The positive impact of an intervention for maternal depression on child emotional and behavioral symptoms in a low-resource setting

Objectives: Children of depressed mothers are at risk of developing mental health problems. We sought to determine whether treatment for maternal depression by community-based health workers would decrease behavioral/emotional symptoms in their children. Interventions for maternal depressive symptoms in a low/middle-income country can have a high global impact. Methods: Community-based health workers were trained to deliver a psychosocial intervention for mothers with depression in a primary care setting. A total of 49 mothers and 60 children were assessed pre-intervention, post-intervention, and at 6 months follow-up. Child behavioral/emotional symptoms were evaluated according to type of change in maternal depressive symptoms: response or remission. Results: An overall decrease in maternal depressive symptoms from baseline to post-intervention and 6 months follow-up were found. Response or remission was associated with better outcomes in child behavioral/emotional symptoms at 6 months follow-up (p = 0.0247, Cohen’s d: 0.76; p = 0.0224, Cohen’s f: 0.44) but not at post-intervention (p = 0.1636, Cohen’s d: 0.48; p = 0.0720, Cohen’s f: 0.33). Conclusions: Improvement in maternal depression was related to decreased behavioral/emotional symptoms in their children. Our results suggest that providing interventions for maternal depression in primary care is a viable strategy to prevent behavioral/emotional symptoms in the next generation. Revictimization as a high-risk factor for development of posttraumatic stress disorder: a systematic review of the literature

Objective: Much research has been published on the role of sexual revictimization in the emergence of mental disorders in adulthood, but findings have sometimes been contradictory. The present systematic review sought to assess the state of the evidence on revictimization as a potential factor for the emergence of posttraumatic stress disorder (PTSD). Methods: Electronic searches were conducted in five databases (MEDLINE/PubMed, Cochrane Library, Campbell Library, PsycINFO, and LILACS), using the terms PTSD, posttraumatic stress disorder, child abuse, and rape. Results: We identified nine articles that established a connection among childhood sexual abuse (CSA), sexual revictimization in adulthood, and development of PTSD. Eight of the nine papers included were classified as having strong methodological quality (grade VI). One was classified as IV, with an average quality-of-evidence rating. The mean methodological quality score of the articles was 5.5, and the quality of evidence was deemed strong. Conclusion: In the included studies, PTSD symptoms were most prevalent in the CSA + adult sexual assault groups, providing further evidence for the revictimization hypothesis.

Objective: Associations between parental/caregiver depression and adverse child outcomes are well established and have been described through one or more mechanisms: child psychopathology following exposure to a depressed caregiver, child psychopathology exacerbating a caregiver’s depression, and caregiver and offspring depression sharing the same etiology. Data from low and middle-income countries is scarce. We examined correlations between common symptoms of mental disorders in caregivers and their offspring’s psychopathology in a Brazilian sample. Methods: In this cross-sectional study, adult caregivers were screened for depression during routine home visits by community health workers as part of the Brazilian Family Health Strategy. Caregivers with suspected depression were assessed using the Zung Self-Rating Depression Scale and the Self-Reporting Questionnaire (SRQ-20). Children’s symptoms were evaluated using the Strengths and Difficulties Questionnaire (SDQ). Results: The sample included 68 primary caregivers and 110 children aged 6 to 15 years. Higher caregiver scores on the SRQ-20 correlated significantly with psychiatric symptoms in offspring. Conclusion: These results substantiate our hypothesis that child psychopathology correlates with caregivers’ psychiatric symptoms. This paper adds to the growing literature on community mental health assessment and can help guide future strategies for reducing the burden of common mental disorders in caregivers and children alike in low and middle-income countries.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Lack of association between the 5-HTTLPR and positive screening for mental disorders among children exposed to urban violence and maltreatment

Objective: To ascertain whether genetic variations in the serotonin transporter gene (5-HTTLPR 44-bp insertion/deletion polymorphism) influence an increase in depressive and anxiety symptoms in children and adolescents exposed to high levels of violence. Methods: Saliva samples were collected from a group of children who were working on the streets and from their siblings who did not work on the streets. DNA was extracted from the saliva samples and analyzed for 5-HTTLPR polymorphism genotypes. Results: One hundred and seventy-seven children between the ages of 7 and 14 years were analyzed (114 child workers and 63 siblings). Data on socioeconomic conditions, mental symptoms, and presence and severity of maltreatment and urban violence were collected using a sociodemographic inventory and clinical instruments. There was no positive correlation between the 5-HTTLPR polymorphism and presence of mental symptoms in our sample, although the children were exposed to high levels of abuse, neglect, and urban violence. Conclusions: Despite previous studies that associated adult psychiatric disorders with the 5-HTTLPR polymorphism and a history of childhood maltreatment, no such association was found in this sample of children at risk.

