In this work, the local electrochemical activity of the zones coupled by Friction Stir Welding (FSW) of an Al-Cu-Li alloy was studied and the results were correlated to the microstructural characteristics of each zone. Electrochemical studies were carried out in the zones affected by welding using cyclic voltammetry (CV) and scanning electrochemical techniques (namely, SECM - Scanning Electrochemical Microscopy and LEIS - Local electrochemical impedance spectroscopy). The results showed that the welding joint (WJ) is predominantly cathodic relatively to the heat affected zones (HAZ). The HAZ was always anodic and showed the highest electrochemical activities among the tested ones. The high electrochemical activity of the HAZ was associated with the effect of galvanic coupling between the cathodic region (WJ) and the anodic region (HAZ). In addition, the advancing side (AS) presented increased electrochemical activity compared to the retreating one (RS).

Insights on the evolution of Ululodini (Insecta: Neuroptera: Myrmeleontidae: Ascalaphinae), focusing on the systematics of the genus Ascalorphne Banks, 1915

ABSTRACT The tribe Ululodini is one of the most remarkable taxa of Neotropical Neuroptera, mainly due to its great flight ability, eyes divided into two lobes, and production of abortive eggs that act as a chemical barrier against predators. Nonetheless, Ululodini remains little studied. One genus of this tribe, Ascalorphne Banks 1915, currently comprises four valid species: A. macrocerca (Burmeister, 1839), A. impavida (Walker, 1853), A. umbrina (Gerstaecker, 1884), and A. leisewitzi (Navás, 1911). This study reviews the taxonomy of Ascalorphne, redescribing the external morphology and, for the first time, the male and female genitalia. Furthermore, A. leisewitzi is proposed as a new synonym of A. umbrina, an identification key is presented, and the geographic distribution of the genus is updated based on collected specimens and data available in the citizen science project iNaturalist. Also, the body and genital morphology of Ululodini were comparatively studied and analyzed in a phylogenetic context with all other Ascalaphinae tribes that occur in the Neotropics, allowing a better understanding of the evolution and biology of these insects. As a result, new synapomorphies for Ululodini are presented, Albardia furcata Oswald, 1903 is recovered as the sister taxon to the rest of the tribe, Ameropterus Esben-Petersen, 1922 was recovered as paraphyletic, and Ascalorphne was recovered as monophyletic in all analyses.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

ABSTRACT The complex taxonomy of Albardia furcata van der Weele is totally related to the unique characters of the adults, which hinder a morphological comparison with other owlfly genera. The behavior and life history of the immature stages of A. furcata are known, but the larval morphology remains undescribed. Herein, we present a morphological description of the A. furcata larva, the most iconic Neotropical owlfly (Myrmeleontidae: Ascalaphinae), a Brazilian species endemic to Cerrado, Caatinga, and Atlantic Forest biomes. The morphological description of the larva was based on three third instar larvae collected in areas of Cerrado and reared at laboratory conditions. We provide high-resolution images from the life cycle, including immature and adults, as well as an updated distributional map. Comments are done on the similarities and differences of the larva regarding other owlflies species and also on biogeography in Neotropical region.

