ABSTRACT. An experiment was conducted to evaluate the effect of diets with reduced energy level content, supplemented with carbohydrase, on broiler performance and the coefficient of metabolizability of nutrients. A total of 720 one-day-old male Cobb-500 chicks were distributed in a completely randomized design, with six treatments, eight repetitions of 15 birds each. The treatments were: (1) a positive control, basal diet to meet the requirements of broiler chickens (PC); (2) a negative control, basal diet with a reduction of 80 kcal kg-1 (NC); (3) NC + alphagalactosydase; (4) NC + xylanase; (5) NC + xylanase and alphagalactosydase, and (6) NC + enzymatic blend (alphagalactosydase, xylanase, pectinase and amylase). The nutrient digestibility was not improved by use of enzymes. At 7 days of age, the broilers which were fed diets supplemented with enzymes showed a lower feed intake (FI) and better feed conversion ratio (FCR) than the broilers fed on PC. Both the NC and enzymatic blend resulted in a worse performance of the broilers at 21, 35 and 42 days old. The use of alphagalactosydase and xylanase, isolated or in combination, in a corn-soybean meal-based diet is effective in improving the growth performance of broilers fed energy-reduced diets.

RESUMO As Diretrizes Brasileiras para Reabilitação do AVC são fruto de um esforço conjunto do Departamento Científico de Reabilitação Neurológica da Academia Brasileira de Neurologia com o objetivo de orientar os profissionais envolvidos no processo de reabilitação para a redução da incapacidade funcional e aumento da autonomia dos indivíduos. Membros do grupo acima participaram de fóruns de discussão na web com pré-temas, seguidos de reuniões por videoconferência em que as controvérsias foram discutidas, levando a um consenso. Essas diretrizes, divididas em duas partes, focam as implicações de recentes ensaios clínicos, revisões sistemáticas e metanálises sobre reabilitação do AVC. O objetivo principal é servir de orientação a médicos, fisioterapeutas, fonoaudiólogos, terapeutas ocupacionais, enfermeiros, nutricionistas e demais profissionais envolvidos no cuidado pós-AVC. As recomendações e níveis de evidência foram adaptados de acordo com a literatura disponível atualmente. Aqui é apresentada a Parte I sobre tópicos de reabilitação na fase aguda, prevenção e tratamento de doenças e comorbidades frequentes após o AVC.

ABSTRACT The Guidelines for Stroke Rehabilitation are the result of a joint effort by the Scientific Department of Neurological Rehabilitation of the Brazilian Academy of Neurology aiming to guide professionals involved in the rehabilitation process to reduce functional disability and increase individual autonomy. Members of the group participated in web discussion forums with predefined themes, followed by videoconference meetings in which issues were discussed, leading to a consensus. These guidelines, divided into two parts, focus on the implications of recent clinical trials, systematic reviews, and meta-analyses in stroke rehabilitation literature. The main objective was to guide physicians, physiotherapists, speech therapists, occupational therapists, nurses, nutritionists, and other professionals involved in post-stroke care. Recommendations and levels of evidence were adapted according to the currently available literature. Part I discusses topics on rehabilitation in the acute phase, as well as prevention and management of frequent conditions and comorbidities after stroke.

Resumo O objetivo deste trabalho foi determinar a solubilidade do pó da rocha biotita xisto por diferentes métodos, bem como a potencial toxicidade/acumulação dos seus metais no peixe dulciaquícola D anio re r io. A solubilidade foi testada em água ultrapura e em solução de CaCl2 (10 mmol L-1). Analisou-se a presença de metais por espectrometria de emissão óptica por plasma acoplado indutivamente. Os peixes foram separados em diferentes grupos e expostos ao pó da biotita xisto. A solubilidade dos elementos tóxicos na água não é letal, e, 30 dias após o término da exposição, não há acumulação de metais nos peixes.

Abstract The objective of this work was to determine the solubility of biotite schist rock powder by different methods, as well as the potential toxicity/accumulation of its metals in Danio rerio freshwater fish. Solubility was tested in ultrapure water and in CaCl2 solution (10 mmol L-1). Metal contents were analyzed by inductively coupled plasma optical emission spectroscopy. Fish were separated into different groups and then exposed to biotite schist powder. The solubility of toxic elements in water is not lethal, and, 30 days after the end of exposure, there is no accumulation of metals in the fish.