Objective: To quantitatively study the exposure to childhood maltreatment and urban violence in children from families with at least one child working on the streets and to investigate the relationship between these factors and street work. Methods: Families who participated in a nongovernmental organization (NGO) program to eliminate child labor were included. Data concerning sociodemographic characteristics, punishment methods used in the family environment against the children, five types of abuse and neglect perpetrated by the caregivers, urban violence exposure and family functioning were collected. Results: The sample included 126 children who were working on the streets and 65 siblings who were not working on the streets. Caregivers reported high levels of severe physical punishment. The children reported high levels of abuse and neglect, and high levels of urban violence exposure. The families showed a predominance of dysfunctional and unsatisfactory relationships. A multiple logistic regression model showed that age older than 12 years and severe physical punishment at home were associated with street work. Conclusion: Interventions to decrease the risk of child street work should be family-focused and should aim to reduce violence in the family environment.

RESUMO Novas evidências sugerem que o cerebelo apresenta alterações estruturais e funcionais nos transtornos psiquiátricos. Objetivo: Medir o volume do cerebelo e de suas sub-regiões em indivíduos portadores de transtornos psiquiátricos e relacionar tais achados aos sintomas. Métodos: Foi realizada a identificação de pacientes com diferentes graus de prejuízo cognitivo proveniente de um estudo populacional (Epidemiologia do Idoso - UNIFESP), pacientes com transtorno do estresse pós-traumático proveniente de outro estudo populacional e portadores de transtorno bipolar proveniente de um ambulatório especializado (Universidade Federal da Bahia). Todos os sujeitos foram submetidos à ressonância magnética estrutural de 1.5T. As medidas de volume, assim como os sintomas medidos por escalas psicométricas foram comparadas entre pacientes e controles. Resultados: Foi observado que o volume do cerebelo está reduzido nos portadores de prejuízo cognitivo sem demência e com demência, no transtorno do estresse pós-traumático e no transtorno bipolar quando comparados aos controles. Na demência e no transtorno do estresse pós-traumático o volume do hemisfério cerebelar esquerdo e do vérmis estão reduzidos. No transtorno bipolar os volumes de ambos os hemisférios e do vérmis estão reduzidos. Nos dois primeiros estudos estas reduções correlacionaram com os sintomas. Conclusão: A natureza exata do envolvimento do cerebelo nos processos mentais ainda não é compreendida. Entretanto, anormalidades na estrutura cerebelar e em suas funções têm sido relatadas em algumas dessas doenças. Pesquisas futuras, com amostras maiores, ainda são necessárias para esclarecer tais achados e investigar se são importantes para o tratamento e prognóstico.
ABSTRACT. New evidence suggests that the cerebellum has structural and functional abnormalities in psychiatric disorders. Objective: In this research, the goal was to measure the volume of the cerebellum and its subregions in individuals with psychiatric disorders and to relate these findings to their symptoms. Methods: Patients with different degrees of cognitive impairment (Epidemiology of the Elderly - UNIFESP) and patients with post-traumatic stress disorder (PTSD) from population studies were analyzed. Also, patients with bipolar disorder from an outpatient clinic (Center for the Study of Mood and Anxiety Disorders, Universidade Federal da Bahia) were recruited for this study. All subjects underwent a 1.5T structural magnetic resonance scan. Volumetric measures and symptom measurements, by psychometric scales, were performed and compared between patients and controls. Results: The cerebellum volume was reduced in patients with cognitive impairment without dementia and with dementia, in patients with PTSD, and in patients with bipolar disorder compared to controls. In dementia and PTSD, the left cerebellar hemisphere and vermis volume were reduced. In bipolar disorder, volumes of both hemispheres and the vermis were reduced. In the first two studies, these cerebellar volumetric reductions correlated with symptoms of the disease. Conclusion: The exact nature of cerebellar involvement in mental processes is still not fully understood. However, abnormalities in cerebellar structure and its functions have been reported in some of these diseases. Future studies with larger samples are needed to clarify these findings and investigate whether they are important for treatment and prognosis.

