The objective were to describe the prevalence of underweight and overweight, assessed by body mass index (BMI), stratified by sex and age group, and to analyze the sociodemographic characteristics associated with BMI in older women and men. This is a cross-sectional analysis of 8,974 participants aged ≥ 50 years from the baseline of the Brazilian Longitudinal Study of Aging (ELSI-Brasil, 2015-2016). BMI was classified as underweight, eutrophy, and overweight according to the participant’s age. A multinomial logistic regression model was used, considering the sociodemographic characteristics of women and men. The results showed a higher prevalence of overweight in women compared to men (64.1% vs. 57.3%). In both sexes, the prevalence of underweight was higher in the longest-lived individuals, while overweight was lower. In women, the chance of underweight was higher than the chance of eutrophy in those who were single/widowed/divorced (OR = 1.95; 95%CI: 1.42-2.66) and in those living in rural areas (OR = 1.58; 95%CI: 1.01-2.49), while the chance of being overweight was lower than the chance of being eutrophy in those living in rural areas (OR = 0.78; 95%CI: 0.62-0.97) and in all geographic macro-regions related to the South Region. For men, the chance of being overweight was lower than the chance of being eutrophy among single/widowed/divorced individuals (OR = 0.58; 95%CI: 0.48-0.69). The richest had a lower chance of being underweight (OR = 0.59; 95%CI: 0.38-0.90), as well as a higher chance of being overweight (OR = 1.52; 95%CI: 1.20-1.92). In conclusion, the sociodemographic characteristics associated with BMI differed between the sexes.

Los objetivos fueron describir la prevalencia de bajo peso y sobrepeso, evaluados a través del índice de masa corporal (IMC), estratificada por sexo y grupo de edad, y analizar las características sociodemográficas asociadas al IMC en mujeres y hombres mayores. Se trata de un análisis transversal de 8.974 participantes con ≥ 50 años de la línea de base del Estudio Longitudinal Brasileño sobre el Envejecimiento (ELSI-Brasil, 2015-2016). Se clasificó el IMC en bajo peso, eutrofia y sobrepeso conforme la edad del participante. Se utilizó el modelo de regresión logística multinomial, teniendo en cuenta las características sociodemográficas de mujeres y hombres. Los resultados evidenciaron una prevalencia más alta de sobrepeso en las mujeres en comparación con los hombres (64,1% vs. 57,3%). En ambos sexos, la prevalencia de bajo peso fue más alta en los grupos de mayor edad, mientras que la prevalencia del sobrepeso fue menor. La chance de bajo peso fue más alta que la chance de eutrofia en las mujeres solteras/viudas/divorciadas (OR = 1,95; IC95%: 1,42-2,66) y en las que viven en el área rural (OR = 1,58; IC95%: 1,01-2,49), mientras que la chance de sobrepeso fue menor que la chance de eutrofia en las que viven en el área rural (OR = 0,78; IC95%: 0,62-0,97) y en todas las macrorregiones geográficas relacionadas a la región Sur. La chance de sobrepeso fue menor que la chance de eutrofia entre los hombres solteros/viudos/divorciados (OR = 0,58; IC95%: 0,48-0,69). Los más ricos presentaron una chance menor de bajo peso (OR = 0,59; IC95%: 0,38-0,90), así como una chance más alta de sobrepeso (OR = 1,52; IC95%: 1,20-1,92). En conclusión, las características sociodemográficas asociadas al IMC difirieron entre los sexos.

Os objetivos foram descrever a prevalência de baixo peso e excesso de peso, avaliados pelo índice de massa corporal (IMC), estratificada por sexo e faixa etária, e analisar as características sociodemográficas associadas ao IMC em mulheres e homens mais velhos. Trata-se de uma análise transversal de 8.974 participantes com ≥ 50 anos da linha de base do Estudo Longitudinal da Saúde dos Idosos Brasileiros (ELSI-Brasil, 2015-16). O IMC foi classificado em baixo peso, eutrofia e excesso de peso de acordo com a idade do participante. Foi utilizado modelo de regressão logística multinominal, considerando-se as características sociodemográficas de mulheres e homens. Os resultados evidenciaram maior prevalência de excesso de peso nas mulheres em comparação aos homens (64,1% vs. 57,3%). Em ambos os sexos, a prevalência de baixo peso foi maior nos mais longevos, enquanto que o excesso de peso foi menor. Nas mulheres, a chance de baixo peso foi maior do que a chance de eutrofia naquelas solteiras/viúvas/divorciadas (OR = 1,95; IC95%: 1,42-2,66) e nas residentes na área rural (OR = 1,58; IC95%: 1,01-2,49), ao passo que a chance de excesso de peso foi menor do que a chance de eutrofia nas residentes na área rural (OR = 0,78; IC95%: 0,62-0,97) e em todas as macrorregiões geográficas relativas à Região Sul. Para os homens, a chance de excesso de peso foi menor do que a chance de eutrofia entre solteiros/viúvos/divorciados (OR = 0,58; IC95%: 0,48-0,69). Os mais ricos apresentaram menor chance de baixo peso (OR = 0,59; IC95%: 0,38-0,90), bem como maior chance de excesso de peso (OR = 1,52; IC95%: 1,20-1,92). Em conclusão, as características sociodemográficas associadas ao IMC diferiram entre os sexos.

