tratamentos (controle - sem FMA, e três isolados de FMA exóticos: UFLA351 - Rhizoglomus clarum; UFLA372 - Claroideoglomus etunicatum e UFLA401 - Acaulospora morrowiae), com quatro repetições. O capim P. millegrana é colonizado por FMA exóticos das espécies R. clarum (UFLA351; 11,9%), C. etunicatum (UFLA372; 39,6%) e A. morrowiae (UFLA401; 51,2%). O P. millegrana foi colonizado por fungos endofíticos DSE nativos, mas estes não interferem na colonização por FMA exóticos e no desenvolvimento das plantas. O P. millegrana foi responsivo à inoculação dos isolados UFLAs de FMA exóticos, o que pode contribuir para o crescimento e sobrevivência do capim em condições de campo. E o processo de desinfestação superficial das sementes não elimina micro-organismos endofíticos, cuja presença pode influenciar na colonização das plantas por FMA, bem como no desenvolvimento da planta hospedeira.
ABSTRACT Arbuscular mycorrhizal fungi (AMF) and dark septate endophytic fungi (DSE) promote increase in plant biomass, depending on the soil and climate conditions and the interactions with the host plant. The objective of this study was to evaluate the interaction of exotic arbuscular mycorrhizal fungi and native DSE fungi on the initial growth of P. millegrana. A completely randomized experimental design comprising the Paspallum millegrana cutilvar with the following treatments: control - without AMF, and three exotic AMF isolates (UFLA351 - Rhizoglomus clarum, UFLA372 - Claroideoglomus etunicatum and UFLA401 - Acaulospora morrowiae), with four replications each. P. millegrana grass was colonized by exotic AMF by R. clarum (UFLA351, 11.9%), C. etunicatum (UFLA372, 39.6%), and A. morrowiae (UFLA401, 51.2%). P. millegrana was also colonized by native DSE fungi, but these did not interfere with the colonization by exotic AMF and plant development. P. millegrana is responsive to the inoculation of UFLAs isolates of exotic AMF, which may contribute to the grass growth and survival under field conditions. The process of surface disinfestation of seeds does not eliminate endophytic microorganisms, whose presence may influence plant colonization by AMF, as well as development of the host plant.

Abstract Ralstonia mannitolilytica, a Gram-negative bacterium, is rarely isolated in clinical laboratories. It has been associated with outbreaks due to its ability to survive in liquid media and hospital devices. We describe three cases of bacteremia caused by R. mannitolilytica in a neonatal intensive care unit in Curitiba, Southern Brazil. All isolates presented the same PFGE profile. The common source of infection was undetected in surveillance cultures for the outbreak survey. All patients received antimicrobial treatment and were discharged from the maternity. Due to the characteristics of the microorganism, clinicians and microbiologists should pay attention to the emergence of Ralstonia spp. infections.

ABSTRACT The tick Rhipicephalus (Boophilus) microplus (Canestrini, 1887) (Acari: Ixodidae) causes large economic losses to cattle breeders and its control is hampered by problems of resistance to the main commercial synthetic acaricides and the risk of residues in animal products. Alternative controls are important for the sustainability of cattle breeding in tropical regions, principally for organic milk production. This study evaluated the efficacy of a homeopathic preparation to control natural tick infestations. The evaluations were conducted with 24 taurine dairy cows during 12 months. The homeopathic medicine (30 CH + Sulfur 30 CH) was administered in a proportion of 1:100 (v/w) in a formulation with sugar, added to 30 kg of mineral salt. This preparation was supplied along with concentrated feed (2 kg/animal/day). The control group received the same feed mixture without the medicine. The groups were kept in separate paddocks. Data were collected on body weight, packed cell volume and number of engorged female ticks on each animal. Temperature, relative humidity and rainfall were recorded. Engorged females were collected from each group for in vitro comparison of reproductive parameters, and semi-engorged ticks for morpho-histological analysis. The mean results of body weight, packed cell volume and tick number were not statistically different between the control and treated groups: 616.5 kg and 618.6 kg; 27.9% and 27.3%; and 12.19 and 13.58 ticks, respectively. In the in vitro analyses, the average weights of the ticks (0.18 and 0.17 g) and eggs (0.07 and 0.06 g), larval hatching rate (70.1 and 76.6%) and reproductive efficiency index (54.3 and 60.6%) of the control and treated groups, respectively, also were not statistically different. Furthermore, no alteration in the ovaries was detected by microscopic analysis. Therefore, the homeopathic medicine was ineffective to control R. (B.) microplus, although the inclusion of agro-ecological practices might enhance its effect of homeopathy in controlling tick infestations.