Resumo Objetivo Verificar a associação entre os níveis de fator neurotrófico derivado do cérebro (brain-derived neurotrophic factor [BDNF]) e transtorno de estresse agudo (TEA) em pacientes que sofreram trauma físico. Métodos Os dados foram coletados em um hospital de emergência de Porto Alegre, Rio Grande do Sul, Brasil. Os participantes eram maiores de 18 anos, vítimas de trauma físico e estavam hospitalizados por um período mínimo de 48 horas. Um total de 117 pacientes hospitalizados que concordaram em participar da pesquisa foram agrupados de acordo com o turno de realização da coleta de sangue (38 sujeitos no turno da manhã e 79 sujeitos no turno da tarde), tiveram seus níveis de BDNF medidos e responderam a outros questionários. Os entrevistados também foram agrupados por idade em três faixas etárias: 18-30, 31-50 e 51-70 anos. Resultados Encontramos uma diferença significativa na distribuição de BDNF entre os turnos, sendo que o grupo da tarde apresentou níveis maiores de BDNF (U = 1906,5, p = 0,018). Houve diferença entre o grupo de 18-30 anos e o de 51-70 anos no turno da tarde (Umanhã = 1107, pmanhã = 0,575; Utarde = 7175, ptarde = 0,028). Conclusões A população cuja coleta ocorreu à tarde apresentou valores significativamente maiores de BDNF em relação à coleta do turno da manhã. Esta mesma população apresentou menores níveis dessa neurotrofina quando associada com os subtipos A1, A2 e A de TEA. É possível hipotetizar que os menores valores de BDNF aferidos na coleta do turno da manhã se devam a uma resposta ao ciclo circadiano do cortisol, cuja ação inibe a expressão de neurotrofinas séricas.
Abstract Objective To assess the association between brain-derived neurotrophic factor (BDNF) levels and acute stress disorder (ASD) in patients who have suffered physical trauma. Methods Data were collected at an emergency hospital in Porto Alegre, state of Rio Grande do Sul, southern Brazil. Participants were over 18 years of age, victims of physical trauma, and had been hospitalized for a minimum of 48 hours. A total of 117 hospitalized patients who agreed to participate in the research were grouped according to the shift in which blood was collected (38 subjects from the morning shift and 79 from the afternoon shift), had their BDNF levels measured and responded to other questionnaires. Respondents were further grouped by age into three ranges: 18-30, 31-50 and 51-70 years. Results We found a significant difference in the distribution of BDNF between the two shifts in which blood samples were collected, with the afternoon group having higher BDNF levels (U = 1906.5, p = 0.018). A difference was observed only between the 18-30 group and the 51-70 group in the afternoon shift (Umorning = 1107, pmorning = 0.575; Uafternoon = 7175, pafternoon = 0.028). Conclusions The population whose blood samples were collected in the afternoon showed significantly higher values of BDNF compared to those of the morning shift. This same population presented lower BDNF levels when associated with ASD subtypes A1, A2, and A. We hypothesize that the lower values of BDNF measured in the morning shift were due to a response to the circadian cycle of cortisol, whose action inhibits the expression of serum neurotrophins.

RESUMO Introdução: As infecções sexualmente transmissíveis (ISTs) ainda são um importante problema de saúde pública, e a vigilância é essencial para sua prevenção e seu controle. Objetivo: Avaliar a prevalência das ISTs em uma unidade de referência ginecológica no Rio de Janeiro, Brasil. Materiais e métodos: Trata-se de um estudo transversal realizado entre agosto de 2016 e junho de 2017. Foram coletados sangue total e células cervicais de 62 mulheres e realizados testes para detecção de papiloma vírus humano (HPV), vírus da imunodeficiência humana (HIV), sífilis, clamídia, gonorreia e vírus do herpes simples 1 e 2 (HSV-1/2). As lesões cervicais foram diagnosticadas por citopatologia, e em algumas pacientes, por colposcopia (79%). Outras ISTs foram avaliadas durante o exame clínico. Resultados: Foram encontradas lesões cervicais pelo exame citopatológico em 46,8% das pacientes; aquelas com história de quatro parceiros sexuais tiveram mais risco de desenvolvê-las. Houve concordância moderada entre os resultados da citopatologia e colposcopia (Kappa = 0,69). A prevalência de HSV (96,7%), sífilis (6,4%) e HIV (3,2%) foi maior que a descrita na literatura, enquanto a prevalência de clamídia (6,4%) e gonorreia (1,6%) foi similar. O HPV foi detectado em 53,2% das mulheres, sendo 32,3% delas infectadas pelo HPV 16. No contexto das coinfecções, 38 mulheres (61,3%) tinham mais de uma IST. Conclusão: Visto que a maioria das mulheres analisadas era acometida por mais de uma IST, nossos resultados sugerem que uma triagem rotineira dessas infecções nas unidades de saúde poderia auxiliar na detecção precoce, bem como no tratamento e na prevenção. Essas medidas também impactariam no controle do câncer cervical das pacientes.
ABSTRACT Introduction: Sexually transmitted infections (STI) remain a major public health problem and surveillance is crucial for prevention and control strategies. Objective: Our aim was to assess the prevalence of STI in a reference center for gynecology in Rio de Janeiro, Brazil. Materials and methods: It is a cross-sectional study conducted between August 2016 and June 2017. Whole blood and cervical cells were collected from 62 women and tested for human papillomavirus (HPV), human immunodeficiency virus (HIV), syphilis, chlamydia, gonorrhea and herpes simples virus 1 and 2 (HSV-1/2). Cervical lesions were diagnosed by cytopathology and in some patients by colposcopy (79%). Other STI were evaluated during clinical examination. Results: Cervical lesions were detected by cytopathology examinations in 46.8% of patients; those with a history of four sexual partners were at higher risk of developing them. There was moderate agreement between the cytopathology e colposcopy results (Kappa = 0.69). The prevalence of HSV (96.7%), syphilis (6.4%) and HIV (3.2%) were higher than that described in the literature, while the prevalence of chlamydia (6.4%) and gonorrhea (1.6%) were similar. HPV was detected in 53.2% of women, 32.3% of which were infected by HPV 16. In the context of co-infections, 38 women (61.3%) presented more than one STI. Conclusion: Since most of the women analyzed were affected by more than one STI, our results suggest that routine screening for these infections at health centers would help in early detection, treatment and prevention of these infections. These measures would also impact on patients’ cervical cancer control.