Abstract The objective of the study was to measure the impacts of Pronaf access on family farming (FF) performance measures with analysis for the FF and its typologies Pronaf B and Pronaf V of the 2017 Agricultural Census. Variables related to family farmers, their establishments and climate were used. The methodology was composed of Entropy Balancing for treatment analysis and Minimal Weighted Squares to measure the impacts. The use of different levels of intensity confirmed the presence of regional concentrations in access in all analyzes. The results of the impacts on FF showed to be related to the Pronaf V typology, confirming the need to analyze the disaggregated FF. The impacts on the Pronaf B typology were negative, suggesting that these farmers are potentially using the obtained credit inappropriately and/or inefficiently. This reaffirms the need to integrate Pronaf with other policies, such as technical assistance and rural extension.

Resumo O objetivo do estudo foi mensurar os impactos do acesso ao Pronaf nas medidas de desempenho da agricultura familiar (AF) com análise para o AF e suas tipologias Pronaf B e Pronaf V do Censo Agropecuário 2017. Foram utilizadas variáveis relacionadas aos agricultores familiares, seus estabelecimentos e o clima. A metodologia foi composta por Balanceamento por Entropia para análise do tratamento e Mínimos Quadrados Ponderados para a mensuração dos impactos. O uso de diferentes níveis de intensidade confirmou a presença de concentrações regionais no acesso em todas as análises. Os resultados dos impactos sobre a AF mostraram-se relacionados à tipologia Pronaf V, confirmando a necessidade de analisar a AF desagregada. Os impactos na tipologia Pronaf B foram negativos, sugerindo que esses agricultores estão potencialmente utilizando o crédito obtido de forma inadequada e/ou ineficiente. Isso reafirma a necessidade de integrar o Pronaf a outras políticas, como assistência técnica e extensão rural.

ABSTRACT: The domestic and international market is increasingly seeking for foods with reduced chemical additives. The viability using a versatile product as mechanically separate meat (CMS) to produce safety food is necessary to expand the product market. Hansenulawingei strain was selected, a yeast known as a killer toxin for poultry CMs application. The first part of this study consisted of defining the best growth condition for yeast, static or agitation, as well as the time either growth. The result obtained was the medium without agitation and 108 hours of growth at room temperature. The extract was subjected to the atomization drying process and the pulverized extract obtained was evaluated in vitro for minimum inhibitory concentration (MIC) tests for Salmonellasp, Staphylococcus positive coagulase, E. coli and mesophilic bacteria. Proximal composition, which the most relevant values to be analyzed were directed to total proteins that were 4.26g% (± 0.66) and 4.37g% (± 0.01) for agitation and unrestrained system, respectively. In the result of MIC, dry extracts were efficient in controlling the growth of all tested bacteria, including Salmonella spp. In the in situ tests, in which mechanically separated meat was raw materials to determine the antimicrobial factor action against food pathogens the concentration of 0.083g/ml of dry extract in its raw form obtained an inhibition factor as satisfactory as healing salts traditionally applied by the carneous industry. The viability of its growth and concentration for drying, can be considered a viable antimicrobial with good prospects for bio-conservative action.

RESUMO: O mercado nacional e internacional busca cada vez mais por alimentos com reduzido teor de aditivos químicos. Estudar a viabilidade de um novo método de conservação natural de um produto tão versátil como a carne mecanicamente separada (CMS) se faz necessário para expandir o mercado do produto, seguindo as tendências das demandas alimentares dos consumidores atuais. Portanto foi selecionada uma cepa de Hansenula wingei, uma levedura conhecida pela produção de toxina killer para aplicação em CMS de aves. A primeira parte deste estudo consistiu em definir qual a melhor condição de crescimento para a levedura, consistindo este em ser estático ou em agitação, bem como o tempo no qual haveria o maior pico de crescimento da mesma. O resultado obtido foi o meio sem agitação e com 108 horas de crescimento a temperatura ambiente. Na sequência o extrato foi submetido ao processo de secagem por atomização e o pó obtido foi avaliado in vitro para testes de concentração mínima inibitória (MIC) para Salmonella sp, Staphylococcus coagulase positivo, E. coli e Mesofilos aeróbicos e definição de sua composição proximal, a qual os valores mais relevantes a serem analisados eram direcionados para proteínas totais que foram 4.26g% (±0.66) e 4.37g% (±0.01) para sistema com agitação e sem agitação, respectivamente. Como resultado de MIC, os extratos secos mostraram-se eficientes no controle do crescimento de todas as bactérias testadas, inclusive Salmonella spp. Nos testes in situ, no qual a carne mecanicamente separada foi a matéria prima testada para determinar a ação fator antimicrobiano contra patógenos de alimentos a concentração de 0.083g/ml do extrato seco, em sua forma bruta, obteve um fator de inibição tão satisfatório quanto aos sais de cura aplicados tradicionalmente pela indústria cárnea e pela viabilidade de seu crescimento e concentração por secagem, pode ser considerado um antimicrobiano viável e com boas perspectivas para ação bioconservante.

