ABSTRACT OBJETIVE To investigate the relationship between the masticatory function and the frailty of older people. METHODS Exploratory cross-sectional study using secondary data from the FIBRA Project on the frailty conditions of older people living in urban areas of six Brazilian municipalities, from 2008 to 2009. The population consisted of older adults aged 65 and over with no cognitive impairment. A single session identification questionnaire and the Mini-Mental State Examination (MMSE) were applied, followed by collecting sociodemographic data and data on the participants’ self-assessment of masticatory function, general health, and oral health. RESULTS 2,341 older people (164 frail older adults), of whom 63.2% reported not having seen a dentist in the last year, and approximately 26% of them had three or more functional feeding problems. Older participants (OR = 2.88; 95%CI: 2.01–4.13), who are retired (OR = 2.31; 95%CI: 1.18–5.53), those who were depressed (OR = 2.31; 95%CI: 1.58–3.38), and those who self-assessed their general health as worse compared to others of the same age (OR = 3.91; 95%CI: 2.39–6.40) were at higher risk of frailty. Three or more problems related to the functional aspects of mastication were associated with a greater chance of frailty (OR = 2.06; 95%CI: 1.25–3.41). CONCLUSION This study found an association between masticatory function and a greater chance of frailty among the studied population.

Resumo O objetivo deste artigo é verificar a condição bucal e a capacidade mastigatória autopercebidas em idosos longevos (≥ 80 anos). Foram utilizados dados do estudo multicêntrico FIBRA. A amostra foi composta por 2.341 indivíduos com ausência de déficit cognitivo, com idade ≥ 65 anos, no período entre 2008 e 2009, quando os idosos foram submetidos à coleta de dados. Quanto ao desfecho, a idade foi dicotomizada em 65-79 e 80 anos ou mais. A associação entre longevidade e as variáveis sociodemográficas, comportamentais, de saúde geral e bucal e a capacidade mastigatória autopercebida foram avaliadas pela regressão de Poisson. Dados completos disponíveis para 2.126 idosos. A idade média foi 70,91 (DP = ±4,11) anos entre os idosos jovens e 82,70 (DP = ±2,61) entre os longevos. A amostra de idosos com idade ≥ 80 anos foi de 11,7%. Baixa escolaridade, estado civil solteiro e ausência de trabalho estiveram associados à longevidade, assim como autorrelato de não possuir dentes naturais e dificuldade ou dor ao mastigar alimentos duros. Os resultados sugerem a associação entre longevidade com características sociodemográficas e de saúde bucal.

Abstract The scope of this article is to assess the oral health conditions and self-perceived chewing ability among the elderly seniors (≥ 80 years). Data from the FIBRA multicentric study were used. The sample consisted of 2,341 individuals with no cognitive impairment, aged ≥ 65 years, between 2008 and 2009, where the elderly were subjected to data collection. Regarding the outcome, age was dichotomized into 65-79 and 80 and over. The association between longevity and the socio-demographic and behavioral variables, general and oral health and self-perceived chewing ability was assessed by Poisson regression. Complete data are now available for 2,126 seniors. The mean age was 70.91 (SD =± 4.11) years among the young elderly and 82.70 (SD =± 2.61) among the elderly seniors. The sample of individuals aged ≥ 80 years was 11.7%. Low education, single marital status and lack of work were associated with longevity, as well as self-reporting of having no natural teeth and difficulty or pain when chewing hard foods. The results suggest an association between longevity with sociodemographic characteristics and oral health.

Resumo: O aumento da população idosa leva a uma crescente demanda por serviços de saúde e expansão dos gastos, com consequências relevantes sobre a estabilidade econômica. Nesse contexto, o objetivo do estudo foi analisar o perfil das morbidades e gastos hospitalares com idosos em relação às condições socioeconômicas e demográficas do Paraná, Brasil, no período de 2008 a 2012, aplicando-se a análise fatorial em componentes principais e de agrupamentos. As regiões com maior e menor desenvolvimento econômico e humano apresentaram alta prevalência das internações e dos custos relativos às doenças do aparelho circulatório, respiratório e do coração para os sexos masculino e feminino. Já nas regiões com nível de desenvolvimento intermediário, as doenças do sistema nervoso em homens e do aparelho circulatório em mulheres se destacaram com relação às morbidades e gastos hospitalares. Assim, ações de prevenção e promoção de saúde a essa população idosa podem ser direcionadas para investigações regionais.

Abstract: Growth in the elderly population has increased both the demand for health services and healthcare expenses, with relevant consequences for economic stability. The current study aimed to analyze the morbidity profile and hospital expenses with elderly patients in relation to socioeconomic and demographic conditions in Paraná State, Brazil, from 2008 to 2012, applying principal components analysis and groupings. Regions with higher and lower economic and human development showed high prevalence of hospitalizations and costs pertaining to the circulatory and respiratory systems and cardiac diseases for males and females. In regions with intermediary development, diseases of the nervous system in men and of the circulatory system in women had the highest morbidity and hospital expenses. Thus, measures for prevention and health promotion in this elderly population should be targeted to regional investigations.

