Abstract There is a growing concern about the participation of wild hosts and reservoirs in the epidemiology of several pathogens, particularly within the context of environmental changes and the expansion of the One Health concept. The aim of this study was to investigate the presence of hemoplasmas in opossums rescued from the metropolitan region of Rio de Janeiro state, Brazil. Blood samples were collected from 15 Didelphis aurita and subjected to DNA extraction and PCR using primers for the 16S rRNA and 23S rRNA genes. Physical examination and hematological analysis were also performed. Three out of 15 opossums tested positive for hemotropic Mycoplasma spp. by PCR and showed hematological alterations such as anemia and leukocytosis. Clinical signs were non-specific and associated to traumatic lesions. The phylogenetic analysis indicated that the hemoplasma detected was positioned between 'Ca. Mycoplasma haemodidelphis' detected in D. virginiana from North American and hemoplasmas recently detected in D. aurita from the state of Minas Gerais, Brazil. This study indicates the existence of hemoplasma infections in D. aurita from the metropolitan region of Rio de Janeiro, and reinforce the need for new epidemiological inquiries to clarify the participation of these in the dynamics of circulation of tick-borne pathogens.
Resumo Há uma crescente preocupação com a participação de hospedeiros e reservatórios silvestres na epidemiologia de diversos patógenos, principalmente no contexto das mudanças ambientais e da expansão do conceito "One Health". O objetivo deste estudo foi investigar a presença de hemoplasmas em gambás resgatados da região metropolitana do estado do Rio de Janeiro, Brasil. Amostras de sangue foram coletadas de 15 Didelphis aurita e submetidas à extração de DNA e PCR utilizando-se "primers" para os genes 16S rRNA e 23S rRNA. O exame físico e a análise hematológica também foram realizados. Três dos 15 gambás testaram positivo para Mycoplasma spp. hemotrópico por PCR. Os sinais clínicos eram inespecíficos e associados a lesões traumáticas. Anemia e leucocitose foram detectadas em animais positivos. A análise filogenética indicou que o hemoplasma detectado estava posicionado entre 'Ca. Mycoplasma haemodidelphis' detectado em D. virginiana da América do Norte e hemoplasmas recentemente detectados em D. aurita do estado de Minas Gerais, Brasil. Este estudo indica a existência de infecções por hemoplasmas em D. aurita, da região metropolitana do Rio de Janeiro, e reforça a necessidade de novos inquéritos epidemiológicos, para esclarecer a participação destes na dinâmica de circulação de patógenos transmitidos por carrapatos.

ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

ABSTRACT BACKGROUND: We aimed to describe the clinical characteristics of coronavirus disease 2019 (COVID-19) among healthcare workers (HCWs) in Curitiba, Brazil. METHODS: Upper respiratory samples from 1077 HCWs were tested for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) using reverse transcription polymerase chain reaction from June 16, 2020 to December 9, 2020. RESULTS: Overall, 32.7% of HCWs were infected. The positivity rates in symptomatic and asymptomatic HCWs were 39.2% and 15.9%, respectively. Hospital departments categorized as high-risk for exposure had the highest number of infected HCWs. CONCLUSIONS: Early diagnosis and isolation of infected HCWs remain key in controlling SARS-CoV-2 transmission because HCWs in close contact with COVID-19 patients are more likely to be infected than those who are not.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

Qualitative and quantitative analysis of Mercury ions on the surface of amalgam restorations after home bleaching