Resumo A flavomicina é um aditivo que pertence à classe dos não ionóforos, contudo, com poucos estudos realizados com bovinos confinados em fase de terminação. Diante disso, o objetivo do presente estudo foi avaliar a eficácia da flavomicina sobre o desempenho produtivo, comportamento ingestivo, características de carcaça e os parâmetros bioquímicos de novilhos terminados em confinamento. Foram avaliados 32 novilhos inteiros, ½ sangue Angus ½ sangue Nelore, provenientes do mesmo rebanho, com idade média de 11 meses ± 1,5 meses e peso corporal inicial de 337 kg ± 6 kg. O delineamento experimental foi o de blocos casualizados, composto por dois tratamentos e oito repetições, sendo: Dieta sem flavomicina (controle) e Dieta com flavomicina (0,5 g animal dia-1 do produto FLAVIMPEX®80). Não ocorrendo diferença entre os tratamentos, o consumo de matéria seca médio dos animais foi de 10,03 kg dia-1, eficiência alimentar de 0,158 kg, ganho médio diário de 1,593 kg dia-1, digestibilidade aparente da dieta de 61,69%. O uso da flavomicina não foi eficaz no desempenho animal, assim como não trouxe alterações no comportamento ingestivo e melhorias nas características de carcaça dos animais. A Proteína Plasmática Total, Aspartato amino-transferase e creatinina foram inferiores para os animais suplementados com flavomicina em relação ao grupo controle. Em relação ao período experimental houve redução nos índices de Proteína Plasmática Total, aumento na albumina, Gama-Glutamil Transferase e ureia dos bovinos, mas todos se mantiveram dentro dos valores de referência para a espécie.

Abstract Flavomycin is a non-ionophore additive little studied in finishing confined cattle. Therefore, this study aimed to evaluate the effectiveness of flavomycin on productive performance, ingestive behavior, carcass traits and biochemical parameters of steers finished in confinement. 32 whole steers, ½ Angus e ½ Nellore blood, from the same herd, with a mean age of 11 ± 1.5 months and initial body weight of 337 ± 6 kg were evaluated. The experiment was a randomized block design, consisting of two treatments and eight replications, as follows: Diet without flavomycin (control) and Diet with flavomycin (0.5 g FLAVIMPEX®80 animal day-1). There was no difference between treatments, the average dry matter intake of the animals was 10.03 kg day-1, feed efficiency was 0.158 kg, average daily gain was 1.593 kg day-1, apparent diet digestibility was 61.69%. The use of flavomycin was not effective in animal performance, and caused no changes in ingestive behavior and carcass traits of the animals. Total Plasma Protein, Aspartate amino-transferase and creatinine were lower for animals supplemented with flavomycin compared to the control group. In relation to the experimental period, there was a reduction in the levels of Total Plasma Protein, an increase in albumin, Gamma-Glutamyl Transferase and urea in cattle, but all remained within the reference range for the species.

Abstract The aim of this study was to evaluate the antimicrobial effect of the essential oils of cinnamon, cardamom, clove, oregano, and thyme and their synergism on vegetative cells and endospores of Clostridium perfringens type A inoculated in meat sausage (mortadella), as well as the influence of blends on the color, and lipid oxidation through the determination of thiobarbituric acid reactive substances (TBARS index). The anticlostridial action of the oil blends was established. The two added oil blends (Treat. 1: oregano, clove, and thyme; Treat. 2: oregano, clove, and cinnamon) in combination with reduced nitrite content (75 ppm) promoted a lower growth of C. perfringens in mortadella stored at 15 °C for 21 days in comparison to treatments containing only 75 ppm of nitrite. The essential oil blends showed antioxidant action and did not alter food color, thus possessing potential application as a preservative for the meat products industry.

