Introduction: Surgical procedures were systematized and scientifically based at the end of the 19th century by William Stewart Halsted. Through scientific studies, anatomical principles, knowledge of physiology and pathology, strict aseptic techniques, gentle tissue manipulation, among others, were systematized. This lasted for centuries. But the modern technological era invented videosurgery and complicated the previously used teaching method. Objective: Review previously published teaching methods using simulators. Method: This is a narrative review carried out in the PubMed and Scielo databases. The search used the following keywords: videosurgery, teaching, simulators, training. The inclusion criteria comprised review articles and experimental studies, written in Portuguese and English and available in full text. The selected articles were analyzed based on the technologies covered, future perspectives and challenges mentioned. Result: 28 articles were included. Conclusion: As the results of this review on simulators show, once trained residents begin learning videolaparoscopic operations much more easily and safely.

Introdução: Os procedimentos cirúrgicos foram sistematizados e embasados cientificamente no final do século XIX por William Stewart Halsted. Por meio de estudos científicos, princípios anatômicos, conhecimento de fisiologia e patologia que permitiram sistematizar técnica asséptica estrita, manipulação tecidual gentil, dentro de outros. Isto perdurou por séculos. Mas a moderna era tecnológica inventou a videocirurgia e complicou o método de ensino anteriormente usado. Objetivo: Revisar os métodos de ensino feito por simuladores já publicados. Método: Trata-se de revisão narrativa realizada nas bases de dados PubMed e Scielo. A busca utilizou as seguintes palavras-chave: videocirurgia, ensino, simuladores, treinamento. Os critérios de inclusão compreenderam artigos de revisão e estudos experimentais, escritos em português e inglês e disponíveis em texto completo. Os trabalhos selecionados foram analisados com base nas tecnologias abordadas, perspectivas futuras e desafios mencionados. Resultado: Foram incluídos 28 artigos. Conclusão: Conforme mostram os resultados desta revisão sobre simuladores, os residentes depois de treinados, iniciariam o aprendizado de operações videolaparoscópicas com muito mais facilidade e segurança.

Abstract Objectives: to identify the scientific evidence on excessively resistant and multidrug resistant tuberculosis in pediatric patients. Methods: this is a scope review of the literature, with a guiding question: “What is the scientific evidence on multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis in pediatric patients?”. The research used the descriptors: “extensively drug-resistant tuberculosis” OR “multidrug-resistant tuberculosis” AND “pediatrics”. The research was carried out in a double-blind manner in the following databases of the Medical Literature Analysis and Retrieval System Online, Regional Office for the Western Pacific’s Institutional Repository for Information Sharing, Embase/Elsevier and International Clinical Trials Registry Platform, with a temporal cut-off from 2011 to 2021, sending a final synthesized sample of 18 articles, which evaluated the methodological content through the level of evidence. Results: the results show the lack of research with a high level of evidence related to MDR-TB in children, the lack of adequate dosage of second-line drugs for the pediatric population and the importance of drug sensitivity testing for the cases of treatment Conclusions: it was identified that the obstacles to MDR-TB treatment were concentrated in the lack of detailed protocols, safe drug dosages with a low side effect, and mainly in the social health determinants and disease process involving MDR-TB.

Resumo Objetivos: identificar as evidências científicas sobre tuberculose excessivamente resistente e multidroga resistente em pacientes pediátricos. Métodos: trata-se de uma revisão de escopo da literatura, tendo como questão norteadora: “Quais as evidências científicas sobre tuberculose multidroga-resistente (TB-MDR) e tuberculose extensivamente resistente em pacientes pediátricos?” A pesquisa usou os descritores: “tuberculose extensivamente resistente a medicamentos” OR “tuberculose resistente a múltiplos medicamentos” AND “pediatria”. A pesquisa foi realizada de modo duplo-cego nas bases de dados Medical Literature Analysis and Retrieval System Online, Regional Office for the Western Pacific’s Institutional Repository for Information Sharing, Embase/Elsevier e International Clinical Trials Registry Platform, com um corte temporal de 2011 a 2021, sendo a amostra final sintetizada de 18 artigos, nos quais avaliou-se o conteúdo metodológico por meio do nível de evidência. Resultados: os resultados mostraram a escassez de pesquisas de alto nível de evidência relacionadas à TB-MDR em crianças, ausência de posologia adequada das drogas de segunda linha para o público pediátrico e a importância do teste de sensibilidade a drogas para o tratamento dos casos. Conclusões: identificou-se que os obstáculos do tratamento TB-MDR se concentraram na ausência de protocolos detalhados, de dosagens medicamentosas seguras e com menor efeito colateral, e, principalmente, nos determinantes sociais do processo saúde e doença que envolvem a TB-MDR.