RESUMO Objetivo Os agentes clareadores podem interagir com materiais restauradores, como o amálgama, proporcionando um aumento na liberação de íons de mercúrio (Hg), cujo efeito tóxico é conhecido. Considerando que muitos pacientes possuem restaurações de amálgama (AR) e buscam tratamentos clareadores, o presente estudo teve como objetivo avaliar in vitro a quantidade de Hg liberado a partir da AR confeccionada com liga tipo esférica, após serem submetidas a diferentes concentrações de peróxido de carbamida (PC) para clareamento caseiro. Métodos 40 AR foram confeccionadas em dentes bovinos classe I. Após a restauração, as amostras foram distribuídas aleatoriamente em 4 grupos (n = 10): C (grupo controle), CP10 (PC 10%) CP15 (PC 15%) CP20 (PC 20%) e seus tratamentos foram simulados por 14 dias, por 6 horas diárias. Os íons de mercúrio liberados foram medidos por espectroscopia de energia dispersiva de raios-X (EDS) e as mudanças de superfície foram avaliadas pelo Microscópio Eletrônico de Varredura (MEV). Resultados A análise estatística demonstrou que o tratamento clareador resultou em uma liberação de íons mercúrio (p <0,05), embora nenhuma alteração visível tenha sido identificada nas micrografias quando compara-se o grupo C com os outros grupos (CP10, CP15, CP20). Conclusão Pode-se observar aumento da liberação de mercúrio durante o clareamento caseiro simulado.
ABSTRACT Objective The bleaching agents may interact with restorative materials, such as dental amalgam, providing an increase mercury ions (Hg) release, whose toxic effect is known. Whereas many patients have amalgam restorations (AR) and seek bleaching treatments, the present study aimed to evaluate in vitro the amount of Hg released from AR made with spherical alloy, after being subjected to different concentrations of carbamide peroxide (CP) for home bleaching. Methods 40 class I AR were prepared in bovine teeth. Afterthe restoration, the samples were randomly allocated into 4 groups (n = 10): C (control group), CP10 (CP 10%) CP15 (CP 15%) CP20 (CP 20%) and its treatments were simulated for 14 days for 6 hours daily. The Hg ions released was measured by energy dispersive X-ray spectroscopy (EDS) and surface changes were assessed in the Scanning Electron Microscope (SEM). Hg levels (percent mass) were recorded and the differences were statistically analyzed using the Kruskal-Wallis test and Dunn’s “Post hoc” test. Results Statisticalanalysis showed the bleaching treatment resulted in a higher Hg release (p <0.05), although no visible changes were identified in micrographs when comparing the control group with the other groups (CP10, CP15, CP20). Conclusion Increased Hg release may be observed during simulated home bleaching.

Resumo O objetivo do estudo foi compreender quais são os principais desafios para adesão ao tratamento antirretroviral em Moçambique na perspectiva dos profissionais de saúde que lidam com a temática do HIV/aids no país. O estudo é qualitativo, observacional e exploratório. Entrevistas semiestruturadas foram realizadas com 11 profissionais de saúde que prestam assistência em HIV/aids em sete das onze províncias do país. Os dados foram analisados por meio da técnica de análise do Discurso do Sujeito Coletivo. Os resultados foram categorizados em nove ideias centrais. Dentre os problemas mencionados pelos profissionais estão aspectos relacionados à cultura local, ao receio da revelação forçada do diagnóstico de HIV positivo a terceiros, à ausência de uma adequada compreensão sobre os benefícios do tratamento, à forma de organização dos serviços de saúde, a questões de gênero, a aspectos sociais como fome, dificuldades financeiras e problemas com transporte, dentre outros.
Abstract The goal of the study was to understand the main challenges for adherence to antiretroviral treatment in Mozambique from the perspective of health professionals who deal with HIV/AIDS. The study is qualitative, observational, and exploratory. Semi-structured interviews were conducted with 11 health professionals who provide HIV/AIDS assistance in seven of the country's eleven provinces. Data analysis was based on the Discourse of the Collective Subject. Results were categorized into nine central ideas. Among the problems mentioned by health professionals there are aspects related to culture, the fear of forced disclosure of HIV positive diagnosis, the absence of an adequate understanding of the benefits of treatment, the way health services are organized, gender issues, and social aspects such as hunger, financial difficulties and problems with transportation, among others.

Absence of cytomegalovirus in gingivitis and chronic periodontitis in HIV-1 patients in Northern Brazil

Abstract INTRODUCTION The influence of cytomegalovirus (CMV) on the progression of chronic periodontitis in HIV patients is poorly investigated. METHODS ELISA was used for anti-CMV antibody IgG titer measurements and real-time polymerase chain reaction for qualitative and quantitative CMV detection. Data on the CD4 + T lymphocyte count and plasma HIV viral load were obtained from patient records. RESULTS CMV DNA was detected in samples of subgingival biofilm in only three individuals, two of them with chronic periodontitis (4%) and one with gingivitis (3.3%). CONCLUSIONS The prevalence of CMV is very low both in HIV-1 patients with gingivitis and chronic periodontitis.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