Abstract Objective To compare the use of mirabegron with anticholinergics drugs for the treatment of overactive bladder (OB). Data Source Systematic searches were conducted in EMBASE, PUBMED, Cochrane, and LILACS databases from inception to September 2021. We included RCTs, women with clinically proven OB symptoms, studies that compared mirabegron to antimuscarinic drugs, and that evaluated the efficacy, safety or adherence. Data Collection RevMan 5.4 was used to combine results across studies. We derived risk ratios (RRs) and mean differences with 95% CIs using a random-effects meta-analytic model. Cochrane Collaboration Tool and GRADE was applied for risk of bias and quality of the evidence. Data Synthesis We included 14 studies with a total of 10,774 patients. Fewer total adverse events was reported in mirabegron group than in antimuscarinics group [RR 0.93 (0.89–0.98)]. The risk of gastrointestinal tract disorders and dry mouth were lower with mirabegron [RR 0,58 (0.48–0.68); 9375 patients; RR 0.44 (0.35–0.56), 9375 patients, respectively]. No difference was reported between mirabegron and antimuscarinics drugs for efficacy. The adherence to treatment was 87.7% in both groups [RR 0.99 (0.98–1.00)]. Conclusion Mirabegron and antimuscarinics have comparable efficacy and adherence rates; however, mirabegron showed fewer total and isolated adverse events.

Resumo Objetivo Comparar o uso de mirabegrom com anticolinérgicos para o tratamento da bexiga hiperativa (BH). Fonte de Dados Buscas sistemáticas foram realizadas nas bases de dados EMBASE, PUBMED, Cochrane e LILACS desde o início até setembro de 2021. Incluímos ECR, mulheres com sintomas de BH clinicamente comprovados, estudos que compararam mirabegrom a medicamentos antimuscarínicos e avaliaram a eficácia, segurança ou adesão. Coleta de Dados RevMan 5.4 foi usado para combinar os resultados entre os estudos. Derivamos razões de risco (RRs) e diferenças médias com intervalo de confiança (IC) de 95% usando um modelo meta-analítico de efeitos aleatórios. Cochrane Collaboration Tool e GRADE foi aplicado para risco de viés e qualidade da evidência. Síntese dos Dados Foram incluídos 14 estudos com um total de 10.774 pacientes. Menos eventos adversos totais foram relatados no grupo mirabegrom do que no grupo antimuscarínicos [RR: 0,93 (0,89–0,98)]. O risco de distúrbios do trato gastrointestinal e boca seca foram menores com mirabegrom [RR: 0,58 (0,48–0,68); 9.375 pacientes; RR: 0,44 (0,35–0,56), 9.375 pacientes, respectivamente]. Nenhuma diferença foi relatada entre mirabegrom e drogas antimuscarínicos para eficácia. A adesão ao tratamento foi de 87,7% em ambos os grupos [RR: 0,99 (0,98–1,00)]. Conclusão Mirabegrom e antimuscarínicos têm eficácia e taxas de adesão comparáveis, porém o mirabegrom apresentou menos eventos adversos totais e isolados.

Abstract Introduction: SARS-CoV-2 infection is characterized by hyperproduction of pro-inflammatory cytokines that impact the cardiovascular system. In addition, several cardiovascular risk factors, like arterial hypertension, were identified as risk factors for greater disease severity and mortality in these patients. The objective of this study is to evaluate the prevalence of hypertension in patients with COVID-19 and its association with complications, comorbidities and mortality. Methods: Retrospective study of the patients with COVID-19 admitted to Internal Medicine ward between March 2020 and February 2021. Data was collected from the digital clinical file. Logistic regression was applied to clarify the effect of independent variables on mortality. Results: The sample included 1291 patients with COVID-19, a median age of 73 years (IQR: 22) and male gender predominance (n = 701, 54.3%). There were 65.5% (n = 845) of patients with hypertension, 54.9% (n = 709) dyslipidemia and about a third had diabetes mellitus and obesity (31 and 38%, respectively). Comparatively to the non-hypertensive patients, hypertensive group presented with higher prevalence of other cardiovascular risk factors (diabetes mellitus, obesity and dyslipidemia (p <0.001)) and lower functionality and more comorbidities (as calculated by AVDezIS and Charlson modified scores, p<0.001). There was also higher proportion of severe COVID-19 disease (p = 0.003), greater number of patients admitted to intensive care units (p <0.001) and higher in-hospital mortality (p <0.001). Hypertension was not a mortality predictive factor in these patients. Conclusion: In this study, patients with hypertension presented with higher severity of COVID-19 disease, higher pre-valence of other cardiovascular risk factors, complications during hospitalization and in-hospital mortality, however, its presence was not a mortality predictive factor.