ABSTRACT Purpose To describe the characteristics of the initial public health training for speech-language-hearing therapists in Northeastern Brazil. Methods The research was based on the curricular framework of public higher education institutions in the Northeast. The analysis approached each institution’s general speech-language-hearing program data (state where it is located, total course load, and most recent curricular framework reformulation) and specific data on public health courses (their individual course load, the term when it is offered, syllabus content, whether it was theoretical, practical, both, or internship, and whether it was required or elective). Results The data show that eight public higher education institutions in the Northeast offer speech-language-hearing programs, whose pedagogical frameworks date from 2009 to 2021. The total course load of those related to public health ranges from 7.5% to 20.5% of the total program among those analyzed. Most courses were elective, exclusively theoretical, and were offered in the first half of the program. Conclusion The initial public health training for speech-language-hearing therapists in public higher education institutions in the Northeast still seems to be based on traditional practices. These create a distance between students, public health, and practices that meet the principles of Health Unic System (in Portuguese - Sistema Único de Saúde) and the population’s real needs, especially in primary healthcare

RESUMO Objetivo Descrever as características da formação inicial em saúde coletiva de fonoaudiólogos do Nordeste do Brasil. Método A pesquisa foi realizada com base nas matrizes curriculares de Instituições de Ensino Superior (IES) públicas do Nordeste a partir das quais foram analisados dados gerais dos cursos de Fonoaudiologia de cada Instituição ー estado em que está localizado, carga horária total e última reformulação de matriz curricular; e dados que diziam respeito especificamente às disciplinas relacionadas à Saúde Coletiva ー carga horária de cada disciplina, semestre de oferta, conteúdo da ementa, caráter teórico, prático, teórico-prático ou estágio, se obrigatória ou optativa. Resultados Os dados revelam que existem oito IES públicas do nordeste que oferecem o curso de Fonoaudiologia, sendo que os Projetos Pedagógicos de Cursos datam de 2009 a 2021. A Carga Horária (CH) total das disciplinas vinculadas à área de Saúde Coletiva varia de 7,5% a 20,5% da CH total dos cursos analisados. A maioria destas disciplinas é eletiva, tem caráter teórico e é ofertada na primeira metade da formação. Conclusão A formação inicial em saúde coletiva de fonoaudiólogos de IES públicas do nordeste parece ainda estar embasada em práticas tradicionais, que acabam distanciando os discentes do campo da saúde coletiva e de práticas que atendam os princípios do Sistema Único de Saúde, e as reais necessidades da população, especialmente na Atenção Primária à Saúde.

ABSTRACT Objective: To analyze the compliance with the commercialization of children's products included in the Brazilian Code of Marketing of Infant and Toddlers Food and Childcare-Related Products (NBCAL) in drugstores in Uberlândia/MG. Methods: A cross-sectional study was carried out in 143 drugstores that sold infant products: infant formula (IF), follow-up IF, nipples, teats, pacifiers and nipple shields; FI for young children, transition foods and cereal-based foods, fluid or powdered milk, modified/similar milks of plant origin and dairy compounds. The location of drugstores in the five geographic sectors was performed by geoprocessing. The data collected were: types of promotion and types of drugstore administration (drugstore chains/drugstores with independent administration). Irregular commercial promotion was expressed as absolute and relative frequencies. Results: Irregular commercial promotion was found in 11.7% of nipples, pacifiers and bottles, in 10.0% of IF and follow-up formula, in 9.5% of IF for young children, in 11.1% fluid or powdered milk, in 25.0% of transition foods and cereal-based foods and in 59.1% of dairy compounds. In commercial drugstore chains, the presence of promotion for dairy (81.8 vs. 28.6%, respectively) was higher than in drugstores with independent administration. The opposite ocurred for fluid or powdered milk, modified and similar milks of plant origin. The downtown and eastern sectors had the highest percentages of promotions (26%). Conclusions: NBCAL violations still occur in drugstores, mainly in the sale of young children's foods and in the commercial network drugstores.