RESUMO Síndrome pós-polio (SPP) é uma condição que afeta sobreviventes da poliomielite aguda (PAA), anos após a recuperação de um ataque agudo inicial do vírus. Na maioria das vezes, os sobreviventes da polio começam a apresentar nova paresia gradual nos músculos que foram previamente afetados pela infecção. A incidência real de doenças cardiovasculares (DCV) em indivíduos que sofrem de SPP não é conhecida. Entretanto, há indícios para suspeitar de que sujeitos com SPP podem estar em maior risco. Método Realizou-se uma busca de artigos nas bases de dados: Bireme, Scielo e Pubmed, utilizando as seguintes palavras-chave: síndrome pós-poliomielite, função cardiorrespiratória e reabilitação, nos idiomas Inglês, Francês e Espanhol. Embora tenhamos selecionado um número expressivo de artigos, somente foram considerados os duplo-cegos, randomizados-controlados, além de consensos. Resultados e Discussão Certas características da SPP, tais como fadiga muscular, paresia, dor muscular e/ou articulares podem resultar em descondicionamento por inatividade física, além de obesidade e dislipidemia. Dificuldades respiratórias são comuns e podem resultar em hipoxemia. Conclusão Somente quando avaliados e tratados em tempo hábil, alguns pacientes são capazes de obter os benefícios do uso dos músculos respiratórios auxiliares em termos de qualidade de vida.
ABSTRACT Post-polio syndrome (PPS) is a condition that affects polio survivors years after recovery from an initial acute attack of the poliomyelitis virus. Most often, polio survivors experience a gradual new weakening in muscles that were previously affected by the polio infection. The actual incidence of cardiovascular diseases (CVDs) in individuals suffering from PPS is not known. However, there is a reason to suspect that individuals with PPS might be at increased risk. Method A search for papers was made in the databases Bireme, Scielo and Pubmed with the following keywords: post polio syndrome, cardiorespiratory and rehabilitation in English, French and Spanish languages. Although we targeted only seek current studies on the topic in question, only the relevant (double-blind, randomized-controlled and consensus articles) were considered. Results and Discussion Certain features of PPS such as generalized fatigue, generalized and specific muscle weakness, joint and/or muscle pain may result in physical inactivity deconditioning obesity and dyslipidemia. Respiratory difficulties are common and may result in hypoxemia. Conclusion Only when evaluated and treated promptly, somE patients can obtain the full benefits of the use of respiratory muscles aids as far as quality of life is concerned.

Resumo A flexibilidade alimentar e onivoria são características das espécies de peixes de água doce neotropicais, especialmente em espécies de Astyanax. Tradicionalmente a maioria dos recursos na dieta de peixes é reconhecidamente originária do ambiente aquático, porém estudos recentes têm apontado para a importância das fontes alóctones. Além disso, a colonização de macrófi tas, comuns em vários reservatórios tropicais, pode melhorar os aspectos de insetivoria na dieta dos peixes expandindo ou concentrando a área disponível para a alimentação. Aqui analisamos o conteúdo estomacal de duas espécies de Astyanax simpátricas para avaliar a importância das diferenças de habitats, como a complexidade espacial, em dois reservatórios tropicais com diferentes características ambientais. A análise NMDS indicou separação na dieta destas espécies entre reservatórios (stress = 8.28%). Além disso, a análise de variância revelou uma diferença signifi cativa no volume de itens alimentares consumidos entre os reservatórios (Anova one-way F (1. 132) = 4,4446; p = 0.037), salientando a importância das condições ambientais na composição da dieta de peixes. O estudo destacou o hábito alimentar insetívoro das espécies de Astyanax e revelou diferentes estratégias de alimentação entre esses peixes simpátricos apesar da alta sobreposição de nicho nos dois ambientes. A heterogeneidade de hábitat aumentando a disponibilidade de recursos alimentares teve importância sobre a estratégia alimentar das espécies de Astyanax e consequentemente na manutenção constante de ambas nos dois diferentes reservatórios.
Abstract Food flexibility and omnivory are important features pronounced in Neotropical freshwater fish species, particularly for Astyanax species. Traditionally most fish diet resources are known to be originated in the aquatic environment, however recent studies have pointed to the importance of allochthonous sources. Besides, the colonization of macrophytes, common at several tropical reservoirs, may enhance insectivory in fish diet expanding or concentrating the area of available resources for feeding. Here we employed stomach contents analysis of two sympatric Astyanax species to access the importance of habitat differentiations as spatial complexity in two tropical reservoirs with different environmental features. The NMDS analysis indicated separation in the diet of these species between reservoirs (Stress= 8.28%). Additionally, analysis of variance revealed a significative difference in the volume of food itens ingested between the reservoirs (Anova one-way F(1, 132)=4.4446; p= 0.037). This points out the importance of environmental conditions on the composition of the diet of fishes. This study highlighted the insectivorous feeding habit of Astyanax species and revealed different feeding strategies between sympatric fishes despite high niche overlap in both environments. Habitat heterogeneity increasing food resources availability plays an important role in the diet strategy of these Astyanax species and on their constant maintenance in the two different reservoirs.