Abstract The purpose of this study was to determine the effect of thermal vacuum processing and thermal processing without vacuum on the content of bioactive compounds and the antioxidant activity of low-calorie mixed Brazilian cerrado fruit jam. The mixed jam consisted of 60% of mixed pulp of Marolo, soursop and sweet passion fruit, and 40% other ingredients. This jam was utilized a completely randomized design to evaluate the antioxidant activity, total phenolic profile and content, carotenoid profile, and ascorbic acid content. The results showed that there was a reduction in bioactive compounds in the jam of 44.37% for total phenolics, 50.56% for ascorbic acid, 78.40% total carotenoids, and 65.14% for antioxidant activity when compared with the mixed pulps. In the carotenoid profile, reduction was observed for all compounds in relation to the mixed pulps, especially β-carotene (average loss of 81.89%). Independent of the processing, there was a significant increase in m-coumaric acid in the jam compared with the mixed pulp. The concentrations of other phenolic compounds decreased after processing. These results suggest that thermal vacuum processing is most suitable for the production of jam with higher nutritional value.

The population of small towns and riverine communities in Amazonia are extremely vulnerable to COVID-19 due to the absence of basic health care infrastructure in the region. The objective of this study was to (1) evaluate the effect of social distancing measures in the dissemination of COVID-19 in small towns, and (2) to estimate the impact of reducing contact between rural and urban populations on the contamination of riverine communities of Central Amazon. Our results indicate that social distancing can significantly reduce the speed of dissemination of COVID-19 in the population of small towns. However, we also observed that even in towns with intense actions to combat COVID-19, social distancing is below the ideal 70% isolation mark. Regarding the contamination of riverine communities, we observed that the reduction in the number of visits and duration of stay, as well as the interruption of visits during the five weeks with larger probability of contamination has major impact on delaying the arrival of COVID-19 in these vulnerable populations. We recommend that, given the low effectiveness of social distancing measures and the rapid contamination of urban populations, each municipality should evaluate implementing more restrictive measures such as a full or partial lockdown. Our results also suggest that three measures can be effective in delaying the arrival of COVID-19 in riverine communities of Amazonia: (1) the reduction in the number of visits that each community makes to a town, (2) the reduction in duration of each visit, and (3) avoiding visits during the five weeks with the largest number of infected people in towns. It is imperative that implementation of any of the restrictive measures suggested in this publication be accompanied by a vast communication campaign as well as social assistance actions for the rural population and poor families of small towns to guarantee their basic needs for survival.

As populações da Amazônia são extremamente vulneráveis à COVID-19 devido ao intenso fluxo entre as localidades rurais e as sedes municipais e à falta de infraestrutura de saúde básica adequada na região. O objetivo deste estudo foi (1) avaliar o efeito da medida de distanciamento social sobre a disseminação da COVID-19 em sedes municipais e (2) estimar o impacto da redução do contato entre a população rural e urbana sobre a contaminação de localidades rurais da Amazônia Central, região do Médio Solimões. Nossos resultados indicam que o distanciamento social pode reduzir significativamente a velocidade de contaminação da população em sedes municipais. No entanto, mesmo em sedes municipais com atividade intensa no combate à COVID-19, o distanciamento social está muito abaixo do ideal de 70%. Observamos também que a redução do número de visitas e do tempo de permanência durante cada visita, além da interrupção do fluxo durante as cinco semanas com maior probabilidade infecção têm impacto marcante sobre a velocidade de chegada da COVID-19 nas localidades rurais. Recomendamos que, dada a baixa efetividade de medidas de distanciamento social e o rápido contágio nas sedes municipais, cada município avalie a implementação de lockdown. Nossos resultados sugerem também que três medidas podem ser eficientes em atrasar a chegada da COVID-19 às localidades rurais: (1) a redução no número de visitas que cada uma faz a um centro urbano, (2) a redução do tempo de permanência durante cada visita e (3) evitar qualquer ida às sedes municipais durante as cinco semanas com maior número de pessoas infectadas. Destacamos que todas as medidas restritivas sugeridas precisam ser acompanhadas por uma ampla campanha de conscientização e ações de assistência à população rural e às famílias mais carentes das sedes municipais para garantir acesso a itens básicos para sua sobrevivência.