RESUMO Objetivo: Analisar a conformidade da comercialização dos produtos infantis incluídos na Norma Brasileira de Comercialização de Alimentos para Lactentes e Crianças de Primeira Infância, Bicos, Chupetas e Mamadeira (NBCAL) e de compostos lácteos em drogarias de Uberlândia/MG. Métodos: Estudo transversal realizado em 143 drogarias que vendiam produtos infantis: fórmulas infantis (FI) para lactentes, FI de seguimento para lactentes, mamadeiras, bicos, chupetas e protetores de mamilo; FI para crianças de primeira infância, alimentos de transição e alimentos à base de cereais, leites fluidos/em pó, leites modificados/similares de origem vegetal e composto lácteo. A localização das drogarias nos cinco setores geográficos foi realizada por geoprocessamento. Os dados coletados foram: tipos de promoção comercial irregular e tipo de administração da drogaria (rede/independente). As promoções comerciais irregulares foram expressas em frequências absoluta e relativa. Resultados: Verificamos a presença de promoção comercial irregular em 11,7% dos bicos, chupetas e mamadeiras, em 10,0% das FI lactentes/seguimento de lactentes, em 9,5% das FI para crianças de primeira infância, em 11,1% dos leites, em 25,0% de alimentos de transição e em 59,1% dos compostos lácteos. Nas drogarias de rede, a presença de promoção comercial irregular foi maior para compostos lácteos (81,8 vs. 28,6%, respectivamente) e, para leites, foi maior nas drogarias independentes (30,8 vs. 6,0%). Os setores central e leste apresentaram os maiores percentuais de promoção comercial irregular (26%). Conclusões: As violações à NBCAL ainda ocorrem nas drogarias, principalmente para os produtos destinados às crianças de primeira infância, e nas drogarias de rede.

Abstract Objective Training a competent physician requires to direct the resident profile of graduate students for practice activities. We sought to identify the doctor-patient relationship orientation and the self-assessment of the core competencies, which they pointed out needed to be developed. Methods All 56 orthopedic residents admitted between 2016 and 2019 participated in the present prospective observational study. The Patient Practitioner Orientation Scale (PPOS) and a self-assessment questionnaire were answered at the beginning and end of the first year of residency (R1) in Orthopedics and Traumatology. We calculated mean and standard deviation for PPOS items and scores and analyzed them through the paired t-test. Self-Assessment Questionnaire answer options were “yes” or “I need to improve it” and skills were classified in decreasing order of the frequency of “I need to improve it” responses with description of absolute number and percentage. We compared frequencies using Fisher Test. P-values < 0.05 were considered statistically significant. GraphPad Prism 8.4.3 (GraphPad Software, San Diego, CA, USA) and Microsoft Excel (Microsoft Corporation, Redmond, WA, USA) were used for statistical analysis. Results In the period between the beginning and the end of R1, the total PPOS mean score significantly decreased from 4.63 to 4.50 (p= 0.024), more biomedical-focused. Around one-third of the residents identified competencies of patient care, practice-based learning and improvement, and interpersonal and communication skills as needed to improve. Conclusions The PPOS and self-assessment activities could promote reflection practices and are possible tools for learner-centered competency assessment. Biomedical guidance tends to prevail as the training of physicians progresses, and periodic self-assessments can be worked on to build a growth mindset.