RESUMO Este estudo teve como objetivo caracterizar a estrutura populacional e os aspectos da regeneração de Araucaria angustifolia (Bertol.) Kuntze, Blepharocalyx salicifolius (Kunth) O. Berg e Ilex paraguariensis A. St.-Hil em diferentes condições ambientais e histórico de perturbação na Floresta Nacional de São Francisco de Paula - RS. Foram utilizados seis conglomerados de um hectare, com 16 parcelas (20 x 20 m) cada, nos quais foram amostradas as espécies em três classes de tamanho. A classe de tamanho II (CT II - diâmetro a altura do peito (DAP) ≥ 9,55 cm) foi inventariada nas 96 parcelas, a CT I (4,8 ≤ DAP < 9,55 cm) em 36 subparcelas de 10 x 10 m e a regeneração natural estabelecida (RNE) (1 ≤ DAP < 4,8 cm) em 36 células de 3,16 x 3,16 m. Em todas as parcelas de 20 x 20 m foram avaliados o banco de plântulas (BP - altura ≥ 30 cm e DAP < 1 cm), a chuva de sementes (CS) e o banco de sementes do solo (BS). Os dados foram analisados considerando todos os ambientes conjunta e separadamente para os agrupamentos identificados em estudo prévio na área. Foram avaliadas a estrutura diamétrica para a CT II e densidade e frequência absoluta para todas as etapas do ciclo de vida (CS, BS, BP, RNE, CT I e CT II). Os resultados indicaram grande variabilidade na densidade destas espécies nos diferentes ciclos de vida e ambientes florestais. Araucaria angustifolia demonstrou falhas no processo de regeneração em ambiente com menor histórico de perturbação, não apresentando padrão de distribuição "J invertido" (CT II) e menor capacidade de regeneração em relação às demais espécies nos mecanismos de regeneração (CS, BS e BP). Ilex paraguariensis e Blepharocalyx salicifolius apresentaram melhor estabilidade populacional em ambiente com menor nível de interferência antrópica (CT II). A primeira espécie apresentou melhores condições de regeneração em ambientes de solo com boa drenagem e a segunda espécie demonstrou preferência para solo mais úmido. Além disso, falhas no processo de regeneração das populações foram detectadas e variaram de acordo com as condições ambientais e o histórico de perturbação de cada área. Assim, identifica-se a importância de considerar os diferentes ambientes florestais, a autoecologia das espécies e de aplicar possíveis intervenções que visem melhorar a regeneração e equilibrar a dinâmica populacional destas espécies.
ABSTRACT The aim of this study was to characterize the population structure and aspects of regeneration of Araucaria angustifolia (Bertol.) Kuntze, Blepharocalyx salicifolius (Kunth) O. Berg and Ilex paraguariensis A. St.-Hil. in different environmental and disturbance conditions in the National Forest of São Francisco de Paula, RS state. Six conglomerates of one hectare were used, with 16 plots (20 x 20 m) each, where the species were sampled in three size classes. The size class II (SC II - diameter at breast height (DBH) ≥ 9.55 cm) was sampled in 96 plots, the SC I (4.8 ≤ DBH < 9.55 cm) in 36 subplots of 10 x 10 m, and the established natural regeneration (ENR) (1 ≤ DBH < 4.8 cm) in 36 cells of 3.16 x 3.16 m. The seedling bank (SB - height ≥ 30 cm and DBH < 1 cm), the seed rain (SR) and the soil seed bank (SSB) were sampled in all plots of 20 x 20 m. The data were analysed by taking into account all the different vegetation groups together and separately for each cluster identified in a previous study. For SC II, the frequency distribution of DBH was analysed combined with the analysis of absolute density and frequency for all life stages (SR, SSB, SB, ENR, SC I and SC II). The results indicated high variability in density of these species among the different life stages and environmental conditions. Araucaria angustifolia presented gaps in the regeneration process in areas that had low past disturbance, not showing the "reverse-J" distribution (SC II) and with lower regeneration capacity in relation to the other species (SR, SSB and SB). Ilex paraguariensis and Blepharocalyx salicifolius showed higher population stability in environment with low level of human interference (SC II). The first species demonstrated preference for soils with good drainage, while the second species for humid soils. Moreover, gaps in the regeneration process of different populations were detected, which changed according to environmental conditions and disturbance levels. Therefore, the results suggest the importance of taking into account the different vegetation groups, the species autecology and to apply possible interventions that aim to improve regeneration and balance the population dynamics of these species.

