Exercise can prevent and improve the pathophysiology of diseases and promote healthy aging. Thus, understanding the mechanisms that regulate the beneficial effects of exercise may lead to the development of new strategies to enhance quality of life and to counteract chronic diseases. Voluntary wheel running is an interesting model to study the effects of exercise in mice. Compared to forced treadmill exercise, voluntary wheel running presents several advantages such as: 1) running pattern is similar to natural running behavior of mice; 2) it is performed under non-stressed conditions, according to the rhythmicity of the animal; 3) it does not require direct interference from the researcher, and can be easily applied in long-term studies. Mice run spontaneously when given access to running wheels, for a total distance of ∼4 to 20 km per day and a total activity time of ∼3 to 7 hours a day. Hence, voluntary wheel running can result in robust endurance-like adaptation in skeletal and cardiac muscles and protect from sarcopenia. However, due to the lack of control over exercise parameters in voluntary exercise models, it is important for the researcher to understand the patterns and variability of wheel running in mice, as well as the factors that can affect voluntary running activity. Overall, voluntary wheel running in mice is a very interesting approach to study the chronic adaptation to exercise, analyze the effects of exercise, and test exercise capacity in different experimental models.

The tumor lysis syndrome (TLS) is a life-threatening complication caused by the massive release of nucleic acids, potassium and phosphate into the blood. This complication is the result of tumor cell lysis, which may occur due to treatment of drug sensitive and is characterized by rapid capacity of proliferation, that is often hematological origin. Moreover, the TLS can be observed before starting the treatment due to spontaneous tumor cell death, and frequently worsens when chemotherapy is initiated. TLS has high mortality, so that its prevention continues to be the most important therapeutic measure. In the intensive care unit (ICU), physicians should be aware of the clinical characteristics of TLS, which results in severe electrolyte metabolism disorders, especially hyperkalemia, hyperphosphatemia and hypocalcemia, and acute kidney injury which is a major cause of ICU mortality. An adequate strategy for the management of the TLS, combining hydration, urate oxidase, and an early admission to ICU can control this complication in most patients. The aim of this review is to provide diagnostic tools that allow to the ICU physician to recognize the population at high risk for developing the TLS, and outline a proper strategy for treating and preventing this serious complication.

El síndrome de lisis tumoral (SLT) es una complicación potencialmente letal provocada por la liberación masiva de ácidos nucleicos, potasio y fosfato hacia la circulación sistémica. Este cuadro es el resultado de la lisis celular de neoplasias, con frecuencia hematológicas, las cuales se caracterizan por una rápida capacidad de proliferación y alta sensibilidad a fármacos. Por otra parte, el SLT se puede observar por muerte celular espontánea previo al inicio del tratamiento citoreductor, agravándose luego de iniciada la quimioterapia. El SLT presenta una alta mortalidad, por lo que su prevención continúa siendo la medida terapéutica más importante. En la unidad de cuidados intensivos los médicos deben conocer su cuadro clínico, el cual se caracteriza por la existencia de graves trastornos del metabolismo hidroelectrolítico, en particular hiperpotasemia, hiperfosfatemia e hipocalcemia y por la aparición de una lesión renal aguda. Una adecuada intervención terapéutica implica la rápida admisión a unidad de cuidados intensivos, hidratación intravenosa y aporte de la enzima urato-oxidasa como las medidas más importantes. El objetivo de la presente revisión es proporcionar herramientas diagnósticas y terapéuticas que le permiten al médico intensivista reconocer la población en riesgo de desarrollar este síndrome, así como establecer una adecuada estrategia terapéutica y profiláctica.

In a low deciduous forest in Yucatan, Mexico, we describe the architecture and analyzer the morphometry attributes of Caesalpinia gaumeri and Gymnopodium floribundum in three size classes and two soil types. Caesalpinia gaumeri is used in rural construction and G. floribundum in beekeeping. We identified and classified 24 trees of both species in three size classes: seedlings, saplings and poles, in soil luvisol and leptosol. The architecture of C. gaumeri corresponded to the model Troll and G. floribundum is Champagnat. Both species showed reiterations related of natural causes. We found statistics differences for morphometry attributes between size classes and soil types for both species. The tree poles of C. gaumeri were higher (Tukey, P < 0.05) in diameter at breast height, total height of crown and total height a first branch, lengths of branch of first order and internodes between size classes. The trees of this species growth in soil luvisol were higher (Tukey, P < 0.05) in diameter at breast height and insertion angle of branches of third order compared with trees growth in leptosol. The trees seedlings of G. floribundum were higher (Tukey, P < 0.05) in insertion angle of branches of first order compared with saplings and poles. The trees of this species growth in soil luvisol were higher (Tukey, P < 0.05) in insertion angle of branches of second and third order compared with trees growth in leptosol.