Resumo Objetivo A formação de um médico competente requer direcionar o perfil de pós-graduação residente para atividades práticas. Buscou-se identificar a orientação de relacionamento médico-paciente e a autoavaliação das competências fundamentais que eles apontaram que precisavam ser desenvolvidas. Métodos Todos os 56 residentes em ortopedia admitidos entre 2016 e 2019 participaram do presente estudo observacional prospectivo. A Escala de Orientação Médico-Paciente (Patient Practitioner Orientation Scale [PPOS, na sigla em inglês]) e um questionário de autoavaliação foram respondidos no início e no final do primeiro ano de residência (R1) em Ortopedia e Traumatologia. Calculamos o desvio médio e padrão para itens e pontuações de PPOS e os analisamos através do teste t emparelhado. As opções de resposta do Questionário de Autoavaliação foram "sim" ou "preciso melhorar" e as habilidades foram classificadas na ordem decrescente da frequência das respostas "preciso melhorar" com descrição de número absoluto e percentual. Comparamos frequências usando o teste de Fisher. Consideramos significativos valores-p < 0,05. Os programas GraphPad Prism 8.4.3 (GraphPad Software, San Diego, CA, EUA) e Microsoft Excel (Microsoft Corporation, Redmond, WA, EUA) foram utilizados para análise estatística. Resultados No período entre o início e o final do R1, a média total de PPOS diminuiu significativamente, de 4,63 para 4,50 (p= 0,024), mais focada em biomédica. Cerca de um terço dos residentes identificou competências do cuidado ao paciente, aprendizagem e melhoria baseadas na prática e habilidades interpessoais e de comunicação, como necessitando melhorar. Conclusões As atividades de PPOS e autoavaliação podem promover práticas de reflexão e são possíveis ferramentas para avaliação de competência centrada no aluno. A orientação biomédica tende a prevalecer à medida que a formação dos médicos progride e as autoavaliações periódicas podem ser trabalhadas para construir uma mentalidade de crescimento.

RESUMO A Atenção Primária à Saúde é um nível de atenção estratégico para a conformação de um sistema de saúde sustentável e capaz de responder a necessidades diversas. Este artigo teve como objetivo analisar a normatização federal e suas implicações para a organização das equipes de atenção primária no Brasil. Pesquisa exploratória de métodos mistos, envolvendo a análise de 25 portarias federais e de dados secundários de abrangência nacional referentes às equipes homologadas no Cadastro Nacional de Estabelecimentos de Saúde, no período de 2017 a 2021. Os resultados indicam mudanças na direcionalidade da política quanto à configuração, ao financiamento e ao credenciamento das equipes. Verificaram-se expansão das equipes de atenção primária, redução de Agentes Comunitários de Saúde e enfraquecimento do Núcleo de Apoio à Saúde da Família. Os resultados sugerem que os estímulos a outros arranjos de equipes e a flexibilização da cobertura do agente comunitário e da atuação multiprofissional comprometem a sustentabilidade do modelo de Saúde da Família no Sistema Único Saúde.

ABSTRACT Primary Health Care is a strategic level of care for forming a sustainable health system that responds to diverse needs. This article analyzed federal regulation and its implications for establishing primary care teams in Brazil. Exploratory mixed methods research involves analyzing 25 federal ordinances and secondary data of national scope referring to the teams approved in the National Register of Health Establishments from 2017 to 2021. The results indicate changes in the direction of the policy regarding the configuration, funding, and accreditation of teams. There was an expansion of primary care teams, a reduction in community health agents, and a weakening of the Family Health Support Center. The results suggest that the incentives for other team arrangements, the flexibility of the coverage of the community agent, and the multidisciplinary action compromise the sustainability of the Family Health model in the Unified Health System.

This is a multicenter, cross-sectional study carried out with students regularly enrolled in undergraduate courses at eight Brazilian IFES. Data collection was conducted between October 2021 and February 2022 using an online self-completed questionnaire, which addressed sociodemographic and academic characteristics, lifestyle habits, and health conditions. Symptoms of anxiety and depression were assessed using the Depression, Anxiety, and Stress Scale-21 (DASS-21). Variables were analyzed descriptively using frequency distribution and Pearson’s chi-square test. To estimate the prevalence of symptoms of anxiety and depression, the proportion and 95% confidence interval (95%CI) were used. The level of adopted statistical significance was 5%.