Resumo A produção in vitro (PIV) de embriões de bovinos não é apenas de grande importância econômica para a pecuária, mas é também um importante modelo para estudar o desenvolvimento embrionário. O objetivo deste estudo foi avaliar a modificação de histona, H3R26me2 durante o desenvolvimento pré-implantacional em embriões bovinos produzidos in vitro, cultivados com ou sem suplementação de soro fetal bovino (SFB), bem como comparar essa modificação específica entre mórulas produzidas in vitro e in vivo. Após a maturação in vitro e fertilização, embriões foram cultivados com suplementação de 0 ou 2,5% SFB. O desenvolvimento embrionário foi avaliado e embriões foram coletados e fixados em diferentes fases durante o desenvolvimento (2, 4, 8 e 16 células, mórula e blastocisto). Os embriões fixados foram avaliados por imunofluorescência utilizando um anticorpo para H3R26me2. Imagens de embriões corados foram analisadas baseadas na porcentagem do DNA total. Embriões cultivados com 2,5% SFB tiveram uma taxa de desenvolvimento ao estágio de blastocisto maior que o grupo que não recebeu suplementação com SFB (34.85±5,43% vs 23.38±,93%; P<0,05). Níveis de H3R26me2 variaram para ambos os grupos ao longo do desenvolvimento. No grupo 0% SFB, a marcação para H3R26me2 foi mais intensa nos estágios de 4 células (P<0,05), 16 células (P<0,05) e mórula (P<0.05). No grupo 2.5% SFB, apenas os embriões de 4 células tiveram marcação significativamente maior que todas as outras fases (P<0,01). Mórulas produzidas in vivo apresentaram níveis de H3R26me2 semelhantes ao grupo 0% SFB, e ambos foram significativamente maiores que o grupo 2.5% SFB. Estes resultados sugerem que a modificação de histona H3R26me2 é regulada durante o desenvolvimento pré-implantacional de embriões bovinos, e que as condições de cultura alteram de maneira importante esta regulação.
Abstract In vitro production (IVP) of bovine embryos is not only of great economic importance to the cattle industry, but is also an important model for studying embryo development. The aim of this study was to evaluate the histone modification, H3R26me2 during pre-implantation development of IVP bovine embryos cultured with or without serum supplementation and how these in vitro treatments compared to in vivo embryos at the morula stage. After in vitro maturation and fertilization, bovine embryos were cultured with either 0 or 2.5% fetal bovine serum (FBS). Development was evaluated and embryos were collected and fixed at different stages during development (2-, 4-, 8-, 16-cell, morula and blastocyst). Fixed embryos were then used for immunofluorescence utilizing an antibody for H3R26me2. Images of stained embryos were analyzed as a percentage of total DNA. Embryos cultured with 2.5% FBS developed to blastocysts at a greater rate than 0%FBS groups (34.85±5.43% vs. 23.38±2.93%; P<0.05). Levels of H3R26me2 changed for both groups over development. In the 0%FBS group, the greatest amount of H3R26me2 staining was at the 4-cell (P<0.05), 16-cell (P<0.05) and morula (P<0.05) stages. In the 2.5%FBS group, only 4-cell stage embryos were significantly higher than all other stages (P<0.01). Morula stage in vivo embryos had similar levels as the 0%FBS group, and both were significantly higher than the 2.5%FBS group. These results suggest that the histone modification H3R26me2 is regulated during development of pre-implantation bovine embryos, and that culture conditions greatly alter this regulation.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

