ABSTRACT BACKGROUND AND OBJECTIVES: The mechanisms underlying nociplastic pain, such as fibromyalgia (FM), are not fully understood, however, it is believed that altered sensory processing and pain modulation play prominent roles in the maintenance of nociplastic pain. The hypothesis is that changes in the primary somesthetic cortex (S1) contribute to the generalized pain character of FM. The objective of this study was to evaluate the involvement of the primary somesthetic cortex in humans with fibromyalgia, as well as to investigate possible associations between S1 changes and clinical signs and symptoms of FM. CONTENTS: For this integrative review, the following databases were used: Pubmed and Web of Science, including observational studies carried out in humans with FM. In total, 541 studies were identified and four were included. The majority of studies are case-control studies, published between 2016 and 2022. In total, data from 161 individuals were included in this review. It was identified that there are morphological changes, hyperactivation and increased functional connectivity between S1 and periaqueductal gray matter and between S1 and anterior cingulate cortex. CONCLUSION: Patients with FM present morphological changes and hyperactivation in S1, as well as increased functional connectivity between S1 and periaqueductal gray matter and S1 and limbic system. Furthermore, different bilateral somatotropic subregions (legs, chest, fingers, hands, face and back) showed reduced functional connectivity in patients with FM. These regions are often presented as “tender points” in FM.

RESUMO JUSTIFICATIVA E OBJETIVOS: Os mecanismos subjacentes à dor nociplástica, como a fibromialgia (FM), não são totalmente compreendidos, contudo acredita-se que o processamento sensorial e a modulação da dor alterados desempenham papéis proeminentes para a manutenção da dor nociplástica. Com a hipótese de que alterações no córtex somestésico primário (S1) contribuam para o caráter de dor generalizada da FM, o objetivo deste estudo foi avaliar o envolvimento do córtex somestésico primário em humanos com FM, bem como investigar possíveis associações entre alterações de S1 com sinais e sintomas clínicos da FM. CONTEÚDO: Para esta revisão integrativa, foram utilizadas as seguintes bases de dados: Pubmed e Web of Science, incluindo estudos observacionais realizados em humanos com FM. No total, 541 estudos foram identificados e quatro foram incluídos. A maioria dos estudos são do tipo caso-controle, publicados entre 2016 e 2022. Ao todo, dados de 161 indivíduos foram incluídos. Foi identificado que há alterações morfológicas, hiperativação e aumento da conectividade funcional entre S1 e substância cinzenta periaquedutal e entre S1 e córtex cingulado anterior. CONCLUSÃO: Pacientes com FM apresentam alterações morfológicas e hiperativação em S1, bem como aumento da conectividade funcional entre S1 e substância cinzenta periaquedutal e S1 e sistema límbico. Ademais, diferentes sub-regiões somatotrópicas bilaterais (pernas, tórax, dedos, mãos, face e costas) apresentaram redução da conectividade funcional em pacientes com FM. Essas regiões são frequentemente apresentadas como “tender points” na FM.

ABSTRACT Purpose To verify the association between sociodemographic factors, vocal behavior, morbidities, and self-perception of voice, hearing, and general health in older women with voice disorders. Methods The sample had 95 older women aged 60 to 84 years (mean of 69,5). They were interviewed with a structured questionnaire on sociodemographic aspects, health, and vocal behavior. The Screening Protocol for Voice Disorders in Older Adults (RAVI) was used to identify the presence of voice disorders. Results Participants who had finished high school and were retired predominated. The number of older women with voice disorders according to RAVI was 46.3%. Physical sensations such as dry throat, throat clearing, and itchy throat were the most common complaints. The group of older women with voice disorders had worse self-perception of voice quality, hearing, and general health and a higher frequency of upper airway infections than those without voice disorders (p ≤ 0.05). Conclusion The vocal self-assessment measured with RAVI was statistically associated with self-perception of voice quality, hearing, general health, sore throat, sinusitis, and respiratory allergies.