OBJETIVOS: Avaliar a prevalência e descrever alterações da morfologia genital em recém-nascidos em duas maternidades-escola de gestão pública. MÉTODOS: Estudo caso/controle, descritivo, transversal. Utilizou-se protocolo clínico para avaliação da morfologia genital de 2.916 recém-nascidos entre 19/04/2010 e 18/04/2011. O grupo controle foi formado pelos nascimentos sem anormalidades morfológicas ocorridos no dia e na maternidade em que o caso foi detectado. Teste exato de Fisher foi utilizado para análises de variáveis categóricas, e de Kruskal-Wallis, para igualdade de médias. RESULTADOS: Foram detectados 29 recém-nascidos com anormalidade genital (1:100). A maioria (93,2%) foi examinada nos três primeiros dias de vida e apresentava apenas uma anormalidade. Os defeitos morfológicos compreenderam: ambiguidade genital evidente (1/29), fusão posterior de grandes lábios (1/29), micropênis (2/29), clitoromegalia (6/29), criptorquidia bilateral (6/29), hipospádia (9/29) e defeitos combinados (4/29) casos. Em apenas um caso os campos da Declaração de Nascido Vivo foram preenchidos corretamente. Prematuridade foi observada em 13/29, sendo esta a única variável estatisticamente associada à presença de anormalidade genital. Oito casos aderiram à proposta de investigação complementar, entre os quais três tiveram diagnóstico clínico de distúrbio da diferenciação do sexo. CONCLUSÕES: Evidenciou-se alta prevalência, subdiagnóstico e sub-registro de defeitos genitais nas maternidades estudadas. Os resultados reforçam a importância do exame cuidadoso de recém-nascidos com o objetivo de identificar anormalidades genitais sutis que podem compor o quadro clínico de distúrbio da diferenciação do sexo.
OBJECTIVES: To assess the prevalence of genital abnormalities among neonates in two public maternity-schools. METHODS: Case-control, cross-sectional descriptive study. Genital morphology of 2,916 neonates was assessed using a clinical protocol between 04/19/2010 and 04/18/2011. Control group included neonates without birth defects, born at the same maternity unit and in the same day in which a case was identified. Fisher and Kruskal-Wallis tests were used for statistics. RESULTS: The study identified 29 (1:100) neonates with genital abnormalities. Most of them were examined within 3 days of life and presented only one genital defect. Morphological abnormalities comprised: genital ambiguity (1/29), fusion of labia majora (1/29), micropenis (2/29), enlarged clitoris (6/29), hypospadia (9/29), and combined defects (4/29). Only one case reported the genital abnormality in the statement of live birth correctly. Prematurity occurred in 13/29 cases and was the only variable statistically associated with genital defects. Eight cases agreed on the complementary investigation of the genital defect, among which three were diagnosed with disorder of sex development. CONCLUSIONS: There is a high prevalence of genital abnormalities in the maternity units included in the present study and most cases are under-diagnosed and under-reported. Our results reinforce the importance of a careful examination of genital morphology in neonatal period towards the recognition of minor defects that can be clinical features of a disorder of sex development.