Resumo Introdução: A infeção por SARS-CoV-2 caracteriza-se por um estado de sobreprodução de citocinas pró-inflamatórias que podem culminar com atingimento cardiovascular. Além disso, vários fatores de risco cardiovascular, como a hipertensão arterial (HTA), têm sido associados a maior severidade e mortalidade na COVID-19. O objetivo deste estudo é avaliar a prevalência da HTA nos doentes com COVID-19 e a sua associação com complicações, comorbilidades e mortalidade. Métodos: Estudo retrospetivo dos doentes admitidos em enfermaria por COVID-19 entre março de 2020 e fevereiro de 2021. Foi realizada colheita de dados através do processo clínico. Aplicada regressão logística para clarificar o efeito de variáveis independentes na mortalidade. Resultados: Incluíram-se 1291 doentes com COVID-19 internados no período descrito, com mediana de idades de 73 anos (AIQ: 22) e predomínio do sexo masculino (n = 701, 54,3%). Observou-se que 65,5% (n = 845) dos doentes tinham HTA, 54,9% (n = 709) dislipidemia e 31% e 38% tinham diabetes mellitus e obesidade, respetivamente. Comparativamente com os indivíduos sem HTA, o grupo com HTA apresentou maior prevalência de fatores de risco cardiovascular (diabetes, obesidade e dislipidemia, p <0,001) bem como menor grau de funcionalidade e maior carga de doença (respetivamente score de AVDezIs e Charlson modificado, p <0,001). Verificou-se uma maior proporção de doença grave (p = 0,003), maior número admissões em Unidade de Cuidados Intensivos (p <0,001) e maior taxa de mortalidade intra-hospitalar (p <0,001). A presença de HTA não se revelou como preditor de mortalidade nos doentes com COVID-19. Conclusão: Nesta amostra os doentes com HTA apresentaram múltiplos fatores de risco cardiovascular, doença COVID-19 mais grave, mais complicações e mortalidade intra-hospitalares, contudo, a sua presença não foi um fator preditor de mortalidade por COVID-19.

ABSTRACT OBJECTIVE To analyze the association between municipal rates of ambulatory care sensitive conditions (ACSC) hospitalization and the quality of primary health care (PHC), socioeconomic, and demographic variables and those related to local characteristics of the health system from 2010 to 2019. METHOD Ecological time series study in Brazilian municipalities analyzing the correlation of ACSC hospitalization rates with PHC quality measured by the three cycles of the Primary Care Access and Program for improving primary care access and quality (PMAQ-AB). The study included municipalities whose teams participated in 80% or more of at least two PMAQ-AB cycles. The correlation between standardized ACSC hospitalization rates and PHC quality and other variables was analyzed. Spearman’s test was used between the response variable and numerical explanatory variables. Generalized equations estimation was used as a multivariate model associating ACSC hospitalization rates with the other variables over the years. RESULTS A total of 3,500 municipalities were included in the models. The quality of PHC (PMAQ-AB score) showed an inverse association with the variation in ACSC hospitalization rates. Hospitalization rates fell by -2% per year every ten-point increase in the PMAQ-AB score, adjusted by the remaining variables. A one-unit increase in the beds per 1,000 inhabitants variable had an impact of approximately +6.4% on ACSC hospitalization rates. Regarding population size, larger municipalities had lower ACSC hospitalization rates. Increased PHC coverage and lower socioeconomic inequality were also associated with the reduction in hospitalizations. CONCLUSIONS The reduction in ACSC hospitalization rates over time was associated with an increase in the quality of PHC. It was also associated with a reduction in the number of hospital beds and municipalities with better socioeconomic indicators.

RESUMO OBJETIVO Analisar a associação entre taxas municipais de internações por condições sensíveis à atenção primária (ICSAP), com a qualidade da atenção primária à saúde (APS), variáveis socioeconômicas, demográficas e relacionadas a características locais do sistema de saúde, de 2010 a 2019. MÉTODOS Estudo ecológico de séries temporais nos municípios brasileiros analisando a correlação das taxas de ICSAP, com a qualidade da APS medida pelos três ciclos do Programa de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ-AB). Foram incluídos municípios que participaram com 80% ou mais de suas equipes em, ao menos, dois ciclos do PMAQ-AB. Foi analisada a correlação entre as taxas de ICSAP padronizadas com a qualidade da APS e demais variáveis. Empregou-se o teste de Spearman entre a variável resposta e as variáveis explicativas numéricas. Foi usado o generalized equations estimating como modelo multivariado associando as taxas de ICSAP e as demais variáveis ao longo dos anos. RESULTADOS Foram incluídos 3.500 municípios nos modelos. A qualidade da APS (nota do PMAQ-AB) apresentou associação inversa com a variação das taxas de ICSAP. As taxas de internação tiveram queda de -2% ao ano a cada aumento de dez pontos na nota do PMAQ-AB, ajustado pelas demais variáveis. O aumento de uma unidade na variável leitos por mil habitantes impactou em uma elevação de aproximadamente +6,4% nas taxas de ICSAP. Quanto ao porte populacional, municípios maiores tiveram menores taxas de ICSAP. Também se associaram à redução das internações o aumento da cobertura da APS e a menor desigualdade socioeconômica. CONCLUSÕES A redução das taxas de ICSAP ao longo do tempo mostrou-se associada com o aumento da qualidade da APS. Além disso, esteve associada com diminuição do número de leitos hospitalares e a municípios com melhores indicadores socioeconômicos.

