RESUMO Objetivo Avaliar o estado nutricional e a frequência de consumo de alimentos inflamatórios e anti-inflamatórios por portadores de doenças inflamatórias intestinais. Metodologia Estudo transversal com indivíduos assistidos pelo ambulatório interdisciplinar de doenças inflamatórias intestinais da UNIVALI-SC. Avaliados por meio do estado nutricional e classificados de acordo com o índice de massa corporal, bem como através de um questionário contendo dados socioeconômicos, hábitos de vida e frequência do consumo de alimentos inflamatórios e anti-inflamatórios. Resultados Dos 65 pacientes, 57% eram portadores de doença de Crohn e 43% de retocolite ulcerativa. De acordo com a atividade da doença, 71% encontravam-se em remissão e 29% em atividade. Da amostra, 57% foram classificados como acima do peso. Não foi possível correlacionar estado nutricional e o tipo de doenças inflamatórias intestinais, estado nutricional e renda ou estado nutricional e escolaridade. Os alimentos inflamatórios mais consumidos foram carne de gado (65%) e café (60%), já os anti-inflamatórios foram alho (75%), azeite de oliva (54%) e batata doce (23%). Não houve associação entre os alimentos inflamatórios e anti-inflamatórios mais consumidos e o índice de massa corporal. Conclusão Segundo os resultados, observou-se que a maioria dos pacientes apresentava excesso de peso. Os alimentos inflamatórios mais consumidos foram carne de gado e café e os anti-inflamatórios foram alho, azeite de oliva e batata doce.

ABSTRACT Objective Evaluation of nutritional status and consumption frequency of inflammatory and anti-inflammatory food by patients with inflammatory bowel diseases. Methods An observational study of the patients assisted by the interdisciplinary inflammatory bowel diseases ambulatory of UNIVALI-SC. The nutritional status of patients was evaluated and each patient was categorized according to his/her body max index and also through a research questionnaire of the individual social-economy situation, life habits, and inflammatory and anti-inflammatory food consumption in a determinate period of time. Results Out of the 65 patients, 57% had Crohn’s disease and 43% had ulcerative colitis. According to the disease activity, 71% were in remission and 29% in activity. Of the sample, 57% were classified as overweight. It was not possible to correlate nutritional status and type of inflammatory bowel diseases, nutritional status and income or nutritional status and level of education. The most inflammatory foods were beef (65%) and coffee (60%), while the anti-inflammatory ones were garlic (75%), olive oil (54%), and sweet potatoes (23%). There was no association between the most consumed inflammatory and anti-inflammatory food and body max index. Conclusion According to the results, most of the patients were overweight. The most commonly consumed inflammatory foods were beef and coffee and the anti-inflammatory ones were garlic, olive oil, and sweet potatoes.

RESUMO CONTEXTO: Os pacientes com doenças inflamatórias intestinais (DII) vacinados para hepatite B possuem baixa taxa de sucesso em alcançar níveis protetores de anticorpos. Os principais fatores sugeridos para isso são a própria DII e o uso de medicamentos imunossupressores. OBJETIVO: Avaliar a titulação de anticorpos anti-HBs e verificar fatores associados a efetividade da vacinação contra hepatite B em pacientes com DII. MÉTODOS: Trata-se de um estudo prospectivo e consecutivo, de caráter observacional, descritivo e analítico, não-randomizado, qualiquantitativo, que avaliou a titulação de anticorpos anti-HBs em pacientes portadores de DII no Ambulatório Interdisciplinar de Doença Inflamatória Intestinal da Unidade de Saúde da Família e Comunitária da UNIVALI - Itajaí, Santa Catarina. RESULTADOS: Trinta e seis pacientes foram vacinados contra o vírus da hepatite B (VHB), destes, 29 eram do sexo feminino. A média de idade foi de 46,2 anos. Em relação ao tipo de DII, 24 pacientes eram portadores de doença de Crohn e o tempo médio de doença inflamatória intestinal encontrado foi de 74 meses. Quinze pacientes estavam em uso de terapia imunossupressora concomitante à vacinação. A taxa de resposta à vacina contra o VHB foi de 72,2%, verificada através de titulação de anti-HBs ≥10 UI/L. A análise estatística revelou uma resposta negativa à vacinação em pacientes em uso de medicamentos imunossupressores e portadores de doença de Crohn. CONCLUSÃO: A taxa de sucesso na imunização contra o VHB em pacientes com DII é baixo quando comparado à população em geral. Tipo de doença e uso de medicamentos imunossupressores parecem desempenhar influência na resposta vacinal.