É relativamente pequeno o número de estudos que enfatiza os danos que uma formulação acaricida spray pode desencadear sobre os parâmetros reprodutivos das teleóginas. O presente estudo teve como objetivo avaliar os efeitos deletérios de uma formulação spray comercial (dichlorvos 60% + Clorpirifós 20%), sobre os parâmetros reprodutivos de uma população susceptível de R. (B.) microplus, desprendidas de bovinos experimentalmente infestados, utilizandose o teste de estábulo. Os animais foram alocados aos grupos de tratamentos de acordo com a contagem média de fêmeas desprendidas dos bovinos nos dias -3, -2 e -1. O número de teleóginas desprendidas foi quantificado do dia 1 ao 30. Para cada grupo, diariamente 20 fêmeas, ou a quantidade disponível, foram selecionadas e submetidas à avaliação dos parâmetros reprodutivos. A associação de organofosforados demonstrou elevada eficácia acaricida e também apresentou efeitos deletérios sob os parâmetros reprodutivos de Rhipicephalus (B.) microplus, diminuindo (P≤0,05) o peso das teleóginas (dos dias 1 ao 7), o peso da massa de ovos (dos dias 5 ao 10) e a eclodibilidade das larvas (dos dias 5 ao 19). Talvez uma formulação pode apresentar efeitos deletérios sobre os parâmetros reprodutivos de fêmeas de R. (B.) microplus, quando existe um elevado grau de sensibilidade dessa cepa de carrapato a um determinado composto. De qualquer maneira, futuros estudos devem ser realizados.
The number of studies emphasizing the possible damage that acaricidal spray formulations can cause on engorged female ticks' reproductive parameters is small. The present study evaluated the deleterious effects of a spray formulation (dichlorvos 60% + chlorpyrifos 20%) on the reproductive parameters of a susceptible population of Rhipicephalus (B.) microplus females, using the Stall Test. The ticks were allocated randomly to treatments according to the mean numbers of females detached from each cow on days -3, -2 and -1 and the cattle pen location. The numbers of engorged female ticks that naturally detached from the cattle were counted daily from day 1 to day 30. For each group, 20 detached engorged female ticks or the available number collected daily were evaluated regarding reproductive parameters. Associations of organophosphates demonstrated elevated acaricidal efficacy, as well as deleterious effects on the reproductive parameters of R. (B.) microplus females. The engorged female weight (days 1 to 7), weight of egg masses (days 5 to 10) and larval hatching percentage (days 5 to 19) were decreased (P ≤ 0.05). It is possible that a formulation can lead to deleterious effects on R. (B.) microplus females when the tick population analyzed shows elevated sensitivity towards a particular formulation. However, further studies need to be conducted.

Objective Histological details of positive surgical margins in radical prostatectomy specimens have been related to outcome after surgery in rare studies recently published. Our objective is to assess whether the status of surgical margins, the extent and the Gleason score of positive margins, and the extent of the extraprostatic extension are predictive of biochemical recurrence post-radical prostatectomy.Materials and Methods Three hundred sixty-five radical prostatectomy specimens were analyzed. The length of the positive surgical margin and extraprostatic extension and the Gleason score of the margin were recorded. Statistical analyses examined the predictive value of these variables for biochemical recurrence.Results 236 patients were stage pT2R0, 58 pT2R1, 25 pT3R0 and 46 pT3R1. Biochemical recurrence occurred in 11%, 31%, 20% and 45.7% of pT2R0, pT2R1, pT3R0 and pT3R1, respectively. The extent of the positive surgical margins and the Gleason score of the positive surgical margins were not associated with biochemical recurrence in univariate analysis in a mean follow up period of 35.9 months. In multivariate analyses, only the status of the surgical margins and the global Gleason score were associated with biochemical recurrence, with a risk of recurrence of 3.1 for positive surgical margins and of 3.8 for a Gleason score > 7.Conclusion Positive surgical margin and the global Gleason score are significant risk factors for biochemical recurrence post-radical prostatectomy, regardless of the extent of the surgical margin, the extent of the extraprostatic extension, or the local Gleason score of the positive surgical margin or extraprostatic tissue. pT2R1 disease behaves as pT3R0 and should be treated similarly.