ABSTRACT BACKGROUND: Achalasia is an esophageal motility disorder, with clinical presentation of dysphagia and regurgitation. This is a chronic condition with no cure. Current treatment options aim to reduce lower esophageal sphincter tone by pharmacological, endoscopic or surgical means, with the aim of improving patients’ symptoms. Peroral endoscopic myotomy (POEM) is an alternative endoscopic surgery to Heller cardiomyotomy, in which the procedure is performed orally, by endoscopy, offering efficacy comparable to surgical myotomy, with relative ease and minimal invasion, without external incisions. AIMS: To study the safety of POEM by analyzing its results, adverse events and perioperative complications and the main ways to overcome them, in addition to evaluating the effectiveness of the procedure and the short-term postoperative quality of life. METHODS: A qualitative and quantitative, observational and cross-sectional study that analyzed patients who underwent the POEM in a reference center, from December 2016 to December 2022, maintaining the technical standard of pre-, peri- and postoperative protocol. RESULTS: A total of 94 patients were included in the study, and only three had postoperative complications. The average early postoperative Eckardt score was 0.93 and the late 1.40, with a mean improvement of 7.1 in early results and 6.63 in late results (p<0.05). CONCLUSIONS: POEM can be reproduced with an excellent safety profile, significant relief of symptoms and improvement in esophageal emptying, and in quality of life.

RESUMO RACIONAL: A acalasia é uma desordem da motilidade esofágica, com apresentação clínica de disfagia e regurgitação, uma condição crônica sem cura. As opções de tratamentos atuais visam reduzir o tônus do esfíncter esofagiano inferior (EEI) por meios farmacológicos, endoscópicos ou cirúrgicos, com o objetivo de aliviar os sintomas dos pacientes. A miotomia endoscópica peroral, sigla do inglês (POEM) é uma cirurgia endoscópica alternativa à cardiomiotomia a Heller (CH), em que a miotomia é realizada por via oral, por endoscopia, oferecendo eficácia comparável a miotomia cirúrgica, com relativa facilidade e mínima invasão, sem incisões externas. OBJETIVOS: Analisar a segurança do POEM analisando seus resultados, eventos adversos, complicações perioperatórias e suas principais formas de contorná-los, além de avaliar a eficácia do procedimento e a qualidade de vida pós-operatória, em curto prazo. MÉTODOS: Estudo qualitativo e quantitativo, observacional e transversal avaliando pacientes que realizaram o POEM em um centro de referência, entre dezembro de 2016 até dezembro de 2022, mantendo padrão técnico de preparo pré, peri e pós-operatório. RESULTADOS: Foram incluídos no estudo 94 pacientes e apenas 3 apresentaram complicações pós-operátorias. O escore de Eckardt médio precoce pós-operatório foi de 0,93 e o tardio de 1,40, com melhora média de 7,1 em resultados precoces e de 6,63 em resultados tardios (p<0,05). CONCLUSÕES: O POEM pode ser reproduzido com excelente perfil de segurança, alívio significativo dos sintomas e melhora do esvaziamento do esôfago, oferecendo melhora na qualidade de vida.

Abstract Projects on river basin integration are keen social-economical drivers in dry regions like the Brazilian semiarid, however, there are concerning ecological impacts implied in those projects. In a long-term analysis, ichthyofauna colonization and spread through the East Axis of the São Francisco River Integration Project (SFIP) was monitored to assess possible impacts on the receiving Paraíba River basin. The fish were collected semiannually (2012 to 2021) from 19 sites in the São Francisco (SF) and Paraíba (PB) basins. A total of 69 fish species were recorded, with distinct fish assemblages between SF (n = 50), PB (n = 35), and the SFIP artificial reservoirs (n = 25). The SFIP reservoirs were colonized by species from the donor basin (SF). In a pioneer finding, Anchoviella vaillanti was recorded for the first time in the receiving basin and it is in the process of establishment. The two SF species that reached PB through the SFIP canals (A. vaillanti and Moenkhausia costae) may be using their year-round reproduction and wide diet to successful spread and colonize the new environment. Since we detected species with potential to reach the receiving basin and became invasives, the implementation of barriers to contain their spread are recommended.

Resumo Projetos de integração de bacias hidrográficas são socialmente importantes em regiões como o semiárido brasileiro, porém há impactos ecológicos preocupantes implícitos nesses grandes projetos de infraestrutura. A colonização e dispersão da ictiofauna pelo Eixo Leste do Projeto de Integração do Rio São Francisco (PISF) foi monitorada para avaliar possíveis impactos na bacia receptora do rio Paraíba do Norte. Os peixes foram coletados semestralmente (2012 a 2021) em 19 locais das bacias do São Francisco (SF) e Paraíba (PB). Foram registradas 69 espécies de peixes, sendo 50 nos pontos do SF, 25 nos reservatórios artificiais ao longo do PISF e 35 nos pontos do PB. As assembleias de peixes das bacias do SF, PB e dos reservatórios do PISF foram significativamente distintas. Os reservatórios do PISF foram colonizados por espécies provenientes da bacia doadora (SF). Anchoviella vaillanti foi registrada pela primeira vez na bacia receptora do PB e está em processo de estabelecimento. As duas espécies do SF que chegaram ao PB pelos canais do PISF (A. vaillanti e Moenkhausia costae) apresentaram dieta e estratégias reprodutivas que permitem o sucesso na disseminação e colonização. Uma vez que foram detectadas espécies com potencial de atingir a bacia receptora, recomenda-se o monitoramento e manejo contínuos.