ABSTRACT BACKGROUND: Patients with inflammatory bowel disease (IBD) vaccinated for hepatitis B have a low success rate in achieving protective antibody levels. The main factors suggested for this are IBD itself and the use of immunosuppressive drugs. OBJECTIVE: To evaluate the concentration of anti-HBs antibodies and to verify factors associated with the effectiveness of hepatitis B vaccination in patients with IBD. METHODS: This is a prospective, consecutive, observational, descriptive and analytical, non-randomized, qualitative study that evaluated the levels of anti-HBs antibodies in IBD patients at the Interdisciplinary Inflammatory Bowel Disease Clinic of the Family and Community Health Unit of UNIVALI - Itajaí, Santa Catarina. RESULTS: Thirty-six patients were vaccinated against hepatitis B virus (HBV), of which 29 were female. The average age was 46.2 years. Regarding the type of IBD, twenty-four patients had Crohn’s disease and the duration of inflammatory bowel disease was 74 months. Fifteen patients were on concomitant immunosuppressive therapy. The effective response rate to HBV vaccine was 72.2%, verified by anti-HBs titration ≥10 UI/L. Statistical analysis revealed a negative response to vaccination in patients with Crohn’s disease and immunosuppressive drugs. CONCLUSION: The success rate of HBV immunization in IBD patients is low compared to the general population. Type of disease and use of immunosuppressive drugs appear to influence the vaccine response.

RESUMO CONTEXTO: A doença inflamatória intestinal engloba entidades patológicas, sendo as principais a doença de Crohn e a retocolite ulcerativa. Ambas se caracterizam por inflamação crônica do intestino. Acomete jovens em idade ativa comprometendo a situação desses pacientes, principalmente a qualidade de vida, que sofre uma forte deterioração frente à condição clínica dos portadores, desde aspectos físicos como também sociais e emocionais. OBJETIVO: Avaliar a qualidade de vida dos pacientes atendidos no ambulatório multidisciplinar de referência para o tratamento de doenças inflamatórias intestinais, através de dados sociodemográficos e de questionários específicos sobre a doença, avaliando os sintomas intestinais e sistêmicos e os aspectos sociais e emocionais. Realizar um comparativo entre os questionários específicos utilizados para obtenção dos dados. MÉTODOS: Realizou-se um estudo de corte transversal em que foram avaliados pacientes com diagnóstico de doença inflamatória intestinal, acompanhados em um ambulatório de referência para o tratamento, entre maio de 2017 e dezembro de 2018. Os participantes responderam ao Protocolo Sociodemográfico e Clínico, ao questionário de qualidade de vida geral SF-36 e ao específico Inflammatory Bowel Disease Questionnaire, além da correlação realizada entre as duas escalas através da Correlação de Pearson (escala métrica), cujos dados foram analisados por meio da estatística descritiva e o nível de significância adotado foi de 5% (P≤0,05%). A amostra contou com 71 pacientes, sendo excluídos do trabalho gestantes ou nutrizes e menores de 18 anos. RESULTADOS: Setenta e um pacientes participaram do estudo, com média de idade de 46,5 anos e desvio padrão de ±13,8; 45 eram portadores de doença de Crohn e 26 possuíam o diagnóstico de retocolite ulcerativa; 73,2% eram mulheres; 64,8% casados; 8,4%, fumantes; 50,7% relataram fazer algum tipo de atividade física. Foi observada boa distribuição dos pacientes entre os domínios de cada questionário; não foram encontrados baixos escores para a qualidade de vida, sendo os sintomas sistêmicos e os aspectos emocionais aqueles com menor pontuação dentre os parâmetros do Inflammatory Bowel Disease Questionnaire; os aspectos físicos (40,6±44,4) e os emocionais (49.5±46.0) tiveram menor pontuação entre os domínios do SF-36. A correlação entre os dois questionários mostrou-se significativa. CONCLUSÃO: O perfil clínico dos pacientes seguiu as características de distribuição e prevalência dessas doenças. O impacto das doenças na qualidade de vida foi observado em diversos aspectos, principalmente os relacionados aos componentes psicológicos. O acompanhamento multidisciplinar, bem como suporte psicológico, social, nutricional e educacional devem ser considerados importantes determinantes para manter ou melhorar a qualidade de vida desses pacientes.