Objective: to analyse the relationship between physical activity (PA) environment at schools and obesity in children, the light of complex systems. Methods: this is a cross sectional study involved 1.200 children (8.1 ± 1.0 years old and 50% boys) from eight public schools from socially vulnerable neighbourhoods of Petrolina, Brazil. Weight and height measurements were assessed to calculate the Body Mass Index (BMI) and classified as normal weight and overweight. To assess the PA environment at the school, a questionnaire about the structural and organizational environment of the school was applied through a face-to-face interview with the school´s manager was conducted. The association between the PA environment at school and obesity was tested using a Machine Learning technique (Network Analysis) performed on the Jasp software. Results: positive associations between BMI and Physical Education classes (0.847), physical education teacher (0.349), break duration (0.564) and indoor sports court (0.662) were observed. Negative associations were seen with sex (-0.212) age (- 0.387), extracurricular PA (-0.492) and playground (-0.557). Additionally, the centrality indicators highlighted extracurricular PA (1.789) as the variables with the highest betweenness values, and BMI with the highest closeness (2.239) and strength (1.230) values. Conclusion: Extracurricular PA and the presence of playgrounds at school´s environment are associated with a healthier weight status low-income children.

Abstract Compulsory teleworking has affected workers in several countries, increasing concerns about managerial support. This study aimed to build and present validity evidence of the Short Scale for Managerial Support to Teleworkers. We carried out four successive steps, namely: adapting items from other managerial support scales or related constructs to the teleworking context; exploratory factor analysis and network analysis (N=3,769); confirmatory factor analysis (N=3,839); invariance analysis considering gender and education (N=7,608). As a result, the scale has a unifactorial structure with five items and quite favorable psychometric indicators regarding validity, reliability, and invariance. We recommend using the scale applied to different samples of teleworkers and research with other organizational or individual variables.

Resumen El teletrabajo obligatorio ha afectado a los trabajadores en varios países, aumentando la preocupación por el apoyo de los directivos. Este estudio tuvo como objetivo construir y presentar evidencias de validez de la Escala Corta de Apoyo Organizacional a Teletrabajadores. Se realizaron cuatro pasos sucesivos, a saber: adaptar ítems de otras escalas de apoyo organizacional o constructos relacionados al contexto del teletrabajo; análisis factorial exploratorio y análisis de redes (N=3.769); análisis factorial confirmatorio (N=3.839); análisis de la invariancia considerando género y escolaridad (N=7.608). La escala posee de una estructura unifactorial con cinco ítems e indicadores psicométricos altamente favorables en términos de validez, confiabilidad e invarianza. Se recomienda el uso de la escala en diferentes muestras de teletrabajadores, y para investigaciones con otras variables organizacionales o individuales.

Resumo O teletrabalho compulsório atingiu trabalhadores em diversos países, aumentando a preocupação com o suporte gerencial. Este estudo objetivou construir e apresentar evidências de validade da Escala Breve de Suporte Gerencial ao Teletrabalhador. Foram realizadas quatro etapas sucessivas, a saber: adaptação, ao contexto do teletrabalho, de itens pertencentes a escalas de suporte ou construtos correlatos; análise fatorial exploratória e análise de redes (N=3.769); análise fatorial confirmatória (N=3.839); análise de invariância considerando gênero e escolaridade (N=7.608). A escala possui estrutura unifatorial com cinco itens e indicadores psicométricos muito favoráveis em termos de validade, confiabilidade e invariância. Recomenda-se o uso da escala em diferentes amostras de teletrabalhadores e para investigações conjuntas com outras variáveis organizacionais ou individuais.

Abstract Objective The present study evaluated the profile of germline mutations present in patients who underwent genetic counseling for risk assessment for breast cancer (BC), ovarian cancer (OC), and endometrial cancer (EC) with a possible hereditary pattern. Methods Medical records of 382 patients who underwent genetic counseling after signing an informed consent form were analyzed. A total of 55.76% of patients (213/382) were symptomatic (personal history of cancer), and 44.24% (169/382) were asymptomatic (absence of the disease). The variables analyzed were age, sex, place of birth, personal or family history of BC, OC, EC, as well as other types of cancer associated with hereditary syndromes. The Human Genome Variation Society (HGVS) nomenclature guidelines were used to name the variants, and their biological significance was determined by comparing 11 databases. Results We identified 53 distinct mutations: 29 pathogenic variants, 13 variants of undetermined significance (VUS), and 11 benign. The most frequent mutations were BRCA1 c.470_471delCT, BRCA1 c.4675 + 1G > T, and BRCA2 c.2T> G. Furthermore, 21 variants appear to have been described for the first time in Brazil. In addition to BRCA1/2 mutations, variants in other genes related to hereditary syndromes that predispose to gynecological cancers were found. Conclusion This study allowed a deeper understanding of the main mutations identified in families in the state of Minas Gerais and demonstrates the need to assess the family history of non-gynecological cancer for risk assessment of BC, OC, and EC. Moreover, it is an effort that contributes to population studies to evaluate the cancer risk mutation profile in Brazil.