RESUMO Objetivo Verificar a associação de fatores sociodemográficos, do comportamento vocal, morbidades e autopercepção da voz, auditiva e de saúde geral de idosas com distúrbio vocal. Método Participaram 95 idosas com idades entre 60 e 84 anos (média 69,5). Realizou-se uma entrevista com apoio de um questionário estruturado contendo questões sociodemográficas, de saúde e do comportamento vocal. Utilizou-se o Protocolo para Rastreamento de Alterações Vocais em Idosos (RAVI) para identificar a presença de distúrbio vocal. Resultados Houve o predomínio de participantes com ensino médio completo e aposentadas. O número de idosas que apresentaram distúrbio vocal segundo o RAVI foi de 46,3%. Queixas relacionadas às sensações físicas como garganta seca, pigarro e coceira na garganta foram as mais presentes. O grupo de idosas com distúrbio vocal apresentou pior autopercepção da qualidade vocal, audição, saúde geral e maior frequência de infecções de vias aéreas superiores quando comparadas àquelas sem distúrbio vocal (p≤0,05). Conclusão Houve associação estatística entre a autoavaliação vocal mensurada pelo RAVI e a autopercepção da qualidade da voz, da audição, da saúde geral, inflamações de garganta, sinusite e alergias respiratórias.

SUMMARY OBJECTIVE: The aim of this study was to evaluate the prevalence of early neonatal sepsis in pregnant women with a positive culture for group B beta-hemolytic Streptococcus in a middle-income city in Southeastern Brazil. METHODS: A retrospective cohort study was conducted, involving singleton low- and high-risk pregnancies in whom group B beta-hemolytic Streptococcus cultures were evaluated between 35 and 37 weeks of gestation using vaginal and anal swabs. A specific medium (Todd-Hewitt) was used for culturing. The pregnant women were divided into two groups based on positive (n==201) and negative (n==420) cultures for group B beta-hemolytic Streptococcus. RESULTS: The maternal colonization rate by group B beta-hemolytic Streptococcus was 32.3%. The prevalence of early neonatal sepsis was 1.0% (2/201) among patients with a positive group B beta-hemolytic Streptococcus culture and 1.9% (8/420) among patients with a negative culture. Among the patients who underwent adequate prophylaxis, crystalline penicillin G was used in 51.9% (54/104), followed by cefazolin in 43.3% (45/104), ampicillin in 3.8% (4/104), and clindamycin in 1.0% (1/104). A model that included prematurity (p==0.001) proved to be an independent risk predictor of early neonatal sepsis [χ2 (1)==15.0, odds ratio: 16.9, 95% confidence interval: 4.7–61.6, p<0.001, Nagelkerke R2==0.157]. CONCLUSION: The prevalence of a positive culture for group B beta-hemolytic Streptococcus was high. However, the prevalence of early neonatal sepsis was low in pregnant women with both positive and negative group B beta-hemolytic Streptococcus cultures and in pregnant women with a positive culture who underwent both adequate and inadequate antibiotic prophylaxis. Prematurity proved to be an independent predictor of early neonatal sepsis, considering the entire study population.

ABSTRACT Objective: To evaluate the psychometric properties and reliability of the Brazilian version of the tool Parent Attitudes about Childhood Vaccine (PACV-BR). Methods: The sample included 110 parents of children up to two years old served by Family Health Basic Units. The tool's internal consistency and factor validity were respectively assessed by Cronbach's alpha and exploratory factor analysis (EFA). The test-retest reliability was assessed by the intraclass correlation coefficient (ICC). Results: The EFA results indicated a proper structural adequacy of the PACV-BR (15 items and two factors). The reliability generated Cronbach's alpha values between 0.715 and 0.854 for the items, of 0.918 for the tool as a whole, of 0.877 for factor 1 and of 0.825 for factor 2, in addition to an ICC of 0.984. Conclusions: The PACV-BR showed evidence of construct validity and reliability.

RESUMO Objetivo: Avaliar as propriedades psicométricas e a confiabilidade da versão brasileira do instrumento Parent Attitudes about Childhood Vaccine (PACV-BR). Métodos: A amostra incluiu 110 pais de crianças de até dois anos atendidas em Unidades Básicas de Saúde da Família. A consistência interna e a validade fatorial do instrumento foram avaliadas, respectivamente, pelo alfa de Cronbach e pela análise fatorial exploratória (EFA). A confiabilidade teste-reteste foi avaliada pelo coeficiente de correlação intraclasse (ICC). Resultados: Os resultados da AFE indicaram adequação estrutural do PACV-BR (15 itens e dois fatores). A confiabilidade indicou valores de alfa de Cronbach entre 0,715 e 0,854 para os itens, de 0,918 para o instrumento como um todo, de 0,877 para o fator 1 e de 0,825 para o fator 2, além de ICC de 0,984. Conclusões: O PACV-BR apresentou evidências de validade de construto e confiabilidade.