ABSTRACT BACKGROUND: Inflammatory bowel disease encompasses pathological entities, the main being Crohn’s disease and ulcerative rectocolitis. Both are characterized by chronic inflammation of the intestine. It affects young people of active age, compromising the situation of those patients, especially their quality of life, experiencing a strong deterioration in their clinical condition, from physical to social and emotional aspects. OBJECTIVE: Evaluate the quality of life of patients assisted in the multidisciplinary reference outpatient clinic for the treatment of inflammatory bowel diseases, through sociodemographic data and specific questionnaires on the disease, evaluating the intestinal and systemic symptoms and the social and emotional aspects. Make a comparison between the two scales used to obtain the data. METHODS: A cross-sectional study was carried out in which patients diagnosed with inflammatory bowel disease were evaluated and observed at a reference outpatient clinic for treatment from May 2017 through December 2018. The participants responded to the Socio-demographic and Clinical Protocol, the SF-36 general quality of life questionnaire and the specific Inflammatory Bowel Disease questionnaire, in addition the correlation between the two scales was performed using Pearson’s Correlation (metric scale), which data were analyzed by means of descriptive statistics and the significance level adopted was 5% (P≤0.05). The population studied consisted of 71 patients, excluding pregnant or nursing women and patients under 18 years of age. RESULTS: Seventy-one patients participated in the study, with an average age of 46.5 years and standard deviation of ±13.8; 45 patients had Crohn’s disease and 26 were diagnosed with ulcerative rectocolitis; 73.2% were women; 64.8% married; 8.4%, smokers; 50.7% reported practising some type of physical activity. A good distribution of patients was observed between the domains of each questionnaire; no low scores were found for quality of life, and systemic symptoms and emotional aspects were those with the lowest scores among the parameters of the Inflammatory Bowel Disease Questionnaire; physical (40.6±44.4) and emotional aspects (49.5±46.0) had lower scores among the Short Form-36 domains. The correlation between the two questionnaires proved to be significant. CONCLUSION: The clinical profile of the patients followed the characteristics of distribution and prevalence of these diseases. The impact of diseases on quality of life was observed in several aspects, especially those related to psychological components. Multidisciplinary follow-up, as well as psychological, social, nutritional and educational support should be considered important determinants to maintain or improve the quality of life of these patients.

OBJECTIVES: We aimed to evaluate the incidence of pancreatic alterations in Crohn's disease using endoscopic ultrasound (EUS) and to correlate the number of alterations with current clinical data. METHODS: Patients diagnosed with Crohn's disease (n=51) were examined using EUS, and 11 variables were analyzed. A control group consisted of patients with no history of pancreatic disease or Crohn's disease. Patients presenting with three or more alterations underwent magnetic resonance imaging (MRI). Pancreatic function was determined using a fecal elastase assay. RESULTS: Two of the 51 patients (3.9%) presented with four EUS alterations, 3 (5.9%) presented with three, 11 (21.5%) presented with two, and 13 (25.5%) presented with one; in the control group, only 16% presented with one EUS alteration (p<0.001). Parenchymal abnormalities accounted for 39 of the EUS findings, and ductal abnormalities accounted for 11. Pancreatic lesions were not detected by MRI. Low fecal elastase levels were observed in 4 patients, none of whom presented with significant pancreatic alterations after undergoing EUS. Ileal involvement was predictive of the number of EUS alterations. CONCLUSION: A higher incidence of pancreatic abnormalities was found in patients with Crohn's disease than in individuals in the control group. The majority of these abnormalities are related to parenchymal alterations. In this group of patients, future studies should be conducted to determine whether such morphological abnormalities could evolve to induce exocrine or endocrine pancreatic insufficiency and, if so, identify the risk factors and determine which patients should undergo EUS.

RESUMO Objetivo: Avaliar as concentrações séricas de vitamina D e sua relação com as doenças inflamatórias intestinais. Métodos: Estudo quantitativo e descritivo com indivíduos assistidos pelo ambulatório interdisciplinar de Doença Inflamatória Intestinal da Unidade de Saúde Familiar e Comunitária de Itajaí/SC no período de setembro 2015 até outubro 2016. Foram coletados dados socioeconômicos, hábitos de vida, exames bioquímicos e foram plicados os índices clínicos de classificação da atividade da doença: Índice de Harvey-Bradshaw (Doença de Crohn) e Escore Mayo Parcial (Retocolite Ulcerativa Inespecífica Crônica). Resultados: Dos 60 pacientes avaliados, 57% (n = 34) eram portadores de Doença de Crohn e 43% (n = 26) de Retocolite Ulcerativa Inespecífica Crônica. De acordo com a atividade da doença, 75% (n = 45) encontravam-se em fase de remissão, 13% (n = 8) em atividade leve e 9% (n = 5) em atividade moderada. Com relação a vitamina D, 63% (n = 38) apresentavam deficiência e 37% (n = 22) demonstravam suficiência. Associando as concentrações séricas de vitamina D e a atividade da doença, observou-se significância estatística entre as variáveis (p = 0,005). Com relação aos exames bioquímicos, a maioria dos pacientes com calprotectina fecal elevada apresentou deficiência de vitamina D (p = 0,025). Houve correlação estatisticamente significativa entre VHS e vitamina D (p = 0,0001). Conclusão: Segundo os achados do estudo, deficiência de vitamina D relaciona-se com atividade clínica e laboratorial das doenças inflamatórias intestinais.