OBJETIVOS: Determinar a frequência da restrição de crescimento extrauterino em recém-nascidos prétermos de muito baixo peso e avaliar o impacto de variáveis perinatais, práticas clínicas e morbidades neonatais nesta morbidade. MATERIAIS E MÉTODOS: Foi realizado um estudo longitudinal em 4 unidades neonatais do Rio de Janeiro. Foram analisados 570 recém-nascidos pré-termos de muito baixo peso. Foram incluídas no estudo variáveis perinatais, variáveis relacionadas às práticas clínicas e morbidades incidentes nestes recém-nascidos. A restrição de crescimento extrauterino foi definida pelos escores z de peso ou perímetro cefálico < -2 para idade corrigida. Na análise estatística foram utilizados o software SPSS e o software R. RESULTADOS: Foram analisados 570 recém-nascidos dos quais 49% eram do sexo masculino e 33% nasceram pequenos para idade gestacional. A média do peso e perímetro cefálico ao nascimento foi respectivamente 1113 ± 267 g e 27 ± 2 cm. As médias de escore z do peso ao nascimento e na alta foram respectivamente, -0,96 ± 0,78 e -1,54 ± 0,75 e as do perímetro cefálico foram -0,63± 1,18 e -0,45 ± 0,94. A frequência de restrição de crescimento extrauterino considerando o peso foi 26% do perímetro cefálico foi de 5%. Nascer pequeno para idade gestacional foi a variável de maior impacto na restrição de crescimento tanto para o peso (RP 4,33) quanto parao perímetro cefálico (RP 2,11) nas analises ajustadas. CONCLUSÃO: A restrição de crescimento extrauterino foi alta na população, especialmente para os recém-nascidos PIG e com morbidades neonatais.
OBJECTIVES: To determine the rate of extrauterine growth restriction in very low birth weight infants and to evaluate the influence of perinatal variables, clinical practices, and neonatal morbidities on this outcome. METHODS: A longitudinal study was performed in four neonatal units in the city of Rio de Janeiro. 570 very low birth weight infants were analyzed. The study included perinatal variables, variables related to clinical practices, and incident morbidities in these preterm infants. Extrauterine growth restriction was defined using z-scores for weight or head circumference < -2 for cor-rected age. Statistical analysis was performed using the Statistical Package for Social Sciences (SPSS) and R software. RESULTS: This study comprised 570 infants, of which 49% were males, and 33% were small for gestational age (SGA). The mean weight and head circumference at birth were 1,113 ± 267 g and 27 ± 2 cm, respectively. The mean z-scores of birth weight and weight at discharge were -0.96± 0.78 and -1.54 ± 0.75, respectively; for head circumference, the mean z-scores at birth and at discharge were -0.63 ± 1.18 and -0.45 ± 0.94, respectively. The rate of extrauterine growth restriction considering the weight was 26% (149/570) and considering the head circumference, 5% (29/570). SGA was the variable with the greatest impact on both growth restriction for weight (PR = 4.33) and for head circumference (PR = 2.11) in adjusted analyses. CONCLUSION: Extrauterine growth restriction was high in the population, especially for SGA newborns and those with neonatal morbidities