ABSTRACT: Leptospirosis, a disease of significant economic impact on livestock and public health concern, may exhibit unique epidemiological characteristics in indigenous villages, particularly those located within semiarid climates such as the Caatinga biome in Brazil. This environment often presents conditions unfavorable for leptospire survival. This study conducted serological diagnoses of livestock in indigenous villages within the Caatinga biome, Northeastern Brazil. The survey was carried out in the Mãe Maria and Santa Helena indigenous villages, home to the Xukuru de Cimbres tribe, in the county of Pesqueira, Pernambuco State, Brazil, within the Caatinga biome. Blood samples were collected from cattle (n = 42), goats (n = 41), and sheep (n = 19). Out of the 102 samples tested, 73 (71.6%; 95% CI = 62.8% - 80.3%) were seroreactive (cut-off ≥ 100). The seropositivity rates for cattle, goats, and sheep were 54.8%, 80.5%, and 89.5%, respectively. A statistically significant difference was observed in the proportion of seropositive animals between cattle and goats and cattle and sheep (P < 0.05). Icterohaemorrhagiae was identified as the most probable serogroup across all animal species. The study revealed that livestock in indigenous villages within the Caatinga biome are frequently exposed to leptospires. This finding underscores the importance of a One Health approach, emphasizing the need for comprehensive studies in areas where large human and animal populations coexist. Such studies are crucial for investigating zoonotic infections and for planning and implementing control measures against livestock-associated leptospirosis.

RESUMO: A leptospirose causa perdas econômicas na pecuária e é um problema de saúde pública, sendo possível que existam peculiaridades na epidemiologia dessa doença em aldeias indígenas, principalmente naquelas de áreas de clima semiárido, como o bioma Caatinga no Nordeste do Brasil, onde o ambiente é frequentemente adverso à sobrevivência de leptospiras. Assim, o objetivo deste estudo foi realizar o diagnóstico sorológico em rebanhos de aldeias indígenas no bioma Caatinga. A pesquisa foi realizada nas aldeias indígenas Mãe Maria e Santa Helena, tribo Xukuru de Cimbres, município de Pesqueira, estado de Pernambuco, Brasil, dentro do bioma Caatinga. Amostras de sangue foram colhidas de bovinos (n = 42), caprinos (n = 41) e ovinos (n = 19). No geral, das 102 amostras testadas 73 (71,6%; IC 95% = 62,8% - 80,3%) foram sororreativas (ponto de corte ≥ 100). Para bovinos, caprinos e ovinos as taxas de soropositividade foram de 54,8%, 80,5% e 89,5%, respectivamente. Houve diferença estatística na proporção de animais soropositivos entre bovinos e caprinos e bovinos e ovinos (P < 0,05). Icterohaemorrhagiae foi o sorogrupo mais provável em todas as espécies animais. O estudo mostra que animais de produção de aldeias indígenas são comumente expostos a leptospiras no bioma Caatinga, o que constitui uma preocupação do ponto de vista de Saúde Única e demonstra a importância de estudos amplos em que coexistem grandes números de humanos e animais na investigação de infecções zoonóticas e no planejamento e implementação de medidas de controle da leptospirose associada a animais de produção.

