The integration of supply chains has become a global operations strategy for many organisations because it allows them to improve customer service, minimise operating costs, and synchronise channels across the supply chain. This paper presents the design and implementation of a fuzzy expert system for the integration of collaborative supply chains. A measurement instrument that was statistically validated and formulated using a mathematical model was designed to implement the fuzzy expert system. This instrument was applied in 44 small-, medium-, and large-sized Mexican enterprises to determine their integration level. Specific actions were proposed, based on the results, to improve the attained integration level.

OBJETIVO: Analisar os efeitos da terapêutica adotada para o canal arterial (CA) em recém-nascidos (RN) < 1.000gadmitidos em unidades neonatais (UN) da Rede Brasileira de Pesquisas Neonatais (RBPN), sobre os desfechos: óbito, displasia broncopulmonar (DBP), hemorragia intraventricular grave (HIVIII/IV), retinopatia da prematuridade cirúrgica (ROPcir), enterocolite necrosante cirúrgica (ECNcir) e o desfecho combinado óbito e DBP. MÉTODOS: Estudo multicêntrico, de coorte, coleta de dados retrospectiva, incluindo RN de 16 UN da RBPN de 01/01/2010 a 31/12/2011, PN < 1.000 g, idade gestacional (IG) < 33 semanas e diagnóstico ecocardiográfico de PCA. Excluídos: óbitos ou transferências até o terceiro dia de vida, infecções congênitas ou malformações. Grupos:G1 - conservadora (sem intervenção medicamentosa ou cirúrgica), G2 - farmacológica (indometacina ou ibuprofeno) e G3 - cirúrgico (com ou sem tratamento farmacológico anterior). Analisou-se: uso de esteroide antenatal, parto cesárea, PN, IG, Apgar5' < 4, sexo masculino, SNAPPE II, síndrome do dDesconforto respiratório (SDR), sepse tardia, ventilação mecânica (VM), surfactante < 2 horas de vida, tempo de VM e os desfechos: óbito, dependência de oxigênio com 36 semanas (DBP36s), HIV III/IV, ROPcir, ECNcir e óbito/DBP36s. Estatística: Teste t-Student, Qui-Quadrado ou teste Exato de Fisher. Testes de Regressão Binária Logística e Regressão Múltipla Stepwise Backward. MedCalc (Medical Calculator) software, versão 12.1.4.0.p < 0,05. RESULTADOS: Foram selecionados 1.097 RN e 494 foram incluídos: G1-187 (37,8%), G2-205 (41,5%) e G3-102 (20,6%). Verificou-se: maior mortalidade (51,3%) no G1 e menor no G3(14,7%); maior frequência DBP36s (70,6%) e ROPcir (23,5%) no G3; maior frequência de óbito/DBP36s no G2 (58,0%). As abordagens farmacológica (OR-0,29; 95%, IC-0,14-0,62) e conservadora (OR-0,34; 95%, IC- 0,14-0,79) foram protetoras somente para o desfecho óbito/DBP36sem. CONCLUSÃO: Em RN com PCA, a abordagem conservadora relacionou-se à maior mortalidade, a cirúrgica à ocorrência de DBP36s e ROPcir., enquanto o tratamento farmacológico mostrou-se protetor para o desfecho óbito/DBP36sem.