Abstract Introduction Depression in aging may lead to loss of autonomy and worsening of comorbidities. Understanding how positive attributes contribute to healthier and happier aging has been one ofthe purposes of Positive Psychology. However, the literature still lacks studies that evaluate how depression in the elderly is related to constructs considered positive. Objective The present study aimed comparing scores of constructs of spiritual well-being, social support, selfesteem, life satisfaction, affection, optimism, and hope in the elderly with minimal, mild, moderate, and severe depression and healthy controls in order to investigate possible indirect and mediated relationships between positive constructs and depression. Methods A cross-sectional study was conducted with elderly, 62 of whom were diagnosed with different severity of Major Depression (DSM-V) (minimum, mild, moderate, and severe according to the Beck Depression Inventory — BDI) and 66 healthy controls matched by age, sex and schooling. The instruments used were adapted and validated versions of the Spirituality Self-Rating Scale, the Rosenberg Self-Esteem Scale, the Medical Outcomes Social Scale of Support, the Life Satisfaction Scale, the Positive and Negative Affect Schedule, the Revised Life Orientation Test, and the Adult Dispositional Hope Scale. After comparing the means of scores between groups, an analysis of normalized partial association networks was performed to investigate the direct and mediated relationships between depression and other evaluated constructs. Results Scores of spiritual well-being, social support, self-esteem, life satisfaction, positive affect, optimism, negative affects, and hope differed significantly between the control group and the degrees of depression (p < 0.001). The analysis of normalized partial association networks has shown that the relations of depression with the constructs of life satisfaction, self-esteem, and social support are mediated, while the constructs of dispositional hope, positive affect, spiritual well-being, and optimism are indirectly related to depression. The social class was also positively related to depression. Conclusion Depression in different degrees is associated with a reduction in the scores of instruments that evaluate positive attributes. The constructs directly associated with depression are spiritual well-being, optimism, positive affect, and dispositional hope. The others had mediated relationship. These results may contribute to the planning of future interventions for the prevention of depression among the elderly.

Abstract Objective This review synthesized existing studies on the prevalence of chronic pain in Brazil and its associated factors to produce a recent estimation to guide public health politics. Methods A search was carried out in the Ovid Medline, Embase, Web of Science, and BVS Regional/Lilacs databases to identify population-based cross-sectional studies from 2005 to 2020, which reported the prevalence of benign chronic pain in Brazil (more than three months). The risk of bias was assessed using design, sample size determination, and random selection as essential issues. Pooled prevalence estimates were calculated for chronic pain in the general and elderly populations. The protocol was registered on Prospero (CRD42021249678). Results Of the 682 identified, 15 macheted the authors’ inclusion criteria. Chronic pain prevalence in the general adult population ranged from 23.02% to 41.4% (pooled estimate 35.70%, 95% Cis 30.42 to 41.17) and was described as moderate to intense. It was associated with female sex, old age, lower education, intense professional activity, excessive alcohol consumption, smoking, central obesity, mood disorder, and sedentarism. The Southeastern and Southern regions presented a higher prevalence. The prevalence in the elderly population ranged from 29.3% to 76.2% (pooled estimate 47.32%, 95% Cis 33.73 to 61.11). In addition, this population visited doctors more frequently, had more sleep disorders, and was more dependent on daily living activities. Almost fifty percent of both populations with chronic pain reported pain-induced disability. Conclusion Chronic Pain is highly prevalent in Brazil and associated with significant distress, disability, and poorly controlled.