En una selva baja caducifolia de Yucatán, México, se describió la arquitectura y se analizó los atributos dasométrricos de Caesalpinia gaumeri y Gymnopodium floribundum en tres clases de tamaño y dos tipos de suelo. Caesalpinia gaumeri se utiliza en construcción rural y G. floribundum en apicultura. Se identificaron y clasificaron 24 individuos de ambas especies en tres clases de tamaño: brinzales, latizales y fustales y en suelos luvisol y leptosol. La arquitectura de C. gaumeri correspondió al modelo Troll, mientras que G. floribundum al Champagnat. En ambas especies, se observaron reiteraciones relacionadas a causas naturales. Se encontró diferencias estadísticas para los atributos dasométricos entre clases de tamaños y entre tipos de suelo para ambas especies. Los árboles fustales de C. gaumeri tuvieron mayores (Tukey, P < 0.05) diámetros a la altura del pecho, alturas totales de copa y a la primera rama, longitudes de rama de primer orden y de entrenudos entre clases de tamaño. Los individuos de esta especie que crecen en suelo luvisol cuentan con mayores (Tukey, P < 0.05) valores de diámetro a la altura del pecho y ángulo de inserción de ramas de tercer orden a diferencia de los ubicados en leptosol. Los árboles brinzales de G. floribundum tuvieron ángulos de inserción de ramas de primer orden mayores (Tukey, P < 0.05) al de los latizales y fustales. Los individuos de esta especie que crecen en suelo luvisol tuvieron mayores (Tukey, P < 0.05) ángulos de inserción de ramas de segundo y tercer orden en comparación con los que crecen en leptosol.

High dose intravenous selenium may be associated with a significant reduction in mortality among critically ill patients with systemic inflammation. Currently, parenteral selenium as sodium selenite seems to be a cornerstone of the antioxidant defence in the critically ill. So far, several clinical trials have evaluated the effects of selenium in monotherapy or as part of a multi-micronutrient approach, on relevant clinical end points for critically ill patients. Nonetheless, the results from these studies have sometimes been contradictory. We now have a better understanding of the pharmacokinetics of the initial and transient pro-oxidant effect of an intravenous bolus followed by the antioxidant effect of continuous infusion, which seems efficacious and safe among critically ill patients. Clinical confirmation of the potentially advantageous synergism between selenium and glutamine may soon be forthcoming but the most appropriate and the optimum time of supplementation remains undetermined. Short-term intravenous selenite (bolus injection plus continuous infusion) has shown to be safe and capable of optimizing serum selenium and antioxidant selenoenzymes activities. However, additional dose-ranging trials are necessary to elucidate an optimal and safe posology with confirmed pharmacokinetic profiles before more definitive phase III trials can be conducted.

Los antioxidantes y en particular el selenio parenteral parecen vincularse con una reducción significativa de la mortalidad en los pacientes críticos con Síndrome de Respuesta Inflamatoria Sistémica (SRIS). Actualmente, el selenio parenteral es considerado la piedra angular dentro de la estrategia de tratamiento antioxidante en los pacientes críticos. En los últimos años, diferentes estudios clínicos han evaluado el impacto clínico del uso de selenio intravenoso (selenito) aportado como monoterapia o asociado a otros micronutrientes antioxidantes ("cócteles antioxidantes"). Sin embargo, los resultados de estos estudios no han sido concluyentes. En la actualidad existe una mejor comprensión sobre la farmacocinética y farmacodinamia del selenio intravenoso, siendo reconocido el efecto pro-oxidante del bolo intravenoso inicial así como el efecto antioxidante de la infusión continua. Sin embargo, la dosis óptima y el tiempo de suplementación de selenio no han sido aun definidos planteándose la existencia de una posible sinergia entre selenio y glutamina parenteral. Esta revisión analiza la evidencia actual sobre la suplementación de selenio en pacientes críticos con SRIS. El uso de altas dosis de selenio por vía parenteral (bolo inicial e infusión continua) parece ser una estrategia segura y efectiva en optimizar los niveles séricos de selenio y la actividad de ciertas selenoenzimas antioxidantes tales como la Glutatión Peroxidasa y la Selenoproteína P. Sin embargo, son necesarios nuevos estudios sobre la posología y farmacocinética del selenio en pacientes críticos. Estas pesquisas deberían demostrar un beneficio definitivo del uso de selenio sobre end point clínicos de relevancia.