CONTEXTUALIZAÇÃO: A estimulação elétrica transcutânea (EET) é um método não farmacológico utilizado no alívio da dor do trabalho de parto. Sua aplicação representa um método adjuvante que não se propõe a substituir outras técnicas. OBJETIVOS: Desenvolver uma revisão sistemática para avaliar a eficácia da EET comparada à ausência de EET ou placebo, segundo os desfechos: alívio da dor do trabalho de parto (desfecho primário), uso de analgesia complementar, duração do trabalho de parto, satisfação da parturiente, tipo de parto e repercussões fetais (desfechos secundários). MÉTODOS: Realizou-se uma busca sistemática nas bases de dados PubMed, LILACS e SCIELO por ensaios clínicos randomizados e quasirandomizados, publicados entre 1966 e 2008. As palavras-chave utilizadas foram 'TENS', 'labor', 'labor pain' e 'labor obstetric'. A seleção de artigos elegíveis e a avaliação de sua qualidade metodológica foram feitas independentemente por dois revisores. Meta-análises de efeitos randômicos foram realizadas quando estudos que investigavam um mesmo desfecho eram suficientemente homogêneos. RESULTADOS: Nove estudos, incluindo 1076 gestantes, foram selecionados. Não houve diferença significativa entre os grupos no alívio da dor do parto (pooled RR=1,09; 95%IC=0,72-1,65) ou na necessidade de analgesia complementar (pooled RR=0,89; 95%IC=0,74-1,08). Não se observou evidência da interferência da EET em nenhum dos desfechos, exceto no desejo da parturiente em utilizar a EET em futuros partos. CONCLUSÕES: A utilização da EET não demonstrou nenhum impacto sobre a mãe ou concepto e nenhuma influência no trabalho de parto. De acordo com os resultados desta revisão, não há evidência de que a EET reduz o uso de analgesia complementar.
BACKGROUND: Transcutaneous electrical stimulation (TENS) is a non-pharmacological pain relief method. It is an auxiliary method and not intended to replace other techniques. OBJECTIVES: To perform a systematic review assessing the effectiveness of TENS compared to no TENS treatment or placebo with the following outcomes: pain relief (primary outcome), analgesic requirements, duration of labor, the mother's satisfaction, type of delivery and fetal repercussions (secondary outcomes). METHODS: The Pubmed, LILACS and Scielo databases were searched for randomized controlled trials and quasi-randomized trials published between 1966 and 2008 using the keywords 'TENS', 'Labor', 'Labor pain' and 'obstetric labor'. The selection of eligible items and assessment of methodological quality were performed independently by two researchers. Random effects meta-analysis was performed for studies that were sufficiently homogeneous. RESULTS: Nine studies involving a total of 1076 pregnant women were included. There was no statistically significant difference between groups in pain relief during labor (pooled RR = 1.09, 95% CI = 0.72 to 1.65) or the need of additional analgesia (pooled RR = 0.89, 95% CI = 0.74 to 1.08). There was no evidence that TENS interfered in any of the outcomes except the mothers' desire to use TENS in future deliveries. CONCLUSIONS: The use of TENS had no impact on mother or child and no influence on labor. According to the results of this review, there is no evidence that TENS reduces the use of additional analgesia.

OBJETIVO: A meta deste artigo foi a de estudar as relações ente maus-tratos na infância e psicopatologia no adulto, como reflexo de uma disfunção do eixo hipotálamo-pituitária-adrenal. MÉTODO: Uma revisão seletiva da literatura relevante foi feita para identificar achados-chave e ilustrativos. RESULTADOS: Existe atualmente um volume significativo de achados científicos pré-clínicos e clínicos derivados de paradigmas experimentais, que demonstram que o estresse precoce está relacionado à função do eixo hipotálamo-pituitária-adrenal e a estados psicológicos no indivíduo adulto, e como esta relação pode ser modulada por outros fatores. DISCUSSÃO: O risco para o desenvolvimento de psicopatologia no adulto e disfunções do eixo hipotálamo-pituitária-adrenal está relacionado à complexa interação de múltiplos fatores vivenciais, assim como a genes que levam a uma susceptibilidade, que interagem com estes fatores. Embora as respostas agudas do eixo hipotálamo-pituitária-adrenal sejam geralmente adaptativas, as respostas excessivas podem levar a efeitos deletérios. O estresse precoce pode alterar a função do eixo hipotálamo-pituitária-adrenal assim como o comportamento, porém, o padrão da disfunção do eixo hipotálamo-pituitária-adrenal e a evolução psicológica na vida adulta refletem ambas as características do estressor e outros fatores modificadores. CONCLUSÃO: A pesquisa atual identificou múltiplos determinantes da disfunção do eixo hipotálamo-pituitária-adrenal encontrados em adultos com história de maus-tratos na infância ou outros estressores precoces. Trabalhos futuros são necessários para estabelecer se as anormalidades do eixo hipotálamo-pituitária-adrenal neste contexto podem ser usadas para o desenvolvimento de endofenótipos de risco para doenças físicas ou psiquiátricas.
OBJECTIVE: The aim of this paper was to examine the relationship between childhood maltreatment and adult psychopathology, as reflected in hypothalamic-pituitary-adrenal axis dysfunction. METHOD: A selective review of the relevant literature was undertaken in order to identify key and illustrative research findings. RESULTS: There is now a substantial body of preclinical and clinical evidence derived from a variety of experimental paradigms showing how early-life stress is related to hypothalamic-pituitary-adrenal axis function and psychological state in adulthood, and how that relationship can be modulated by other factors. DISCUSSION: The risk for adult psychopathology and hypothalamic-pituitary-adrenal axis dysfunction is related to a complex interaction among multiple experiential factors, as well as to susceptibility genes that interact with those factors. Although acute hypothalamic-pituitary-adrenal axis responses to stress are generally adaptive, excessive responses can lead to deleterious effects. Early-life stress alters hypothalamic-pituitary-adrenal axis function and behavior, but the pattern of hypothalamic-pituitary-adrenal dysfunction and psychological outcome in adulthood reflect both the characteristics of the stressor and other modifying factors. CONCLUSION: Research to date has identified multiple determinants of the hypothalamic-pituitary-adrenal axis dysfunction seen in adults with a history of childhood maltreatment or other early-life stress. Further work is needed to establish whether hypothalamic-pituitary-adrenal axis abnormalities in this context can be used to develop risk endophenotypes for psychiatric and physical illnesses.