INTRODUÇÃO: Nos últimos anos, um número crescente de estudos sugere a participação dos herpesvírus na doença periodontal. OBJETIVO: Este trabalho investiga a relação entre a presença do herpesvírus Epstein-Barr (EBV) e a infecção periodontal em pacientes com periodontite crônica. METODOLOGIA: Foram coletadas amostras de biofilme subgengival de sítios com profundidades de sondagem de 4 a 6 mm e > 7 mm, de 28 pacientes com periodontite crônica. Como controles, foram incluídos 16 indivíduos, sistemicamente saudáveis e sem doença periodontal. Adicionalmente, parâmetros clínicos de profundidade de sondagem (PS), nível clínico de inserção (NCI) e índice de sangramento à sondagem (SS) foram registrados. RESULTADO: Os resultados demonstraram médias de 2,7 mm PS, 1,7 mm NCI e 0,3% dos sítios apresentaram SS. A investigação do EBV no biofilme subgengival dos grupos foi realizada por meio da reação em cadeia da polimerase com primer espécie-específico. Os resultados da análise viral indicaram ausência de EBV em todas as amostras subgengivais analisadas. CONCLUSÃO: A partir destes resultados, não foi encontrada relação entre a presença do herpesvírus Epstein-Barr e a periodontite crônica.
INTRODUCTION: In recent years, a growing number of studies have suggested the participation of the herpes virus in periodontal disease. OBJECTIVE: this study investigates the relationship between the presence of the Epstein-Barr herpes virus and periodontal infection in patients with chronic periodontitis. METHODOLOGY: subgingival biofilm samples were collected of subgingival sites with probing depths of 4 mm to 6 mm, and > 7 of 28 patients with chronic periodontitis. The control group consisted of 16 healthy subjects without clinical evidence of chronic periodontitis. Additionally, clinical parameters of probing depth, attachment level and Bleeding index were recorded. RESULT: the results showed averages of 2 mm probing depth, 1, 7 mm attachment level and 0.3 % bleeding on probing. Investigation of the herpes virus in the subgingival biofilm of the groups was performed using polymerase chain reaction with species-specific primer. Results of viral analysis indicated the absence of EBV in all subgingival samples analyzed. CONCLUSION: these results found no relationship between the presence of the Epstein-Barr herpes virus and chronic periodontitis.

Bem-estar psicológico, enfrentamento e lipodistrofia em pessoas vivendo com HIV/AIDS

Os objetivos do estudo foram investigar os efeitos da lipodistrofia sobre o bem-estar psicológico e a adesão ao tratamento anti-retroviral, e identificar as estratégias de enfrentamento utilizadas por pessoas soropositivas afetadas pela síndrome. Participaram 21 pessoas, com idades entre 29 e 52 anos, das quais quatorze eram homens. As entrevistas foram analisadas com base na análise de conteúdo categorial. Os resultados apontaram que a descoberta e a vivência da lipodistrofia podem afetar o bem-estar psicológico, ocasionando redução da auto-estima, percepção negativa da imagem corporal e evitação de contatos sociais. Não se observou impacto negativo sobre a conduta de adesão. As estratégias de enfrentamento relatadas com maior freqüência foram: prática de atividade física, uso de roupas que escondam as mudanças corporais, realização de procedimentos estéticos e adoção de novos hábitos alimentares. Conclui-se que a ocorrência da lipodistrofia pode acarretar dificuldades psicológicas, merecendo atenção das equipes de saúde que atuam em HIV/aids.
The study investigates the effects of the HIV lipodystrophy syndrome on the psychological well-being and on the adherence to anti-retroviral treatment. It also identifies the coping strategies used. Twenty-one people, age bracket 29 - 52 years old, including fourteen males, took part in the research. Data procedures included structured and semi-structured interview. Results showed that disclosure of lipodystrophy may affect the psychological well-being, leading towards a decrease in self-esteem, negative perception of body image and avoidance of social relationships. The most related coping strategies consisted of practice of physical activities, usage of clothing which can hide body changes, realization of esthetic procedures and the adoption of new feeding habits. Results show that lipodystrophy experience may cause relevant emotional and psychological difficulties to affected people. This fact suggests that it should be given more attention to health professionals who deal with HIV/AIDS.
Los objetivos del estudio fueron explorar los efectos de la lipodistrofia en el bienestar psicológico, la adhesión al tratamiento antirretroviral, bien como identificar las estrategias de afrontamiento utilizadas por personas soropositivas con lipodistrofia. Participaran de la investigación 21 personas, con edades entre 29 y 52 años, catorce hombres. El análisis de las entrevistas siguió la técnica de análisis de contenido categorial. Los resultados indican que la descubierta y vivencia de la lipodistrofia pueden afectan el bienestar psicológico, lo que ocasiona reducción en la autoestima, percepción negativa de la imagen corporal y evitación de personas de la red social. No se ha observado impacto negativo en las conductas de adhesión al tratamiento. Las más frecuentes estrategias de afrontamiento relatadas fueron: práctica de actividad física, uso de ropas que disfrazan los cambios corporales, realización de procedimientos estéticos y adopción de nuevos hábitos alimentares. Se ha concluido que la lipodistrofia puede causar dificultades psicológicas, lo que justifica una atención especial de los equipos de salud que actúan en VIH/Sida.