ABSTRACT Seed vigor is one of main attributes of the physiological quality from seed lot. Thus, identify which mechanisms are used by seeds of higher vigor that favor the formation of seedlings with better performance is a key aspect in agriculture. The enzyme activity of phytase and alpha-amylase favor the availability of phosphorus and sugars, respectively, during germination and can be determinants of seed vigor. Thus, the objective of the present work was to identify the association of phytase and alpha-amylase enzymes with the vigor of a seed lot of common beans. The results showed a positive correlation between seed vigor and the enzymes evaluated. Based on the findings, it can be concluded that seeds with high vigor have a high capacity to form seedlings with better performance, because they present high enzymatic activity. Also, in seed lots of the same genotype, they can be used to determine seed vigor during germination. Resumen: Las habilidades de gestión del tiempo proporcionan una buena herramienta en la superación de la dilación y en la gestión de tareas. Estos, a su vez, son dos problemas que están presentes en la vida de muchos estudiantes de educación superior. La mala gestión del tiempo puede resultar en una relación desequilibrada con las responsabilidades académicas, lo que puede conducir a un aumento del estrés. La falta de planificación de las actividades, la toma de un número de tareas excesivo para sí y la ausencia de estrategias para la eliminación de distracciones pueden minar el bienestar en la vida académica. Estas prácticas pueden remediarse adoptando estrategias de gestión del tiempo. Con el fin de divulgar y fomentar tales estrategias, se elaboraron talleres online. Participaron en los talleres un total de 17 estudiantes, de los cuales 12 presentaron mejoras en su relación con la gestión del tiempo, dos de estos presentaron cambios positivos particularmente notables. Se concluye a partir de estos resultados que tales intervenciones pueden ser de gran valor y que incluso con intervenciones breves es posible obtener resultados que confirmen la mejora en las habilidades de gestión del tiempo de los participantes.

Abstract: Those who make use of time management skills find them to be a useful tool for the purpose of overcoming procrastination tendencies and fostering better task management. These two matters - procrastination and task management - are two issues of considerable relevance in the lives of many university students. Poor time management skills can result in an unbalanced relationship with one’s academic responsibilities, resulting in an increase in stress levels. Uneven distribution of tasks throughout the week, procrastination, task overload and the absence of strategies for eliminating distractions are some of the possible means through which a student can undermine their own well-being as it relates to academic life. These bad practices may be remedied by adopting time management strategies. With the purpose of fostering such strategies, two online workshops were developed. A total of 17 students participated in the intervention, of whom, 12 showed improvements in their time management skills, with two displaying particularly remarkable positive changes. From these results, it was concluded that these interventions can be of great value, and that, despite the interventions being brief, it is possible to obtain results that lead to the improvement of participants’ time management skills.

Resumo: As habilidades de gestão do tempo fornecem àquele que se utiliza delas uma boa ferramenta na superação da procrastinação e na gestão de tarefas. Estes, por sua vez, são dois problemas que estão presentes na vida de muitos estudantes do Ensino Superior. Má gestão do tempo pode resultar em uma relação desequilibrada com as responsabilidades acadêmicas, o que pode levar a um aumento do estresse. A falta de planejamento das atividades, a tomada de um número de tarefas excessivo para si e a ausência de estratégias para eliminação de distratores podem minar o bem-estar na vida acadêmica. Tais práticas podem ser remediadas pela adoção de estratégias de gestão do tempo. Com o fim de divulgar e fomentar tais estratégias, foram elaboradas oficinas em formato online. Participaram das oficinas um total de 17 estudantes, dos quais, 12 apresentaram melhoras na sua relação com a gestão do tempo e, destes, dois apresentaram mudanças positivas particularmente notáveis. Conclui-se a partir destes resultados que tais intervenções podem ser de grande valia, e que mesmo com intervenções breves é possível obter resultados que confirmem a melhora nas habilidades de gestão do tempo dos participantes.

ABSTRACT As a result of the debates held during the XI Seminar on Service, Research and Public Policy, on the theme “Urban Agriculture and Food and Nutrition Security: organic food in school meals”, this article summarizes the speeches of researchers, agricultural producers, representatives of civil society institutions and government agencies on urban food production. From the perspective of the of urban agriculture contribution to the quality of food, issues related to the scope and limits of the distribution of these foods through public facilities and destination to public policies, such as the school meal program, are addressed in light of experiences in the cities of São Paulo and Castro, in Paraná. Considering the supply of urban centers and the importance of organizational and food environments, debate took national and international guidelines on the social determinants of contemporary health and nutrition as a theoretical reference. The difficulties that mark the production of urban agriculture were addressed, and the actions of local initiatives that made it possible to advance towards a viable economic activity that promotes healthy eating and equity were presented. The role of public food policies, still limited in the field of urban agriculture, is evident to strengthen the work of farmers and public managers engaged in initiatives that already point to the potential for growth in this form of food production.