OBJECTIVE: To analyze the effects of treatment approach on the outcomes of newborns (birth weight [BW] < 1,000 g) with patent ductus arteriosus (PDA), from the Brazilian Neonatal Research Network (BNRN) on: death, bronchopulmonary dysplasia (BPD), severe intraventricular hemorrhage (IVH III/IV), retinopathy of prematurity requiring surgical (ROPsur), necrotizing enterocolitis requiring surgery (NECsur), and death/BPD. METHODS: This was a multicentric cohort study, retrospective data collection, including newborns (BW < 1000 g) with gestational age (GA) < 33 weeks and echocardiographic diagnosis of PDA, from 16 neonatal units of the BNRN from January 1, 2010 to Dec 31, 2011. Newborns who died or were transferred until the third day of life, and those with presence of congenital malformation or infection were excluded. Groups: G1 - conservative approach (without treatment), G2 - pharmacologic (indomethacin or ibuprofen), G3 - surgical ligation (independent of previous treatment). Factors analyzed: antenatal corticosteroid, cesarean section, BW, GA, 5 min. Apgar score < 4, male gender, Score for Neonatal Acute Physiology Perinatal Extension (SNAPPE II), respiratory distress syndrome (RDS), late sepsis (LS), mechanical ventilation (MV), surfactant (< 2 h of life), and time of MV. Outcomes: death, O2 dependence at 36 weeks (BPD36wks), IVH III/IV, ROPsur, NECsur, and death/BPD36wks. Statistics: Student's t-test, chi-squared test, or Fisher's exact test; Odds ratio (95% CI); logistic binary regression and backward stepwise multiple regression. Software: MedCalc (Medical Calculator) software, version 12.1.4.0. p-values < 0.05 were considered statistically significant. RESULTS: 1,097 newborns were selected and 494 newborns were included: G1 - 187 (37.8%), G2 - 205 (41.5%), and G3 - 102 (20.6%). The highest mortality was observed in G1 (51.3%) and the lowest in G3 (14.7%). The highest frequencies of BPD36wks (70.6%) and ROPsur were observed in G3 (23.5%). The lowest occurrence of death/BPD36wks occurred in G2 (58.0%). Pharmacological (OR 0.29; 95% CI: 0.14-0.62) and conservative (OR 0.34; 95% CI: 0.14-0.79) treatments were protective for the outcome death/BPD36wks. CONCLUSION: The conservative approach of PDA was associated to high mortality, the surgical approach to the occurrence of BPD36wks and ROPsur, and the pharmacological treatment was protective for the outcome death/BPD36wks.

Foi avaliado o efeito da suplementação da glutamina em dietas contendo ingredientes de origem animal sobre o desempenho e a integridade intestinal de pintos de corte, criados até 21 dias de idade. Os tratamentos constituíram-se de combinações entre tipos de dieta, com ingredientes de origem vegetal ou vegetal+animal e suplementação de glutamina (0,0; 0,5; 1,0 e 2,0%). O delineamento foi em blocos ao acaso em arranjo fatorial 2x4, tipos de ração x percentagem de glutamina, com cinco repetições e 12 pintos por unidade experimental. Não houve efeito da interação tipo de dieta versus suplementação de glutamina sobre o desempenho, e os tratamentos não influenciaram o desempenho de pintos de corte. Houve efeito quadrático da suplementação de glutamina sobre o coeficiente de digestibilidade da proteína bruta. A suplementação com glutamina aumentou altura de vilos e profundidade de cripta no duodeno. A utilização de produtos de origem animal em dietas para pintos na fase inicial não prejudica o desempenho, e a inclusão de glutamina melhora a integridade intestinal.

The effect of glutamine supplementation in diets formulated with animal by-products on the performance and integrity of the small intestine of broiler chicks up to 21 days of age was evaluated. The treatments were the combination of types of diets (only with ingredients from a vegetal source or vegetal plus animal source) and levels of glutamine (0.0; 0.5; 1.0 and 2.0%). The experimental design was randomized blocks in a 2x4 factorial scheme (kinds of diets x levels of glutamine), with five replicates and 12 birds per experimental unit. No interaction between diets and glutamine supplementation and treatment effects on the performance traits were observed during the chick starter phase. There was a quadratic effect of glutamine on the crude protein digestibility coefficient. Birds supplemented with glutamine diets showed higher villus height and crypt depth in the duodenum. Broiler diets formulated with animal ingredients have no effect on chick performance during the initial phase, and glutamine supplementation improved the small intestine integrity.