Abstract BACKGROUND: Although dermatophytes are considered the major cause of onychomycosis, many reports have incriminated non-dermatophyte moulds and yeasts in the disease’s etiology. Successive Trichosporon isolation from onychomycosis has led to the genus being suspected as a nail primary pathogen. OBJECTIVE: To determine the prevalence of Trichosporon isolation in onychomycosis patients who attended a mycology diagnostic service in Rio de Janeiro, Brazil, between January 2003 and December 2006. The study also includes a worldwide review on Trichosporon isolation prevalence in ungueal disease, emphasizing T. ovoides. METHODS: This retrospective study was conducted with the support of staff from the Mycology Laboratory at the Dermatological Service of Rio de Janeiro’s Santa Casa da Misericórdia (MLDS). RESULTS: Mycological analysis provided positive results equaling 47/5036 (0.93%) for Trichosporon spp.; obtained mainly as a single agent (72.35%), and from mixed cultures (27.65%; X2= 6.397; p= 0.018). The great majority belongs to the T. ovoides species (91.5%; n=43), obtained as a single isolate (74.41%; n= 32/43; X2 = 7.023; p= 0.014). CONCLUSIONS: Although T. ovoides is classically associated as an etiologic agent of white piedra, this study highlights its potential as a human nail disease pathogen. Our study opens doors for future epidemiologic and virulence factors aimed at determining whether T. ovoides is an important causative agent of onychomycosis in Brazil.

O objetivo deste trabalho foi realizar um estudo bacteriológico comparativo entre os sistemas de umidificação aquoso aquecido (UAA) e filtro trocador de calor e umidade (FTCU) quanto à colonização bacteriana e a incidência de infecção respiratória em pacientes submetidos à ventilação mecânica (VM). Trata-se de uma pesquisa prospectiva, controlada e randomizada, na qual 15 pacientes internados na Unidade de Terapia Intensiva (UTI) foram distribuídos em dois grupos. O primeiro fez uso de UAA (n=7) e o outro de FTCU (n=8). Foram coletadas amostras da secreção traqueal, condensado do circuito e FTCU na admissão do paciente, no quarto e oitavo dias, e realizada análise bacteriológica dos mesmos. Quanto às características antropométricas, não observou-se diferenças entre os grupos estudados. A prevalência de pneumonia associada à ventilação (PAV) foi de 57,1% no UAA e 62,5% no FTCU. Ao realizar a análise bacteriológica quantitativa entre eles, não foram observadas variações, sugerindo não haver diferença na prevenção de PAV entre os sistemas de umidificação; porém a presença das mesmas bactérias na secreção traqueal e no condensado e ausência destas na membrana do FTCU podem indicar que a principal fonte de contaminação é o próprio paciente.

The aim of this study was to conduct a bacteriological research comparing the aqueous heated humidification systems (HH) and filter heat and moisture exchanger (FHME) and to bacterial colonization and the incidence of respiratory infection in patients undergoing mechanical ventilation. It is a prospective, controlled trial, in that 15 intensive care unit (ICU) patients were divided into two groups. The first made use of HH (n=7) and the other, FHME (n=8). We collected samples of tracheal secretions, and condensate circuit FHME at admission in the fourth and eighth day and bacteriological analysis of the same place. Regarding the anthropometric characteristics, no differences were observed between the groups. The prevalence of ventilator associated pneumonia (VAP) was 57.1% in the HH and 62.5% in FHME. When performing quantitative bacteriological analysis between the group and HH and FHME, differences were not observed, suggesting no variation in the prevention of VAP between the humidification systems, but the presence of these bacteria in the tracheal and condensate and in the absence of these membrane FHME may indicate that the main source of contamination is the patient himself.