ABSTRACT Objective: To describe the technique, analyze possible radiographic correction and evaluate the clinical result of medial and plantar calcaneal displacement osteotomy associated with opening wedge cuboid osteotomy for flexible flatfoot correction. Methods: 23 patients (30 feet) diagnosed with flexible flat foot treated with plantar and medial calcaneal displacement osteotomy associated with opening wedge cuboid osteotomy were evaluated retrospectively. In the lateral radiographs calcaneal pitch and Meary’s angle were the radiographic parameters evaluated; while the talonavicular coverage angle was evaluated in the anteroposterior radiographs. To assess the clinical outcome of the surgical procedure, the American Orthopedic Foot and Ankle Society Score (AOFAS) for the ankle and hindfoot was adopted. Results: The mean values of the evaluated angles and AOFAS score for ankle and hindfoot significantly improved when comparing pre- and postoperative values. Conclusion: Plantar and medial calcaneal displacement osteotomy associated with an opening wedge cuboid osteotomy is able to improve radiological and clinical parameters of child patients with flexible flatfoot. RESUMO Objetivo: Descrever a técnica, analisar possíveis correções radiográficas e avaliar o resultado clínico da osteotomia de deslocamento medial e plantar do calcâneo associada à osteotomia em cunha de adição do cuboide para correção do pé plano flexível de crianças. Métodos: Foram avaliados retrospectivamente 23 pacientes (30 pés) com diagnóstico de pé plano flexível tratadas com osteotomia de deslocamento plantar e medial do calcâneo associada à osteotomia em cunha de adição do cuboide. Os parâmetros radiográficos avaliados nas imagens em perfil foram o pitch do calcâneo e o ângulo de Meary, enquanto nas radiografias anteroposteriores o ângulo de cobertura do tálus. Para avaliar o resultado clínico do procedimento cirúrgico, foi adotado o escore da American Orthopaedic Foot and Ankle Society (AOFAS) para tornozelo e retropé. Resultados: Os valores médios dos ângulos avaliados e do escore AOFAS para tornozelo e retropé melhoraram significativamente na comparação dos resultados pré e pós-operatórios. Conclusão: A osteotomia de deslocamento plantar e medial do calcâneo associada à osteotomia em cunha de adição do cuboide é capaz de melhorar os parâmetros radiológicos e clínicos de crianças com pé plano flexível. Nível de Evidência III, Estudo Comparativo Restrospectivo.

This work compares the anticorrosive properties of DC electrodeposited CuCoNi, CuCo, and CuNi alloy coatings in a NaCl 0.5 mol L-1 solution. The results showed that j and the bath composition influenced the chemical, morphological, and electrochemical characteristics of the alloy coatings. Among all the studied coatings, the CuCo, CuNi, and CuCoNi ones produced at j = 10 A m-2, j = 60 A m-2, and j = 25 A m-2, respectively, showed the best anticorrosive performances in the saline medium, exhibiting charge transfer resistances (Rtc) higher than 2000 Ω cm2 and an electrical double layer capacitances (Cdl) lower than 8 X10-4 F cm-2. These results agree with the compact morphologies and smaller grain sizes presented by these coatings. Therefore, the anticorrosive properties of the coatings cannot be related only to the less noble metal(s) contents in the alloy but also to their morphologies.

Abstract Co-development of knowledge is a useful tool for addressing the management of complex social-ecological systems like the ribeirinhos (riverine communities) of Brazil. Incorporating local knowledge and environmental perception of ribeirinhos into assessments of river degradation can lead to management outcomes that better support human and ecosystem health. In the state of Piauí, Brazil, riverine people depend on the Igaraçu River for various subsistence needs. However, river pollution has increased. The objective of this study was to comprehend the perceptions of ribeirinhos regarding the environmental degradation of the Igaraçu River in Piauí and how they believed it affected their life and ecosystem. We conducted interviews with 79 informants to gather insights on environmental degradation, its causes, and consequences. The results were analyzed using a Loyalty Level (LL) metric. The majority of respondents (67.86%, n=38) perceived that degradation was indeed taking place. They attributed the primary causes of degradation to discarded garbage (LL= 74.41) and the discharge of polluted water (LL= 11.60). Informants reported that local fauna and flora (LL= 35.70) were being impacted by water contamination. However, despite their reliance on the river for sustenance, including food, water, and the harvesting of forest products, they did not perceive any significant harm to their livelihoods. This study demonstrates that individuals who have a direct dependency on local natural resources can offer firsthand data regarding environmental changes. However, additional efforts are required to assist local communities (those who directly utilize the local environment) in developing a deeper understanding of how environmental degradation impacts both the present and future of their communities, as well as their connection to the local ecosystem.

ABSTRACT Objective: The aim of this randomized clinical trial was to evaluate and compare, during the first week of rapid maxillary expansion (RME), the impact caused by two types of appliances: Hyrax and Hybrid Hyrax. Methods: Forty-two patients who met the eligibility criteria (aged 11-14 years, with transverse maxillary deficiency, posterior crossbite, and presence of maxillary first premolars and first permanent molars) were selected and randomly divided into two groups: TBB GROUP (tooth-bone-borne expander), treated with Hybrid Hyrax (12 females and 9 males, mean age 13.3 ± 1.3 years), and TB GROUP (tooth-borne expander), treated with Hyrax (5 females and 16 males, mean age 13.3 ± 1.4 years). Pain and discomfort were assessed in two times: after the first day of activation (T1) and four days after, by means of the numerical rate scale and the instrument MFIQ (Mandibular Functional Impairment Questionnaire). Descriptive statistics and the Mann-Whitney test were used for comparison between groups and between sexes. A 5% significance level was adopted. Results: Both appliances had a negative impact, generating pain and discomfort, and reducing functional capacity. However, the scores obtained were of low intensity and no significant differences were observed between the groups. Considering sexes, there were statistically significant differences, with the female sex presenting higher scores for pain and functional limitation. Conclusions: Despite causing impact in pain and increase in the functional limitation, these changes were of low intensity, with no statistical difference between the groups. Females were more sensitive to the impact caused by the RME.

