RESUMO Objetivo: Verificar a incidência do sinal do halo invertido (SHI) associado a infartos pulmonares (IP) relacionados ao tromboembolismo pulmonar (TEP) agudo, identificado por angiotomografia (angio-TC) de artérias pulmonares, e demonstrar as principais características morfológicas do SHI. Métodos: Foram avaliadas 993 angio-TCs, após estratificação de risco clínico para TEP entre janeiro de 2010 e dezembro de 2014. TEP foi encontrado em 164 pacientes (16,5%), sendo que três exames foram descartados devido a artefatos de movimentação respiratória. Dos 161 exames restantes, em 75 (46,6%) foram identificadas imagens compatíveis com IP, totalizando 86 lesões; o SHI foi observado em 33 (38,4% dos pacientes) dessas lesões. Resultados: Dos 29 pacientes com lesões características de IP com o SHI, 25 pacientes (86,2%) apresentavam lesão única e 4 (13,8%), lesão dupla. Todas as imagens compatíveis com SHI eram de localização subpleural. Para padronizar a análise, todas as imagens foram interpretadas no plano axial. Em relação à distribuição lobar das 33 lesões, o SHI estava localizado no lobo inferior direito, em 17 (51,5%); no lobo inferior esquerdo, em 10 (30,3%); na língula, em 5 (15,2%) e no lobo superior direito, em 1 (3,0%). Áreas de baixa atenuação no interior dos IPs com o SHI foram observadas em 29 das 33 lesões (87,9%). O SHI apresentava formato ovalado em 24 (72,7%) dos casos e formato arredondado, em 9 (27,3%). Derrame pleural foi encontrado associado aos IP com o SHI em 21 pacientes (72,4%). Conclusões: O achado de imagens com essas características, mesmo em pacientes com sintomatologia inespecífica, deve alertar para a possibilidade do diagnóstico de TEP.

ABSTRACT Objective: To determine the incidence of the reversed halo sign (RHS) in patients with pulmonary infarction (PI) due to acute pulmonary embolism (PE), detected by computed tomography angiography (CTA) of the pulmonary arteries, and to describe the main morphological features of the RHS. Methods: We evaluated 993 CTA scans, stratified by the risk of PE, performed between January of 2010 and December of 2014. Although PE was detected in 164 scans (16.5%), three of those scans were excluded because of respiratory motion artifacts. Of the remaining 161 scans, 75 (46.6%) showed lesions consistent with PI, totaling 86 lesions. Among those lesions, the RHS was seen in 33 (38.4%, in 29 patients). Results: Among the 29 patients with scans showing lesions characteristic of PI with the RHS, 25 (86.2%) had a single lesion and 4 (13.8%) had two, totaling 33 lesions. In all cases, the RHS was in a subpleural location. To standardize the analysis, all images were interpreted in the axial plane. Among those 33 lesions, the RHS was in the right lower lobe in 17 (51.5%), in the left lower lobe in 10 (30.3%), in the lingula in 5 (15.2%), and in the right upper lobe in 1 (3.0%). Among those same 33 lesions, areas of low attenuation were seen in 29 (87.9%). The RHS was oval in 24 (72.7%) of the cases and round in 9 (27.3%). Pleural effusion was seen in 21 (72.4%) of the 29 patients with PI and the RHS. Conclusions: A diagnosis of PE should be considered when there are findings such as those described here, even in patients with nonspecific clinical symptoms.

The product of human T-cell lymphotropic virus type 1 (HTLV-1) tax gene has a transactivating effect of the viral and cellular gene expression. Genetic variations in this gene have been correlated with differences in clinical outcomes. Based upon its diversity, two closely related substrains, namely tax A and tax B, have been described. The tax A substrain has been found at a higher frequency among human T-cell leukemia virus type 1 (TSP/HAM) patients than among healthy HTLV-I-infected asymptomatic subjects in Japan. In this study, we determined the distribution of tax substrains in HTLV-I-infected subjects in the city of São Paulo, Brazil. Using the ACCII restriction enzyme site, we detected only tax A substrain from 48 TSP/HAM patients and 28 healthy HTLV-I carriers. The sequenced tax genes from nine TSP/HAM patients and five asymptomatic HTLV-I carriers showed a similar pattern of mutation, which characterizes tax A. Our results indicate that HTLV-I tax subtypes have no significant influences on TSP/HAM disease progression. Furthermore, monophyletic introduction of HTLV-I to Brazil probably occurred during the African slave trade many years ago.