ABSTRACT Objective: To evaluate the serum concentrations of vitamin D and their relation with inflammatory bowel diseases. Methods: This is a quantitative and descriptive study, with individuals assisted by the interdisciplinary ambulatory of Inflammatory Bowel Disease of the Family and Community Health Unit of Itajaí/SC from September 2015 to October 2016. Socioeconomic data, life habits, and biochemical tests were collected, with the use of clinical indexes of classification of the disease activity: Harvey-Bradshaw Index (Crohn's Disease) and Partial Mayo Score (Chronic Nonspecific Ulcerative Colitis). Results: Of the 60 patients evaluated, 57% (n = 34) had Crohn's Disease and 43% (n = 26) had Chronic Nonspecific Ulcerative Colitis. According to disease activity, 75% (n = 45) were in the remission phase, 13% (n = 8) had mild activity, and 9% (n = 5) had moderate activity. Regarding vitamin D, 63% (n = 38) had deficiency of this vitamin and 37% (n = 22) presented sufficiency. With the association of serum vitamin D concentrations and disease activity, we observed statistical significance among the variables (p = 0.005). Regarding biochemical exams, the majority of patients with fecal calprotectin elevation presented vitamin D deficiency (p = 0.025). Statistically significant correlation between HSV and vitamin D (p = 0.0001) was found. Conclusion: According to the findings of this study, vitamin D deficiency is related to the clinical and laboratory activity of inflammatory bowel diseases.

INTRODUÇÃO: Atualmente há uso crescente dos agentes biológicos no manejo da doença de Crohn (DC). Há escassez de dados referentes ao perfil epidemiológico dos usuários de infliximabe (IFX) e adalimumabe (ADA) para DC no Brasil. OBJETIVO: Identificar as características epidemiológicas dos pacientes com DC submetidos à terapia biológica. MÉTODO: Estudo retrospectivo, multicêntrico, com portadores de DC que utilizaram terapia biológica. Variáveis analisadas: gênero, idade ao início do tratamento, classificação de Montreal, doença perianal concomitante e tabagismo. RESULTADOS: Foram incluídos 175 pacientes, sem exposição prévia a biológicos, sendo 93 (53%) homens. A média de idade no início do tratamento biológico foi de 35,5 (2-79) anos. O tempo médio de doença ao início do tratamento foi de 46,9 (0-480) meses. Do total da amostra, 117 (66,9%) utilizaram IFX e 58 (33,1%) ADA. Classificação de Montreal: idade ao diagnóstico ― A1 (n=21; 12%), A2 (n=102; 58,3%) e A3 (n=52; 29,7%). Localização da DC ― L1 (n=42; 24%), L2 (n=51; 29,1%), L3 (n=81; 46,3%) e L4 (n=1; 0,6%). Forma de apresentação ― B1 (n=59; 33,7%), B2 (n=46; 26,3%) e B3 (n=70; 40%). Doença perianal foi encontrada em 89 (50,9%) dos pacientes. CONCLUSÕES: Os dados epidemiológicos dos pacientes foram compatíveis com os da literatura internacional. Houve uma alta prevalência de pacientes com a forma fistulizante da DC.

INTRODUCTION: There is currently an increasing use of biological agents in the management of Crohn's disease (CD). There is lack of data regarding the epidemiological profile of patients on infliximab (IFX) and adalimumab (ADA) for CD in Brazil. OBJECTIVE: To identify the epidemiological characteristics of patients with CD who underwent biological therapy. METHOD: Retrospective multicenter study, with CD patients on biological therapy. Analyzed variables: gender, age at treatment initiation, Montreal classification, concomitant perianal disease and smoking status. RESULTS: 175 patients without previous exposure to biological agents were included, 93 (53%) were male. The mean age at treatment initiation was 35.5 (2-79) years old an the mean disease duration was 46.9 (0-480) months. Overall, 117 (66.9%) patients used IFX and 58 (33.1%), ADA. Montreal classification: age at diagnosis ― A1 (n=21; 12%), A2 (n=102; 58.3%), and A3 (n=52; 29.7%). CD location ― L1 (n=42; 24%), L2 (n=51; 29.1%), L3 (n=81; 46.3%), and L4 (n=1, 0.6%). Phenotype ―B1 (n=59; 33.7%), B2 (n=46; 26.3%), and B3 (n=70; 40%). Perianal disease was found in 89 (50.9%) patients. CONCLUSIONS: The epidemiological profile of patients was similar to the literature. There was a high prevalence of patients with fistulizing CD.