RESUMO Produto dos debates realizados por ocasião do XI Seminário Serviço, Pesquisa e Política Pública, sobre o tema “Agricultura Urbana e Segurança Alimentar e Nutricional: o alimento orgânico na alimentação escolar”, este artigo sintetiza a fala de pesquisadores, produtores agrícolas, representantes de instituições da sociedade civil e de órgãos governamentais sobre a produção urbana de alimentos. Tendo como perspectiva a contribuição da agricultura urbana para a qualidade da alimentação, são tratados temas relativos aos alcances e limites para a distribuição destes alimentos por equipamentos públicos e destinação a políticas públicas como a alimentação escolar, à luz de experiências nas cidades de São Paulo e Castro, no Paraná. Considerações sobre o abastecimento de centros urbanos e a importância de ambientes organizacionais e ambientes alimentares tomaram como referencial diretrizes nacionais e internacionais sobre os determinantes sociais da situação de saúde e nutrição na contemporaneidade. São apontadas as dificuldades que marcam a produção da agricultura urbana, e as ações de iniciativas locais que possibilitaram avançar na direção de uma atividade econômica viável e promotora de alimentação saudável e de equidade. O papel das políticas públicas de alimentação, ainda limitado no que se refere à agricultura urbana, é patente para promover o fortalecimento do trabalho de agricultores e gestores públicos engajados nas iniciativas que já apontam o potencial de crescimento desta forma de produzir alimentos.

Resumo Introdução: A educação na infância tem sido reconhecida como aspecto fundamental para o desenvolvimento global da criança, sendo a escola, o espaço onde se proporciona condições para ofertas pedagógicas intencionalmente planejadas, oferecendo condições para que as crianças exerçam o papel de ator social e sujeito de direitos. Nessa premissa, a documentação pedagógica, estratégia contemplada nas atuais pedagogias participativas, tem despontado como ferramenta relevante no cenário de Educação Infantil, sendo necessário apropriação prática para a obtenção de êxito nos resultados. Objetivo: Compreender essa centralidade de documentação pedagógica como estratégia contemplada para efetivação de pedagogias participativas na Educação Infantil, segundo a literatura sobre a temática. Método: Revisão de Literatura, realizada nas bases de dados Scientific Electronic Library Online (Scielo), Educational Resources Information Center (ERIC), Scopus e Google Acadêmico, no período que compreende entre outubro de 2022 e novembro de 2022. Resultados: Foram constituídas três bases analíticas, a saber: Uma prática investigativa, adocumentação pedagógica e a observação e os registros na documentação pedagógica. Considerações finais: Evidenciou-se que, na prática, a documentação pedagógica impacta em condições para a reflexão das crianças sobre suas vivências, impulsionando-as a se inserirem em atividades investigativas a partir de suas problematizações, durante o processo pedagógico na Educação Infantil e, como via de mão dupla, oferece condições para a autoformação dos professores e professoras, com ênfase na reflexão sobre as práticas docentes.

Abstract Introduction: Early childhood education has been recognized as a fundamental aspect for the overall development of the child, with the school being the space where conditions are provided for intentionally planned pedagogical offers, offering conditions for children to exercise the role of social actor and subject of rights. On this premise, pedagogical documentation, a strategy contemplated in current participatory pedagogies, has emerged as a relevant tool in the Early Childhood Education scenario, requiring practical appropriation to obtain successful results. Objective: To understand this centrality of pedagogical documentation as a contemplated strategy for implementing participatory pedagogies in Early Childhood Education, according to the literature on the subject. Method: Literature review, carried out in the Scientific Electronic Library Online (Scielo), Educational Resources Information Center (ERIC), Scopus and Google Scholar databases, in the period between October 2022 and November 2022. Results: Three analytical bases, namely: An investigative practice, pedagogical documentation and observation and records in pedagogical documentation. Final considerations: It was evidenced that, in practice, the pedagogical documentation impacts on conditions for children's reflection on their experiences, impelling them to be inserted in investigative activities based on their problematizations, during the pedagogical process in Early Childhood Education and, as a two-way street, it offers conditions for the self-training of male and female teachers, with an emphasis on reflection on teaching practices.