OBJETIVO: Avaliar a ocorrência e a correlação de tromboembolismo pulmonar (TEP) e trombose venosa profunda (TVP) por meio de um protocolo único de angiotomografia computadorizada. MATERIAIS E MÉTODOS: Estudo prospectivo realizado de julho de 2003 a junho de 2004 no Hospital Copa D'Or, Rio de Janeiro, RJ. Foram analisadas 116 angiotomografias de pacientes com suspeita clínica de TEP. Após o estudo do tórax, com um intervalo de três minutos e sem injeção adicional de contraste, foram obtidos cortes do diafragma até os joelhos, a fim de pesquisar TVP. RESULTADOS: De 116 pacientes, 23 (19,8%) cursaram com TEP, 24 com TVP (20,7%), 15 (12,9%) apresentaram tanto TEP quanto TVP e 9 (7,8%) apresentaram TVP na ausência de TEP. Dos 23 casos positivos de TEP, 15 apresentaram concomitantemente TVP (65,2%), enquanto em 8 (34,8%) foi visto apenas TEP. Dos 24 casos positivos de TVP, 15 (62,5%) apresentaram TEP associado, enquanto em 9 (37,5%) foi encontrada TVP isolada. CONCLUSÃO: Os resultados demonstraram a forte relação entre TEP e TVP, a importância de pesquisar TVP nos casos com suspeita de TEP e a utilidade do uso combinado da angiotomografia de tórax e da venotomografia como alternativa de único exame de investigação de TEP e TVP simultaneamente.

OBJECTIVE: To evaluate the occurrence and the correlation between pulmonary embolism (PE) and deep venous thrombosis (DVT) using a single CT angiography protocol. MATERIALS AND METHODS: This was a prospective study performed at Hospital Copa D'Or, Rio de Janeiro, Brazil from July 2003 to June 2004. We analyzed 116 CT angiography examinations of patients with clinical suspicion of PE. After an interval of approximately three minutes, venous phase images from the diaphragm to the knees were acquired without additional contrast injection in order to determine the presence of DVT. RESULTS: From the 116 patients studied, 23 (19.8%) had PE, 24 (20.7%) had DVT, 15 (12.9%) had both PE and DVT and 9 (7.8%) had DVT alone. Among the 23 cases of PE, 15 (65.2%) had concomitant DVT whereas 8 (34.8%) had only PE. Among the 24 cases of DVT, 15 (62.5%) had associated PE and 9 (37.5%) had DVT alone. CONCLUSION: The results showed a strong relationship between PE and DVT, as well as the importance of investigating DVT in cases with suspected PE and the benefits of performing indirect CT venography after pulmonary CT angiography as a single examination alternative method for simultaneous investigation of PE and DVT.

Atualmente existe no Brasil um interesse crescente na criação e exploração comercial da carne de javali e para atender a uma demanda diferenciada é importante que os atributos qualitativos do produto sejam bem estabelecidos. Com o objetivo de caracterizar a carne de javali nos parâmetros de qualidade e compará-la com a carne suína comercial, as mudanças nos músculos Longissimus dorsi e Semimembranosus, no post mortem, foram acompanhadas com medidas de pH, temperatura e cor (CIE L*a*b*). A capacidade de retenção de água (CRA) foi determinada pelo método de compressão e a perda de exsudato (PE) pelo teste de "drip loss". A queda de pH na carne de javali ocorreu de forma gradual, enquanto que no Longissimus dorsi de suíno a diminuição foi mais rápida e mais extensa. Diferenças de temperatura foram verificadas em alguns tempos post mortem, sendo que os menores valores foram encontrados nos javalis. A carne de javali teve valores menores de L* (luminosidade) e b* (intensidade de amarelo) e maiores de a* (intensidade de vermelho) que a carne suína. A CRA da carne dos javalis foi semelhante à da carne suína, mas a PE na carne de javali fêmea foi menor do que na de suíno.

Presently there is a growing interest in the production and marketing of wild boar meat, and to attend a differentiated consumer demand the quality attributes of this product should be well established. To characterize the quality of wild boar meat in comparison to commercial pork, post mortem changes in the longissimus dorsi and semimembranosus muscles were determined by pH and temperature decline, and color (CIE L*a*b*) measurements. Water holding capacity (WHC) was determined by the compression method and the exudate loss (EL) by the drip loss test. Decline in longissimus dorsi muscle pH of wild boar was gradual and in the pork it was faster and more extensive. Temperature differences were observed in some post mortem times, and the lowest values were found in wild boar carcasses. Wild boar meat presented lower values of L* (brightness) and b* (yellow color intensity), and higher values of a* (red color intensity) than pork. The WHC of the wild boar meat was similar to pork, but the EL in female wild boar meat was lower than in pork.