The aim of this study was to evaluate the antimicrobial activity of bio-oil (obtained by pyrolysis of biomass: soybean oil, eucalyptus sawdust and coffee grounds) added to the blend B10 (diesel and biodiesel). The bio-oil is compatible to diesel oil and it contains promising compounds that can exhibit antimicrobial activity during fuel storage. We evaluated the antimicrobial activity of bio-oil added to diesel oil B10 by determining the minimum inhibitory concentration at different concentrations (0-10%) using oil-deteriorating microorganisms for 10 days at 30 °C. Bio-oil was separated from the aqueous phase by solid phase extraction using ultrapure water and sodium hydroxide. The aqueous phase containing the solubilized compounds of bio-oil was characterized by comprehensive two-dimensional gas chromatography with time-of-flight mass spectrometric detection. Minimum inhibitory concentration in the range of 0.25% to 4% was observed for the tested inocula. The chromatographic analysis of both extracts allowed the identification of several oxygenated compounds, which the majority of the analytes consisted of phenolic compounds, followed by ketones.

A novel carbon composite was prepared from a mixture of coffee waste and clay with inorganic:organic ratio of 1.3 (CC-1.3). The mixture was pyrolysed at 700 °C. Considering the application of this adsorbent for removal of anionic dyes, the CC-1.3 was treated with a 6 mol L-1 HCl for 24 h to obtain ACC-1.3. Fourier transform infrared (FTIR), N2 adsorption/desorption curves, scanning electron microscope (SEM) and powder X-ray diffractometry (XRD) were used for characterisation of CC-1.3 and ACC-1.3 carbon adsorbents. The adsorbents were effectively utilised for removal of reactive blue 19 (RB-19) and reactive violet 5 (RV-5) textile dyes from aqueous solutions. The maximum amounts of RB-19 dye adsorbed at 25 °C are 63.59 (CC-1.3) and 110.6 mg g-1 (ACC-1.3), and 54.34 (CC-1.3) and 94.32 mg g-1 (ACC-1.3) for RV-5 dye. Four simulated dye-house effluents were used to test the application of the adsorbents for treatment of effluents.

A síndrome de Wolf-Hirschhorn (WHS) é uma condição genética caracterizada por anomalias craniofaciais e sistêmicas, causada por deleção cromossômica na região 4p. Paciente de 3 meses de idade, gênero feminino, foi encaminhada para avaliação de fissura de lábio e fissura palatina, associada a uma síndrome não diagnosticada. A paciente apresentava-se com hipotonia, baixa estatura, malformação cardíaca, clitóris hipertrofiado e implantação atípica do polegar nas duas mãos. Microcefalia, baixa implantação da orelha, glabela proeminente, inclinação baixa das fissuras palpebrais, aparência característica de capacete de guerreiro grego, micrognatia, fossetas em orelhas, fissura labial bilateral incompleta e fissura palatina incompleta foram observadas como características craniofaciais. Com um diagnóstico clínico de WHS, foi realizado o cariótipo, que mostrou a deleção 4p15.2, consistente com a condição. Esse relato de caso apresenta um caso de WHS, com uma fissura oral incomum, ampliando o espectro fenotípico da doença. A paciente foi encaminhada a tratamento com equipe multidisciplinar para correção cirúrgica da fissura labial e palatina. Encontra-se em acompanhamento médico bem como odontológico, fisioterapêutico e em terapia ocupacional e psicológica. Uma correlação entre genótipo e fenótipo pode ser observada nesse relato da síndrome de WHS.
Wolf-Hirschhorn syndrome (WHS) is a syndrome with craniofacial and systemic abnormalities, which is related to 4p deletion. A 3-month old girl with an undiagnosed syndrome was referred for evaluation of the cleft lip and palate. Hypotonia, short stature, cardiac malformation, hypertrophied clitoris, and atypical thumb of both hands was observed. Microcephaly, low-set ear, prominent glabella, downslanting palpebral fissures, a characteristic "Greek warrior helmet" appearance, micrognathia, ears with pits/tags and bilateral incomplete cleft lip apart from incomplete cleft palate were observed as craniofacial findings. With clinical diagnosis of WHS, blood was subjected to karyotyping, which showed a 4p15.2 deletion, consistent with the condition. Here is reported the case of this WHS patient with an uncommon oral cleft extending the phenotypic spectrum of the disorder. The child was referred to a multidisciplinary team to reparative surgery of the cleft lip and palate. The patient is on regular medical follow-up and will be further assisted by dentists, physical therapists, occupational therapists and psychologists. The genotype-phenotype correlation of the affected patient with previous WSH syndrome reports is described.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Applicability of Two International Risk Scores in Cardiac Surgery in a Reference Center in Brazil