OBJETIVO: Determinar a época do diagnóstico de fendas orofaciais típicas em diferentes regiões brasileiras e sua influência na idade da correção cirúrgica. MÉTODO: Estudo prospectivo, descritivo e transversal realizado em centros médicos do Sudeste, Sul e Nordeste do Brasil. Fonoaudiólogos e geneticistas treinados realizaram entrevista, previamente validada, com pais de crianças afetadas. Utilizaram-se os programas Epi-Info e SPSS. Adotou-se nível de significância de 5% (p < 0,05). RESULTADOS: A amostra contou com 215 entrevistas para análise: 21,9% (47) aplicadas no Sudeste, 51,1% (110) no Sul e 27% (58) no Nordeste. A renda mensal no Sudeste foi maior (p < 0,05). A fenda labiopalatal foi encontrada em 61,4% (132) dos casos, a palatal, em 20,9% (45), e a labial, em 17,7% (38). Em 75,3% (162) dos casos, o diagnóstico ocorreu na maternidade, em 14% (30), no pré-natal e, em 10,2% (22), após a alta da maternidade. O Sudeste apresentou maior frequência de diagnóstico pré-natal (27,7%), possivelmente relacionada ao maior poder aquisitivo e a oportunidades de investigação. Dos diagnósticos em maternidades, 74,4% ocorreram no Nordeste. Entretanto, não houve diferença na comparação entre época de diagnóstico, região e idade da primeira cirurgia. CONCLUSÃO: Considerando que o diagnóstico é mais frequente em maternidades, sugere-se o treinamento das equipes de saúde desses locais, visando efetiva coordenação do atendimento inicial. Apesar da época do diagnóstico não influenciar a idade das cirurgias, ela favorece o planejamento dos cuidados neonatais e terapêuticos dos afetados.
OBJECTIVE: To determine the time of diagnosis of typical orofacial clefts in different Brazilian regions and its influence on age at surgical correction. METHOD: This was a prospective, descriptive, cross-sectional study conducted in medical centers in the Southeast, South, and Northeast of Brazil. Trained speech therapists and geneticists interviewed the parents of affected children using a previously validated questionnaire. Epi-Info and SPSS were used for data analysis. Significance level was set at 5% (p < 0.05). RESULTS: The sample consisted of 215 interviews conducted in the following regions: 21.9% (47) in the Southeast, 51.1% (110) in the South, and 27% (58) in the Northeast. Monthly family income was higher in the Southeast (p < 0.05). Cleft lip and palate were found in 61.4% (132) of cases, cleft palate in 20.9% (45), and cleft lip in 17.7% (38). Diagnosis occurred in the maternity ward in 75.3% (162) of cases, during the prenatal period in 14% (30), and after hospital discharge in 10.2% (22). The Southeast had a higher frequency of prenatal diagnosis (27.7%), possibly related to greater purchasing power in this region and greater availability of prenatal investigation. Of all cases diagnosed in the maternity ward, 74.4% occurred in the Northeast. However, no significant difference was found when comparing time of diagnosis, region, and age at first surgery. CONCLUSION: Considering that diagnosis is more common in the maternity ward, local health care teams should be trained in order to effectively improve the initial care of these patients. Although time of diagnosis did not affect age at surgery, it favors the planning of neonatal care and treatment of affected infants.

Organização e narração de histórias por escolares em desenvolvimento típico de linguagem

BACKGROUND: the narrative abilities provide valuable information about the linguistic, cognitive and social development of school-age children with typical language development (TLD). AIM: to examine the temporal ordering of figures and the narrative abilities of school-age children with TLD in relation to the type of discourse presented. METHOD: participants of this study included 60 children with TLD aged between seven and ten years of age. Fifteen stories were used in the study. Each story was illustrated by four figures. The sequences of figures were created and classified as mechanical, behavioral and intentional according to the relationships presented among the characters. Data were transcribed and analyzed regarding the type of discourse (descriptive, causal and intentional) and the type of organization of the figures. RESULTS: no differences between age groups were observed for temporal ordering. For all age groups, the most frequently presented discourse type was the causal one. Statistically significant differences were observed among age groups for the causal and intentional discourse type. Inasmuch as the age increased, school-age children with TLD reduced the use of the descriptive discourse and increased the use of the intentional one. CONCLUSION: the ability of temporal ordering is already developed in children with TLD at seven-years of age. The type of discourse was influenced by age and by the type of story presented.
TEMA: habilidades narrativas em escolares com desenvolvimento típico de linguagem. OBJETIVO: analisar tanto a ordenação de figuras que compõem histórias, quanto a classificação do tipo de discurso empregado na narração dessas histórias por escolares em desenvolvimento típico de linguagem. MÉTODO: participaram deste estudo 60 escolares na faixa etária entre sete e dez anos de idade com desenvolvimento típico de linguagem. Foi utilizada uma série de 15 histórias, representadas por figuras, compostas por quatro cenas cada. Essas sequências foram criadas e classificadas em mecânicas, comportamentais e intencionais, segundo as relações envolvidas entre as personagens. Os dados foram transcritos e analisados conforme o tipo de discurso (descritivo, causal e intencional) e, além disso, foi pontuado o tipo de organização das figuras realizadas pelas crianças. RESULTADOS: não foram observadas diferenças entre as faixas etárias em relação à ordenação temporal. Para todas as faixas etárias o discurso predominante foi o do tipo causal e houve diferenças estatisticamente significantes entre as faixas etárias para os tipos de discurso causal e intencional. Também se verificou que com o aumento da complexidade das histórias fornecidas e com o aumento da idade houve aumento do tipo de discurso intencional e diminuição do tipo de discurso descritivo. CONCLUSÕES: a capacidade de ordenação temporal já está desenvolvida aos sete anos e os tipos de discurso realizados sofrem influência da idade e do tipo de história fornecida.