Objective: the aim of this study was to determine the prevalence of celiac disease among the adult population of Madrid by measuring antibodies against tissue transglutaminase as serologic screening method. Population and methods: 2,215 subjects participated voluntarily in this study. All of them completed a clinical questionnaire. We determined the levels of total IgA and antibodies to tissue transglutaminase (tTG). An intestinal biopsy by endoscopy was proposed to all subjects who were tTG-positive. The histologic lesion was classified in accordance to Marsh. Results:three known CD cases were identified by the questionnaire. Eleven donors with tTG positivity were detected, all of them asymptomatic. Four subjects rejected the intestinal biopsy. Seven out of 11 positive subjects consented to undergo a duodenal biopsy - 3 had villous atrophy and 4 had increased intraepithelial lymphocyte counts with normal villi. In our study the number of donors with biopsy-proven CD was 6, and the prevalence was 1/370. If we include the subcategories of gluten sensitive enteropathy (Marsh I), the prevalence would be 1/222. When we considered antibody positivity the prevalence of gluten sensitivity was 1 in 201, and it reached 1 in 158 when the three known CD cases were included. Conclusions: data on CD prevalence in this study confirm that CD is a first-line healthcare problem that may warrant universal screening. We detected a high number of lymphocytic enteritis cases, and thus some sort of action is mandatory.

Objetivo: conocer la prevalencia de enfermedad celiaca en la población adulta de la Comunidad de Madrid utilizando como método de despistaje los anticuerpos frente a la transglutaminasa tisular. Población y métodos: han participado de modo voluntario 2.215 donantes de sangre. Todos ellos rellenaron una encuesta. Se determinó la IgA sérica total y los anticuerpos antitransglutaminasa tisular. A los donantes con anticuerpos positivos, se les ofreció la realización de biopsia intestinal por endoscopia. La histología de la mucosa intestinal se graduó según los criterios de Marsh. Resultados: mediante la encuesta se identificaron tres celiacos diagnosticados previamente. Once donantes presentaban anticuerpos positivos, todos ellos asintomáticos. Cuatro rechazaron la biopsia intestinal. De los siete en los que se realizó, tres tenían atrofia vellositaria y cuatro infiltrado linfocitario con vellosidades normales. En nuestro estudio el número total de donantes con enfermedad celiaca confirmada por biopsia fue de seis, lo que supone una prevalencia del 1/370. Considerando el grado I de Marsh, la prevalencia de la enteropatía por gluten sería de 1/222. La respuesta inmunológica anómala al gluten medida por la positividad de los anticuerpos fue de 1/201 que alcanza valores de 1/158 si consideramos los tres celiacos diagnosticados previamente. Conclusiones: los datos de prevalencia hallados en este estudio confirman que la enfermedad celiaca constituye un problema sanitario de primer orden, que podría justificar la instauración de un programa de despistaje universal. Se ha detectado un alto número de casos de enteritis linfocitaria lo que obliga a plantearse la actitud a seguir.

In this article, we consider the relationship between productive and cultural aspects within a framework of rural development with gender perspective. The backdrop is the community of Hueyapan, Morelos, where the population speaks Nahuatl and preserves its customs and traditions. The hypothesis was that productive projects in which women participate are influenced by gender roles in their success or failure. The methodology used was based on a qualitative approach and the techniques were: literature review, participative observation, semi-structured interviews, life histories and workshops. The conclusions indicate that traditional gender stereotypes (in regards to division of labor, couple relationships and kinship relationships) influence women's participation in projects. A successful experience in participation positively influences the rearrangement of gender patterns, making them more flexible for women and men.

En el presente trabajo se reflexiona sobre la relación entre los aspectos productivos y culturales, dentro del marco del desarrollo rural con perspectiva de género. El escenario es la comunidad de Hueyapan, Morelos, su población habla nahuatl y conserva sus costumbres y tradiciones. La hipótesis planteada es que los proyectos productivos en que participan las mujeres son influenciados en su éxito o fracaso por los roles de género. La metodología empleada se basó en un enfoque cualitativo y las técnicas fueron: revisión bibliográfica, observación participante, entrevista semi estructurada, historia de vida y talleres. Las conclusiones indican que los estereotipos tradicionales de género (relativos a la división del trabajo, las relaciones de pareja y las relaciones de parentesco) influyen en la participación de las mujeres en los proyectos. La experiencia exitosa de participación influye positivamente en el reacomodo de los patrones de género haciéndolos más flexibles para mujeres y hombres.

This work presents a computational model for the viscous flow through rectilinear cascades of axial turbomachinery. The model is based on modifications of the classical Hess & Smith panel method. The viscous effect of the attached flow portion is introduced by means of normal transpiration velocities obtained from the boundary layer calculations on the airfoil contour. At the separated flow portion, fictitious velocities semi-empirical normal velocities are introduced assuming a constant pressure in the wake. When the separation is not detected, it is possible to simulate the effect of the small wake near the trailing edge by using an injected flow on a distance based on the Gostelow (1974) fairing-in procedure. The numerical model presents two iteration cycles: the first one to find the separation point, and the second one to accomplish the viscous-inviscid interaction, in which the transpiration velocities and the flow injection are submitted to a relaxation process in order to guarantee the convergence of the method. Results for the pressure distributions, flow turning angles and lift coefficients are compared with experimental data for the model validation.