Resumo O artigo investiga a correlação entre engajamento e qualidade de vida no trabalho dos profissionais de enfermagem de um hospital público do interior do estado de São Paulo, no início da pandemia de COVID-19. Estudo transversal, descritivo e correlacional, com profissionais de enfermagem, realizado entre dezembro de 2020 e janeiro de 2021. Foram utilizadas as versões brasileiras da Utrecht Work Engagement Scale (UWES) e da Escala de Avaliação da Qualidade de Vida no Trabalho (QVT), segundo modelo de Walton. Os profissionais de enfermagem apresentaram correlação forte e positiva (r≥0.70) entre o domínio integração social da QVT e a dimensão vigor da UWES (r=0,88; p=<0,001); correlação positiva moderada (r≥0.40≤0.69) entre as condições de trabalho (QVT) e as dimensões vigor (r=0,40; p=<0,001), dedicação (r=0,40; p=<0,001) e escore geral (r=0,41; p=<0,001) da UWES. As correlações foram positivas e fracas (r≤0.39) para os demais domínios da QVT e as dimensões da UWES. Concluiu-se que os profissionais com níveis satisfatórios de qualidade de vida tendem a ter níveis mais altos de engajamento no trabalho. No início da pandemia de COVID-19, os profissionais de enfermagem estavam fortemente engajados e satisfeitos com sua qualidade de vida no trabalho.

Abstract This article investigates the correlation between work engagement and work-related quality of life among nursing staff at a public hospital in São Paulo State, at the onset of the COVID-19 pandemic. A descriptive, correlational, cross-sectional study of nursing personnel was conducted from December 2020 to January 2021, using the Brazilian versions of the Work & Well-being Survey (Utrecht Work Engagement Scale - UWES) and the Work-related Quality of Life (WRQoL) scale, following Walton’s model. Nursing staffs returned a strong positive correlation (r≥0.70) between the social integration domain of the WRQoL and the vigour dimension of the UWES (r=0.88; p=<0.001); a moderate positive correlation (r≥0.40≤0.69) between working conditions (WRQOL) and the vigour (r=0.40; p<0.001), dedication (r=0.40; p<0.001) and overall score (r=0.41; p<0.001) dimensions of the UWES. Correlations were positive and weak (r≤0.39) for the other WRQoL domains and the UWES dimensions. It was concluded that personnel with satisfactory levels of quality of life tend to show higher levels of engagement at work. At the onset of the COVID-19 pandemic, nursing professionals were strongly engaged and satisfied with their work-related quality of life.

Resumo Introdução: O delirium é uma das complicações mais frequentes em cuidados paliativos com uma prevalência de 88% nos dias que precedem o fim de vida. Os dados sobre delirium no seguimento domiciliário em cuidados paliativos são escassos. O objetivo deste trabalho é caracterizar a popula-ção seguida por uma equipa de cuidados paliativos em regime intra-hospitalar e domiciliário, avaliar a prevalência de delirium, a estratégia terapêutica, o tempo de resolução e os fatores que se associam a maior ou menor prevalência de delirium. Métodos: Foi realizado um estudo de coorte retrospetivo, que incluiu todos os doentes acompanhados no hospital e no domicílio por uma equipa de cuidados paliativos em Portugal durante o ano de 2019. Resultados: Foram incluídos 496 doentes, dos quais 308 (62,1%) tiveram acompanhamento intra-hospitalar. Identificado delirium em 112 (22,6%) doentes e o fármaco de primeira linha foi o haloperidol (n = 58, 51,8%). Registaram-se 283 (57,1%) óbitos e a ocorrência de óbito foi significativamente maior naqueles que desenvolveram delirium (72,3%, p <0,001). Foram associadas a uma maior incidência de delirium o seguimento intra-hospitalar (27,9%, p <0,001), infeção (53,9%, p <0,001), alterações endócrinas (84,6%, p <0,001) e iónicas (55,0%, p <0,001), o uso de anticolinérgicos (54,1%, p = 0,001), antipsicóticos (42,6%, p = 0,031), corticosteroides (37,7%, p = 0,008) e a presença de metastização cerebral (50,0%, p = 0,046). Conclusão: Existe uma prevalência significativa de delirium em doentes em fim de vida, sendo este um preditor de mau prognóstico. A implementação de cuidados paliativos de qualidade com uma abordagem multidisciplinar pode proporcionar um controlo mais eficaz do delirium e reduzir sua prevalência geral.