OBJETIVOS: Estudar os fatores de risco relacionados ao desenvolvimento do comportamento agressivo. MÉTODO: Foi realizada uma busca em duas bases de dados eletrônicas, Medline e SciElo, por estudos retrospectivos, longitudinais e de revisão que avaliaram fatores de risco para o desenvolvimento do comportamento agressivo. RESULTADOS: Foram selecionados 11 estudos longitudinais (8 prospectivos e 3 de casos-controle) e um transversal que avaliaram os fatores de risco biológicos e socioambientais relacionados ao comportamento agressivo. Cinco estudos avaliaram a expressão gênica, cinco a exposição ao tabaco, ao álcool e a cocaína no período pré-natal, um avaliou as implicações da desnutrição precoce no desenvolvimento do comportamento agressivo e um avaliou o impacto dos maus tratos na infância. CONCLUSÃO: os principais fatores biológicos encontrados foram: genéticos (baixa expressão do gene monoaminaoxidase e do gene transportador de serotonina, variações nos genes transportador e receptor de dopamina), exposição a substâncias durante o desenvolvimento intrauterino (tabaco, álcool e cocaína) e nutricionais (desnutrição infantil). os principais fatores socioambientais encontrados foram: maus tratos na infância, pobreza, criminalidade e comportamento antissocial na infância, sendo que o maior nível de evidência esteve relacionado à negligência precoce. A interação entre fatores biológicos e ambientais pode ser catalisada por um ambiente hostil aumentando os riscos para o desenvolvimento de comportamentos agressivos.
OBJECTIVES: To study the risk factors related to the development of aggressive behavior. METHOD: A search was carried out in two electronic databases, Medline and SciElo by retrospective studies, longitudinal and review that assessed risk factors for the development of aggressive behavior. RESULTS: There were selected 11 longitudinal studies (8 prospective and 3 case-control studies) and a cross sectional study that evaluated the risk factors and socio-biological related to aggressive behavior. Five studies have evaluated gene expression, five evaluated exposure to tobacco, alcohol and cocaine in the prenatal period, one evaluated the effect of early malnutrition on the development of aggressive behavior and one assessed the impact of child maltreatment. CONCLUSION: The main biological factors were: genetic (low expression of the monoamine oxidase gene and serotonin transporter gene, variations in transporter and dopamine receptor genes), exposure to substances during intrauterine development (tobacco, alcohol and cocaine) and nutrition (malnutrition). The main environmental factors were: child abuse, poverty, crime and antisocial behavior in childhood, while the highest level of evidence was related to early neglect. The interaction between biological and environmental factors can be catalyzed by a hostile environment, increasing the risk for the development of aggressive behavior.

