ABSTRACT Objective The incidence of thrombotic events and acute kidney injury is high in critically ill patients with COVID-19. We aimed to evaluate and compare the coagulation profiles of patients with COVID-19 developing acute kidney injury versus those who did not, during their intensive care unit stay. Methods Conventional coagulation and platelet function tests, fibrinolysis, endogenous inhibitors of coagulation tests, and rotational thromboelastometry were conducted on days 0, 1, 3, 7, and 14 following intensive care unit admission. Results Out of 30 patients included, 13 (43.4%) met the criteria for acute kidney injury. Comparing both groups, patients with acute kidney injury were older: 73 (60-84) versus 54 (47-64) years, p=0.027, and had a lower baseline glomerular filtration rate: 70 (51-81) versus 93 (83-106) mL/min/1.73m2, p=0.004. On day 1, D-dimer and fibrinogen levels were elevated but similar between groups: 1780 (1319-5517) versus 1794 (726-2324) ng/mL, p=0.145 and 608 (550-700) versus 642 (469-722) g/dL, p=0.95, respectively. Rotational thromboelastometry data were also similar between groups. However, antithrombin activity and protein C levels were lower in patients who developed acute kidney injury: 82 (75-92) versus 98 (90-116), p=0.028 and 70 (52-82) versus 88 (78-101) µ/mL, p=0.038, respectively. Mean protein C levels were lower in the group with acute kidney injury across multiple time points during their stay in the intensive care unit. Conclusion Critically ill patients experiencing acute kidney injury exhibited lower endogenous anticoagulant levels. Further studies are needed to understand the role of natural anticoagulants in the pathophysiology of acute kidney injury within this population.

OBJETIVOS: Avaliar a prevalência destas algias na coluna vertebral, identificar sua localização e a associação entre idade, idade gestacional, acometimento nervoso e a presença de dor anterior à gravidez. MÉTODOS: A dor nas costas foi identificada através de estudo descritivo, no qual se realizaram entrevistas através de um questionário estruturado pelos autores a 203 gestantes, selecionadas em salas de espera de Unidades Básicas de Saúde da cidade de Paulínia (Estado de São Paulo), no momento em que chegavam para a assistência pré-natal, entre janeiro e dezembro de 2001. RESULTADOS: A prevalência de dor nas costas foi de 79,8%, a região lombar foi referida por 80,8% e a sacroilíaca por 49,1% das gestantes. A dor foi mais freqüente entre as gestantes mais jovens. A prevalência de dor nas costas não aumentou com o avanço da idade gestacional. Os sintomas de acometimento nervoso foram relatados por 47,7% das gestantes, sem relação com o local da dor na coluna vertebral. CONCLUSÃO: Cerca de 80% das gestantes relataram dor na coluna vertebral em algum período da gravidez, sendo que os locais mais referidos foram as regiões lombar e/ou sacroilíaca. A dor foi mais freqüente entre as mulheres mais jovens. Metade das grávidas apresentou sintomas de acometimento nervoso.

OBJECTIVES: To evaluate the prevalence of back pains, identify their location and the association between age, time of pregnancy, nervous injury and the presence of pain prior to pregnancy. METHODS: Back pain was identified by means of a descriptive study. From January to December of 2001, using a questionnaire structured by the authors, 203 pregnant women arriving for Pre-natal care were interviewed after selection in the waiting rooms of the Primary Health care Units of the city of Paulínia (state of São Paulo). RESULTS: Prevalence of back pain was of 79.8%; of these location in the lumbar region was reported by 80.8% and in the sacroiliac by 49.1% of the pregnant women. Pain was more frequent among younger women. Prevalence of back pain did not increase with progress of pregnancy. Symptoms of nervous injury not related to the site of the back pain were reported by 47.7% of those pregnant. CONCLUSION: Almost 80% of the pregnant women reported back pain at some time during pregnancy. The most often reported sites were the lumbar and sacroiliac. Pain was more frequent among the younger women. Half of the pregnant women reported symptoms of nervous injury.

OBJETIVO: avaliar a efetividade do método dos exercícios stretching global ativo (SGA) comparativamente às orientações médicas para resolver as dores lombares e/ou pélvica posterior durante a gestação. MÉTODOS: foram selecionadas 69 grávidas que apresentaram dores lombares e/ou pélvica posterior para participar de ensaio clínico prospectivo randomizado. As pacientes foram divididas aleatoriamente em dois grupos: grupo SGA, que praticou exercícios orientados pelo método SGA, e grupo ORI, que seguiu as orientações médicas. As gestantes foram acompanhadas por oito semanas. Foi utilizada a escala análogo-visual para medir a intensidade da dor e os testes de provocação de dor lombar e pélvica posterior para confirmação das mesmas. RESULTADOS: após o tratamento das grávidas que participaram dos exercícios do SGA, 61% (p<0,01) não apresentaram queixa de dor nas regiões lombar e pélvica posterior, resultado que não se observou nas gestantes do grupo ORI, que seguiu as orientações médicas e não praticou exercícios pelo método, sendo que 11% (p=0,50) apresentaram melhora. CONCLUSÕES: o método de exercícios SGA diminuiu e reduziu a intensidade da dor lombar e pélvica posterior, entretanto as orientações médicas não foram efetivas para amenizar as dores das gestantes. As algias lombares e/ou pélvica posterior da gestação não apresentaram relação significativa com a dor pré-gestacional e nem com a prática de exercício físico um ano antes da gestação atual.

PURPOSE: to evaluate the effectiveness of the "global active stretching" (GAS) method and the routine medical recommendations for lumbar and/or posterior pelvic pain in pregnancy. METHODS: sixty-nine pregnant women who experienced lumbar or posterior pelvic pain were selected and identified through a randomized controlled clinical trial and were randomly divided into two groups. One group practiced GAS-oriented exercises and the other followed the routine medical recommendations. The pregnant women were followed up for eight weeks. The severity of pain was estimated by the visual analog scale and posterior pelvic pain and lumbar back pain were confirmed by provocation tests. RESULTS: after treatment, 61% (p<0.01) of the women of the GAS group reported no pain at the lumbar/or posterior pelvic area compared with 11% (p=0.50) of the group who followed routine medical recommendations. CONCLUSIONS: the GAS method relieved and diminished the intensity of lumbar and/or pelvic pains more effectively than routine medical recommendations.