OBJETIVO: Caracterizar o comportamento visual e perfil socioeconômico e demográfico de recém-nascidos prematuros no primeiro mês de idade cronológica. MÉTODOS: Estudo descritivo de corte transversal, aninhado a um estudo longitudinal intitulado: "Comparação do comportamento visual no primeiro trimestre de vida de lactentes nascidos pré-termo em duas maternidades da cidade do Recife/PE". A amostra foi composta por 52 recém-nascidos prematuros recrutados de junho de 2007 a junho de 2008, na Maternidade do Hospital das Clínicas da Universidade Federal de Pernambuco. Foram coletados dados biológicos, socioeconômicos e demográficos dos recém-nascidos e famílias, por meio de consulta aos prontuários e entrevista com as genitoras. Para caracterizar o comportamento visual dos recém-nascidos utilizou-se o Método de Avaliação do Comportamento Visual de Lactentes. RESULTADOS: Observou-se que a maioria dos recém-nascidos era do sexo masculino, com idade gestacional entre 33 semanas e 36 semanas e 6 dias, apresentaram comportamento visuomotor adequado para a faixa etária pesquisada e que a maioria das famílias apresentava boas condições socioeconômicas e demográficas. Ainda foi possível detectar em 19% da amostra sinais oculares, os quais foram encaminhados para um Serviço de Oftalmologia. CONCLUSÕES: Os resultados apontam o método como um instrumento importante na triagem visual, possibilitando a detecção oportuna de alterações visuais em lactentes prematuros desde o 1º mês de vida. O olhar clínico da avaliação terapêutica ocupacional de lactentes deve voltar-se para a observação do comportamento considerando não apenas os riscos biológicos, mas também a influência que os fatores ambientais e sociais exercem sobre o desempenho funcional da criança.

OBJECTIVE: To characterize the behavior of premature newborns in the first year of chronological age. METHODS: This is a cross-sectional descriptive study, bound to a longitudinal study titled: "Comparison of visual behavior on the first quarter of year of life of premature nursling born at two maternities of Recife/PE." The sample was composed by 52 premature newborns selected from June, 2007 to June, 2008 from the Maternity of the Federal University of Pernambuco (UFPE). Biological, socioeconomic and demographic data was collected through medical records and interviews with progeny. Newborns were evaluated by the Assessment Guide of Visual Ability in Infants. RESULTS: Most of the newborns were male at a gestational period between 33 weeks and 36 weeks and 6 days, showed a good visual behavior development for the age researched, and most of the families showed good socioeconomical and demographic profile. Besides, it was possible to detect ocular signs in 19% of sample, that were referred to an Ophthalmology Service. CONCLUSION: This study results point out the method like an important key in the early detection and visual screening for premature nursling since the first month of life and it led us to believe that clinical view for occupational therapy intervention must be focused not only on biological risks but also at the influence environment in newborn performance.

Cross-sectional quantitative study aiming at evaluating foot care by people with Diabetes Mellitus and the changes in their lower limbs, in primary health care service. Fifty-one patients with type 2 Diabetes Mellitus took part on the research done in a community service center in the backlands of the state of São Paulo, Brazil. Data were collected from medical records, structured interviews and foot exams. Care performed was found to be restricted to hygiene and appropriate footwear. Most frequent changes in feet were as follows: dry skin, interdigital mycosis, cracks, callosities, feet deformity and toes superposition. Most patients had never undergone foot exams since the diagnosis. The systematic evaluation of foot can detect possible problems and help with awareness and development of self care abilities in preventing diabetic foot.

Estudio cuantitativo transversal que tuvo como objetivo evaluar los cuidados con los pies adoptados por personas con Diabetes Mellitus y las alteraciones en sus miembros inferiores, en un servicio de atención primaria a la salud. Participaron 51 personas con Diabetes Mellitus tipo 2 atendidas en un centro de investigación y extensión universitaria del interior paulista-Brasil, en 2008. Los datos fueron obtenidos mediante consulta a la ficha de registro, entrevista dirigida y examen de los pies. Los cuidados adoptados fueron restringidos a la higiene y al uso de calzados adecuados. Las alteraciones en los pies más frecuentes fueron: piel seca, micosis interdigital, fisuras, callosidades, deformidades en los pies y superposición de los dedos. La mayoría nunca había realizado el examen de los pies desde el diagnóstico. La evaluación sistemática de los cuidados con los pies, además de detectar posibles problemas, posibilita sensibilizar las personas para el desarrollo de habilidades de autocuidado en la prevención del pie diabético.