Abstract Introduction: Delirium is one of the most frequent complications in palliative care with a prevalence as high as 88% in the days preceding the end-of-life. Data on delirium home-based palliative care are scarce. The aim of this work is to characterize the population followed by a palliative care team, both in-hospital and at home, to assess the prevalence of delirium, the therapeutic strategy, its resolution, and the factors associated with the higher or lower prevalence of delirium. Methods: A retrospective cohort study was carried out, including all patients followed in the hospital and at home by a palliative care team in Portugal during the year of 2019. Results: A total of 496 patients were included, of whom 308 (62.1%) had in-hospital follow-ups. Delirium was identi-fied in 112 (22.6%) of all patients and the first line drug used was haloperidol (51.8%). A total of 283 (57.1%) patients died, of whom 81 (28.6%) developed delirium. The occurrence of death was significantly higher in those who developed delirium (72.3%, p <0.001). Associated with higher incidence of delirium were identified in-hospital follow-up (27.9%, p <0.001), infection (53.9%, p <0.001), endocrine (84.6%, p <0.001) and ionic changes (55.0%, p <0.001), use of anticholinergics (54.1%, p = 0.001), antipsychotics (42.6%, p = 0.031), corti-costeroids (37.7%, p = 0.008), and presence of brain metastasis (50.0%, p = 0.046). Conclusion: There is a significant prevalence of delirium in end-of-life patients, which is a predictor of poor prognosis. The implementation of quality palliative care with a multidisciplinary approach may provide more effective control of delirium and reduce its overall prevalence.

ABSTRACT The Caatinga biome has plant and animal species that have developed mechanisms that allow them to resist the edaphoclimatic conditions of the region. Therefore, this work identified the phytosociological composition of a fragment of caatinga forest located at the rural community of Mocambo de Baixo, in the municipality of Patos-PB, Brazil, and analyzed the potentialities of the tree component by means of a forest inventory, considering the density, frequency, dominance, cover and importance values of the tree species, and the Shannon-Weaver and the Pielou equability index values of the adult and regenerating tree communities. A total of 1402 individuals were found: 534 adult, 510 regenerating and 358 dead plants. There were 10 species, 10 genera and 06 families of adult individuals, and 08 species, 08 genera and 05 families for the regenerating plants. The most expressive botanical families of adult or regenerating plants were Apocynaceae, Euphorbiaceae and Fabaceae. Densities of 953.7 and 910.7 ind.ha-1 were found for adult and regenerant plants, respectively. The highest values of tree cover and importance were estimated for Aspidosperma pyrifolium. The Shannon- Waever and the Pielou equability index values were 1.38 and 0.60, respectively. Adult individuals were most abundant in class I for diameter (1.9 cm < D < 4.4 cm) and height (1.4 m < H < 3.4 m), and regeneranting plants were most abundant in class II (H > 1 m). The estimated volume of stacked wood averaged approximately 38 st.ha-1..