Resumo Objetivo O presente estudo avaliou o perfil de mutações germinativas presentes em pacientes submetidas a aconselhamento genético para avaliação de risco para câncer de mama (CM), câncer de ovário (OC) e câncer de endométrio (CE) com possível padrão hereditário. Métodos Foram analisados os prontuários de 382 pacientes que realizaram aconselhamento genético após consentimento informado. Um total de 55,76% dos pacientes (213/382) eram sintomáticos (história pessoal de câncer), e 44,24% (169/382) eram assintomáticos (ausência da doença). As variáveis analisadas foram idade, sexo, naturalidade, história pessoal ou familiar de CM, OC, CE bem como outros tipos de câncer associados a síndromes hereditárias. As diretrizes de nomenclatura da Human Genome Variation Society (HGVS) foram usadas para nomear as variantes e seu significado biológico foi determinado pela comparação de 11 bancos de dados. Resultados Identificamos 53 mutações distintas: 29 variantes patogênicas, 13 variantes de significado indeterminado e 11 benignas. As mutações mais frequentes foram BRCA1 c.470_471delCT, BRCA1 c.4675 + 1G > T e BRCA2 c.2T > G. Além disso, 21 variantes parecem ter sido descritas pela primeira vez no Brasil. Além das mutações BRCA1/2, foram encontradas variantes em outros genes relacionados a síndromes hereditárias que predispõem a cânceres ginecológicos. Conclusão Este estudo permitiu conhecer melhor as principais mutações identificadas nas famílias do estado de Minas Gerais e demonstra a necessidade de avaliar a história familiar de câncer não ginecológico para avaliação do risco de CM, OC e CE. Além disso, é um esforço que contribui com estudos populacionais para avaliar o perfil de mutações de risco para câncer no Brasil.

ABSTRACT Lipomas are mesenchymal neoplasms relatively uncommon in the oral cavity. Lipomas can exhibit histopathological features mimicking atypical lipomatous tumors (ALT) or dysplastic lipoma (DL) in the presence of degenerative changes. Relevantly, immunohistochemistry assists in the correct diagnosis. Herein, we present the case of a 54-year-old male with a sessile nodule located on the dorsum of the tongue. The histopathological analysis showed a diffuse, non-circumscribed adipocytic proliferation constituted by cells of variable size containing cytoplasmic vacuoles and displaced nuclei, some resembling lipoblasts supported by fibrous connective tissue stroma. By immunohistochemistry, tumor cells were positive for vimentin, S100, FASN, CD10, and p16. Rb expression was intact. Moreover, CD34, p53, MDM2, and CDK4 were negative. After 2-year of follow-up, no alteration or recurrence was observed. In conclusion, MDM2, CDK4, p53, and Rb immunomarkers can be used reliably to differentiate benign lipoma with degenerative changes from ALT and DL.

Resumo: Este estudo transversal objetivou investigar as diferenças nos determinantes do envelhecimento ativo entre idosos brasileiros e ingleses, e verificar a associação de determinantes comportamentais, pessoais e sociais com a saúde física. A pesquisa baseou-se nas coortes ELSI-Brasil (2015-2016) e ELSA (2016-2017). Os determinantes do envelhecimento ativo incluíram os determinantes comportamentais (tabagismo, sedentarismo e má qualidade do sono), pessoais (função cognitiva e satisfação com a vida) e sociais (educação, solidão e voluntariado), de acordo com a Organização Mundial da Saúde. A saúde física incluiu limitação de atividades e multimorbidade. Prevalências ajustadas por idade e sexo foram calculadas para cada indicador e escores médios, utilizando-se a regressão binomial negativa para a análise estatística. A pesquisa incluiu 16.642 participantes, sendo 9.409 do Brasil e 7.233 da Inglaterra. No geral, todos os determinantes do envelhecimento ativo foram piores no Brasil do que na Inglaterra, exceto a satisfação com a vida (sem diferença). A diferença mais marcante refere-se ao escore de determinantes sociais no Brasil (diferença média de 0,18; p < 0,05), principalmente devido à escolaridade significativamente menor no Brasil (70,6%; intervalo de 95% de confiança - IC95%: 69,7-71,5) do que na Inglaterra (37,1%; IC95%: 35,1-39,1). Todos os determinantes (comportamentais, pessoais e sociais) estiveram associados à saúde no Brasil e na Inglaterra. No entanto, o domínio comportamental foi mais fortemente associado à saúde na Inglaterra (coeficiente = 2,76; IC95%: 2,46-3,10) do que no Brasil (coeficiente = 1,38; IC95%: 1,26-1,50) (p < 0,001). Idosos ingleses se beneficiam mais de comportamentos mais saudáveis do que os brasileiros, que dependem mais de políticas sociais.