OBJETIVO: O objetivo deste estudo é avaliar a resistência mecânica de três montagens de fixador externo linear utilizando pinos de 4,5 mm e de 5,5 mm com alma 3,2mm, e de 5,5 mm com alma de 4,5 mm no setor rosqueado dos pinos. MATERIAL E MÉTODOS: Foram montados fixadores externos lineares em tubos de polipropileno de 500 mm, com um corte obliquo a 45º no seu centro, com dois pinos de Schanz em cada segmento. Foram estudados 18 corpos de prova, divididos em três grupos de seis peças cada. No grupo 1 foram utilizados pinos de Schanz de 4,5 mm, no grupo 2 pinos de 5,5 mm ambos com alma de 3,2 mm e no grupo 3 pinos de 5,5 mm com alma de 4,5 mm de diâmetro. Os testes mecânicos foram realizadas em máquina de torção MT-100, e as medidas de resistência realizadas com 4,5º, 9,0º, 13,5º e 18,0º de torção. RESULTADOS: Os pinos de Schanz de 4,5 mm e alma de 3,2 mm mostraram como média de resistência para as torções de 4,5º, 9,0º, 13,5º e 18,0º, respectivamente: 12,0 N/mm, 21,0 N/mm, 33,0 N/mm e 46,0 N/mm. Os pinos de 5,5 com alma de 3,2 mm mostraram como resistência média: 13,2 N/mm, 25,3 N/mm, 40,0 N/mm e 51,2 N/mm, respectivamente. Os testes com os pinos de Schanz de 5,5 mm com alma de 4,5 mm mostraram resistência média de: 15,2 N/mm, 33,5 N/mm, 53,0 N/mm e 70,0 N/mm. Estudo estatístico com o teste da Análise de Variância e o teste de Bonferroni mostraram ausência de diferença estatisticamente significante entre os grupos com pinos com diâmetro da alma da parte roscada de 3,2mm (de 4,5 mm e 5,5 mm). Houve diferença estatisticamente significante (alfa <0,05) entre o grupo com pinos de 5,5 mm com alma 4,5 mm e os outros dois. CONCLUSÕES: 1- Não houve diferença estatisticamente significante entre as montagens de fixador externo linear com pinos de Schanz de 4,5 mm e 5,5 mm com alma de 3,2 mm, aos esforços de torção. 2- A montagem do fixador externo linear com pinos de Schanz de 5,5 mm com alma de 4,5 mm mostrou resistência mecânica maior à torção, estatisticamente significante, em relação aos outros dois grupos.
OBJECTIVE: The objective of this study was to evaluate the mechanical force of three different assemblies of a linear external fixator using 4.5 mm and 5.5 mm Schanz screws with a 3.2 mm-diameter root, and another screw with 5.5 mm diameter with a 4,5mm root. MATERIAL AND METHODS: The linear external fixator was assembled in a 500 mm-long polypropylene tube, with a 45-degree oblique angle cut in the center of the cylinder, with two Schanz screws in each segment. Eighteen assemblies were studied, divided into three groups of six pieces each. In the group 1, 4.5mm-diameter Schanz screws were used, in the group 2, 5.5mm-diameter Schanz screws, both with 3,2mm root were used. In the group 3, 5.5mm-diameter Schanz screws with a diameter root of 4.5mm were used. The mechanical tests were performed in a MT-100 torsion machine, and the resistance measurements were performed at 4.5º, 9.0º, 13.5º, and 18º of torsion. RESULTS: The 4.5mm Schanz screws with 3,2 mm root showed an average torsion resistance at 4.5o, 9.0 o, 13.5 o and 18.0 o, respectively of: 12.0 N/mm, 21.0 N/mm, 33.0 N/mm, and 46.0 N/mm. The 5.5mm Schanz screw with 3.2 mm root, showed as average resistance: 13.2 N/mm, 25.3 N/mm, 40.0 N/mm, and 51.2 N/mm, respectively. The tests with 5.5 mm Schanz screws with 4.5 mm root showed an average resistance of: 15.2 N/mm, 33.5 N/mm, 53.0 N/mm, and 70.0 N/mm. Statistical analysis with the Variance Analysis test and the Bonferroni's test showed the absence of statistically significant differences among groups with root diameter of 3.2mm (4.5 mm and 5.5 mm). There was a statistically significant difference between the group with the 5.5mm Schanz screws with 4.5 mm root and the other two groups. CONCLUSIONS: 1- There was no statistically significant difference among assemblies of linear external fixator using 4.5mm and 5.5mm Schanz screws with 3.2mm root. 2- The linear external fixator with 5.5mm Schanz screws with 4.5mm root, showed a stronger mechanical resistance to the torsion, statistically significant, regarding the other two groups.