Estudo quantitativo transversal que teve como objetivo avaliar os cuidados com os pés adotados por pessoas com Diabetes Mellitus e as alterações em seus membros inferiores, em um serviço de atenção primária à saúde. Participaram 51 pessoas com Diabetes Mellitus tipo 2 atendidas em um centro de pesquisa e extensão universitária do interior paulista, em 2008. Os dados foram obtidos mediante consulta à ficha de cadastro, entrevista dirigida e exame dos pés. Os cuidados adotados restringiram-se à higiene e ao uso de calçados adequados. As alterações nos pés mais frequentes foram: pele seca, micose interdigital, rachaduras, calosidades, deformidades nos pés e sobreposição dos dedos. A maioria dos participantes nunca havia realizado o exame dos pés desde o diagnóstico. A avaliação sistemática dos cuidados com os pés, além de detectar possíveis problemas, possibilita sensibilizar as pessoas para o desenvolvimento de habilidades de autocuidado na prevenção do pé diabético.

INTRODUÇÃO: A cirurgia cardíaca com circulação extracorpórea aumenta a permeabilidade vascular, com incremento da morbidade e da mortalidade pós-operatória. A ultrafiltração modificada na população pediátrica demonstrou melhora da função pulmonar e hemodinâmica, contudo benefício semelhante não está bem estabelecido em adultos. Nós temos a hipótese que a ultrafiltração modificada pode melhorar a função pulmonar, hemodinâmica e a coagulação no pós-operatório em pacientes adultos. MÉTODOS: Estudo prospectivo e cego para a equipe anestésica e da terapia intensiva em pacientes eletivos submetidos à revascularização do miocárdio. Todos os pacientes foram monitorados quanto à função hemodinâmica, pulmonar e hematológica no intraoperatório e até 48 horas de pós-operatório. Os pacientes foram divididos em dois grupos: um submetido à ultrafiltração modificada por 15 minutos após a saída de circulação extracorpórea e um grupo sem ser submetido à ultrafiltração. Os dados foram estudados com análise de variância com dois fatores para medidas repetidas. RESULTADOS: O grupo ultrafiltração modificada apresentou menor sangramento pós-operatório ao final de 48 horas (598 ± 123 ml vs. 848 ± 455 ml; P = 0,04) e menor necessidade de transfusão de unidades de hemácias (0,6 ± 0,6 unidades/ paciente vs. 1,6 ± 1,1 unidades/paciente; P =0,03). O grupo ultrafiltração apresentou menor resistência de vias aéreas quando comparado ao controle (9,3 ± 0,4 vs. 12,1 ± 0,8 cmH2O. L-1s-1; P =0,04) e menor complacência quando comparado ao controle (47,3 ± 2,0 mLcmH2O vs. 53,1 ± 3,1 mLcmH2O; P=0,04). CONCLUSÃO: O uso ultrafiltração modificada diminuiu o sangramento pós-operatório e a necessidade de transfusão, contudo sem diferenças no resultado clínico final. O uso da ultrafiltração modificada não foi associado com instabilidade hemodinâmica.

OBJECTIVE: The inflammatory response after cardiac surgery increases vascular permeability leading to higher mortality and morbidity in the post operative time. The modified ultrafiltration (MUF) had shown benefits on respiratory, and hemodynamic in pediatric patients. This approach in adults is not well established yet. We hypothesize that modified ultrafiltration may improve respiratory, hemodynamic and coagulation function in adults after cardiac surgeries. METHODS: A prospective randomized study was carried out with 37 patients who underwent coronary artery bypass graft surgery (CABG) were randomized either to MUF (n=20) at the end of bypass or to control (no MUF) (n=17). The anesthesia and ICU team were blinded for the group selection. The MUF were carried out for 15 minutes after the end of bypass. The patients data were taken at beginning of anesthesia, ending of bypass, ending MUF, 24 hours, and 48 hours after surgery. For clinical outcome the pulmonary, hemodynamic and coagulation function were evaluated. RESULTS: We observed lower drain loss in the MUF group compared to control group after 48 hours (598 ± 123 ml vs. 848 ± 455 ml; P=0.04) and required less red blood cells units transfusion compared to control group (0.6 ± 0.6 units/patient vs.1.6 ± 1.1 units/patient; P=0.03). The MUF group showed lower airway resistance (9.3 ± 0.4 cmH2O.L-1s-1 vs. 12.1 ± 0.8 cmH2O.L-1s-1; P=0.04). There were no deaths in both groups. CONCLUSION: The MUF reduces post operatory bleeding and red blood cells units transfusion, but with no differences on clinical outcome were observed. The routinely MUF employment was not associated with hemodynamic instability.