Fundamento:A aplicabilidade de escores de risco (ER) internacionais em cirurgia cardíaca (CC) ainda não está bem definida em centros fora da América do Norte e Europa.Objetivo:Avaliar a capacidade do Berstein Parsonnet 2000 (BP) e do EuroSCORE (ES) em predizer mortalidade intra-hospitalar (MIH) em pacientes submetidos a CC em hospital de referência do Brasil e identificar os preditores de risco (PR).Métodos:Coorte retrospectiva de 1.065 pacientes. Foram incluídos dados de cirurgias realizadas entre janeiro de 2007 e julho de 2008, quando 60,3% dos pacientes realizaram CRM, 32,7%, cirurgia valvar, e 7,0%, CRM combinada com cirurgia valvar. Foram calculados os modelos aditivos e logísticos dos escores, a área sob a curva ROC (AUC) e a razão de mortalidade padronizada (RMP). Regressão logística multivariada foi realizada para identificar os PR.Resultados:A MIH da amostra foi de 7,8%. As características basais dos pacientes da amostra foram significativamente diferentes em relação ao BP e ao ES. As AUCs do BP aditivo e logístico foram 0,72 (IC95%; 0,66-0,78 p = 0,74) e as do ES foram 0,73 (IC95% 0,67-0,79 p = 0,80). O cálculo da RMP no BP foi 1,59 (IC95%; 1,27-1,99) e no ES foi 1,43 (IC95%; 1,14-1,79). Sete PR de MIH foram identificados: idade, creatinina sérica > 2,26 mg/dL, endocardite ativa, pressão arterial pulmonar > 60 mmHg, uma ou mais CC, CRM combinada com cirurgia valvar e diabetes melito.Conclusão:Ambos os ER mostraram-se inadequados na avaliação pré-operatória de pacientes submetidos a CC, reforçando a necessidade de se desenvolverem ER baseados na realidade local.
Background:The applicability of international risk scores in heart surgery (HS) is not well defined in centers outside of North America and Europe.Objective:To evaluate the capacity of the Parsonnet Bernstein 2000 (BP) and EuroSCORE (ES) in predicting in-hospital mortality (IHM) in patients undergoing HS at a reference hospital in Brazil and to identify risk predictors (RP).Methods:Retrospective cohort study of 1,065 patients, with 60.3% patients underwent coronary artery bypass grafting (CABG), 32.7%, valve surgery and 7.0%, CABG combined with valve surgery. Additive and logistic scores models, the area under the ROC (Receiver Operating Characteristic) curve (AUC) and the standardized mortality ratio (SMR) were calculated. Multivariate logistic regression was performed to identify the RP.Results:Overall mortality was 7.8%. The baseline characteristics of the patients were significantly different in relation to BP and ES. AUCs of the logistic and additive BP were 0.72 (95% CI, from 0.66 to 0.78 p = 0.74), and of ES they were 0.73 (95% CI; 0.67 to 0.79 p = 0.80). The calculation of the SMR in BP was 1.59 (95% CI; 1.27 to 1.99) and in ES, 1.43 (95% CI; 1.14 to 1.79). Seven RP of IHM were identified: age, serum creatinine > 2.26 mg/dL, active endocarditis, systolic pulmonary arterial pressure > 60 mmHg, one or more previous HS, CABG combined with valve surgery and diabetes mellitus.Conclusion:Local scores, based on the real situation of local populations, must be developed for better assessment of risk in cardiac surgery.