RESUMO O bioma Caatinga possui espécies vegetais e animais que desenvolveram mecanismos que as permitem resistir às condições edafoclimáticas da região. Diante disso, este trabalho identificou a composição florística de um fragmento de uma floresta de Caatinga na comunidade rural Mocambo de Baixo, município de Patos – PB, Brasil, e analisou as potencialidades do componente vegetal arbóreo através de inventário florestal, considerando os parâmetros de densidade, frequência, dominância, valor de cobertura e valor de importância das espécies arbóreas, e os índices de diversidade de Shannon-Weaver e de equabilidade de Pielou das comunidades de indivíduos adultos e regenerantes de espécies arbóreas. Foi constatado um total de 1402 indivíduos: 534 adultos, 510 regenerantes e 358 mortos. Foram verificadas 10 espécies, 10 gêneros e 06 famílias de indivíduos adultos, e 08 espécies, 08 gêneros e 05 famílias para os regenerantes. As famílias botânicas com maior expressividade entre os indivíduos adultos ou regenerantes foram Apocynaceae, Euphorbiaceae e Fabaceae. Foram encontradas densidades de 953,7 e 910,7 ind.ha-1 para adultos e regenerantes, respectivamente. Os maiores valores de cobertura e de importância foram obtidos por Aspidosperma pyrifolium. Os valores estimados para os índices de Shannon-Weaver e de equabilidade de Pielou foram 1,38 e 0,60, respectivamente. Os indivíduos adultos se concentraram na classe I de diâmetro (1,9 cm < D < 4,4 cm) e altura (1,4 m < H < 3,4 m), e entre os regenerantes, a maior quantidade foi observada na classe II (H > 1 m). O valor estimado do volume empilhado médio de madeira foi de 38 st.ha-1.

ABSTRACT Objective: To update the recommendations to support decisions regarding the pharmacological treatment of patients hospitalized with COVID-19 in Brazil. Methods: Experts, including representatives of the Ministry of Health and methodologists, created this guideline. The method used for the rapid development of guidelines was based on the adoption and/or adaptation of existing international guidelines (GRADE ADOLOPMENT) and supported by the e-COVID-19 RecMap platform. The quality of the evidence and the preparation of the recommendations followed the GRADE method. Results: Twenty-one recommendations were generated, including strong recommendations for the use of corticosteroids in patients using supplemental oxygen and conditional recommendations for the use of tocilizumab and baricitinib for patients on supplemental oxygen or on noninvasive ventilation and anticoagulants to prevent thromboembolism. Due to suspension of use authorization, it was not possible to make recommendations regarding the use of casirivimab + imdevimab. Strong recommendations against the use of azithromycin in patients without suspected bacterial infection, hydroxychloroquine, convalescent plasma, colchicine, and lopinavir + ritonavir and conditional recommendations against the use of ivermectin and remdesivir were made. Conclusion: New recommendations for the treatment of hospitalized patients with COVID-19 were generated, such as those for tocilizumab and baricitinib. Corticosteroids and prophylaxis for thromboembolism are still recommended, the latter with conditional recommendation. Several drugs were considered ineffective and should not be used to provide the best treatment according to the principles of evidence-based medicine and to promote resource economy.

RESUMO Objetivo: Atualizar as recomendações para embasar as decisões para o tratamento farmacológico de pacientes hospitalizados com COVID-19 no Brasil. Métodos: A elaboração desta diretriz foi feita por especialistas, incluindo representantes do Ministério da Saúde e metodologistas. O método utilizado para o desenvolvimento rápido de diretrizes baseou-se na adoção e/ou adaptação de diretrizes internacionais existentes (GRADE ADOLOPMENT) e contou com o apoio da plataforma e-COVID-19 RecMap. A qualidade das evidências e a elaboração das recomendações seguiram o método GRADE. Resultados: Chegaram-se a 21 recomendações, incluindo recomendações fortes quanto ao uso de corticosteroides em pacientes em uso de oxigênio suplementar e recomendações condicionais para o uso de tocilizumabe e baricitinibe, em pacientes com oxigênio suplementar ou ventilação não invasiva, e de anticoagulantes, para prevenção de tromboembolismo. Devido à suspensão da autorização de uso, não foi possível fazer recomendações para o tratamento com casirivimabe + imdevimabe. Foram feitas recomendações fortes contra o uso de azitromicina em pacientes sem suspeita de infecção bacteriana, hidroxicloroquina, plasma convalescente, colchicina e lopinavir + ritonavir, além de recomendações condicionais contra o uso de ivermectina e rendesivir. Conclusão: Foram criadas novas recomendações para o tratamento de pacientes hospitalizados com COVID-19, como as recomendações de tocilizumabe e baricitinibe. Ainda são recomendados corticosteroides e profilaxia contra tromboembolismo, esta em caráter condicional. Vários medicamentos foram considerados ineficazes e não devem ser usados, no intuito de proporcionar o melhor tratamento segundo os princípios da medicina baseada em evidências e promover a economia de recursos.