OBJETIVO: Investigar a eficácia da ultrafiltração na remoção de mediadores inflamatórios liberados pela circulação extracorpórea e correlacionar ultrafiltração com alterações da função orgânica de acordo com o "Sequencial Organ Failure Assessment Score". MÉTODOS: Quarenta pacientes foram incluídos e randomizados em dois grupos: "sem ultrafiltração" (n=20; Grupo I) e "ultrafiltração" (n=20; Grupo II). Complementos 3 e 4 ativados, interleucina 1beta, 6, 8 e fator de necrose tumoral alfa foram dosados antes da indução anestésica (T1), 5 minutos antes da circulação extracorpórea (T2), no líquido ultrafiltrado (T3), 30 minutos (T4), 6 (T5), 12 (T6), 24 (T7), 36 (T8) e 48 (T9) horas após término da circulação extracorpórea. "Sequencial Organ Failure Assessment Score" foi avaliado nos tempos 1, 6 e 9. Significância estatística foi estabelecida com p < 0,05. RESULTADOS: No líquido ultrafiltrado, apenas níveis de fator de necrose tumoral alfa foram detectados. Níveis de complemento 3 ativado, nos tempos 5 e 7, e complemento 4 ativado, nos tempos 5 e 6, foram significativamente elevados no grupo sem ultrafiltração, e níveis de interleucina 6 foram elevados no grupo ultrafiltrado, nos tempos 7 e 8. Interleucina 1beta, 8, fator de necrose tumoral alfa, e "Sequencial Organ Failure Assessment Score" não tiveram diferenças significantes entre os grupos. CONCLUSÕES: Ultrafiltração filtra significativamente fator de necrose tumoral alfa, mas isto não influencia nos níveis séricos desta citocina. Ultrafiltração com o tipo de filtro usado neste estudo não tem efeito na disfunção orgânica e deverá ser usada apenas para controle volêmico nos pacientes submetidos à circulação extracorpórea.

OBJECTIVE: To investigate the effectiveness of ultrafiltration in removing inflammatory mediators released by cardiopulmonary bypass and to correlate ultrafiltration with alterations in organic function according to the Sequential Organ Failure Assessment Score. METHODS: Forty patients were included and randomized into two groups: "no ultrafiltration" (n=20; Group I) and "ultrafiltration" (n=20; Group II). Activated complement 3 and 4, interleukins 1beta, 6, 8 and tumor necrosis factor alfa were measured prior to anesthesia induction (Time 1), 5 minutes before cardiopulmonary bypass (Time 2), in the ultrafiltrated fluid (Time 3), 30 minutes (Time 4), and 6 (Time 5), 12 (Time 6), 24 (Time 7), 36 (Time 8) and 48 (Time 9) hours following cardiopulmonary bypass. Sequential Organ Failure Assessment Score was evaluated at Time 1, 6 and 9. Statistical significance was established at p < 0.05. RESULTS: In the ultrafiltrated fluid, only tumor necrosis factor alfa levels were detected. Levels of activated complement 3 at Times 5 and 7 and activated complement 4 at Times 5 and 6 were significantly higher in the unfiltered Group, and levels of interleukin 6 were higher in the filtered Group at Times 7 and 8. Interleukins 1beta, 8, tumor necrosis factor alfa, and the Sequential Organ Failure Assessment score were not significantly different between the groups. CONCLUSIONS: Ultrafiltration significantly filtered tumor necrosis factor alfa but did not influences serum levels of this cytokine. Ultrafiltration with the type of filter used in this study had no effect in organic dysfunction and should be used only for volemic control in patients undergo cardiopulmonary bypass.