Abstract: This study aimed to investigate differences in determinants of active aging between older Brazilian and English adults and to verify the association of behavioral, personal, and social determinants with physical health. This cross-sectional study was based on the ELSI-Brazil (2015-2016) and ELSA (2016-2017) cohorts. Active aging determinants included behavior (smoking, sedentary lifestyle, and poor sleep quality), personal (cognitive function and life satisfaction), and social determinants (education, loneliness, and volunteering), according to the World Health Organization. Physical health included activities limitation and multimorbidity. We estimated age- and sex-adjusted prevalence for each indicator and mean score, and used the negative binomial regression for statistical analysis. We included 16,642 participants, 9,409 from Brazil and 7,233 from England. Overall, all active aging determinants were worse in Brazil than in England, except for life satisfaction (no difference). The most remarkable difference was found for social determinants score in Brazil (mean difference of 0.18; p < 0.05), mainly due to a significantly lower education level in Brazil (70.6%; 95% confidence interval - 95%CI: 69.7-71.5) than England (37.1%; 95%CI: 35.1-39.1). All determinants (behavioral, personal, and social) were associated with health in Brazil and in England. However, the behavioral domain was stronger associated with health in England (coefficient = 2.76; 95%CI: 2.46-3.10) than in Brazil (coefficient = 1.38; 95%CI: 1.26-1.50; p < 0.001). Older English adults beneficiate more from healthier behaviors than Brazilians, which depend more on social policies.

ABSTRACT Podzols are highly sandy soils, in which elements, such as potassium, needed by crops, are easily leached. Studies have indicated that biochar can contribute to increasing cation exchange capacity of the soil, which can improve the retention of bases. This study aimed to evaluate the effect of the poultry litter biochar combined or not with increasing doses of KCl, on leaching and soil base content, on the production of green manure biomass, and on the distribution of K in the soil-water-plant system. A 2 × 3 factorial experiment was conducted in randomized blocks with 6 repetitions: without and with biochar (1% v / v, produced from poultry litter) × without K, with 53.65 and 107.30 mg dm-3 of K (as KCl). The experiment was carried out in four steps: Step 1) Soil incubation with the treatments (for 20 days) followed by chemical analysis of the soil; Step 2) First leaching: the containers (made with PVC with 0.30 m high) received the newly incubated soil in the surface layer and soil without treatment in the subsurface layer. The containers received water until saturation plus 50 % of the total pore volume and the leached water was collected and analyzed; Step 3) Cultivation of green manure (Mucuna aterrima) in the containers of Step 2 and harvest within 40 days (evaluation of biomass and K content); Step 4) after the mucuna harvest, the containers were subjected to second leaching. The leaching solution and the soil from the surface and subsurface layers were collected for chemical analysis. Biochar increased the pH, cationic exchange capacity, P, K, Ca, and Mg levels in the newly incubated soil, in the surface and subsurface layer. The increasing doses of KCl proportionally increased biomass production and K accumulation in green manure, and the biochar increased this response and reduces the need for potassium fertilization by 50 %. With biochar, the accumulation of total K in the soil-plant-water system increases by 125 % and, in the soil-plant system, by 145 %. Biochar changes the K ratio in the soil-plant-water system. With biochar, proportionally, the highest accumulated percentage of K occurred mainly in the soil surface and subsurface layer, and did not result in higher percentages of K in the leached water. Biochar produced from poultry litter can be used as a good alternative to improve the chemical properties of Podzols and reduce nutrient losses.