It was evaluated the oocyte and embryo in vitro production of 42 Nellore mixed breed cows (Bos taurus indicus) come from de same genetic group, aging from 4 to 9 years, with mean body weight of 420 kg, stimulated with FSH and eCG. The animals were distributed into three groups: group 1, control (n= 14), only with follicular aspiration (OPU); group 2, treated with 1400 UI of eCG in a single dose plus OPU (n= 14); and group 3, treated with 120 UI of FSH administered four times plus OPU with 12 h of interval (n= 14). All groups received auricular implant with 3 mg of Norgestomet on the first day (D0) associated with 2 mg of estradiol benzoate intramuscularly. At the seventh day (D7) the implants were removed and the ovarian aspirations were realized on the cows of group 1(OPU). Group 2 had the eCG applied at the fifth day and the OPU was performed at D7. The animals from group 3 were treated with FSH at fifth and sixth day and the aspiration occurred at D7 (OPU). The follicular aspiration procedure was done via transvaginal guided with ultrasound. It was performed 42 aspirations obtaining 627 oocytes, where 502 were viable and 125 non-viable. Then, it was realized the in vitro maturation and fecundation. The cleavage, blastocyst and eclosion rate were evaluated. There were no significant difference (p>0.05) at these parameters between the groups. In conclusion, the gonadotrophic stimulation with FSH and eCG, at the doses used in this study, was insufficient to increase the quality of the viable oocytes submitted to in vitro fertilization to produce embryos.

Avaliou-se a recuperação de oócitos e produção de embriões in vitro de 42 vacas, mestiça Nelore proveniente de grupo genético homogêneo, com idade de 4 a 9 anos, com peso médio de 420 kg, estimuladas com FSH ou eCG. Estas foram distribuídas em três grupos: grupo 1, controle (n=14), apenas OPU, grupo 2, tratadas com 1400 UI de eCG, em dose única seguida de aspiração folicular (OPU) (n= 14); e, grupo 3, tratadas com 120 UI de FSH, administrados com intervalo de 12 horas em quatro doses seguida de OPU (n= 14). Todos os grupos receberam implante auricular contendo 3 mg de Norgestomet no primeiro dia (D0) associado a administração de 2 mg de benzoato de estradiol via intramuscular. No sétimo dia (D7) foram retirados todos os implantes e na sequência foram realizadas as aspirações ovarianas das vacas do grupo 1. O grupo 2 recebeu a aplicação de eCG no quinto dia e a OPU foi realizada no D7. Já o grupo 3 recebeu o tratamento com FSH no quinto e sexto dia, e foram aspiradas em D7. O procedimento de aspiração folicular foi realizado via transvaginal guiada por ultrassom. Foram realizadas 42 aspirações com obtenção de 627 oócitos, sendo 502 viáveis e 125 inviáveis. Realizou-se a maturação e fecundação in vitro. Avaliaram-se as taxas de clivagem, blastocisto e eclosão. Não houve diferenças significativas (p>0,05) nos parâmetros avaliados entre os grupos. Conclui-se que o estímulo ovariano com FSH ou com eCG nas doses utilizadas, foram insuficientes para incrementar o número e a qualidade de oócitos viáveis submetidos a fertilização in vitro para produção de embriões.

Foram isoladas 19 cepas monoclonais de 8 pacientes da unidade de terapia intensiva, resistentes aos carbapenêmicos. Todas as cepas apresentaram o gene blaOXA-51-like e por análise do perfil de proteínas de membrana notou-se ausência da fração de 22 kDa, sugerindo a combinação de dois mecanismos de resistência aos carbapenêmicos.

Nineteen clonally related imipenem-resistant Acinetobacter baumannii isolates were recovered from eight intensive care unit patients. All isolates harboured blaOXA-51-like β-lactamase genes and showed the absence of 22 kDa fraction in outer membrane porin profile analysis. It suggests a combination of two mechanisms as responsible for carbapenemresistant phenotypes.