Resumen: El aumento de la población anciana conlleva una creciente demanda de servicios de salud y expansión de gastos, con consecuencias relevantes sobre la estabilidad económica. En este contexto, el objetivo del estudio fue analizar el perfil de las morbilidades y gastos hospitalarios con ancianos en relación con las condiciones socioeconómicas y demográficas de Paraná, Brasil, durante el período de 2008 a 2012, aplicándose el análisis factorial en componentes principales y de agrupamientos. Las regiones con mayor y menor desarrollo económico y humano presentaron una alta prevalencia de los internamientos y de los costes relativos a las enfermedades del aparato circulatorio, respiratorio y del corazón para el sexo masculino y femenino. Desemprego, divórcio, viuvez, filhos que não saem nunca da casa dos pais são as razões que explicam porque, no Brasil, as gerações mais velhas coabitam cada vez mais com as mais jovens. Retornar para a casa familiar, ou ir morar na casa de um(a) filho(a) introduz uma nova dinâmica na organização doméstica: a reorganização do espaço - abrir, criar lugares para receber o novo(s) membro(s); a adaptação das regras, horários e tarefas às necessidades e atividades de cada um, e do grupo em particular. Morar com três ou quatro gerações implica em uma contribuição material e financeira, mas, também, em um suporte moral e afetivo no apoio logístico do cotidiano doméstico. A vida em comum só é possível se existir este circuito de reciprocidades, senão a coabitação pode quebrar a harmonia familiar e tornar-se violenta.

Unemployment, divorce, widowhood, children who never leave their parents' house, these are the reasons that explain why the older generations in Brazil cohabit with the younger generations more than before. Returning to one’s parents’ house, or moving to the children's house introduces a new dynamic into the domestic organization: the reorganization of space - the creation of places to receive new family members; the adaptation of rules, schedules and tasks, to the necessities and activities of each member and to the group's in particular. Living with three or four generations implies a material contribution as well as emotional and moral support for the logistics of daily domestic life. Living together is only possible when there is a circuit of reciprocities; otherwise, cohabitation can provoke violent rupture in the family’s harmony.

Foi realizado um estudo não probabilístico em idosos que vivem na comunidade. A primeira análise teve um delineamento transversal e análises adicionais um delineamento caso-controle. Na primeira análise foram incluídos 745 indivíduos. Análises adicionais foram feitas em 46 idosos saudáveis (HE) e em 33 adultos jovens (YA) incluindo: análises clínicas, hematológicas e bioquímicas, recordatório nutricional de 24 horas e questionário de freqüência dietética. Os idosos saudáveis foram selecionados através do uso do protocolo Seniour. Para evitar viés de seleção na amostra HE, uma análise adicional comparando variáveis culturais, socioeconômicas e saúde com a dos idosos não selecionados (DE). A prevalência de HE foi de 17% (n=46) sendo que, entre estes, 4,3% estavam anêmicos. Não ocorreram diferenças estatísticas de hematócrito, hemoglobina, leucócitos, neutrófilos e vitamina B12 entre os grupos AHEe YA. Níveis médios de MCV, RDW, eosinofilos folato and ferritina foram mais altos no grupo AHE do que no YA. Ao contrário, as plaquetas foram mais altas no grupo YA. Não ocorreram diferenças significativas entre os grupos AHE e DE quando os indicadores nutricionais foram comparados. A comparação entre indicadore nutricionais de idosos anêmicos e não-anêmicos em idosos aparentemente saudáveis mostrou diferenças significativas na ingestão de vitamina B12 e de proteína, que foi mais baixa nos idosos anêmicos. Os resultados sugerem diferenças biológicas independentes entre os parâmetros hematológicos de idosos e jovens.

A non-probabilistic study in an elderly free living community was performed. The first analysis was cross-sectional in design and the additional analysis, a case-control design. In the first analysis, 745 subjects were enrolled. Additional analyses, made in 46 apparently healthy elderly subjects (AHE) and 33 young adults (YA) included: clinical, hematological and biochemical analyses, 24-hours nutritional recall and diet frequency questionnaire. The AHE were selected using the Senieur test protocol. To avoid selection bias in the AHE sample, additional analysis comparing cultural, socioeconomic, and health variables were compared between AHE and non-selected elderly with same dysfunction or morbidity (DE). The prevalence of AHE was 6.17% (n=46) among whom 4.3% were anemic. There were no statistically differences in hematocrit, hemoglobin, leukocytes and vitamin B12 levels between AHE and YA groups. Mean values of MCV, RDW, eosinophils, folate and ferritin were higher in the AHE than the YA group. On the other hand, platelets were higher in the YA group. No statistically significant difference occurred between the AHE and DE groups when nutritional indicators were compared. The comparison between nutritional indicators of anemic and non-anemic apparently healthy elderly people showed statistically significant differences in vitamin B12 and protein intake, which were lower in the anemic elderly. The results suggest independent biological differences between hematological parameters of elderly and young individuals.