RESUMO Objetivo: O objetivo deste ensaio clínico randomizado foi avaliar e comparar, durante a primeira semana de expansão rápida da maxila (ERM), o impacto causado por dois tipos de aparelhos: Hyrax e Hyrax Híbrido. Métodos: Quarenta e dois pacientes que atendiam aos critérios de seleção (idade de 11 a 14 anos, com deficiência transversal da maxila, mordida cruzada posterior e presença de primeiros pré-molares e primeiros molares permanentes superiores) foram selecionados e divididos aleatoriamente em dois grupos: Grupo DOS (expansor dento-osseossuportado), tratado com Hyrax Híbrido (12 mulheres e 9 homens, idade média 13,3 ± 1,3 anos), e Grupo DS (expansor dentossuportado), tratado com Hyrax (5 mulheres e 16 homens, idade média de 13,3 ± 1,4 anos). A dor e o desconforto foram avaliados em dois momentos: após o primeiro dia de ativação (T1) e após quatro dias, por meio da escala de frequência numérica e do instrumento MFIQ (Questionário de Limitação Funcional Mandibular). A estatística descritiva e o teste de Mann-Whitney foram utilizados para comparação entre os grupos e entre os sexos. Adotou-se nível de significância de 5%. Resultados: Ambos os aparelhos tiveram impacto negativo, gerando dor e desconforto e reduzindo a capacidade funcional. No entanto, os escores obtidos foram de baixa intensidade e não foram observadas diferenças significativas entre os grupos. Considerando os sexos, houve diferenças estatisticamente significativas, com o sexo feminino apresentando maiores escores para dor e limitação funcional. Conclusões: Apesar de causar impacto na dor e aumento na limitação funcional, essas alterações foram de baixa intensidade, sem diferença estatística entre os grupos. As mulheres foram mais sensíveis ao impacto causado pela ERM.

ABSTRACT Objective: To identify factors associated with prolonged weaning and mortality in critically ill COVID-19 patients admitted to ICUs and under invasive mechanical ventilation. Methods: Between March of 2020 and July of 2021, we retrospectively recorded clinical and ventilatory characteristics of critically ill COVID-19 patients from the day of intubation to the outcome. We classified the patients regarding the weaning period in accordance with established criteria. A logistic regression analysis was performed to identify variables associated with prolonged weaning and mortality. Results: The study involved 303 patients, 100 of whom (33.0%) had a prolonged weaning period. Most of the patients were male (69.6%), 136 (44.8%) had more than 50% of pulmonary involvement on chest CT, and 93 (30.6%) had severe ARDS. Within the prolonged weaning group, 62% died within 60 days. Multivariate analysis revealed that lung involvement greater than 50% on CT and delay from intubation to the first separation attempt from mechanical ventilation were significantly associated with prolonged weaning, whereas age and prolonged weaning were significantly associated with mortality. Conclusions: Prolonged weaning can be used as a milestone in predicting mortality in critically ill COVID-19 patients. Lung involvement greater than 50% on CT and delay from intubation to the first separation attempt from mechanical ventilation were identified as significant predictors of prolonged weaning. These results might provide valuable information for healthcare professionals when making clinical decisions regarding the management of critically ill COVID-19 patients who are on mechanical ventilation.

RESUMO Objetivo: Identificar fatores associados ao desmame prolongado e à mortalidade em pacientes críticos com COVID-19 admitidos em UTI e sob ventilação mecânica invasiva. Métodos: Entre março de 2020 e julho de 2021, registramos retrospectivamente as características clínicas e ventilatórias de pacientes críticos com COVID-19 desde o dia da intubação até o desfecho. Os pacientes foram classificados quanto ao período de desmame de acordo com critérios estabelecidos. Foi realizada análise de regressão logística para identificar variáveis associadas ao desmame prolongado e à mortalidade. Resultados: O estudo incluiu 303 pacientes, 100 dos quais (33,0%) apresentaram período de desmame prolongado. A maioria dos pacientes era do sexo masculino (69,6%), 136 (44,8%) apresentaram mais de 50% de acometimento pulmonar na TC de tórax, e 93 (30,6%) apresentaram SDRA grave. No grupo desmame prolongado, 62% foram a óbito em 60 dias. A análise multivariada revelou que o acometimento pulmonar maior que 50% na TC e a demora na primeira tentativa de retirada da ventilação mecânica após a intubação apresentaram associação significativa com o desmame prolongado, enquanto a idade e o desmame prolongado apresentaram associação significativa com a mortalidade. Conclusões: O desmame prolongado pode ser utilizado como marco na predição de mortalidade em pacientes críticos com COVID-19. O acometimento pulmonar maior que 50% na TC e a demora na primeira tentativa de retirada da ventilação mecânica após a intubação foram identificados como preditores significativos de desmame prolongado. Esses resultados podem fornecer informações valiosas para os profissionais de saúde na tomada de decisões clínicas sobre o manejo de pacientes críticos com COVID-19 e em ventilação mecânica.
